This article delves into the distinctive intra- and interregional geographical heterogeneity of Italy, emphasizing demographic and socio-economic variations and the role of foreign employment, considering the labour market as a fundamental driver for migration and local inclusion. The article identifies a gap in understanding the employed foreign population as a multiscale process in Lombardy and Campania, representative regions as case studies from the North and South divide using a MGWR approach. The results reveal contrasting effects of the Italian labour force's unemployment rate (URI). In Lombardy, a positive effect suggests working competition between labour force components while, in Campania, the relation is less clear. The analysis underscores significant local heterogeneity, emphasizing the importance and urgency of employing local scale analysis for accurate statistics. The study emphasizes the multiscale nature of the analysed process, demonstrating variable effects across different regional contexts. While the study is limited to two regions and cross-sectional data, it marks the first attempt in Italy to address the foreign presence as a multiscale process, highlighting the need for localized and multiscale approaches in understanding spatial processes related to demography and population issues. Heart failure (HF) and chronic kidney disease (CKD) are two pathological conditions with a high prevalence in the general population. When they coexist in the same patient, a strict interplay between them is observed, such that patients affected require a clinical multidisciplinary and personalized management. The diagnosis of HF and CKD relies on signs and symptoms of the patient but several additional tools, such as blood-based biomarkers and imaging techniques, are needed to clarify and discriminate the main characteristics of these diseases. Improved survival due to new recommended drugs in HF has increasingly challenged physicians to manage patients with multiple diseases, especially in case of CKD. However, the safe administration of these drugs in patients with HF and CKD is often challenging. Knowing up to which values ​​of creatinine or renal clearance each drug can be administered is fundamental. With this review we sought to give an insight on this sizable and complex topic, in order to get clearer ideas and a more precise reference about the diagnostic assessment and therapeutic management of HF and CKD. Purpose of Review: Heart failure (HF) and erectile dysfunction (ED) are two common conditions that affect millions of men worldwide and impair their quality of life. ED is a frequent complication of HF, as well as a possible predictor of cardiovascular events and mortality. ED deserves more attention from clinicians and researchers. Recent Findings: The pathophysiology of ED in HF involves multiple factors, such as endothelial dysfunction, reduced cardiac output, neurohormonal activation, autonomic imbalance, oxidative stress, inflammation, and drug side effects. The diagnosis of ED in HF patients should be based on validated questionnaires or objective tests, as part of the routine cardiovascular risk assessment. The therapeutic management of ED in HF patients should be individualized and multidisciplinary, considering the patient's preferences, expectations, comorbidities, and potential drug interactions. The first-line pharmacological treatment for ED in HF patients with mild to moderate symptoms (NYHA class I–II) is phosphodiesterase type 5 inhibitors (PDE5Is), which improve both sexual function and cardiopulmonary parameters. PDE5Is are contraindicated in patients who use nitrates or nitric oxide donors for angina relief, and these patients should be advised to avoid sexual activity or to use alternative treatments for ED. Non-pharmacological treatments for ED, such as psychotherapy or couples therapy, should also be considered if there are significant psychosocial factors affecting the patient's sexual function or relationship. Summary: This review aims to summarize the most recent evidence regarding the prevalence of ED, the pathophysiology of this condition with an exhaustive analysis of factors involved in ED development in HF patients, a thorough discussion on diagnosis and management of ED in HF patients, providing practical recommendations for clinicians. Cangrelor, the first intravenous P2Y12 inhibitor (P2Y12-I), has been approved on the basis of three large RCTs from the CHAMPION program which nevertheless have been criticized for the low bleeding risk of the enrolled patients, the large quote of chronic coronary syndromes, and the use of Clopidogrel as control arm even in the setting of acute coronary syndromes (ACS). We sought to investigate, in the setting of ACS, the comparative performance of Cangrelor in terms of in-hospital ischemic and haemorrhagic outcomes compared with the current gold-standard of oral P2Y12-I. The study retrospectively enrolled 686 consecutive patients admitted to the Divisions of Cardiology of Policlinico of Bari and L. Bonomo Hospital of Andria for ACS and treated with percutaneous coronary intervention. The study population was divided according to the P2Y12-I treatment strategy in two groups: patients given an oral P2Y12-I and patients receiving Cangrelor in the cath lab followed by an oral P2Y12-I. Clinical endpoints included death, ischemic and bleeding events occurring during hospital stay. Cangrelor treated patients presented higher clinical risk profile at presentation and faced higher death rate. However, after PS matching, in-hospital mortality resulted comparable between the groups and Cangrelor use was associated with reduced in-hospital definite stent thrombosis (p = 0.03). Data from our real-world registry highlight that, in the setting of ACS, Cangrelor is prevalently used