Background: In primarily unresectable liver tumors, ALPPS (Associating Liver Partition and Portal Vein Ligation for Staged hepatectomy) may offer curative two-stage hepatectomy trough a fast and extensive hypertrophy. However, concerns have been raised about the invasiveness of the procedure. Full robotic ALPPS has the potential to reduce the postoperative morbidity trough a less invasive access. The aim of this study was to compare the perioperative outcomes of open and full robotic ALPPS. Methods: The bicentric study included open ALPPS cases from the University Hospital Zurich, Switzerland and robotic ALPPS cases from the University of Modena and Reggio Emilia, Italy from 01/2015 to 07/2022. Main outcomes were intraoperative parameters and overall complications. Results: Open and full robotic ALPPS were performed in 36 and 7 cases. Robotic ALPPS was associated with less blood loss after both stages (418 ± 237 ml vs. 319 ± 197 ml; P = 0.04 and 631 ± 354 ml vs. 258 ± 53 ml; P = 0.01) as well as a higher rate of interstage discharge (86% vs. 37%; P = 0.02). OT was longer with robotic ALPPS after both stages (371 ± 70 min vs. 449 ± 81 min; P = 0.01 and 282 ± 87 min vs. 373 ± 90 min; P = 0.02). After ALPPS stage 2, there was no difference for overall complications (86% vs. 86%; P = 1.00) and major complications (43% vs. 39%; P = 0.86). The total length of hospital stay was similar (23 ± 17 days vs. 26 ± 13; P = 0.56). Conclusion: Robotic ALPPS was safely implemented and showed potential for improved perioperative outcomes compared to open ALPPS in an experienced robotic center. Purpose: Trabecular bone score (TBS), as a texture indicator of bone microarchitecture, predicts the risk of fracture. This study aims to explore the knowledge map of TBS. Methods: We searched Scopus for "trabecular bone score" or "trabecular score" from the beginning to 2021. Our inclusion criteria were original articles and reviews that were related to TBS and our exclusion criteria were non-English articles, non-related to TBS, and document type other than original articles and reviews. and related documents were included for bibliometric analysis. Excel, VOS viewer, and Science of Science (Sci2) software were used for data synthesis. Results: From 749 retrieved articles, 652 articles were included for analysis. These documents were cited 12,153 times and had an H-index of 56. The most productivity belonged to the USA (n = 130 documents), Switzerland (n = 101), and Italy (n = 67). "Osteoporosis International" (n = 80) had the highest participation in publishing. The research topics of interest were mainly related to the applicability of TBS for fracture risk assessment in chronic endocrine disorders such as osteoporosis and diabetes mellitus. Bursting analysis of the title and abstract revealed the initial focus of the discriminative power of TBS for osteoporotic fracture and the more recent focus on comparing bone mineral density (BMD) and TBS in a variety of chronic diseases. Conclusion: The number of annual publications on TBS has increased, especially after 2016. These publications highlight the importance of in-depth knowledge of TBS in predicting fracture risk and also its strengths and limitations of treatment monitoring in different health conditions. Unintentional introduction of natural enemies has increased in recent years due to a massive rise in global trade and tourism. One such natural enemy is the Asian egg parasitoid Trissolcus japonicus, a promising agent for classical biological control of Halyomorpha halys. In Europe, adventive T. japonicus populations have been detected in Switzerland, Italy and Germany. Host specificity testing demonstrated that its fundamental host range is fairly broad; however, it is unclear whether spatial or temporal refuges reduce parasitism of non-target species in the field. To address this, the realized host range of T. japonicus was assessed over three years by exposing sentinel egg masses of H. halys and 18 non-target species and collecting naturally laid egg masses in Switzerland and Italy. In total, 15 of 18 non-target species were successfully parasitized by T. japonicus in the field, confirming its broad fundamental host range. However, most non-target species were less parasitized by T. japonicus than H. halys, profiting from either partial temporal or spatial refuges from parasitism. Species with an unusual life cycle and the same ecological niche as H. halys, such as Pentatoma rufipes, which was the most parasitized non-target species in both countries, potentially face an increased risk of parasitism. In contrast, beneficial non-target effects may occur for the invasive pest, Nezara viridula, which suffered high non-reproductive mortality induced by T. japonicus. Purpose: To investigate time trends in daily fruit consumption among Western European adolescents and in related socioeconomic inequalities. Methods: We used nationally representative data from 18 countries participating in five rounds (2002 to 2018) of the cross-sectional "Health Behaviour in School-aged Children" (HBSC) survey (n = 458,973). The questionnaire, standardised across countries and rounds, was self-administered at school by 11-, 13- and 15-year-old adolescents. Daily fruit consumption was assessed using a short food frequency questionnaire (sFFQ). Socioeconomic inequalities were measured using the Family Affluence Scale (FAS). Multilevel logistic regressions were applied to study linear time trends in daily fruit consumption, overall, by country and by FAS. Results: Between 2002 and 2018, daily fruit consumption increased in 10 countries (OR range, 1.04 to 1.13, p < 0.05) and decreased in 3 (OR range 0.96 to 0.98, p < 0.05). In all survey years combined, prevalence of daily fruit consumption was significantly higher among high FAS groups (42.6%) compared to medium (36.1%) and low FAS groups (31.7%; all countries: p < 0.001). Between 2002 and 2018, socioeconomic inequalities in fruit consumption increased in Austria, Germany, Italy, Netherlands, Scotland, Sweden, and Switzerland. Only in Norway FAS inequalities decreased while the prevalence increased. Conclusion: The prevalence of daily fruit consumption generally increased among adolescents between 2002 and 2018 in Western European countries, yet socioeconomic inequalities increased in some countries. Swifts (Apodidae) are an unusual group of birds that spend most of their lives in flight, landing only when breeding. Although this aerial lifestyle greatly reduces their likelihood of being bitten by vectors and infected by vector-born parasites, swifts can still be heavily infested during breeding by nest-based vectors such as louse flies (Hippoboscidae). Here, we investigated host, vector, and vector-borne parasite relationships in the three most widespread swift species in the Western Palearctic (WP): common swifts (Apus apus), pallid swifts (A. pallidus), and alpine swifts (Tachymarptis melba), their nest-based louse flies (Crataerina pallida and C. melbae) and avian haemosporidians (genera Haemoproteus, Plasmodium, and Leucocytozoon). Studies of haemosporidian infections in Apodidae remain limited, with clear evidence of infection found to date in just four Neotropical and one Australasian species. The possible role of louse flies in transmitting haemosporidian infections has never been tested in swifts. We assessed the occurrence of haemosporidian infection by PCR screenings of DNA from blood samples from 34 common swifts and 44 pallid swifts from Italy, and 45 alpine swifts from Switzerland. We also screened 20 ectoparasitic louse flies present on 20 birds and identified them by both morphological features and cytochrome oxidase subunit 1 (COI) barcodes. Our results provide no evidence of haemosporidian infection in the 123 swifts tested or in the two louse fly species we identified. Our findings are consistent with available knowledge showing no haemosporidian occurrence in WP swift species and that the most likely infection route for these highly aerial species (via louse fly ectoparasites during nesting) is unlikely. Tremolite asbestos is classified as a carcinogenic