Abstract Persistence of malaria parasites in asymptomatic hosts is crucial in areas of seasonally-interrupted transmission, where P. falciparum bridges wet seasons months apart. During the dry season, infected erythrocytes exhibit extended circulation with reduced cytoadherence, increasing the risk of splenic clearance of infected cells and hindering parasitaemia increase. However, what determines parasite persistence for long periods of time remains unknown. Here, we investigated whether seasonality affects plasma composition so that P. falciparum can detect and adjust to changing serological cues; or if alternatively, parasite infection length dictates clinical presentation and persistency. Data from Malian children exposed to alternating ~6-month wet and dry seasons show that plasma composition is unrelated to time of year in non-infected children, and that carrying P. falciparum only minimally affects plasma constitution in asymptomatic hosts. Parasites persisting in the blood of asymptomatic children from the dry into the ensuing wet season rarely if ever appeared to cause malaria in their hosts as seasons changed. In vitro culture in the presence of plasma collected in the dry or the wet seasons did not affect parasite development, replication or host-cell remodelling. The absence of a parasite-encoded sensing mechanism was further supported by the observation of similar features in P. falciparum persisting asymptomatically in the dry season and parasites in age- and sex-matched asymptomatic children in the wet season. Conversely, we show that P. falciparum clones transmitted early in the wet season had lower chance of surviving until the end of the following dry season, contrasting with a higher likelihood of survival of clones transmitted towards the end of the wet season, allowing for the re-initiation of transmission. We propose that the decreased virulence observed in persisting parasites during the dry season is not due to the parasites sensing ability, nor is it linked to a decreased capacity for parasite replication but rather a consequence decreased cytoadhesion associated with infection length.

Early warning systems (EWSs) for landslides are becoming a pivotal tool to safeguard assets and stakeholders. With this mission, an EWS should be capable of reliably forecasting the failure time when the ground accelerates. There are analytical methods developed to this end that use time-series kinematics: inverse velocity (INV), minimum inverse velocity (MINV), slope gradient (SLO), and velocity over acceleration (VOA). Although an abundant number of studies applied these methods, they have been majorly examined in a back-analysis context where all the measurements are incorporated into the forecasting process. A successful operation of EWSs in raising meaningful alarms calls for an examination in which the forecasting method is evaluated synchronously. This study evaluates the ability of the four mentioned methods to provide reliable forecasts in real time using a comprehensive database including 75 historical failures. For the first time, the methods are evaluated using a quantitative metric called reliability fitness index (RFI) that measures the portion of forecasts meeting an accuracy threshold. For accuracy thresholds of 50, 75 and 90%, INV showed the highest RFI values of 16, 7, and 4% followed by SLO values of 12, 5, and 2%, respectively. In this paper we investigate the application of phase-shifting digital holography for the real-time characterization of electromagnetic sources in the THz frequency range. We use an off-the-shelf terahertz detector array composed of 64 × 64 power-sensitive pixels, over an area of 96 mm × 96 mm , to record intensity interferograms cast between the coherent radiation emitted from a reference source and an unknown antenna under test. This approach parallelizes the acquisition process with respect to conventional near-field point scanning methods, reducing the measurement time by orders of magnitude. In our system, the measurement time is limited only by the refresh rate of the detector array and the speed of a delay line stage that is used to phase-shift the reference wave. As a proof-of-principle demonstration, we map the 2D near-field distribution and estimate the far-field radiation pattern emitted by a plano-convex PTFE spherical lens antenna illuminated by a diagonal horn at 290 GHz frequency with ∼ 1 Hz refresh rate. The Achilles tendon (AT) insertion is the most common site of enthesitis in psoriatic arthritis (PsA). The structure and function of the AT in PsA, and the prevalence of mid-portion pathology, is unknown. To compare the structure and function of the AT in people with PsA with self-reported AT pain (PsA + AT), PsA without self-reported AT pain (PsA-AT) and healthy controls. A cross-sectional, observational study was conducted. The ATs were assessed by clinical and US examination (B-mode and Power Doppler), performance-based testing (bilateral heel raise test (HRT) and 10 m walk test), and patient-reported outcome measures (PROMs) (including the Victorian Institute of Sport Assessment-Achilles [VISA-A]). Between-group differences were described using descriptive statistics, Chi-squared testing, parametric (1-way ANOVA) and non-parametric (Mann-Whitney or Kruskal-Wallis) testing. 22 PsA (11 per group) and 11 healthy control participants who were comparable in terms of sex, age, and BMI (PsA-AT = longer PsA disease duration) were recruited. VISA-A scores were significantly worse in the PsA + AT group compared to the PsA-AT group and healthy controls (p < 0.001). Inflammatory US features were significantly more prevalent in the PsA + AT group (p < 0.001). Mid-portion AT pathology was observed in the PsA + AT group, irrespective of entheseal disease. Clinical examination alone missed 5/7 cases of 'active' US-confirmed AT enthesitis. AT functional deficits were significant in the PsA + AT group and both PsA groups had lower HRT repetition rates and walked slower compared to healthy controls. Less than 1/3 of the PsA + AT group had received podiatry or physiotherapy care. Significant differences in the structure and function of the AT in PsA were noted. Purpose: Forensic verification of cyanide (CN) poisoning by direct CN analysis in postmortem blood is challenging due to instability of CN in biological samples. CN metabolites, thiocyanate (SCN−) and 2-aminothiazoline-4-carboxylic acid (ATCA), have been proposed as more stable biomarkers, yet it is unclear if either is appropriate for this purpose. In this study, we evaluated the behavior of CN biomarkers in postmortem swine and postmortem blood to determine which serves as the best biomarker of CN exposure. Methods: CN, SCN−, and ATCA were measured in postmortem swine (N = 8) stored at 4 °C and postmortem blood stored at 25 °C (room temperature, RT) and 37 °C (typical human body temperature, HBT). Results: Following CN poisoning, the concentration of each CN biomarker increased well above the baseline. In postmortem swine, CN concentrations declined rapidly (t1/2 = 34.3 h) versus SCN− (t1/2 = 359 h, 15 days) and ATCA (t1/2 = 544 h, 23 days). CN instability in postmortem blood increased at RT (t1/2 = 10.7 h) and HBT (t1/2 = 6.6 h). SCN− and ATCA were more stable than CN at all storage conditions. In postmortem swine, the t1/2s of SCN− and ATCA were 15 and 23 days, respectively. While both the t1/2s of SCN− and ATCA were relatively lengthy, endogenous levels of SCN− were much more variable than ATCA. Conclusion: While there are still questions to be answered, ATCA was the most adept forensic marker of CN poisoning (i.e., ATCA produced the longest half-life, the largest increase above baseline levels, and most stable background concentrations). Forecasting landslide failure time is important for understanding the kinematics of landslides. In addition to the famous inverse velocity method (INV), other methods such as the minimum inverse velocity method (MINV), slope gradient (SLO), and velocity over