Objective:To examine the longitudinal associations between different physical activity (PA) intensities and cardiometabolic risk factors among a sample of Canadian youth.Methods:The findings are based on a 2-year prospective cohort study in a convenience sample of 315 youth aged 9-15 years at baseline from rural and urban schools in Alberta, Canada. Different intensities (light, moderate and vigorous) of PA were objectively assessed with Actical accelerometers. The main outcome measures were body mass index (BMI) z-score, waist circumference, cardiorespiratory fitness and systolic blood pressure at 2-year-follow-up and conditional BMI z-score velocity. A series of linear regression models were conducted to investigate the associations after adjusting for potential confounders.Results:At follow-up, cardiorespiratory fitness increased (quartile 1 vs quartile 4=43.3 vs 50.2; Ptrend<0.01) and waist circumference decreased (quartile 1 vs quartile 4=79.0 vs 72.6; Ptrend=0.04; boys only) in a dose-response manner across quartiles of baseline vigorous-intensity PA. A similar trend was observed for systolic blood pressure (quartile 1 vs quartile 4=121.8 vs 115.3; Ptrend=0.07; boys only). Compared with quartile 1 of vigorous-intensity PA, BMI z-score at follow-up and conditional BMI z-score velocity were significantly lower in the quartile 2 and 3 (P<0.05). Waist circumference at follow-up also decreased (quartile 1 vs quartile 4=75.3 vs 73.8; Ptrend=0.04) across quartiles of baseline moderate-intensity PA.Conclusions:Time spent in vigorous-intensity PA was associated with several positive health outcomes 2 years later. BACKGROUND/OBJECTIVES: In parallel with the obesity epidemic, consumption of sugar-sweetened beverages (SSB) has risen over the same period. Our aim was to investigate associations between the consumption of SSB in childhood and adolescence with subsequent changes in body fatness in early adulthood. SUBJECTS/METHODS: A longitudinal study of 9-year-old children (n=283) enrolled in the Danish part of the European Youth Heart Study with a 6-year and 12-year follow-up. Data were collected at ages 9, 15 and 21 years. Multivariate regression analyses with adjustment for potential confounders were used to evaluate the effect of SSB consumption at 9 and 15 years and change in SSB consumption from 9–15 years on subsequent change in body fatness until 21 years. RESULTS: Subjects who consumed more than one serve of SSB daily at age 15 years had larger increases in body mass index (BMI) (β=0.92, P=0.046) and waist circumference (WC) (β=2.69, P=0.04) compared to non-consumers over the subsequent 6 years. In addition, subjects who increased their SSB consumption from age 9–15 years also had larger increases in BMI (β=0.91, P=0.09) and WC (β=2.72, P=0.04) from 15–21 years, compared to those who reported no change in consumption. No significant association was observed from 9–21 years. CONCLUSION: This study provides new evidence that SSB consumption in adolescence and changes in SSB consumption from childhood to adolescence are both significant predictors of change in body fatness later in early adulthood. A primary goal of the paper is to provide an example of an evaluation design and analytic method that can be used to strengthen causal inference in nonexperimental prevention research. We used this method in a nonexperimental multisite study to evaluate short-term outcomes of a preventive intervention, and we accounted for effects of two types of selection bias: self-selection into the program and differential dropout. To provide context for our analytic approach, we present an overview of the counterfactual model (also known as Rubin's causal model or the potential outcomes model) and several methods derived from that model, including propensity score matching, the Heckman two-step approach, and full information maximum likelihood based on a bivariate probit model and its trivariate generalization. We provide an example using evaluation data from a community-based family intervention and a nonexperimental control group constructed from the Washington State biennial Healthy Youth Survey (HYS) risk behavior data (HYS n = 68,846; intervention n = 1,502). We identified significant effects of participant, program, and community attributes in self-selection into the program and program completion. Identification of specific selection effects is useful for developing recruitment and retention strategies, and failure to identify selection may lead to inaccurate estimation of outcomes and their public health impact. Counterfactual models allow us to evaluate interventions in uncontrolled settings and still maintain some confidence in the internal validity of our inferences; their application holds great promise for the field of prevention science as we scale up to community dissemination of preventive interventions. Transepidermal water loss (TEWL) is regarded as one of the most important parameters for characterizing skin barrier function but an agreed upon definition of what a 'normal' TEWL is does not exist. In order to determine generalizable TEWL values for healthy adults, a systematic review and meta-analysis was conducted. The databases MEDLINE and EMBASE and publication lists were screened. After full-text appraisal of 398 studies, 231 studies were excluded due to unclear or insufficient reporting. 