Background: Defecation is a complex process and up to 25% of the population suffer from symptoms of defecatory dysfunction. For functional testing, diagnostics, and therapy of anorectal disorders, it is important to know the optimal defecation position. is The aim of this study was to evaluate defecation pressure patterns in side lying, seated and squatting defecation positions in normal subjects using a simulated stool device called Fecobionics. Methods: The Fecobionics expulsion parameters were assessed in an interventional study design conducted from May 29 to December 9 2019. Subjects were invited to participate in the study through advertisement at The Chinese University of Hong Kong. The Fecobionics device consisted of a core containing pressure sensors at the front (caudal end) and rear (cranial end) and a polyester-urethane bag spanning most of the core length which also contained sensors. The Fecobionics bag was distended to 50 ml in the rectum of normal subjects (no present and past symptoms of defecatory disorders, no prior abdominal surgery, medication or chronic diseases). Studies were done in side lying (left lateral recumbent position), seated (hip flexed 90°) and squatting position (hip flexed 25°). Pressure endpoints including the rear-front pressure diagram and defecation indices were compared between positions. Results: Twelve subjects (6 females/6 males, mean age 26.3 ± 2.6 [19.0–48.0] years) were included and underwent the planned procedures. The resting anal pressure for side lying and seated positions were 33.1 ± 4.1 cmH2O and 37.1 ± 4.0 cmH2O (p > 0.3). The anal squeeze pressure for side lying and seated positions were 98.4 ± 6.9 cmH2O and 142.3 ± 16.4 cmH2O (p < 0.05). The expulsion duration for the side lying, seated and squatting positions were 108.9 ± 8.3 s, 15.0 ± 2.1 s and 16.1 ± 2.9 s, respectively (p < 0.01 between lying and the two other positions). The maximum evacuation pressure for seated and squatting were 130.1 ± 12.4 cmH2O and 134.0 ± 11.1 cmH2O (p > 0.5). Rear-front pressure diagrams and distensibility indices demonstrated distinct differences in pressure patterns between the side lying position group and the other positions. Conclusions: The delay in expelling the Fecobionics device in the lying position was associated with dyssynergic pressure patterns on the device. Purpose: Polidocanol injections have been used to treat chronic Achilles tendinopathy in clinical settings, but the few studies published show inconsistent results. The aim of this study was to evaluate the mid-term effect of Polidocanol in patients with chronic Achilles tendinopathy. It was hypothesised that patients treated with Polidocanol would have significant improvements in the outcome measures investigated compared to patients treated with a placebo treatment at mid-term follow-up.Methods: This randomised controlled trial included forty-eight patients aged 32-77 years with a history of Achilles tendinopathy for at least 3 months and with neovascularisation demonstrated by ultrasonography was included. A minimum of 3 months of eccentric exercise treatment was required before participating. The patients were allocated to a maximum of two injection of either Polidocanol or Lidocaine (placebo). The primary outcome measure was pain during walking reported on a visual analogue scale. Secondary outcome measures were Foot and Ankle Outcome Score (FAOS), patient satisfaction with treatment and, shortly after inclusion, the Victorian Institute of Sports Assessment-Achilles questionnaire (VISA-A) was also included. Follow-up examinations were performed after 3 and 6 months.Results: Pain during walking decreased during the 6-month follow-up period, but no significant differences were seen between the two groups. The same tendency was seen for FAOS and VISA-A in which both groups showed an improvement at 3- and 6-month follow-up, but no mid-term differences between the groups were seen. An equal number of patients in the two groups were satisfied with the treatment at follow-up.Conclusions: The results indicate that Polidocanol is a safe treatment, but the mid-term effects are the same as a placebo treatment. Until recently, only retrospective studies had been published on salvage high-dose melphalan (HDM) with autologous stem cell 'transplantation' (ASCT). In a prospective, nonrandomized phase-2 study, we treated 53 bortezomib-naïve patients with bortezomib-dexamethasone as induction and bortezomib included in the conditioning regimen along with the HDM. Median progression-free survival (PFS), time to next treatment (TNT) and overall survival (OS) after start of reinduction therapy were 21.6, 22.8 