Purpose: Investigating the use of the EORTC bladder cancer (BC) modules by evaluating: (a) study contexts/designs; (b) languages/countries in which the modules were administered; (c) their acceptance by patients/investigators; and (d) their psychometric properties. Methods: A systematic review was performed with studies from 1998 until 20/10/2021 in five databases. Articles/conference abstracts using the EORTC-QLQBLM30 (muscle invasive BC) and the EORTC-QLQNMIBC24 (previously referred to as QLQ-BLS24; non-muscle invasive BC) were included. Two authors independently screened titles/abstracts/full-texts and performed data extraction. Results: A total of 76 eligible studies were identified. Most studies included the BLM30 (n = 53), were in a urological surgery context (n = 41) and were cross-sectional (n = 35) or prospective (n = 30) in design. The BC modules were administered in 14 languages across 19 countries. Missing data were low-moderate for all non-sex related questions (< 1% to 15%). Sex-related questions had higher rates of missing data (ranging from 6.9% to 84%). Most investigators did not use all scales of the questionnaires. One validation study for the original BLS24 led to the development of the NMIBC24, which adopted a new scale structure for which good structural validity was confirmed (n = 3). Good reliability and validity was shown for the NMIBC24 module, except for malaise and bloating/flatulence scales. Psychometric evidence for BLM30 is lacking. Conclusion: These results provide insight into how the EORTC BC quality of life modules could be further improved. Current work is ongoing to update the modules and to determine if the two modules can be combined into a single questionnaire that works well in both the NMIBC and MIBC settings. Recent earthquakes as the 2012 Emilia earthquake sequence showed that recently built unreinforced masonry (URM) buildings behaved much better than expected and sustained, despite the maximum PGA values ranged between 0.20–0.30 g, either minor damage or structural damage that is deemed repairable. Especially low-rise residential and commercial masonry buildings with a code-conforming seismic design and detailing behaved in general very well without substantial damages. The low damage grades of modern masonry buildings that was observed during this earthquake series highlighted again that codified design procedures based on linear analysis can be rather conservative. Although advances in simulation tools make nonlinear calculation methods more readily accessible to designers, linear analyses will still be the standard design method for years to come. The present paper aims to improve the linear seismic design method by providing a proper definition of the q-factor of URM buildings. These q-factors are derived for low-rise URM buildings with rigid diaphragms which represent recent construction practise in low to moderate seismic areas of Italy and Germany. The behaviour factor components for deformation and energy dissipation capacity and for overstrength due to the redistribution of forces are derived by means of pushover analyses. Furthermore, considerations on the behaviour factor component due to other sources of overstrength in masonry buildings are presented. As a result of the investigations, rationally based values of the behaviour factor q to be used in linear analyses in the range of 2.0–3.0 are proposed. [ABSTRACT FROM AUTHOR]

Purpose: The purpose of this study was to examine facilitators of behavior change and weight loss among African-American women who participated in the Moving Forward Efficacy trial.Methods: Linear mixed models were used to examine the role of self-efficacy, social support, and perceived access to healthy eating, exercise, and neighborhood safety on weight, physical activity, and diet. We also examined the mediation of self-efficacy, social support, and perceived access to healthy eating, exercise, and neighborhood safety on weight loss, physical activity, and diet using the Freedman Schatzkin statistic.Results: We found no evidence to suggest mediation, but some direct associations of self-efficacy, certain types of social support and perceived access to exercise on weight loss, and behavior change.Conclusion: We determined that self-efficacy, social support, and perceived access to exercise played a role in weight loss, increased MVPA, and better diet. Background: Three-dimensional (3D) laparoscopy improves the surgical skills of novice surgeons and positively affects the learning curve in experimental settings. This study aimed to investigate the effect of a 3D passive polarizing display (3DPPD) and a novel 3D head-mounted display (3DHMD; HMS-3000MT) on the performance of the first laparoscopic inguinal hernia repair by novices and compare both systems with standard high-definition 2D (HD2D) laparoscopy.Methods: Patients with symptomatic inguinal hernia underwent transabdominal preperitoneal (TAPP) approach hernia repair using 3DHMD, 3DPPD, or a conventional HD2D laparoscopic system. All surgeries were performed for the first time by three laparoscopically novice surgeons. Operative performance was compared in terms of the time taken for mesh placement and peritoneal suturing under standardized conditions. Additionally, visual perception parameters and adverse effects were assessed.Results: The use of both 3D techniques shortened the time required for mesh placement and peritoneal suturing compared with the conventional HD2D approach. Generally, 3D laparoscopy was superior to HD2D laparoscopy in terms of visual perception parameters such as depth