People who believe they are invulnerable to infectious diseases often fail to protect themselves against the disease threats that others pose to them. The current paper hypothesizes that social pain—the experience of feeling interpersonally hurt or rejected—can sensitize the behavioral-immune system by giving people added reason to see others as worthy of protecting themselves against. We obtained four daily diary samples involving 2,794 participants who reported how hurt/rejected they felt by those they knew, how personally concerned they were about the spread of illness/COVID-19, and how vigilantly they engaged in self-protective behaviors to safeguard their health each day. An integrative data analysis revealed robust evidence that people who believed they were invulnerable to infectious disease engaged in more concerted efforts to protect themselves against the greater daily risk of contracting COVID-19 when being in acute social pain gave them added reason to see others as harmful to them.

Elections and pandemics highlight how much one’s safety depends on fellow community members, a realization that is especially threatening when this collective perceives political realities inconsistent with one’s own. Two longitudinal studies examined how people restored safety to social bonds when everyday experience suggested that fellow community members inhabited inconsistent realities. We operationalized consensus political realities through the negativity of daily nationwide social media posts mentioning President Trump (Studies 1 and 2), and the risks of depending on fellow community members through the pending transition to a divided Congress during the 2018 election season (Study 1), and escalating daily U.S. COVID-19 infections (Study 2). On days that revealed people could not count on fellow community members to perceive the same reality of President Trump’s stewardship they perceived, being at greater risk from the judgment and behavior of the collective community motivated people to find greater happiness in their family relationships.

Mental disorders are among the leading causes of global disease burden. To respond effectively, a strong understanding of the structure of psychopathology is critical. We empirically compared two competing frameworks, dynamic-mutualism theory and common-cause theory, that vie to explain the development of psychopathology. We formalized these theories in statistical models and applied them to explain change in the general factor of psychopathology (p factor) from early to late adolescence ( N = 1,482) and major depression in middle adulthood and old age ( N = 6,443). Change in the p factor was better explained by mutualism according to model-fit indices. However, a core prediction of mutualism was not supported (i.e., predominantly positive causal interactions among distinct domains). The evidence for change in depression was more ambiguous. Our results support a multicausal approach to understanding psychopathology and showcase the value of translating theories into testable statistical models for understanding developmental processes in clinical sciences.

Kinsa Inc. sells Food and Drug Administration–cleared smart thermometers, which synchronize with a mobile application, and may aid influenza forecasting efforts. We compare smart thermometer and mobile application data to regional influenza and influenza-like illness surveillance data from the California Department of Public Health. We evaluated the correlation between the regional California surveillance data and smart thermometer data, tested the hypothesis that smart thermometer readings and symptom reports provide regionally specific predictions, and determined whether smart thermometer and mobile application improved disease forecasts. Smart thermometer readings are highly correlated with regional surveillance data, are more predictive of surveillance data for their own region and season than for other times and places, and improve predictions of influenza, but not predictions of influenza-like illness. These results are consistent with the hypothesis that smart thermometer readings and symptom reports reflect underlying disease transmission in California. Data from such cloud-based devices could supplement syndromic influenza surveillance data.

The Global Burden of Disease 2015 study aims to use all available data of sufficient quality to generate reliable and valid prevalence, incidence, and disability-adjusted life year (DALY) estimates of oral conditions for the period of 1990 to 2015. Since death as a direct result of oral diseases is rare, DALY estimates were based on years lived with disability, which are estimated only on those persons with unmet need for dental care. We used our data to assess progress toward the Federation Dental International, World Health Organization, and International Association for Dental Research's oral health goals of reducing the level of oral diseases and minimizing their impact by 2020. Oral health has not improved in the last 25 y, and oral conditions remained a major public health challenge all over the world in 2015. Due to demographic changes, including population growth and aging, the cumulative burden of oral conditions dramatically increased between 1990 and 2015. The number of people with untreated oral conditions rose from 2.5 billion in 1990 to 3.5 billion in 2015, with a 64% increase in DALYs due to oral conditions throughout the world. Clearly, oral diseases are highly prevalent in the globe, posing a very serious public health challenge to policy makers. Greater efforts and potentially different approaches are needed if the oral health goal of reducing the level of oral diseases and minimizing their impact is to be achieved by 2020. Despite some challenges with current measurement methodologies for oral diseases, measurable specific oral health goals should be developed to advance global public health.

