Objectives: Justification of medical exposures from medical imaging is fundamental to radiation protection. Referral guidelines are intended to help physicians decide when an imaging study is justified. For two decades, referral guidelines have been a legally binding requirement for European Union member states. Recently, the European Society of Radiology (ESR) developed iGuide tool, which provides evidence-based referral guidance for imaging inclusive of children. The aim of this survey was to assess the availability, use and familiarity of referral guidelines for medical imaging in children and knowledge about the availability of ESR iGuide among ESR member radiologists. Methods: Over a 2-month period (15 September–15 November 2019), 33,257 ESR member radiologists were invited to respond to an anonymised web-based questionnaire, which consisted of 12 multiple-choice questions. Results: In total, 2067/33,257 responses (6.3%) were received from 52 countries. A total of 1068 out of 2067 (51.7%) respondents were aware that imaging referral guidelines are a legal requirement. One thousand five (48.6%) of all respondents did not know whether dedicated guidelines for imaging in children were available, and only 653 (31.2%) were aware of the mainstays of the available guidelines. Similarly, just 746 (36.1%) of all respondents were aware of ESR iGuide availability and features. Conclusions: The information gathered confirms that effective and widespread adoption of imaging referral guidelines is lacking, especially in children. Further work is required to improve uptake and awareness. In a previous work, we have presented an innovative theoretical model to describe the evolution of the life cycle of a new technology. We have proposed a mathematical approach based on a rate equation, similar to that used to describe quantum level transitions. The model is able to describe the hype curve evolution in many relevant conditions, which can be associated with various external parameters. In this article, we apply this model to describe the evolution of the number of publications in some different research fields that are very current and extremely advanced in terms of social impact. Integrated magnetic sensors based on niobium dc SQUID (Superconducting Quantum Interference Device) for nanoparticle characterizations are presented. The SQUIDs consists of two Dayem bridges of 90 nm × 250 nm and loop area of 4, 1, and 0.55 μm. The devices are realized by using an e-beam lithography nano-fabrication process which can directly pattern the devices in an electron-positive resist and then transferred to a 20 nm single niobium layer by a lift-off post-process. The SQUIDs were designed to have a hysteretic current-voltage characteristic in order to work as a magnetic flux-current transducer. The presence of an integrated niobium coil, tightly coupled to the SQUID, allows us to easily excite the SQUID and to flux bias the SQUID at its optimal working point. Current-voltage characteristics, critical current as a function of the external magnetic field and switching current distributions were performed at liquid helium temperature. A critical current modulation of about 20% and a current-magnetic flux transfer coefficient (responsivity) of 30 μA/Φ have been obtained, resulting in a magnetic flux resolution better than 1 mΦ. The authors performed preliminary measurements with and without iron oxide nanoparticles on the SQUID loop in order to show the device sensitivity in view of nano-magnetism applications. Purpose: Tunneled indwelling central venous catheters (CVC) are essential in the management of children with cancer, hematological, nephrological disorders and for parenteral nutrition. The aim of this study is to present the experience of a single center of the transition from traditional open surgical cut down procedure (OSC) to ultrasound (US)-guided percutaneous CVC insertion, focusing on learning curve and related complications.Methods: All CVCs inserted between April 2008 and November 2009 in children at the Gaslini Children Hospital were revised, and data on methods of cannulation, intraoperative and device-related complications and re-intervention were recorded.Results: 194 CVCs were positioned in 188 patients. 128 out of 194 CVCs were positioned through an OSC technique, whereas the remaining 66 CVCs were inserted percutaneously with US guidance. Of the 27 recorded complications, 15 were mechanical events, 7 cases developed infection, whereas the remaining 5 (2.6%) were classified as intraoperative complications. A second surgical procedure was described in 23 (11.8%) cases.Conclusion: Shifting from OSC to US-guided percutaneous CVC insertion inevitably involves a challenging learning curve which is generally associated with high complication rates. Invasive mycoses represent a rare but severe complication following hemopoietic SCT (HSCT) in children. Their incidence is related to the type of donor, being higher after allogeneic transplant, especially from alternative donors. Moreover, the incidence of invasive mycoses