There has been significant progress in the field of spinal disorders in very young children over the past two decades. However, the treatment of such conditions remains challenging due to the age of the patient, different etiologies, associated morbidities and the progressive nature of the deformity. Depending on the etiology of the deformity, these children are often cared for by multiple specialists including pediatricians, pulmonologists, pediatric orthopedists or orthopedic spine surgeons, pediatric surgeons, pediatric neurosurgeons, neurologists and oncologists. Health professionals in all of the mentioned disciplines are involved in the management of these patients, which is why compiling a comprehensive text that is not limited to orthopedic specialists is essential. Since the second edition of this book was published, there has been a significant body of literature that has been published and advances made in the care of young children with spinal and thoracic deformities. The respiratory system is unique in that it is constantly 2 2 exposed to a barrage of foreign substances from both the internal environment (at any one point in time, approximately one-half of the cardiac output is received by the lungs) and the external environment (with each breath, the respiratory tract is exposed to pollens, viruses, bacteria, smoke, etc). According to the Centers for Disease Control and Prevention, diseases of the res- ratory system were the seventh and eighth leading causes of deaths in children aged 1 to 19 years in 2003 [1]. Dr. George A. Gregory, one of the founding fathers of pediatric critical care me- cine, once estimated that acute respiratory failure accounts for nearly 50% of all admissions to the pediatric intensive care unit (PICU) [2]. In most parts of the world the treatment of adults with surgically correctable abnormalities of the thorax and its contents is provided by specially trained thoracic surgeons. On the other hand, in children pediatric general surgeons usually take on this role because of their training in the surgical care of sick infants and children, and their experience with the management of congenital malformations.