This article identifies distinct mass media reporting stages used in the coverage of mass killings, and the inspiration they provide for future killers. Ethnographic content analysis was used to identify common and ordered stages/themes expressed through mass media accounts of the massacres committed by Dylan Klebold and Eric Harris (Columbine High School), Seung-Hui Cho (Virginia Tech), James Holmes (Aurora Movie Theater), Adam Lanza (Sandy Hook Elementary School), and Omar Mateen (Pulse nightclub Orlando). Many of these infamous killers reference/discuss their well-publicized prior homicidal role models in self-created archival documents they leave behind. They do not just copycat prior killers, they often relate to them, are inspired by them, and want to outdo them. The entertainment form and logic of mass mediated news provides the inspiration and fuel for later killings.

This article proposes and examines a parsimonious framework for assessing quality in therapeutic residential care. The synthesized conceptual framework includes four potentially critical domains of quality: setting, staffing, safety, and treatment. Data from a recently completed quasi-experimental study of group homes were used to examine prevalence of various key indicators within each of these domains and to explore relationships between these indicators and youth-level outcomes. Findings suggest that, among this sample of licensed homes in a southeastern state, licensure may be an initial indicator of baseline quality. Beyond this most basic level, utilization of an evidence-informed model (in this case, the teaching family model) was associated with better outcomes. Net of model, findings suggest that positively focused motivational systems, youth’s perceptions of staff’s fairness and helpfulness, provision of age/interest-appropriate toys/books/games, more preservice training, and prohibition of physical restraint were all associated with better outcomes for youth. Additional work is needed to assess the generalizability and utility of this framework for assessing quality in group homes.

We aimed to consolidate all epidemiologic data about untreated caries and subsequently generate internally consistent prevalence and incidence estimates for all countries, 20 age groups, and both sexes for 1990 and 2010. The systematic search of the literature yielded 18,311 unique citations. After screening titles and abstracts, we excluded 10,461 citations as clearly irrelevant to this systematic review, leaving 1,682 for full-text review. Furthermore, 1,373 publications were excluded following the validity assessment. Overall, 192 studies of 1,502,260 children aged 1 to 14 y in 74 countries and 186 studies of 3,265,546 individuals aged 5 y or older in 67 countries were included in separate metaregressions for untreated caries in deciduous and permanent teeth, respectively, using modeling resources from the Global Burden of Disease 2010 study. In 2010, untreated caries in permanent teeth was the most prevalent condition worldwide, affecting 2.4 billion people, and untreated caries in deciduous teeth was the 10th-most prevalent condition, affecting 621 million children worldwide. The global age-standardized prevalence and incidence of untreated caries remained static between 1990 and 2010. There is evidence that the burden of untreated caries is shifting from children to adults, with 3 peaks in prevalence at ages 6, 25, and 70 y. Also, there were considerable variations in prevalence and incidence between regions and countries. Policy makers need to be aware of a predictable increasing burden of untreated caries due to population growth and longevity and a significant decrease in the prevalence of total tooth loss throughout the world from 1990 to 2010.

Support from romantic partners can have tremendous benefits for people’s personal goal pursuits. However, when relationships end in breakup, this goal support is lost. Thus, breakup may have important consequences for goal progress. A longitudinal study found that breaking up with goal instrumental partners predicted poorer subsequent goal progress. Breaking up with noninstrumental partners did not impair goal progress. In contrast, breakup did not predict changes in goal importance, suggesting that people still valued goals associated with their former partners. These findings are the first to reveal a hidden potential cost of breakup: losing the support people once relied on to achieve their valued personal goals. Thus, finding alternative sources of goal support may be essential to coping successfully with breakup.

A daily diary study of married couples tested the hypothesis that automatic partner attitudes regulate self-protection for low, but not high, self-esteem people. For 14 days both partners reported trust in the other’s caring and perceived and actual rejecting and selfish behavior. On days after low self-esteem people reported less trust in their partner’s caring, those with more positive automatic partner attitudes perceived their partner to be less rejecting and selfish. They also engaged in less rejecting and selfish behavior toward their partner and their partner engaged in less selfish and rejecting behavior toward them. The current findings present the first evidence that automatic partner attitudes may help low self-esteem people inhibit the rejection sensitivity and distancing behaviors that too often undermine their relationships.