substance because it can cause a number of diseases when inhaled. The tremolite breakdown temperature reported in the literature varies by about 280 °C and has not been fully characterized from a thermal perspective. In order to address this gap in the scientific literature, a systematic study of the thermal behavior was conducted of twelve tremolite samples using thermogravimetric and differential scanning calorimetry (TG/DSC) supplemented by PXRD, SEM/EDS, EPMA/EDS and TEM/EDS. Tremolite samples from different locations (i.e., Italy, Switzerland and USA) were selected for their social, health, economic and industrial relevance. Data reveal that the breakdown of tremolite occurs in a temperature range of about 200 °C. The implications of this finding are discussed in light of chemical and morphological data. Variations in the breakdown temperature are mainly ascribed to the morphology of the studied tremolite samples. This study suggests that thermal analysis could be a new method for effectively distinguishing between asbestos and non-asbestos tremolite samples. The thermal oxidation of iron in the various tremolite samples occurs at different temperatures, and hence reduction of tremolite reactivity through the thermal oxidation of iron should be planned on a sample-by-sample basis. Given the different temperatures recorded for tremolite breakdown, the effectively detecting the presence of tremolite in bulk natural samples by thermal analysis can only be carried out if a thermogram of that specific sample is available. Purpose: Antimicrobial resistance poses a major threat to human health globally and antibiotic overuse is a main driver of resistance. Antimicrobial stewardship (AMS) was developed to improve the rationale use of antibiotics. The Choosing Wisely campaign was initiated to ameliorate medical practice through avoidance of unnecessary diagnostic and therapeutic procedures. Our objective was to give an overview on the Choosing Wisely recommendations related to AMS practices from a selection of different countries in order to define future needs. Methods: We evaluated the seven countries already analyzed for Choosing Wisely recommendations related to topics of infectious medicine before. Finally, we included five of the former countries (Australia/New Zealand, Canada, Italy, Switzerland, and USA) and Germany with easily accessible recommendations and selected those related to six categories of AMS as following: diagnostics, indication, choice of antiinfective drugs, dosing, application and duration of therapy. Results: In total, 213 recommendations could be extracted related to AMS for the six countries and were matched to the chosen categories. Interestingly, no recommendations were found for the category "dosing." Topics related to indication and diagnostics were most frequently found with 85 and 78 recommendations, respectively. Perioperative prophylaxis was a frequently addressed issue – both related to application, indication and duration. Avoiding antibiotic treatment of asymptomatic bacteriuria and upper respiratory tract infections were central topics of all countries. Conclusion: AMS is an important strategy to fight increasing resistance and is frequently addressed by Choosing Wisely recommendations of different countries. Since the beginning of the pandemic in 2020, people have been worried about COVID-19. As one of the risk groups, persons aged 65 and older are especially vulnerable. Additionally, minorities and migrants are hit harder by the pandemic than natives. Using data from the TransAge survey, a study including over 3000 older persons (65+) living in Switzerland and Italy, we show that the levels of worry about the pandemic are significantly higher among Italian international migrants living in Switzerland than among Swiss natives. We are not able to fully explain the difference using sociodemographic variables, the COVID-19 situation at the time of the interview, and international migrants' transnationalism behavior. Nevertheless, transnationalism explains a large part of the difference in worry between the two groups and our study sheds light on the importance of two specific transnational aspects, having Swiss nationality and voting behavior, for the prevention of elevated levels of worry of international migrants. Objective: This study aims to analyse the impact of the pandemic on the amount of use and new medication dispensation for chronic diseases in the Italian population aged 65 years and older (almost 14 million inhabitants). Methods: The "Pharmaceutical Prescriptions database", which gathers data on medications, reimbursed by the National Health Service and dispensed by community pharmacies, was employed. Data were analysed as amount of use (defined daily dose—DDD per 1000 inhabitants); variation in DDD between 2020 and 2019 was calculated for the 30 categories with major consumption in 2020. Trends in prevalence and incidence of dispensations between 2020 and 2019 were calculated for four categories: antidiabetics, antihypertensives, antidepressants and drugs for respiratory diseases. Results: All medications showed a negative variation in DDD/1000 inhabitants between 2020 and 2019 except for anticoagulants (+ 5%). The percentage variation ranged from − 27.7% for antibiotics to − 6.4% for antipsychotics in 85 + year-old persons, but increased for most classes in the youngest (65–69 years). On the other hand, a decrease of the dispensation incidence of antidiabetics, antihypertensives, antidepressants and drugs for pulmonary disease was high, especially in the two extreme age groups, the youngest and the oldest one. Conclusions and relevance: Great variation in medication use between 2020 and 2019 was observed probably reflecting the low rate of infectious diseases due to the widespread use of protective devices and self-isolation, reduced healthcare access because of the lockdowns and the fear of going to hospital, and the reduction of screening and diagnostics due to health-care system overload. Dissolved oxygen (DO) concentration is a fundamental metric to describe climate-related alterations in deep lakes. Increasing water temperatures enhance thermal stratification, leading in temperate basins to a growing isolation of deep waters. This leads to the depletion of hypolimnetic DO, which adds up to limited nutrient circulation and restricted replenishment of the trophogenic layers. With vanishing convective mixing, it is commonly believed that the only source of hypolimnetic DO replenishment will be represented by deep intrusions of cold oxygenated waters from the tributaries. In this study, we first analyse the 1993–2020 long-term observed trends of DO concentrations in the subalpine deep oligomictic Lake Maggiore (Italy/Switzerland). Then, through an algorithm calculating daily intrusion depths and mass discharges of DO for the major tributaries, we show that deep insertions are suppressed for increasing winter water temperatures and residual thermal stratification. Turbulent entrainment is proved fundamental for DO replenishment, leading to mass discharges of DO released into the deep hypolimnion up to more than two orders of magnitude larger than the original ones from the tributaries. Last, we discuss the results of simulations made through a one-dimensional coupled ecological–hydrodynamic model about the possible effects of a full turnover on DO concentrations in the deep hypolimnion. Two cases are displayed, with the turnover taking place either now or with an anoxic hypolimnion deriving from decades of isolation due to severe climate warming. Human-induced water level fluctuations (WLFs) are among the major pressures threatening lake ecosystems. Their effect on meiobenthic species of the littoral zone has been poorly investigated. In this study, we aimed at assessing the effects of human-induced WLFs on the composition and functionality of the benthic copepod assemblages of the littoral zone of Lake Maggiore, Italy and Switzerland. From 1942 to present, the water level of Lake Maggiore has been regulated through the Miorina Dam. We monitored copepod assemblages during different water levels defined within the period of regulation by the dam, using taxonomy- and trait-based metrics. We observed variation in the overall abundance and biomass of copepods, as well as in the number of individuals belonging to some trait classes such as ovigerous females, opportunists, omnivores and deposit feeders. None of the investigated trait class was completely lost. The results of our study suggest that ecosystem services supplied by the littoral zone of Lake Maggiore are likely altered by human-induced WLFs. Our findings provide a first picture of WLF impacts on benthic copepod assemblages, which can be useful for future research expanding on other functional traits. [ABSTRACT FROM AUTHOR]

