Gundelia tournefortii L. (Asteraceae) is an artichoke-like wild edible vegetable that grows in the semi-arid climate of the East Mediterranean. Due to its high cultural and economic values for culinary and therapeutic uses, this plant is exposed to overharvesting driven by household consumption and trade, threatening the survival of natural populations. Some limited data on the nutrient composition of G. tournefortii exists indicating presence of folic acid and several essential amino acids. Research on seed germination reports that mechanical scarification, gibberellic acid, and cold stratification are all effective treatments for seed dormancy breaking and therefore to propagate plants from seed. Successful vegetative propagation from the plant meristems is also available. However, despite some exceptions, the species is still not widely cultivated due to its thorny habit and complex seed germination requirements, and the ability to ensure seed germination under natural field conditions remains to be addressed. In higher education, the mobility of students across a country to take a degree is receiving increasing attention in recent years, mainly because of the effects on the economic development and competitiveness of the territorial areas where universities are located. In Italy, an important source of information is represented by the National Student Registry. Among others, the Registry allows us to analyze the student mobility across the country both at the moment of the first enrollment in a bachelor or 5-years degree program and, then, at the enrollment in a master degree program. In the former case, students may attend university in the area of residence or move somewhere else (first-level mobility); in the latter case bachelor graduates may prosecute the academic career with a master degree in the same university or move in another higher academic institution (second-level mobility). In this contribution we propose a set of instruments to analyze student mobility. These instruments will be applied to the information gathered by the National Student Registry. We first define two indicators of the capability of retention and attraction of students computed at the first- and the second-level mobility; we also illustrate how these indices may be synthesize through a composite indicator and how they can be used to produce a graphical display of the ego-centric network generated by the flow of incoming and outgoing movements. Then, we formulate a gravity model to detect how the mobility is affected by university characteristics, such as quality of structures and quality of life of the territorial area of influence of the university. Helicobacter pylori affects gastric secretion. This functional effect might have a morphometric counterpart. Therefore, the gastric cell secretory compartment was morphometrically assessed in different pathophysiologic conditions related to Helicobacter pylori infection. Nineteen Helicobacter pylori-positive nonduodenal ulcer subjects, 15 omeprazole chronically treated subjects, and 19 duodenal ulcer patients were studied against 19 controls. Somatostatin, gastrin, enterochromaffin-like, and parietal cell density was assessed in gastric biopsies. No differences in any cell type density were found between Helicobacter pylori-positive nonduodenal ulcer subjects and controls. On the contrary, differences were significant when comparing omeprazole and duodenal ulcer patients to controls (higher density of gastrin, enterochromaffin-like, and parietal cells, lower density of somatostatin cells). In duodenal ulcer a reversion to control values followed Helicobacter pylori eradication and ulcer healing. A direct linear correlation between enterochromaffin-like, gastrin, and parietal cell density was demonstrated. An almost complete map of mucosal cells involved in gastric secretion is provided by this study. The cell density pattern, identical to the omeprazole group, points to an impaired feedback control of secretion in duodenal ulcer. The reversion to control values after Helicobacter pylori eradication and ulcer healing demonstrates the pathogenetic role of Helicobacter pylori-host interaction in these changes. Aims/hypothesis. We investigated in normotensive Type I (insulin-dependent) diabetic patients with normoalbuminuria the role of growth hormone-induced IGF-1 in the modulation of renal haemodynamics.¶Methods. We measured glomerular filtration (GFR) and renal plasma flow (RPF) at baseline and at 24 h after injection of different doses of growth hormone (0.1, 0.2, 0.4 U · kg–1· body weight–1) in six patients with normal GFR under a euglycaemic clamp. We also examined a 24-h profile of plasma growth hormone and IGF-1 during usual insulin therapy in two other groups each with seven patients with a lower (from 93 to 114 ml · min–1· (1.73 m2)–1) and higher (from 121 to 146 ml · min–1· (1.73 m2)–1) GFR.¶Results. Plasma growth hormone concentrations peaked 2 h after its injection and plasma concentrations of IGF-1 peaked about 24 h after the growth hormone injection. There was a significant increase in GFR and RPF 24 h after the highest dose of the growth hormone injection (corresponding to the highest IGF-1 concentration), from baseline values of 115 ± 24 and 536 ± 141 ml · min–1· (1.73 m2)–1 to 160 ± 33 and 657 ± 137 ml · min–1· (1.73 m2)–1, respectively (p < 0.01 for GFR and p < 0.05 for RPF). No differences were observed in the 24-h profile of growth hormone and IGF-1 plasma concentrations between the two groups; growth hormone and IGF-1 concentrations were lower than those obtained after the injection of 0.4 U · kg–1· body weight–1 of growth hormone.