In this paper, we contend that urban middle-class older Indians engaged in "serious leisure" as a way to reimagine and reconfigure the structure of everyday life during the pandemic-led epochal downtime. In particular, we heuristically show that leisure activity patterns and constraint negotiation strategies among older Indians followed conceptual semblances with the dominant leisure-based typology of Serious Leisure Perspective. By thematically analysing household surveys (n = 71), time-use diaries and in-depth interviews (n = 15) of middle to upper middle-class individuals (55–80 years), we show how both men and women distinguished between serious leisure that is marked by motivation, agency and perseverance with that of unstructured, routinized free-time (or causal leisure). Time-use diaries suggested that despite the changed realities of heightened domestic time available to both genders due to the pandemic, women recorded higher proportion of their daily hours in household management and caregiving. Although women were governed by moral-cultural self-descriptions in their engagement with leisure, it was often associated with an enhanced sense of self-actualisation, self-management and identity. Elder abuse is evil in human society. The present paper unveils this social issue from two major factors psychological and physical abuse. The study sensitizes the subject matter of study by examining the effects of demographic variables like gender and age on elders. For achieving the purpose of the study the research is a design by exploring and validating the factors of measuring elder abuse through the mix method approach, exploratory factor analysis (EFA) and confirmatory factor analysis (CFA). To test the established hypotheses of the effects of the demographic variable on elders the two-way ANOVA was applied. The present study verdicts the development of a sound measurement scale with two influence factors. The separate and composite effect of aging and gender type on elder abuse was evidenced. These findings are crucial especially when the prevalence of elder abuse is higher during COVID-19. The limited novel understudied variable opens an avenue for further research in behavioral and demographic variables like marital status. The present study has practical insinuation for caring the elders in any human society like physical and psychological treatment of elders to avoid abusive situations. In addition, the study attempts to validate the novel issues like psychological and physical abuse of elders in the dimensions of demographic variables. Studies on the association between population ageing and health spending are inconclusive. Though per capita health spending has a strong age gradient, evidence from the developed countries attributes increasing health expenditure to high spending in terminal year of life. In India, though studies have examined the level, trends and determinants of health care spending, no attempt has been made on association of population ageing and per capita health spending across the districts of India. The objective of this paper was to examine the spatial dependencies and heterogeneities of population ageing and per capita health spending across 640 districts of India. Data from the Census of India (2011) and the consumption expenditure rounds of the National Sample Survey Organisation (NSSO) (2009–10 and 2011–2012) were used in the analysis. Univariate and bivariate LISA analysis, ordinary least square regression, and spatial error model were used to decipher the spatial pattern of population ageing and health spending in the districts of India. Results suggest that per capita health expenditure is positively associated with the proportion of the older adults (aged 60 years and above) in the districts of India. Districts with less than 5% older adults have an annual per capita health expenditure of ₹629 compared to ₹2432 in districts with older adults 12% or more. Districts of India also exhibit large spatial heterogeneity in household health spending. Bivariate Moran's I statistic of proportion of older adults and per capita annual health expenditure was 0.48, suggesting spatially association of the share of the older adults and the per capita annual health expenditure in India. Results of the spatial error model confirmed that share of urban population, mean wealth score, and mean household size are significant predictors of per capita annual health expenditure in India. This study is important to monitor the progress of universal health coverage across the disadvantaged districts and to integrate age component in universal health coverage programmes. Background: Population ageing represents a significant global challenge, particularly pronounced in countries like India. Aims: This study aims to explore how factors such as socio-economic status, behaviour, and health influence healthy ageing across the Indian older population. Methods: In this study, we utilized the Longitudinal Ageing Study in India – wave 1 dataset for analysis purposes. Scores were generated for five dimensions of healthy aging, including physical, functional, mental, cognitive, and social aspects and these scores were treated as the target variables. Multivariate Regression Trees analysis was employed to identify the behavioural and socio-demographic factors associated with each dimension of healthy ageing. Results: Years of education emerge as crucial across all dimensions, positively impacting cognitive health and mitigating age-related decline in healthy ageing. Marital status, engagement in household activities, spiritual practices, and living arrangements impacts the scores of different