acceleration (VOA) have been proposed. Despite the importance of adopting a forecasting technique, very limited insights regarding the mentioned methods have been published. This study aims to address this gap by conducting a comparative examination of INV, MINV, SLO, and VOA on a comprehensive database of 75 historical landslides. This access to a large volume of historical data enabled us to assess the advantages and limitations of each method. Results show that MINV is the only method with better performance than INV, by 23% on average and, in 32% of cases, by at least 100%. On the other hand, the accuracy of SLO forecasts is on average 87% worse than INV forecasts. VOA results have a 6.84 to 11.68 times higher error compared to INV. VOA is highly sensitive, even more so than INV, to scatter in measurements, but the positive effect of filtration is offset by data artifacts caused by the data filtration technique. SLO, owing to its mathematical configuration, is rather insensitive to measurement scatter. Finally, the failure velocities, as calibrated by MINV, are presented for each failure mode; this can enhance the detection of the onset of acceleration and be used toward more adaptive warning thresholds in monitoring systems depending on the site setting. Studies examining relationships between neurotypical and autistic siblings have reported inconsistent findings when focussing on children and adolescents, with little research investigating adult autistic sibling relationships. This is of particular interest as siblings can often take over the guardianship or care of a sibling when their parents are no longer able to. The current study examined knowledge and attitudes of adult siblings towards autism. 50 mixed sibling types (autistic sibling + neurotypical sibling) & 59 matched sibling types (neurotypical sibling + neurotypical sibling) completed knowledge and attitude questionnaires. In addition, a thematic analysis of responses provided by neurotypical siblings from the mixed sibling types was used to explore experiences with autism based on sibling relationships. Results showed a non-significant difference of attitudes towards autism and a significant difference in knowledge of autism. From the thematic analyses four themes were identified: Robbed Childhood, Sibling over Self, Concerns for the Future, and Autism Awareness. The study found that having a sibling diagnosed with autism can predict knowledge of autism, however, knowledge of autism does not drive attitudes, therefore further research is needed to understand which factors drive neurotypical sibling attitudes since attitudes predict behaviour. The qualitative research showed mixed experiences within childhood and adulthood, with implications on care in the future. The adult perspectives of the autistic sibling relationships are discussed. Genomic surveillance of Plasmodium falciparum malaria can provide policy-relevant information about antimalarial drug resistance, diagnostic test failure, and the evolution of vaccine targets. Yet the large and low complexity genome of P. falciparum complicates the development of genomic methods, while resource constraints in malaria endemic regions can limit their deployment. Here, we demonstrate an approach for targeted nanopore sequencing of P. falciparum from dried blood spots (DBS) that enables cost-effective genomic surveillance of malaria in low-resource settings. We release software that facilitates flexible design of amplicon sequencing panels and use this software to design two target panels for P. falciparum. The panels generate 3–4 kbp reads for eight and sixteen targets respectively, covering key drug-resistance associated genes, diagnostic test antigens, polymorphic markers and the vaccine target csp. We validate our approach on mock and field samples, demonstrating robust sequencing coverage, accurate variant calls within coding sequences, the ability to explore P. falciparum within-sample diversity and to detect deletions underlying rapid diagnostic test failure. Genomic surveillance of Plasmodium falciparum could improve monitoring of drug resistance, but implementation has been hampered due to the large and complex genome. Interferometric synthetic aperture radar (InSAR) has gained considerable attention as a landslide monitoring strategy owing to its high accuracy, large coverage, and relatively low associated costs. A crucial drawback of InSAR, however, has limited its further incorporation: one-dimensional estimations along the sensor's line-of-sight (LOS). This leads to an ambiguity in results and a less intuitive understanding of landslide kinematics. A frequently exercised approach to address this issue has been taking inspiration from the topography to establish compatibility assumptions between velocity components, yet little insight exists on the performance of these methods. The objective of this paper is to investigate the performance of four renowned topography-based methods—Surface Parallel Flow Model (SPFM), SPFM coupled with least-squares method (SPF-LSM), Aspect Parallel Flow Model (APFM), and Steepest Terrain Following Model (STFM)—in evaluating the magnitude and geometry of total velocity vectors. To this end, the analysis is performed on 202 Radarsat-2 and 243 Sentinel-1 scenes acquired over a section of the Thompson River valley, a critical railway corridor in Western Canada traversing 14 landslides. The results indicate that the APFM provides estimations with the lowest magnitude error (15–19 mm/yr or 18.75–23.75% of in situ measurements) compared to the other approaches. SPFM and SPF-LSM are highly sensitive to LOS variance and tend to bias the interpreted vectors toward the north orientation. However, APFM and STFM reflect more realistic aspect angles, with the former inclined to steeper travel angles and the latter suffering from erratic upward travel angles due to local topographies. Numerous dams in North America are constructed on foundations and/or abutments consisting of unstable weak materials or low-friction pre-sheared surfaces such as the Oldman River Dam (ORD), in southern Alberta, Canada. The objective of this paper is to examine the performance of Interferometric Synthetic Aperture Radar (InSAR) on the slow-moving ORD site by comparing the mapped velocities obtained through InSAR analysis in 2015–2022 with velocities from previous investigations. Additionally, the study aims to determine the precision of estimated velocities by analyzing InSAR temporal decorrelation and incorporating the error of simplifying the assumption necessary to decompose line-of-sight velocities. The results indicate the spillway, covering an area of ~ 26 × 103 m2, has been moving at an average total velocity of 1.26 mm/year. On the slope of the embankment, measuring ~ 160 × 103 m2, an active area next to the left abutment is also experiencing a higher average velocity of 8.29 mm/year (both rates in millimeters per year). An analysis of the precision of the estimations showed an accuracy of 0.72 and 2.78 mm/year for the spillway and embankment, respectively. Overall, the spillway velocities have not deviated much from 1 mm/year in the study period but the embankment’s active area decelerated from > 20 to 5 mm/year toward the end of 2022. Purpose: To evaluate the effectiveness and acceptability of a 6-week acceptance and commitment therapy (ACT)–based group programme on participants’ fear of cancer recurrence (FCR), quality of life (QoL), psychological distress and psychological flexibility at the end of the programme and 12-week follow-up. Methods: A one-group, post-test service evaluation of a real-world psychological programme was carried out to evaluate collected outcome measures and attendance for a total of 21 groups facilitated between 2017 and 2019. Participants were breast cancer survivors who attended a 6-week group programme led by NHS clinicians. Descriptive statistics and repeated measures ANOVA analyses were carried out for each outcome measure. Attendance levels were