- Abstract
Background: Attention-Deficit Hyperactivity Disorder (ADHD) is associated with neuropsychological deficits including executive and attentional impairments. The cognitive remediation therapy is a new promising technique designed to improve the neurocognitive abilities damaged in ADHD., Aim: Adapt and apply the cognitive remediation program CRT (Cognitive Remediation Therapy) for children and adolescents with ADHD and to evaluate its effectiveness on attentional and executive abilities., Methods: Children and adolescents with ADHD, diagnosed using the K-SADS-PL questionnaire was involved in the study. The CRT program was administered for an average period of fourteen weeks with a rate of one session per week of 40 minutes each. The Attentional Network Test was administered prior to the intervention and one week after., Results: Of the 30 patients included, 14 had reached the end of the program. Among them, nine patients passed the attentional test post CRT, thus constituting our final sample. Their average age was 9 years. The mean number of sessions performed was 14.5. Post CRT, the mean of patient's response time was found to be shorter (p=0,004) and the frequency of omissions errors was also lesser than that found at the initial assessment. Patients also committed fewer errors in incongruent situations in post program, with a significant improvement of the conflict effect (p= 0.009) signing a better executive control., Conclusions: Cognitive remediation is a promising new modality in the treatment of ADHD. Further research is needed to better document its effects and the optimal conditions required for setting it up.

- Abstract
Unlabelled: Squamous cell carcinoma can occur on a apparently healthy mucosa or be preceded by lesions Chronic called potentially malignant, often asymptomatic, rarely showing the reason for consultation of patients, particularly in our Moroccan background which is characterized by a care difficulty of access and the absence of priority to oral health., Objective: To assess the prevalence of diseases potentially and malignant squamous cell carcinomas, and the role of certain risk factors in the onset of these lesions., Methods: In the consultant population center Dental Consultation and Treatment of Casablanca CHU, we conducted a descriptive survey with 306 patients seen for different patterns between May and July 2010., Results: 64.7% of the population had a defective hygiene. In addition, for 40 patients having an oral lesion mucosa, only 16 viewed for these lesions; the others were diagnosed incidentally. Five of these lesions were carcinomas, 32 white lesions of leukoplakia and 256 lichen planus. 7.2% of the population was smoking which nearly half were alcolo-smoking. The most risk factor according to this study significant is the alcohol and tobacco intoxication., Conclusion: This study confirms the importance of early screening of symptomatic chronic lesions potentially malignant as well as squamous cell carcinoma which allows to improve prognosis.

Background: If all authors agree on the presence of fundamental changes in mood, as the basis of the diagnosis of depression, little importance is accorded to other symptoms that can mislead the diagnosis: in particular, age and cultural influence on depressive symptoms., Aim: Our work aims to describe and assess the frequency of somatic complaints and symptoms of conversion in Tunisian children and adolescents followed for depressive disorders, and to compare, through a study of literature to populations from different cultures., Methods: This work consisted in a retrospective review of children who consulted the Department of Child Psychiatry between 2008 and 2011 and in whom the diagnosis of depressive disorders was retained according to DSM IV criteria. Were searched all the conversion symptoms corresponding to the DSM IV diagnoses of conversion and dissociative disorders, as well as all the somatic complaints reported. Children with organic were excluded in order not to confuse the pain secondary to these disorders from those associated with depression. Also, children with mental retardation, moderate, severe or profound were not included because their difficulties of expression can alter the semiological assessment., Results: 119 children and adolescents aged 6 to19 years were included in the study. The mean age of the population was 11.6 years. Somatic complains were present in 37% of cases. Headaches were mostly represented (27.7%), followed by abdominal pain (14.3%). The presence of somatic complaints was significantly correlated with the presence of a comorbid anxiety (p=0.035). Conversion symptoms were present in 22.7% of cases. They were significantly more frequent among girls (p=0.006) and in children whose father had a low level of education (p=0,013). Among the conversions, fainting were more common in girls (p=0.004), when the father's educational level was low (p=0.001), and when the geographical origin was rural (p=0.016)., Conclusion: Conversion symptoms and somatic complaints are frequent in depressed children.

Background: TINU syndrome is a rare entity known Dobrin disease, combining uveitis mostly bilateral and recurrent with acute tubulo interstitial nephritis (AIN) occurring in young adults. The etiology and pathogenesis of this syndrome remain unknown., Aim: To report a new case of TINU syndrome and to discuss its etiopathogenesis., Case Report: This 33 year old women is hospitalized for exploration of acute renal failure, in a context of deterioration of general condition, associated with a unilateral anterior uveitis. The bronchoalveolar lavage showed lymphoid interstitial alveolitis with CD4 and the renal biopsy found lesions characteristic of AIN. Evolution under corticoïd treatment was favorable., Conclusion: Lymphoid interstitial alveolitis associated with TINU syndrome has been reported only twice before. The discovery of a TINU syndrome must practice bronchoalveolar lavage in search of sub clinical pulmonary involvement.

Background: The periarteritis nodosa (PAN) is a serious necrotizing vasculitis. Healing is, classically, obtained after a long-term treatment using corticosteroids and immunosuppressive agents., Purpose: Reporting the case of a NAP or was spontaneous healing without having recourse to any immunosuppressive treatment., Case Report: We report an observation of a patient aged 27 monitoring for manic-depressive psychosis hospitalized for a fever during the course associated with high blood pressure (hypertension), a weight loss, asthenia and myalgia. Biology was a biological inflammatory syndrome (SIB), a creatinine 115 micromol/l. The survey infectious and immunological balance were negative. The arteriography showed a strongly evocative of the NAP. We have seen no sign of poor prognosis. The recovery was spontaneous after a short period of evolution with an amendment clinical and biological weapons. The patient was put under anti-HTA and reviewed regularly to the consultation., Conclusion: An analogy is drawn between such developments and already reported in some cases of viral PAN. The actual frequency of self-limitting PAN is unknown. The scarcity of cases reported in the literature may in part be attributed to ignorance.

