Introduction Since 2001, the Health Resources and Services Administration’s Maternal and Child Health Bureau (HRSA MCHB) has funded and directed the National Survey of Children’s Health (NSCH) and the National Survey of Children with Special Health Care Needs (NS-CSHCN), unique sources of national and state-level data on child health and health care. Between 2012 and 2015, HRSA MCHB redesigned the surveys, combining content into a single survey, and shifting from a periodic interviewer-assisted telephone survey to an annual self-administered web/paper-based survey utilizing an address-based sampling frame. Methods The U.S. Census Bureau fielded the redesigned NSCH using a random sample of addresses drawn from the Census Master Address File, supplemented with a unique administrative flag to identify households most likely to include children. Data were collected June 2016-February 2017 using a multi-mode design, encouraging web-based responses while allowing for paper mail-in responses. A parent/caregiver knowledgeable about the child’s health completed an age-appropriate questionnaire. Experiments on incentives, branding, and contact strategies were conducted. Results Data were released in September 2017. The final sample size was 50,212 children; the overall weighted response rate was 40.7%. Comparison of 2016 estimates to those from previous survey iterations are not appropriate due to sampling and mode changes. Discussion The NSCH remains an invaluable data source for key measures of child health and attendant health care system, family, and community factors. Previous research has indicated that healthcare support for sexuality after spinal cord injury appears to be neglected or inadequately provided. The perspectives of people living with spinal cord injuries regarding what is lacking and what support they would like to be provided, and why, has yet to be explored. This paper uses critical theory to explore how societal constructs of sexuality and disability shape how sexuality is supported after a spinal cord injury. To do this, we conducted in-depth semi-structured interviews with 11 people with a spinal cord injury from Australia, the United Kingdom, and the United States. We drew from the works of various critical disability theorists to analyze what discourses underpinned the experiences people with spinal cord injuries had when receiving sexuality support. Analysis suggested that the following discourses were apparent: (1) broken bodies require repair, (2) sex is penetration, ejaculation then reproduction, and (3) sexual autonomy is required. Findings indicated healthcare professionals need to recognize people with spinal cord injuries as sexual beings with a diverse range of desires, lives, relations, and bodies. Provision of individualized comprehensive high-quality sexuality support is therefore warranted. This paper reports the results of a randomized controlled trial (RCT) to assess the efficacy of Nexus, a telehealth delivered intervention that combines Couples' HIV counseling and testing (CHTC) with home-based HIV-testing, examining the impact of the intervention on the couples' formation and adherence to safer sexual agreements. Between 2016 and 2018, 424 couples were recruited online from the U.S and randomized to the intervention arm (a telehealth delivered CHTC session with two home HIV-testing kits) or a control arm (two home HIV-testing kits), with study assessments at baseline, 3 and 6 months. Outcomes were the formation and adherence to safer sexual agreements, dyadic discordance in sexual agreements, breakage of sexual agreements, and perceptions of PrEP. Couples in the intervention arm had significantly greater odds of reporting a safer sexual agreement (3 months OR 1.87, p-value 0.005, and 6 months OR 1.84, p-value 0.007), lower odds of reporting discordant sexual agreements at 6 months (OR 0.62, p-value 0.048), and a significantly lower odds of reporting breaking their sexual agreement (3 months OR 0.51, p-value 0.035, and 6 months OR 0.23, p-value 0.000). By 6 months, couples in the intervention arm were less likely to say PrEP was beneficial to one (RRR 0.33, P = 0.000) or both of them (RRR 0.29, P = 0.000) than being beneficial to neither of the partners. The high levels of acceptability and efficacy of the intervention demonstrate strong potential for the scale-up of this efficacious intervention that is delivered through a low-cost telehealth platform. Objectives: Population-based recruitment of a cohort of women who are currently pregnant or who may become pregnant in a given timeframe presents challenges unique to identifying pregnancy status or the likelihood of future pregnancy. Little is known about the performance of individual eligibility items on pregnancy screeners although they are critical to participant recruitment. This paper examined the patterns and respondent characteristics of key pregnancy screener items used in a large national study.Methods: Cross-sectional analyses were conducted. Descriptive statistics and multivariable logistic regression models were used to examine nonresponse patterns to three questions (currently pregnant, trying to get pregnant and able to get pregnant). The questions were asked of 50,529 women in 17 locations across the US, as part of eligibility screening for the National Children's Study Vanguard Study household-based recruitment.Results: Most respondents were willing to provide information about current pregnancy, trying, and able to get pregnant: 99.3% of respondents answered all three questions and 97.4% provided meaningful answers. Nonresponse ranged from 0.3 to 2.5% for individual items. Multivariable logistic regression results identified small but statistically significant differences in nonresponse by respondent age, marital status, race/ethnicity-language, and household-based recruitment group.Conclusions for Practice: The high levels of response to pregnancy-related items are impressive considering that the eligibility questions were fairly sensitive, were administered at households, and were not part of a respondent-initiated encounter. Child birth order (CBO) in the family has received little attention in the field of prevention science. CBO is relevant to early interventionists from a public health perspective, as the most widely disseminated home-visiting program has traditionally targeted mothers and their first-born children. The current paper revisits a previous publication by Shaw et al. (2009) on the effectiveness of the Family Check-Up (FCU) to evaluate CBO (firstborn vs. middle vs. youngest) as a moderator of treatment effects of the FCU in relation to improvements in parenting, maternal depressive symptoms, and child outcomes from ages 2 to 4 in a sample of low-income, ethnically diverse families (N = 709) with multiple children. Results suggest that the FCU elicited improvements in observed parenting from ages 2 to 3 primarily for target children who were the youngest or middle children, but not for firstborns. Personal and family history of suicidal thoughts and behaviors (PSH and FSH, respectively) are significant risk factors associated with suicides. Research is limited in automatic identification of such data from clinical notes in Electronic Health Records. This study developed deep learning (DL) tools utilizing transformer models (Bio_ClinicalBERT and GatorTron) to detect PSH and FSH in clinical notes derived from three academic medical centers, and compared their performance with a rule-based natural language processing tool. For detecting PSH, the rule-based approach obtained an F1-score of 0.75 ± 0.07, while the Bio_ClinicalBERT and GatorTron DL tools scored 0.83 ± 0.09 and 0.84 ± 0.07, respectively. For detecting FSH, the rule-based approach achieved an F1-score of 0.69 ± 0.11, compared to 0.89 ± 0.10 for Bio_ClinicalBERT and 0.92 ± 0.07 for GatorTron. Across sites, the DL tools identified more than 80% of patients at elevated risk for suicide who remain undiagnosed and untreated. [ABSTRACT FROM AUTHOR]