perception, sharpness, ghosting, and contrast. However, compared with the use of HD2D laparoscopy, the use of 3DHMD significantly impaired a surgeon's comfort, with the greatest impairment caused by ear discomfort, headaches, and facial and physical discomforts.Conclusions: The 3DHMD and 3DPPD systems showed clear improvement in first hernia repair laparoscopy by novice surgeons in terms of surgical performance, as well as visual perception; however, the 3DHMD system was not superior to the 3DPPD system. Purpose: In the era of biological therapy of ulcerative colitis (UC), surgical treatment frequently consists of colectomy, end ileostomy, and rectal stump closure before patients go on towards restorative proctocolectomy. We aimed to evaluate possible risk factors for the occurrence of postoperative complications and investigate those after initial colectomy in these patients. Methods: Retrospective analysis of 180 patients (76 female, 104 male) undergoing colectomy for UC with formation of a rectal stump and terminal ileostomy between March 2008 and March 2018 at Charité University Hospital Berlin, Campus Benjamin Franklin. A panel of possible postoperative complications was established, patient history was screened, and postoperative complications were analyzed using the Clavien Dindo Classification. Results: Postoperative complication rate was 27.7%. Mortality was 0.5%. Postoperative ileus occurred in 15.3% and rectal stump leakage in 14.8%. Complications were categorized as Clavien Dindo 3 in 80%. Risk factors for surgical complications after multivariate analysis were ASA classification (p = 0.004), preoperative anemia (Hemoglobin < 8 mg/dl) (p = 0.025), use of immunosuppressants (p = 0.003), more than two cardiovascular diseases (p = 0.016), and peritonitis (p = 0.000). Reoperation rate of patients with surgical complications was 27.7%. Conclusion: Colectomy in high-risk UC patients is associated with significant morbidity. However, most of the surgical complications can be treated conservatively. Overall mortality is low. Patient-related risk factors are associated with postoperative complications. Optimizing these risk factors or earlier indication for surgery in the course of UC may help to reduce morbidity of this procedure. [ABSTRACT FROM AUTHOR]

The decay of a massive pseudoscalar, scalar and U(1) boson into an electron-positron pair in the presence of strong electromagnetic backgrounds is calculated. Of particular interest is the constant-crossed-field limit, relevant for experiments that aim to measure high-energy axion-like-particle conversion into electron-positron pairs in a magnetic field. The total probability depends on the quantum nonlinearity parameter — a product of field and lightfront momentum invariants. Depending on the seed particle mass, different decay regimes are identified. In the below-threshold case, we find the probability depends on a non-perturbative tunneling exponent depending on the quantum parameter and the particle mass. In the above-threshold case, we find that when the quantum parameter is varied linearly, the probability oscillates nonlinearly around the spontaneous decay probability. A strong-field limit is identified in which the threshold is found to disappear. In modelling the fall-off of a quasi-constant-crossed magnetic field, we calculate probabilities beyond the constant limit and investigate when the decay probability can be regarded as locally constant. Background: 3D laparoscopy has proven to be superior to the 2D approach in experimental settings. The aim of the present study was to investigate the influence of 3D laparoscopy using a novel head-mounted display on the performance of defined steps within a laparoscopic inguinal hernia repair. This effect was investigated both in laparoscopically advanced surgeons and in beginners.Methods: Patients suffering from symptomatic inguinal hernia were randomly assigned to laparoscopic hernia repair using either a head-mounted 3D display or a conventional 2D laparoscopic approach. Operative performance of both groups was compared in terms of the time taken for mesh placement and for peritoneal suturing. Additionally, quality of imaging and physical discomfort were assessed.Results: The use of a head-mounted 3D display was able to shorten the time required for placement of the mesh as well as that for peritoneal suturing, both for experienced and novice surgeons when compared to the conventional 2D approach. 3D laparoscopy was significantly superior to 2D laparoscopy in terms of depth perception, image sharpness and image contrast. Additionally, increased impairment caused by ghosting effects could not be detected in 3D laparoscopy. Evaluation of image quality was independent of experience in laparoscopic surgery. However, use of a head-mounted 3D display resulted in a significant impairment of surgeon's comfort when compared to 2D laparoscopy. Thereby, the greatest impairment was caused by ear discomfort.Conclusions: This is the first study examining the effect of a head-mounted 3D system on the performance of laparoscopy in a randomized controlled trial (RCT) showing a clear advantage of this system in surgical performance as well as in depth perception and image quality. Despite intensive research efforts peritonitis leading to subsequent sepsis remains associated with a high mortality. The initial effector cells are the locally residing cells of the peritoneum, such as mesothelial cells, mast cells, macrophages and lymphocytes. Through the secretion of chemokines, an influx of neutrophils initially takes place followed by monocytes. The latter