examined to assess acceptability. Results: A total of 97 group participants who had completed curative treatment for breast cancer took part. Of whom, 89% completed at least 4 of the 6 weekly group sessions and 76% attended the 12-week follow-up session. Eighty-four (87%) participants returned outcome measures at all three time points relative to group participation (T1 = pre, T2 = post T3 = 12-week follow-up). Group participants were female, mean age 51.9 years. FCR was highest at T1 (mean 25.2, SD 4.7), reduced T2 (mean 21.2, SD 5.4) and further lowered T3 (mean 19.5, SD 6.2). This difference was statistically significant (p < 0.001). QoL was lowest at T1 (mean 62.4, SD 15.7), increased T2 (mean 71.7, SD 18.1) and further increased at T3 (mean 75.9, SD 17.5). This difference was statistically significant (p < 0.001). Psychological distress measures were shown to reduce, and psychological flexibility increased. Conclusions: This real-world evaluation of an ACT-based group programme led to improvements in FCR, QoL, psychological distress and psychological flexibility in this population. Single amino acid (AA) supplementations in foods are increasing, however their potential nutritional and physiological impacts are not fully understood. This study examined the effects of l-lysine (Lys) supplementation on protein quality of diets, serum AA concentrations and associations between the ratio of supplemental Lys to dietary protein (X) with body weight gain (BWG) in Sprague–Dawley male rats. Rats were fed one of 10 diets containing either 7% or 20% casein and supplemented with 0% (Control), 1.5%, 3%, 6% Lys or 6% Lys + 3% l-arginine (Arg) (8 rats/diet group) for 1 week. Lys supplementation reduced the protein quality of the casein-based diets (p < 0.01). BWG was reduced by supplemental Lys when X > 0.18. Free Lys supplementation dose-dependently increased serum Lys levels (p < 0.01), while increased protein-bound Lys (1.4% vs 0.52%) had little effect on serum Lys (p > 0.05). In the 7% casein diets, ≥ 1.5% supplemental Lys reduced serum alanine, asparagine, glycine, isoleucine, leucine, serine, tyrosine, valine, carnitine, ornithine, and increased urea. Supplementation of ≥ 3% Lys additionally reduced tryptophan and increased histidine, methionine and α-aminoadipic acid (α-AAA) compared to the Control (p < 0.05). In the 20% casein diets, addition of ≥ 1.5% Lys reduced serum asparagine and threonine, and ≥ 3% Lys reduced leucine, proline, tryptophan, valine, and ornithine, and 6% Lys reduced carnitine, and increased histidine, methionine, and α-AAA. Overall, this study showed that free Lys supplementation in a Lys-sufficient diet reduced the protein quality of the diets and modified the serum concentrations of many amino acids. Excess free Lys intake adversely affected growth and utilization of nutrients due to AA imbalance or antagonism. Cyanide (in the form of cyanide anion (CN−) or hydrogen cyanide (HCN), inclusively represented as CN) can be a rapidly acting and deadly poison, but it is also a common chemical component of a variety of natural and anthropogenic substances. The main mechanism of acute CN toxicity is based on blocking terminal electron transfer by inhibiting cytochrome c oxidase, resulting in cellular hypoxia, cytotoxic anoxia, and potential death. Due to the well-established link between blood CN concentrations and the manifestation of symptoms, the determination of blood concentration of CN, along with the major metabolite, thiocyanate (SCN–), is critical. Because currently there is no method of analysis available for the simultaneous detection of CN and SCN– from blood, a sensitive method for the simultaneous analysis of CN and SCN– from human ante- and postmortem blood via liquid chromatography–tandem MS analysis was developed. For this method, sample preparation for CN involved active microdiffusion with subsequent chemical modification using naphthalene-2,3-dicarboxaldehyde (NDA) and taurine (i.e., the capture solution). Preparation for SCN– was accomplished via protein precipitation and monobromobimane (MBB) modification. The method produced good sensitivity for CN with antemortem limit of detection (LODs) of 219 nM and 605 nM for CN and SCN–, respectively, and postmortem LODs of 352 nM and 509 nM. The dynamic ranges of the method were 5–500 µM and 10–500 µM in ante- and postmortem blood, respectively. In addition, the method produced good accuracy (100 ± 15%) and precision (≤ 15.2% relative standard deviation). The method was able to detect elevated levels of CN and SCN– in both antemortem (N = 5) and postmortem (N = 4) blood samples from CN-exposed swine compared to nonexposed swine. There is a high burden of critical illness in low-income countries (LICs), adding pressure to already strained health systems. Over the next decade, the need for critical care is expected to grow due to ageing populations with increasing medical complexity; limited access to primary care; climate change; natural disasters; and conflict. In 2019, the 72nd World Health Assembly emphasised that an essential part of universal health coverage is improved access to effective emergency and critical care and to "ensure the timely and effective delivery of life-saving health care services to those in need". In this narrative review, we examine critical care capacity building in LICs from a health systems perspective. We conducted a systematic literature search, using the World Heath Organisation (WHO) health systems framework to structure findings within six core components or "building blocks": (1) service delivery; (2) health workforce; (3) health information systems; (4) access to essential medicines and equipment; (5) financing; and (6) leadership and governance. We provide recommendations using this framework, derived from the literature identified in our review. These recommendations are useful for policy makers, health service researchers and healthcare workers to inform critical care capacity building in low-resource settings. [ABSTRACT FROM AUTHOR]

Purpose: Small bowel obstruction (SBO) is the most common indication for laparotomy in the UK. While general surgeons have become increasingly subspecialised in their elective practice, emergency admissions commonly remain undifferentiated. This study aimed to assess temporal trends in the management of adhesional SBO and explore the influence of subspecialisation on patient outcomes. Methods: Data was collected for patients admitted acutely with adhesional SBO across acute NHS trusts in Northern England between 01/01/02 and 31/12/16, including demographics, co-morbidities and procedures performed. Patients were excluded if a potentially non-adhesional cause was identified and were grouped by the responsible consultant's subspecialty. The primary outcome of interest was 30-day inpatient mortality. Results: Overall, 2818 patients were admitted with adhesional SBO during a 15-year period. There was a consistent female preponderance, but age and comorbidity increased significantly over time (both p < 0.001). In recent years, more patients were managed operatively with a trend away from delayed surgery also evident (2002–2006: 65.7% vs. 2012–2016: 42.7%, p < 0.001). Delayed surgery was associated with an increased mortality risk on multivariable regression analysis (OR: 2.46 (1.46–4.23, p = 0.001)). CT scanning was not associated with management strategy or timing of surgery (p = 0.369). There was an increased propensity for patients to be managed by gastrointestinal (colorectal and upper gastrointestinal) subspecialists over time. Length of stay (p < 0.001) and 30-day mortality (p < 0.001) both improved in recent years, with the best outcomes seen in colorectal (2.6%) and vascular subspecialists (2.4%). However, following adjustment for confounding variables, consultant subspecialty was not a predictor of mortality. Conclusion: Outcomes for patients presenting with adhesional SBO have improved despite the increasing burden of age and co-morbidity. Background: Instrumented