6–11-year-old children provide a critical window for physical activity (PA) interventions. The Virtual Fitness Buddy ecosystem is a precision health PA intervention for children integrating mixed reality technology to connect people and devices. A cluster randomized, controlled trial was conducted across 19 afterschool sites over two 6-month cohorts to test its efficacy in increasing PA and decreasing sedentary behavior. In the treatment group, a custom virtual dog via a mixed reality kiosk helped children set PA goals while sharing progress with parents to receive feedback and support. Children in the control group set PA goals using a computer without support from the virtual dog or parents. 303 children had 8+ hours of PA data on at least one day of each of the 3 intervention time intervals. Conversion of sedentary time was primarily to light-intensity PA and was strongest for children with low baseline moderate-to-vigorous PA than children above 45 min of baseline moderate-to-vigorous PA. Background: The Fire service Organizational Culture of Safety (FOCUS) survey is an assessment tool comprised of psychometrically validated metrics of safety climate, safety behavior, and downstream outcomes (organizational and injury) that are specific to the U.S. fire and rescue service. Methods: This analysis consists of a descriptive summary of two independent survey waves (FOCUS 1.0 and 2.0). The fire departments included in these survey waves were from convenience sampling (n1.0 = 275; n2.0 = 170). In addition to department level characteristics, we examined individual level characteristics for firefighters and EMS providers in participating departments (n1.0 = 22,719; n2.0 = 16,882). We conducted regression analyses to examine the associations between safety climate and safety behaviors, organizational outcomes, and safety outcomes. All analyses were stratified by organization type (career, volunteer). Results: Our analysis indicated that a majority of respondents were males (90.7%FOCUS 1.0; 90.4%FOCUS 2.0), non-officers (68.4%FOCUS 1.0; 66.4%FOCUS 2.0), and non-Hispanic Whites (70.8%FOCUS 1.0; 69.5%FOCUS 2.0). For both samples there was a higher prevalence of injuries among individuals in career departments (nFOCUS 1.0 = 3778 [17.5%]; nFOCUS 2.0 = 3072 [18.7%]) than volunteer departments (nFOCUS 1.0 = 103 [8.8%]; nFOCUS 2.0 = 34 [7.4%]). We observed an approximate 10-point difference between the mean scores of Management Commitment to Safety for career and volunteer departments in both samples. We observed associations for two organizational outcomes, Safety Behavior and Job Satisfaction, with Management Commitment to Safety and Supervisor Support for Safety overall and when stratified by organization type. We observed a decrease in the odds of injuries associated with a one-unit increase in Management Commitment to Safety (OR1.0 overall: 0.98, 95% CI 0.97–0.99; OR2.0 volunteer: 0.90, 95% CI 0.85–0.95) and Supervisor Support for Safety (OR1.0 overall: 0.95, 95% CI 0.93–0.97; OR1.0 career: 0.95, 95% CI 0.92–0.98). Conclusions: From our current study, and a prior analysis of a geographically stratified random sample of U.S. fire departments, we identified that from all the organizational outcomes, job satisfaction was most consistently associated with FOCUS safety climate. Further, firefighters in our samples consistently rated Supervisor Support for Safety higher than Management Commitment to Safety. We examined differences in reactions to climate change as a function of belief in God. We studied four samples, convenience samples of university students in the USA (n = 627) and in Poland (n = 628), a nationally representative sample of adults in Poland (n = 1154), and a nationally representative sample of adults in the USA (n = 1098). In each study we measured the distress people felt about climate change, belief in God, and left–right political orientation. These constructs were measured slightly differently across the studies. Regardless of how these constructs were measured, believers were less distressed by climate change than non-believers, and with only a few exceptions, these differences disappeared after covarying political orientation (left–right or liberal-conservative). Contrary to those who argue that there is something inherent in religious belief that predisposes people to deny or ignore climate change, the present results suggest that it is the (growing) confluence of faith and conservative political orientation that is responsible for the fact that some people of faith tend to deny climate change or actively oppose efforts to combat it. The purpose of this study was to estimate a regression model that best predict symptoms of depression among Black Seventh-day Adventists in the United States. The sample (n = 3,570) was drawn from the Biopsychosocial Religion and Health Study, a sub-study of the larger Adventist Health Study-2 consisting of a random sample (n = 10,998) of Adventists. The results of the study showed that poor sleep quality, hostility, stress, and perceived discrimination were all predictors of symptoms of depression, while religious involvement decreased the likelihood of experiencing those symptoms. This paper studies the design effect of counting rules, for linking deaths to housing units where they are enumerated in the survey, on the sampling variance of dual system and single system estimators of death registration completeness. It investigates estimators based on conventional rules that uniquely link each death to a single housing unit as well as estimators based on multiplicity rules which permit deaths to be linked to more than one housing unit. Sampling variance formulas are derived containing parameters that reflect the efficiency of the counting rule. Estimates of these parameters for different counting rules are compared utilizing information that was collected in a mortality survey experiment. Previous studies show that certain minority and ethnic communities experience low human papillomavirus (HPV) vaccination rates despite a higher cervical cancer burden. HPV is known to be responsible for almost all cervical cancer cases. Hmong Americans, a growing Asian American population, appear to be at increased risk. The cervical cancer incidence rate among Hmong American women is three times higher than other Asian/Pacific Islanders and more than four times higher than Non-Hispanic Whites. Despite such alarming statistics, there is limited research focusing on HPV literacy and its associated factors in the Hmong American community. This study's objectives are to investigate: (1) the level of HPV knowledge among Hmong Americans; (2) HPV vaccination initiation and completion rates of Hmong Americans; and (3) factors associated with HPV literacy in the Hmong American community. Andersen's Behavioral Model of Health Services Use was used as the study's theoretical framework. A self-administered paper and online health survey was completed by192 Hmong Americans living in a major metropolitan area in Minnesota. Results revealed a mean score of 4.76 (SD 1.67) for the 7-item questionnaire measuring HPV knowledge. The HPV vaccination initiation rate was 46.3 % (n = 56), with 32.7 % completing the recommended three doses. Multiple regression analysis found that participants' level of education, number of doctor visits, and cervical cancer screening literacy were significantly associated with HPV knowledge. This study's results indicate the important role of health providers in educating Hmong Americans patients about HPV and cervical cancer prevention to decrease the cervical cancer burden in this high-risk population. The increasing use of wearable actimetry devices in cohort studies can provide a deep and objective insight in physical activity (PA) patterns. For reliable and reproducible pattern recognition, and to minimize the influence of specific device characteristics, there is a need for a generic method to identify relevant PA events in sensor data sets on the basis of comprehensive features such as PA duration and intensity. The objectives of this paper are to present a method to identify universal event detection thresholds for such parameters, and to attempt to find stable meta-clusters of PA behaviour. PA events of 5, 10, 20 and 30 min with low, medium and high intensity thresholds found in literature and intensity deciles were computed for a random sample ( N = 100) of the NHANES 2005-06 accelerometer data set ( N = 7457). On the basis of all