and 46.6 months, respectively. For 49 patients who completed salvage bortezomib-HDM(II) with ASCT, there was no significant difference of PFS and TNT after HDM (II) compared with after the initial HDM(I), and thus patients were their own controls (PFS (I: 20.1 vs II: 19.3 months (P=0.8)) or TNT (I: 24.4 vs II: 20.7 months (P=0.8)). No significant differences in the response rates after salvage ASCT compared with the initial ASCT. Bortezomib-HDM conditioning combo was feasible, and toxicity was as expected for patients treated with bortezomib and ASCT. In conclusion, in bortezomib-naïve patients treated at first relapse with salvage ASCT including bortezomib, PSF and TNT did not differ significantly from initial ASCT and median OS was almost 5.5 years with acceptable toxicity. Background: The muscle structures surrounding the anal canal are of major importance in maintaining continence but their anatomy and function vary along its length. Standard manometry does not provide detailed information about mechanical properties of the anal canal. A new functional luminal imaging probe (FLIP) has been developed for this purpose. The aim of our study was to investigate whether FLIP allows detailed evaluation of dynamic biomechanical properties along the length of the anal canal. Methods: The in vitro validity and reproducibility of the FLIP system were tested. Fifteen healthy volunteers (age 32-65 years, mean 51 years), of whom 12 were females, were investigated. The integrity and dimensions of the anal sphincter apparatus were evaluated with endoanal ultrasonography and standard anal manometry. During standardized distensions with the FLIP, 16 cross-sectional areas of the anal canal were measured at 5-mm intervals. Distensibility of the following three segments was evaluated: upper anal canal (surrounded by the internal anal sphincter and the puborectalis muscle), mid-anal canal (surrounded by the internal anal sphincter and the external anal sphincter) and lower anal canal (surrounded by the external anal sphincter). Color contour plots were generated from the FLIP-based dynamic recordings of serial cross-sections. Results: In vitro tests confirmed the validity and reproducibility of the FLIP system. The luminal geometry during distension and the biomechanical properties of the anal canal differed at the three levels. Both at rest and during squeeze the mid-anal canal was significantly less distensible than the upper ( p < 0.01) and the lower ( p < 0.05) anal canal. Background: Using manometry, the classification of motility-related disorders in the esophagus is vague and overlapping. We present a new method, which combines manometry and axial force measurements in a single catheter. Aim: The aim was to examine the manometric and axial force recordings during swallows. Methods: Recordings from 20 patients suffering from diffuse esophageal spasms (DES) (8), achalasia (5) and other diseases including gastro-oesophageal reflux (7) were compared to recordings made in ten healthy subjects. The probe was capable of measuring axial force 6.5-cm proximal to the lower esophageal sphincter (LES) and pressures 8-, 10- and 12-cm proximal to the LES. After insertion, five dry and five wet swallows were made. Swallows were repeated with 0, 2, 4 and 6 ml of water in a bag mounted distal to the axial force recording site. Each contraction was analysed for duration and amplitude, and was categorised according to its configuration. Results: The number of failed contractions measured with axial force was lower for the achalasia ( P < 0.001) and DES groups ( P < 0.001) compared to the healthy volunteers. The number of multi-peaked contractions was unchanged for the achalasia and DES groups while it increased for the group of healthy volunteers. On several occasions a negative traction force was encountered though the manometric pattern appeared normal. Conclusions: Measurements of axial force generated by primary peristalsis provide additional information about esophageal neuromuscular function in different diseases that is not demonstrable with manometry alone. Background: Little is known about the risk of colorectal cancer among patients with irritable bowel syndrome (IBS).Methods: We conducted a nationwide cohort study using data from the Danish National Registry of Patients and the Danish Cancer Registry from 1977 to 2008. We included patients with a first-time hospital contact for IBS and followed them for colorectal cancer. We estimated the expected number of cancers by applying national rates and we computed standardised incidence ratios (SIRs) by comparing the observed number of colorectal cancers with the expected