aspects of healthy ageing. Gender disparities in healthy aging are noticeable in the 60–74 age group, with women generally having lower scores. Safety of the living environment is a crucial determinant of the mental health of the elderly across all age groups.These findings highlight the complex interplay of factors in healthy ageing outcomes. Conclusion: Our study emphasizes the pivotal role of education in fostering healthy ageing in India. Factors such as environmental safety and social participation also influence well-being. Background: Music learning induces significant neural changes, enhancing auditory, verbal, and visuospatial skills, IQ, and speech perception. Research studies reveal structural and functional brain plasticity due to music training in adults and children. While musical abilities are linked to formal training, the existence of "musical sleepers" with heightened speech perception without formal training is noteworthy in adults. This study addresses the gap in understanding such phenomena in children, exploring psychophysical abilities in musically adept children, and aiming to impact rehabilitation models. Materials and method: A pre-experimental study involved 164 typically developing children (mean age: 9.93) without musical training. Musical abilities were evaluated using the abbreviated version of Montreal Battery for Evaluation of Musical Abilities (MBEMA) test in DMDX software, covering melody, rhythm, and memory subtests. Psychophysical tests in MATLAB assessed temporal, frequency, intensity, timbre, and binaural resolution. Results: Using a median split based on MBEMA test scores (median: 42), 84 children exhibited poor musical abilities, while 80 showed good ones. Descriptive statistics for MBEMA scores and psychophysical tests were conducted. As data did not follow normal distribution, non-parametric inferential statistics were employed. Mann–Whitney U tests revealed significant differences favouring good musical abilities in all assessments: gap deduction (p = 0.01), pitch discrimination at 500 Hz and 4 kHz (p = 0.00), intensity discrimination at 500 Hz (p = 0.00) and 4 kHz (p = 0.01), profile analysis (p = 0.01), interaural level difference (p = 0.023), and interaural time difference (p = 0.038). Spearman correlation showed highest correlation with pitch discrimination at 500 Hz (r = − 0.538), 4 kHz (− 0.416), intensity discrimination at 4 kHz (r = − 0.367), and 500 Hz (r = − 0.311), profile analysis (r = − 0.313), interaural level difference (r = − 0.276), and gap deduction (r = − 0.235). All were significant (p < 0.01), except for the interaural time difference. Conclusion: Children excelling in music surpassed those with weaker abilities in psychophysical tasks. This echoes past research, underscoring auditory discrimination's influence on musical skills in untrained children. These results potentially support the concept of musical sleepers in children. Key predictors include pitch discrimination, while interaural time difference exhibited minimal correlation. Intellectual disability in India is substantially under-reported, especially amongst females. This study quantifies the prevalence and gender bias in household reporting of intellectual disability by estimating the age-and-gender specific prevalence of the intellectually disabled by education, Socio-Demographic Index (SDI) score, place of residence, (rural/urban) and income of household head. We estimated prevalence (per 100,000) at 179 (95% CI: 173 to 185) for males and 120 (95% CI: 115 to 125) for females. Gender differences declined sharply with increased education, was higher for lower ages and low income and varied little by state development. In this paper abridged nuptiality tables for the single population of India have been constructed for the three consecutive decades 1901-1911, 1911-1921 and 1921-1931. No significant time trend has been observed in the nuptiality rates among the single Population of India. The rates are initially small, but increase rapidly until they reach a maximum at ages 25-30 for bachelors and 15-20 for spinsters, following which they gradually decline. During 1901-1931, unlike Western countries, India had not experienced any revolution in marriage habits encompassing traditional child marriages. The distinctive marks of the Indian age patterns of marriage are higher age-specific marriage rates combined with lower ages at marriage and lower proportions of people who never marry with relative stability of marriage habits during the early part of the twentieth century. Depending upon various factors the surgical procedures in childhood are divided into three groups--immediate, intermediate and elective. The timing of surgery is probably the most important factor governing the outcome of surgical correction in pediatric surgery. With continuing research and clinical experience our understanding of the various conditions have improved and with that has come a change in the optimum timing of many surgical procedures. This paper highlights the best timing of surgery for some of the commoner pediatric surgical procedures and the reasons behind these so that the children may be referred to pediatric surgeons in time. The purpose of this paper has been to examine the age patterns of Indian women at marriage, cohabitation, and first birth. This task is complicated by features of the Indian marriage institution that differentiate it from the institution of marriage in Western cultures. It is also complicated by the diversity of Indian marriage customs and by the possibility that Indian marriage patterns have changed over time. Nevertheless, our analysis of survey data for two