combinations of the above, activity events were detected, and parameters mean duration, mean intensity and event regularity were computed. Results were clustered using x-Means clustering and visualized for 5-, 10-, 20-, and 30-min events. Stable clustering results are obtained with intensity thresholds up to the 8th decile and for event durations up to 10 min. Two stable meta-clusters were detected: 'irregularly active' (intensity at 52nd percentile) and 'regularly active' (intensity at 42nd percentile). Distinct generic thresholds could be identified and are proposed. They may prove useful for further investigations of similar actimetry data sets, minimising the influence of specific device characteristics. The results also confirm that distinct PA event patterns - including event regularity - can be identified using wearable sensor devices, especially when regarding low-intensity, short-term activities which do not correspond to current PA recommendations. Further research is necessary to evaluate actual associations between sensor-based PA parameters and health outcome. The author identified generic intensity and duration thresholds for analysing objective PA data from wearable devices. To reduce current HIV disparities among African American youth, it is imperative to find effective ways to extend the reach of evidence-based HIV prevention. One promising community resource to support this effort is faith-based organizations (FBOs), a credible and respected resource in the African American community. This paper describes the experiences, perceptions, and challenges that African American FBOs and faith leaders face in engaging in adolescent HIV prevention and highlights facilitators and barriers to implementing HIV prevention in African American FBOs. The findings suggest that African American FBOs and faith-based leaders are uniquely positioned to be instrumental resources in reducing African American youth HIV disparities. This paper examines body mass index (BMI) trajectories among children from different race/ethnic and maternal nativity backgrounds in the United States and England from early- to middle-childhood. This study is the first to examine race/ethnic and maternal nativity differences in BMI trajectories in both countries. We use two longitudinal birth cohort studies-The Fragile Families and Child Wellbeing Study (n = 3,285) for the United States and the Millennium Cohort Study (n = 6,700) for England to estimate trajectories in child BMI by race/ethnicity and maternal nativity status using multilevel growth models. In the United States our sample includes white, black, and Hispanic children; in England the sample includes white, black, and Asian children. We find significant race/ethnic differences in the initial BMI and BMI trajectories of children in both countries, with all non-white groups having significantly steeper BMI growth trajectories than whites. Nativity differences in BMI trajectories vary by race/ethnic group and are only statistically significantly higher for children of foreign-born blacks in England. Disparities in BMI trajectories are pervasive in the United States and England, despite lower overall BMI among English children. Future studies should consider both race/ethnicity and maternal nativity status subgroups when examining disparities in BMI in the United States and England. Differences in BMI are apparent in early childhood, which suggests that interventions targeting pre-school age children may be most effective at stemming childhood disparities in BMI. The significant number of annual US jail admissions is intricately tied to the increasing population of children with incarcerated parents. Some proportion of these children will visit their parents in jail, and the limited research linking visits to young children's well-being is mixed. Sesame Street developed multimedia educational materials to support young children with incarcerated parents, including specific messages around visiting. The educational materials have been found to positively shape how caregivers talk to children about parental incarceration, though a gap remains regarding young children's self-reported experiences. In a preliminary randomized efficacy trial of these educational materials, the current study examined 67 young children's (aged 3–8) self-reported feelings while at the jail following viewing of the video materials, including their feelings about their caregivers, incarcerated parents, families, and visiting in general. Data were collected when children arrived at the jail (before half were randomized to watch the intervention materials) and then again following the intervention. In the treatment group, the proportion of children reporting positive feelings increased from pre- to post-test, most saliently for feelings about families, while feelings decreased overall for those in the control group. The intervention was associated with positive feelings about family, especially for those children who were told developmentally appropriate information about the parent's incarceration prior to arrival at the jail. The exploratory findings shed light on young children's emotions when visiting parents in jail and the buffering role that intervention materials can have in offering support to help manage feelings during jail visits. Highlights: Exploratory findings suggest that the intervention was associated with positive changes in self-reported feelings for young children visiting parents at jail. Feelings about family were most significantly strengthened for young children following the multimedia intervention at jail. Background: A common method of pain assessment is the numerical rating scale, where patients are asked to rate their pain on a scale from 0 to 10, where 0 is "no pain" and 10 is "pain as bad as you can imagine". We hypothesize such language is suboptimal as it involves a test of a cognitive skill, imagination, in the assessment of symptom severity. Methods: We used a large-scale online research registry, ResearchMatch, to conduct a randomized controlled trial to compare the distributions of pain scores of two different pain scale anchors. We recruited adults located in the United States who reported a chronic pain problem (> 3 months) and were currently in pain. Participants were randomized in a 1:1 ratio to receive pain assessment based on a modified Brief Pain Inventory (BPI), where the anchor for a score of 10 was either "extremely severe pain", or the original BPI, with the anchor "pain as bad as you can imagine". Participants in both groups also answered additional questions about pain, other symptomatology and creativity. Results: Data were obtained from 405 participants for the modified and 424 for the original BPI. Distribution of responses to pain questions were similar between groups (all p-values ≥ 0.12). We did not see evidence that the relationship between pain score and the anchor text differed based on self-perceived creativity (all interaction p-values ≥ 0.2). However, in the key analysis, correlations between current pain assessments and known correlates (fatigue, anxiety, depression, current pain compared to a typical day, pain compared to other people) were stronger for "extreme" vs. "imaginable" anchor text (p = 0.005). Conclusion: Pain rating scales should utilize the modified anchor text "extremely severe pain" instead of "pain as bad as you can imagine". The aim of this study is to test a multifaceted model across gender, consisting of bonding systems in relation to violent behaviors of youth and explaining the relationships among them. The National Survey on Drug Use and Health in 2006 was the source of data. This paper reports findings of a study examining gender-sensitive models with adolescent violent behavior, by testing multiple bonding systems in the adolescents’ environment simultaneously with use of (1) bi-variate statistical test, (2) t test, and (3) estimation of the structural equation modeling among all the concepts of violent behavior and bonding systems for the whole juvenile sample and separated sample of male and female juveniles. Both peer bonding and school bonding have an effect in decreasing violent behavior of both male and female youth. However, some different bonding systems