number. We stratified the SIRs according to age, gender, and time of follow-up.Results: Among 57,851 IBS patients, we identified 407 cases of colon cancer during a combined follow-up of 506,930 years (SIR, 1.14 (95% confidence interval (CI): 1.03-1.25) and 115 cases of rectal cancer, corresponding to a SIR of 0.67 (95% CI: 0.52-0.85). In the first 3 months after an IBS diagnosis, the SIR was 8.42 (95% CI: 6.48-10.75) for colon cancer and 4.81 (95% CI: 2.85-7.60) for rectal cancer. Thereafter, the SIRs declined and 4-10 years after an IBS diagnosis, the SIRs for both colon and rectal cancer remained below 0.95.Conclusion: We found a decreased risk of colorectal cancer in the period 1-10 years after an IBS diagnosis. However, in the first 3 months after an IBS diagnosis, the risk of colon cancer was more than eight-fold increased and the risk of rectal cancer was five-fold increased. Proinflammatory cytokines are suspected to play a role in the pathogenesis of multiple myeloma (MM). Therefore, it is possible that inborn genetic variations leading to a modified expression of these cytokines will influence the outcome for these patients. We investigated 348 MM patients undergoing high-dose melphalan treatment followed by Auto-SCT and examined the influence of single nucleotide polymorphisms (SNPs) in genes involved in the inflammatory response. We found that the polymorphism IL-1β T-31C significantly influenced overall survival (OS; P=0.02) and that carriers of the variant C-allele had a significantly longer survival than homozygous wild-type allele TT-carriers (relative risk 0.6 (95% CI=0.5–0.9); P=0.008). The polymorphisms IL-6 G-174C, IL-10 C592A, PPARγ2 Pro12Ala, COX-2 A-1195G, COX-2 T8473C and NFKB1 ins/del did not influence the OS in this group of patients. Furthermore, homozygous carriers of the variant allele of IL-1β T-31C were at 1.37-fold (CI=1.05–1.80) increased risk of MM as compared with population-based controls (P=0.02). Aims/hypothesis. Morphometric and passive biomechanical properties were studied in the duodenum, jejunum and ileum in 10 non-diabetic and 40 streptozotocin-induced diabetic rats. Methods. The diabetic rats were divided into groups living 4 days, 1, 2, and 4 weeks after diabetes was induced (n=10 for each groups). The mechanical test was done as a ramp distension experiment. The intestinal diameter and length were obtained from digitised images of the intestinal segments at pre-selected pressures and at no-load and zero-stress states. Circumferential and longitudinal stresses (force per area) and strains (deformation) were computed from the length, diameter and pressure data and from the zero-stress state geometry. Results. The blood glucose concentration increased four- to fivefold in the diabetic rats. Streptozotocin-induced diabetes generated pronounced increase in the weight per centimetre length, wall thickness and wall cross-sectional area in all intestinal segments during diabetes (p<0.05). Histological analysis showed that the thickness of the intestinal layers was increased in all segments during diabetes (p<0.05). In the duodenum the opening angle did not change in the first 2 weeks and decreased after 4 weeks (p<0.05). In the jejunum and ileum the opening angle increased after 1 week in the diabetic group. The residual strain showed the same pattern as the opening angle. Furthermore, it was found that the circumferential and longitudinal stiffness of the intestinal wall increased with the duration of diabetes (p<0.05 and p<0.01). Conclusion/interpretation. We evaluated the effect of the somatostatin analogue lanreotide on the development of surgically induced experimental strictures in the anterior urethra of the male rabbit. A total of 74 male rabbits were randomly allocated into four groups. Lanreotide was administered to the rabbits in groups 2 and 4 from day 0 to 14. To create a stricture, a resection was made in the urethra of the rabbits in groups 3 and 4 on day 2. On day 30, all rabbits were examined with urethrography, impedance planimetry and either histology or for collagen content. Urethrography and impedance planimetry demonstrated a urethral stricture in all operated animals. No difference was found between the two stricture groups, regardless of lanreotide administration, with respect to luminal cross-sectional area (CSA), circumferential tension-strain relation, histology or collagen content. The CSA of the urethra of the normal controls treated with lanreotide was smaller than the CSA of the normal controls not treated with lanreotide, however, no difference was found in histology or collagen content. Lanreotide had no measurable effect on the