Self-control allows people to curb destructive behavior and behave more pro-socially in relationships. Thus, individuals generally trust partners with high dispositional self-control more. However, it is not clear whether partner self-control influences individuals’ responses to acutely risky situations, such as when partners are rejecting. A daily diary study of married and cohabiting couples examined whether actors with high self-control partners behave less self-protectively in risky situations. On days partners were highly rejecting, actors were less likely to retaliate against and more likely to value high self-control partners. On days after partners had been rejecting, actors also reported that high self-control partners behaved more responsively. Actors also trusted partners with high self-control more regardless of risk. Taken together, our findings suggest that partners’ greater self-control may help foster more positive interaction cycles in romantic relationships.

Two experiments examined how low and high self-esteem people regulate attention in the face of uncertainty about their partner’s caring. We primed risk regulation processes by leading experimental participants to believe their partner’s caring and responsiveness was in question. We then assessed directed attention to the partner’s positive and negative qualities using a dot-probe paradigm. High, but not low, automatically directed attention away from their partner’s negative traits in response to uncertainty.

Voluntary standard and certification systems are proliferating in the United States and around the world. While the majority of certification research draws on case studies, we pursue a cross-sectional empirical analysis of an original database of 108 certifications operating in the United States. We analyse the rise and configuration of private regulatory initiatives, variations in programme breadth and focus, and variations in programme participation and oversight requirements. We highlight the predominance of international and multi-stakeholder initiatives in the United States. Our research identifies substantial convergence in the ecological and social priorities of certification programmes and even greater convergence in their assessment and oversight procedures. Statistical analysis helps explain the prevalence of multi-stakeholder initiatives, but we find no significant differences between these and industry-led initiatives in their foci or procedures. We argue that there is a standardization of certification norms and practices which may mask important programme differences.

The current research examined the novel hypothesis that a romantic partner’s same-sex friends can elicit jealousy by threatening people’s central role in their partner’s life. Thus, we expected that people whose partners were highly central to their lives would be particularly likely to experience jealousy toward their partner’s same-sex friends and that jealousy would be exacerbated when they had reason to doubt their partner’s commitment. Two studies supported our hypotheses. This research highlights how people alter perceptions of their partner’s broader social context to minimize perceived threats to their romantic relationships.

The Global Burden of Disease (GBD) 2010 Study produced comparable estimates of the burden of 291 diseases and injuries in 1990, 2005, and 2010. This article reports on the global burden of untreated caries, severe periodontitis, and severe tooth loss in 2010 and compares those figures with new estimates for 1990. We used disability-adjusted life-years (DALYs) and years lived with disability (YLDs) metrics to quantify burden. Oral conditions affected 3.9 billion people, and untreated caries in permanent teeth was the most prevalent condition evaluated for the entire GBD 2010 Study (global prevalence of 35% for all ages combined). Oral conditions combined accounted for 15 million DALYs globally (1.9% of all YLDs; 0.6% of all DALYs), implying an average health loss of 224 years per 100,000 population. DALYs due to oral conditions increased 20.8% between 1990 and 2010, mainly due to population growth and aging. While DALYs due to severe periodontitis and untreated caries increased, those due to severe tooth loss decreased. DALYs differed by age groups and regions, but not by genders. The findings highlight the challenge in responding to the diversity of urgent oral health needs worldwide, particularly in developing communities.

Background: Host genes serving potential roles in virus replication may be exploited as novel antiviral targets. Methods: Small interfering RNA (siRNA)-mediated knockdown of host gene expression was used to validate candidate genes in screens against six unrelated viruses, most importantly influenza. A mouse model of influenza A virus infection was used to evaluate the efficacy of a candidate FDA-approved drug identified in the screening effort. Results: Several genes in the PI3K-AKT-mTOR pathway were found to support broad-spectrum viral replication in vitro by RNA interference. This led to the discovery that everolimus, an mTOR inhibitor, showed in vitro antiviral activity against cowpox, dengue type 2, influenza A, rhino- and respiratory syncytial viruses. In a lethal mouse infection model of influenza A (H1N1 and H5N1) virus infection, everolimus treatment (1 mg/ kg/day) significantly delayed death but could not prevent mortality. Fourteen days of treatment was more beneficial in delaying the time to death than treatment for seven days. Pathological findings in everolimus-treated mice showed reduced lung haemorrhage and lung weights in response to infection. Conclusions: These results provide proof of concept that cellular targets can be identified by gene knockout methods, and highlight the importance of the PI3K-AKT-mTOR pathway in supporting viral infections.