ball indentation offers a promising approach to determining mechanical properties and tensile stress–strain relationships of materials, non-destructively. Objective: Using indentation load-depth experimentation to determine mechanical properties such as Young's modulus, yield stress, strain hardening exponent and hardness, and tensile flow curve based on conventional contact mechanics principles. Method: Nine different rail steel specimens are subjected to instrumented indentation testing (IIT) using an in-house developed ball indentation equipment that considers pile-up effect for effective estimation of the contact area and specimen's stiffness required for mechanical characterization of the rail steels. Results: The mechanical property response using micro-scaled ball indentation followed a comparative outcome with tensile test outcomes. The evolution of the hardness across the depth experiences a sudden increase followed by a decrease in hardness which suggests the effect of localized hardening due to indenter size effect mechanism. In order to establish the tensile flow curve via indentation, an adjusting parameter (κ) is included as part of the parameter (ϕ) that describes the development of the plastic zone beneath the indenter tip to correct the effect of multi-axial stresses and increased stresses due to the indenter size effect. Conclusions: The flow curve via the ball indentation strongly correlates with tensile stress–strain relationship, showing a promising possibility of using non-destructive indentation to determine tensile properties and flow curve for high-strength rail steels. In ecology and evolution, the small population paradigm posits that reduced genetic variation will result in limited phenotypic variation that, in turn, will affect population resilience and potential for adaptation. Over the last decade though, such a paradigm has been questioned, with evidence that mechanisms independent of genetic variation may be also important in shaping phenotypic variation. However, there are few large-scale empirical examples, especially from aquatic ecosystems. Using the large-scale natural experiment afforded by the global invasion of brown trout (Salmo trutta), we quantify standing phenotypic variation in morphology among different introduced ranges, relative to the native range. By using shape variation and morphological integration as indicators of phenotypic variation, we show that neither founding population size nor time since founding (i.e., effect of selection regime) are correlated to the amount of morphological variation, contrarily to common expectations. Beyond founding population size and time since founding, the amount of morphological variation is mostly controlled by factors at the population level rather than at the region level, and is not lower in invaded regions compared to the native range. These results suggest that the dynamics of phenotypic variation may be largely independent of population size and mostly determined by site-specific patterns of selection. [ABSTRACT FROM AUTHOR]

Gastrointestinal (GI) discomfort is a hallmark of most gut disorders and represents an important component of chronic visceral pain1. For the growing population afflicted by irritable bowel syndrome, GI hypersensitivity and pain persist long after tissue injury has resolved2. Irritable bowel syndrome also exhibits a strong sex bias, afflicting women three times more than men1. Here, we focus on enterochromaffin (EC) cells, which are rare excitable, serotonergic neuroendocrine cells in the gut epithelium3–5. EC cells detect and transduce noxious stimuli to nearby mucosal nerve endings3,6 but involvement of this signalling pathway in visceral pain and attendant sex differences has not been assessed. By enhancing or suppressing EC cell function in vivo, we show that these cells are sufficient to elicit hypersensitivity to gut distension and necessary for the sensitizing actions of isovalerate, a bacterial short-chain fatty acid associated with GI inflammation7,8. Remarkably, prolonged EC cell activation produced persistent visceral hypersensitivity, even in the absence of an instigating inflammatory episode. Furthermore, perturbing EC cell activity promoted anxiety-like behaviours which normalized after blockade of serotonergic signalling. Sex differences were noted across a range of paradigms, indicating that the EC cell–mucosal afferent circuit is tonically engaged in females. Teacher self- and collective efficacy are important motivational factors in determining teachers’ effort in their professional practice. Teachers’ higher levels of self- and collective efficacy are positively associated with student achievement in specific domains, and teachers’ enhanced wellbeing and commitment to their profession. This qualitative study explores primary teachers’ own views about naturalistic and characteristic experiences and influences that have strengthened and weakened their efficacy in teaching reading. Five themes of naturalistic sources of enhancement in self- and collective efficacy are identified that are consistent with predictions of Bandura’s social cognitive theory and studies involving interventions in schools by expert trainers. The themes are combined into a model of growth in teachers’ efficacy and can potentially inform decision-making in schools in supporting and enhancing teachers’ self- and collective efficacy when teaching reading. Coworking spaces represent a new trend for future workplaces. As more building owners are interested in running coworking space businesses, it becomes important to understand both why a potential member chooses one space over another and how to keep existing members at a coworking site. A sound understanding of member needs can make a difference. Unfortunately, very few studies have been conducted as to understanding a member's basic needs in coworking spaces. In this paper, we aim to identify member's needs in three coworking spaces in Sweden. Participant observations, immersion, and interviews were used for data collection. The member needs are categorized and structured through the lens of self-determination theory. In total, we uncovered, formulated, and categorized 21 member needs. We found that the fulfillment of one need may lead to the inhibition of another, thus creating tensions between and within coworking members. This research contributes to the literature by addressing the importance and definition of member needs for coworking as well as the created tensions related to these needs, which have been lacking in coworking studies. Coherence Time of a wireless communication channel is needed to determine the duration of time during which the channel will be static. If the receiving end is aware of this Coherence Time parameter, another Coherence Time related parameter, Doppler Shift, will be recognized. Very few of the numerous Doppler Shift studies use Coherence Time as the basis for detecting Doppler Shift. This is because the Coherence Time parameters for each channel model will have different values and be hard to detect. Our previous studies have been carried out to determine the Coherence Time detection method on a V2V channel but its correlation coefficient was below 0.5, which is not compatible with the theory. Therefore, this study aims to enhance the method with better correlation coefficient value above 0.5. This method was applied on V2V channel with moving scatterers and Correlated Double-Ring (CDR) channel models. This study can be therefore utilized as a guide for detecting the duration of Coherence Time on wireless communications channels, with the findings of Coherence Time detection being compared to the Coherence Time value calculated theoretically to get the accuracy rate. The accuracy rate on the V2V channel with moving scatterer has an accuracy value of up to 80.2%, while the accuracy rate on the CDR channel has an average accuracy value of up to 90.2%, according to the test results of the Coherence Time detection