varies in the different post transplant phases. Neutropenia, lymphopenia, GvHD, high-dose steroids or other immunosuppressive drugs represent well-known risk factors. The clinical features of invasive mycoses after HSCT in children are similar to those observed in adults, and the diagnostic tools, including Aspergillus galactomannan antigen detection, are feasible also in pediatrics. In May 1999, a cluster of cases of African tick-bite fever was detected in six Italian tourists who had returned from South Africa. All of the patients had moderate fever and cutaneous eschars. Regional lymphangitis was observed in three of the patients and skin rash in two. By comparing the number of eschars with the number of detectable bite sites it was suggested that at least two-thirds of the biting vectors were capable of transmitting Rickettsia africae. The clinical course of disease was mild in all cases, and all but one of the patients recovered spontaneously before antibiotic treatment was initiated. We present experiments on macroscopic quantum effects in Josephson junctions with different relevant junction parameters at a temperature well above the classical-quantum crossover temperature. This has been possible by extending the measurements on the escape rate out of the metastable state at higher sweeping frequency (dI/dt up to 100 A/sec.) in order to induce non-stationary conditions in the energy potential describing the junction dynamics. The non-stationary regime for the system allows a direct observation of energy level quantitation when the rate of change of the external energy, measured in terms of the level spacing, is fast with respect to the thermal transitions between levels. Wilms' tumor (WT) usually has a good outcome, although a poor prognosis is often related to more advanced stages and anaplastic features. Apoptosis occurs with variable frequency in malignant tumors, and may have a role in reducing their growth rate. The bcl-2 proto-oncogene inhibits apoptosis, and the consequent increase in the number of cells may play a role in the development of tumors. The aim of this study was to analyze the role of apoptosis and bcl-2 expression in WT. Twenty-six resected WT specimens were studied; 12 patients had stage I tumor, 4 stage II, 5 stage III, 3 stage IV, and 2 stage V. Twenty-three tumors were classified as favorable histology (FH) and 3 as unfavorable (UH). The mean follow-up was 34 months; 22 patients were alive and 4 were dead (2 with FH: 1 stage III and 1 stage IV, and 2 with UH stages 4). Apoptosis was detected by the in-situ end-labelling technique; bcl-2 expression was detected by immunohistochemistry. An apoptotic index (AI) was calculated as the ratio of apoptotic to normal cells in each specimen. The AI was lower in higher tumor stages, with a significant difference between stages I and IV ( P < 0.05). In cases with UH, Al was lower than in tumors with FH (P < 0.01). The AI was also lower in patients who died than in those who survived ( P < 0.01). In all specimens no correlation between AI and bcl- 2 expression was observed. Progression to advanced stages of WT and a poor prognosis f anaplastic tumors may be linked with disruption of the mechanisms that control apoptosis. Bcl-2 does not play a role as a regulator of apoptosis in WT, other oncogenes and tumor-suppression genes may be more involved in inhibiting apoptosis in WT. [ABSTRACT FROM AUTHOR]

We present a theoretical analysis of the resonant macroscopic quantum tunneling in SQUID systems including the effect of both the temperature and the sweeping rate of the external flux, and the study of the phenomenon both in quasi-stationary and non-stationary conditions. A procedure for fabricating all refractory superconducting tunnel junctions with a photoconducting barrier is reported. The process is based on the deposition of a multilayer structure without breaking the vacuum. Both symmetrical Nb/Al-CdS(AlO)-Al/Nb and asymmetrical Nb/Al-CdS(A10)/Nb junctions have been fabricated. The photoconducting behavior of the CdS film gives the possibility of modifying the tunnel barrier transparency during the experiments by a suitable optical input. Results of experimental investigations performed on samples having a barrier thickness in the range 7-50 nm are discussed. [ABSTRACT FROM AUTHOR]

An evaluation of all pediatric patients with primary or secondary pulmonary disease operated upon from January 1993 to July 1996 by the same senior surgeon was carried out. The inclusion criterion was a lung resection in patients aged less than 14 years. Children were divided into two categories according to the neoplastic or non-neoplastic nature of their disease. In the first group a lobectomy was performed for primary lesions and wedge resection for secondary ones. In the second group lobar emphysema and cystic dysplasia were the major indications for lobectomy, while diagnostic wedge resections were performed for interstitial/infiltrative lesions. Several groups of techniques were identified according to the type of approach and the suture method. Video-assisted thoracoscopic surgery and a muscle-sparing approach were compared to classic posterolateral thoracotomy. The mechanical stapler-suturing method was compared to the manual suturing. Our results demonstrate the importance of mechanical suturing, particularly in decreasing anesthesia time and reducing the risk of dehiscence. The minimally invasive approach associated with mini-thoracotomy was particularly useful for patients with reduced oxygen saturation due to ventilatory and gas-exchange problems. The roles of staplers in lung parenchymal resection and minimally invasive procedures for improving the postoperative thoracic compliance of pediatric patients are stressed. [ABSTRACT FROM AUTHOR]

