Individuals with chronic pain commonly report significant functional impairment and reduced quality of life. Despite this, little is known about psychological processes and mechanisms underpinning enhancements in well-being within this population. The study aimed to investigate whether (1) increased levels of pain intensity and interference were associated with lower levels of mental well-being, (2) increased positive goal engagement was associated with higher levels of mental well-being and (3) whether the relationships between pain characteristics and mental well-being were mediated by increased positive goal engagement. A total of 586 individuals with chronic pain participated in the cross-sectional, online study. Participants completed self-report measures to assess pain intensity and interference, mental well-being and goal motivation variables. Results showed that pain interference and positive goal engagement were associated with mental well-being. Moreover, the relationship between pain interference and mental well-being was partially mediated by positive goal engagement. The results provide tentative evidence for the protective role of positive goal engagement in enabling individuals with chronic pain to maintain a sense of mental well-being. The study develops the biopsychosocial model of chronic pain by examining the roles and relationships of relevant yet previously unexplored psychological constructs. The promotion of mental well-being through the enhancement of positive goal engagement is discussed, offering a platform for further research and clinical interventions. [ABSTRACT FROM AUTHOR]

Background: Microalbuminuria represents vascular and endothelial dysfunction. Thyroid hormones can influence urine albumin excretion as it exerts crucial effects on the kidney and on the vascular system. This study explores the relationship between serum thyrotropin and urine albumin excretion in euthyroid patients with diabetes.Methods: A total of 433 patients with type 1 or 2 diabetes were included in this retrospective cross-sectional study. Data included anthropometric measurements and biochemical parameters from diabetes clinic. Males with urine albumin creatinine ratio >2.5 and female's >3.5 mg/mmoL were considered to have microalbuminuria.Results: 34.9% of the patients had microalbuminuria. Prevalence of microalbuminuria increased according to TSH quartiles (26.9, 34.6, 38.5 and 44.9%, P for trend = 0.02). In a fully adjusted logistic regression model, higher TSH concentrations were associated with high prevalence of microalbuminuria (adjusted odds ratio 2.06 [95% CI: 1.14-3.72]; P = 0.02), while comparing the highest with the lowest quartile of TSH. Multiple linear regression analysis showed an independent association between serum TSH and urine albumin creatinine ratio (β = 0.007, t = 2.03 and P = 0.04). The risk of having microalbuminuria was higher with rise in TSH concentration in patients with younger age (<65 years), raised body mass index (≥25 kg/m2), hypertension, type 2 diabetes and hyperlipidaemia and age was the most important determinant ( P for interaction = 0.02).Conclusion: Serum TSH even in the euthyroid range was positively associated with microalbuminuria in euthyroid patients with diabetes independent of traditional risk factors. Child maltreatment is a prevalent societal problem that has been linked to a wide range of social, psychological, and emotional difficulties. Maltreatment impacts on two putative evolved psychobiological systems in particular, the attachment system and the social rank system. The maltreatment may disrupt the child’s ability to form trusting and reassuring relationships and also creates a power imbalance where the child may feel powerless and ashamed. The aim of the current article is to outline an evolutionary theory for understanding the impact of child maltreatment, focusing on the interaction between the attachment and the social rank system. We provide a narrative review of the relevant literature relating to child maltreatment and these two theories. This research highlights how, in instances of maltreatment, these ordinarily adaptive systems may become maladaptive and contribute to psychopathology. We identify a number of novel hypotheses that can be drawn from this theory, providing a guide for future research. We finally explore how this theory provides a guide for the treatment of victims of child maltreatment. In conclusion, the integrated theory provides a framework for understanding and predicting the consequences of maltreatment, but further research is required to test several hypotheses made by this theory. In this article, various authors provide their thoughts on the appropriate ways for creating geopolitical boundaries. Topics addressed by these scholars include the relationship between globalization, borders, and urban growth; the unporous border between South Sudan and Sudan, created in 2011; and the way in which national boundaries and nation states give rise to ethnic conflict. Also discussed are the ways in which telecommunications can bridge borders and unequal freedom of movement among peoples of different origins.

In this article the authors discuss research conducted by the Globalization and World Cities Research Network, which examines the geography of the global spatial economy before the financial crisis worsen in 2008. They present two charts, which list the top 20 cities in command-and-control centres in terms of financial command index (FCI) and business command index (BCI) in early 2008. They remark that the study allows them to do contemporary analyses of the geo-economic transitions.