can differentiate into inflammatory macrophages. The non-directed activity of neutrophilic granulocytes is limited by the induction of apoptotic programs. Through the breaching of cytokines, bacteria and microbial products into the circulation, a systemic reaction in the sense of systemic inflammatory response syndrome (SIRS) or sepsis arises. This is viewed as a concomitant derailing of inflammatory as well as anti-inflammatory responses, which leads to extensive apoptosis of lymphocytes. The presentation of apoptotic cells leads to a strong immunosuppression. Due to the coexistence of hyperinflammation and immunosuppression, exact knowledge of the current immune status of the patient is a prerequisite in the development of immunotherapies for the treatment of sepsis. This paper presents the results of a series of shake-table tests on a half-scale, four-storey building with reinforced concrete and unreinforced masonry walls. Due to the lack of reference tests, the seismic behaviour of such mixed structures is poorly understood. The test unit was subjected to several runs of increasing intensity yielding performance states between minor damage and near collapse. Before the test, the expected peak table accelerations leading to different limit states were estimated using the capacity spectrum method, and the predicted values corresponded rather well to actual sustained accelerations. Next to these analyses, the paper describes the test unit, instrumentation and input motion, and comments on the response of the mixed structure in terms of damage evolution and global response quantities, such as force-displacement response and drift and acceleration profiles. The raw and post-processed data sets are made publically available, and all relevant information with regard to data organisation and post-processing procedure is described in an appendix to this paper. The test serves therefore as a benchmark for the validation of numerical models of such mixed structures. The project aims at providing a foundation for the development of seismic design and assessment methods of mixed structures, which are currently not covered by structural codes, including Eurocode 8. Recent research showed that the in-plane horizontal displacement capacity of unreinforced masonry (URM) walls depends on numerous factors that are not yet captured by current empirical drift capacity models; e.g., axial stress, shear span, geometry of the walls and the material used. In order to improve the performance-based assessment of URM wall buildings, future research should aim at developing numerical and mechanical models that link the global force-displacement response of URM walls to local deformation measures such as strains. This paper addresses the behaviour of modern clay brick masonry and makes first contributions to such an endeavour by the evaluation of experimental results: first, two sets of limit states are proposed that link local damage limit states to characteristic points of the global force-displacement response of the URM wall. The two sets define limit states for walls developing a shear or a flexural mechanism respectively. Second, local deformation measures deemed suitable for the characterisation of these limit states are evaluated from optical measurement data of quasi-static cyclic wall tests. When testing multi-storey structures, most testing facilities require the testing of a reduced-scale model. A literature review on tests of scaled masonry structural components revealed that scaling of masonry was rather challenging and often significant differences in stiffness, strength and failure mechanisms between the different sized masonry were reported. This paper addresses the scaling of hollow clay brick masonry with fully mortared head and bed joints. We investigate different choices of scaling brick units and mortar joints. Based on the results of an extensive test programme including standard material tests and quasi-static cyclic tests on masonry walls subjected to horizontal and axial loads, we formulate recommendations for the production of a half-scale model of unreinforced masonry structures. The experimental results show a good match between full-scale and half-scale masonry. Assessing maternal dietary habits across Europe during pregnancy in relation to their national pregnancy recommendations. A collaborative, multi-centre, birth cohort study in nine European countries was conducted as part of European Union funded EuroPrevall project. Standardised baseline questionnaire data included details of food intake, nutritional supplement use, exposure to cigarette smoke during pregnancy and socio-demographic data. Pregnancy recommendations were collected from all nine countries from the appropriate national organisations. The most commonly taken supplement in pregnancy was folic acid (55.6 % Lithuania-97.8 % Spain) and was favoured by older, well-educated mothers. Vitamin D supplementation across the cohort was very poor (0.3 % Spain-5.1 % Lithuania). There were significant differences in foods consumed in different countries during pregnancy e.g. only 2.7 % Dutch mothers avoided eating peanut, while 44.4 % of British mothers avoided it. Some countries have minimal pregnancy recommendations i.e. Lithuania, Poland and Spain while others have similar, very specific recommendations i.e. UK, the Netherlands, Iceland, Greece. Allergy specific recommendations were associated with food avoidance during pregnancy [relative rate (RR) 1.18 95 % CI 0.02-1.37]. Nutritional supplement recommendations were also associated with avoidance (RR 1.08, 1.00-1.16). Maternal