were found to be effective only for a certain gender, community bonding for male youths and parental bonding for female youths. The satisfactory model fit was confirmed with whole, male-only, and female-only samples, along with new findings of several male-only and one female-only significant causal linkage among bonding systems. This paper discusses characteristics that are associated with enhanced maternal and child health (MCH) epidemiology functioning in state health agencies. The concept of the 'MCH Epidemiology Effort' is introduced as 'the epidemiologic work carried out by multiple units and agencies aimed at informing program planning and policy development on behalf of women, children and families.' This concept focuses attention on MCH epidemiology functioning at the organizational level rather than on individual MCH epidemiologists. The analysis used data from all 50 states and the District of Columbia. Each state participated in a telephone interview and submitted material that demonstrated the breadth, depth, and capacity of its MCH Epidemiology Effort. Several organizations, including the Council for State and Territorial Epidemiologists, the Health Resources and Services Administration/Maternal and Child Health Bureau, and the Centers for Disease Control and Prevention provided additional secondary data. The outcome for analysis was a three-category measure of MCH epidemiology functioning. The findings are consistent with, and add specificity to, those from prior assessments. In a multivariable model, agenda-setting by consensus, involvement of external stakeholders, the total of doctorally trained staff, and accessing CDC assignees or other staff were all significantly related to higher level MCH epidemiology functioning (ORs of 6.1, 6.6, 2.5, and 6.4, respectively; P < 0.05). Organizational visibility of the MCH Epidemiology Effort and a data environment marked by routine data-sharing and data integration were marginally related. Billions of dollars are lost each year to Medicare and Medicaid fraud. Using three real payment populations, we consider the operating characteristics of commonly used sampling-and-extrapolation strategies for these audits: simple random sampling using (1) the simple expansion estimator or (2) the ratio estimator; and (3) stratified sampling where the basis of stratification is the payment amount. The achieved confidence level (=rate of under-recoupment) of the lower confidence bound based on the ratio estimator fell far below the government-prescribed 90% level for all three populations in commonly encountered high denial-rate scenarios. For the expansion estimator in simple random sampling, the achieved confidence level depends on the skew of the overpayment population: if it is left skewed, the level will fall below 90%, sometimes far below; if it is right-skewed, it will exceed 90%. In the latter case, careless stratification by payment amount can destroy this conservatism. When there is strong right skew, limited stratification can sometimes preserve the 90% confidence while yielding improvements in overpayment recovery. In any population where 90% under-recoupment is not achieved by extrapolation methods based on the central limit theorem, methods based on sample counts and the hypergeometric distribution (Edwards et al., Health Serv Outcomes Res Methodol 4:241-263, ; Gilliland and Feng, Health Serv Outcomes Res Methodol 10:154-164, 2010; Edwards et al., Pennysampling. This paper reports baseline behavioral and biological data collected from a cohort of 535 African American HIV serodiscordant couples enrolled in the Eban study across four urban metro areas. Data were collected on (1) the prevalence of risky sexual behaviors that occur within a couple and with concurrent sexual partners, (2) the STD prevalence for each member of the couple and (3) the correlates of STDs in the male partner as well as in the female partner. Presentation of the sociodemographic characterization and HIV risk behavior profiles of African American HIV serodiscordant couples represents an important initial description of a hidden, vulnerable population. Future research should be conducted with diverse samples of African American couples (i.e., younger couples, non-stable couples) to explore other potential correlates of STD prevalence. A large body of research suggests that US cropland soils can also sequester significant amounts of C and are a promising source of C credits. This paper presents a framework for assessing the transactions costs associated with per-hectare and per-credit contract types and addresses the potential magnitude of transactions costs associated with measuring soil C credits under a per-credit contract within the dry-land crop region of Montana, USA. In the empirical analysis, we estimate the total measurement costs for soil C credits and investigate how changes in contract (and region) size as well as increases in C credit variability affect total measurement costs. The empirical analyses suggest that increasing the size of the contract and aggregating credits over a larger number of producers can lower measurement costs associated with the per-credit contract, even in the face of increasing C variability. Thus contracts for large quantities of soil credits increase the likelihood that the per-credit contract remains more efficient than the per-hectare contract. However, these empirical results reflect the specific data and conditions present within the case study region. The theoretical expectation is that sample size and measurement costs can either increase or decrease as the population to be sampled increases. Interest in natural resources and the environment has led to the development of new federal monitoring efforts, the expansion of existing federal inventory programmes, and discussions of inter-agency collaboration for natural resource assessment data collection. As federal programmes evolve, knowledge gained from existing long-term survey programmes can provide valuable contributions to statistical and operational aspects of survey efforts. This paper describes the National Resources Inventory (NRI), which has been conducted by the US Department of Agriculture's Natural Resources Conservation Service in cooperation with the Iowa State University Statistical Laboratory for several decades. The current NRI is a longitudinal survey of soil, water, and related environmental resources designed to assess conditions and trends every five years on non-federal US lands. An historical overview is provided highlighting the development of the survey programme. Sample design, data collection, and estimation procedures used in the 1992 NRI are described, and statistical issues related to long-term monitoring are discussed. [ABSTRACT FROM AUTHOR]

Public stigma toward those experiencing symptoms of schizophrenia in the general population is high; yet research into such stigma within the diverse Latino communities remains under-investigated. This study employed a randomized experimental vignette methodology to assess various domains of public stigma toward individuals experiencing psychosis and/or diabetes within Latino communities. A communitybased sample of 243 Latino adults participated. Contrary to our expectations, respondents who were more sympathetic toward those with mental health problems tended to score higher on public stigma. The belief that a person was responsible for their own mental health problems was associated with higher levels of stigma. Results indicate that perceptions of dangerousness toward someone experiencing psychosis were common, and the perception that a person was responsible for their mental health problems was associated with higher levels of stigma Results emphasize the complex nature of stigma within the diverse Latino communities and the need for ongoing research. The article provides information on the study concerning the ultrasound guided intussusception reduction in the U.S. The author implies that the next step after the incorporation of the new therapeutic strategy would be to assess its efficacy and effectiveness. Moreover, a randomized trial could be performed comparing two techniques but it would likely require a multi-center design to obtain enough sample size.