There has been little research on domestic violence (DV) within ethnic minority communities in high-income countries. This study reports on the findings of a meta-ethnography that examined the barriers and facilitators of help-seeking behaviors in South Asian women living in high-income countries who have experienced DV to inform practice, understand the limits of the evidence, and identify research gaps. Qualitative studies were identified which were available in English by electronic databases. After an initial search, 2,465 articles were reviewed by title and abstract and 135 articles were reviewed for full text. Thirty-five papers were included for this review and were synthesized using meta-ethnography. Key findings included barriers and facilitators of help-seeking behaviors: (1) Socio-cultural norms to prohibit help-seeking behaviors, (2) Fear of negative consequences, (3) Negative aspects of immigration status, (4) Insufficient support from statutory, and voluntary agencies, (5) Safety strategies and facilitators for surviving. Although this review investigated the perceptions of two different populations (survivors and service providers) both groups had similar views about the barriers and facilitators of help-seeking behaviors. It is crucial for the government and non-government organizations to understand the barriers for women who are DV survivors to seek help from their organizations and also from South Asian ethnicities. The awareness and understanding of these barriers and facilitators may help support the development of interventions to encourage effective help-seeking amongst South Asian women affected by DV. In a time of rapid change, when global forces are re-shaping the ability of ordinary people to influence the decisions which affect their lives, social change practitioners are challenged to learn new skills and competences, and to develop their capacities for learning through critical reflection on action. Drawing on two international dialogues, and linking to the authors’ perceptions of the fault-lines that underlie some elements of higher education, the article explores learning needs for action researchers who aspire to promote participation as a key element of social change. The article presents the story of an innovative masters teaching programme within which action research is central to the overall learning process. Highlighting key challenges and also some unanticipated learning outcomes in regard to personal inquiry into identity, relationships, positionality and power, the article highlights issues relating to teaching and learning methods of reflective practice, bridging the personal and political and linking individual to systemic change. Staphylococcus aureus is a major pathogen both within hospitals and in the community. Methicillin, a β-lactam antibiotic, acts by inhibiting penicillin-binding proteins (PBPs) that are involved in the synthesis of peptidoglycan, an essential mesh-like polymer that surrounds the cell. S. aureus can become resistant to methicillin and other β-lactam antibiotics through the expression of a foreign PBP, PBP2a, that is resistant to the action of methicillin but which can perform the functions of the host PBPs. Methicillin-resistant S. aureus isolates are often resistant to other classes of antibiotics (through different mechanisms) making treatment options limited, and this has led to the search for new compounds active against these strains. An understanding of the mechanism of methicillin resistance has led to the discovery of accessory factors that influence the level and nature of methicillin resistance. Accessory factors, such as Fem factors, provide possible new targets, while compounds that modulate methicillin resistance such as epicatechin gallate, derived from green tea, and corilagin, provide possible lead compounds for development of inhibitors. In world-systems analysis, the United Provinces are interpreted as one of only throe hegemonic states in the history of the capitalist world-economy. Unlike the subsequent hesemons Britain and the United States, the United Provinces only became an independent country just before its rise to hegemony in the early seventeenth century. This essay explores how this new small state became the first hegemon of the modern world-system. Two questions are asked: why did the area of northern Netherlands became a state, and why did this state became a hegemon? Using Mann's sources of social power, it is shown how a promiscuous combination of ideological, military, political, and economic power produced a unique state combining the economic policies of city-states with the protective capacity of territorial states. It is concluded that the Dutch promotion of an economic raison d'etat was a necessary component for the consolidation of a competitive interstate system, itself a necessary requirement for the expansion of the capitalist world-economy. The literature on firm location selection allows us to retrospectively explain why firms did locate in particular places. However, it remains challenging to prospectively predict where they will locate. In this article, we propose a simple conceptual model of firm location decisions, then operationalize it using the ordinal stochastic degree sequence model (oSDSM). We use this model to predict whether 104 advanced producer service firms will expand, contract, or maintain their presence in each of 525 cities, and find that these predictions are accurate in more than 86 percent of cases. Objectives: Patient-reported outcome measures (PROMs) are evaluated in randomized controlled trials (RCTs) in patients with systemic lupus erythematosus (SLE), but not widely used in clinical practice. However, interest in incorporating PROMs into the management of SLE is increasing as PROMs provide a unique insight into the patient's perception of lupus disease activity. The objective was to assess agreement in PROMs answered using a web app versus an outpatient touchscreen among patients with SLE. Methods: In a crossover RCT, SLE patients answered