While cash will eventually become a thing of the past, marketing researchers have given little attention to the rise of cashless markets and the obstacles and opportunities they present. In fact, research that addresses the strategic approach to planning, coordinating, and executing the cashless adoption and experience for consumers is scant. To stimulate discussion and scholarly investigations into marketing's contribution toward the evolution of cashless economies, this Idea Corner presents a research agenda that delineates the role of DPS 2.0, a new era of digital payment systems, in fueling the demonetization process. We offer that, compared to traditional payment systems (DPS 1.0), DPS 2.0 provides consumers and merchants cashless, virtual, automated, flexible, faster, and interoperable (The ability of DPS 2.0 systems to be compatible and operable across providers, software, and payment portals) means of payment. However, the promise of DPS 2.0 is clouded with concerns of opportunism, security, and fraud. This paper outlines these issues and provides corresponding future research opportunities within five areas of DPS 2.0 (digital wallets, cryptocurrency, virtual currency, facial recognition, and mobile payments). CMOS-based computing systems that employ the von Neumann architecture are relatively limited when it comes to parallel data storage and processing. In contrast, the human brain is a living computational signal processing unit that operates with extreme parallelism and energy efficiency. Although numerous neuromorphic electronic devices have emerged in the last decade, most of them are rigid or contain materials that are toxic to biological systems. In this work, we report on biocompatible bilayer graphene-based artificial synaptic transistors (BLAST) capable of mimicking synaptic behavior. The BLAST devices leverage a dry ion-selective membrane, enabling long-term potentiation, with ~50 aJ/µm2 switching energy efficiency, at least an order of magnitude lower than previous reports on two-dimensional material-based artificial synapses. The devices show unique metaplasticity, a useful feature for generalizable deep neural networks, and we demonstrate that metaplastic BLASTs outperform ideal linear synapses in classic image classification tasks. With switching energy well below the 1 fJ energy estimated per biological synapse, the proposed devices are powerful candidates for bio-interfaced online learning, bridging the gap between artificial and biological neural networks. The freshwater opossum shrimp Mysis diluviana can undergo extensive diel vertical migration (DVM) to feed in shallow, prey rich strata at night. Bright moonlight limits their night-time migration presumably due to predator avoidance. Using a linked, foraging-bioenergetics model, we evaluated the cost of avoiding predators by simulating the effects of prey density, water temperature, and light intensity on daily feeding and growth of M. diluviana in Lake Pend Oreille, Idaho, USA. We found that when mysid distribution was not limited by moonlight intensity, simulated food consumption (10.3 J day−1) increased 1.6-fold compared to estimated consumption (6.1 J day−1) based on their observed, vertical distribution. Moreover, simulated growth of mysids (0.61 mg day−1) increased 74% compared to that estimated from observed distribution patterns (0.35 mg day−1), when they were located in deeper, darker strata. Given recent insights into partial DVM by M. diluviana, we note that proximate factors associated with predator avoidance in pelagic (light availability) and benthic (hunger level, body size and reproductive status) habitats may convey complimentary benefits to M. diluviana fitness by reducing predation mortality and increasing metabolic efficiency. Aims/hypothesis: We aimed to determine associations of regression to normal glucose tolerance (NGT), maintaining impaired glucose tolerance (IGT) or progression to diabetes with subsequent risks of CVD and microvascular disease among Chinese adults with IGT. Methods: We conducted an observational study among 540 participants in the Da Qing Diabetes Prevention Study, a 6 year lifestyle intervention trial in people with IGT, defined by 1985 WHO criteria as fasting plasma glucose <7.8 mmol/l and 2 h post-load plasma glucose ≥7.8 and <11.1 mmol/l. At the end of the trial, the groups that had regressed to NGT, remained with IGT or progressed to diabetes were identified. Participants were then followed for 24 years after completion of the trial, during which we compared the incidence and hazard ratios for CVD and microvascular disease in each group and estimated the differences in their median time to onset from parametric Weibull distribution models. Results: At the end of the 6 year trial, 252 (46.7%) participants had developed diabetes, 114 (21.1%) had remained with IGT and 174 (32.2%) had regressed to NGT. Compared with those who developed diabetes during the trial, the median time to onset of diabetes was delayed by 14.86 years (95% CI 12.49, 17.25) in the NGT and 9.87 years (95% CI 8.12, 11.68) in the IGT groups. After completion of the trial, among those with diabetes, IGT and NGT, the 24 year cumulative incidence of CVD was 64.5%, 48.5% and 45.1%, respectively, and 36.8%, 21.7% and 16.5% for microvascular diseases. Compared with participants who had progressed to diabetes during the trial, those who regressed to NGT had a 37% (HR 0.63; 95% CI 0.47, 0.85) reduction in CVD incidence and a median delay of 7.45 years (95% CI 1.91, 12.99) in onset, and those who remained with IGT had a 34% (HR 0.66; 95% CI 0.47, 0.91) lower CVD incidence with a median delay in onset of 5.69 years (95% CI 1.0, 10.38). Participants with NGT had a 66% (HR 0.34; 95% CI 0.20, 0.56) lower incidence of microvascular diseases and a median delay in the onset of 18.66 years (95% CI 6.08, 31.24), and those remaining with IGT had a 52% (HR 0.48; 95% CI 0.29, 0.81) lower incidence with a median delay of 12.56 years (95% CI 2.49, 22.63). Conclusions/interpretation: People with IGT who reverted to NGT or remained with IGT at the end of the 6 year trial subsequently had significantly lower incidences of CVD and microvascular disease than those who had developed diabetes. The Critically Endangered shorttail nurse shark Pseudoginglymostoma brevicaudatum (Günther, 1866) is a small-bodied Orectolobiform shark found only in the Western Indian Ocean (WIO). We report here on a significant range extension for P. brevicaudatum based on four records of the species in southern Mozambique, recorded during research surveys, including a specimen collected in 1967 and video surveys in 2019, and opportunistic assessments of shore-angler photographic catch records. These are the first records of P. brevicaudatum in Mozambique. The potential factors for this range extension and its implications for the management of this species are discussed. We also