Racial socialization is a dynamic process for youth of color, especially for Black adolescents. What is less known is how the provision of racial socialization practices and messages may differ for varying ethnic groups of Black adolescents and what sociodemographic factors contribute to this difference. As such, this cross-sectional study investigated how African American and Black Caribbean adolescents are racially socialized by parents and support systems, including socialization frequency, sociodemographic contributors, and content. A sample of 1170 adolescents (Black Caribbean: n = 360; African American: n = 810) from the National Survey of American Life-Adolescents were assessed. Latent class analysis depicted two classes of socialization provision in both African American and Black Caribbean samples, as adolescents with greater provision received it most from family members across ethnic groups while friends were a distinguishing socializing agent across classes and ethnicities. Further, Black Caribbean adolescents socialized at a high frequency were not often socialized by other adults or friends, just parents and other family members. Some key demographic differences differentiated this provision, with region and income being the most prominent for African American and Black Caribbean families respectively. Finally, messages were fairly consistent across ethnic groups, save the advice to "keep it real" being significantly more provided to African American adolescents. Implications of these findings are relevant for interventions seeking to bolster socialization efforts across support systems and ethnic groups within the Black diaspora, with specific attention towards adolescents receiving "low" levels of socialization across their support system. Objective: Access to trauma-informed sexual and reproductive health services (SRHS) among persons with mental illness (MI) is inadequate and may contribute to health disparities among this population. This qualitative study investigated health professionals' perspectives of SRHS delivery to patients receiving care at a behavioral health hospital. Methods: Semi-structured interviews were conducted in a convenience sample of 26 mental health professionals recruited from one behavioral health hospital. Interviews assessed participants' perceptions of current SRHS offerings, barriers, and opportunities to implement patient-centered SRHS at the hospital. A constant comparison thematic analysis was employed. Results: Participants noted patients' overlapping sexual and reproductive health (SRH) and mental health needs. Most participants indicated clinical responsibility to address these needs. However, participants stated they infrequently provided SRHS because of structural barriers within the healthcare system. Barriers included a lack of hospital policy and health professional education on SRH. Some participants who navigated around these barriers reported using an existing SRH screening tool. Conclusions: Patients receiving care at this behavioral health hospital have unmet SRHS needs. This lack of care may undermine the health of patients with MI. Addressing system level gaps requires clinical re-prioritization. Promising approaches to address this gap include reverse-integrated care models and improved SRHS training of health professionals. Background: Although gun owners overwhelmingly support violence prevention policies, they are hesitant to speak up publicly to advocate for these policies. We tested a series of communication messages on gun owners' level of support for various firearm violence prevention policies and on their willingness to engage in gun violence prevention advocacy. Methods: We conducted three consecutive experiments, testing a total of thirteen messages on a sample of gun owners over 18 years old who live in the U.S. The first was a random experiment, the second a quasi-experiment, and the third a randomized control trial. The goal of having these varied methods was to develop messages applicable to different contexts with different levels of information about the audience. Results: The most effective message was a script showing respect for gun owners' decisions to purchase a firearm and proposing a balanced policy roadmap to end gun violence, which led to an increase in gun owner's willingness to engage in eight different advocacy activities. We also found a value-based message conveying loyalty to increase support for domestic violence related prohibitions and willingness to engage in advocacy for gun violence prevention policies. Conclusions: Public health professionals need to develop communication strategies that are aligned with gun owners' values and that affirm respect for gun culture and for gun owners' decisions to own a gun. Our primary objective was to document COVID-19 induced changes to perinatal care across the USA and examine the implication of these changes for maternal mental health. We performed an observational cross-sectional study with convenience sampling using direct patient reports from 1918 postpartum and 3868 pregnant individuals collected between April 2020 and December 2020 from 10 states across the USA. We leverage a subgroup of these participants who gave birth prior to March 2020 to estimate the pre-pandemic prevalence of specific birthing practices as a comparison. Our primary analyses describe the prevalence and timing of perinatal care changes, compare perinatal care changes depending on when and where individuals gave birth, and assess the linkage between perinatal care alterations and maternal anxiety and depressive symptoms. Seventy-eight percent of pregnant participants and 63% of postpartum participants reported at least one change to their perinatal care between March and August 2020. However, the prevalence and nature of specific perinatal care changes occurred unevenly over time and across geographic locations. The separation of infants and mothers immediately after birth and the cancelation of prenatal visits were associated with worsened depression and anxiety symptoms in mothers after controlling for sociodemographic factors, mental health history, number of pregnancy complications, and general stress about the COVID-19 pandemic. Our analyses reveal widespread changes to perinatal care across the US that fluctuated depending on where and when individuals gave birth. Disruptions to perinatal care may also exacerbate mental health concerns, so focused treatments that can mitigate the negative psychiatric sequelae of interrupted care are warranted. Aims: We sought to gather experts' perspectives on Medicaid coverage gaps during reentry to identify high-yield policy solutions to improve the health of justice-involved individuals in the United States. Subject and methods: We interviewed 28 experts at the intersection of Medicaid and criminal justice via telephone between November 2018 and April 2019. Interviewees included Medicaid administrators, health and justice officials, policy makers, and health policy researchers. We performed thematic analysis of semi-structured interview transcripts to identify emergent themes and distill policy recommendations. Results: Three themes emerged: 1) Medicaid coverage gaps during reentry contribute to poor health outcomes and recidivism, 2) excessive burden on justice-involved people to re-activate Medicaid leads to coverage gaps, and 3) scalable policy solutions exist to eliminate Medicaid coverage gaps during reentry. Policy recommendations centered on ending the federal "inmate exclusion," delaying Medicaid de-activation at intake, and promoting re-activation by reentry. Experts viewed coverage gaps as problematic, viewed current approaches as inefficient and burdensome, and recommended several policy solutions. Conclusion: By pursuing strategies to eliminate Medicaid gaps