the following PROMs in a random order using the web app and the outpatient touchscreen: Systemic Lupus Erythematosus Activity Questionnaire (SLAQ) Global Health, SLAQ Symptom, SLAQ Total, SLAQ Worsening, Pain Visual Analog Scale (VAS), Fatigue VAS, Patient Global Health VAS, Health Assessment Questionnaire Disability Index (HAQ-DI), Patient Acceptable Symptom State (PASS), and an Anchoring Question. Equivalence between the two device types was demonstrated if the 95% confidence interval (95% CI) of the difference in PROM scores was within the prespecified equivalence margin. Agreement between the two device types was assessed using mixed linear models. Results: Thirty-four patients with SLE were included. Equivalence was demonstrated between the two device types for SLAQ Global Health with a difference of −0.21 (95% CI: −0.65 to 0.23). Moreover, equivalence was also found for HAQ-DI, Pain VAS, and Fatigue VAS whereas only comparability within the limits of the Minimal Clinically Important Difference (MCID) was demonstrated for VAS Patient Global Health. Statistical comparability was demonstrated for SLAQ Total, SLAQ Worsening, PASS, and Anchoring Question (no predefined MCID/equivalence margins available). However, a statistically significant difference between device types was observed for the SLAQ Symptom of −0.56 (95% CI: −1.10 to −0.01). The difference was, however, very small when considering the scale range of 0–24; thus, it was not judged to be of clinical relevance. Preference for the web app was very high (91.2%). Conclusion: For the first time ever, equivalence and comparability between two electronic device types for various PROMs were demonstrated among patients with SLE. Networks have become ubiquitous in the analysis of socioeconomic patterns. Importantly, most of these networks have an inherent geographical dimension, and are therefore a relevant object for geographical research and related thinking on cartographic mapping and visualization. The visualization of spatial networks is, however, still in its infancy. In this paper, we present and discuss some of the typical issues associated with the visualization of spatial networks, as well as the emerging possibilities to tackle these issues. In particular, we focus on a state-of-the-art graph layout algorithm that uses edge bundling on network layers in conventional maps. The Local Government (Miscellaneous Provisions) (Scotland) Act marks a major change in central-local relations in allowing selective intervention in the expenditure decisions of individual councils. The criteria for action, based upon the concept of 'excessive and unreasonable' expenditure are broadly drawn. In taking action against seven councils in 1981-2, the Scottish Office applied its own criteria inconsistently. The case against the councils tacked intellectual credibility and an alternative 'hit list' could as plausibly be produced. The councils' reactions and the exchanges with the Scottish Office show a varied pattern, with a minority stressing the principle of local autonomy. The Act has failed to achieve the objectives which central government appeared to have in mind. It has, instead, further eroded local autonomy and destabilized central-local relations in Scotland. The 18kDa Translocator Protein (TSPO) is the most commonly used tissue-specific marker of inflammation in positron emission tomography (PET) studies. It is expressed in myeloid cells such as microglia and macrophages, and in rodent myeloid cells expression increases with cellular activation. We assessed the effect of myeloid cell activation on TSPO gene expression in both primary human and rodent microglia and macrophages in vitro, and also measured TSPO radioligand binding with 3H-PBR28 in primary human macrophages. As observed previously, we found that TSPO expression increases (∼9-fold) in rodent-derived macrophages and microglia upon pro-inflammatory stimulation. However, TSPO expression does not increase with classical pro-inflammatory activation in primary human microglia (fold change 0.85 [95% CI 0.58–1.12], p = 0.47). In contrast, pro-inflammatory activation of human monocyte-derived macrophages is associated with a reduction of both TSPO gene expression (fold change 0.60 [95% CI 0.45–0.74], p = 0.02) and TSPO binding site abundance (fold change 0.61 [95% CI 0.49–0.73], p < 0.0001). These findings have important implications for understanding the biology of TSPO in activated macrophages and microglia in humans. They are also clinically relevant for the interpretation of PET studies using TSPO targeting radioligands, as they suggest changes in TSPO expression may reflect microglial and macrophage density rather than activation phenotype. A series of novel podocarpa-8,11,13-triene-7- and 13-nitriles were prepared from the naturally occurring labdane diterpenoid (+)-manool, using podocarp-8(14)-en-13-one and 13-methoxypodocarpa-8,11,13-trien-7-one as key intermediates and TMSCN and acetonitrile as source of the nitrile. The synthesised compounds were screened for cytotoxicity against mouse macrophage cell line (RAW 264.7), human colon adenocarcinoma cell lines (HT-29) and human prostate adenocarcinoma cell lines (PC3) and for anti-inflammatory, as measured by the inhibition of nitric oxide (NO) production by RAW cells. 7-Oxopodocarpa-8,11,13-triene-13-nitrile exhibited significant inhibition of NO production (IC50 = 6 μM) and was not cytotoxic. In this paper we report on a major empirical study of centripetal and centrifugal forces in the City of London financial services agglomeration. The study sheds light on (1) the manner and magnitude of firm interaction in the agglomeration; (2) the characteristics of the agglomeration that aid the competitiveness of incumbent firms; and (3) the problems associated with agglomeration. In addressing these issues, we use the data to (1) test emerging theory that explains the high productivity and innovation of agglomerations in terms of their ability to generate and diffuse knowledge; and (2) evaluate the ‘end of geography’ thesis.