method that we present. Further, our proposed method could be implemented to determine the data symbol sent in a single frame in order to obtain a higher BER value in wireless communications such as Wi-fi, Cellular or V2V. The study aims to characterise the species identification and antimicrobial susceptibility testing (AST) results of Nocardial isolates from adult patients across major public hospitals in Queensland, Australia, over a 15-year period. A multi-centre retrospective observational study of Nocardia sp. isolates was conducted from 7 major public hospitals in Queensland, Australia, over a 15-year period. Clinical samples from patients aged ≥ 18 years that isolated Nocardia sp. were included. Demographic and clinical data were collected, along with species identification and AST results. Overall, 484 Nocardia sp. were isolated. Most patients were male (297, 61%) with a mean (IQR) age of 60 (51–75) and a median (IQR) Charlson Comorbidity Index of 4 (2–6). Of these, 239 (49%) patients were immunosuppressed. Organisms were most frequently isolated from sputum (174, 36%), and superficial swabs (102, 21%). Patients presented with pulmonary infections (165, 35%) and superficial skin and soft tissue infections (87, 18%) most commonly. One hundred (21%) isolates were deemed pulmonary colonisation and were not treated. Of the speciated organisms, N. nova complex was the most common (93, 19%), followed by N. farcinica complex (79, 16%). Organisms were reliably susceptible to linezolid (240/245, 98%), amikacin (455/470, 97%), and trimethoprim/sulfamethoxazole (459/476, 96%), but less so to imipenem (243/472, 51%) and ceftriaxone (261/448, 58%). This is the largest Australian description of Nocardia sp. to date. Given antimicrobials are often commenced prior to AST results and sometimes even speciation, characterisation of local species and antibiogram data is important to guide empiric choices and local guidelines. The objective of the present study was to establish reliability and validity evidence of a simple procedure to measuring conceptual knowledge, the Concept Retrieval Technique (CRT). The CRT procedure requires test takers to freely retrieve concepts from memory they feel are relevant to a given topic, with raters awarding one mark for each correctly retrieved concept when matched against a target word list. Four studies were conducted. In Study 1 (N = 73), inter-rater agreement for the marking procedure of the CRT was determined by means of Cohen's kappa. The kappa was κ =.85. Study 2 (N = 164) explored how consistent inter-rater agreement was across different subjects, age groups, and raters. Mean kappa was κ =.92 suggesting that the CRT can reliably be marked. Study 3 was concerned with establishing the convergent and discriminant validity of the CRT (N = 55). The correlation between the CRT scores and scores on essay-type items was r =.69 and between assessments that measured other ability domains r =.04 to.21 (n.s.). In Study 4, an experiment was conducted to determine the construct validity of the CRT (N = 45). Participants either acquired, or did not acquire, new knowledge of a particular topic and this manipulation was consistently reflected in their CRT scores. Patients with rheumatoid arthritis (RA) are at an increased risk of cardiovascular disease and vascular morbidity. The association between peripheral arterial disease (PAD) and RA has not been previously investigated within the scope of a review. Conjoined disease manifestations may impact patient well-being, perpetuating increased mortality and quality of life deficits. To investigate the association between RA and PAD, along with RA and the ankle-brachial pressure index (ABPI), the impact of disease concomitance on health-related quality of life (HRQOL) and functional capacity (FC) was also investigated. Individual study appraisal was completed using the Crowe Critical Appraisal Tool (CCAT). A level of evidence analysis was conducted using the American Society of Plastic Surgeons (ASPS) Evidence Rating Scale for Prognostic/Risk Studies. AMED®, CINAHL®, Health Source: Nursing/Academic Edition, MEDLINE®, AHFS®, Scopus, Web of Science, Cochrane Library and Google scholar. Ten studies produced a CCAT rating of ≥ 30 (75%) and were deemed high quality, while a single study demonstrated a score of 26 (65%) suggesting moderate quality. A grade "II" levels of evidence was awarded to positive association between RA and PAD. A gradation of "I" was awarded to the association between ABPI and RA. The impact of concomitant manifestations on HRQOL and FC did not qualify for a level of evidence analysis. The systematic inflammatory nature of RA likely contributes to the increased incidence of PAD within the population. Riverine dissolved silicon (DSi) and biogenic silica (BSi) are modulated along the estuarine gradient by several biotic and abiotic processes governed by physical forcings. An important area controlling silicon transport in alluvial estuaries with large intertidal mudflats is the benthic diatom-dominated biofilm system. Here, the hypertidal Severn Estuary, UK, has been used as a case study to improve our understanding of silicon transport in these benthic-dominated systems. We present the first time-series dataset of Si concentrations in the Severn. River and tidal hydrodynamics drove spatio-temporal changes in DSi. The longitudinal profile of DSi followed the classical view of dilution with downstream transport. Despite low riverine supply of BSi and low siliceous-phytoplankton production, relatively high BSi concentrations were measured in the Severn Estuary (maximum of 14.9 mg/L), which accounted for over 70% of the total bioavailable silicon present and were characterised by isotopically heavy waters (δ30Si of + 0.9 to + 1.1‰). Benthic biofilms (microphytobenthos) on the intertidal mudflats contained significant biomass (measured as chlorophyll a concentration with a maximum of 116.8 ± 16.2 µg/g dw. sed) with high productivity, driven by their photoprotective adaptions to these harsh intertidal environments, contributing to isotopically heavy mudflat water (δ30Si of + 1.19 to + 2.03‰), and resulting in high benthic BSi content in the spring (0.74 ± 0.03%) and summer (0.76 ± 0.05%). The fast-flowing tidal currents resulted in high bottom shear stress which likely exceeded the erosion thresholds of the biofilms, transporting the sediment-BSi matrix into the water column. Suspended particulate matter (SPM) and BSi remained tightly coupled in the estuarine water column (bioflocculation), and experienced the series of erosion–deposition events, burial/dissolution and export out of the estuary. Our novel observations improve understanding of the complex processes governing Si transport in hypertidal, benthic-dominated estuaries, and highlights the importance of tightly coupled benthic-pelagic systems in influencing the terrestrial silicon export to a marine zone. Laser hemorrhoidoplasty (LHP) is known as a new minimally invasive and painless procedure for symptomatic hemorrhoids. However, Milligan-Morgan (MM) may offer the best result of long-term cure rates. In this study, we aim to compare the efficacy between LHP and MM for hemorrhoidal disease treatment. Using specific keywords, we comprehensively go through the potential articles on PubMed, Europe PMC, and Google Scholar sources until April 19, 2022. All published studies on LHP and MM hemorrhoidectomy were collected. Statistical analysis was done by using Review Manager 5.4 software. Twelve studies with a total of 1756 patients with hemorrhoid grades II-IV were included for the analysis. Our pooled analysis revealed that LHP was associated with shorter operative time (p < 0.00001), shorter length of hospital stay (p = 0.0005), lower risk of urinary retention (p = 0.005) and anal stenosis (p = 0.0004), and lower VAS 24-h post-operative (p < 0.00001) when compared with MM. However, LHP and MM did not differ in terms of recurrence rate (p = 0.70). LHP was superior to MM procedure in terms of shortening the recovery time and minimizing post-operative complications for patients with hemorrhoidal disease. [ABSTRACT FROM AUTHOR]