167 studies providing data about 50 skin areas were included in the final data synthesis. Pooled sample sizes ranged from n = 5 for the left cheek and the left lower back to a maximum of n = 2,838 for the right midvolar forearm area. The lowest TEWL of 2.3 (95 % CI 1.9-2.7) g/m/h was calculated for the breast skin, the highest TEWL of 44.0 (39.8-48.2) g/m/h for the axilla. TEWL in individuals being 65 years and above was consistently lower compared to the group of 18- to 64-year-old individuals. Background/Objectives:Vitamin D levels are often observed to be low in Canadian youth, despite the mandatory fortification of fluid milk. We identified modifiable correlates of plasma vitamin D concentrations to inform public health efforts to remediate low-vitamin D status.Subjects/Methods:We recruited 159 children aged 8-11 years, who were at at high risk of obesity, non-systematically during different seasons. Vitamin D status was assessed by measuring plasma 25-hydroxyvitamin D (25(OH)D) using a radioimmunoassay. Dietary intake, including vitamin supplements, was measured using three dietitian-administered 24 h diet recalls. Fat mass was measured by dual energy X-ray absorptiometry. Accelerometers were worn for 7 days to estimate physical activity. Independent correlates of plasma 25(OH)D concentrations were identified using multiple regression in an analysis controlling for season of measurement.Results:Approximately, 7% of youth had hypovitaminosis D (25(OH)D 37.5 nmol/l) during winter and spring when vitamin D levels are at their nadir. Only 55% of participants had vitamin D levels, which the Institute of Medicine considers optimal (25(OH)D >50 nmol/l). The mean dietary vitamin D intake, 6.6 mcg, was well below current recommendations set at 15 mcg. A serving increase in milk consumption and a s.d. increase in physical activity were associated with only a 2.9 and 2.1 nmol/l increase in plasma 25(OH)D, respectively. There was no association between 25(OH)D and adiposity.Conclusions:Our results indicate the challenges of obtaining adequate vitamin D intake from the current food supply to support a level of 25(OD)D >50 nmol/l. In this pilot study, we examined the relationship between health factors, sociodemographic factors, and body mass index (BMI) across two generations ( n = 41 parent-child pairs). Generation 1 study variables included parent- and family-focused characteristics and health variables, the Generation 2 variables included child demographic factors, and the outcome variable was youths’ physical health (operationalized as BMI). Regression models revealed that Generation 1 variables, taken together, accounted for 26% of the variance in youth BMI. However, only the parent’s mental health symptoms (i.e., depression symptoms) made a unique contribution to the variance in youth BMI. Logistic regression analysis revealed that the youths’ race and age—but no other demographic factor—were significantly related to youth BMI-for-age. Our findings suggest that youth race, age, and parent mental health are each associated with youth physical health (i.e., BMI), confirming previous study findings that parental factors and demographic factors should be considered when exploring youth health outcomes. Rates of overweight in youth have reached epidemic proportions and are associated with adverse health outcomes. Family-based programs have been widely used to treat overweight in youth. However, few programs incorporate a theoretical framework for studying a family systems approach in relation to youth health behavior change. Therefore, this review provides a family systems theory framework for evaluating family-level variables in weight loss, physical activity, and dietary approaches in youth. Studies were reviewed and effect sizes were calculated for interventions that manipulated the family system, including components that targeted parenting styles, parenting skills, or family functioning, or which had novel approaches for including the family. Twenty-one weight loss interventions were identified, and 25 interventions related to physical activity and/or diet were identified. Overall, family-based treatment programs that incorporated training for authoritative parenting styles, parenting skills, or child management, and family functioning had positive effects on youth weight loss. Programs to improve physical activity and dietary behaviors that targeted the family system also demonstrated improvements in youth health behaviors; however, direct effects of parent-targeted programming is not clear. Both treatment and prevention programs would benefit from evaluating family functioning and parenting styles as possible mediators of intervention outcomes. This study examines the longitudinal effects of childhood community contexts on young adult outcomes. The study uses a sample of 14,000 adolescents (52% female) derived from the 1990 US Census and the National Longitudinal Study of Adolescent Health (Addhealth). The study examines whether community and family environments exert separate and/or joint long-term influences on young adult achievement and depression. We found both direct and indirect long-term influences of childhood community adversity on young adult educational attainment. The indirect influences of childhood community adversity operated through family and individual-level