Background: Being overweight or obese is associated with poor outcomes and an increased risk of failure after rotator cuff (RC) surgery. However, the effect of obesity on enthesis healing has not been well characterized. Hypotheses: Diet-induced obesity (DIO) would result in inferior enthesis healing in a rat model of RC repair, and a dietary intervention in the perioperative period would improve enthesis healing. Study Design: Controlled laboratory study. Methods: Male Sprague-Dawley rats were divided into 3 weight-matched groups (n = 26 per group): control diet (CD), high-fat diet (HFD), or HFD until surgery and then CD thereafter (HF-CD). After 12 weeks, the left supraspinatus tendon was detached, followed by immediate repair. Animals were sacrificed, and RCs were harvested at 2 and 12 weeks after surgery for biomechanical and histological evaluations. Metabolic end points were assessed using dual-energy X-ray absorptiometry and plasma analyses. Results: DIO was established in the HFD and HF-CD groups before surgery and subsequently reversed in the HF-CD group after surgery. At 12 weeks after surgery, the body fat percentage ( P = .0021) and plasma leptin concentration ( P = .0025) were higher in the HFD group compared with the CD group. Histologically, the appearance of the repaired entheses was poorer in both the HFD and HF-CD groups compared with the CD group at 12 weeks after surgery, with semiquantitative scores of 6.20 ( P = .0078), 4.98 ( P = .0003), and 8.68 of 15, respectively. The repaired entheses in the HF-CD group had a significantly lower load to failure ( P = .0278) at 12 weeks after surgery compared with the CD group, while the load to failure in the HFD group was low but not significantly different ( P = .0960). There were no differences in the biomechanical and histological results between the groups at 2 weeks after surgery. Body mass at the time of surgery, plasma leptin concentration, and body fat percentage were negatively correlated with histology scores and plasma leptin concentration was correlated with load to failure at 12 weeks after surgery. Conclusion: DIO impaired enthesis healing in this rat RC repair model, with inferior biomechanical and histological outcomes. Restoring a normal weight with dietary changes after surgery did not improve healing outcomes. Clinical Relevance: Obesity is a potentially modifiable factor that impairs RC healing and increases the risk of failure after surgery. Exploring interventions that improve the metabolic state of obese patients and counseling patients appropriately about their modest expectations after repair should be considered.

Due to the unpredictability of global events such as a COVID-19 pandemic, it is necessary to prepare for a crisis management strategy at the macro-level in each country to respond to crises that bring about social change. This study is designed to discover critical priorities and agendas in crisis management in Saudi Arabia by grasping the most recent social issues of the COVID-19 pandemic addressed in the most trusted media sources. The data were collected from news media (Saudi Press Agency and Al Arabiya) and social media (Twitter and YouTube) in big data from February to August 2020, then analyzed through semantic network analysis. This study provides the uncanny and marvelous power to help any individual, organization, even society, and country to be more innovative and proactive by acquiring trend, agenda-based approaches into uncertain future crises using advanced technology in big data. This research also shows how a country can use those big data to detect “key social issues” and then make a subsequent strategy or decision-making system to develop a public communication or new policy in future events. Accordingly, this study provides meaningful and insightful implications for managing a coming crisis and social change, ensuring stability and sustainability at the micro- and macro-level.

This article conceptualizes organizational advocacy as a relational communication strategy by which organizations take stances on controversial, sociopolitical issues to signal shared commitment with key publics. The authors conducted a series of two-by-two experimental surveys—controlling whether an organization took a defined position (advocacy vs. silence) and whether it acted alone or in line with peers (leader vs. follower)—across both less partisan (Study 1) and highly partisan (Study 2) issues. Findings indicate advocacy is an effective relational communication strategy that strengthens organization–public relationships (OPRs) and increases publics’ support for the organization. Theoretical contributions and practical applications are discussed.

This study examines how frequently committed ethical misconduct regarding values closely aligned to an organizational mission affects stakeholders’ pro-organizational responses to an offending nonprofit. Using a 2 × 2 experimental survey, findings showed that ethical misconduct in primary values resulted in significantly decreased positive attitudes toward and donation intentions to the organization. Also, the patterns of unethical behaviors significantly increased stakeholders’ negative attitudes toward the organization and willingness to assign the organization responsibility for unethical behavior. Perceived organizational responsibility for ethical misconduct and deteriorating organization–public relationships (OPRs) were significant mediators for the effects of primary ethical violations.