An advanced ultrasound imaging technique, sonoelastography (SE) is used to evaluate tissue elasticity. To determine SE potential to detect pathological-related changes, and characteristics related to tendon pathology we aimed to (1) compare quadriceps and patellar tendon findings in individuals with knee osteoarthritis (KOA) and asymptomatic older adults (AC), and (2) explore associations between SE, participant characteristics (age, BMI, and leg circumference) and KOA status. 84 participants (47; KOA and 37; asymptomatic older adults) underwent SE examination of quadriceps (distal) and patellar (distal, proximal) tendon in a supine position with the knee bent at 30°. Colour score (CS) and Elasticity Ratio (ER) analysis were performed by a blinded experienced operator using Esaote Mylab 70 XVG Ultrasound equipment. Significantly reduced elasticity in the distal quadriceps (median (IQR) 2(2), 3(1), p = 0.033 for KOA and AC, respectively) and proximal patellar (3(1), 3(0), p = 0.001) tendons and more elastic distal patellar (1.50 (0.55), 1.87 (0.72), p = 0.034) tendons were observed in the KOA group. Significant associations) were identified between SE and participant BMI (Rs = − 0.249–0.750, p < 0.05) and leg circumference (Rs = − 0.260–0.903, p < 0.05). Age, BMI and KOA status, were independent explanatory variables of SE CS findings at the distal quadriceps tendon patellar tendon, proximal patellar tendon and distal patellar tendon, explaining 66%, 81% and 64% of variance, respectively. Age, BMI and KOA status were independent explanatory variables of SE ER findings at the distal patellar tendon explaining 19% of variance. Potentially clinically relevant altered tendon stiffness were observed between individuals with KOA and asymptomatic controls. Key KOA risk factors and participant characteristics explained variance in tendon stiffness. Although patient-reported outcome measures (PROMs) are recommended in clinical practice, their application in routine care is limited. The Rheumatoid Arthritis Foot Disease Activity Index (RADAI-F5) is a validated PROM for assessing foot disease in rheumatoid arthritis (RA). To explore patient and clinician opinions and perceptions of the clinical utility of the Rheumatoid Arthritis Foot Disease Activity Index (RADAI-F5), eight RA patients and eight clinicians routinely involved in the management of RA patients participated in one semi-structured remote video-based interview. They provided their perspectives on the barriers and facilitators to clinical implementation of the RADAI-F5. Three global themes were identified; 'Feet are a priority' as the impact of RA on the feet negatively impacted upon patient quality of life. The second theme was 'Need for a clinically feasible foot PROM' as participants recognised the current lack of a clinically feasible tool to determine RA foot disease. The third global theme of 'Implementation' was drawn together to form two subordinate themes: 'Facilitators to RADAI-F5 implementation' as the tool can promote communication, guide management, help screen foot symptoms, monitor foot disease status and treatments, and promote patient education and; 'Barriers to RADAI-F5 implementation' as there were associated practical difficulties, including lack of appointment time, administrative burdens, IT barriers and preference for further RADAI-F5 validation using imaging. The RADAI-F5 has significant potential as a clinical tool to aid foot disease management. However, implementation challenges must be overcome before broad adoption in rheumatology clinics. Non-alcoholic fatty liver disease (NAFLD) is the most common chronic liver disease and affects about 25% of the population globally. Obesity and diabetes are the main causes of the disease characterized by excessive accumulation of lipids in the liver. There is currently no direct pharmacological treatments for NAFLD. Dietary intervention and lifestyle modification are the key strategies in the prevention and treatment of the disease. Soy consumption is associated with many health benefits such as decreased incidence of coronary heart disease, type-2 diabetes, atherosclerosis and obesity. The hypolipidemic functions of soy components have been shown in both animal studies and human clinical trials. Dietary soy proteins and associated isoflavones suppressed the formation and accumulation of lipid droplets in the liver and improved NAFLD-associated metabolic syndrome. The molecular mechanism(s) underlying the effects of soy components are mainly through modulation of transcription factors, sterol regulatory element-binding protein-1 and peroxisome proliferator-activated receptor-γ2, and expressions of their target genes involved in lipogenesis and lipolysis as well as lipid droplet-promoting protein, fat-specific protein-27. Inclusion of appropriate amounts of soy protein and isoflavones in the diets might be a useful approach to decrease the prevalence of NAFLD and mitigate disease burden. [ABSTRACT FROM AUTHOR]

Sleep problems in Autism Spectrum Disorder (ASD) emerge early in development, yet the origin remains unclear. Here, we characterise developmental trajectories in sleep onset latency (SOL) and night awakenings in infants at elevated likelihood (EL) for ASD (who have an older sibling with ASD) and infants at typical likelihood (TL) for ASD. Further, we test whether the ability to gate tactile input, using an EEG tactile suppression index (TSI), associates with variation in SOL and night awakenings. Parent-reported night awakenings and SOL from 124 infants (97 at EL for ASD) at 5, 10 and 14 months were analyzed using generalized estimating equations. Compared to TL infants, infants at EL had significantly more awakenings and longer SOL at 10 and 14 months. The TSI predicted SOL concurrently at 10 months, independent of ASD likelihood status, but not longitudinally at 14 months. The TSI did not predict night awakenings concurrently or longitudinally. These results imply that infants at EL for ASD wake up more frequently during the night and take longer to fall asleep from 10 months of age. At 10 months, sensory gating predicts SOL, but not night awakenings, suggesting sensory gating differentially affects neural mechanisms of sleep initiation and maintenance. [ABSTRACT FROM AUTHOR]

Although cyanide's biological effects are pleiotropic, its most obvious effects are as a metabolic poison. Cyanide potently inhibits cytochrome c oxidase and potentially other metabolic enzymes, thereby unleashing a cascade of metabolic perturbations that are believed to cause lethality. From systematic screens of human metabolites using a zebrafish model of cyanide toxicity, we have identified the TCA-derived small molecule glyoxylate as a potential cyanide countermeasure. Following cyanide exposure, treatment with glyoxylate in both mammalian and non-mammalian animal models confers resistance to cyanide toxicity with greater efficacy and faster kinetics than known cyanide scavengers. Glyoxylate-mediated cyanide resistance is accompanied by rapid pyruvate consumption without an accompanying increase in lactate concentration. Lactate dehydrogenase is required for this effect which distinguishes the mechanism of glyoxylate rescue as distinct from countermeasures based solely on chemical cyanide scavenging. Our metabolic data together support the hypothesis that glyoxylate confers survival at least in part by reversing the cyanide-induced redox imbalances in the cytosol and mitochondria. Slowing light in a non-dispersive and controllable fashion opens the door to many new phenomena in photonics. As such, many schemes have been put forward to decrease the velocity of light, most of which are limited in bandwidth or incur high losses. In this paper we show that a long metallic helix supports a low-loss, broadband slow wave with a mode index that can be controlled via