Objective To identify differentially expressed genes (DEGs)in membranous nephropathy (MN)based on microarray data, and to find new potential gene targets for the diagnosis and treatment of MN. Methods The GSE108109 and GSE115857 datasets were downloaded from the Gene Expression Omnibus (GEO)database. The DEGs between healthy controls and MN patients were screened by the GEO2R web tool. By using the David database, DEGs were enriched by Gene ontology (GO)and Kyoto Encyclopedia of Genes and Genomes (KEGG)pathway. Then, the proteinprotein interaction (PPI)network of DEGs was constructed and visualized by using the String tool and Cytoscape software, and key genes were identified by the Degree analysis. The relationship between expression of key genes and clinical features of MN was analyzed by using nephonseq V5 online platform. Results A total of 240 DEGs were screened out, of which 149 were up-regulated and 91 were down-regulated. DEGs involved many functions and expression pathways, such as cell morphology regulation and cytoskeleton construction. In PPI network, 13 key genes were identified, included tumor protein p53 (TP53), phosphoinositide-3-kinase regulatory subunit 1 (PIK3R1), KIT proto-oncogene, receptor tyrosine kinase (KIT), quiescin sulfhydryl oxidase 1 (QSOX1), laminin subunit beta 1 (LAMB1), SHC adaptor protein 1 (SHC1), cyclin dependent kinase 2 (CDK2), apolipoprotein L1 (APOL1), nestin (NES), protein tyrosine phosphatase non-receptor type 6 (PTPN6), RNA polymerase II subunit A (POLR2A), protein kinase cAMP-activated catalytic subunit alpha (PRKACA)and latent transforming growth factor beta binding protein 1 (LTBP1). Among them, the expression of PRKACA (r=0. 858, P=0. 006)and LTBP1 (r=0. 799, P=0. 017)was positively correlated with urine protein, and the expression of POLR2A was negatively correlated with urine protein (r=-0. 601, P=0. 008). PRKACA gene expression was negatively correlated with serum creatinine, and POLR2A expression was positively correlated with serum creatinine (r=0. 583, P=0. 011). Conclusions In this study, we find 240 DEGs and 13 key genes by bioinformatics analysis of renal tissue microarray data. Objective To explore the optimal subsequent therapeutic regimen for newly diagnosed patients with type 2 diabetes mellitus ( T2DM ) after short-term intensive insulin therapy with insulin pump. Methods Eighty-four newly diagnosed T2DM patients with up-to-standard blood glucose after short-term intensive insulin therapy with insulin pump were selected. They were randomly divided into the observation group and control group，with 42 cases in each group. Patients in the control group were administered oral hypoglycemic agents for subsequent treatment， while patients in the observation group received subcutaneous injection of basal insulin for subsequent treatment. The fasting blood glucose ( FBG )，glycosylated hemoglobin ( HbA1c ) and fasting C peptide ( FCP ) were detected before treatment and after 2-year follow-up. Moreover， the BMI， insulin secretion index ( HOAM-β ) and insulin resistance index ( HOAM-IR ) were calculated. Meanwhile， the carotid intima-media thickness ( IMT ) was detected on the same day，and the carotid plaque detection rate was also calculated. Results HbA1c and BMI in both groups after treatment were lower than those before treatment ( all P < 0. 05 )， but the differences after treatment between these two groups were not statistically significant ( all P > 0. 05 ). HOMA-IR in both groups after treatment was lower than that before treatment，while HOAM-β was higher than that before treatment，and the changes in the observation group were more significant ( all P < 0. 05 ). Carotid IMT in the control group after treatment was higher than that before treatment ( P < 0. 05 )，but the difference in the observation group before and after treatment was not statistically significant ( P > 0. 05 ). In addition， carotid IMT in the observation group after treatment was lower than that in the control group after treatment ( P < 0 . 05 ). Differences in carotid plaque detection rate between two groups before and after treatment were not statistically significant ( all P > 0 .05 ). Conclusion Subcutaneous insulin injection in newly diagnosed T2DM patients after short-term intensive insulin therapy with insulin pump is more effective in improving the pancreatic function than the subsequent treatment with oral hypoglycemic agents. 目的分析甲状腺功能亢进症(简称甲亢)患者并发肺动脉高压的危险因素。方法选取126例甲亢患者,其中单纯甲亢患者86例、甲亢合并肺动脉高压患者40例,采用χ2检验进行单因素分析比较两者性别、年龄、甲亢病程、BMI、吸烟史、饮酒史、左心室射血分数(LVEF)、心率、血清总三碘甲状腺原氨酸(TT3)、血清总甲状腺素(TT4)、甲状腺激素(TSH),采用logistic回归对甲亢并发肺动脉高压危险因素进行多因素分析。结果单因素分析结果显示,单纯甲亢患者与甲亢合并肺动脉高压患者性别、年龄、饮酒、LVEF、TT3比较P均<0.05。多因素logistic回归分析显示,甲亢患者并发肺动脉高压的相关因素有年龄(OR=1.068,95%CI:0.862~0.974,P<0.01)、饮酒(OR=0.721,95%CI:1.189~2.135,P<0.05)、LVEF(OR=0.911,95%CI:1.035~1.672,P<0.05)。结论高龄、饮酒、左心室功能减弱是甲亢并发肺动脉高压的危险因素。 [ABSTRACT FROM AUTHOR]