during reentry, policymakers can improve health outcomes and efficiency of government spending on healthcare, and may reduce cycles of incarceration. While Asian Indians (AIs) are the third fastest-growing Asian minority population in the United States, they fall short of the Healthy People cervical cancer screening target goals, with rates averaging 70% compared to 83% among non-Hispanic Whites. The purpose of this systematic review is to examine factors influencing cervical cancer screening behaviors among AIs in the US. Medline, CINAHL, PubMed, PsychINFO, and ProQuest databases were searched for qualitative and quantitative studies conducted between 1990 and 2017 that focused on cervical cancer screening behaviors among AIs in the US. The seven selected quantitative cross-sectional descriptive studies show that acculturation, length of stay in the US, age, education level, employment, cultural beliefs, and language influence cervical cancer screening behaviors among AIs in the US. In 2014 approximately 14.4% of Americans ages 18 to 24 had some type of disability. Compared to their peers without disabilities, disabled emerging adults typically have more difficulty dating and finding romantic and sexual partners. One way that they may escape disability stigma and smaller dating pools is through online dating. However, little is known about their experiences seeking romantic and sexual relationships online, and even less is known about the experiences of emerging adults with disabilities who identify with sexual orientations and have gender identities that are not heterosexual or cisgender. In the present study 27 LGBTQ+ emerging adults with various disabilities, genders, and sexual orientations completed 47 item online surveys before COVID in which they described their experiences with online dating and starting and sustaining romantic relationships. Results from qualitative data analyses suggest that rather than being neutral spaces that exist separately from stigma and social prejudice, dating apps and web sites typically perpetuate normative ideas of ableism, sexuality, and gender and stimulate questions of timing of disability disclosure. Despite challenges, however, online dating also offers opportunities to interact with multiple interested daters and appears to offer LGBTQ+ emerging adults with disabilities an often beneficial though sometimes vexing venue for relationship development. This study adapted Andersen’s Health Belief Model to examine the predictors of mental health services utilization among Korean American (KA) immigrants. A cross-sectional survey was used to gather data on 363 KA immigrants 18 years and older residing in New York City. Predisposing factors included gender, age, marital status, education, length of stay in the US, and religion; the need factor was depression; and enabling factors included health insurance, English proficiency, income, and perceived need for help. Approximately 8.5 % of participants reported having utilized mental health services, while 23 % reported having depressive symptoms. Shorter duration of residence in the US, lower income, and the presence of perceived need for help were significantly related to use of mental health services. The perceived need for help mediated the relationship between depression and mental health service utilization. Failure to perceive the need for psychological help continues to be a major reason that KA immigrants do not use mental health services. [ABSTRACT FROM AUTHOR]

Social factors (e.g. housing, food security, etc.) contribute significantly to health. The purpose of this study is to describe social risk and social exclusion factors in one of the largest Middle Eastern and North African (MENA) populations in the U.S. and their association with health outcomes. We conducted a cross-sectional study with a community convenience sample of 412 adults who self-identify as MENA. Weighted, adjusted linear regression models were used to examine relationships of interest. Prevalent social risks included transportation barriers to healthcare (33%), food insecurity (33%), and financial strain (25%). In adjusted models, perception of being treated unfairly (Estimate (SE) 0.08 (0.04), p < 0.05) and fear of deportation (0.26 (0.06), p < 0.001) were associated with more social risk factors. More social risk factors were associated with worse self-reported health (0.09 (0.03), p < 0.01), more chronic conditions (0.11 (0.03), p < 0.004), and more mental health symptoms (0.34 (0.14) p < 0.01).Social risk is high among those perceiving unfairness and fear deportation. Those with more social risk factors reported worse health. These findings have implications for social needs screening and referral models that can best serve U.S. MENA sub-populations. Purpose An increasing number of workers in the US have chronic health conditions that limit their ability to work, and few worksite interventions have been tested to improve worker coping and problem solving at work. The purpose of this study was to evaluate a worksite-based health self-management program designed to improve workplace function among workers with chronic health conditions. Methods We conducted a randomized, controlled trial of a worksite self-management program ("Manage at Work") (clinicaltrials.gov #NCT01978392) for workers with chronic health conditions (N = 119; 82% female, ages 20–69). Most workers were recruited from the health care or light manufacturing industry sectors. Workers attended a 5-session, facilitated psychoeducational program using concepts of health self-management, self-efficacy, ergonomics, and communication. Changes on outcomes of work engagement, work limitation, job satisfaction, work fatigue, work self-efficacy, days absent, and turnover intention at 6-month follow-up were compared to wait-list controls. Results The most prevalent chronic health conditions were musculoskeletal pain, headaches, vision problems, gastrointestinal disorders, respiratory disorders, and mental health disorders. The self-management program showed greater improvement in work engagement and turnover intent at 6-month follow-up, but there was no evidence of a parallel reduction in perceived work limitation. Trends for improved outcomes of work self-efficacy, job satisfaction, and work fatigue in the intervention group did not reach statistical significance in a group x time interaction test. Conclusions Offering a worksite self-management program to workers with chronic health conditions may be a feasible and beneficial strategy to engage and retain skilled workers who are risking disability. Introduction: Hispanics are the largest minority group in the United States, constituting 18 % of the population. Mexicans are the largest Hispanic subgroup and are at disproportionate risk for overweight/obesity. Lifestyle interventions targeting dietary change and physical activity have resulted in significant weight loss in several large randomized clinical trials in the general population, but few studies have tailored interventions to Mexican Americans. We conducted a community needs assessment from 2018 to 2020 in accordance with Domenech-Rodriguez and Wieling's Cultural Adaptation Process (CAP) model to inform the development of SANOS (SAlud y Nutrición para todOS) (Health and Nutrition for All), a culturally-tailored, community-based diet and lifestyle education and counseling program that addresses overweight/obesity among U.S. Mexicans. Methods: Five Spanish-language focus groups were conducted until thematic saturation with 31 overweight/obese Mexicans in New York City about their knowledge, priorities, and preferences regarding diet, exercise, and evidence-based strategies for behavioral change. A grounded theory approach was used to analyze the data. Results: Five themes were identified: (1) A