The intense explosive and effusive volcanic activity of the last 1000 years at La Fossa volcano (Vulcano Island, Italy) was characterized by the eruption of magmas ranging in composition from latites to trachytes and rhyolites, as well as K-rich trachytes. Evidence of syn-eruptive mixing among these magmas is frequently observed in the form of magmatic enclaves and bands in lava flows and pyroclastic products. The petrological and volcanological diversity of the erupted materials suggests that complex differentiation processes occurred in the shallow part of the plumbing system. With the aim to reconstruct the magmatic feeding system and to identify the differentiation processes behind such a petrologic complexity, we analysed lavas and pyroclastic products representative of the recent eruptive sequences at La Fossa and combined the petro-chemical features with thermo-barometric calculations, geochemical modelling and temperature gradient experiments. Thermo-barometric calculations indicate that the K-rich trachytic magma crystallized at lower pressure (160 ± 54 MPa) compared to the latitic (307 ± 47 MPa) and trachytic (208 ± 30 MPa) magmas. Differentiation modelling suggests that both trachytic and rhyolitic compositions can be obtained through differentiation of a common latitic magma, essentially by varying the plagioclase/sanidine ratio. Temperature gradient experiments, performed at the conditions inferred for the shallow plumbing system of La Fossa volcano (150 MPa and 1050-900 °C), indicate different paths of melt differentiation that overall produce an increase of the SiO.sub.2/K.sub.2O ratio with the increasing H.sub.2O in the system (from 0 to 4 wt.%). This is consistent with the origin of K-rich trachytes at lower pressure and lower H.sub.2O content. In turn, the formation of crystal-poor rhyolites is explained by the segregation of the interstitial melt formed in a latitic-trachytic crystal mush, favoured by the second boiling of the melt and consequent exsolution of a fluid phase., Author(s): S. Conventional and SHRIMP U-Pb analyses of zircon, monazite, titanite and apatite from the high grade rocks of the Northampton Complex in Western Australia provide constraints on the timing of metamorphic processes and deformation events in the northern Darling Mobile Belt (western margin of the Archean Yilgarn Craton). Paragneisses and mafic volcanics and/or intrusions have undergone granulite facies metamorphism in a probable extensional tectonic setting prior to formation of W- to NW-verging folds and thrusts cut by normal shears (interpreted as late collapse structures) during the main deformation event ([D.sub.1]). These structures are folded by open to tight folds with NW-striking axial surfaces developed in a second, NE-SW contractional event ([D.sub.2]). Zircons from a mafic granulite provide an age of 1079 [+ or -] 3 Ma attributed to new zircon growth prior to, or at the peak of regional granulite facies metamorphism. Metamorphic monazites extracted from a paragneiss yield an identical age of 1083 [+ or -] 3 Ma. The similarity of ages between zircons from the mafic granulite (1079 [+ or -] 3 Ma) and monazites from the paragneiss (1083 [+ or -] 3 Ma) is interpreted to reflect fast cooling and/or rapid uplift, which is consistent with thrusting of the gneissic units during the first deformation event ([D.sub.1]) associated with the onset of retrograde metamorphism. Granitic activity at 1068 [+ or -] 13 Ma was followed by intrusion of post-[D.sub.2] pegmatite (989 [+ or -] 2 Ma), which constrains the end of metamorphism and associated deformation. Cooling of the complex to about 500 [degrees] C is timed by the apatite age of 921 [+ or -] 23 Ma. SHRIMP U-Pb ages of detrital zircons from a paragneiss sample yield a maximum age of 2043 Ma, with no evidence of an Archean Yilgarn signature. A majority of ages between 1.6 and 1.9 Ga are consistent with derivation from the Capricorn Orogen on the northern margin of the Yilgarn Craton. Younger detrital zircons with 1150-1450 Ma ages, however, indicate an additional source that had undergone early Grenvillian igneous or metamorphic event(s) and also places a maximum age constraint upon deposition. The source of this clastic material may have been from within the southern Darling Mobile Belt or from Greater India (adjacent to the Northampton Complex in Rodinia reconstructions). This study documents an extended Grenvillian history, with basin formation, sedimentation, granulite facies metamorphism, contractional tectonics (two periods with orthogonal directions of shortening) and late pegmatite emplacement taking place between 1150-989 Ma on the western margin of the Yilgarn Craton. Ages recorded in this study indicate that the proposed global distribution of Grenvillian belts during assembly of the Rodinia supercontinent should be reassessed to include the Darling Mobile Belt. Objective: To assess the possible influence of third-order shim coils on the behavior of the gradient field and in gradient-magnet interactions at 7 T and above., Materials and Methods: Gradient impulse response function measurements were performed at 5 sites spanning field strengths from 7 to 11.7 T, all of them sharing the same exact whole-body gradient coil design. Mechanical fixation and boundary conditions of the gradient coil were altered in several ways at one site to study the impact of mechanical coupling with the magnet on the field perturbations. Vibrations, power deposition in the He bath, and field dynamics were characterized at 11.7 T with the third-order shim coils connected and disconnected inside the Faraday cage., Results: For the same whole-body gradient coil design, all measurements differed greatly based on the third-order shim coil configuration (connected or not). Vibrations and gradient transfer function peaks could be affected by a factor of 2 or more, depending on the resonances. Disconnecting the third-order shim coils at 11.7 T also suppressed almost completely power deposition peaks at some frequencies., Discussion: Third-order shim coil configurations can have major impact in gradient-magnet interactions with consequences on potential hardware damage, magnet heating, and image quality going beyond EPI acquisitions., (© 2024. Objective: With modern optimization methods, free optimization of parallel transmit pulses together with their gradient waveforms can be performed on-line within a short time. A toolbox which uses PyTorch's autodifferentiation for simultaneous optimization of RF and gradient waveforms is presented and its performance is evaluated., Methods: MR measurements were performed on a 9.4T MRI scanner using a 3D saturated single-shot turboFlash sequence for [Formula: see text] mapping. RF pulse simulation and optimization were done using a Python toolbox and a dedicated server. An RF- and Gradient pulse design toolbox was developed, including a cost function to balance different metrics and respect hardware and regulatory limits. Pulse performance was evaluated in GRE and MPRAGE imaging. Pulses for non-selective and for slab-selective excitation were designed., Results: Universal pulses for non-selective excitation reduced the flip angle error to an NRMSE of (12.3±1.7)% relative to the targeted flip angle in simulations, compared to (42.0±1.4)% in CP mode. The tailored pulses performed best, resulting in a narrow flip angle distribution with NRMSE of (8.2±1.0)%. The tailored pulses could be created in only 66 s, making it feasible to design them during an experiment. A 90° pulse was designed as preparation pulse for a satTFL sequence and achieved a NRMSE of 7.1%. We showed that both MPRAGE and GRE imaging benefited from the pTx pulses created with our toolbox., Conclusion: The pTx pulse design toolbox can freely optimize gradient and pTx RF waveforms in a short time. Purpose: Type D personality, defined as a combination of social inhibition and negative affectivity, has been associated with poor medication adherence and lower adherence to continuous positive airway pressure in patients with sleep-disordered breathing. Up to this date, the association of patient's personality with adherence with a mandibular advancement device (MAD) has not been studied. The purposes of this study were to examine the association between type D personality and poor adherence to MAD treatment and to examine the impact of type D personality on perceived side effects during this treatment., Methods: Eighty-two patients out of 113 patients with a known baseline type D scale who have started MAD treatment between June 2006 and December 2009 were included. Information about side effects and adherence were collected via a postal questionnaire. Thirty-three patients were using a monobloc MAD and 49 patients were using a duobloc MAD., Results: Forty-five percent of type D patients discontinued MAD treatment, whereas only 15 % of non-type D patients reported treatment discontinuation. The odds ratio for treatment discontinuation was 6.03 (95 % confidence interval 1.22-29.81; p = 0.027) for type D personality, adjusted for age, gender, MAD type (monobloc or duobloc), and decrease in apnea severity. In continuing MAD users, no significant difference in perceived side effects was reported between the personality types., Conclusion: This is the first study to examine the relationship between type D personality and adherence to MAD treatment. Type D patients reported a significantly higher discontinuation rate when compared to patients without type D personality.