dietary habits and the use of dietary supplements during pregnancy vary significantly across Europe and in some instances may be influenced by national recommendations. One possible way of mitigating carbon dioxide (CO) emissions from fossil fuel combustion is using carbon dioxide capture and storage (CCS) technology. However, public perception concerning CO storage in the geosphere is generally negative, being particularly motivated by perceived leakage risks. Therefore, a main issue when attempting to gain public acceptance is ensuring provision of appropriate monitoring practices, aimed at providing health, safety and environmental risk assessment, so that potential risks from CO storage are minimized. Naturally occurring CO deposits provide unique natural analogues for evaluating and validating methods used for the detection and monitoring of CO spreading and degassing into the atmosphere. Geological and hydrological structures of the Cheb Basin (NW Bohemia, Czech Republic) represent such a natural analogue for investigating CO leakage and offer a perfect location at which to verify monitoring tools used for direct investigation of processes along preferential migration paths. This shallow basin dating from the Tertiary age is characterized by up to 300 m thick Neogene sediment deposits and several tectonically active faults. The objectives of this paper are to introduce the CO analogues concept to present the Eger Rift as a suitable location for a natural CO analogue site and to demonstrate to what extent such an analogue site should be used (with a case study). The case study presents the results obtained from a joint application of geoelectrical measurements in combination with soil CO concentration and flux determination methods, for the detection and characterization of natural CO releases at gas seeps (as part of a hierarchic monitoring concept). To highlight discharge-controlling structural near surface features was the initial motivation for the application of geoelectrical measurements. Soil-gas concentration and flux measurement techniques are relatively simple to employ and are valuable methods that can be used to monitor seeping CO along preferential pathways. Joint interpretation of both approaches yields a first insight into fluid paths and reveals that the thickness and permeability of site-specific near surface sedimentary deposits have a great influence upon the spatial distribution of the CO degassing pattern at surface level. The purpose of the present study is to show the feasibility of examining hydrogen desorption in technical iron samples using neutron radiography at the ANTARES facility of the FRM II research reactor, Technische Universität München. It has been shown that this method is appropriate for in situ determination of hydrogen desorption for concentrations as low as 20 ppm. Experiments were carried out in the temperature range from room temperature up to 260 °C. Measurement was based on direct comparison between electrochemically hydrogen-loaded iron samples and hydrogen-free reference samples at the same temperature. This enables the determination of hydrogen concentration as a function of time and temperature. Ex situ carrier gas hot extraction experiments using the same temperature-time profiles as the neutron radiography experiments have been used to calibrate the greyscale values of the radiographs to defined hydrogen concentrations. Although Bouvet Island is of considerable importance for Southern Ocean species conservation, information on the marine community species inventory and trophic functioning is scarce. Our combined study of stable isotopes and feeding relationships shows that (1) the marine system conforms to the trophic pattern described for other Antarctic systems within the Antarctic circumpolar current (ACC); (2) both the benthic and the pelagic subsystem are almost exclusively linked via suspended particulate organic matter (SPOM); and (3) there is no evidence of a subsystem driven by macroalgae. Bouvet Island can therefore be characterized as a benthic “oasis” within a self-sustaining open ocean pelagic system. [ABSTRACT FROM AUTHOR]

The results from several genome scans indicate that chromosome 2q21-q33 is likely to contain an autism susceptibility locus. We studied the potential contribution of nine positional and functional candidate genes: TBR-1; GAD1; DLX1; DLX2; cAMP-GEFII; CHN1; ATF2; HOXD1 and NEUROD1. Screening these genes for DNA variants and association analysis using intragenic single nucleotide polymorphisms did not provide evidence for a major role in the aetiology of autism. Four rare nonsynonymous variants were identified, however, in the cAMP-GEFII gene. These variants were present in five families, where they segregate with the autistic phenotype, and were not observed in control individuals. The significance of these variants is unclear, as their low frequency in IMGSAC families does not account for the relatively strong linkage signal at the 2q locus. Genetic studies indicate that chromosome 7q is likely to contain an autism susceptibility locus (AUTS1). We have followed a positional candidate gene approach to identify relevant gene(s) and report here the analysis of reelin (RELN), a gene located under our peak of linkage. Screening RELN for DNA changes identified novel missense variants absent in a large control group; however, the low frequency of these mutations does not explain the relatively strong linkage results on 7q. Furthermore, analysis of a previously reported triplet repeat polymorphism and intragenic single nucleotide polymorphisms, using