factors. The long-term influence of childhood community adversity on young adult depression was only indirect. Overall, community influences on young adult achievement outcomes were mediated by family context and by the adolescents’ adjustments and transitions, including adolescent depression, school adjustment, and disruptive transitional events. The moderating effect of childhood community adversity suggests that the protective effects of family resources on young adult outcomes dissipate significantly in extremely adverse neighborhoods. The findings demonstrate the importance of integrating multiple theoretical perspectives for longitudinal research to capture pathways of community influence on adolescent developmental and young adulthood outcomes. We examine the consequences of child health for economic and health outcomes in adulthood, using height as a marker of childhood health. After reviewing previous evidence, we present a conceptual framework that highlights data limitations and methodological problems that complicate the study of this topic. We then present estimates of the associations between height and a range of outcomes—including schooling, employment, earnings, health, and cognitive ability—measured in five data sets from early to late adulthood. These results indicate that, on average, taller individuals attain higher levels of education. Height is also positively associated with better economic, health, and cognitive outcomes. These associations are only partially explained by the higher average educational attainment of taller individuals. We then use data from the National Longitudinal Survey of Youth 1979 Children and Young Adults survey to document the associations between health, cognitive development, and growth in childhood. Even among children with the same mother, taller siblings score better on cognitive tests and progress through school more quickly. Part of the differences found between siblings arises from differences in their birth weights and lengths attributable to mother's behaviors while pregnant. Poor sleep quality commonly co-occurs with anxiety among youth. However, little research has examined linkages between sleep quality and factors thought to underlie panic vulnerability. This study tested the association between self-reported sleep quality and anxiety and fear reported during panic-relevant abrupt increases in bodily arousal elicited by a laboratory-based biological challenge procedure among community-recruited adolescents ( n = 88; Mage = 14.00 years; SD = 2.37). As predicted, both anxiety and fear reported during a well-established 3-min voluntary hyperventilation procedure were significantly positively associated with self-reported sleep quality, after controlling for age, gender, and disease concerns sub-facet of anxiety sensitivity. Findings are discussed in terms of additional research needed to better understand why sleep quality and anxiety reported during elevated bodily arousal are related among youth. Objective:to explore the effect of organized youth sport on metabolic syndrome (MetS) in adulthood.Design:Longitudinal study data from the cardiovascular risk in young Finns study.Subjects:A total of 1493 males (n=704) and females (n=789) aged 3, 6, 9, 12, 15 and 18 years were randomly selected from five university towns and their rural surroundings in 1980. They were followed up for 21 years. In 2001 they were 24, 27, 30, 33, 36 and 39 years old.Measurements:Youth sports participation data (participation in sport-club training and competitions) were assessed in 1980 and 1983 using a self-report questionnaire completed in connection with a medical examination. Participants were divided into athletes and non-athletes at each measurement point, and then classified into four groups: Persistent athlete, Starter, Leaver and Non-athlete. A mean score of youth sport was assessed by calculating the average of four consecutive measurements (1980–1989). MetS risk in 2001 was defined as a categorical variable based on the guidelines of the European Group for the Study of Insulin Resistance (EGIR) and as a continuous MetS-score variable by summing the z-scores of individual metabolic variables.Results:In males and females, intense participation in youth sports over 3 years was inversely and significantly associated with clustered MetS score and prevalence of MetS defined by EGIR in adulthood (P<0.05). The association remained significant after adjustment for age, baseline clustered MetS score, smoking and total caloric intake and after additional adjustments for adult leisure-time physical activity. Starters during 3 years were less likely to have MetS than non-athletes. Leavers were at a higher risk for MetS than persistent athletes. These associations were attenuated in males by adjustment for all potential confounders. Similar associations were found using EGIR MetS as an outcome.Conclusions:Sustained participation in organized sport lasting at least 3 years in youth is associated with reduced risk for developing MetS in adulthood. A paucity of research exists in which the co-occurrence of substance use, mental illness, and violence in young adults is examined. Concurrently, there is also a lack of research explicating the contribution of theoretically based risk factors for these problematic outcomes in this population. This lack of