geometrical design. For one particular geometry, we characterise the dispersion of the mode, finding a relatively constant mode index of ∼ 45 between 10 and 30 GHz. We compare our experimental results to both a geometrical model and full numerical simulation to quantify and understand the limitations in bandwidth. We find that the bandwidth of the region of linear dispersion is associated with the degree of hybridisation between the fields of a helical mode that travels around the helical wire and an axial mode that disperses along the light line. Finally, we discuss approaches to broaden the frequency range of near-constant mode index: we find that placing a straight wire along the axis of the helix suppresses the interaction between the axial and high index modes supported by the helix, leading to both an increase in bandwidth and a more linear dispersion. Unprecedented and dramatic transformations are occurring in the Arctic in response to climate change, but academic, public, and political discourse has disproportionately focussed on the most visible and direct aspects of change, including sea ice melt, permafrost thaw, the fate of charismatic megafauna, and the expansion of fisheries. Such narratives disregard the importance of less visible and indirect processes and, in particular, miss the substantive contribution of the shelf seafloor in regulating nutrients and sequestering carbon. Here, we summarise the biogeochemical functioning of the Arctic shelf seafloor before considering how climate change and regional adjustments to human activities may alter its biogeochemical and ecological dynamics, including ecosystem function, carbon burial, or nutrient recycling. We highlight the importance of the Arctic benthic system in mitigating climatic and anthropogenic change and, with a focus on the Barents Sea, offer some observations and our perspectives on future management and policy. Adjuvant trastuzumab has been associated with superior survival in women with ≥ T1c or node-positive HER2-positive early-stage breast cancer; however, there is a lack of phase III trials in women with T1a/bN0 disease. Our study aimed to assess the outcomes of women with HER2-positive T1a/bN0 breast cancer who received adjuvant trastuzumab in Saskatchewan, Canada. We evaluated all women diagnosed with HER2-positive T1a/bN0 breast cancer in Saskatchewan between 2008 and 2017. We performed Cox proportional multivariable analysis to determine factors correlated with survival. In addition, inverse probability treatment weighting (IPTW) using propensity score was performed to assess benefit of adjuvant trastuzumab. Ninety-one eligible women with a median age of 61 years (range 30–89) were identified. Thirty-nine (43%) women received adjuvant trastuzumab. Women who received trastuzumab were younger and had a higher rate of T1b disease. Overall, 3% of women who received trastuzumab compared to 12% of women who did not receive trastuzumab developed breast cancer recurrence (p = 0.23). Five-year disease-free survival (DFS) of women who received adjuvant trastuzumab was 94.8% compared to 82.7% of women who did not receive trastuzumab (p = 0.22). Five-year overall survival was 100% of women who received trastuzumab compared to 90.4% of women who did not receive adjuvant trastuzumab (p = 0.038). In the multivariable analysis, grade III tumors were correlated with inferior DFS (hazard ratio [HR] 5.5, 95% CI [1.7–17.7]). The propensity score using the inverse probability of treatment weighting showed that lack of adjuvant trastuzumab was correlated inferior DFS, with an HR of 4 (95% CI 1.05–15.5). Women with HER2-positive T1a/bN0 breast cancer had overall low recurrence of breast cancer. Background: Trauma is among the leading causes of morbidity and mortality among pediatric and adolescent populations worldwide, with over ninety percent of childhood injuries occurring in low-income and middle-income countries. Lack of region-specific data on pediatric injuries is among the major challenges limiting the ability of health systems to implement interventions to prevent injuries and improve outcomes. We aim to characterize the burden of pediatric health injuries, initial healthcare interventions and outcomes seen in thirteen diverse healthcare facilities in Tanzania. Methods: This was a prospective cohort study of children aged up to 18 years presenting to emergency units (EUs) of thirteen multi-level health facilities in Tanzania from 1st October 2019 to 30th September 2020. We describe injury patterns, mechanisms and early interventions performed at the emergency units of these health facilities. Results: Among 18,553 trauma patients seen in all thirteen-health facilities, 4368 (23.5%) were children, of whom 2894 (66.7%) were male. The overall median age was 8 years (Interquartile range 4–12 years). Fall 1592 (36.5%) and road traffic crash (RTC) 840 (19.2%) were the top mechanisms of injury. Most patients 3748 (85.8%) arrived at EU directly from the injury site, using motorized (two or three) wheeled vehicles 2401 (55%). At EU, 651 (14.9%) were triaged as an emergency category. Multiple superficial injuries (14.4%), fracture of forearm (11.7%) and open wounds (11.1%) were the top EU diagnoses, while 223 (5.2%) had intracranial injuries. Children aged 0–4 years had the highest proportion (16.3%) of burn injuries. Being referred and being triaged as an emergency category were associated with high likelihood of serious injuries with adjusted odds ratio (AOR) 4.18 (95%CI 3.07–5.68) and 2.11 (95%CI 1.75–2.56), respectively. 1095 (25.1%) of patients were admitted to inpatient care, 14 (0.3%) taken to operation theatre, and 25 (0.6%) died in the EU. Conclusions: In these multilevel health facilities in Tanzania, pediatric injuries accounted for nearly one-quarter of all injuries. Over half of injuries occurred at home. Fall from height was the leading mechanism of injury, followed by RTC. Most patients sustained fractures of extremities. Future studies of pediatric injuries should focus on evaluating various preventive strategies that can be instituted at home to reduce the incidence and associated impact of such injuries. Background: Due to the high prevalence of human immunodeficiency virus (HIV) in Tanzania, provider-initiated HIV testing for patients attending any health care setting is recommended. However, follow-up and linkage to care by those tested remain poor. We determined the feasibility and efficacy of text messaging to promote follow-up among otherwise healthy trauma patients who underwent provider-initiated HIV testing and counseling at an emergency department (ED) in Tanzania. Material and methods: This randomized controlled trial (RCT) was conducted at Muhimbili National Hospital (MNH) ED between September 2019 and February 2020. Adult trauma patients consenting to HIV testing and follow-up text messaging were randomized to standard care (pre-test and post-test counseling) or standard care plus a series of three short message service (SMS) text message reminders for follow-up in an HIV clinic, if positive, or for retesting, if negative. Investigators blinded to the study assignment called participants 2 months after the ED visit if HIV-positive or 4 months if HIV-negative. We compared the proportion of people in the intervention and control groups completing recommended follow-up. Secondary outcomes were the proportion of patients agreeing to testing, proportion of patients agreeing to receiving text messages, and the proportion of HIV-positive and HIV-negative patients in each study arm who followed up. Results: Of the 290 patients approached, 255 (87.9%) opted-in for testing and agreed to receive a text message. The median age of the study population was 29 [IQR 24–40] years. There were 127 patients randomized to the intervention group and 128 to the control group. The automated SMS system verified that 381 text messages in total were successfully sent. We traced 242 (94.9%) participants: 124 (51.2%) in the intervention group and 18 (488%) in the control group. A total of 100 (39.2%) subjects reported completing a follow-up visit, of which 77 (60.6%) were from the intervention group and 23 (17.9%) were from the control group (RR = 3.4, 95% CI 2.3–5.0). This resulted in a number needed to treat (NNT) of 2.3. Of the 246 HIV-negative participants, 37% underwent repeat screening: 59% of those in the intervention group and 16% in the control group (RR = 3.7, P = < 0.0001, NNT 2.3). Among the nine positive patients, all five in the intervention group and only three in the controls had follow-up visits. Understanding the kinematics and triggers of a landslide is central to assessing the risk of a landslide. This understanding can be achieved through a robust instrumentation plan that addresses key factors such as meteorological conditions, groundwater conditions, and deformation trends and velocity. High-frequency landslide displacement measurements at multiple locations across a sliding mass allow for the evaluation of the impact of meteorological events in the short and long terms. Installation of a monitoring system that provides reliable and accurate high-frequency displacement data from multiple locations can be expensive, and thus, this approach is not commonly used. Single-frequency differential GNSS (dGNSS) provides a relatively low-cost alternative to achieve long-term monitoring goals at multiple locations within a landslide. Networks of single-frequency Geocube™ dGNSS units were installed at test sites at two slow-moving Canadian landslides: the Ten-mile landslide near Lillooet, BC, and the Chin Coulee landslide near Taber, AB. The Geocube system monitored surficial deformation every 60 s for the duration of the monitoring program (> 500 day) and achieved a precision in direct measurements characterized by a standard deviation of 10.5 mm or less. The quality of the collected data was compared to data provided by a high-end multi-frequency GMX 910 Leica GPS, installed at the Ripley landslide near Ashcroft, BC. The data collected from the Geocube systems revealed that displacement orientations and rates varied within each of the sliding masses. The application of multiple monitoring points provided a relatively low-cost method to evaluate the shape of the sliding shear surface and the causal factors driving movement at each site. A practical evaluation of cost-effective dGNSS systems that provide reliable information as required for enhanced understanding of landslide kinematics and the development of early warning systems is presented in this paper. How ecological divergence causes strong reproductive isolation between populations in close geographic contact remains poorly understood at the genomic level. We here study this question in a stickleback fish population pair adapted to contiguous, ecologically different lake and stream habitats. Clinal whole-genome sequence data reveal numerous genome regions (nearly) fixed for alternative alleles over a distance of just a few hundred meters. This strong polygenic adaptive divergence must constitute a genome-wide barrier to gene flow because a steep cline in allele frequencies is observed across the entire genome, and because the cline center closely matches the habitat transition. Simulations confirm that such strong divergence can be maintained by polygenic selection despite high dispersal and small per-locus selection coefficients. Finally, comparing samples from near the habitat transition before and after an unusual ecological perturbation demonstrates the fragility of the balance between gene flow and selection. Overall, our study highlights the efficacy of divergent selection in maintaining reproductive isolation without physical isolation, and the analytical power of studying speciation at a fine eco-geographic and genomic scale. Introduction: Cyanide is a deadly poison, particularly with oral exposure where larger doses can occur before symptoms develop. Prior studies and multiple governmentagencies highlight oral cyanide as an agent with the potential for use in a terrorist attack. Currently, there are no FDA approved antidotes specific to oralcyanide. An oral countermeasure that can neutralize and prevent absorption of cyanide from the GI tract after oral exposure is needed. Our objective was toevaluate the efficacy of oral sodium thiosulfate on survival and clinical outcomes in a large, swine model of severe cyanide toxicity. Methods: Swine (45-55kg) were instrumented, sedated, and stabilized. Potassium cyanide (8 mg/kg KCN) in saline was delivered as a one-time bolus via an orogastric tube. Three minutes after cyanide, animals randomized to the treatment group received sodium thiosulfate (510 mg/kg, 3.25 M solution) via orogastric tube. Our primary outcome was survival at 60 minutes after exposure. We compared survival between groups by log-rank, Mantel-Cox analysis and trended labs and vital signs. Results: At baseline and time of treatment all animals had similar weights, vital signs, and laboratory values. Survival at 60 min was 100% in treated animals compared to 0% in the control group (p=0.0027). Animals in the control group became apneic and subsequently died by 35.0 min (20.2,48.5) after cyanide exposure. Mean arterial pressure was significantly higher in the treatment group compared to controls (p=0.008). Blood lactate (p=0.02) and oxygen saturation (p=0.02) were also significantly different between treatment and control groups at study end. Conclusion: Oral administration of sodium thiosulfate improved survival, blood pressure, respirations, and blood lactate concentrations in a large animal model of acute oral cyanide toxicity. The first principle of chemical composition is that elements are actually present in their compounds. It is a golden thread running through the history of compositional thinking in chemistry since before the chemical revolution. Opposed to this principle, which I call Actually Present Elements (APE), is the idea that elements are merely potentially present in their compounds: although not actually present, it is possible to recover them. In this paper I follow that golden thread, and then discuss the status of APE itself: is it true? What arguments were there for it, and when? I argue that APE is a metaphysical principle, albeit at a lower level of generality and abstraction than the term 'metaphysical' usually suggests. I critically examine a range of different views on how metaphysical principles might be involved in research programmes in empirical science, and conclude by endorsing Elie Zahar's view that metaphysical principles such as APE are to be found at the very heart of science. The stable isotopic composition of diatom silica is used as a proxy for nutrient utilisation in natural waters. This approach provides essential insight into the current and historic links between biological production, carbon cycling and climate. However, estimates of isotopic fractionation during diatom silica production from both laboratory and field studies are variable, and the biochemical pathways responsible remain unknown. Here, we investigate silicon isotopic fractionation through a series of chemical precipitation experiments that are analogous to the first stages of intracellular silica formation within the diatom silicon deposition vesicle. The novelty of our experiment is the inclusion of the R5 peptide, which is closely related to a natural biomolecule known to play a role in diatom silicification. Our results suggest that the presence of R5 induces a systematic but non-significant difference in fractionation behaviour. It thus appears that silicon isotopic fractionation in vitro is largely driven by an early kinetic fractionation during rapid precipitation that correlates with the initial amount of dissolved silica in the system. Our findings raise the question of how environmental changes might impact silicon isotopic fractionation in diatoms, and whether frustule archives record information in addition to silica consumption in surface water.