Viral infections are a rare complication in autologous hemopoietic stem cell transplant (HSCT) recipients but represent a frequent cause of disease after allogeneic HSCT. In the last years, there has been an increase in the number of viral diseases observed in these patients. This prospective study was designed to evaluate the incidence of lichen sclerosus et atrophicus (LSA) in a pediatric population with hypospadias and phimosis in order to discuss the indications for circumcision and utilization of preputial skin for urethral surgery. All 115 boys, 55 with congenital phimosis, 45 with acquired phimosis, 13 with hypospadias, and 2 with recurrent chronic balanitis, underwent full-thickness biopsies of the foreskin that were examined by a single pathologist. Of the patients with acquired phimosis, 88% showed inflammatory features in the foreskin; 60% had LSA. Of the patients with congenital phimosis, 82% showed inflammatory disease in the prepuce; 30% had LSA. Of the patients operated upon for hypospadias, 61% showed histologic findings of chronic inflammation of the foreskin and LSA was evident in 15%. The high incidence of LSA in the prepuce of patients with phimosis suggests that circumcision should be performed to correct this disease. The frequent presence of chronic inflammation is a possible cause of stenosis when the foreskin is used to perform a urethroplasty in patients with hypospadias. [ABSTRACT FROM AUTHOR]

The unique deformability and the compliance ability of thin sheets on soft substrates attract much interest for studying the phenomena related to elastic instabilities as well as for sensing very weak forces such as those generated by live cells in vitro. However, the techniques used currently for producing such platforms are affected by a high degree of complexity and poor repeatability. Moreover, their deformability is usually used as a passive response to the action of an external force. Herein we propose a novel concept for a reliable and dynamic skin-over-liquid system made of a periodic array of highly compliant microbumps actuated through electrode-free electrohydrodynamic (EHD) pressure. We demonstrate that these structures are highly repeatable and capable of swelling and deflating easily under a simple thermal stimulation driven by the pyroelectric effect, thus providing a challenging platform that can be actively controlled at the microscale. Furthermore, we show the proof of principle by swelling these microbumps for mechanically stimulating live cells in vitro, thus opening the route to more reliable and easy to accomplish assays in the field of mechanobiology. Researchers curious about how mechanical stress impacts cell growth can now turn to silicone plastics that grow microbumps on demand. Simonetta Grilli from Italy's Institute of Applied Sciences and Intelligent Systems in Pozzuoli and colleagues created a controllable 'skin-over-liquid' system by coating a silicone polymer solution onto lithium niobate, a crystal that contains patterned regions of electric charge. An initial plasma treatment produced a flat stiff film on top of the liquid silicone. Thermal heating caused the skin to swell and form dimpled, air mattress-like patterns, corresponding to the underlying lithium niobate domains. Removal of heat restored the sheet to its initial state. The chemically inert skin supported live fibroblast cells and was used to determine reactions to mechanical stress. Significant variations in nuclei and cytoskeletons were observed between cells grown under flat or swollen-skin conditions. The authors report a rate case of carcinoid syndrome (CS) in a 12-year-old child that was caused by a carcinoid tumor located in the ileum that had metastasized mainly to the liver. After resection of the primary tumor, treatment with octreotide and polychemotherapy (five monthly cycles of 5-flourouracil, epidoxorubicin, and deticene) was ineffective for both reducing the metastatic liver disease and controlling the clinical symptoms. The patient's poor prognosis led the authors to perform a liver transplantation. The results of liver transplantation in patients with metastatic liver disease are generally not good. However, neuroendocrine tumors seem to be an exception due to their slow growth. At present, the child is in complete clinical and laboratory remission 4 months after transplantation.