Background: Loneliness and social isolation are associated with anxiety and psychological discomfort, especially amongst the oldest and fragile persons. Aims: SILVER evaluates the acceptance of video calls by old hospitalized patients and their relatives during the ban on visits due to the COVID-19. Moreover, SILVER evaluates if the use of different communication technology is associated with different outcomes in terms of anxiety, fear of self and of others' death and mood. Methods: SILVER is an observational multicentre study. Patients hospitalized in two geriatric units in Switzerland and in one orthogeriatric unit in Italy and their relatives were enrolled. Participants can freely choose to use phone or video calls and were evaluated over a week. We measured anxiety, fear of death and mood at baseline and at the end of the study with standard scales. The use of video or phone calls was associated to a change in these parameters by two-way ANOVA for repeated measures. Results: Sixty-four patients and relatives were enrolled, 26.5% used phone calls and 73.5% video calls. The use of video calls was associated with a reduction in anxiety and fear of death in patients and relatives as compared to participants using phone calls. Discussion: Old patients and their relatives accepted and appreciated the use of video calls during hospitalization; moreover, participant using video calls appears to be less anxious and less afraid of death. Conclusions: Video calls may be a useful communication tool for hospitalized older patients to keep social relationships with relatives and reduce their anxiety and fear of death. Triggering mechanisms and causative processes of deep-seated gravitational slope deformations (DSGSD) in Alpine and high mountain areas include, among others, post-glacial de-buttressing, earthquake-induced ground shaking, or co-seismic surface faulting. Distinguishing between climatic or tectonically driven factors is challenging since faults and fracture systems can play both an active and/or passive role in the process initiation. We applied an integrated approach, including morpho-structural analysis, geologic field survey, a paleoseismological approach applied to trenching, radiocarbon dating and detailed sedimentological analysis, to a DSGSD located in the Cavargna Valley (N Italy), an area that was never occupied by extensive ice tongues in the Upper Pleistocene and Holocene. We were able to document at least two phases of movement from the Middle Holocene onwards and related triggering factors. Thanks to the comparison with dated lacustrine turbidites in the Alpine Lakes, we argue a seismic triggering for the landslide onset, with a possible source located in southern Switzerland, close to the Insubric Line. The later evolution of the landslide was instead driven by climatic predisposing conditions (humid and cold), instead, corresponding to a phase of enhanced slope instability. [ABSTRACT FROM AUTHOR]

The management of emergencies affecting interdependent critical infrastructure (CI) systems is a complex issue of increasing concern. The existence of multiple cascading effects, limited situational awareness, and the need for coordination between several actors are factors that justify the conceptualisation of CI as a complex adaptive system (CAS). Although the capability concept has been extensively adopted in emergency management (EM) literature, proper classification and modelling of CI systems is lacking. This study aims to adopt a capability-based approach for EM to improve the adaptability to the prevailing and unpredictable circumstances, based on a combination of literature review and field research. It proposes a classification and modelling framework for the analysis of the intra- and inter-organisational capabilities using a pilot application involving the Italy–Switzerland cross-border transport infrastructure. This framework is suitable for modelling the EM capabilities under different operational contexts and emergency scenarios. Moreover, it enables the representation and description of a CI system through elementary components that capture the main features of a CAS. Background: Drug survival analysis of biologic agents in psoriasis is of extreme importance, as it allows not only the evaluation of objective clinical outcomes (such as effectiveness and safety) but also of factors that are associated with patients' adherence to treatment. The aim of this study was to evaluate and compare the drug survival of the most recent biologic agents approved for the treatment of moderate-to-severe psoriasis—ustekinumab, secukinumab, ixekizumab, brodalumab, guselkumab, and risankizumab—and to identify clinical predictors that can influence the drug survival of these drugs. Methods: This retrospective multicentric cohort study from 16 dermatology centers in Portugal, Spain, Italy, Switzerland, Czech Republic, Canada, and the United States included patients that started IL-12/23, IL-17 (IL-17A and IL-17R) and IL-23 inhibitors for the treatment of psoriasis between January 1, 2012 and December 31, 2019. Survival analysis was performed using a Kaplan-Meier estimator, to obtain descriptive survival curves, and proportional hazard Cox regression models. Results: A total of 3312 treatment courses (total patients: 3145) were included in the study; 1118 (33.8%) with an IL-12/23 inhibitor (ustekinumab), 1678 (50.7%) with an IL-17 inhibitor [911 (27.5%) on secukinumab, 651 (19.7%) on ixekizumab, 116 (3.5%) on brodalumab], and 516 (15.5%) with an IL-23 inhibitor [398 (12.0%) on guselkumab, 118 (3.5%) on risankizumab]. At 18 months, the cumulative probability of survival was 96.4% for risankizumab, 91.1% for guselkumab, 86.3% for brodalumab, 86.1% for ustekinumab, 82.0% for ixekizumab, and 79.9% for secukinumab. Using ustekinumab as reference, drug survival of guselkumab was higher (HR 0.609; 95% CI 0.418–0.887) and that of secukinumab was lower (HR 1.490; 95% CI 1.257–1.766). In the final multivariable model, secukinumab, female sex, higher BMI, and prior exposure to biologic agents significantly increased the risk of drug discontinuation, whereas risankizumab was protective. Conclusion: In this multinational cohort with 8439 patient-years of follow-up, the cumulative probability of drug survival for all drugs was >79% at 18 months. Prescribed biologic, female sex, higher BMI, and previous exposure to biologic agents were predictors of drug discontinuation. Attempts to restore Lake Lugano, Switzerland and Italy, from eutrophication have produced weak responses in the target variables (primary productivity and hypolimnetic oxygen concentrations), indicating shortcomings in the underlying eutrophication model. An analysis of monitoring data showed that the decrease in phosphorus concentration, although nearly compliant with restoration targets, produced only slight decreases in primary productivity and no change in hypolimnetic oxygen conditions. These target variables were equally or more sensitive to factors external to trophic state, including plankton structure, which influenced primary productivity, and the depth of mixing during turnovers, which influenced hypolimnetic oxygen. To improve the chance of success, the restoration approach should revise the phosphorus concentration target and explicitly account for the influence of external variation, especially mixing depth. In this study we apply a new monitoring technique for