Background: The fast development of New Communication and Information Technologies (NTIC) and their use in increasingly numerous fields in medicine; raised new ethical issues. The aim of this paper is to highlight ethical and deontological guidelines for health professionals and physicians which must guide their use of the NTIC., Method: A research in the Anglophone and French medical literature and on the Internet, using various key words related to ethics and medical computing., Results: Ethical issues are mainly related to the property, the access and the use of the broad data bases of medical records, the legal obligations and ethics which must govern the course of a teleconsultation or a therapeutic remote intervention and the rules which must define the relationship between the teleconsultants and their patients. The use, increasingly frequent, of various expert systems of decision-making and/or of expertise in various clinical settings, also, poses ethical problems relating to the reliability of such systems, the qualifications necessary for their application and the responsibility for their development, maintenance and use. Medical Information available on the Internet, of a very unequal and not controlled quality, raises many questions about the responsibility for the medical professionals in the education for the patients and their warning against sometimes erroneous information published on the Web. The ethical rules of research on the Internet must also be clearly formulated and diffused among the researchers., Conclusion: The general principles in front of an ethical problem related to the use of the NTIC remain based on the interest of the patient, the absence of a harmful effect, the respect of the professional secrecy and the safeguarding of the quality of the care and its conformity to the most recent scientific data.

- Abstract
Algodystrophy syndrome is characterized by pain, vasomotor disorders and/or trophic skin changes, without laboratory signs of inflammation. Among the etiological factors of this syndrome (trauma, cerebrovascular accident, diabetes mellitus...), iatrogenic ones including phenobarbital can be observed. We report a case of phenobarbital induced recurrent bilateral shoulder-hand syndrome in a 67 years old women with hypertension, diabetes, and history of epileptic crisis following ischemic cerebrovascular accidents. The patient recovered after barbiturate withdrawal and treatment with calcitonin. It's necessary to diagnose promptly shoulder-hand syndrome of iatrogenic cause in order to withdraw definitively the responsible drug, to start an effective therapy, at this stage, on pain and bone demineralization and to prevent severe sequels.

- Abstract
The objective of this studies is to review clinical and laboratory features of lupus panniculitis. The authors report 3 cases of lupus profundus from a group of 70 lupus erythematosus. In both cases the lupus panniculitis presented as subcutaneous infiltrated and indurated nodules. The diagnosis was confirmed on clinical, histological and therapeutic data. The evolution is slow and is characterised by regression of the inflammatory lesions with treatment by antimalarial drugs. The lupus panniculitis has generally a favorable prognosis.

- Abstract
Herein, we report a case of a 51 year old man who experienced three ischemic cerebral infarcts in a time of few months. The patient consulted after the third accident. Neurological presentation included pseudobulbar syndrome with a mild cognitive deficit, aphasia, left hemiparesia, hemiasomatognosia and homonymous lateral hemianopsia. Cerebral tomodensitometry and magnetic resonance imaging evidenced large infarcts images involving right middle cerebral artery territory and bilateral borderline zones in the junction of the territories of the middle and posterior cerebral arteries. Ambulatory 24 hours ECG recording (Holter) revealed two hits of non-sustained ventricular tachycardia. Transoesophageal echocardiography conveyed to the diagnosis of hypertrophic cardiomyopathy and displayed the presence of a left auricular thrombus. Anticoagulant therapy and rehabilitation allowed a substantial recovering of the patient's cognitive functions and wasting of the intracardiac thrombus. The clinical features observed in our patient meet the recommended DSM IV diagnosis criteria of vascular dementia, an exceptional complication of HCM. The clinical findings, neuroimagery investigation results, and the chronological link between cerebral attacks and cognitive function deterioration argue for a demential syndrome of vascular origin resulting from multiple embolic infarcts involving medium sized arteries (multi-infarct dementia). The authors emphasize the rarity of such observation. HCM must be considered as a potential cause of embolic stroke and likewise a multi-infarct dementia.

- Abstract
In this study, we are presented the clinical and the aetiologic features of 47 patients having hypercalcemia (total serum calcium > or = 2.7 mmol/l). Our results show a positive significantly correlation between the degree of the hypercalcemia and the severity of the clinical symptomatology. As for to the aetiology, the multiple myeloma, the hyperparathyroid and the neoplastic process constituted 78% of the total aetiology.

- Abstract
The cases of vasculitis associated to malignant tumour are uncommon, their course which is often parallel suggests a direct link between them. We report the observation of periarteritis nodosa associated with a lung cancer discovered in the following of vasculitis; whereas the course is favorable in 83 years old man without particular history. The frequency of neoplasia with vasculitis is estimated between 3 and 8%, and periarteritis nodosa can't be a paraneoplastic syndrome. Lung and colic tumours are the most frequent reported of the solid tumours. The delay of appearance of vasculitis varies from 25 months before to 9 months after cancer diagnosis. There are many factors, either some treatment, the neoantigens involve the formation of immune complexes, lymphokines and some vasoactive substances.