the transmission disequilibrium test, provided no evidence for association with autism in IMGSAC and German singleton families. Abstract The interaction of the cyclic decapeptide antibiotic gramicidin-S (GrS) with the nonionic detergent octaethylene glycol mono-n-dodecyl ether was studied by NMR spectroscopy. Detergent binding led to a slightly altered average conformation in the D-Phe side chains of the peptide. The changing diamagnetic shielding of nearby protons resulted in chemical shift variations, the largest effect being observed for the D-Phe C[sub a] proton. The continuous upfield shift of this proton resonance, indicating rapid exchange of the peptide between detergent-associated and unassociated states, was employed for an evaluation of the detergent/peptide aggregation equilibria. The nonlinear binding plot thus obtained was attributed to essentially different aggregational states, depending on the detergent/peptide ratio. The almost linear dependence of the spin-lattice relaxation rate and of the hydrogen-deuterium exchange rate on the fraction of detergent-associated GrS could be reconciled with a simple model, comprising binding of detergent monomers and cooperative binding of micelles at low and high detergent/peptide molar ratios, respectively. Thus, GrS provides a useful model for a study of backbone dynamics and water penetration in detergent and membrane-bound peptides and proteins. Neutron activation analysis (NAA) is a useful method to determine the concentration of trace elements in biological samples. But it has its special problems and error sources. An extensive study was performed to prove that weight loss affects both accuracy and precision of NAA based on post-irradiation weights. Weight losses of 5 ± 3% and 14 ± 7% (mean ± SD) were observed after irradiating blood and its main components for more than 12 h with thermal neutron fluxes of about 10n · cm · s in two reactors with different cooling facilities. These losses were flux-dependent, but independent of irradiation time, sample donor, type and size, and post-irradiation time and handling. In order to avoid this NAA-specific error elemental concentrations should be based on pre-irradiation weights. [ABSTRACT FROM AUTHOR]

Metabolic oxidative profiles of diazepam (I) were obtained by aromatic C-4′-hydroxylation, N-1-demethylation, and 3-hydroxylation using a supernatant of rat liver. Incubations of 3-methyldiazepam (VI), which suppressed 3-hydroxylation, and N-1-nor-3-methyldiazepam (VII), were used to separately investigate these three oxidative pathways. Treatment of animals with phenobarbital enhanced N-1-demethylation and 3-hydroxylation, and to a variable extent C-4′-hydroxylation. Application of metyrapone reduced metabolite formation by 3-hydroxylation and N-1-demethylation, but had no effect on C-4′-hydroxylation. Metyrapone inhibition was more pronounced following than prior to phenobarbital treatment. C-2-hydroxylation was studied using medazepam (XX) incubations. This pathway was increased by phenobarbital pretreatment and reduced by metyrapone inhibition which was again more pronounced following than prior to phenobarbital pretreatment. These results support earlier conclusions on the heterogeneity of liver microsomes and suggest the presence of different species of hepatic microsomal terminal oxidases. Phenobarbital treatment and metyrapone change the metabolic profile via induction and inhibition, respectively, and, thus, in the case of 1,4-benzodiazepines, the formation of metabolites with varying pharmacological activity. This could become important in clinical situations as a diagnostic mean to determine induction under various treatment or, possibly, during cumulation of metabolites with a long half-life. Masonry aggregates have developed throughout city centres of Europe due to a centuries-long densification process that generally lacked consistent planning or engineering. Adjacent units are connected either through interlocking stones or a layer of mortar. Without interlocking stones, the connection between the units is weak, and an out of-phase response of the units can lead to separation and pounding. Modelling guidelines and code instructions are missing for modelling the interaction of such adjacent units because of scarce experimental data. Therefore, in this study an unreinforced stone masonry aggregate was tested on the bidirectional shake table with an incremental seismic protocol as a part of the SERA AIMS—Adjacent Interacting Masonry Structures project. The aggregate was constructed at half-scale with double-leaf undressed stone masonry without interlocking between the units. Floors were built with timber beams and one layer of planks, with different beam span orientation for each unit. After significant damage, one of the units was retrofitted by anchoring the timber beams to the walls to prevent out-of-plane failure and testing was continued. Significant interaction between the units was observed with specific damage mechanisms. Cracking and separation were observed at the interface in both longitudinal and transverse direction, starting at lower intensity runs and progressively increasing. Bidirectional seismic excitation affected the unit separation, with friction forces seemingly playing a role in the transverse direction. Signs of pounding at the interface were observed during higher intensity runs, together with the formation of a soft storey mechanism at the upper storey of the higher unit. The mechanism involved an out-of-plane response of the shared wall, with a horizontal crack at the height of the interaction.