butterflies in order to get more insights into the habitat use patterns of Erebia christi Rätzer 1890 (Nymphalidae: Satyrinae). This European endemic is a rare and very localized butterfly species, found alongside steep slopes in a restricted area in the Alps, at the border between Italy and Switzerland. Even though it was discovered almost 140 years ago, captivating the interest of many lepidopterists, there are still several aspects about the biology of E. christi that are not fully known. We selected two monitoring sites in Veglia–Devero Natural Park, Italy and tested whether E. christi was more likely to use rocky, almost vertical slopes than other types of habitat. Since this habitat was almost inaccessible to humans, we used the single rope access technique, in which field operators rappelled down the slopes to explore the rock faces. We demonstrate that the main characteristics of the typical habitat of E. christi are indeed very steep rock faces with grassy ledges where Festuca sp. plants are present. We also show that, E. christi is more abundant than it was previously thought, with populations that could probably be structured in a metapopulation system. Long-term monitoring with the rope access technique could represent an optimal method to provide key insights into the biology and ecology not only of E. christi but of other butterflies that use similar habitats. Finally, we propose the upgrading of its IUCN category from Vulnerable to Endangered at European level. Implications for insect conservation: Our findings demonstrate how appropriate, long-term monitoring can contribute to improve the scarce knowledge of the ecology of an elusive species, and to devise informed proposals regarding the management and protection of populations, especially for species of conservation concern. WHO declared SARS-CoV-2 a global pandemic. The present aim was to propose an hypothesis that there is a potential association between mean levels of vitamin D in various countries with cases and mortality caused by COVID-19. The mean levels of vitamin D for 20 European countries and morbidity and mortality caused by COVID-19 were acquired. Negative correlations between mean levels of vitamin D (average 56 mmol/L, STDEV 10.61) in each country and the number of COVID-19 cases/1 M (mean 295.95, STDEV 298.7, and mortality/1 M (mean 5.96, STDEV 15.13) were observed. Vitamin D levels are severely low in the aging population especially in Spain, Italy and Switzerland. This is also the most vulnerable group of the population in relation to COVID-19. It should be advisable to perform dedicated studies about vitamin D levels in COVID-19 patients with different degrees of disease severity. [ABSTRACT FROM AUTHOR]

Dryocosmus kuriphilus has become a major threat to Castanea sativa and chestnut cultivation in Europe since its discovery in Italy in 2002. Thankfully, the biological control agent Torymus sinensis has proven to effectively control and reduce the D. kuriphilus population. The reaction of chestnut trees to decreasing D. kuriphilus pressure, however, has been sparsely investigated. In this study, we analyze the recovery patterns of chestnut trees since the pest epidemic peak in 15 sites in southern Switzerland that were differently affected by the dephased arrival of D. kuriphilus and T. sinensis. By using various tree damage and insect population indicators, we show how damage varies as a function of the time elapsed between pest arrival and biological control by T. sinensis. Specifically, trees in sites experiencing seven years of D. kuriphilus uncontrolled attacks show large dying crown portions and stress-induced reactions (e.g., suckers along the stem) compared to sites where the two insects arrived nearly simultaneously. Moreover, in warm and south-exposed sites, the D. kuriphilus population remains significant and damage on chestnut trees still persists suggesting a desynchronisation in the phenology of the antagonist with respect to the pest. The brown marmorated stink bug, Halyomorpha halys, native to East Asia, is an invasive alien pest that arrived in Europe in the early 2000s and poses an imminent threat to a wide variety of crops. Adventive populations of the Asian egg parasitoid Trissolcus japonicus, the most promising agent for classical biological control of H. halys, have recently been detected in Italy and Switzerland. Its prospective fundamental host range in Europe was evaluated in behavioural no-choice tests, followed by large-arena choice tests presenting host plants with naturally laid egg masses of target and non-target hosts. Developmental suitability of European non-target host species for T. japonicus was demonstrated, via no-choice tests, by offspring emergence (successful parasitism) from eleven out of thirteen non-target species tested (85%). Whereas successful parasitism of most non-target species was significantly lower, acceptance of Arma custos, Palomena prasina, Pentatoma rufipes, and Rhaphigaster nebulosa was not significantly different from H. halys controls. When eggs of H. halys and non-target species were exposed in a semi-natural situation in large-arena choice tests, the degree of non-target parasitism was substantially reduced for three out of four tested species, whereas parasitism of Pa. prasina eggs was not. It remains unclear if there are behavioural barriers to parasitism that may exist and preclude excessive parasitism of potentially threatened species in the field, but field data from the invaded areas in Switzerland and Italy could contribute to a risk–benefit evaluation of releasing or re-locating adventive T. japonicus populations into other parts of Europe. Purpose: Little is known about the quality of life following pulmonary embolism (PE). The aim of the study was to assess the 12-month illness burden in terms of health-related quality of life (HrQoL) and mortality, in relation to differences in patient characteristics.Methods: The PREFER in VTE registry, a prospective, observational study conducted in seven European countries, was used. Within 2 weeks following an acute symptomatic PE, patients were recruited and followed up for 12 months. Associations between patient characteristics and HrQoL (EQ-5D-5L) and mortality were examined using a regression approach.Results: Among 1399 PE patients, the EQ-5D-5L index score at baseline was 0.712 (SD 0.265), which among survivors gradually improved to 0.835 (0.212) at 12 months. For those patients with and without active cancer, the average index score at baseline was 0.658 (0.275) and 0.717 (0.264), respectively. Age and previous stroke were significant factors for predicting index scores in those with/without active cancer. Bleeding events but not recurrences had a noticeable impact on the HrQoL of patients without active cancer. The 12-month mortality rate post-acute period was 8.1%, ranging from 1.4% in Germany, Switzerland, and Austria to 16.8% in Italy. Mortality differed between patients with active cancer and those without (42.7% vs. 4.7%).Conclusion: PE is associated with a substantial decrease in HrQoL at baseline which normalizes following treatment. The FATF requires each country to undertake a national risk assessment (NRA) to show the government's knowledge of money laundering risks. There is little guidance as to how these NRAs are to be conducted, and those that have been published show great variation in terms of data used, analytical methods, and the depth of policy analysis. After expounding some of the concepts basic to any risk analysis, we analyze two of the more detailed published NRAs, from Italy and Switzerland. The Italian NRA, focused on domestic criminal threats, relies almost exclusively on expert opinion. Its most distinctive product is an analysis of the high-threat sectors and the need for specific kinds of policy interventions. The Swiss NRA, focused primarily on threats from other countries, presents far more quantitative data, almost exclusively from suspicious activity reports, to supplement expert opinion. Though both NRAs provide useful insights about money laundering risks, neither is conceptually clear; in particular, neither