development of a surgically induced stricture in the male rabbit anterior urethra, however, lanreotide seems to exert an inhibitory effect on the normal growth of the urethra. OBJECTIVE:Hypersensitivity of the esophageal wall may contribute to the pathogenesis of functional chest pain. Whether the hypersensitivity is more uniformly distributed along the esophageal wall or is segmental is not known.METHODS:Graded balloon distentions were performed randomly at the smooth muscle as well as at the striated muscle portions of the esophagus in 20 patients with functional chest pain and in 15 healthy volunteers, using impedance planimetry. Sensory thresholds and cross-sectional area were examined in relation to the esophageal wall tension, and the results were compared between two levels as well as the two groups of subjects.RESULTS:During balloon distention, 17 (85%) patients reported typical chest pain, 11 (55%) at both levels, four (20%) at the smooth muscle level, and two (10%) at the striated muscle level only. The sensory thresholds for perception, discomfort, or pain were lower in patients than in controls (p < 0.05). The cross-sectional area and the esophageal wall stiffness at the smooth muscle level were lower than those obtained at the striated muscle level both in controls and in patients (p < 0.01). The wall tension at which moderate discomfort and pain were reported was lower in patients than controls (p < 0.05).CONCLUSIONS:Although in most patients the esophagus is uniformly hypersensitive, in some either the smooth muscle or the striated muscle segment can be hypersensitive. If considering balloon distention at only one level, we recommend balloon placement at 10 cm above the lower esophageal sphincter because of a higher yield of hypersensitivity. Obtaining data relating intestinal mechanical properties and histology is a step towards the next level in the hierarchy of structure of living tissue, and may provide new insight into the mechanisms of intestinal function and disease such as obstruction. Due to lack of methodology, however, such data are currently sparse. Scanning acoustic microscopy (SAM) can measure the propagation speed of sound ( C) and the acoustic impedance ( Z) with micrometer resolution in tissue. By use of elementary theory of elasticity, the elastic stiffness (c11) can be computed from C and Z. We used 5-μm-thick transverse sections of ethanol treated guinea pig small intestine as the experimental model and measured the distribution of C and Z across the intestinal wall using SAM at 500 MHz. The individual layers mucosa, submucosa, and circular and longitudinal muscle were discerned with ease in the images and varied significantly with respect to both C and Z in most cases. The measured values (median values) of C ranged from 1550 to 1669 m s−1, and Z ranged from 2.10 to 2.60 MPa s m−1.c11 differed between all layers ranging from 3.25 to 4.27 GPa with the following sequence of magnitude: circular muscle>submucosa>mucosa>longitudinal muscle ( p < 0.001). Tension-strain relations and morphometric data were studied in isolated segments of the jejunum and distal ileum in untreated diabetic rats, insulin-treated diabetic rats, and nondiabetic control rats. Diabetes was induced by a single intravenous injection of streptozotocin (28 mg/kg body wt). All injected rats developed hyperglycemia. The experiment was terminated after 28 days and the intestinal segments were superfused with saline solution containing papaverine to abolish contractile activity. Stepwise inflation of a balloon in which the cross-sectional area (CSA) was measured provided the luminal pressure-loading stimulus. The circumferential tension-strain relation was derived from steady-state values of internal radius and applied pressure. The intestinal weight, length, and weight per unit length increased significantly in untreated diabetic rats compared to the two other groups (P < 0.05). The body weight decreased in untreated diabetic rats compared to the two other groups (P < 0.05). The pressure-CSA relations differed between jejunum and distal ileum (P < 0.001) but not between the groups (P > 0.2). The tension-strain relations in jejunum and distal ileum were nonlinear and the curves for the two diabetic groups were shifted to the left compared to the curve for controls (P < 0.05), indicating increased wall stiffness. The histomorphometric data showed increased wall thickness in untreated diabetic rats compared to the two other groups both in jejunum and distal ileum (P < 0.02). Mucosal, submucosal, and muscle layer thicknesses did not differ between the three groups. No significant association was found between