both outcome and explanatory research equally affects the utility of theories and interventions for this population. This article utilizes a sample of N = 633 21-year-olds to examine the prevalence of (1) violence and substance use, (2) mental illness (i.e., mood and anxiety disorders) and substance use, and (3) the use of multiple substances and investigates the relationship between various social determinants and said outcomes. Overall, the prevalence rates for the comorbid conditions were low; although on average males had higher rates than did females. Individual attitudes, perceived opportunities, and recent stressful life events were associated with the co-occurrence of outcomes. Implications for behavioral health are explored. Discrete wavelet analysis is used to resolve the center of pressure time series data into several timescale components, providing new insights into postural control. Healthy young and elderly participants stood quietly with their eyes open or closed and either performed a secondary task or stood quietly. Without vision, both younger and older participants had reduced energy in the long timescales, supporting the concept that vision is used to control low frequency postural sway. Furthermore, energy was increased at timescales corresponding to closed-loop (somatosensory and vestibular) and open-loop mechanisms, consistent with the idea of a shift from visual control to other control mechanisms. However, a relatively greater increase was observed for older adults. With a secondary task a similar pattern was observed—increased energy at the short and moderate timescales, decreased energy at long timescales. Declining student interest and scholastic abilities in the sciences are concerns for the health professions. Additionally, the National Institutes of Health is committed to promoting more research on health behaviors among US youth, where one of the most striking contemporary issues is obesity. This paper reports findings on the impact of a partnership between Oregon Health and Science University (OHSU) and the Oregon Museum of Science and Industry linked to a 17-week exhibition of BodyWorlds3 and designed to inform rural underserved youth about science and health research. Self-administered survey measures included health knowledge, attitudes, intended health behaviors, and interest in the health professions. Four hundred four surveys (88% of participants) were included in analyses. Ninety percent or more found both the BodyWorlds ( n = 404) and OHSU ( n = 239) exhibits interesting. Dental care habits showed the highest level of intended behavior change (Dental = 45%, Exercise = 34%, Eating = 30%). Overall, females and middle school students were more likely than male and high school students, respectively, to state an intention to change exercise, eating and dental care habits. Females and high school students were more likely to have considered a career in health or science prior to their exhibit visit and, following the exhibit, were more likely to report that this intention had been reinforced. About 6% of those who had not previously considered a career in health or science ( n = 225) reported being more likely to do so after viewing the exhibits. In conclusion, high quality experiential learning best created by community-academic partnerships appears to have the ability to stimulate interest and influence intentions to change health behaviors among middle and high school students. The study focuses on the serious issue related to the adolescents’ and adults’ behavior and health. It aims to identify the risk factors for drug abuse from samples taken from a town of Eastern Nepal. This is a matched case-control study. The conditional logistic regression method was adopted for data analysis. The diagnosis cut off was determined by Receiver Operating Characteristic curve. A representative sample of 150 matched pairs was recruited from Dharan in 2006. The final model after adjusting 17 possible variables each other, detected some factors like education, occupation, short temper, depression, etc that were significantly associated with drug abuse, but shy behavior was not a significant predictor for drug abuse among the study sample. The level of education, occupation and depression were the strong predictors as identified by the model. Adolescent methamphetamine use is a prominent concern for Canadian media and government. Few empirical studies, however, have established the scope of adolescent methamphetamine use or associated outpatient substance abuse treatment utilization. The current study aimed to answer the following questions: (1) What was the overall proportion of primary methamphetamine-related cases seen at youth outpatient substance abuse treatment centres in British Columbia in 2005–2006?; (2) What kind of treatment programming did these treatment centres offer primary methamphetamine users?; and (3) How prepared were these treatment programs to provide services to primary methamphetamine users? We developed a comprehensive list of all sites offering outpatient substance-abuse treatment programs for youth in British Columbia. We asked the Executive Director (or designate) of each facility about the program’s annual caseload, proportion of primary methamphetamine-related cases, treatment approach, and preparedness to provide effective treatment