reflects contemporary practice in the use of expert opinion. Our critique is aimed at helping strengthen the next round of NRAs, and identifies lessons learned for all countries. Our recommendations include the use of risk assessment standards from other fields, the addition of a measure of uncertainty, and a more critical assessment by FATF in its NRA evaluations. Background: The European Randomised Study of Prostate Cancer Screening has shown a 21% relative reduction in prostate cancer mortality at 13 years. The causes of death can be misattributed, particularly in elderly men with multiple comorbidities, and therefore accurate assessment of the underlying cause of death is crucial for valid results. To address potential unreliability of end-point assessment, and its possible impact on mortality results, we analysed the study outcome adjudication data in six countries.Methods: Latent class statistical models were formulated to compare the accuracy of individual adjudicators, and to assess whether accuracy differed between the trial arms. We used the model to assess whether correcting for adjudication inaccuracies might modify the study results.Results: There was some heterogeneity in adjudication accuracy of causes of death, but no consistent differential accuracy by trial arm. Correcting the estimated screening effect for misclassification did not alter the estimated mortality effect of screening.Conclusions: Our findings were consistent with earlier reports on the European screening trial. Observer variation, while demonstrably present, is unlikely to have materially biased the main study results. We measured methane concentrations in the surface water of the northern basin of Lake Lugano in spring (May 2012) and autumn (October 2011, 2012), and calculated turbulent diffusive methane fluxes to the atmosphere. Surface water methane concentrations were highly variable in space and time but always exceeded atmospheric equilibrium. Methane concentrations were significantly lower in spring (on average 16 nmol L) than during the autumn sampling campaigns (on average 57 nmol L in 2011 and 45 nmol L in 2012). This suggests methane accumulation in the surface mixed layer during the summer productive season. The origin of the methane in the lake's surface waters requires further assessment, but the observed concentration profiles indicate that the excess methane originates from a near-surface source, rather than from the large deep-water methane pool in the anoxic monimolimnion. As a consequence of the higher surface water methane concentrations and increased buoyancy turbulence caused by autumnal cooling of the surface boundary layer, diffusive fluxes were much higher in October (average ~97 μmol m day, compared to 7 μmol m day in May 2012). The increase in methane concentration in the surface water between spring and autumn suggests links between methane accumulation and the annual biological cycle, yet seasonal changes in wind and temperature forcing of methane emission likely play an important modulating role. While the relative importance of biological versus physical controls on methane emission in Lake Lugano awaits further investigations, our study underscores that lakes can act as an important source of methane to the atmosphere, even when the lake-internal microbial methane filter in the water column seems to work efficiently. From June 1995 to August 2012, 18 children were treated surgically using elastic stable intramedullary nails for displaced closed fractures of the distal metaphysis of the tibia. The patients were followed radiographically and clinically on a regular basis until union was clinically and radiographically achieved. Thirteen boys and five girls with a mean age at trauma of 11 ± 2.9 years (range 6-15) were included in the study. Radiographically, all fractures healed without evidence of delayed union, re-fracture, hardware migration. Purpose: Obesity represents a growing public health concern worldwide. The latest data in Switzerland rely on self-reported body mass index (BMI), leading to underestimation of prevalence. We reassessed the prevalence of obesity and overweight in a sample of the Swiss population using measured BMI and waist circumference (WC) and explored the association with nutritional factors and living in different linguistic-cultural regions. Methods: Data of 1,505 participants of a cross-sectional population-based survey in the three linguistic regions of Switzerland were analyzed. BMI and WC were measured, and a 24-h urine collection was performed to evaluate dietary sodium, potassium and protein intake. Results: The prevalence of overweight, obesity and abdominal obesity was 32.2, 14.2 and 33.6 %, respectively. Significant differences were observed in the regional distribution, with a lower prevalence in the Italian-speaking population. Low educational level, current smoking, scarce physical activity and being migrant were associated with an higher prevalence of obesity. Sodium, potassium and protein intake increased significantly across BMI categories. Conclusions: Obesity and overweight affect almost half of the Swiss adolescents and adults, and the prevalence appears to increase. Using BMI and WC to define obesity led to different prevalences. Differences were furthermore observed across Swiss linguistic-cultural regions, despite a common socio-economic and governmental framework. This paper presents a case study of a railway timetable optimization for the very dense Simplon corridor, a major railway connection in the Alps between Switzerland and Italy. The key to deal with the complexity of this scenario is the use of a novel aggregation-disaggregation method. Starting from a detailed microscopic representation as it is used in railway simulation, the data is transformed by an automatic procedure into a less detailed macroscopic representation, that is sufficient for the purpose of capacity planning and amenable to state-of-the-art integer programming optimization methods. This macroscopic railway network is saturated with trains. Finally, the optimized timetable is re-transformed to the microscopic level in such a way that it can be operated without any conflicts among the train paths. Using this micro-macro aggregation-disaggregation approach in combination with integer programming methods, it becomes for the first time possible to generate a profit maximal and conflict free timetable for the complete Simplon corridor over an entire day by a simultaneous optimization of all trains requests. Diatoms have been often used to track trophic changes from sedimentary records: recent studies demonstrated that these organisms can even be valuable indicators of climatic variability, although it is often difficult to discriminate the role of trophic and climatic drivers. Moving from the hypothesis that oligotrophication and climate affected the composition of the diatom assemblages by changing the resource ratio, we analysed the vernal diatoms succession in Lake Maggiore, between 1984 and 2007, using multivariate techniques (cluster analysis, canonical correspondence analysis, multivariate regression trees), in order to single out the oligotrophication effects from those attributable to climatic variability. Our results point out that Si, TP, temperature and wind emerged as key explanatory variables in species selection, with a stronger link between trophic and climatic drivers after the lake reached a stable oligotrophic status. Peculiar climate-driven events (deep mixing and floods) affected the in-lake Si:P ratio, giving an advantage to diatoms that are excellent P, but poor Si competitors. The classical role of Fragilaria and Tabellaria as early-warning indicators of eutrophication should be reconsidered, taking into account that both can be useful indicators of climate change, when links between their physiological resource needs and environmental data coming from robust limnological investigations can be established. Fires shape fundamental properties of many forest ecosystems and climate change will increase their relevance in regions where fires occur infrequently today. In