Twenty years have passed since I became a professor in the First Department of Pathology, Hiroshima University School of Medicine. It is my great pleas­ ure that Molecular Pathology o[ Gastroenterological Cancer-Application to Clinical Practice has been published by Springer-Verlag Tokyo to commemo­ rate the 20th anniversary of my professorship. Seeing the academic achievements of our department during these 20 years, I am confident that we could establish a department of oncology to research the pathogenesis of human cancer through systemic application of a variety of molecular techniques. We have demonstrated that the develop­ me nt and progression of esophageal, gastric, and colon cancer require mul­ tiple alterations affecting DNA mismatch repair genes, oncogenes, and tumorsuppressor genes, and that common and uncommon genetic changes exist for esophageal, gastric, and colorectal carcinomas. In addition to these genetic changes, the majority of gastrointestinal cancers express telomerase activity, with overexpression of telomerase RNA, indicating a powerful addi­ tional tool for early detection of gastrointestinal cancer. By transferring these basic observations to the clinic, we now are able to make accurate cancer diagnoses, thus determing the grade of malignancy and patient prognosis. We also can identify patients at high risk for developing cancer and create new therapeutic approaches. In fact, we have routinely implemented a new molecular diagnosis strategy at the Hiroshima City Medi­ cal Association Clinical Laboratory since August 1993.

As human longevity continues to be extended, so will the impact of age-associated dementia on individual lives and society. Alzheimer's disease as the most common cause of dementia in the elderly remains a sentinal problem and its underlying pathology is still poorly understood. Available therapeutic strategies require considerable refinement and the development of new therapeutic strategies need input from basic research. Thus continued efforts are necessary both to understand basic mechanisms of the condition and to achieve more powerfull therapies. This volume brings together the reports of basic scientists and clinical investigators. The chapters provide a spectrum of information valuable for clinicians and scientists. This issue bridges the gap between laboratory work in basic science and the development of urgently needed therapeutic strategies. Areas presented are the molecular and cellular biology of the disease, pathogenetic mechanisms and potential therapeutic targets, genetics, risk factors, strategies of prevention and treatment as well as practical aspects of medical and social care for patients with Alzheimer's disease.

Volume 5 of the series'Advances in Research on Neurodegeneration'is concerned with themes which are currently the focus of intensive research, and in which advances in our understanding of the pathological mechanisms un­ derlying neurodegenerative diseases are expected in the near future. The first section contains five reviews devoted to the various neuroimaging technolo­ gies. The discussion is concerned with the question of whether neuroimaging techniques make it possible to follow the process of degeneration as it occurs, and which methods offer the required sensitivity and quantifiability for this purpose. However, the question needs to be examined of whether, given the physical and chemical limitations of these techniques, even under optimal conditions, anatomical resolution can be improved to the extent that neuro­ degenerative diseases can be diagnosed earlier than currently possible and a confident diagnosis made. The possibilities of using neuroimaging techniques to provide information regarding the effects of neuroprotective or neuroregen­ erative therapeutic strategies, and for correlating the results of neuropsycho­ logical research with imaging data are also discussed. The second section is concerned with the significance of endogenous or exogenous neurotoxins as triggers for neurodegenerative processes that may lead to Parkinsonism. Vulnerability factors, which include such factors as nerve ending sensitivity, the synergistic effects of drugs and the various mechanisms underlying different toxins are discussed.

Neurodegeneration is one of the most important subjects of the investigation now and in the coming 21st century. Alzheimer's disease is the leading cause of dementia in the elderly people and Parkinson's disease is one of the major neurologic disorders with the prevalence between 1 and 2/1 000 population in advanced countries. Many others are suffering from intractable neurodegener­ ative disorders such as amyotrophic lateral sclerosis, Huntington's disease, or spinocerebellar degeneration. No truly effective treatment is available for any of these neurodegenerative disorders except for Parkinson's disease; even in Parkinson's disease, still it is impossible to slow down the disease process with the currently available treatment. It is urgently needed to develop new effective technique to halt or slow down the disease process in each of those disorders. Recent advance in the molecular biological and molecular genetic technique has brought us great progress in the understanding of etiology and pathogenesis of these disorders, but still it is not known how neurons are going to die in these disorders. To explore the question, mutual cooperation and exchange of ideas between basic scientists and clinical peoples are of utmost importance.