A prevalent view of visual working memory (VWM) is that visual information is actively maintained in the form of perceptually integrated objects. Such reliance on object-based representations would predict that after an object is fully encoded into VWM, all features of that object would need to be maintained as a coherent unit. Here, we evaluated this idea by testing whether memory resources can be redeployed to a specific feature of an object already stored in VWM. We found that observers can utilize a retrospective cue presented during the maintenance period to attenuate both the gradual deterioration and complete loss of memory for a cued feature over time, but at the cost of accelerated loss of information regarding the uncued feature. Our findings demonstrate that object representations held within VWM can be decomposed into individual features and that having to retain additional features imposes greater demands on active maintenance processes.

In this study, we investigated the effect of a mixture of Platycladus orientalis (L.) Franco leaf extract and α-terpineol, a natural monoterpene alcohol (PEaT), on hair growth and its mechanisms. C57/BL6 mice (total n = 14) in the telogen phase of hair growth were used. Either distilled water as vehicle or PEaT was topically applied to the dorsal skin for 17 days. Chronological hair growth change was examined by hair growth-promoting scores. In addition, to find out mechanisms of PEaT on hair growth, insulin-like growth factor-1 (IGF-1), vascular endothelial growth factor (VEGF), Ki-67, wnt3, and β-catenin expressions were investigated by using immunohistochemistry. We found that PEaT remarkably promoted hair growth by inducing early anagen transition compared with the control group. In addition, treatment with PEaT significantly increased numbers of Ki-67-positive cells and expressions of IGF-1, VEGF, wnt3, and β-catenin in the outer root sheath. These results indicate that PEaT used in this study might be a good hair growth promoter, showing that PEaT treatment increased growth factors and cell proliferation through upregulation of wnt3 and β-catenin expressions.

Background:The treatment of symptomatic acromioclavicular joint (ACJ) injury in the rotator cuff (RC) tear has not been well clarified.Purpose:To compare the clinical results between patients who had distal clavicle resection (DCR) and those who did not during RC repair.Study Design:Randomized controlled trial; Level of evidence, 1.Methods:From August 2008 to December 2009, a total of 56 consecutive patients (58 shoulders) were included. All patients had either a full-thickness or high-grade (>50%) RC tear, ACJ tenderness, arthritic change visible on plain radiographs, and a positive ACJ lidocaine injection test the day before surgery. Patients were randomized into 2 groups: DCR and RC repair (DCR+RCR group) and RC repair only (isolated RCR). Evaluation was performed preoperatively, at 6 months postoperatively, and at a final follow-up a minimum of 24 months postoperatively using the American Shoulder and Elbow Surgeons (ASES) score, the Constant shoulder score, range of motion examination, and pain visual analog scale (VAS).Results:After simple randomization, 26 shoulders were allocated in the DCR+RCR group, and 32 were placed in the isolated RCR group. Five shoulders in the DCR+RCR group and 6 in the isolated RCR group were excluded from analysis due to loss of follow-up. Therefore, the evaluation was performed for 21 shoulders in the DCR+RCR group and 26 shoulders in the isolated RCR group. The mean follow-up period was 44.2 months in the DCR+RCR group and 44.0 months in the isolated RCR group. There were no differences in age, sex, symptom duration, RC tear size, or preoperative ASES, Constant, and VAS scores between the 2 groups ( P > .05). At final follow-up, the ASES, Constant, and VAS scores were significantly improved in both groups ( P < .001). There were no differences in ASES, Constant, and VAS scores between the 2 groups at final follow-up ( P > .05), and there was no difference in residual ACJ tenderness (7 in the DCR+RCR group and 5 in the isolated RCR group) between the 2 groups ( P = .270).Conclusion:There was no difference in the clinical evaluations between the combined arthroscopic DCR and RCR group and the isolated RCR group at a minimum 24-month follow-up. Arthroscopic DCR should be carefully considered in patients who have symptomatic ACJ arthritis with RC tears.