Gluten proteins from wheat can induce celiac disease (CD) in genetically susceptible individuals. Specific gluten peptides can be presented by antigen presenting cells to gluten-sensitive T-cell lymphocytes leading to CD. During the last decades, a significant increase has been observed in the prevalence of CD. This may partly be attributed to an increase in awareness and to improved diagnostic techniques, but increased wheat and gluten consumption is also considered a major cause. To analyze whether wheat breeding contributed to the increase of the prevalence of CD, we have compared the genetic diversity of gluten proteins for the presence of two CD epitopes (Glia-α9 and Glia-α20) in 36 modern European wheat varieties and in 50 landraces representing the wheat varieties grown up to around a century ago. Glia-α9 is a major (immunodominant) epitope that is recognized by the majority of CD patients. The minor Glia-α20 was included as a technical reference. Overall, the presence of the Glia-α9 epitope was higher in the modern varieties, whereas the presence of the Glia-α20 epitope was lower, as compared to the landraces. This suggests that modern wheat breeding practices may have led to an increased exposure to CD epitopes. On the other hand, some modern varieties and landraces have been identified that have relatively low contents of both epitopes. Such selected lines may serve as a start to breed wheat for the introduction of 'low CD toxic' as a new breeding trait. Large-scale culture and consumption of such varieties would considerably aid in decreasing the prevalence of CD.