ecosystems that are not adapted to fire, post-fire tree recruitment is often sparse, a fact that might be attributed to a transient lack of mycorrhizae. Ectomycorrhizal (EcM) fungi play an important role for recruitment by enhancing nutrient and water uptake of their hosts. The questions arise whether and for how long the EcM community is transformed by fire. We investigated the resistance and resilience of EcM fungal communities on a chronosequence of 12 Pinus sylvestris stands in Valais (Switzerland) and Val d'Aosta (Italy) affected by fire between 1990 and 2006. Soil samples from burnt and non-burnt forests were analyzed with respect to EcM fungi by means of a bioassay. The number of EcM species was significantly lower in samples from recently (2-5 years) burnt sites than non-burnt forest, and increased with time since fire reaching levels of adjacent forests after 15-18 years. Community composition changed after fire but did not converge to that of non-burnt sites over the 18 year period. Only Rhizopogon roseolus and Cenococcum geophilum were abundant in both burnt sites and adjacent forest. Our data indicate fire resistance of some EcM fungal species as well as rapid resilience in terms of species number, but not in species composition. As long as the function of different EcM species for seedling establishment is unknown, the consequences of long-term shifts in EcM community composition for tree recruitment remain unclear. The Valais units in Savoy (Zone des Brèches de Tarentaise) have been re-mapped in great detail and are subject of combined stratigraphic, structural and petrological investigations summarized in this contribution. The sediments and rare relics of basement, together with Cretaceous age mafic and ultramafic rocks of the Valais palaeogeographical domain, represent the heavily deformed relics of the former distal European margin (External Valais units) and an ocean-continent transition (Internal Valais unit or Versoyen unit) that formed during rifting. This rifting led to the opening of the Valais ocean, a northern branch of the Alpine Tethys. Post-rift sediments referred to as 'Valais trilogy' stratigraphically overlie both External and Internal Valais successions above an angular unconformity formed in Barremian to Aptian times, providing robust evidence for the timing of the opening of the Valais ocean. The Valais units in Savoy are part of a second and more external mid-Eocene high-pressure belt in the Alps that sutured the Briançonnais microcontinent to Europe. Top-N D1-deformation led to the formation of a nappe stack that emplaced the largely eclogite-facies Internal Valais unit (Versoyen) onto blueschist-facies External Valais units. The latter originally consisted of, from internal to external, the Petit St. Bernard unit, the Roc de l'Enfer unit, the Moûtiers unit and the Quermoz unit. Ongoing top-N D2-thrusting and folding substantially modified this nappe stack. Post 35 Ma D3 folding led to relatively minor modifications of the nappe stack within the Valais units but was associated with substantial top-WNW thrusting of the Valais units over the Dauphinois units along the Roselend thrust during W-directed indentation of the Adria block contributing to the formation of the arc of the Western Alps. The Austroalpine nappe systems in SE-Switzerland and N-Italy preserve remnants of the Adriatic rifted margin. Based on new maps and cross-sections, we suggest that the complex structure of the Campo, Grosina/Languard, and Bernina nappes is inherited largely from Jurassic rifting. We propose a classification of the Austroalpine domain into Upper, Middle and Lower Austroalpine nappes that is new because it is based primarily on the rift-related Jurassic structure and paleogeography of these nappes. Based on the Alpine structures and pre-Alpine, rift-related geometry of the Lower (Bernina) and Middle (Campo, Grosina/Languard) Austroalpine nappes, we restore these nappes to their original positions along the former margin, as a means of understanding the formation and emplacement of the nappes during initial reactivation of the Alpine Tethyan margin. The Campo and Grosina/Languard nappes can be interpreted as remnants of a former necking zone that comprised pre-rift upper and middle crust. These nappes were juxtaposed with the Mesozoic cover of the Bernina nappe during Jurassic rifting. We find evidence for low-angle detachment faults and extensional allochthons in the Bernina nappe similar to those previously described in the Err nappe and explain their role during subsequent reactivation. Our observations reveal a strong control of rift-related structures during the subsequent Alpine reactivation on all scales of the former distal margin. Two zones of intense deformation, referred to as the Albula-Zebru and Lunghin-Mortirolo movement zones, have been reactivated during Alpine deformation and cannot be described as simple monophase faults or shear zones. We propose a tectonic model for the Austroalpine nappe systems that link inherited, rift-related structures with present-day Alpine structures. In conclusion, we believe that apart from the direct regional implications, the results of this paper are of general interest in understanding the control of rift structures during reactivation of distal-rifted margins. This study monitors regional changes in the crystallinity of carbonaceous matter (CM) by applying Micro-Raman spectroscopy to a total of 214 metasediment samples (largely so-called Bündnerschiefer) dominantly metamorphosed under blueschist- to amphibolite-facies conditions. They were collected within the northeastern margin of the Lepontine dome and easterly adjacent areas of the Swiss Central Alps. Three-dimensional mapping of isotemperature contours in map and profile views shows that the isotemperature contours associated with the Miocene Barrow-type Lepontine metamorphic event cut across refolded nappe contacts, both along and across strike within the northeastern margin of the Lepontine dome and adjacent areas. Further to the northeast, the isotemperature contours reflect temperatures reached during the Late Eocene subduction-related blueschist-facies event and/or during subsequent near-isothermal decompression; these contours appear folded by younger, large-scale post-nappe-stacking folds. A substantial jump in the recorded maximum temperatures across the tectonic contact between the frontal Adula nappe complex and surrounding metasediments indicates that this contact accommodated differential tectonic movement of the Adula nappe with respect to the enveloping Bündnerschiefer after maximum temperatures were reached within the northern Adula nappe, i.e. after Late Eocene time. (U-Th)/He and fission-track analyses of apatite along deep-seated tunnels crossing high-relief mountain ranges offer the opportunity to investigate climate-tectonic forcing on topographic evolution. In this study, the thermochronologic analysis along the Simplon tunnel (western-central Alps; Italy and Switzerland) constrains in detail the mechanisms controlling the topographic evolution of the Simplon Massif. Cooling rates vary from about 10°C/Ma at about 10 Ma to about 35°C/Ma in the last 3 Ma. Such increase in cooling rates corresponds to the inception of glacial cycles in the northern hemisphere. Age patterns show correlation with faults distribution until 2 Ma, suggesting that tectonics-controlled rocks exhumed up to that age. After 2 Ma thermo-chronometric data show that the Simplon area has experienced primarily erosional exhumation. All age patterns provided are not affected by topographic effects, thus indicating that present-day topography has been carved in the last 2 Ma, most likely controlled by glacial erosion. Background: Corticosteroids are a versatile option for the treatment of mild-to-moderate psoriasis due to their availability in a wide range of potencies and formulations. Occlusion of the corticosteroid is a widely accepted procedure to enhance the penetration of the medication, thereby improving its effectiveness. Betamethasone valerate (BMV) is a moderately potent corticosteroid that is available as a