strong desire for tangible information related to diet and health, (2) Family as a primary motivator for behavior change, (3) Desire for group-based motivation and accountability to sustain intervention participation, (4) Belief in short-term goal setting to prevent loss of motivation, and (5) Time and workplace-related barriers to intervention adoption. Conclusions: Ecological factors such as the effect of acculturation on diet, family members' role in behavior change, and socioenvironmental barriers to healthy dietary practices and physical activity should be considered when adapting evidence-based treatments for Mexican Americans. Abstract: In the analysis of continuous variables collected through a complex sample design, the sparseness of data in the tail region may lead to relatively poor performance for design-based estimation of distribution functions and also to potential instability of direct design-based estimators of their covariance matrices. Consequently, it is of interest to consider approximation methods that may lead to more stable covariance matrix estimators. Accordingly, one may seek to obtain better inference methods by fitting an appropriate parametric model to data from these tail regions. This paper develops one such approximation method by examining the effect of cluster sampling on the covariance and correlation matrices of sample distribution functions based on a superpopulation model. The results are applied to data from a stratified multistage sampling design. To mitigate the negative impact of resource needs on child health, practices serving low-income immigrant families have implemented screening programs to connect families to community resources. Little is known about how duration of US residence relates to patterns of resource needs and indicators of acculturation such as community resource knowledge/experience or self-efficacy. We conducted a cross-sectional analysis of a convenience sample of immigrant families with young children at an urban primary care clinic. These families were seen 5/2018–1/2020 for well child care, screening positive for ≥1 social need using a tool derived from Health Leads. Analysis of 114 families found that newly arrived families with a shorter duration of US residence (≤5 years) were more likely to report immediate material hardships like food insecurity and need for essential child supplies. Newly arrived families were also less likely to have access to technology resources such as a computer or smartphone. Long-term families with a longer duration of US residence (≥15 years) were more likely to report chronic needs like poor housing conditions, but also reported increased community resource knowledge/experience and increased self-efficacy. Primary care pediatric practices should assess immigration contextual factors to identify subgroups such as newly arrived families with young children to target resources (e.g., increase screening frequency) or enhance services (e.g., patient navigators) to relieve resource needs. Despite improvements in HIV prevention and treatment, Black individuals continue to be disproportionately affected by the HIV epidemic in the US. Using data from the National Survey on HIV in the Black Community, we examined the differences in demographic characteristics, risk behaviors, and HIV testing between US and non-US born Black adults. 868 individuals completed the survey and provided baseline data on sexual risk. Participants were grouped as US-born (N = 763) and non-US born (N = 101) based on self-reported place of birth. Amongst US-born participants, males were less likely to test for HIV, whereas those who reported a lifetime history of anal sex and sexually transmitted infections (STIs) were more likely to test for HIV. Non-US born participants who reported a single marital status were less likely to test for HIV, whereas those who reported a lifetime history of STIs were more likely to test for HIV. Some differences in predictors of HIV testing exist by place of birth. Understanding these differences is needed to develop HIV/AIDS prevention and treatment programs for US and non-US born Black individuals. [ABSTRACT FROM AUTHOR]

Breast cancer is the most common cancer in women. Asian American women have experienced steadily increasing breast cancer incidence rates over the past several decades. The increased rate might be in part due to acculturation. We tested the hypothesis that higher level of acculturation was associated with higher mammographic breast density (MBD), an indicator of breast cancer risk, in a cohort of 425 premenopausal Chinese immigrant women in Philadelphia. Generalized estimating equations accounted for repeated observations and adjusted for age, type of mammographic image, body mass index, months of breastfeeding, number of live births, age at first birth, and menopausal stage (pre, early peri, late peri, post). Results indicated that acculturation level was not associated with any of the MBD measures. Findings were contrary to our hypothesis and previous, cross-sectional studies. This paper reports on the evaluation of a culturally grounded prevention intervention targeting substance use among urban middle-school students. The curriculum consists of 10 lessons promoting antidrug norms and teaching resistance and other social skills, reinforced by booster activities and a media campaign. Three versions were delivered: Mexican American, combined African American and European American, and Multicultural. Thirty-five middle schools were randomly assigned to 1 of the 3 versions or the control. Students completed baseline and follow-up questionnaires over a 2-year period (total 6,035 respondents). Analyses utilizing a generalized estimating equations approach assessed the overall effectiveness of cultural grounding and the cultural matching hypothesis. Support was found for the intervention's overall effectiveness, with statistically significant effects on gateway drug use as well as norms, attitudes, and resistance strategies but with little support for the cultural matching hypothesis. We analyze a sample of Washington State hospitals with a stochastic frontier panel data model, specifying the cost function as a generalized Leontief function which, according to a Hausman test, performs better in this case than the translog form. A one-stage FGLS estimation procedure which directly models the inefficiency effects improves the efficiency of our estimates. We find that hospitals with higher casemix indices or more beds are less efficient while for-profit hospitals and those with higher proportion of Medicare patient days are more efficient. Aim: College students are presumed to be exposed to a vast amount of sexual health information and may participate in various sexual behaviors. Without appropriate sources of sexual health information, the sexual health of college students could be severely affected. The purpose of this study was to identify the sources of sexual information and determine the relationship between socio-demographic factors and discussing sexual matters with father, mother, brother, and/or sister. Subject and method: A school-based cross-sectional study design was conducted in the spring of 2018 (January to May). A simple random sampling was used to select a total of 808 students attending Central Michigan University. Participants answered a questionnaire from the World Health Organization administrated through Qualtrics software. The data was extracted from Qualtrics, and further analysis was done using SPSS software. The outcome of interest was discussing sexual matters with father, mother, brother, and/or sister. Descriptive statistics and bivariate and logistic regression analyses were performed, and all associations were set by p-values less than 0.05 and reported by unadjusted and adjusted odds ratios