for methamphetamine users. Responses were received from 68 of the 80 centres on our final master list. Approximately 11% (780/6,840) of all cases at the sites were due primarily to methamphetamine use. The proportion of primary methamphetamine-related cases varied widely across sites in B.C., with contacted centres reporting between 0–65% of their caseload in 2005–2006 as primarily methamphetamine-related. Almost all facilities integrated primary-methamphetamine users into treatment-as-usual, and few centres (12%) felt inadequately prepared to offer treatment for methamphetamine users. The current study found a substantial proportion of all cases seen at youth outpatient substance abuse treatment centres in British Columbia in 2005–2006 were primarily due to methamphetamine misuse. While most treatment sites integrated youth methamphetamine users into regular treatment programming and few treatment directors indicated that their programs were inadequately prepared to offer treatment services to methamphetamine-using clients, more evidence is needed to ascertain the most effective treatment strategies for methamphetamine users. Anal fissures can cause morbidity in an otherwise healthy young patient. The process of evacuation results in stretching and descent of the anoderm and perineum especially posteriorly. Posterior perineal support may provide counter pressure at the posterior aspect of the pelvic floor, balancing the pressure exerted by the faeces on the anal wall, thus improving evacuation and reducing the trauma associated with it, and reducing symptoms of anal fissures. Symptoms of constipation may also be reduced secondarily. We report the preliminary results with a novel, simple and noninvasive method of treatment provided by a toilet seat device. A prospective study was performed in 32 patients. The study was designed mainly to investigate the patients’ subjective perceptions of their symptoms related to anal fissures and constipation. Questionnaires were provided to patients before, during and after treatment. The study revealed statistically significant improvement in pain, bleeding, symptoms of constipation and abdominal discomfort after 3 months usage of the device. The odds of patients perceiving an improvement in symptoms were also significantly increased after 3 months of treatment compared to 2 weeks of treatment. This preliminary study revealed that a posterior perineal support device can bring about sig nificant improvement in the symptoms of patients with anal fissures. There may also be secondary benefits of a reduction in the symptoms of constipation. Although not conclusive, these results should serve as a springboard for further research into this area. The purpose of this study was to examine the effects of postactivation potentiation (PAP) on the torque and rate of torque development for contractions evoked by short trains of stimuli at different frequencies, in young and elderly adults. Individual mechanical contributions to each electrical stimulus within trains were also analysed. Single pulse, and two- (PT2) and three-pulse trains (PT3) delivered at 20, 50, 80 and 100 Hz were evoked before and during a 10 min period after a 6-s conditioning MVC. The results show that PAP of the torque for PT2 decreased with the increase of the stimulation frequency for young (from 184.5 to 140.4% of control values) and elderly (from 140.5 to 109.6%). Regardless of the stimulation frequency, the peak of PAP was greater in young than in elderly and occurred immediately after the conditioning MVC but was delayed for the 100 Hz condition in elderly adults. For PT3, the results were similar although the extent of PAP was less. The PAP of the mechanical contributions within the trains also decreased with the augmentation of the stimulation frequency. For most of the frequencies above 20 Hz, the peak of PAP for each mechanical contribution was delayed by 1 min after the conditioning MVC. These results indicate an age- and frequency-related PAP saturation of the successive mechanical contributions within a train of stimuli that decrease with time. Objective To examine the prevalence of health risk-behaviors among American Indian/Alaska Native (AI/AN) youth in urban areas. Methods Data from the national Youth Risk Behavior Survey (YRBS) for the years 1997–2003 were used for the analyses (Urban sample = 52,364). The YRBS is a self-report questionnaire administered to a sample of 9th–12th grade students intended to monitor health risk-behaviors. “Urban” is defined as areas within a Metropolitan Statistical Area. Whites are used as the comparison group for the examination of AI/AN estimates. Results Urban AI/AN youth represented 1% of the urban sample (N = 513). The presence of a number of risk-behaviors were at least threefold higher in AI/AN compared to white youth in urban areas, including suicidal behaviors, feeling unsafe at school and needing medical treatment from a fight. Other factors were over twofold higher among AI/AN, including sexual behaviors, illegal drug use, violence at school, and experiences of rape, assault and pregnancy. Conclusions The higher prevalence of health risk-behaviors in urban AI/AN compared to white youth reflects a need for interventions focused on urban AI/AN youth. [ABSTRACT FROM AUTHOR]