Several events on university campuses in recent years have raised awareness about the importance of effective mass emergencies notification systems (MNS). These systems are developed to deliver critical information during life threatening events. Congress created a grant program specific for purchasing mass notification technology. When a mass notification system is in place an individual or group of individuals can send out prerecorded messages or customized messages to any or all of the system's various group lists. Message delivery can be sent using text messaging, e-mail, PDAs, sirens, flashing strobes, intercoms, outdoor emergency phones, computer screen displays, cell and landline phones, and live or recorded voice announcements. Schools, universities, medical buildings, cities, law enforcement, and military have embraced such systems. Any organization can use mass notification to better their emergency management. These systems can also be used to distribute non-emergency messages such as breaking news that is of interest to students or employees. Legislation has been introduced in the U.S. House of Representatives to require colleges to immediately notify their students and employees when an emergency happens on campus. As recent incidents have demonstrated, minutes can mean the difference between life and death. Mass notification technology helps ensure that information is communicated, empowering students and employees so that they can take steps to protect themselves during a crisis. Students, parents, teachers, and college instructors have already embraced these systems. Missouri University of Science and Technology (Missouri S&T) installed an MNS to notify the campus community of emergency information via phone, email, SMS Text, and instant messaging. The Missouri S&T expanded its emergency notification system in September 2007 through a partnership between the University of Missouri and 3n (National Notification Network), the leading mass notification system provider after the incident in February 2007 at Missouri S&T. The university has since tested the system twice. This paper presents the results of the tests, issues that have been identified that affect the system's effectiveness, and recommendations for other organizations wishing to implement such systems. This paper also reviews these notification systems including a discussion and analysis of many features common in emergency notification systems. To address the issues that affect the system's effectiveness the results of two system tests conducted by Missouri S&T are analyzed. Both system test results indicate low confirmation rates although there is a slight improvement in the second test. The reasons behind this ineffectiveness are examined with an on campus survey. The survey results illustrated that even though the mass emergency notification system features are mostly recognized by students, the lack of awareness and belief in the mass emergency notification systems may be the reasons behind the low confirmation rates. Therefore, organizations should pay significant attention to both implementing the right mass emergency notification system and creating the awareness and emphasizing the importance for it.

This article presents an overview of ethical issues surrounding psychopharmacology as it applies to the field of couple and family counseling. The authors ground their discussion of these issues in the ethical codes of the American Counseling Association and the International Association of Marriage and Family Counselors. Case studies illustrate the ethical issues that couple and family counselors may face when working with clients who have been prescribed psychotropic medications.

A longitudinal daily diary study examined the origins and consequences of perceiving a partner's acceptance and love as contingent on professional success. Both members of 154 couples completed a diary for 21 days. Multilevel analyses revealed that low self-esteem men and women felt more accepted and loved by their partner on days when their professional lives were marked by success, and low self-esteem women felt less accepted and loved on days when their professional lives were marked by failure. No such spillover effects between people's professional and relationship lives emerged for people high in chronic selfesteem. A 1-year longitudinal follow-up revealed that people who initially felt less accepted across days reported decreased satisfaction. Men also became especially distressed when their wives felt less accepted initially and (incorrectly) perceived their husbands' regard as contingent.