present morphological descriptions of the first recorded P. brevicaudatum specimen from Mozambique and of the dentition of this species (based on samples from Madagascar), and comment on the current status of knowledge and knowledge gaps for this species. Owing to its poor conservation status, limited inshore distribution and potential and confirmed fishery threats, improved knowledge and conservation should be considered priorities for P. brevicaudatum. In their article, 'That is not dead which can eternal lie: the aestivation hypothesis for resolving Fermi's paradox', Sandberg et al. try to explain the Fermi paradox (we see no aliens) by claiming that Landauer's principle implies that a civilization can in principle perform far more ( ∼ 10 30 times more) irreversible logical operations (e.g., error-correcting bit erasures) if it conserves its resources until the distant future when the cosmic background temperature is very low. So perhaps aliens are out there, but quietly waiting. Sandberg et al. implicitly assume, however, that computer-generated entropy can only be disposed of by transferring it to the cosmological background. In fact, while this assumption may apply in the distant future, our universe today contains vast reservoirs and other physical systems in non-maximal entropy states, and computer-generated entropy can be transferred to them at the adiabatic conversion rate of one bit of negentropy to erase one bit of error. This can be done at any time, and is not improved by waiting for a low cosmic background temperature. Thus aliens need not wait to be active. The global epidemic of type 2 diabetes has prompted numerous studies and public health efforts to reduce its development. A variety of interventions, including lifestyle modifications and pharmacological agents directed at ameliorating the major risk factors for type 2 diabetes, are of proven efficacy in reducing the development of type 2 diabetes in people with impaired glucose tolerance. While prevention of the hyperglycaemia characteristic of diabetes is arguably an important, clinically relevant outcome, a more compelling outcome with greater clinical significance is the prevention or reduction of the relatively diabetes-specific microvascular and less-specific cardiovascular disease (CVD) complications associated with diabetes. These complications cause the majority of morbidity and excess mortality associated with diabetes. Any reduction in diabetes should, logically, also reduce the occurrence of its long-term complications; however, most diabetes prevention trials have not been of sufficient duration to allow such an evaluation. The limited long-term data, largely from the Da Qing Diabetes Prevention Study (DQDPS) and the Diabetes Prevention Program (DPP) and their respective follow-up studies (DQDPOS and DPPOS), suggest a reduction in microvascular complications and amelioration of CVD risk factors. Only the DQDPOS and Study to Prevent Non-Insulin-Dependent Diabetes Mellitus (STOP-NIDDM) studies have shown a reduction in CVD events and only DQDPOS has demonstrated a decrease in CVD and overall mortality. While these limited data are promising, whether diabetes prevention directly reduces complication-related morbidity and mortality remains unclear. Longer follow-up of prevention studies is needed to supplement the limited current clinical trial data, to help differentiate the effects of diabetes prevention itself from the means used to reduce diabetes development and to understand the balance among benefits, risks and costs of prevention. Tourism generated through bonefish (Albula spp.) fishing contributes to the economies of many isolated tropical islands and atolls. However, little research has been conducted on bonefish in the Indian Ocean. This study aimed to contribute to the understanding of bonefish ecology in the Indian Ocean by quantifying the spatial and temporal movements of Albula glossodonta at a near-pristine and predator-rich atoll in the Seychelles; however, to achieve this, an analysis to identify the occurrence of possible post-release predation bias was first necessary. An acoustic telemetry study was initiated at the remote St. Joseph Atoll, within an array of 88 automated data-logging acoustic receivers. Thirty bonefish were surgically implanted with Vemco V13 acoustic transmitters and tracked for one year. Only 10% of the tagged bonefish were detected for longer than two weeks. A comparison of the final 100 h of movement data from fish detected for less than two weeks to the movement data of the fish detected for longer periods revealed distinct differences in area use and significant differences in the average daily distance moved, speed of movement and frequency of detections. This suggested that mortality in the form of post-release predation was at least 43% of tagged fish. The three surviving bonefish were tracked for 210 to 367 days. These individuals remained in the atoll and showed high use of the marginal habitats between the shallow sand flats and the lagoon. A generalised linear mixed model identified that water temperature, diel cycle and tide were significant predictors of bonefish presence in the lagoon. The high post-release mortality highlights that catch-and-release is likely not as benign as previously believed and management and policy should be adjusted accordingly. [ABSTRACT FROM AUTHOR]

Decreasing populations of common estuary-associated marine fishes are being documented globally and red flags associated with such declines are often ignored due to the high relative abundance of these species when compared to more rare and threatened taxa. The Cape stumpnose Rhabdosargus holubi (Steindachner 1881) is an abundant and widespread southern African sparid that is dependent on estuaries as a primary nursery area. Historical and current information on the species is comprehensive and the accumulated evidence strongly suggests that this ubiquitous fish is under pressure, mainly due to degradation of estuaries and associated catchments, increasing fishing pressures from recreational and subsistence anglers, habitat loss relating to reduced submerged macrophyte areas in many systems, industrial and agricultural pollution, infrastructural developments in and around estuaries, and climate change. In particular, the temporary loss of Lake St Lucia, which accounts for approximately 50% of the estuarine surface area in South Africa, has drastically reduced the nursery area availability for R. holubi on the subcontinent. Overall we present strong evidence to support the hypothesis that present-day stocks of R. holubi are much smaller than those in the pristine state and that urgent management measures need to be considered and implemented to prevent current depleted populations of the species from declining further. Background: The trend towards laparoscopic surgery seen in other specialties has not occurred at