with 95% CI. Results: Nearly 80% of the study participants were females. Most of the respondents (39.7%) were first- and second-year university students, followed by third- to fifth-year students (38.4%). Approximately 59% of the study sample fell into the group 21 years of age and below. The majority (70.1%) of the study participants identified mothers (rather than fathers, brothers, or sisters) as a source of sexual health information. The majority of participants (39.4%) were non-religious, followed by Christians (35.6%). Most of the study participants (69.9%) earned less than $1000 per year. In a multivariate analysis, this study revealed that there was no statistically significant association between socio-demographic factors and obtaining sexually related information from mothers or sisters. Males also were three times more likely to discuss sexual matters with their fathers when compared to females [AOR: 3.09; 95% CI: 1.92–4.97]. Again, males were 96% more likely to discuss sexual matters with their brothers compared to females [AOR: 1.96; 95% CI:1.11–3.46]. In terms of education, first- and second-year students, and third- to fifth-year students were three times more likely to discuss sexual matters with their fathers compared to graduates [AOR: 3.41; 95% CI: 1.53–7.57, AOR: 3.02; 95% CI:1.53–6.02 respectively]. Conclusion: The majority of participants (70.1%) identified mothers as a source of sexual health matters. Therefore, mothers should be encouraged to be trained for effective communication regarding sexual health issues. Gender showed a significant association with discussing sex matters with fathers and brothers. The year of education also showed a significant association with discussing sexual matters with fathers. Purpose: Opioid use disorder among women of childbearing age has reached epidemic proportions. In rural regions of the United States, recruiting perinatal women who use nonmedical opioids to participate in research is wrought with challenges, including barriers such as community stigma, lack of transportation, and time constraints. The current study describes our process and challenges of recruiting pregnant and postpartum women in rural Indiana consisting of women who misuse opioids and those who do not. Methods included (1) sampling from healthcare facilities based on referrals from front-desk staff and frontline healthcare workers; (2) dissemination of flyers and brochures within healthcare facilities and the community, supported with onsite research assistant presence; (3) digital methods coupled with snowball sampling; and (4) local community talks that provided information about the study. Assessment: Our multiple recruitment efforts revealed that building relationships with community stakeholders was key in recruiting women who use nonmedical opioids, but that digital methods were more effective in recruiting a larger sample of pregnant and postpartum women in a short amount of time. Conclusion: We conclude by making several recommendations to enhance academic-community partnerships in order to bolster sample sizes for prolonged research studies. Research on health outcomes, especially about chronic conditions, among immigrants is necessary to meet the health care needs of this growing population. This study examines the relationship between diabetes and depression among immigrants using the 2006–2015 National Health Interview Survey (NHIS). We find a correlation between having diabetes and depression among foreign-born individuals. Being a woman, poor, and from specific regions in Latin America are associated with a higher odds of comorbid diabetes and depression. Out of the individuals with both diabetes and depression, the burden of both conditions seems to be concentrated among foreign-born individuals from Mexico, Central America, and the Caribbean. We find a correlation between having diabetes and depression among foreign-born individuals, similar to that found in U.S.-born populations. Both of these conditions contribute to the burden of disease in the U.S, and we question whether patterns of both diseases (co-morbidities) are evenly or unevenly distributed in the immigrant population. With 2.3 million individuals incarcerated in the United States, incarceration remains a pressing social influence on health. While risk factors for incarceration are known, research has been slow to identify protective factors. Characterizing adolescent protective and risk factors for incarceration outcomes can inform interventions to prevent incarceration. Using survey data from the National Longitudinal Study of Adolescent to Adult Health, we tested the influence of adolescent risk and protective factors for incarceration of young people. We used ordinal logistic regressions, investigating the relationship between adolescent characteristics (wave I, grades 7–12) with two outcomes: incarceration onset (no incarceration, onset of incarceration as a juvenile, and onset of incarceration as an adult) and incarceration frequency (never incarcerated, incarcerated once, incarcerated more than once) during adolescence and into adulthood (N = 12,136, wave IV, ages 24–32). Adolescent protective factors against incarceration onset and higher incarceration frequency centered on education, including a higher grade point average and a higher likelihood of having future plans to attend college. Risk factors included disruptive behavior, physical or sexual abuse in childhood, parental incarceration, and living with a stepfather figure. Stratified analyses suggested that Latinos may have unique risk profiles compared to African American and White youth. The educational system may be a locus for protective interventions promoting academic achievement and educational aspirations. Understanding both incarceration risk and protective factors may allow interventions to be tailored appropriately. Highlights: Nationally representative study examined risk factors and protective factors in adolescence associated with incarceration. Protective factors in adolescence centered on educational success and future educational plans. Latino youth appeared to have differences in incarceration risk factors as compared to youth from other racial groups that warrant further examination. Background: Transplantation of HIV-positive (HIV+) donor organs for HIV+ recipients (HIV D+/R+) is now being performed as research in the United States, but raises ethical concerns. While patient-reported outcome measures are increasingly used to evaluate clinical interventions, there is no published measure to aptly capture patients' experiences in the unique context of experimental HIV D+/R+ transplantation. Therefore, we developed PROMETHEUS (patient-reported measure of experimental transplants with HIV and ethics in the United States). To do so, we created a conceptual framework, drafted a pilot battery using existing and new measures related to this context, and refined it based on cognitive and pilot testing. PROMETHEUS was administered 6-months post-transplant in a clinical trial evaluating these transplants. We analyzed data from the first 20 patient-participants for reliability and validity by calculating Cronbach's alpha and reviewing item performance characteristics. Results: PROMETHEUS 1.0 consisted of 29 items with 5 putative subscales: Emotions; Trust; Decision Making; Transplant; and Decision Satisfaction. Overall, responses were positive. Cronbach's alpha was > 0.8 for all subscales except Transplant, which was 0.38. Two Transplant subscale items were removed due to poor reliability and construct validity. Conclusions: We developed PROMETHEUS to systematically capture patient-reported experiences with this novel experimental transplantation program, nested it in an actual clinical trial, and obtained preliminary data regarding its performance.