districts in south central India support the following conclusions: Marriage and childbearing have been and continue to be universal. The timing of marriage and childbearing, which showed only a small increase over a fairly long period of time, appears more recently to be undergoing some changes in the direction of longer delays. The significant rural-urban differences in marriage and fertility timing are at least partly the result of the greater impact of education on age at marriage, cohabitation, and first birth in urban areas than in rural areas. Observed characteristics such as religion and education explain more variation in women's marriage and fertility timing in urban areas than in rural areas (i.e., education seems to generate new social norms that tend to displace norms derived from tradition. The incidence of childhood marriages and two-stage marriages has been on the decline, causing the small but noticeable rise in age at marriage to exceed somewhat the increase in age at cohabitation. Age patterns of marriage in India do not closely resemble Western patterns, although age patterns of cohabitation and first birth do. And socioeconomic variables appear to explain less variation in marriage and fertility timing within local communities than across communities. Most important, the results highlight the need for more recent and richer data on marriage and fertility behavior in India. In particular, they strongly suggest that marriage patterns in India are now in the midst of major changes. This conclusion is similar to that reached by Caldwell, Reddy, and Caldwell (1983), who adopted a more qualitative approach to analyzing marriage change than ours, but whose observations are also more recent than ours. This paper presents a simple method for estimating a birth rate and a level of mortality for an intercensal period. The birth rate is estimated from the intercept of a line fitted to data and the level of mortality from the slope of that line. The formula that is developed is based upon a recent generalization of stable population relations. An estimate of childhood mortality level is an optional but significant piece of additional input. Aim: To assess the efficacy of submucosal diathermy (SMD) in Chronic rhinosinusitis and inferior turbinate hypertrophy undergoing Functional Endoscopic Sinus Surgery. Materials and Methods: We conducted a Randomized Prospective study in patients undergoing Functional Endoscopic Sinus surgery for Chronic Rhinosinusitis in a tertiary care centre in South India for a period of 2 years. Patients were divided into two groups, Group A underwent FESS and Group B underwent FESS with SMD. The outcome was evaluated using nasal endoscopy score (NES), modified SNOT score and Modified Lund Kennedy scores. Results: A total of 80 patients were included in this study.40 patients were allotted in each group. Male: Female ratio was 48:32. The age distribution ranged from 19 to 44 years with a mean of 29.55 ± 6.90 yrs. Mean NES scores, Modified SNOT and Modified Lund -kennedy scores were estimated pre-operatively and during 1st, 2nd and 3rd month of post-operative periods. The mean pre-operative sores were comparable in both the groups except NES score, which was higher in group B. Both the groups showed significant improvement in post-operative period. Inter group comparison showed significant difference in all scores with group B having better results when compared to group A. Conclusion: This study proves that FESS combined with SMD improves the postoperative clinical outcomes when compared with FESS without turbinate reduction. We conclude that SMD is a simple, mucosal preserving technique with almost no complications and can be safely done along with FESS to improve the outcomes. [ABSTRACT FROM AUTHOR]

Household age-sex composition in India is changing fast due to persistently declining fertility and improvement in life expectancy, rural–urban migration and change in family orientation. Under such circumstances healthcare of older adults is expected to be affected as they mostly depend on other household members. Keeping this in view the present study attempts to answer the question 'How does the change in household age-sex composition affect out of pocket healthcare expenditure of older adults in India?'. Household age-sex composition and healthcare expenditure for rounds of National Sample Survey Organaization (NSSO) conducted in 1993, 1999–00 and 2007–08 are used for the study. Considerable shift in the share of older adults over period of time is noticed and the change in household age-sex composition is found to have significant effect on healthcare expenditure of older adults. More specifically the share of consumption by older adults above 60 years has increased substantially over time with the increase in their composition. This article describes the psychometric properties of a semantic differential scale developed to evaluate the attitudes towards menstruation among Roman Catholics in India, which is referred to as Menstrual Semantic Differential Scale (MSDS). For this purpose, the south Indian state of Kerala was chosen as the location. First, exploratory factor analysis was conducted on the data collected from a sample of participants of different ages, gender, and caste groups. The analysis produced a 10 item scale with a tri-factorial structure that explained 74 percent of the variance. Subsequently, confirmatory factor analysis on a different sample established that the three-factor model was a good fit. Moreover, Cronbach's alpha coefficients of the subscales ranged from 0.73 to 0.93, confirming the acceptable reliability of the instrument. The findings suggest that the MSDS is a reliable and valid measure for assessing attitudes towards menstruation among Roman Catholics in India. [ABSTRACT FROM AUTHOR]