cream, ointment, and lotion. A ready-to-use occlusive dressing, which provides a continuous sustained release of BMV, has been developed for the treatment of psoriasis. Objective: To evaluate the efficacy and safety of a new BMV 0.1% plaster compared with a BMV 0.1% cream in patients with mild-to-moderate chronic plaque psoriasis. Methods: This was a prospective, randomized, assessor-blind, parallel-group, active-controlled, multicenter, phase III study. Eligible outpatients (aged ≥18 years) with a diagnosis of stable, chronic plaque psoriasis vulgaris with two to four plaques on extensor surfaces of limbs were randomized to receive BMV 0.1% plaster or BMV 0.1% cream for 3-5 weeks; patients with resolution of target plaques then entered a 3-month, treatment-free, follow-up period. The number of patients showing clearing of plaques (remission) at 3 weeks (primary endpoint) and at 5 weeks was independently evaluated from digitized images of target plaques by two blinded assessors, and also assessed by the investigator and patient. Additional endpoints were (i) change from baseline in target plaque size and in Psoriasis Global Assessment (PGA) score, as evaluated by the blinded assessors, investigator, and patient; (ii) change from baseline in symptom (itching, soreness) severity; (iii) treatment satisfaction and ease of use; (iv) clearing and relapse during the follow-up period; and (v) adverse events (AEs). Results: Patients (n = 231) were screened and randomized to treatment with BMV 0.1% plaster (n = 116) and BMV 0.1% cream (n = 115). Significantly more patients achieved clearing after 3 weeks' treatment with BMV plaster than with BMV cream (Cochran-Mantel-Haenszel test, p <0.001); this difference was maintained at 5 weeks. The total plaque area decreased to a larger extent for the BMV plaster group compared with the BMV cream group (analysis of covariance [ANCOVA] model, p=0.01'7 at week 5). PGA scores were significantly lower after 3 and 5 weeks' treatment with BMV plaster (ANCOVA model, all p ≤ 0.016 vs BMV cream). Both treatments reduced itching and soreness to a similar degree, and the incidences of relapse during the follow-up period were comparable between treatment groups. There were no significant differences in AEs between treatment groups. Conclusions: BMV 0.1% plaster is more efficacious than BMV 0.1% cream in the treatment of patients with mild-to-moderate chronic plaque psoriasis in a clinical setting resembling daily clinical practice. Although a lot of research has been conducted on the delinquency of boys who are members of gangs, only a few quantitative studies have analysed the involvement of girl gang members in delinquency and its link with victimisation. In this study, the prevalence rates of girls who are members of gangs in Italy and in Switzerland are shown. We compared the Italian data (N = 5.784) and Swiss data (N = 3.459) from the second wave of the International Self-Reported Delinquency Study (ISRD-2); the population used for this comparison was made up of teenagers from the ages of 13 to 16. Members of deviant youth groups accounted for 5.7% of the Italian sample and 4.7% of the Swiss sample; in both countries, about a third of gang members were girls. In general, girls who are members of gangs commit more delinquent acts than both girls and boys who are not members of gangs. In 1996, dichlorodiphenyltrichloroethane (DDT) pollution of industrial origin was discovered in Lake Maggiore. It was caused by industrial effluents on a tributary of the River Toce, one of the major affluents of the lake in correspondence of Pallanza Bay. This event is the worst case of environmental pollution that has occurred in Western countries in the last 25 years, not due to agricultural use of DDT, but because of an accidental industrial discharge. Heavy polychlorinated biphenyls (PCBs) pollution was also noticed in 2002, with concentration levels three to seven times higher than those measured in other Italian subalpine lakes. In this study, the current DDT and PCBs contamination levels were assessed according to their presence in zebra mussel ( Dreissena polymorpha) specimens sampled in the last 5 years (2003–2008) in eight sampling stations of Lake Maggiore, chosen to cover the entire perimeter of the basin. Moreover, for two stations (Baveno and Pallanza-Villa Taranto) located inside and outside Pallanza Bay, respectively, it is possible to make comparisons starting from 1996. The results obtained show how Lake Maggiore is still an ecosystem with a severe environmental risk, more than 10 years after the original insecticide discharge. DDT contamination continues to evolve, and natural events, like lake overturn, floods, and heavy rains, can have a great influence on the insecticide levels in the lake. By contrast, PCB contamination is absolutely negligible, even if the peak of pollution revealed in 2002 seems to indicate that these pollutants are still present in large quantities in the Lake Maggiore watershed. Fish and piscivorous bird eggs collected in 2003 from Lake Maggiore (Italy), were analysed for PCB and DDT contamination. Lake Maggiore has been severely polluted by DDT through production of the pesticide within its catchment. Although agricultural application of DDT was banned in Italy in the 1978, industrial production continued until 1996, with enough contamination of water and soil for serious bioaccumulation in the lake biota. PCB and DDT concentrations in a whitefish ( Coregonus macrophthalmus Nusslin 1882) were seen to be dependent on season and fish age, but not on sex. The average increase of the lipid-normalised concentration of DDTs and PCBs was two-fold across season and also across age, resulting in an overall increase of four fold. The seasonal variation was related to the eco-physiological cycle of the fish and to the contamination dynamic of the lake, while the effect of the fish age was explained on the base of biomagnification-related mechanisms. A fugacity model was applied to predict the age-dependent bioaccumulation potential of PCBs, whose concentrations were rather stable in recent years in the lake. Predicted values for compounds with negligible biotransformation were in good agreement with experimental data (calculated vs. experimental mean difference of 14%), and a relationship between the increase of experimental age-dependent concentration and K ow was observed. The good correspondence between the predicted and the measured values for most PCB congeners confirmed the general inability of fishes to biotransform these compounds. On the contrary, the importance of biotransformation processes was recognised in birds; eggs of a fish eating bird ( Podiceps cristatus) from the same area selectively bioaccumulated p, p′-DDE. For PCBs, congener 149 appears to be completely metabolized by the bird species, and congeners 95, 101, 132, 151 and 174 were reduced as well. The article presents information on the development of a dynamic aquatic model (DynA Model). DynA Model describes the fate of a chemical in a dynamic scenario. The model was applied to Lake Maggiore emitting DDT at a constant rate until steady-state was reached. Emissions were stopped and later sporadic pulse emissions were added. This was done to calculate the amount of DDT needed to simulate concentrations close to those measured in water and sediments. The model showed the time response of the Lake Maggiore system to varying emission scenarios and provided the reasonable estimates of DDT emissions.

The article presents information on the accumulation of pp'DDT and pp'DDE in Lake Maggiore. In 1998, a heavy DDT and DDE concentrations in some fish species was found. Five water sampling campaigns were carried out from May 2002 to February 2004 at three sampling sites of Lake Maggiore. Connolly & Pedersen proposed a bioenergetic model which was used to assess the bioaccumulation of pp'DDT and pp'DDE in fishes selected among the different species as representative of a secondary consumer level. The average concentrations of pp'DDT and pp'DDE in water to be used as input data in the bioenergetic model were 0.05 and 0.16 ng/L respectively.