the same pace in oesophagectomy. This stems from concerns regarding compromised oncological clearance, and complications associated with gastric tube necrosis and anastomotic failure. We present our experience of minimally invasive oesophagectomy (MIO) compared to open and hybrid surgery. We aim to ascertain non-inferiority of MIO by evaluating impact on survival, oncological clearance by resection margin and lymph node harvest and post-operative complications.Methods: Data were sourced retrospectively 2008-2015. Three approaches were studied. MIO (3-stage Mckeown), hybrid (2-stage Ivor Lewis, laparoscopy, thoracotomy) and open (2-stage Ivor Lewis).Results: Five-year survival was 54.2%. Surgical approach had no significant impact on survival at any stage of disease (Stage 0/I p = 0.98; stage II p = 0.2; stage III p = 0.76). There was no statistically significant difference in oncological clearance by resection margins between procedures when compared by disease stage (p = 0.49). A higher number of nodes were harvested in hybrid [median 27.5 (6-65)] and open surgeries [median 26 (4-54)] than in MIO [median 20 (7-44)] (p > 0.01). Numbers of nodes resected did not impact risk of recurrence [recurrence, median 25 (6-54), no recurrence, 26 (4-65)] (p = 0.25). Anastomotic strictures (22.4%) and potential leaks (17.9%) were more common in MIO (strictures p > 0.01, leaks p = 0.08), although associated morbidity was lower. Respiratory complications were less common in MIO (2.9%) versus hybrid (13.3%) (p = 0.02). Wound infection and chyle leak were also lower (wound 1.5% MIO 3.5% open, p = 0.6; chyle leak 1.5% MIO, 6.7% hybrid, p = 0.2).Conclusions: Our results show no negative impact of MIO on survival or oncological clearance. Respiratory and wound complications are lower in MIO, but rates of anastomotic strictures and potential anastomotic leaks are increased. The phenomenon of negative remanent magnetization (NRM) has been observed experimentally in a number of heterogeneous magnetic systems and has been considered anomalous. The existence of NRM in homogenous magnetic materials is still in debate, mainly due to the lack of compelling support from experimental data and a convincing theoretical explanation for its thermodynamic validation. Here we resolve the long-existing controversy by presenting experimental evidence and physical justification that NRM is real in a prototype homogeneous ferromagnetic nanoparticle, an europium sulfide nanoparticle. We provide novel insights into major and minor hysteresis behavior that illuminate the true nature of the observed inverted hysteresis and validate its thermodynamic permissibility and, for the first time, present counterintuitive magnetic aftereffect behavior that is consistent with the mechanism of magnetization reversal, possessing unique capability to identify NRM. Traditionally, use of household and personal care products has been collected through questionnaires, which is very time consuming, a burden on participants, and prone to recall bias. As part of the SUPERB Project (Study of Use of Products and Exposure-Related Behaviors), a novel platform was developed using bar codes to quickly and reliably determine what household and personal care products people have in their homes and determine the amount used over a 1-week period. We evaluated the acceptability and feasibility of our methodology in a longitudinal field study that included 47 California households, 30 with young children and 17 with an older adult. Acceptability was defined by refusal rates; feasibility was evaluated in terms of readable bar codes, useful product information in our database for all readable barcodes, and ability to find containers at both the start and end of the week. We found 63% of personal care products and 87% of the household care products had readable barcodes with 47% and 41% having sufficient data for product identification, respectively and secondly, the amount used could be determined most of the time. We present distributions for amount used by product category and compare inter- and intra-person variability. In summary, our method appears to be appropriate, acceptable, and useful for gathering information related to potential exposures stemming from the use of personal and household care products. Time-activity data are traditionally collected by telephone interviews or through paper diaries, which are time consuming and costly. As a potential alternative that may greatly save staff time, a web survey to collect time-activity data was developed and tested in this study. We collected 24-h recall web diaries from 151 parents of young children mostly under 55 years of age (who also answered for their children) and 55 older adults (≥55 years of age) both on a weekday and a weekend day every 3 months during an 18-month period. The performance and reliability of the web surveys collected were evaluated, including the survey-completion rate, and the percentage of surveys with unreasonable time being reported as spent sleeping and with missing reports of being in transit between locations. We also compared the web-survey data with time-activity information we collected from the same subjects in telephone interviews and found that these data sources were fairly consistent with each other. However, we observed slightly more compliance issues for the web than the telephone survey, but most of these issues could be addressed and minimized by refining some questions or the survey interface. Our study suggests that it is critical to reduce participants' burden and improve survey interface design for optimal compliance and data quality. Residential insecticide usage and actual application details were collected in a population-based sample of 477 households residing within 22 counties in northern California with at least one child of age 5 years between January 2006 and August 2008. Structured telephone interviews were conducted collecting information on residential use of insecticides, including outdoor sprays, indoor sprays, indoor foggers, applications by professionals, and pet flea/tick control during the previous year. Interviews also covered post-treatment behaviors, which influence post-application exposure levels. Altogether, 80% of the households applied some type of insecticide in the previous year, with half of this population using two or more application methods. Of the households using insecticides, half reported applying insecticides relatively infrequently (<4 times per year), whereas 11-13% reported high frequency of use (>24 times per year). Application frequency was temperature dependent, with significantly more applications during the warmer months from May through October. Spot treatments appeared to be the most prevalent application pattern for sprays. For one out of three of the indoor applications, children played in the treated rooms on the day of the application, and