2. Las medidas de salud pública ante una catástrofe: a propósito del terremoto de Lorca en 2011

The impact of disasters on public health does not always occur immediately. In fact, medium- and long-term effects are often worse than those produced during the acute phase.On May 11th 2011, two seismic events of catastrophic consequences took place in the town Lorca; the first one happened at 5:05 pm (Spanish time) of a magnitude Mw = 4.5, and it was followed by several aftershocks. About two hours later, a stronger earthquake occurred, with a magnitude Mw = 5.1 and an intensity VII on the EMS-98 scale. The earthquakes caused nine deaths, injured about 300 people, and forced 60,000 persons to leave their homes. Three temporary camps were set up around the city, which were unified later.The public health actions and prevention and control measures were analyzed after the catastrophic earthquake. The information for this paper was obtained by interviewing individuals and groups belonging to the Lorca Public Health Service’s staff.The analysis revealed a need to improve procedures and coordination among the different groups. To conclude, the following actions, among others, are suggested: 1) to establish protocols or guidelines to allow a fast identification of risks and also a fast decision-making; 2) to carry out training exercises and drills; 3) to receive emergency and disaster training; 4) a transverse collaboration of the public health team with other Task Groups. El impacto de las catástrofes sobre la salud pública no siempre es inmediato, sino que los efectos a medio o largo plazo son, muchas veces, mayores que los producidos en la fase aguda.El día 11 de mayo de 2011, se produjeron en la ciudad de Lorca dos eventos sísmicos de consecuencias catastróficas; el primero ocurrió a las 17:05 (hora española), de magnitud Mw = 4,5 y fue seguido de varia réplicas. Aproximadamente dos horas más tarde, tuvo lugar un evento símico mayor, de magnitud Mw = 5,1 e intensidad VII en la escala EMS-98. Los terremotos causaron nueve muertos, unos 300 heridos, y 60 000 desalojados de sus hogares. Inicialmente se instalaron tres campamentos provisionales en distintos puntos de la ciudad, que posteriormente quedaron unificados.Se analizan las actuaciones, medidas de prevención y control desarrolladas en materia de salud pública tras el terremoto. La información se obtuvo de entrevistas individuales y en grupos con personal del Servicio de Salud Pública de Lorca.Tras el análisis se ha puesto de manifiesto la necesidad de mejorar las actuaciones y la coordinación, por lo que se concluye con propuestas como: 1) el establecimiento de protocolos o guías que permitan rápida identificación de riesgos y toma de decisiones; 2) realización de ejercicios de adiestramiento y simulacros; 3) recibir formación en materia de emergencias y catástrofes; 4) participación transversal del equipo de salud pública junto a otros Grupos de Acción. O impacto das catástrofes sobre a saúde pública nem sempre é imediato, sendo que os efeitos a médio e longo prazo são, muitas vezes, maios elevados que os produzidos na fase aguda.No dia 11 de maio de 2011, tiveram lugar na cidade de Lorca dois eventos sísmicos de consequências catastróficas; o primeiro ocorreu às 17:05 (hora espanhola), de magnitude Mw = 4,5 e foi seguido de várias réplicas. Aproximadamente duas horas mais tarde, sucedeu um evento sísmico maior, de magnitude Mw = 5,1 e intensidade VII na escala EMS-98. Os terramotos causaram nove mortos, cerca de 300 feridos e 60 000 desalojados. Inicialmente instalaram-se três acampamentos provisórios em diferentes pontos das cidades, que posteriormente foram unificados.Analisaram-se as ações e as medidas de prevenção e controlo desenvolvidas na área da saúde pública apóso terramoto. A informação foi obtida através de entrevistas individuais e em grupo a profissionais do Serviço de Saúde Pública de Lorca.A análise dos dados revelou a necessidade de melhorar o desempenho e a coordenação, pelo que se conclui com as seguintes propostas: 1) estabelecimento de protocolos ou diretrizes que permitam a rápida identificação de riscos e tomada de decisões; 2) realização de exercícios de treino e simulacros; 3) receber formação na área de emergências e catástrofes; 4) participação transversal da equipa de saúde pública junto de outros agentes de proteção civil.