The annual research conference for 1996 of the American Institute for Cancer Re­ search was again held at the Loews L'Enfant Plaza Hotel in Washington, DC, August 29 and 30. The topic for this, the seventh in the series, was'Dietary Fat and Cancer: Genetic and Molecular Mechanisms.'Two separate presentations were given as the conference overview.'Fat and Cancer: The Epidemiologic Evidence in Perspective'noted that die­ tary fat can be saturated, largely from animal or dairy sources, or mono- or polyunsatu­ rated, mostly from plant sources. Unlike animal fats, fish contain relatively high levels of protective omega-3 fatty acids. Although the hypothesis that dietary fat is associated with cancer is plausible, the mechanisms involved are reasonable, and many animal studies support the hypothesis, there are many obstacles in any direct extrapolation to humans, in­ cluding imprecise measures of dietary fat intake, variability in individual diets, and spe­ cies variations. Despite these limitations, there is a weak positive correlation between colon cancer and dietary fat intake, but with substantial differences for various ethnic groups. In the case of breast cancer, there is substantial variation among countries and eth­ nic groups, but the overall evidence indicated an association with fat in the diet. Epidemiologic studies of dietary fat and prostate cancer are more consistent and most show a positive relationship. However, it was not clear which types of dietary fat were im­ plicated in the effect.

The general populations are incidentally exposed to a wide variety of xenobiotics as a consequence of the pollution of the environment by industrial and agricultural chemicals. Xenobiotics entering the animal will undergo one or more of the following fate: (a) elimination unchanged, (b) metabolism by enzymes, (c) spontaneous chemical transformation and (d) remain unchanged in the body. The actions of xenobiotics on the body exhibit certain specificity depending upon the compound's chemical structure and reactivity. Since the processes of metabolism change these chemical properties ofaxenobiotic, bewildering number of reactions continue to pose new challenges to toxicologists and pharmacologists. It necessitates periodic and precise revision of the subject. This book contains invited contributions from learned colleagues that offer an excellent survey of and profound insight into the disposition and metabolism of a few environmentally and industrially significant xenobiotics. The topics range from an assessment of drug metabolising enzymes in the liver, DNA damage by reactive oxygen species generated by pesticides, role of NO in liver injury, hepatotrophicgrowth factor in liver regeneration, extracellular matrix in the liver, oncogene expression in liver injury, the hepatocarcinogenesis to oxidative stress and undifferentiated gene expression. Detailed analysis of the validity of liver function tests has been included. Last Chapter addresses the problem of apoptosis, which plays a key role in the signal transduction system of xenobiotics-induced liver injury. The reader should appreciate that overall exposure to this field is expanding at a rapid pace and selections had to be made.

Itis apowerful and frightening representationbut ifone were to substitute the vulture by a surgeon and Prometheus by a patient laying on a properly prepared operating table, the outcomeoftheprocedurewould not differ significantlyfrom that describedbyGreek poets. Yet few of us who work in the field have stopped long enough to ask where this myth originated. Did the poet observe a case of liver regeneration in a human being? Was it brilliant intuition or perhaps, literally, just a'gut feeling'of a poet looking for good rhymes that led to the prediction that livers grow when part of the tissueisremoved? Thisbookdoesnotattemptto solve these historical issues. Itdoes, instead, cover in detail some of the major modem themes of research on liver regen­ eration, injury and repair. As indicated in Dr. N. Bucher's chapter, the modem phase ofexperimental studies on liver regeneration started in 1931 with the publication by Higgins and Anderson of a method to perform a two-thirds resection of the liver of a rat. The technique described has 3 remarkable features: 1) it is highly reproducible, resulting in the removal of 68% of the liver, 2) it has minimal if any mortality, and 3) it consists only of blood vessel ligation and does not involve cutting through or wounding hepatic tissue.

Brain Repair, addresses all relevant issues underlying the mechanisms of brain damage, brain plasticity and post-traumatic reorganisation after CNS lesions. Multiple Sclerosis (MS) is an enigmatic immune mediated disease of the central nervous system that affects about 350,000 individuals in the US, and many more around the world. The mechanism of this disease is largely unknown and there is no cure for it. However, there are several well-characterized experimental animal models that help us understand and speculate about potential mechanisms of pathology in this disease. Many of the experimental therapies designed for this disease rely on testing the drugs in animal models before using it in clinical trials. This book combines for the first time the different experimental models for MS (including immune-mediated and viral) under one roof, and highlights aspects that are different or shared among these experimental models. It's aim is to improve our understanding of this devastating disease and help us think about potential additional therapies for it.