The psychosocial model of social exclusion postulates that age identity of older persons induce and sustain perceived and enacted discrimination, resulting in psychosocial disability and social exclusion. These propositions were tested by using a cross-sectional survey of 573 older persons, aged 65 years and above. The variables were measured by standardized rating scales. The results reveal age identity significantly influenced perceived (β =.762; p <.001), enacted discrimination (β =.514; p <.001), psychosocial disability (β =.588; p <.001) and social exclusion (β =.389; p <.001). Perceived discrimination significantly increased enacted discrimination (β =.314; p <.001) and enacted discrimination influenced psychosocial disability (β =.279; p <.001) and psychosocial disability increased social exclusion (β =.090; p <.05). Perceived discrimination significantly mediated the negative effect of age identity on enacted discrimination (b =.211; p <.001), psychosocial disability (b =.219; p <.001) and social exclusion (b =.073; p <.05). The conceptual model showed good data model-fit, giving evidence to conclude that this model was useful in explaining process and outcomes of social exclusion of older persons. Aim: There is a well-established association between tobacco consumption with asthma and chronic bronchitis (CB). This study aimed at estimating the prevalence of tobacco consumption and its association with asthma and CB in Kanpur, Uttar Pradesh, India. Subjects and methods: A cross-sectional household survey of 573 males was conducted during January–June 2015 in the Jajmau area of Kanpur, Uttar Pradesh, India. The questionnaire was designed to collect information on self-reported symptoms pertaining to asthma and CB, which were considered as response variables for the study. Results: More than one-third of the respondents (36%) smoked tobacco and 44% consumed smokeless tobacco. Men who were aged 35 years and above were more prone to CB, with odds ratios (ORs) of 2.08 in model 1 and 1.84 in model 2. The prevalence of CB is higher among those from the Scheduled Castes and Scheduled Tribes (12.4%). People consuming any form of tobacco are more likely to get CB (OR = 5.22) than their counterparts who do not consume tobacco. The chances of getting asthma was higher among those who consumed any form of smokeless tobacco (OR = 1.34) and those who consume any form of tobacco (OR = 2.30) in model 2. Conclusions: Our study found a significant association between tobacco consumption (smoking and use of smokeless tobacco) with asthma and CB and the predictors that affect the pattern of tobacco consumption, such as respondents' age, education level, caste, level of exposure to the media, and standard of living index. India is the home of more than 253 million adolescents, which account for almost 21% of the country's population. In the recent past, there has been a rise in the prevalence of mental illness and maladaptive behaviours among adolescents living in slum populations, which are increasing at an alarming rate in many developing countries, mainly due to rural–urban migration, are characterized by poverty, poor housing, overcrowding, poor environment, and high prevalence of communicable diseases. This neglected population has become a major reservoir for a broad spectrum of adverse health conditions. Studies on the mental health status among adolescents in India are relatively scarce. The main objective of the study was to estimate the prevalence of mental health disorder and its determinants among adolescents in slum settings in Lucknow, India; by using GHQ-12. The study used cross-sectional primary data which was collected in 2015–2016 from the adolescents living in various slums of Lucknow city. About 590 adolescents whose parents gave consent to participate were included in the study. Descriptive statistics, Chi square test and ordered logistic regression analysis (OLR) technique were used to assess the mental health. Reliability of GHQ-12 was assessed before performing the analysis which was found to be 0.79. Approximately, 33% of adolescents were found to suffer from severe level of mental disorder. The results of this study depicts that adolescents of age 12–15 years had more severe mental health problems compares to 16–19 years of adolescents. Females were more likely to suffer from severe mental health problems compared to moderate/normal mental health problems (OR 1.72; p < 0.001). Odds of severe mental health problems was significantly higher among Muslims (OR 1.66; p < 0.001) than Hindus. Thus, it was found that age, gender, education, caste, household economic status, media exposure and religion to be significantly associated with mental health problem. Government should provide a wide range of adolescent friendly health services (clinical, mental, nutritional, including counselling) in these slums for a better adolescent health outcome. Purpose: To assess the dietary inadequacies of micronutrients and the associated factors among the apparently healthy urban adults. Methods: This community-based cross-sectional study involved 300 urban adults (distributed into age groups: 21–40, 41–60, and > 60 years) residing in Hyderabad city, South India. Hemoglobin in whole blood, ferritin, folate, and vitamin B12 (B12) in plasma was estimated. Dietary intakes were assessed by three 24-h dietary recalls and calculated the probability of adequacy (PA) using estimated average requirement. Results: The prevalence of anemia (30%), iron