in patients with very challenging clinical presentations. Studies on fertility determinants have frequently pointed to the role that socio-economic, cultural and institutional factors play in shaping reproductive behaviours. Yet, little is known about these determinants at an ecological level, although it is widely recognised that demographic dynamics strongly interact with ecosystems. This research responds to the need to enhance the knowledge on variations in fertility across space with an analysis of the relationship between fertility and population density of Italians and foreigners in Italy at the municipal level for the period 2002–2018. Using global and local autocorrelation measures and a spatial Durbin model, we show that there is a negative association between the fertility and population density of the Italian population, while the density of foreigners is correlated with higher fertility. This second result poses new insights on the relationship between space and fertility. Moreover, we find that the features of neighbouring areas, measured by population density, contribute significantly to explaining spatial fertility variation, confirming the importance of the study of spatial diffusion in demographic processes. This study explores whether the association between living in a single-parent household and children's educational outcomes differs by migration background through comparing natives with first- and second-generation migrant pupils from different areas of origin. While there is strong evidence of an educational gap between migrant and native pupils in Western countries—and particularly in Italy—the interaction with family structure has been under-investigated. We suggest that native children have more socioeconomic resources to lose as a consequence of parental breakups, and thus may experience more negative consequences from living in a single-parent household compared to migrant children, who tend to have poorer educational outcomes regardless of family disruptions. Moreover, for migrant children, family disruption could result from parents' migratory project (transnationalism) rather than separation or divorce, thus not necessarily implying parental conflict and a deteriorating family environment. Empirical analyses of data from the ISTAT 'Integration of the Second Generation' survey (2015) show that native Italian pupils from single-parent households in lower secondary schools are more strongly penalised in terms of grades, and less likely to aspire to the most prestigious upper secondary tracks when compared to second- and, especially, first-generation children. Indeed, the latter have been found to experience virtually no negative consequences from parental absence. Contrary to expectations, we found no substantial differences in the non-intact penalty based on the reason for parental absence (transnationalism vs divorce), nor by migrants' area of origin. International literature has shown that demographic characteristics, socioeconomic status, social ties and supports in the host country play an important role on migrants' subjective wellbeing. The influence of such characteristics has been widely studied so far, especially on immigrants' levels of life satisfaction, but how living transnational partnerships and parenthoods might affect the subjective wellbeing of migrants and how this situation might be reflecting social integration issues still remains poorly understood. This paper is aimed at covering this gap by analyzing each one of the components of subjective well-being: own assessment of life satisfaction (cognitive component), feelings of loneliness (emotional component) and sense of belonging (affective component). Ordinal logistic regressions were run on a selected sample of the Survey on Social Conditions and Integration of Foreign citizens carried out by ISTAT in 2011/2012. Results have shown that living in transnational families differently affect subjective wellbeing, being particularly important for the cognitive and emotional components among married immigrants. Self-perceived social integration has been found to be a protective factor of immigrants' levels of subjective wellbeing, having a preventive role on their levels of life satisfaction and feelings of loneliness. Researchers have devoted much attention both to the analysis of the first sexual experience and to how the couple was established, but little is still known about age differences of partners at their first sexual relationship. The availability of two highly comparable waves of a survey on the sexual behavior of college students in Italy (SELFY—Sexual and Emotional LiFe of Youth) carried out in 2000 and 2017 allowed us to study the predictors of age differences between partners at first sex, filling the existing gap on recent research. Results of multivariate analyses show important gender differences on mate selection: women tend to choose an older partner for having their first sexual experience and are less likely as men to be involved in age discordant first sex relationships with a younger partner. Age gaps between partners also influence age at sexual debut, which tends to occur earlier in a relationship with an older partner and later if having first sex with a younger partner. Another important predictor of the age gap is the type of relationship that linked the respondent to its partner at first sex. Our estimations indicate a lower likelihood of having had an older first sex partner for students who had their first sexual experience with the own boy/girl-friend or with a friend compared to those who have had it with a stranger. Finally, we have found a higher likelihood of first sex relationships among same-age partners relative to older partners through SELFY waves and small changes on variables influencing such relationships.