¶Conclusion/interpretation. These results show that pharmacological growth hormone-induced IGF-1 concentrations are required to modify renal haemodynamics in Type I diabetic patients and suggest that, under the “physiological” conditions of diabetes, IGF-1 has no role as a mediator of glomerular hyperfiltration. The therapeutic options for patients with acute leukemia who relapse after the initial transplant include second bone marrow transplantation (2BMT) and conventional chemotherapy (CC). In this work, we conducted an analysis of published survival data and we evaluated the cost-effectiveness of 2BMT in comparison with CC. We retrieved survival information on 167 patients treated with 2BMT and 299 patients treated with CC. Survival figures were derived from individual patient data and were compared between 2BMT and CC. The mean lifetime survival (MLS) was estimated for each of the two patient cohorts using standard techniques of survival-curve extrapolation. The cost data of patients given 2BMT or CC were estimated from published data. Our analysis of individual survival data showed that 2BMT improved survival at levels of statistical significance (survival gain = 19.6 months per patient). Using an incremental cost of $90 000 per patient, the cost-effectiveness ratio of 2BMT in comparison with CC was calculated as $52 215 discounted dollars per discounted life year gained. Our results indicate that, in patients with acute leukemia who relapse after their first transplant, 2BMT significantly prolongs survival in comparison with CC and seems to have an acceptable cost-effectiveness profile. Thirty-eight second allogeneic bone marrow transplants (BMT) for acute leukemia relapsed after first BMT were performed in 13 Italian centers between 1987 and 1994. Twenty-one patients had acute myelogenous leukemia (AML), 17 acute lymphoblastic leukemia (ALL); at second BMT 24 patients were in complete remission (CR) and 14 in relapse. The median time to relapse after first BMT was 10 months (range 1-70). Grade II or greater acute graft-versus-host disease (GVHD) after second transplant occurred in 34.2% of patients and a chronic GVHD in 31.5% of patients. Twenty-four patients died: seven from early transplant-related mortality (TRM), 13 from relapse and four from late toxicity. As of 31 July 1996, at a median follow-up of 47 months (range 22-85), there are 14 survivors. The three-year probability of TRM, relapse and event-free survival (EFS) is 28%, 40% and 42% respectively. In 20 of 27 evaluable patients, remission duration after second BMT was longer than after the first BMT. A diagnosis of AML was correlated with a better outcome. Life expectancy is shorter in the subset of insulin-dependent diabetic (IDDM) patients who are susceptible to kidney disease. Familial factors may be important. In this study the prevalence of cardiovascular disease mortality and morbidity and of risk factors for cardiovascular disease was compared in the parents of 31 IDDM patients with elevated albumin excretion rate (AER > 45 μg/min; group A) with that of parents of 31 insulin-dependent diabetic patients with normoalbuminuria (AER < 20 μg/min; group B). The two diabetic patient groups were matched for age and duration of disease. Information on deceased parents was obtained from death certificates and clinical records and morbidity for cardiovascular disease was ascertained using the World Health Organization questionnaire and Minnesota coded ECG. Hyperlipidaemia was defined as serum cholesterol higher than 6 mmol/l and/or plasma triglycerides higher than 2.3 mmol/l and/or lipid lowering therapy; arterial hypertension as systolic blood pressure higher than 140 mmHg and/or diastolic blood pressure higher than 90 mmHg and/or antihypertensive treatment. The percentage of dead parents was similar in the two groups (26 vs 20 % for parents of group A vs group B, respectively), but the parents of the diabetic patients with elevated AER had died at a younger age (58 ± 10 vs 70 ± 14 years; p < 0.05). Parents of diabetic patients with nephropathy had a more than three times greater frequency of combined mortality and morbidity for cardiovascular disease than that of the parents of diabetic patients without nephropathy (26 vs 8 %; odds ratio 3.96, 95 % CI 1.3 to 12.2; p < 0.02). Living parents of group A had a higher prevalence of arterial hypertension (42 vs 14 % p < 0.01) and hyperlipidaemia (49 vs 26 % p < 0.05) as well as higher levels of lipoprotein (a) [median (range) 27.2 (1–107) vs 15.6 (0.2–98) mg/dl; p < 0.05]. They also had reduced insulin sensitivity [insulin tolerance test: median (range) Kitt index: 3.7 (0.7–6.2) vs 4.8 (0.7–6.7)% per min; p < 0.05]. In the families of IDDM patients with elevated AER there was a higher frequency of risk factors for cardiovascular disease as well as a predisposition to cardiovascular disease events. This may help explain, in part, the high prevalence of cardiovascular disease mortality and morbidity in those IDDM patients who develop nephropathy. Nitric oxide (NO)-synthase immunoreactivity has been detected for the first time in mast cells of human normal nasal mucosa, with an antibody specific for neuronal NO-synthase. Intense immunoreactivity was revealed in secretion granules of mast cells but was found in mast cell granules free in the extracellular matrix only in some instances; no reactivity was found in the cytoplasm of this or other cell types. These findings suggest that human nasal mast cells contain a particulate isoform of NO-synthase, which shares epitopes with neuronal NO-synthase and is rapidly removed from granules upon exocytosis. The target of human flight in space has changed from permanence on the International Space Station to missions beyond low earth orbit and the Lunar Gateway for deep space exploration and Missions to Mars. Several conditions affecting space missions had to be considered: for example the effect of weightlessness and radiations on the human body, behavioral health decrements or communication latency, and consumable resupply. Telemedicine and telerobotic applications, robot-assisted surgery with some hints on experimental surgical procedures carried out in previous missions, had to be considered as well. The need for greater crew autonomy in health issues is related to the increasing severity of medical and surgical interventions that could occur in these missions, and the presence of a highly trained surgeon on board would be recommended. A surgical robot could be a valuable aid but only inasfar as it is provided with multiple functions, including the capability to perform certain procedures autonomously. Space missions in deep space or on other planets present new challenges for crew health. Providing a multi-function surgical robot is the new frontier. Research in this field shall be paving the way for the development of new structured plans for human health in space, as well as providing new suggestions for clinical applications on Earth., (© 2021.