for 40% of the outdoor applications, pets played in the treated area on the day of the application. These findings describing the intensity of insecticide use and accompanying behaviors in families with young children may inform future insecticide exposure modeling efforts, and ultimately, risk assessments. Friedrich Jeckeln as Higher SS and Police Fuehrer for South Russia in 1941, and then North Russia from 1941 to 1944, was a key player in the development of the Holocast in the occupied Soviet Union. This article explores his career and motivations through the medium of his interrogation reports. Those interrogations were conducted by the NKVD from December 1945 to January 1946. They formed a key part in the development of a case against Jeckeln for involvement in crimes against the Soviet people. Using a variety of coercive techniques Jeckeln's captors forced him to confront the enormity of his crimes. He was brought before a court and executed on the same day in February 1946. Many people spend time in stores and restaurants, yet there has been little investigation of the influence of these microenvironments on personal exposure. Relative to the outdoors, transportation, and the home, these microenvironments have high concentrations of several volatile organic compounds (VOCs). We developed a stochastic model to examine the effect of VOC concentrations in these microenvironments on total personal exposure for (1) non-smoking adults working in offices who spend time in stores and restaurants or bars and (2) non-smoking adults who work in these establishments. We also compared the effect of working in a smoking versus non-smoking restaurant or bar. Input concentrations for each microenvironment were developed from the literature whereas time activity inputs were taken from the National Human Activity Patterns Survey. Time-averaged exposures were simulated for 5000 individuals over a weeklong period for each analysis. Mean contributions to personal exposure from non-working time spent in stores and restaurants or bars range from <5% to 20%, depending on the VOC and time-activity patterns. At the 95th percentile of the distribution of the proportion of personal exposure attributable to time spent in stores and restaurants or bars, these microenvironments can be responsible for over half of a person's total exposure to certain VOCs. People working in restaurants or bars where smoking is allowed had the highest fraction of exposure attributable to their workplace. At the median, people who worked in stores or restaurants tended to have 20–60% of their total exposures from time spent at work. These results indicate that stores and restaurants can be large contributors to personal exposure to VOCs for both workers in those establishments and for a subset of people who visit these places, and that incorporation of these non-residential microenvironments can improve models of personal exposure distributions. Accurate modeling of exposure to volatile organic compounds (VOCs) over a large study population depends on proper characterization of concentrations in the indoor residential environment. However, owing to the high expense of field sampling campaigns for determining indoor air concentrations, such studies have only been conducted for limited populations. Therefore, there is a need to determine the degree to which results can be extrapolated to unstudied settings through the use of models, the most appropriate information required to do so and the potential errors associated with the use of sub-optimal information. The goal of this analysis is to evaluate three different source indicators used to predict indoor VOC concentration distributions for a new study population. Data from two field studies are used. For each data set, source strength, indoor–outdoor (I–O) difference and indoor/outdoor (I/O) ratio, collectively referred to as source indicators, are calculated and fit with distributions. These distributions, as well as distributions for air exchange, volume and outdoor concentrations for the new study population, are used for predicting indoor concentrations using Monte Carlo simulations, which are then compared with actual distributions. As expected, the source strength often provides the most effective predictions (11 out of 20 instances), but is slightly outperformed by, although is still comparable with, the I–O difference on some occasions (4 out of 20). The I/O ratio generally has the greatest prediction errors, given its dependence on outdoor concentrations, but performs optimally in a limited number of cases (5 out of 20). When deciding between the source strength and I–O difference, one must consider the availability and fidelity of both current and future data. On the basis of our findings, exposure-monitoring studies should report the distribution statistics for I–O differences and, if the data are available, for source strengths. [ABSTRACT FROM AUTHOR]

Background: The dispersion-aggregation behaviors of suspended colloids in rivers and estuaries are affected by the compositions of suspended materials (i.e., clay minerals vs. organic macromolecules) and salinity. Laboratory experiments were conducted to investigate the dispersion and aggregation mechanisms of suspended particles under simulated river and estuarine conditions. The average hydrodynamic diameters of suspended particles (representing degree of aggregation) and zeta potential (representing the electrokinetic properties of suspended colloids and aggregates) were determined for systems containing suspended montmorillonite, humic acid, and/or chitin at the circumneutral pH over a range of salinity (0 - 7.2 psu). Results: The montmorillonite-only system increased the degree of aggregation with salinity increase, as would be expected for suspended colloids whose dispersion-aggregation behavior is largely controlled by the surface electrostatic properties and van der Waals forces. When montmorillonite is combined with humic acid or chitin, the aggregation of montmorillonite was effectively inhibited. The surface interaction energy model calculations reveal that the steric repulsion, rather than the increase in electronegativity, is the primary cause for the inhibition of aggregation by the addition of humic acid or chitin. Conclusion: These results help explain the range of dispersion-aggregation behaviors observed in natural river and estuarine systems. It is postulated that the composition of suspended particles, specifically the availability of steric polymers such as those contained in humic acid, determine whether the river suspension is rapidly aggregated and settled or remains dispersed in suspension when it encounters increasingly saline environments of estuaries and oceans. Exercise has many health benefits, including antidepressant actions in depressed human subjects, but the mechanisms underlying these effects have not been elucidated. We used a custom microarray to identify a previously undescribed profile of exercise-regulated genes in the mouse hippocampus, a brain region implicated in mood and antidepressant response. Pathway analysis of the regulated genes shows that exercise upregulates a neurotrophic factor signaling cascade that has been implicated in the actions of antidepressants. One of the most highly regulated target genes of exercise and of the growth factor pathway is the gene encoding the VGF nerve growth factor, a peptide precursor previously shown to influence synaptic plasticity and metabolism. We show that administration of a synthetic VGF-derived peptide produces a robust antidepressant response in mice and, conversely, that mutation of VGF in mice produces the opposite effects. The results suggest a new role for VGF and identify VGF signaling as a potential therapeutic target for antidepressant drug development. [ABSTRACT FROM AUTHOR]