the histomorphometric and biomechanical parameters. This phenomenon was demonstrated more than 15 years ago in cardiovascular research but until recently it was not realized by the gastrointestinal research community. The function of the gastrointestinal tract is to propel food by peristaltic motion, which is a result of the interaction of the tissue forces in the wall and the hydrodynamic forces in the food bolus. To understand the tissue forces, it is necessary to know the stress-strain relationships of the tissues that must be measured in reference to the zero-stress state. It has become clear that the zero-stress configuration of the gastrointestinal tract is very different from that of the no-load condition and that the zero-stress state is sensitive to structural and mechanical remodeling. The purpose of this review is to describe the basic theory and experiments of residual stress and to explore its physiological and pathophysiological implications in the gastrointestinal system. The morphometry at no-load and zero-stress states and residual circumferential strains were determined along the pig duodenum in vitro in seven pigs. The no-load state was obtained by cutting eleven 2-mm-wide rings at 10% intervals along the duodenum. The zero-stress state was obtained by cutting the rings radially. The zero-stress state provides a standard morphological state to describe tissue since internal and external forces do not affect the tissue. The morphometric measures were obtained from digitized images, and the layer thicknesses were measured from histological sections. The mucosal and serosal circumferences, the wall thickness, and the wall thickness-to-mucosal radius ratio were largest in the proximal end of the duodenum (f > 1.9, P < 0.05). The thickness of the submucosal stratum compactum layer and the opening angle increased in distal direction (f = 2.3, P < 0.05 and = 6.5, P < 0.001). The residual strain at the mucosal surface was negative, indicating that the mucosa-submucosa layers of duodenum in no-load state are in compression. Distension experiments showed that the residual strain makes the stress distribution through the wall more uniform in the pressurized state. In conclusion, the large circumferential residual strains must be taken into account in the study of physiological problems, in which the stresses and strains are important, eg, the bolus transport function. Impedance planimetry and high-frequency ultrasound were used to determine circumferential stress and strain from measurements of luminal cross-sectional area and wall thickness during balloon distension of the guinea pig esophagus in vitro (N = 30). The excised esophagus was mounted on two plastic tubes in an organ bath containing oxygenated calcium-free Krebs-Ringer solution with 10(-2) M MgCl2 to abolish smooth muscle contractile activity. One of the plastic tubes was movable in order to stretch the esophagus longitudinally by 15% (elongated state). The impedance planimetry probe was placed with the balloon inside the lumen of the esophagus. A 20-MHz ultrasound transducer was mounted above the esophagus and provided scans in the transverse and longitudinal directions. The luminal cross-sectional area at the highest applied pressure of 2.9 kPa was 13.3 +/- 0.3 mm2 in the resting state. In the elongated state the luminal cross-sectional area at the highest pressure was 12.5 +/- 0.1 mm2 (P < 0.02). The wall thickness decreased from 990 +/- 21 microm at 0 kPa to 640 +/- 9 microm at 2.9 kPa at in vitro length. In the elongated state, the values were 940 +/- 32 microm to 480 +/- 13 microm (P < 0.01). The stress-strain relation was exponential (sigma = alpha(ebetaepsilon - 1), r2 > 0.98, P < 0.01). The circumferential elastic modulus calculated at a Green strain of 0.95 was 44.5 +/- 10.5 kPa in the in vitro state and 81.7 +/- 13.1 kPa in the elongated state. The efficiency of prophylactic antibiotic therapy in amputation surgery was studied in a prospective, randomized trial of a first-generation cephalosporin (cephalothin) compared with a narrow-spectrum beta-lactam stable penicillin (methicillin). Eighty-eight patients received cephalothin 2 g X 4 on the day of operation, while 86 patients received methicillin 1 g X 4. The patients were followed up for 21 days. Infected wounds occurred in 14.8% of the patients in the cephalothin group, compared with 14% in the methicillin group. The frequency of deep infections was 10.2% versus 4.7% (P = 0.1611). The reamputation frequency was 18.2% in the cephalothin group compared with 12.8% in the methicillin group; the frequency of below-knee reamputation was 18.4% versus 7.7% (P = 0.1469). No