deficiency (ID, 23%), and iron deficiency anemia (IDA, 14.3%) was independent of age but higher in women. While folate deficiency (32.2%) was independent of age and gender, B12 deficiency (35.5%) varied by both age and gender. The PA of iron (89%) was higher, while that of folate, B12, and zinc (1–11%) were noticeably low. The mean PA (MPA) across the ten micronutrients was 38%, independent of age and gender, but associated with the educational status. Energy intake was a strong predictor of the MPA. Cereals and millets predominantly contributed to the intake of thiamine, niacin, zinc, and iron; green leafy vegetables and fruits to vitamins A, C, folate, and iron; animal foods to B12; and milk and milk products to calcium, vitamin A, riboflavin, and B12. The unadjusted and adjusted logistic regression models revealed that micronutrient inadequacy was associated with greater risk of IDA and folate deficiency. Conclusions: These results indicate a higher prevalence of micronutrient deficiencies among the healthy urban adults possibly due to the inadequacy of multiple micronutrients. [ABSTRACT FROM AUTHOR]

Objective: To determine the normative data for inferior vena cava (IVC) diameter in children and its correlation with various somatic parameters like height, weight and body surface area in Indian children. Readily available baseline data of IVC diameter in normal children shall be of great help in rapid assessment of variations in sick children.Methods: Total 475 healthy children aged one month to 12 y visiting out patient clinics (OPD's) were enrolled in this study. Weight, height and body surface area were calculated at the time of examination. The maximum and minimum diameters of IVC were measured during the expiratory and inspiratory phase of the respiratory cycle respectively using M mode ultrasonography. Collapsibility Index was also calculated for each subject by measuring difference between the maximum (expiratory) and minimum (inspiratory) IVC diameters divided by the maximum diameter.Results: The mean age of study subjects was 4.72 ± 3.72 y. Out of 475 subjects, 285 (60%) were boys and 190 (40%) were girls. Mean weight for age (%) of subjects was 89.18 ± 13.26%. Correlation was studied between physical parameters and IVC diameter which revealed a positive correlation of age, height and weight with both maximum and minimum IVC diameter. Regression analysis was also performed to derive the equations for maximum and minimum diameters of children from 1 y to 12 y.Conclusions: This study provides reference values of IVC diameters for Indian children of different age groups. Premature mortality from current ambient fine particulate (PM2.5) exposure in India is large, but the trend under climate change is unclear. Here we estimate ambient PM2.5 exposure up to 2100 by applying the relative changes in PM2.5 from baseline period (2001–2005) derived from Coupled Model Inter-comparison Project 5 (CMIP5) models to the satellite-derived baseline PM2.5. We then project the mortality burden using socioeconomic and demographic projections in the Shared Socioeconomic Pathway (SSP) scenarios. Ambient PM2.5 exposure is expected to peak in 2030 under the RCP4.5 and in 2040 under the RCP8.5 scenario. Premature mortality burden is expected to be 2.4–4 and 28.5–38.8% higher under RCP8.5 scenario relative to the RCP4.5 scenario in 2031–2040 and 2091–2100, respectively. Improved health conditions due to economic growth are expected to compensate for the impact of changes in population and age distribution, leading to a reduction in per capita health burden from PM2.5 for all scenarios except the combination of RCP8.5 exposure and SSP3. Objectives: To define the recognition, age at admission, referral time and referral pattern of neonatal cholestasis in India.Methods: This prospective, observational study was conducted from February 2015 through March 2016 in the Pediatric gastroenterology unit of JIPMER, Pondicherry in infants with cholestasis < 6 mo of age.Results: Among 64 infants, median age of admission was 52 d (IQR 28-63 d). Fifty of sixty four infants (78.1%) came with parent-reported cholestasis-related symptoms of either jaundice alone (57.8%) or bleeding manifestations (20.3%). In 21.9% infants, jaundice was detected by physicians at a median age of 45 d (IQR 38.5-53.2 d). Two infants had intracranial bleed. Only 34% infants with pale stools were identified by the mother. The median healthcare-seeking time was 5.5 d (IQR 2.5-12 d). Among infants presenting to primary healthcare physicians (PHPs) with cholestasis-related symptoms, median time to referral was 5 d (IQR 2.5-12 d). The first point of healthcare contact in 54.7% was a PHP; 17.1% PHPs had reassured the parents. Herbal preparations were prescribed by 14.3%. Only 11.8% of those with jaundice as the only problem were given vitamin K before referral. Biliary atresia (BA) was missed in neonatal intensive care units in 9 cases.Conclusions: The above issues need to be accounted for before evaluating or implementing screening strategies in India. We examine several methodological considerations when eliciting probabilistic expectations in a developing country context using the Longitudinal Ageing Study in India (LASI). We conclude that although, on average, individuals are able to understand the concept of probability, responses are sensitive to framing effects and to own versus hypothetical-person effects. We find that overall, people are pessimistic about their survival probabilities compared with state-specific life tables and that socioeconomic status does influence beliefs about own survival expectations as found in previous literature in other countries. Higher levels of education and income have a positive association with survival expectations, and these associations persist even when conditioning on self-reported health. The results remain robust to several alternative specifications. We then compare the survival measures with objective measures of health. We find that activities of daily life, height, and low hemoglobin levels covary with subjective expectations in expected directions. [ABSTRACT FROM AUTHOR]

