One hundred six normal people were studied for variations of eight pulse pressure parameters dealing with time and pressure measurements as obtained through a left brachial artery needle. Two hundred twenty-two patients including nine separate disease states were studied in the same fashion. An increase in the mean pulse wave velocity was found in children under 10 years of age as compared with people from 10 through 39 years of age. An increase in the mean pulse wave velocity was noted in the 57 normal patients from 40 to 79 years of age when compared with the 10 to 39 age group. This increased mean velocity was identical with the mean velocity of the 0 to 9 age group. Within the limits of this study pulse wave velocity did not appear to be related to blood pressure change in hypertensive individuals. A consistent rise in mean upstroke duration (anacrotic limb time) was seen with advancing age. Total systolic mean duration was shorter for the 0 to 9 age group than for either the 10 to 39 or the 49 to 79 age group. 8. The percentage of systole used to reach the peak systolic pressure showed a progressive rise with age. 9. In resting normal people the percentage of total cycle length used for systolic contraction was remarkably constant in all age groups. The mean was 0 to 9 age group, 40 per cent; 10 to 39 age group, 39 per cent; and 40 to 79 age group, 40 per cent. 10. Within the normal range, systolic blood pressure rose consistently with age, diastolic blood pressure showed little or no change and pulse pressure as it reflected the systolic pressure, rose consistently with age. The analysis of six parameters of measurement, related to the contour of the brachial artery pulse pressure revealed a number of interesting observations related to aging but was of no real value for the individual patient as an aid to the diagnosis of the nine disease states studied. The dosage of propranolol needed in the treatment of patients with cardiac arrhythmias or angina poctoris in our study, has depended upon the relief of symptoms, the heart, rate, the degree of fatigue and the blood level attained. 2. A reduction in total dose was not necessary, in the absence of symptoms, unless the heart rate was sustained below 52 beats per minute. 3. Fatigue, in the absence of clinical heart failure, need not signal a reduction in dose unless sustained for two weeks. 4. In this study, a propranolol blood level of .03 μg/ml or higher was associated with the best symptom relief. 5. An increment dose of propranolol every three hours was needed to sustain a blood level of .03 μg/ml or higher in most patients studied. The smallest total daily dose on the q3h schedule for good relief of anginal pain was 80 mgs daily. The average daily dose required for relief of pain was 218 mgs. 6. 1. Seventeen cases of cardiac arrhythmias and one case of chronic orthostatic hypotension have been treated with oral or intravenons propranolol. 2. No effect was noted on the orthostatic hypotension of the one case treated. 3. Fifteen of the seventeen arrhythmias responded with good rate control. In three cases with ventricular arrhythmias and one with supraventricular tachycardia, a normal sinus rhythm was established. 4. The encouraging conversion of ventricular fibrillation to a normal rhythm with propranolol when other measures had failed, warrants consideration for this approach in the desperate situation. 5. Toxic manifestations included chest tightness in three; possible temporary aggravation of a pre-existent hearing loss in one; aggravation of failure and possible precipitation of cardiac arrest in one; possible temporary neurogenic effect in one; and the temporary elevation of transaminases and lactic dehydrogenase in one. 6. The presence of left ventricular failure is a major contraindication to the use of propranolol. 7. The marked effect of propranolol in controlling a variety of cardiac arrhythmias, whether or not related to digitalis intoxication, is highly significant and adds an important new drug to our armamentarium.
