There is a paucity of data on how racial disparities may affect early outcomes following shoulder arthroplasty. The purpose of this study was to evaluate differences in 30-day complications and readmission rates after shoulder arthroplasty based on race. White and black patients who underwent hemiarthroplasty, anatomic or reverse total shoulder arthroplasty (Current Procedural Terminology codes 23470 and 23472) between 2006 and 2015 were identified in the American College of Surgeons National Surgical Quality Improvement Program database. Black patients were propensity score matched 1:4 based on preoperative demographics and comorbidities to white patients. Multivariable analysis was performed to assess postoperative complications based on race. Of the 12,663 patients with shoulder arthroplasty identified, 10,717 (84.6%) were white and 559 (4.4%) were black. Overall, 557 black patients were matched to 2228 white patients, for a total cohort of 2785 patients (mean age, 63.9±11.7 years; female, 61.0%). Surgical indications were similar between black and white patients. The 2 races had similar rates of overall complications, major complications, minor complications, readmissions, and discharge to facility. Mortality was significantly higher among black patients compared with white patients (0.6% vs 0.05%; P=.033). Black patients also experienced longer operative time (mean, 126.4 vs 112.5 minutes; P<.001) and length of stay (mean, 2.4 vs 2.1 days; P<.001). There was a significant disparity with underutilization of shoulder arthroplasty for black patients in the American College of Surgeons National Surgical Quality Improvement Program database. Black and white patients undergoing shoulder arthroplasty experienced similar rates of 30-day complications, readmissions, and discharge to facility. However, black patients experienced greater operative time, total length of stay, and mortality compared with white patients. Reverse total shoulder arthroplasty (rTSA) has become increasingly popular since its introduction to the United States. The purpose of this study was to assess the current trends and use of rTSA, anatomic total shoulder arthroplasty (aTSA), and hemiarthroplasty (HA) from 2011 to 2014. Shoulder arthroplasty data from the National (Nationwide) Inpatient Sample database were analyzed for the years 2011 to 2014 using International Classification of Diseases, Ninth Revision, Clinical Modification procedure codes. For each procedure, use and patient and hospital characteristics were identified. Shoulder arthroplasties increased by 24% between 2011 and 2014, to 79,105 procedures. The proportion of arthroplasties that were aTSA did not change substantially (44% for both years; P=.0585), while the proportion that were rTSA surpassed aTSA in 2014, increasing from 33% to 46% (P<.0001). Use of rTSA topped use of aTSA by 2013 for Medicare patients. The proportion that were HA procedures declined from 23% to 11% (P<.0001). The use of rTSA for fracture increased from 26% to 58% (P<.0001) of all arthroplasties for this indication, while the use of HA for fracture decreased from 69% to 40% (P<.0001). Orthopedists performed rTSA more often than aTSA for Medicare patients by 2013 and the general population by 2014. The use of rTSA for fracture has grown significantly, with rTSA being performed more frequently than HA for this indication. The Latarjet procedure is an established and effective option for the treatment of recurrent anterior shoulder instability. Symptomatic compression of the vasculature around the shoulder and adjacent brachial plexus is uncommon and may be difficult to diagnose and treat. The purpose of this report is to describe a patient with neurovascular compression of the axillary artery and brachial plexus after an open Latarjet procedure. This is the first known report of documented combined vascular and neurologic thoracic outlet syndrome after a Latarjet procedure. Evaluation of this suspected problem requires a detailed clinical examination and a dynamic angiogram to verify which neurovascular structures are compressed. Treatment includes decompression of the brachial plexus and axillary vasculature by releasing tethering scar tissue or the remaining pectoralis minor that is creating a constricting sling effect. An arthroscopic approach provides for a careful and specific decompression. A 21-year-old active duty soldier presented with right shoulder pain and prominence over his right clavicle, with no history of trauma. He recalled that the deformity had been present for a long time, but only recently became painful. The onset of shoulder pain coincided with the beginning of his military service and the requirement to wear over-the-shoulder equipment such as back packs and load-bearing equipment. Physical examination revealed a prominence over the right midshaft clavicle with tenderness to palpation, full active range of motion of the shoulder, and that he was neurovascularly intact., (Copyright 2014, SLACK Incorporated.)

The use of bone tamps for indirect reduction of depressed articular segments is an established method of treatment for intra-articular fractures in a variety of joint injuries. Customized bone tamps can be fabricated intraoperatively using commonly available instruments and supplies consisting of Steinmann pins and T-handled chucks. The technique combines the use of bone tamps through carefully created metaphyseal windows, fluoroscopic guidance, packing with cancellous bone, and adequate fixation. This treatment methodology can allow for a minimally invasive or soft tissue preserving approach for the treatment of some intra-articular fractures while achieving anatomic reduction of the joint surface., (Copyright © 2011, SLACK Incorporated.)

The purpose of this study was to determine if the use of screw hole inserts in empty locking screw holes improves the strength and failure characteristics of locking plates. Twenty 5-hole 1/3 tubular locking plates (Synthes, Paoli, Pennsylvania) were mounted on an oak dowel with a 1-cm gap simulating a fracture with comminution and bone loss. Ten of the 1/3 tubular plates had a screw hole insert placed in the center hole (centered over the simulated fracture), while 10 of the 1/3 tubular plates remained empty in the center hole. The plate-dowel constructs were placed in an Instron 8800 Material Testing Machine and subjected to a series of loading conditions, replicating physiologic loading. The torsional and axial stiffness of each plate-dowel construct was calculated. All plates were then loaded to failure. No significant differences were found in the mechanical properties of the 2 plate constructs. Both the filled screw-hole plate constructs and unfilled screw hole plate constructs demonstrated the same torsional and axial stiffness, before and after being subjected to a combined cyclic and axial torsional load. Additionally, there was no significant difference in ultimate compressive strength or load to failure. Locking plate technology is a relatively new innovation in orthopedic fracture fixation. The evolution of new and varied applications and implants continues. Persistent, fundamental questions exist concerning the basic locking plate design. This study demonstrates that the addition of screw hole inserts does not significantly change the stiffness, torsional strength, or axial loading strength of 1/3 tubular locking plates., (Copyright 2011, SLACK Incorporated.)