We investigated the extent to which clinician-assigned diagnoses of emotional and behavioral disorders and clinicians’ perceptions of client change are consistent with structured measures of youth clinical functioning and parent/family characteristics within the context of usual care or “real world” psychotherapy. A total of 54 therapists from two publicly-funded youth outpatient mental health clinics and 128 youths and parents from the therapists’ combined caseloads were included in the study. Clinician-assigned diagnosis and youth and family demographics were collected at the initial visit, clinician-reported perceived client change was collected at 6-month follow-up, and structured measures of youth clinical functioning and parent/family characteristics were collected at both time points. Results indicate some overlap between clinician-assigned diagnosis, clinician-reported perceived client change, and structured measures of youth clinical functioning and parent/family characteristics after controlling for demographic factors. Analysis reported here examined youth in residential group home placements in order to better understand how sexual behaviors, drug use, and environmental experiences differ by race and gender subgroups. Data were collected from 336 youth aged 14–21 residing in one of 41 group homes in Maryland. Chi square and logistic regression analyses were done to determine differences on study variables among the race-gender groupings. Results demonstrate not only significant health concerns overall but notable differences were also found between race-gender subgroups. BACKGROUND: The World Health Organization encourages the development of youth friendly services, yet little is known on how youth currently present in general practice. OBJECTIVE: To describe the perspectives, expectations, and service receipt of young people presenting to family doctors to inform the development of youth friendly services. DESIGN: Cross-sectional survey. PARTICIPANTS AND MEASUREMENTS: Consecutive young people attending 26 randomly selected practices were recruited in the waiting rooms. Standardized instruments were used to interview them before their consultation. RESULTS: Of 501 young people who were approached, 450 participated (91% participation rate). Most had respiratory (26%) or dermatological complaints (18%). When asked to assess their health status, 59% perceived they had neither a physical nor a mental illness. However. 43% stated they had fears about their health problem and 1 in 5 feared it could be life-threatening. Although only 10% presented with psychological com- plaints. 24% perceived they currently had a mental illness. The most common expectations were treatment (50%) and good communication (42%). Most youth were prescribed medication (60%), but 40% of those who received a prescription had not expected to receive a treatment. A follow-up appointment was offered to 57% of participants. CONCLUSIONS: This study identifies a gap between young people's perception of illness and their presentations to family doctors. It also highlights unexpected fears, and a mismatch between expectations and service receipt. These findings have implications for family medicine training and for clinical practice. This article reviews the habitual physical activity of children and adolescents from member countries of the European Union in relation to methods of assessing and interpreting physical activity. Data are available from all European Union countries except Luxembourg and the trends are very similar. European boys of all ages participate in more physical activity than European girls and the gender difference is more marked when vigorous activity is considered. The physical activity levels of both genders are higher during childhood and decline as young people move through their teen years. Physical activity patterns are sporadic and sustained periods of moderate or vigorous physical activity are seldom achieved by many European children and adolescents. Expert committees have produced guidelines for health-related physical activity for youth but they are evidence-informed rather than evidence-based and where there is evidence of a relationship between physical activity during youth and health status there is little evidence of a particular shape of that relationship. The number of children who experience physical activity of the duration, frequency and intensity recommended by expert committees decreases with age but accurate estimates of how many girls and boys are inactive are clouded by methodological problems. If additional insights into the promotion of health through habitual physical activity during youth are to be made, methods of assessment need to be further refined and recommended guidelines re-visited in relation to the existing evidence base. BACKGROUND:Estimates of the prevalence of overweight and obesity in young people are typically based on body mass index (BMI). However, BMI may not indicate the level of central adiposity. Waist circumference has therefore been recommended to identify young people at risk of morbidity associated with central adiposity.OBJECTIVE:To investigate (a) change in total and central adiposity between 7–8 and 12–13 y (b) agreement between classifying young people as overweight or obese based on total adiposity and central adiposity, and (c) risk factors associated with the development of total