Despite being newcomers, immigrants often exhibit better health relative to native-born populations in industrialized societies. We extend prior efforts to identify whether self-selection and/or protection explain this advantage. We examine migrant height and smoking levels just prior to immigration to test for self-selection; and we analyze smoking behavior since immigration, controlling for self-selection, to assess protection. We study individuals aged 20-49 from five major national origins: India, China, the Philippines, Mexico, and the Dominican Republic. To assess self-selection, we compare migrants, interviewed in the National Health and Interview Surveys (NHIS), with nonmigrant peers in sending nations, interviewed in the World Health Surveys. To test for protection, we contrast migrants' changes in smoking since immigration with two counterfactuals: (1) rates that immigrants would have exhibited had they adopted the behavior of U.S.-born non-Hispanic whites in the NHIS (full "assimilation"); and (2) rates that migrants would have had if they had adopted the rates of nonmigrants in sending countries (no-migration scenario). We find statistically significant and substantial self-selection, particularly among men from both higher-skilled (Indians and Filipinos in height, Chinese in smoking) and lower-skilled (Mexican) undocumented pools. Objective: To study the bias in seeking medical care for female child in various age groups from birth to adolescence.Methods: It is a retrospective analysis of the hospital records for the period January 2010 through December 2012. The sex ratio was calculated for different age groups including newborns delivered, patients attending OPD (0-12 y), children attending immunization centre (0-5 y), patients admitted in pediatric wards segregated in different age groups i.e., < 1 y, 1-4 y, 5-9 y and 10-14 y. Chi square test was used to find out if there was any variation in the sex ratio of patients attending the hospital against the sex ratio in the community.Results: The mean sex ratio of patients (0-12 y) attending the pediatric OPD was 726 which is significantly lower than the sex ratio in the community (p < 0.00001). The sex ratio of patients (0-5 y) visiting immunization centre (846) was also significantly lower than sex ratio in the community (p = 0.0343). Among children delivered in the hospital mean sex ratio at birth was 934 against a sex ratio of 920 at birth in the reference population. The sex ratio of admitted patients was significantly lower in age groups <1 y (617, p value < 0.00001), 1-4 y (665, p value <0.00001), 5-9 y (665, p value <0.05). But the sex ratio improved for inpatients in the age group 10-14 y (794, p = 0.08).Conclusions: There is a strong bias against females for seeking medical attention both for outpatient and inpatient care. There are not many injuries that rival the injured hand in complexity. A better understanding of biologic, behavioral, and socioeconomic risk factors potentially associated with hand injuries can help identify those individuals most at risk and define potential preventative measures to help reduce the incidence. We present a prospective study of 436 consecutive patients of hand and forearm injury treated over a period of 2 years. A serial recording of the demographic profile of the patient along with the type & cause of injury sustained, hand dominance, duration of hospital stay, time lag between injury and admission, type surgery preformed with intra-operative findings and the cost analysis was done. An expected male dominance in economically viable individuals of 21 to 30 years formed 50 % of the patients of which 22.9 % were labourers and students each. The malady was altercation (27.5 %) followed by industrial & road accidents. Post prandial period was most notorious with multiple neuro (27.05 %) vasculo (39.34 %) tendinous (60.66 %) injury common with even simple lacerations. Dominant hand injury was commonest. It is challenging to assess and treat an injured hand. This study defines the demography and the etiology behind the various cases of hand and forearm injury with the detailed trauma profile. The limitation of the study was absence of functional outcome. The necessity of hand trauma registry is a pre-requisite to quantify the burden of hand injuries and formulate a prevention strategy. Alleviating the agonizing pain of critical limb ischemia (CLI) in patients of Buerger's disease (BD) has been challenging, due to lack of definitive treatment; 'Heparin-Dextran' infusion has been tried in this study. Assessment of clinical improvement and vascular changes following therapy. Patients with CLI admitted to emergency surgical ward were studied prospectively. BD was diagnosed by Shionoya's criteria, and confirmed by digital subtraction angiography (DSA). Heparin and Dextran intravenous infusion was administered for 10 days. Severity of rest pain, ischemic changes in the feet, claudication distance and ankle brachial index (ABI) were estimated prior to therapy, at completion and 3 weeks after therapy. Vascular changes were assessed by CT angiography (CTA) performed prior to and 3 weeks after therapy. Twenty consecutive patients were studied. Successful hemodilution reflected by decreased hematocrit (37.4 % to 32.6 %, p < 0.05) and increased mean ABI (0.46 to 0.83, p < 0.01), improved rest pain in 75 % patients (p < 0.001), increased claudication distance in 94 % (p < 0.05) and ulcers healing in 70 % patients. CTA revealed recanalised vessels (decreased length of occluded segments) in 10 (50 %, p = 0.005), increased collaterals in 12 (60 %, p < 0.01) and improved distal run-off in 13 (65 %, p < 0.01) patients. 