clostridial infections were found. The study did not demonstrate any significant difference between cephalothin and methicillin in the prophylaxis for lower-extremity amputations, although the latter drug tended to be the best choice. The aim of the study is to devise a method for direct measurements of pressure, cross-sectional area (CSA) and wall thickness of a vessel when pressurised. Segments of porcine descending thoracic aorta from 22 and 35 kg pigs (in groups 1 and 2, respectively) and of abdominal aorta from 35 kg pigs are studied in vitro. Impedance planimetry provides measurements of the luminal pressure and CSA of the aorta. The wall thickness is measured simultaneously by means of 20 MHz A-mode ultrasound. The pressure, CSA and wall thickness are registered at different pressure at steady state. At maximum pressure, the CSAs are 107 +/- 10, 215 +/- 19 and 257 +/- 17 mm2 in the abdominal, group 1 and 2 segments, respectively. There is a difference between the abdominal group and groups 1 and 2 (p < 0.05), but not between group 1 and 2 segments (p > 0.2). At maximum pressure, the wall thickness is 0.52 +/- 0.04, 0.79 +/- 0.07 and 1.07 +/- 0.02 mm, in the abdominal group 1 and 2 segments, respectively (p < 0.05). The circumferential stress-strain relationship accorded well with stress = a x exp(b x strain). A pattern recognition algorithm has been developed to discriminate between artefacts and contractions in interdigestive motility recorded by a pressure catheter with four channels from the human duodenum. A learning and a test set, both containing natural and induced artefacts, such as respiration and body movement, are obtained from five volunteers. The event classes were phase I, II and III contractions of the interdigestive motility complex and artefacts from respiration, cough, calibration signals and movements. Length, area, amplitude, inter-event interval, up- and downstroke, and correlation to other pressure channels and to respiration, are applied to classify the events. The sensitivity of the computer scoring increases with the number of applied features. When all the features are applied, the sensitivity of the Bayes' classifier against the visually scored contractions and artefacts is 0.96 with a specificity of 0.69. [ABSTRACT FROM AUTHOR]

Measurement of pressure–volume relations is a commonly used technique to elucidate small intestinal stiffness. There is a lack of data on the relation between stiffness and history-dependent mechanical properties of the gastrointestinal tract. We aimed to distinguish between passive properties of the tissue that depend on the time-history of load (viscoelastic effects) versus those that depend on the maximum previous load (strain softening effects). Ten repeated pressure–volume relations were measured at each peak pressure level in six isolated, passive guinea-pig jejuni in vitro during balloon inflation and deflation cycles. With inflation to a new higher peak pressure (ranging from 3 to 15 mm Hg), the pressure–volume relation became less stiff, particularly in the low pressure range, without a significant change in unloaded jejunal volume. We computed the jejunal normalized volume change as a function of the integrated volume–time history and maximum volume. Analysis of covariance revealed significant dependence of the normalized volume change on the volume–time history (P <; 0.001) and the maximum volume history (P <; 0.001). Multiple linear regression analysis showed that approximately 90% of the history dependence could be attributed to the maximum volume. Most softening (loss of stiffness) happens in the low pressure range of the curve (0–3 mm Hg). We adopted the Johnson and Beatty strain softening theory and computed the volume amplification factor. This factor was shown to be a linear function of the normalized peak volume (r2 > 0.999). Since strain softening effects were significantly greater than viscoelastic effects, we conclude that history-dependent changes in jejunal stiffness are more likely to involve alterations to elastic rather than viscous structures in the tissue. These effects must be taken into account when performing balloon distension studies in the gastrointestinal tract for studying physiological and pathophysiological problems in which loading conditions are altered, e.g., mechanoreceptor studies in normal intestine and in acute and chronic obstruction, in order to have an accurate description of the biomechanics. © 1998 Biomedical Engineering Society. We compared the stress-strain distributions obtained from isolated segments of the guinea pig duodenum, jejunum, and distal ileum, and the relation between the