Background: Elderly patients who undergo esophagectomy for cancer often have a high prevalence of coexisting diseases, which may adversely affect their postoperative course. We determined the relationship of advanced age (i.e., > or =70 years) with outcome and evaluated age as a selection criterion for surgery., Methods: Between January 1991 and January 2007, we performed a curative-intent extended transthoracic esophagectomy in 234 patients with cancer of the esophagus. Patients were divided into two age groups: <70 years (group I; 170 patients) and > or =70 years (group II; 64 patients)., Results: Both groups were comparable regarding comorbidity (American Society of Anesthesiologists classification), and tumor and surgical characteristics. The overall in-hospital mortality rate was 6.2% (group I, 5%, vs. group II, 11%, P = 0.09). Advanced age was not a prognostic factor for developing postoperative complications (odds ratio, 1.578; 95% confidence interval, 0.857-2.904; P = 0.143). The overall number of complications was equal with 58% in group I vs. 69% in group II (P = 0.142). Moreover, the occurrence of complications in elderly patients did not influence survival (P = 0.174). Recurrences developed more in patients <70 years (58% vs. 42%, P = 0.028). The overall 5-year survival was 35%, and, when included, postoperative mortality was 33% in both groups (P = 0.676).The presence of comorbidity was an independent prognostic factor for survival (P = 0.002)., Conclusions: Advanced age (> or =70 years) has minor influence on postoperative course, recurrent disease, and survival in patients who underwent an extended esophagectomy. Age alone is not a prognostic indicator for survival. We propose that a radical resection should not be withheld in elderly patients with limited frailty and comorbidity.

Several sympatric lepidopteran species feed on maize plants, and the different components of their species-specific female sex pheromones may play a role in attracting conspecifics and/or deter heterospecific males. In this study, we analyzed the content of Mythimna unipuncta pheromone glands and tested the response of males to components of their own pheromone blend and that of Sesamia nonagrioides in the wind tunnel. Whole pheromone glands, and lures where (Z)-9-hexadecenyl acetate, Z)-11-hexadecen-1-ol or (Z)-9-hexadecenyl acetate + (Z)-11-hexadecen-1-ol were added to the major component, (Z)-11-hexadecenyl acetate, elicited significantly higher responses by M. unipuncta males than lures with main component alone, although the levels varied with concentration. In the field a rather different outcome was observed, as the addition of other compounds found in the female pheromone gland did not improve trap catch over lures with only (Z)-11-hexadecenyl acetate. The addition of (Z)-11-hexadecenal, a compound of the S. nonagrioides pheromone, to (Z)-11-hexadecenyl acetate significantly reduced attraction of M. unipuncta males both in the wind tunnel and in the field, as well as the number of sympatric clover cutworm, Discestra trifolii, under field conditions. The addition of (Z)-9-hexadecenyl acetate, a minor component of the M. unipuncta pheromone blend, reduced the number of S. nonagrioides captured in field traps that were baited with the S. nonagrioides lure. The significance of such inhibition in the reproductive isolation of sympatric species that attack maize is discussed.

Background: We studied the use of (201)Thallium SPECT and L-[1-(11)C]-tyrosine PET in patients with a primary glioblastoma multiforme treated with (192)Ir brachytherapy after surgery and external beam radiation therapy. We hypothesised that the patients most likely to benefit from further surgery after deterioration would be those with radiation necrosis and would be recognised by a negative emission tomography scan., Methods: Twenty-one patients underwent (201)Thallium SPECT performed before brachytherapy, and this was repeated in 19 patients when recurrence was suspected. Nine patients also underwent a PET scan at the same time. Nine patients underwent a second operation., Findings: SPECT and PET were highly concordant concerning the prediction of radionecrosis and/or tumour recurrence. Repeat surgery did not lead to a significant increase in survival. There was no significant association between the duration of survival and tumour-to-background ratio but the number studied was small. Both SPECT and PET showed highly active lesions, which were proved to be recurrent tumour by clinical and histological follow-up., Conclusion: Although PET and SPECT are both highly sensitive in detecting active tumour tissue, emission tomography was not clinically valuable in the investigation of patients with a primary glioblastoma treated with brachytherapy.