'Heparin-Dextran' therapy in patients of CLI from BD improves tissue perfusion by increasing collaterals and recanalisation of vessels, resulting in significant relief from rest pain and clinical improvements. Background: Demographic, socioeconomic and cultural changes in India have increased longevity, delayed childbearing, decreased parity and resulted in a more westernised lifestyle, contributing to the increasing burden of cancer, especially among women.Methods: We evaluated secular changes in the incidence of breast, cervical and ovarian cancer in Mumbai women aged 30-64 between 1976 and 2005. Age-standardised incidence rates were calculated and presented by site and calendar period. An age-period-cohort (APC) analysis quantified recent time trends and the significance of birth cohort and calendar period effects. The estimated annual percent change (EAPC) was obtained from the drift parameter, expressing the linear time trend common to both calendar period and birth cohort.Results: Over the 30-year study period, the age-standardised rates significantly increased for breast cancer (EAPC: 1.1% (95% confidence interval (CI): 1.0, 1.3)), significantly decreased for cervical cancer (EAPC: -1.8% (95% CI: -2.0, -1.6)) and there was no statistically significant change for ovarian cancer (EAPC: 0.3% (95% CI: -0.1, 0.6)). For breast and cervical cancer, the best-fitting model was the APC model.Conclusions: The rates of breast, cervical and ovarian cancer remain low in comparison with western countries, and the divergent trends of breast (increasing) and cervical cancer (decreasing) in Mumbai were similar to those observed in several other Asian countries. The changing risk profile in successive generations - improved education, higher socioeconomic status, later age at marriage and at first child, and lower parity - may in combination partially explain the diverging generational changes in breast and cervical cancer in Mumbai in the last decades. Research on marriage in developing countries has been somewhat narrow in scope because of both conceptual and data limitations. While the feminist literature recognizes marriage as a key institutional site for the production and reproduction of gender hierarchies, little is known about the processes through which this relationship operates. This article uses data from the newly collected India Human Development Survey 2005 for 27,365 ever-married women aged 25-49 to explore ways in which different dimensions of gender in Indian society shape the decisions regarding age at marriage. We explore the impact of three dimensions of gender: (1) economic factors, such as availability of wage employment, dowry expectations, and wedding expenses; (2) indicators offamilial empowerment, such as women s role in household decision making and access to and control over resources; and (3) markers of gender performance, such as observance of purdah and male-female separation in the household. Results from hierarchical linear models confirm the importance of markers of gender performance but fail to demonstrate a large role for economic factors and familial empowerment. In Summary, a common killer disease of yesteryears-ICC, has become rare and treatable in early 1990s. The most plausible reason for this is the change in feeding vessels associated with changing life styles. Increased awareness and improved diagnostic facilities have led to identification of other chronic liver disorders such as Biliary Atresia, chronic Hepatitis and Wilsons Disease. The prognosis in these diseases depends on early detection and regular therapy. Substantial differences exist in features of some of these diseases in India as compared to Western descriptions. And finally, though 50% of the original CLD (ICC) can be considered to be almost eradicated, the fight in the understanding and management of the other 50% will have to continue unabated. It is widely known that modern economic development has been accompanied by the initiation and spread of effective limitation of fertility, and that generally the populations which experienced development at a late date also had a belated reduction in childbearing. Here a surprising relation is found between (and within) broad regions: the areas in which traditional age of entry into marriage was late were the areas in which marital fertility was reduced first. This article outlines a method of estimating probabilities of gross transfers from one age to another due to misreporting of age. An essential ingredient in the computation is the information on the true age structure of the population, which may be estimated by using generalized stable population relationships. The method consists essentially of iteratively adjusting rows and columns of an initial guess matrix so that the application of the resultant transition matrix to the true age distribution produces the recorded age distribution. The initial guess matrix can be either empirically based or theoretically derived. The method is illustrated by using data on India, 1971-1981. The application reveals that in India, although the tendency to exaggerate age is strong at adult ages, the bias does not increase appreciably