Aim: To review management, outcome and the lessons learnt from a laparoscopic approach to GISTs.Method: All cases of GIST presenting to the upper GI MDT between 2000 and 2006 were reviewed. Presentation, preoperative investigations, management and follow-up were recorded. Surgical resection using a laparoscopic approach, where feasible was the preferred management.Results: 25 consecutive patients that included one oesophageal, three oesophago-gastric, 19 gastric and two smallbowel GISTs were treated between 2000 and 2006. There were 11 male and 14 females with a median age of 68 (25-90) years. Clinical presentation was: gastrointestinal bleed 15, pain 6, dysphagia 2, anaemia 3, weight loss 1, and asymptomatic 2. Out of 25, four were inoperable and treated with imatinib. 17 laparoscopic (including 2 conversions) and four open procedures were performed. Two (both GISTs close to the oesophago-gastric junction) required reoperation due to surgical-related morbidity. Of the 25, five were high-, 11 intermediate- and nine low-risk GISTs. Objectives: On graduation, UK dentists wishing to advance their career enter two years of general professional training aimed at consolidating their undergraduate experience. The Membership of the Faculty of Dental Surgery examination (MFDS) attests to its successful completion and is a pre-requisite for entry into training programmes which lead to specialist status. Most MFDS candidates prepare for the examination on their own while in full-time employment and many reinforce this self-directed learning with participation in short revision courses or through distance learning. Here we seek to obtain data on the specific educational needs of these individuals.Methods: Questionnaires were used to interrogate 92 UK graduates attending short MFDS revision courses of up to 1 week's duration to identify which topic areas were perceived as particular areas of weakness. To gain greater insight into the responses obtained, 18/92 respondents were selected at random and followed up with semi-structured interviews informed by the questionnaires.Results: Basic medical science, human diseases, law and ethics and health and safety regulations were the areas of weakness most frequently highlighted by the respondents. Most had undergone comprehensive courses in the first two topics; however, the interviews suggested that this was generally in the early stages of undergraduate training when they had difficulty in contextualising large quantities of new information. In the case of the latter two, teaching had been very varied and several interviewees felt that it had been inadequate.Conclusion: Recent graduates preparing for MFDS have clear educational needs. Goal, Scope and Background. In the first part of this paper, we developed a methodology to incorporate exposure and risk concepts into life cycle impact assessment (LCIA). We argued that both risk assessment and LCIA are needed to consider the impacts of increasing insulation for single-family homes in the US from current practice to the levels recommended by the 2000 International Energy Conservation Codes. In this analysis, we apply our model to the insulation case study and evaluate the benefits and costs of increased insulation for new housing. Results and Discussion. The central estimate of impacts from the complete insulation manufacturing supply chain is approximately 20 premature deaths, 500 asthma attacks, and 8,000 restricted activity days nationwide for one year of increased fiberglass output. Of the health impacts associated with increased insulation manufacturing, 83% are attributable to the supply chain emissions from the mineral wool industry, which is mostly associated with the direct primary PM2.5 emissions from the industry (98%). Reduced energy consumption leads to 1.2 premature deaths, 30 asthma attacks, and 600 restricted activity days avoided per year, indicating a public health 'payback period' on the order of 14 years. About 90% of these benefits were associated with direct emissions from power plants and residential combustion sources. In total, the net present value of economic benefits over a 50-year period for a single-year cohort of new homes is $100 million with a 5% discount rate, with 40 fewer premature deaths in this period. Conclusion, Recommendation and Outlook. We have developed and applied a risk-based model to quantify the public health costs and benefits of increased insulation in new single-family homes in the US, demonstrating positive net economic and public health benefits within the lifetimes of the homes. More broadly, we demonstrated that it is feasible to incorporate exposure and risk concepts into I-O LCA, relying on regression-based intake fractions followed by more refined dispersion modeling. The refinement step is recommended especially if primary PM2.5 is an important source of exposure and if stack heights are relatively low. Where secondary PM2.5 is more important, use of regression-based intake fractions would be sufficient for a reasonable risk approximation. Uncertainties in our risk-based model should be carefully considered; nevertheless, our study can help decision-makers evaluate the costs and benefits of demand-side management policy options from a combined public health and life cycle perspective. Widespread application of contemporary-use insecticides results in low-level exposure for a majority of the population through a variety of pathways. Urinary insecticide biomarkers account for all exposure pathways, but failure to account for temporal within-subject variability of urinary levels can lead to exposure misclassification. To examine temporal variability in urinary markers of contemporary-use insecticides, nine repeated urine samples were collected over 3 months from 10 men participating in an ongoing study of male reproductive health. These 90 samples were analyzed for urinary metabolites of chlorpyrifos (3,5,6-trichloro-2-pyridinol (TCPY)) and carbaryl (1-naphthol (1N)). Volume- based (unadjusted), as well as creatinine (CRE)- and specific gravity (SG)-adjusted concentrations were measured. TCPY had low reliability with an intraclass correlation coefficient between 0.15 and 0.21, while 1N was moderately reliable with an intraclass correlation coefficient between 0.55 and 0.61. When the 10 men were divided into tertiles based on 3-month geometric mean TCPY and 1N levels, a single urine sample performed adequately in classifying a subject into the highest or lowest exposure tertiles. Sensitivity and specificity ranged from 0.44 to 0.84 for TCPY and from 0.56 to 0.89 for 1N. Some differences in the results between unadjusted metabolite concentrations and concentrations adjusted for CRE and SG were observed. Questionnaires were used to assess diet in the 24?h preceding the collection of each urine sample. In mixed-effects models, TCPY was significantly associated with season as well as with consuming grapes and cheese, while 1N levels were associated with consuming strawberries. Goal, Scope and Background. Incorporation of exposure and risk concepts into life cycle impact assessment (LCIA) is often impaired by the number of sources and the complexity of site-specific impact assessment, especially when input-output (I-O) analysis is used to evaluate upstream processes. This makes it difficult to interpret LCIA outputs, especially in policy contexts. In this study, we develop an LCIA tool which takes into account the geographical variability in both emissions and exposure, and which can be applied to all economic sectors in I-O analysis. Our method relies on screening-level risk calculations and methods to estimate population exposure per unit of emissions from specific geographic locations. Methods. We propose a simplified impact assessment approach using the concept of intake fraction, which is the fraction of a pollutant or its precursor emitted that is eventually inhaled or ingested by the population. Instead of running a complex site-specific exposure analysis, intake fractions allow for the accounting of the regional variability in exposure due to meteorological factors and population density without much computational burden. We calculate sector-specific intake fractions using previously-derived regression models and apply these values to the supply chain emissions to screen for the sectors whose emissions largely contribute to the total exposures. Thus, the analytical steps are simplified by relying on these screening-level risk calculations. We estimate population exposure per unit emissions from specific geographic locations only for the facilities and pollutants that pass an initial screening analysis. We test our analytical approach with reference to the case of increasing insulation for new single-family homes in the US. We quantify the public health costs from increasing insulation manufacturing and compare them with the benefits from energy savings, focusing on mortality and morbidity associated with exposure to primary and secondary fine particles (PM25) as well as cancer risk associated with exposure to toxic air pollutants. We estimate health impacts using concentration-response functions from the published literature and compare the costs and benefits of the program by assigning monetary values to the health risks. In the second part of this paper, we present the results of our case study and consider the implications for incorporating exposure and risk concepts into I-OLCA. Conclusions. We have presented a methodology to incorporate regional variability in emissions and exposure into input-output LCA, using reduced-form information about the relationship between emissions and population exposure, along with standard input-output analysis and risk assessment methods. Vehicle emissions have been associated with adverse health effects in multiple epidemiological studies, but the sources or constituents responsible have not been established. Characterization of vehicle-related exposures requires detailed information on spatial and temporal trends of various pollutants and the ability to predict exposures in unmonitored settings. To address these issues, in the summer of 2001 we measured continuously particle-bound polycyclic aromatic hydrocarbons (PAHs), ultrafine particles, and PM2.5 at a number of sites in Roxbury, a neighborhood of Boston, Massachusetts with significant diesel and gasoline-fueled traffic. We took measurements at the side of the road and at varying distances from the road, with simultaneous collection of traffic counts and meteorological conditions. Across all nine sites, median roadside concentrations were 8?ng/m3 of particle-bound PAHs (range: 4-57), 16,000 ultrafine particles/cm3 (range: 11,000-53,000), and 54?µg/m3 of PM2.5 as measured with a DustTrak (range: 12-86). Concentrations of all pollutants were lower at greater distances from the road, upwind, and at higher wind speeds, with greater concentration gradients for PAHs and ultrafine particles. In linear mixed effects regression models accounting for temporal autocorrelation, large diesel vehicle counts were significantly associated with roadside concentrations of PAHs (P=0.02), with a moderate association with ultrafine particles and little relation with PM2.5. Aim/hypothesis: We aimed to determine variations in the prevalence of increased urinary albumin excretion, associated risk factors and complications in patients with diabetes participating in the WHO Multinational Study of Vascular Disease in Diabetes follow-up. Methods: Urinary albumin to urinary creatinine ratios were measured centrally in 2033 of the 2550 (79.7 %) re-examined patients from eight centres in seven countries and the frequency of microalbuminuria and macroalbuminuria and their associations with risk factors and complications were examined. Results: Macroalbuminuria prevalence (overall 15.6 %) varied tenfold (3–37 %) among centres, was higher in American Indian and Asian centres and not clearly related to type of diabetes. Microalbuminuria (overall 19.7 %) varied less (12–31 %). Increased albumin excretion was related overall to baseline fasting plasma glucose in the pooled group in whom it was measured and to increased arterial pressure, insulin use, coronary heart disease, lower extremity amputation, retinopathy and stroke in most centres. Conclusion/interpretation: Centres varied widely in the prevalence of increased albumin excretion but associations with risk factors and vascular complications were generally similar in most centres and in both major types of diabetes with ethnic and genetic differences probably contributing. Aims/hypothesis: We aimed to estimate incidences of any retinopathy and proliferative diabetic retinopathy (PDR) by direct ophthalmoscopy and relate them to baseline risk factors in re-examined diabetic survivors from 10 centres of the WHO Multinational Study of Vascular Disease in Diabetes. Methods: After a mean follow-up of 8.4 years (11.7 years in Oklahoma), 2877 (71.6 %) survivors were resubmitted to standardised direct ophthalmoscopy as at baseline. The presence of any retinopathy and PDR were recorded at each centre and their incidence estimated in those without retinopathy and PDR at baseline. The independent associations of these incidences with baseline risk factors are expressed as odds ratios derived from multiple logistic regression analyses, within individual centres (which included fasting plasma glucose in 8 and triglyceride in 5) and in pooled data. Results: Of the 4662 original patients, 465 (10.4 %) of those without and 77 (43.0 %) of those with baseline PDR had died (p < 0.001). Any retinopathy was newly reported at follow-up in 47.7 % and PDR in 9.7 % of those free of them at baseline, with reported incidences varying substantially among centres. Incident retinopathy appeared earlier in the known course of diabetes but incidence rates rose more slowly with duration in patients with Type II (non-insulin-dependent) diabetes mellitus than in those with Type I (insulin-dependent) diabetes mellitus. In pooled data and in some individual centres, any retinopathy incidence gave significantly positive odds ratios with age, diabetes duration, systolic pressure, plasma cholesterol, BMI, insulin treatment and proteinuria, and with fasting plasma glucose in the centres where it was measured. Positive odds ratios for PDR were similarly obtained for age, duration, insulin treatment, cholesterol, proteinuria and fasting glycaemia. Smoking status odds ratios were negative for both outcomes. Conclusion/interpretation: Incidence of ophthalmoscopically ascertained any retinopathy varied about twofold and of PDR about threefold among centres. Although, in part attributable to differences between observers, variation in incidence in all centres and in some cases within centres was associated with a number of baseline risk factors. Such associations are not likely due to observer variation or selection biases and emerged despite the imprecision of clinical ophthalmoscopy. Aims/hypothesis: This study compared the incidence of vascular disease in subjects with younger-onset diabetes from different ethnic groups. Methods: The incidence of vascular disease end-points has been studied in a sub-group (n = 994) of participants of the World Health Organization Multinational Study of Vascular Disease in Diabetes (WHO MSVDD) who had younger-onset diabetes (diagnosed before the age of 30 years). The study participants have been divided into European (n = 631), Asian (n = 84) and American Indian (n = 91) cohorts. Results: For Type I (insulin-dependent) and Type II (non-insulin-dependent) diabetes mellitus, American Indian men had a higher incidence of lower-extremity amputation and renal failure than the other cohorts, whereas European women had a higher incidence of angina than other cohorts. American Indians also had a higher incidence of any retinopathy, clinical proteinuria and albuminuria than the European and Asian cohorts. Conclusion/interpretation: This study confirms the high burden of large and small-vessel disease complications manifest in American Indian people with younger-onset diabetes. Aims/hypothesis: Rates of vascular complications of diabetes in a cohort of mainland Chinese patients with diabetes, ascertained and examined by similar methodology, are compared with those of the WHO Multinational Study of Vascular Disease in Diabetes (WHO MSVDD). Methods: The standardised procedures carried out in the WHO MSVDD were followed in assembling and examining a Chinese cohort of 447 diabetic patients recruited in Beijing and Tianjin [2]. Results: Compared with the WHO MSVDD centres, the Chinese cohort was slightly older, had a shorter duration of known diabetes and had fewer insulin-treated patients. Arterial pressure, total blood cholesterol and body mass index were substantially lower. Large vessel disease rate for age, sex and duration adjusted data (17.9 %) was about half that of the combined WHO MSVDD centres (33.5 % p < 0.001). However, retinopathy (47.4 % vs 35.8 % p < 0.001) and proteinuria (57.1 vs 24.9 % p < 0.001) rates were significantly higher. Conclusion/interpretation: Relatively low arterial pressures and blood cholesterol are likely contributors to the notably low arterial disease rates in this Chinese diabetic cohort; they reflect low rates in the Chinese mainland general population and resemble the Tokyo and Hong Kong centres of the WHO MSVDD. The high rates of retinopathy and proteinuria could relate to later diagnosis, degree of hyperglycaemia and/or increased susceptibiltiy to microangiopathy. Aims: The incidence of retinal, renal and cardiovascular complications and their relation to baseline risk factors was documented in this follow-up study of 10 of the 14 original centres of the WHO Multinational Study of Vascular Disease in Diabetes (WHO MSVDD). Methods: The incidence of specified items of vascular disease and some associated risk factors was ascertained after 7 to 9 years (11–12 years in Oklahoma, USA) follow-up, re-using baseline examination methodology in 3165 patients (66.9 %) and, through secondary information in 717 (15.2 %) of the 4729 original patients, of whom 540 (11.4 %) had died and 307 (6.5 %) were untraceable. Results: During follow-up, approximately one third of the patients developed hypertension and one third started insulin. Coronary heart disease incidence varied 10 to 20-fold among centres as did limb amputation rates. Inter-centre differences in incident retinopathy and severe visual impairment were smaller but incident clinical proteinuria and renal failure varied markedly. Vascular disease incidence of all categories was high in Native Americans though coronary heart disease incidence was relatively low in Pima Indians and absolutely low in Hong Kong and Tokyo patients. Specific vascular events and their relation with baseline risk factors are analysed in accompanying papers, summarised in the Epilogue. Conclusion/interpretation: These 10 centres reported very different incidence rates of vascular complications. Observer variation, selection biases and competing causes of mortality contributed to these differences but their validity is supported by the more objective outcome indicators. The following papers also suggest that baseline factors such as raised arterial pressure, cholesterol and fasting glucose (in the centres where it was measured) were important and potentially reversible predictors of risk. Aims/hypothesis: We aimed to examine risk factors for, and differences in, renal failure in diabetic patients from 10 centres. Methods: Risk factors for renal failure were examined in 3558 diabetic patients who did not have renal disease at baseline in the WHO Multinational Study of Vascular Disease in Diabetes (WHO MSVDD). Results: In 959 subjects with Type I (insulin-dependent) diabetes mellitus and 2559 with Type II (non-insulin-dependent) diabetes mellitus, the average follow-up was 8.4 years ( ± 2.7). By the end of the follow-up period 53 patients in the Type I diabetic group and 134 patients in the Type II diabetic group had developed renal failure (incidence rate 6.3:1000 person years). Increasing age and duration of diabetes were associated with renal failure in Type II and Type I diabetes. In Type II diabetes duration of diabetes was a more important risk factor than age. In both Type I and Type II diabetic retinopathy and proteinuria were strongly associated with renal failure. Systolic blood pressure was associated with renal failure in Type I but not in Type II diabetic patients. ECG abnormalities at baseline, self-reported smoking and cholesterol were not associated with renal failure. Triglycerides were measured in a subset of centres. Among those with Type II, but not Type I diabetes, triglycerides were associated with renal failure independently of systolic blood pressure, proteinuria or retinopathy. In Type II diabetes fasting plasma glucose was associated with renal failure independently of other risk factors. Conclusion/interpretation: We have confirmed the role of proteinuria and retinopathy as markers of renal failure and the importance of hyperglycaemia in renal failure in Type I and Type II diabetes. Aims/hypothesis: Incidence of severe visual impairment and the ultimate prevalence of all grades of impairment were estimated in the 10 centres of the WHO Multinational Study of Vascular Disease in Diabetes (WHO MSVDD) participating in the follow-up. Methods: Visual function was ascertained at follow-up in 2994 (77.9 %) of the 3845 eligible participating survivors of the 4709 originally recruited for the WHO MSVDD using the same baseline enquiry method. The associations between incident severe visual impairment, follow-up prevalence of all grades of impairment and baseline risk factors were examined by univariate and stepwise multiple logistic regression analysis. Results: Overall, 8.4 year incidence of severe visual impairment was 1.94 % and showed statistically significant univariate correlations with age at diagnosis, diabetes duration, systolic blood pressure, fasting blood glucose and cholesterol, insulin treatment and strongly with baseline retinopathy. Baseline retinopathy, systolic pressure and cholesterol were statistically significant in multivariable analysis. Differences between centres (0.3 % to 3.45 %) were not significant. Ultimate prevalence of all grades of impairment differed between centres and within almost all of them was correlated in multivariable analysis with baseline retinopathy and proteinuria. Conclusion/interpretation: Comparisons of incident severe visual impairment between centres are restricted by selective mortality, low incidence rates and relatively small numbers in each centre but before retinopathy, baseline systolic pressure and cholesterol predicted severe visual impairment. Follow-up prevalence of all degrees of impairment varied among centres and were associated with prior retinopathy and renal disease at baseline. Aims/hypothesis. The predictive value of glomerular structure on progression of renal disease was examined in patients with Type II (non-insulin-dependent) diabetes and microalbuminuria (urinary albumin-to-creatinine ratio = 30–299 mg/g). Methods. Kidney biopsy specimens were obtained from 16 diabetic Pima Indians (6 men, 10 women). Progression of renal disease was assessed by measuring urinary albumin excretion 4 years after the biopsy (UAE4 years) and by computing the change in urinary albumin excretion during the study (Δ UAE). Results. At baseline, the duration of diabetes averaged 13.3 years (range = 4.0–23.8 years) and the mean glomerular filtration rate was 159 ml · min–1· 1.73m–2 (range = 98 – 239 ml · min–1· 1.73m–2). Median urinary albumin excretion was 67 mg/g (range = 25–136 mg/g) and it increased to 625 mg/g (range = 9–13471 mg/g) after 4 years; 10 subjects (63 %; 4 men, 6 women) developed macroalbuminuria (urinary albumin-to-creatinine ratio ≥ 300 mg/g). Neither mean arterial pressure nor HbA1 c changed substantially during follow-up. Among the glomerular morphologic characteristics, the number of visceral epithelial cells, or podocytes, per glomerulus was the strongest predictor of renal disease progression (UAE4 years, r = –0.49, p = 0.05; ΔUAE, r = –0.57, p = 0.02), with fewer cells predicting more rapid progression. Glomerular basement membrane thickness did not predict progression (UAE4 years, r = 0.11, p = 0.67; ΔUAE, r = 0.09, p = 0.73) and mesangial volume fraction had only a modest effect (UAE4 years, r = 0.42, p = 0.11; ΔUAE, r = 0.48, p = 0.06). Whether lower epithelial cell number per glomerulus among those that progressed was due to cellular destruction, a reduced complement of epithelial cells, or both is uncertain. Glomerular filtration rate (iothalamate clearance) was measured serially for 48 months in 26 Pima Indians with impaired glucose tolerance and 27 with normal glucose tolerance. At baseline, the mean glomerular filtration rate (SEM) was 133 ± 8 ml/min in subjects with impaired glucose tolerance and 123 ± 5 ml/min in those with normal glucose tolerance ( p = 0.12). In the 12 subjects with impaired glucose tolerance who progressed to Type II (non-insulin-dependent) diabetes during follow-up, mean glomerular filtration rate increased by 30 % ( p = 0.011). Among the remaining 14 subjects with impaired glucose tolerance, 12 reverted to normoglycaemia. The glomerular filtration rate both at baseline and after 48 months in this subgroup exceeded the values of subjects with normal glucose tolerance by 20 % ( p = 0.008) and 14 % ( p = 0.013), respectively. A pronounced rise in the glomerular filtration rate occurs at the onset of Type II diabetes but a trend to hyperfiltration is also present in those with impaired glucose tolerance. The present study examined the effect of ethanol-related cues in the natural environment of the social drinker (e.g., a bar setting) upon performance on a behavioral task. Fourteen male social drinkers participated in the study. Half were designated low drinkers (3 or fewer drinks per week and little or no experience in bars) and half of the subjects comprised a moderate drinking group (8 to 15 drinks per week and drinking in bars at least 4 times per month). Subjects played a computer-controlled video game prior to and following consumption of a drink containing ethanol and orange juice. Each subject participated for one session in two different environmental settings, a standard experimental room and a room decorated to resemble a bar situation. The results showed significantly more detrimental effect of the ethanol upon performance in the standard experimental room than in the bar setting, that is, more tolerance was exhibited in the bar setting.