and central adiposity.DESIGN:Anthropometric measurements were taken on 342 children in 1996/97 and 5 y later. Risk factors examined included birth weight, physical activity, TV viewing, pubertal status, parental adiposity, diet and socio-economic status.RESULTS:Between 7–8 and 12–13 y indices of central adiposity increased more than total adiposity; waist circumference z-score increased by (mean±s.d.) 0.74±0.92 and BMI z-score increased by 0.18±0.67. At 12–13 y there was moderate agreement between the two measures of adiposity (weighted kappa=0.64). However, waist circumference identified a greater number of young people as overweight or obese compared to BMI (41.2 vs 29.3%, P<0.001). Adiposity status at 7–8 y, maternal obesity, and pubertal stage were the strongest predictors of BMI status at 12–13 y. Risk factors associated with increased central adiposity were similar.CONCLUSIONS:Overweight and obesity, as measured by waist circumference, is a bigger problem than is currently assessed by BMI. OBJECTIVE:: To develop age- and sex-specific waist circumference reference data for Canadian youth. DESIGN:: Cross-sectional population survey. SETTING:: 1981 Canada Fitness Survey. SUBJECTS:: Nationally representative sample of 3064 youth (1540 boys and 1524 girls) 11-18?y of age. INTERVENTIONS:: Waist circumference was measured with a flexible anthropometric tape. MAIN OUTCOME MEASURES:: Smoothed and weighted 5th, 10th, 25th, 50th, 75th, 90th, and 95th waist circumference percentiles derived from LMS regression. RESULTS:: Waist circumference increases with age in both boys and girls, and boys have higher values of waist circumference than girls at every age and percentile level. CONCLUSIONS:: These reference data can be used to identify youth with an elevated risk of developing obesity-related disorders and can serve as a baseline for future studies of temporal trends in waist circumference. We report on results of a one-day survey measuring rates of substance use and HIV risk behaviors among the homeless youth population of Denver, Colorado. On March 15, 2001, staff of Urban Peak, conducted a single-day survey of homeless and runaway youth in the Denver metropolitan region, going to locations known to be frequented by this population. All youth encountered were asked to fill out a brief survey asking about past nine month use of the following substances: alcohol, marijuana, cocaine, methamphetamine, heroin, hallucinogens, ecstasy and ketamine, and HIV risk behaviors. Chi-square analyses of the association of substance used and gender, age, living situation, and ethnicity were conducted. In addition, the use of any club drug was examined. One-hundred-eighty-six homeless or runaway youth were surveyed; 74 percent were between 16 and 25. Rates of use over the last nine months were as follows: alcohol, 69 percent; marijuana, 75 percent; methamphetamine, 18 percent; cocaine, 19 percent; heroin, 12 percent; hallucinogens, 30 percent; ecstasy, 25 percent; and ketamine, 13 percent. Eleven percent reported trading sex for drugs, money, food, or shelter; and 13 percent reported sharing needles. There were significant associations between living situation and use of marijuana, cocaine and hallucinogens. Prevalence rates of club drugs show 75 percent, 77 percent and 77 percent of homeless or runaway youth ihaving used ecstasy, ketamine and hallucinogens one to three times per month over the last nine months, respectively. Prevalence rates of substance use among homeless youth in the Denver metropolitan are similar to rates reported in other larger metropolitan areas. Routine screening for every substance needs to be part of the assessment for all homeless youth. Examine the impact of Colorado's Medicaid mental health carve-out program on children in child welfare and juvenile justice systems. Medicaid claims and encounter data for two experimental managed care sites and one comparison fee-for-service site are used to estimate a two-part model of inpatient, outpatient, and residential treatment center utilization, controlling for patient characteristics. The study finds that juvenile justice and child welfare populations were more strongly affected by managed care than the general youth population, regarding reduced utilization of inpatient and outpatient services. Increases in Residential Treatment Centers use were greater for juvenile justice than either the child welfare sample or the total sample. Youth in child welfare increase utilization of outpatient services. Most utilization effects are stronger for not-for-profit than for-profit managed care organizations. The experience of Colorado implies that a mental health carve-out affects patterns of care for youth and differentially so for youth in juvenile justice and child welfare systems. This study examined the “treated” prevalence, incidence, and pharmacotherapy of child and adolescent mood disorders in a managed care setting. General prevalence patterns across age and sex were similar to those reported in community epidemiology studies, although, not unexpectedly, the overall rates were somewhat lower. Primary care providers identified a substantial proportion of the youth with a mood disorder. Antidepressant medication was used more often by youth identified with a mood disorder in medical care settings than by those youth identified in specialty