at older ages, as is commonly believed. Period fertility rates show a recent rise in the fertility of the Malay population of Peninsular Malaysia that became evident in the late 1970s, after more than fifteen years of slow but steady declines. Detailed analysis of age-parity-specific fertility rates suggests that the recent rise is largely due to a "making up" of lower order births (first, second, and third) that had been postponed by a rapid increase in age at marriage. The period trend in higher order births and cohort trends of cumulative fertility from census data point to a continued reduction in completed Malay fertility. This analysis has identified several factors contributing to the dramatic decline in infant mortality since World War II in Malaysia, as well as one factor that prevented the infant mortality rate from declining even more rapidly. Our main findings are the following: On average, mothers' education more than doubled over the study period, contributing to the decline in their infants' mortality. In addition, the beneficial effect of mothers' education on infant survival appears to have become stronger over the study period. Hence, further advances in education should lead to further improvements in infants' survival prospects. Another analysis of these data (Peterson et al. 1985) found that education is somewhat more influential in affecting child mortality in low-mortality, high-income areas than in the opposite type of areas. Therefore, socioeconomic development may have complemented, instead of substituted for, the the beneficial effect of mothers' education in promoting infant and child survival in Malaysia. Improvements in water and sanitation also contributed to the infant mortality decline, especially for babies who did not breastfeed. However, unlike education, these influences have become less important over time, especially for babies who are not breastfed. Hence, further improvements in water and sanitation, a goal of Malaysia's Rural Environmental Sanitation Programme, may have smaller relative effects on infant mortality than did previous improvements. Targeting such improvements on areas where women breastfeed little or not at all, however, will increase their effectiveness in promoting infant survival. The substantial reductions in breastfeeding that have taken place since World War II have kept the infant mortality rate in Malaysia from declining as rapidly as it would have otherwise. We estimate that, in our sample, the detrimental effects on infant survival of the decline in breastfeeding have more than offset the beneficial effects of improvements in water and sanitation. Unlike some other researchers (e.g., Palloni 1981), we find that changes in fertility levels and in the timing and spacing of births have had negligible effect in explaining the decline in infant mortality within the samples we have considered. TABRAP (TArget Birth Rate Acceptor Program) is a computer programmed model that provides a direct solution to the problem of determining the total annual numbers of contraceptive acceptors required to achieve a prescribed crude birth rate target path. Applied to an initial population for which age structure, the fertility schedule, and expected trends in life expectancy and age-specific proportions of females married are known, TABRAP incorporates the following factors: age at acceptance, with acceptors drawn from currently married nonusers; age-method-specific attrition rates of users; a potential fertility schedule of acceptors that allows for aging and sterility; and allowance both for postpartum anovulation and nine months for gestation to time properly the averted births. TABRAP generates annual data on acceptors, couple-years of use, births averted and age-specific fertility rates that meet the crude birth rate target. Resulting changes in population size, age structure and crude vital rates, also yielded, are invariant with respect to acceptor age and method mix. Assuming a target to reduce the crude birth rate from 45 to 30 in ten years, TABRAP is illustrated for seven mixes of acceptor age-method combinations applied to a population approximately that of Thailand, circa 1965. Using the 1970 1/1,000 Public Use Sample of the U.S. population, the relationships of the closed and open birth intervals with parity and mother's age, with parity and marital duration, and with children ever born/marital duration and mother's age are analyzed. These findings both replicate and expand on Srinivasan's earlier research on the open interval. This study is one of the few multivariate analyses of the relations of several demographic and socioeconomic variables to fertility and the use of contraception. For fertility, 56 of the 92 hypothesized paths are found to be significant at the .05 level or better. The five variables having a significant and direct effect on fertility, as shown by their path coefficients (p), are: duration of marriage (p = .721), spouse's cohort (p = -.093), spouse's age at marriage (p = .052), caste (p = -.071), and number of siblings of husband (p = .050). The use of contraception is affected by, in order of importance, the spouse's education (p = .267), the husband's education (p = .099), the husband's income (p = .089), and surplus children, i.e., number of living children exceeding ideal number of children (p = .059). Child mortality, which is linked to number of living children and thus indirectly to surplus children, is affected by, in order of importance, the number of children ever born (p = .723), the husband's education (p = -.166), the spouse's absence by death or separation (p = .084), and family structure (p = .035).