elastic properties and the collagen content. The segments were immersed in Krebs-Ringer solution containing 10(-2) M MgCl2 to abolish contractile activity. Stepwise inflation of an intraluminal balloon in which the cross-sectional area (CSA) was measured provided the luminal pressure-loading stimulus. The wall thickness was measured by means of 20-MHz A-mode ultrasound. The stress-strain and the incremental elastic modulus-strain distributions were derived from the steady-state values of wall thickness, internal radius, and applied pressure. The CSA-pressure relations and the wall thickness-pressure relations were nonlinear and both differed between the segments (P < 0.01). The stress-strain distributions showed an exponential behavior that fitted well to the equation Y = a x Exp(b x X) (r2 = 0.97 +/- 0.01). The intercept with they axis (a) and the slope of the curves (b) differed between the segments (P < 0.01 and P < 0.05). The collagen contents were 3.99 +/- 0.18 microg/mg, 2.51 +/- 0.13 microg/mg, and 2.10 +/- 0.11 microg/mg in the duodenum, jejunum, and distal ileum, respectively. This difference was significant among all three locations (P > 0.05). The dose- and time-dependent effects of endoscopic sclerotherapy on luminal cross-sectional area and wall distensibility were studied in pigs at 5 and 12 cm proximal to the gastroesophageal junction by means of impedance planimetry. Sixteen healthy animals underwent two sessions of endoscopic sclerotherapy two weeks apart with injections of either 5 or 10 ml of 1% Polidocanol in the distal 7 cm of the esophagus each time. The animals were investigated before sclerotherapy, two weeks after each session, and finally six weeks after the last session. Six healthy animals were studied as controls. Endoscopic sclerotherapy caused luminal narrowing in the sclerosed zone followed by normalization six weeks after the last treatment (P < 0.05 in both groups). Wall distensibility decreased in the sclerosed zone after treatment with 10 ml sclerosant (P < 0.05) followed by partial normalization, while no effect was found after 5 ml sclerosant (P > 0.2). Progressive dilations were observed in the proximal esophagus in both groups and were most pronounced in the 10 ml group (P < 0.05). This study was designed to evaluate the distensibility and secondary peristalsis of the esophagus in patients suffering from systemic sclerosis with severe esophageal involvement. Balloon distension with impedance planimetric measurement of luminal cross-sectional area was done 7 and 15 cm above the lower esophageal sphincter in 13 patients and nine healthy controls. The controls were studied both with and without receiving the anticholinergic drug butylscopolamine. The cross-sectional area--pressure relations were nonlinear with the largest cross-sectional area in patients at both measuring sites when compared to controls (P < 0.001). The anticholinergic drug butylscopolamine increased the cross-sectional area in controls (P < 0.001). The cross-sectional area distensibility, defined as CSA0(-1) delta CSA delta P-1 did not differ between patients and controls. Balloon distensions elicited contractions proximal to the distension site. The amplitude and frequency of contractions at the distal distension site were significantly reduced in the patients when compared to the controls (P < 0.05). In conclusion, the distal esophagus is most severely affected in patients with systemic sclerosis with increased cross-sectional area and impaired peristalsis. [ABSTRACT FROM AUTHOR]

Use of postmenopausal hormone replacement therapy (HRT) has been hypothesised to be associated with a reduced risk of non-Hodgkin's lymphoma (NHL), but the epidemiologic evidence is conflicting. To examine the risk of NHL in HRT users aged 40 and older, we conducted a cohort study in the County of North Jutland, Denmark (population 0.5 million) using data from population-based health registries for the period 1989-2002. We computed age-standardised NHL incidence rates and used Cox regression analysis to compute the relative risk (RR) and corresponding 95% confidence intervals (CI) of NHL among HRT users compared with non-users, adjusting for age and calendar period. The number of prescriptions redeemed (1, 2-4, 5-9, 10-19, or 20 or more prescriptions) was used as a proxy for duration of HRT. We identified 40 NHL cases among HRT users during 179 838 person-years of follow-up and 310 NHL cases among non-users during 1 247 302 person-years of follow-up. The age-standardised incidence rates of NHL were 25.7 per 100,000 among HRT users and 24.2 per 100,000 among non-users, yielding an adjusted RR of 0.99 (95% CI: 0.71-1.39).