Background: To investigate whether STN stimulation is more efficacious than unilateral pallidotomy in advanced Parkinson's disease (PD) one year after surgery., Method: Thirty-four patients with advanced PD were randomly assigned to unilateral pallidotomy or bilateral STN stimulation. Outcome measures were parkinsonian symptoms in off and on phases (UPDRS 3), dyskinesias, functional status, Parkinson's disease quality of life questionnaire, the effects on separate symptoms, timed tests, patient diaries, dopaminergic drugs changes, adverse effects, and global outcome scale. Patients were assessed before surgery, six months and one year after surgery. The primary outcome measure was the off phase UPDRS 3 at six months follow-up., Findings: The off phase UPDRS 3 score improved from 46.5 to 32 points in the pallidotomy patients and from 51.5 to 24 in the STN stimulation patients (p = 0.002). On phase UPDRS 3 and off phase Schwab and England functional scale improved significantly in favour of the STN stimulation patients. Dopaminergic drugs reduction was larger in the STN group although the difference between the treatment groups was not significant. One patient in each group had a major adverse effect., Conclusions: Bilateral STN stimulation is more efficacious than unilateral pallidotomy in advanced PD up to one year after surgery.

Objective: Postoperative radiotherapy is standard treatment for patients with a glioblastoma multiforme (GBM). However, a GBM is radioresistant and almost always recurs, even after a high dose of radiation. A GBM is characterized by its extensive neo-angiogenesis, which can be attributed to the high levels of vascular endothelial growth factor (VEGF). The scope of this study is to investigate the VEGF secretion by GBM cells with different radiosensitivity after irradiation., Methods: Three human GBM cell lines (U251, U251-NG2 and U87) were irradiated with single doses of 0, 5, 10 and 20 Gy of gamma-rays from a (137)Cs source. VEGF levels in medium were measured by ELISA at 24, 48 and 72 h after radiation. Cell survival was measured by the XTT assay 7 days after irradiation., Results: Following single dose radiation, the VEGF levels showed a dose dependent increase in U251, U251-NG2 and U87 glioma cells. Both base-line and radiation-enhanced VEGF levels were about 10-fold higher in U87 compared to U251 and U251-NG2 cells. In addition, in the XTT assay, the U87 was more radioresistant than both U251 and U251-NG2 cell lines (dose modifying factor (DMF) = 1.6 and 1.7 resp)., Conclusion: Irradiation enhanced VEGF secretion in all three tested glioma cell lines (up to eight times basal levels). It is tempting to associate the radiation-enhanced VEGF secretion with an increased angiogenic potential of the tumor, which may be a factor in radioresistance.

Fresh apples can cause birch pollen-related food allergy in northern and central European populations, primarily because of the presence of Mal d 1, the major apple allergen that is cross-reactive to the homologous and sensitizing allergen Bet v 1 from birch. Apple cultivars differ significantly in their allergenicity. Knowledge of the genetic basis of these differences would direct breeding for hypoallergenic cultivars. The PCR genomic cloning and sequencing were performed on two cultivars, Prima and Fiesta, which resulted in 37 different Mal d 1 gDNA sequences. Based on the mapping of sequence-specific molecular markers, these sequences appeared to represent 18 Mal d 1 genes. Sixteen genes were located in two clusters, one cluster with seven genes on linkage group (LG) 13, and the other cluster with nine genes on the homoeologous LG 16. One gene was mapped on LG 6, and one remained unmapped. According to sequence identity, these 18 genes could be subdivided into four subfamilies. Subfamilies I-III had an intron of different size that was subfamily and gene-specific. Subfamily IV consisted of 11 intronless genes. The deduced amino acid sequence identity varied from 65% to 81% among subfamilies, from 82% to 100% among genes within a subfamily, and from 97.5% to 100% among alleles of one gene. This study provides a better understanding of the genetics of Mal d 1 and the basis for further research on the occurrence of allelic diversity among cultivars in relation to allergenicity and their biological functions.