Impaired fibrinolysis and massive elevation of the plasminogen activator inhibitor-1 (PAI-1) is a characteristic feature of meningococcal sepsis. Previously, an association between mortality and the functional 4G/5G promoter polymorphism of the PAI-1gene in a cohort of UK and Dutch children with meningococcal sepsis was reported. We carried out a prospective, multicentre study to investigate the association of the 4G/5G PAI-1 polymorphism, diagnosis, and outcome in meningococcal disease in a Central European and UK population. Blood samples and clinical information of 347 previously healthy children with meningococcal infection were collected from 95 paediatric hospitals in Germany, Switzerland, Italy, the United Kingdom, and Austria from 2000 until 2002. Mortality was significantly associated with the 4G/4G genotype (12 of 90 (13%) vs. 15 of 240 (6%), P = 0.037), resulting in an odds ratio of 2.31. The diagnosis of sepsis (independent of symptoms of meningitis) was significantly more frequent in carriers of the 4G/4G genotype (P = 0.01), resulting in an odds ratio of 2.21 to develop sepsis. Meningitis was not associated with the PAI-1 4G/5G polymorphism, and allele frequencies were similar in patient and control groups.Conclusion: Our data show a correlation between the 4G/4G genotype in the plasminogen activator inhibitor-1 gene and poor outcome in children with meningococcal infection. In addition, 4G homozygous patients were prone to develop sepsis. We investigated the role of fried foods on oral-pharyngeal and oesophageal cancers, using data from two case-control studies conducted in Italy and Switzerland between 1992 and 1999, one with a total of 749 (634 men) cases of oral/pharyngeal cancer and 1772 (1252 men) controls, the other with 395 (351 men) cases of oesophageal cancer and 1066 (875 men) controls. Controls were admitted for acute, non-neoplastic conditions, unrelated to alcohol and smoking consumption. After allowance for sex, age, centre, education, body mass index, tobacco smoking, alcohol drinking and nonalcohol energy intake, the multivariate odds ratios (ORs) for an increment of one portion per week of total fried foods were 1.11 (95% confidence interval (CI): 1.05-1.17) for oral-pharyngeal and 1.16 (95% CI: 1.08-1.26) for oesophageal cancer. Contamination by DDT of industrial origin was detected in 1996 in Lake Maggiore (Northern Italy) causing concern for wildlife and human health. Starting from 1998 a monitoring programme involving different research laboratories was undertaken to establish the pollution level of the lake. In assessing the ecological risk to Lake Maggiore wildlife during this period, the present study integrates information provided by the Commission for the Protection of Italian and Swiss Waters with further analysis and ecotoxicological tests carried out on some autochthonous target species for this insecticide. Sediments collected at the end of 1997 in the most contaminated bay (total DDT 860.2 ng g-1 dry weight) caused a decrease in fertility and growth of benthic oligochaetes and chironomids, while the 48 h IC50 forDaphnia galeata(0.76 µg L-1, c.i. 0.43-1.34) was much higher than the DDT concentration in the lake water. The risk of acute effects on zooplankton should therefore be excluded, while transfer through the trophic web and bioconcentration in fish and fish-eating birds are the most adverse consequences of DDT contamination in Lake Maggiore. Objective: To investigate the relation between various micronutrients and laryngeal cancer risk.Methods: A case-control study was conducted in Italy and Switzerland between 1992 and 2000. Cases were 527 patients with incident cancer of larynx, admitted to the major teaching and general hospitals of the study areas. Controls were 1297 subjects admitted for acute, non-neoplastic diseases to the same network of hospitals. Dietary habits were assessed using a validated food-frequency questionnaire. Odds ratios (OR) and their corresponding 95% confidence intervals (CI) were computed using multiple logistic regression.Results: Significant inverse relations emerged between laryngeal cancer risk and intake of vitamin C (OR = 0.2, for the highest versus the lowest intake quintile; 95% CI: 0.2-0.4), beta-carotene (OR = 0.2; 95% CI: 0.2-0.4), alpha-carotene (OR = 0.3; 95% CI: 0.2-0.5), lutein/zeaxanthin (OR = 0.4; 95% CI: 0.3-0.6), vitamin E (OR = 0.4; 95% CI: 0.3-0.6), beta-criptoxanthin (OR = 0.4; 95% CI: 0.2-0.5), folic acid (OR = 0.4; 95% CI: 0.2-0.6), thiamin (OR = 0.4; 95% CI: 0.3-0.6), glutathione (OR = 0.5; 95% CI: 0.4-0.8), reduced glutathione (OR = 0.6; 95% CI: 0.4-0.8), vitamin B6 (OR = 0.6; 95% CI: 0.4-0.9) and potassium (OR = 0.6; 95% CI: 0.4-0.9). Direct associations were found with zinc (OR = 1.5; 95% CI: 1.0-2.2) and vitamin D (OR = 1.8; 95% CI: 1.2-2.6). Combining low intakes of vitamin C, carotene, vitamin E, and folate with heavy smoking and drinking led to ORs between 80 and 170.Conclusions: This study provides further support that, independently from smoking and alcohol consumption, the intake of several micronutrients, including selected antioxidants, is inversely related to laryngeal cancer risk. Subjective well-being is a major aspect of quality of life and is therefore increasingly used as an endpoint in clinical trials. It is influenced to a great extent by the complex process of coping with the disease and its treatment. Assessment of coping is methodologically demanding, especially in large clinical trials. We therefore developed a single-item measure, the Perceived Adjustment to Chronic Illness Scale (PACIS), as an indicator of coping, complementary to other scales related to quality of life. We sought to validate this instrument in a subgroup of 121 Swiss patients participating in the International Breast Cancer Study Group (IBCSG) adjuvant trials. At months 3 and 6 of adjuvant treatment PACIS showed a distinct pattern of highly significant rank correlations with several disease- and treatment-related problem areas from the Herschbach coping inventory (FBBK); 42% of the variance of PACIS at month 3 was explained by the FBBK (P = .0001). The portion of explained variance was considerably higher for the Italian- (70%) than for the German-speaking (30%) subgroups. Patients who rated more effort to cope with their disease on PACIS indicated more frequent use of 3 of 15 coping strategies in relation to psychological distress. These were "crying and becoming desperate", "taking tranquillizers and alcohol" and "other people are far worse off". These three coping strategies may define a high-risk group for poor psychosocial outcome. Patients whose PACIS scores showed that it required less effort to cope tended to use the strategy "seeing a positive side of the problem". We conclude that PACIS can be used as a global indicator of the coping process in large multicultural clinical trials of adjuvant therapy for breast cancer. [ABSTRACT FROM AUTHOR]

Human impacts on biodiversity are well recognized, but uncertainties remain regarding patterns of diversity change at different spatial and temporal scales. Changes in microbial assemblages are, in particular, not well understood, partly due to the lack of community composition data over relevant scales of space and time. Here, we investigate biodiversity patterns in cyanobacterial assemblages over one century of eutrophication and climate change by sequencing DNA preserved in the sediments of ten European peri-Alpine lakes. We found species losses and gains at the lake scale, while species richness increased at the regional scale over approximately the past 100 years. Our data show a clear signal for beta diversity loss, with the composition and phylogenetic structure of assemblages becoming more similar across sites in the most recent decades, as have the general environmental conditions in and around the lakes. We attribute patterns of change in community composition to raised temperatures affecting the strength of the thermal stratification and, as a consequence, nutrient fluctuations, which favoured cyanobacterial taxa able to regulate buoyancy. Our results reinforce previous reports of human-induced homogenization of natural communities and reveal how potentially toxic and bloom-forming cyanobacteria have widened their geographic distribution in the European temperate region.