mental health care settings. These results are valuable in determining if youth with mood disorders receiving medication prescriptions across settings are treated according to current best practice guidelines (such as the adult depression guidelines [U.S. Department of Health and Human Services, 1993]), that is, primarily with SSRI medications. Summary In a duplicate study during 1987-1991, 478 24-h duplicate samples from 14 homes for elderly people and 10 homes for youth were investigated for their contents of selected harmful substances. The analyses covered 45 active substances of pesticides, 17 PCB-congeners as well as lead, cadmium, and nitrate contents. Pesticides could be detected only in 15 % of the investigated samples. The pesticide contents reached max. 8 % of the respective FAO/WHO-limits. As the mean intake of the three most important PCB-congeners (sum of the congeners 138, 153, and 180) values of 0.9 and 1.1 micro g per day and ration or person, respectively, were found. Also in the worst case the daily PCB intake was below the recommended ADI value of the FDA of 1 micro g/kg of body weight. The daily nutritional intake of lead and cadmium via the investigated daily rations reached about 5.6 % and 20 % of the Provisionally Tolerably Weekly Intake values of the FAO/WHO. The mean nitrate content of the duplicate portions was 101 mg per day and person (median: 79 mg per day and person). We assessed the utility of an empirically-derived classification system for youths with emotional and behavioral disorders in a system of care with a strong representation of juvenile delinquents. Eighty-seven youths served in a system of care were categorized by variables related to past history, current behavioral functioning, and current psychological functioning into four clinical clusters. We evaluated preliminary clinical outcomes after six months of interagency involvement for youths within each cluster and across the four clusters. Results indicated that youths in different clusters improved significantly in need-specific areas. Trends in our data indicated differential change in clinical outcomes across clusters. We discuss our findings within the context of emerging outcomes of youths involved in comprehensive community-based programs and consider implications for outcome research, treatment of juvenile delinquents, and mental health policy. Using data from the Integrated Database on Children and Family Services in Illinois, we examined factors associated with the length of psychiatric hospitalization of youths in state hospitals between July 1, 1987 and June 30, 1992. Univariate analysis indicated that those diagnosed with attention deficit, psychotic, and conduct disorder experienced longer hospitalizations. In contrast, youths diagnosed with depressive disorders, drug and alcohol disorders, and adjustment disorders had shorter hospitalizations. Youths receiving services from multiple sectors of the human service delivery system experienced longer hospitalizations. Youths living in communities with state hospital linked community-based mental health services had shorter hospitalizations. Males experienced longer hospitalizations than females and African American youths experienced longer hospitalizations than White youths. Hispanic youths experienced shorter stays than either White or African American youths. The proportional hazards model indicated that being female, a history of previous admissions, multiservice use, and dangerousness were all associated with the decreased likelihood of discharge. Psychotic, attention deficit, and bipolar disorders were also associated with a decreased likelihood of hospital discharge. Youths with depressive disorders and African Americans with psychotic disorders had an increased likelihood of discharge. Increased age was also associated with the increased likelihood of discharge. Two hundred forty-six children (96 Whites, of whom 51 were males; 150 African- Americans, of whom 69 were males) with a familial history of essential hypertension (EH) were re-evaluated 5 years after an initial evaluation. During the initial visit anthropometric, demographic, and resting cardiovascular (CV) parameters (designated initial baseline levels) were assessed. These CV parameters (systolic and diastolic blood pressure [BP], heart rate, cardiac output index [CI], and total peripheral resistance index [TPRI] were also measured during postural challenge, a video game challenge, and a cold pressor task. At follow-up, resting CV parameters were again evaluated, and designated as follow-up resting levels. Moderate temporal stability (r range = .43-.56) was observed for all resting CV parameters. Mean stress responses for each CV parameter for all 3 stressors during the initial visit were positively related to the respective CV follow-up resting level. BP stress responses to postural change and video game challenge to be significant independent predictors of future resting BP after controlling for standard EH risk factors. Follow-up resting CI was not predicted by any stress responses, whereas follow-up resting TPRI was predicted by TPRI responses to the video game after controlling for standard Eh risk factors. These results contrast with those from an earlier 1-year follow-up, where stress responses for neither CI nor TPRI predicted follow-up resting levels.