Unlabelled: As the value of grading of ependymomas is currently debated we studied the expression of proliferation- and apoptosis-related proteins in these tumors as these mechanisms both are suggested to be important in tumor growth. We characterized the immunohistochemical expression of p53, Mdm2, Bcl-2, and Bax in 51 intracranial ependymomas. We also assessed the apoptosis- and proliferation-index, measured by MIB-1, PCNA-immunohistochemistry, and analyzed the clinical parameters. Of all used antibodies, the correlation with survival and the correlation among ordered categories was assessed. None of the analyzed immunohistochemical variables were significantly correlated with tumor grade. On the other hand, PCNA, MIB-1, and p53 were significantly related to the survival of the patient. In multivariate analysis, p53 was the only independent predictive variable (p = 0.0132)., Conclusion: The strongest predictors of survival in univariate analysis were the expression of PCNA, MIB-1 and p53. In multivariate analysis a p53 expression > 1% showed to be significantly related with a worse survival. The predicting value of p53 expression has to be confirmed by others before solid conclusions can be made. Apoptosis seems not to be an important mechanism in tumor growth in ependymomas. The expression of Mdm2, Bcl-2, and Bax were not related to survival.

In order to study survival and identify prognostic factors in progressive supratentorial low grade astrocytoma, 46 cases of diffusely growing glioma (1982-1989), in which stereotactic biopsy revealed a diagnosis of astrocytoma grade II according to Kernohan, were evaluated. Only patients with definite tumour growth on consecutive CT scans were included. All patients received a full course of fractionated radiotherapy. All tumour biopsy specimens were re-evaluated according to both the Kernohan and Daumas-Duport grading systems. Follow-up for at least 5 years showed a median survival of 47.5 months (1 year 71%, 5 year 41%). In Cox' multivariate analysis 4 factors turned out to give the best fitting prognostic model for survival: age, duration of symptoms, pre-operative neurological examination and CT-contrast enhancement. Numerical scoring was performed by giving either 1 or 0 for: age > or < 40 years, symptom duration < or > 1 year, presence or absence of focal deficit, and presence or absence of CT-contrast enhancement. Combining these factors in a score ranging from 0-4 resulted in 3 distinct groups with respect to median survival (< 2:66.5 months, 2:40 months, > 2:10 months, P = 0.0015). Although 17 cases with Kernohan grade II were upgraded using Daumas-Duport criteria (13 grade III, 4 grade IV), these cases did not form a separate survival-group. Additional parameters are needed to assess prognosis by neuropathological means, as some "low-grade" tumours are less benign than others, giving rise to a variable natural history.

Expression of CD44, particularly of certain splice variants, has been linked to tumor progression and metastatic potential in a number of different animal and human cancers. Although differential expression of CD44 standard epitopes (CD44s) in human brain tumors has been reported, the expression of CD44 variant exon encoded sequences (CD44v) in primary brain tumors in situ has not been studied in detail. In the present study, the expression of CD44s and CD44v epitopes was analyzed immunohistochemically on frozen sections of primary brain tumors. In addition, the expression of CD44 on cultured glioma cells was investigated by immunofluorescence flow cytometry. The results demonstrate the presence of CD44s epitopes and of CD44 splice variants containing CD44v4, v5 and v10 sequences in various types of brain tumors. A subgroup of highly malignant gliomas showed a strong (focal) expression of CD44v5. CD44v6 was absent in all brain tumors examined. CD44s appeared to be the dominant form of CD44 expressed in primary brain tumors, its expression was not correlated with tumor grade. We envisage that CD44 isoforms, in particular CD44s, may contribute to the invasive character of primary tumors by interacting with hyaluronate, one of the most abundant molecules in the extracellular matrix of the brain.

The techniques of stereotactic biopsy of mass lesions of the brain are described, and the indications are discussed. Since computerized axial tomography has become available the decision to perform a stereotactic biopsy is made more frequently than previously. Stereotactic techniques for neuro-oncology are a continously expanding field of largely diagnostic and sometimes therapeutic prodecures. It now seems likely that most centres concerned with brain tumour patients will make use of stereotactic biopsies in the future. A series of 60 stereotactic biopsies performed in the management of otherwise inaccessible tumours of the brain is presented.

Factors that might have contributed to failure of high cervical percutaneous cordotomy in 23 patients with intractable pain were investigated. Cordotomy failed in 3 patients, 20 had initially good pain relief (87%). True recurrence occurred in 5 patients, 3 of them developed pain elsewhere. Analysis of these three types of failure showed a purely technical cause in 2 cases; other failures or recurrences were due to the underlying disease being not strictly unilateral, or possibly to the existence of other ascending pathways carrying nociceptive impulses apart from the spinothalamic tract. There was no influence of age, sex, type of cancer or previous medication on the result of percutaneous cordotomy. Patients with plexus involvement did better than those with bone metastasis.
