This paper reviews the author's experience in becoming a medical sociologist before the field had become formalized. The contributions to medical sociology of sociologist Selden D. Bacon and physician and medical educator William R. Willard are described. The relationship of medical sociology to medical behavioral science, as experienced at the University of Kentucky, is discussed. The paper which follows takes the form of a dialogue between a sociological voice and an unidentified, questioning voice. The two voices explore some of the tasks involved in, and difficulties generated by, the attempt to apply social science to practical issues. The discussion focuses on the area of health economics and, particularly, on recent efforts to provide measures of the quality of life that can be used to solve administrative problems within the NHS. Beginning from close examination of a particular text, the sociological voice claims to reveal some of the background assumptions of health economics as a social practice and to identify some of the ways in which the production and application of economic knowledge are socially contingent. The sociological voice also contrasts the textual form of the present paper with the 'scientific' format normally employed by economists and most other applied social scientists. S/he may be read as asserting that the dialogic character of the present text is in some way more suited to a more collaborative use of social science expertise in the realm of practical action. Visual methods are accepted tools for qualitative research and are increasingly used in a wide range of disciplines, such as sociology, psychology, geography, and health care. A literature review was undertaken with the aims of understanding why visual methods are chosen for use in research, reviewing any evidence regarding outcomes arising from those choices, and reflecting on the role of visual methods in these outcomes. Searches conducted from 2000-2010 across multiple bibliographic databases yielded 109 research papers that cited reasons for their choice of visual method. These were reviewed using a method tailored to the review's purpose but also informed by a narrative synthesis approach. The reasons given were collated and analysed inductively, with two categories of reasons emerging: those principally related to enrichment of data collection or presentation and those concerning the relationship between participants and researchers. Support for these reasons is reviewed and the ethical implications regarding choice of method are discussed. This article concludes that support for the use of visual methods to enhance data richness is strong, but more research is needed to facilitate a better-informed choice of method. There is some support for using visual methods for purposes connected with relational aspects between researcher and participants, but the visual media's contribution derives mainly from the ability of images to facilitate and enrich communication thus enhancing the data. This paper argues that two models of illness can be identified within Parsons's paradigm--a structural model focusing on illness as incapacity for rle performance and a psychodynamic model focusing on illness as motivated deviance. Medical sociology, it will be said, has overlooked the psychodynamic or deviance model while recognizing and criticizing the structural or incapacity model under the misplaced label of deviance. It will further be ventured that while some elements of these two models can easily be fitted into a conservative view of medical practice, other elements are more difficult to reconcile with conservatism. This paper describes the origins and development of the sociology of health and illness in Britain and shows that these were rooted in practical concerns, rather than deriving from sociological theory or general sociology. Medical practitioners and researchers, health administrators, patients and the feminist movement have been influences encouraging sociologists to turn their attention to health and illness. The present state of the sub-discipline is one of great activity, but little theoretical or methodological unity. The contribution to the subject of Parsons, Freidson and Mechanic, the marxists, the British post-Fabians, empiricists and phenomenologists are discussed, as is the contribution of the subdiscipline to health and health policy research. In considering the future of the subject, a sociology of health and illness rather than of medicine is proposed and the inevitable moral and political implications of sociological activity in this area are exposed. The need to face the implications of suffering for social relations in health care and the tendency to ignore these is discussed. The designation 'sociology of health' or 'medical anhropology/sociology' indicates, at best, only the field and not the peculiarities of the field. As a consequence much work in this sub-discipline is of the genre, 'Sociology/Anthropology: Case Study Illness (or Health)'. This paper attempts to regroup the contributions made in this field in order to demonstrate the utility of establishing the specificities of the sub-discipline 'sociology/anthroplogy of illness'. Such an exercise would also provide us with a consistent framework for formulating health policies as well as for appreciating the reasons why WHO's blueprint of 'Health for All' and China's 'Barefoot Doctor' programme ran into difficulties. Furthermore, a systematic pursuit of the specificities of sociology/anthropology of illness would also force us to mutate our understanding of at least a few received formulations on the sociology/anthropology of deviance. professions, and even of tradition and modernity. The introduction to this special issue of the Journal expands the scope of research on "the sociology of medical education" to include studies of the institutions in which physicians are trained as well as studies of the socialization of the physician throughout the life cycle. Research since the 1960s is reviewed briefly and the papers in this issue are discussed. Studies of the macropolitics and micropolitics of academic medicine are called for as well as comparative analyses of the settings in which physicians are trained and research on the relative effects on physicians' attitudes and behavior of different sets of experiences, professional and nonprofessional, over the course of their life cycle-all in the context of massive changes in the organization of health care. The article comments on the paper "Extending Cultural Competence Through Systems Change: Academic, Hospital and Community Partnerships," by Noel Chrisman. It asserts that the issue of cultural competence is an important sociological issue. Moreover, the asset-based model proposed by Chrisman acknowledges the interdependent nature of the process and structural variables related to culturally appropriate care.
