This report details the use of zygomatic oncology osseointegrated implants to support and retain a maxillary obturator in a 13-year-old male patient who underwent a right-sided hemi-maxillectomy (Brown Class 2b) (Brown and Shaw, Lancet Oncol 11:1001–8, 2010) for a myxoid spindle cell carcinoma. At the time of maxillary resection, two zygomatic oncology implants were inserted into the right zygomatic body and subsequently utilised to provide in-defect support and retention for a bar-retained maxillary acrylic obturator prosthesis, which restored the patient’s aesthetics and function to a very high level. Close follow-up over 2 years demonstrated ongoing excellent function and disease control with no deleterious effects on facial or dento-alveolar growth clinically. This is the first clinical report of its kind in the published literature detailing the use of a zygomatic implant-retained obturator in a paediatric patient.

The first published clinical report of tissue expansion was by Neumann in 1957 [1]. Neumann used a subcutaneous rubber balloon to achieve the expansion of an area of the scalp for ear reconstruction. Later, Radovan [2] used a sophisticated silicone implant for breast reconstruction. Following some clinical and experimental studies [3, 4], tissue expansion has been accepted as one of the routine procedures in reconstructive surgery.

The development of the zygoma implants presents a unique alternative for rehabilitating the severely resorbed maxilla, with only one surgical procedure and without the need for grafting. However, the placement of zygoma implant is not without risk due to the anatomically complex operation sites. In this paper, an image guided oral implantology system (IGOIS) is presented to transfer the preoperative plan accurately to the operating theatre. With the fabrication of a polymer resin template, a non-invasive point-based registration method is realized, so that all the preoperative determined implant sites can be drilled carefully under the guidance of 2D/3D interactive image rendering environment. The phantom experiment shows that the average distance deviations at the coronal and apical point of the implant are respectively 1.36±0.59 mm and 1.57±0.59 mm, while average angle deviation between the axes of the planned and the actual implant is 4.1°±0.9°.A clinical report of the application of IGOIS for a patient with a severely atrophic maxilla demonstrates that it is an accurate method for transferring the planning of a complex osteotomy to the actual surgical procedure.

Carter and Eban [1] were the first to report bilateral abnormality of the popliteal artery and gastrocnemius muscles, in 1962. The term popliteal entrapment was then coined in 1965 by Love and Whelan [6], the same year that Hamming and Vink [3] published their clinical report of popliteal artery obstruction at an early age. Popliteal artery entrapment is characterized by extrin- sic compression of the popliteal artery, while in adven- titial cystic disease the artery is compressed by a cyst located in the adventitia of the artery.

Clinical evaluation of a computer-assisted spine surgery system is presented. 40 pedicle screws have been inserted in lumbar and thoracic vertebrae in different types of disorder including scoliosis, spondylolisthesis, and iatrogenic or degenerative instability. 20 screws were inserted with a computer-assisted system while 20 were inserted manually. Surgery was followed by post-operative X-ray and CT examination, on which measurements of screw position relative to pedicle could be done. Comparison between the two groups show that the computer-assisted technique is much more accurate and safe than manual insertion.

The application of hyperthermia as a method of improving radiation response in tumors has been explored for more than 80 years. Probably the first clinical report stemmed from Muller, who described the potential of local diathermy as an adjuvant to radiotherapy (Muller 1910). This was followed by sporadic reports which enthusiastically suggested a likely improvement of the effect of radiation on various superficial tumors. However, it was not until the mid-1970s that the principle of combined heat and radiation was explored in a more scientific manner. This led to the publication of numerous uncontrolled studies, almost all suggesting that adjuvant hyperthermia improves the response to radiation (Overgaard 1989a)

This report presents the experience in a prospective series of 100 operative procedures in all fields of paediatric surgery with the Shaw haemostatic scalpel, which we have subsequently used in more than 3000 further operations. The Shaw scalpel proved to be advantageous in about 80% of major cases. The scalpel cuts tissue with a sharp steel edge, like a cold scalpel, and simultaneously seals blood vessels by heat thermally conducted to the tissue from heated blade which is electri¬cally insulated from the patient. The heat seals most small blood vessels (under 2 mm) as they are cut. Since no electric current passes through the patient, a grounding pad is not needed and the risk of accidental electrical current burns at grounding sites is eliminated. Muscle stimulation as¬sociated with the use of a normal cautery is avoided, improving surgical precision of cutting. The Shaw haemostatic scalpel minimizes damage to the tissue as compared with other ther-mocoagulating instruments. Since it seals small vessels as it cuts tissue, it largely eliminates the flow of blood into the incised area and allows better visibility of the surgical field. The use of the scalpel requires a different cutting technique which is however easy to learn. The Shaw haemosta¬tic scalpel reduces blood loss and overall operating time in major cases. It is relatively inexpensive and can be recommended for use in paediatric surgery.

The demand for meniscus replacement arises when the cruciate ligament substitute material does not have protection for sufficient and permanent stabilization owing to the lack of a breaking action of the inner meniscus dorsal horn in the absence of the anterior cruciate ligament and after medial meniscectomy. Consecutive loosening of the cruciate ligament replacement is the logical consequence. Preliminary animal experiments were carried out in terms of these clinical criteria.

The first clinical report on the effects of hyperthermia on cancer, the very earliest concerning this action of heat, was furnished bv BUSCH (1) in 1866.

Three optical mark reader (OMR) forms are used to store and process clinical and haemodynamic data. One OMR form records details of the cardiac catheterisation in summary form and is processed to produce an audit of catheter laboratory performance and an extract by which all cases having the same criteria can be identified for clinical research. A second OMR form records the symptoms, certain important points in the history, the physical signs and the chest x-ray, electrocardiographic (ECG) and echocardiographic findings. A third OMR form records the results of special investigations aimed at identifying cardiovascular and respiratory pathology. From these last two OMR forms the computer generates a clinical report for distribution to colleagues and the case notes. This report has given a significant saving of secretarial and medical time compared to previous methods and is acceptable to those who receive it.