Objectives To quantify population-level bias in self-reported weight and height as a function of age, sex, and the mode of self-report, and to estimate unbiased trends in national and state level obesity in the USA. Design Statistical analysis of repeated cross-sectional health examination surveys (the National Health and Nutrition Examination Survey [NHANES]) and health surveys (the Behavioral Risk Factor Surveillance System [BRFSS]) in the USA. Setting The 50 states of the USA and the District of Columbia. Results In the USA, on average, women underreported their weight, but men did not. Young and middle-aged (565 years) adult men over-reported their height more than women of the same age. In older age groups, over-reporting of height was similar in men and women. Population-level bias in self-reported weight was larger in telephone interviews (BRFSS) than in-person interviews (NHANES). Except in older adults, height was over-reported more often in telephone interviews than in-person interviews. Using corrected weight and height in the year 2000, Mississippi (31%) and Texas (30%) had the highest prevalence of obesity for men; Texas (37%), Louisiana (37%), Mississippi (37%), District of Columbia (37%), Alabama (37%), and South Carolina (36%) for women. Conclusions Population-level bias in self-reported weight and height is larger in telephone interviews than in-person interviews. Telephone interviews are a low-cost method for regular, nationally- and sub-nationally representative monitoring of obesity. It is possible to obtain corrected estimates of trends and geographical distributions of obesity from telephone interviews by using systematic analysis which measure weight and height from an independent sample of the same population.

To feel secure in romantic relationships, people need to believe that their partners see qualities in them that merit attention, nurturance, and care. This article examines how finding (or failing to find) this sense of security affects three facets of romantic life: (a) the inferences people draw about their partners' regard for them in threatening situations, (b) the inferences people draw about their own value in situations in which they feel rejected, and (c) the kinds of behavioral strategies (whether protective of the self or promotive of the relationship) that people adopt to minimize the likelihood of feeling hurt or rejected again.

The goal of the Global Burden of Disease 2010 Study has been to systematically produce comparable estimates of the burden of 291 diseases and injuries and their associated 1,160 sequelae from 1990 to 2010. We aimed to report here internally consistent prevalence and incidence estimates of severe tooth loss for all countries, 20 age groups, and both sexes for 1990 and 2010. The systematic search of the literature yielded 5,618 unique citations. After titles and abstracts were screened, 5,285 citations were excluded as clearly not relevant to this systematic review, leaving 333 for full-text review; 265 publications were further excluded following the validity assessment. A total of 68 studies—including 285,746 individuals aged 12 yr or older in 26 countries—were included in the meta-analysis using modeling resources of the Global Burden of Disease 2010 Study. Between 1990 and 2010, the global age-standardized prevalence of edentate people decreased from 4.4% (95% uncertainty interval: 4.1%, 4.8%) to 2.4% (95% UI: 2.2%, 2.7%), and incidence rate decreased from 374 cases per 100,000 person-years (95% UI: 347, 406) to 205 cases (95% UI: 187, 226). No differences were found by sex in 2010. Prevalence increased gradually with age, showing a steep increase around the seventh decade of life that was associated with a peak in incidence at 65 years. Geographic differences in prevalence, incidence, and rate of improvement from 1990 to 2010 were stark. Our review of available quality literature on the epidemiology of tooth loss shows a significant decline in the prevalence and incidence of severe tooth loss between 1990 and 2010 at the global, regional, and country levels.

The authors argue that individuals with more negative models of self are involved in less satisfying relationships because they have difficulty believing that they are loved by good partners. Dating and married couples completed measures of self-models, perceptions of the partner’s love, perceptions of the partner, and relationship well-being. The results revealed that individuals troubled by self-doubt underestimated the strength of their partners’ love. Such unwarranted insecurities predicted less positive perceptions of their partners. In conjunction, feeling less loved by a less-valuable partner predicted less satisfaction and less optimism for the future than the partner’s feelings of love and commitment warranted. A dependency regulation model is described, where feeling loved by a good, responsive partner is thought to represent a sense of felt security that diminishes the risks of interdependence and promotes closeness.

Both campus involvement and person-environment fit have been identified as factors predicting students' persistence in college. The Student Activities Interest Questionnaire (SAIQ) was developed in order to assist students in identifying campus involvement opportunities that match their personal interests, based on a classification system corresponding to Holland's hexagonal model of occupational preference. In this article, the authors describe the construction and validation of the instrument, and discuss its potential uses in promoting student retention.

Managed care is increasingly subject to political micromanagement and to attacks in the press. Managed care is undergoing the kind of bureaucrat bashing familiar to government employees, because managed care plans are being asked to perform the same type of allocation of social resources (in this case, access to health care) typically conducted by government employees. In so doing, managed care has run afoul of two deeply ingrained American traditions: bureaucrat bashing and overhead democracy. This article uses a case study of managed care bashing to argue that private interests who perform a role in allocating social resources will be subject to the same type of criticism government employees face in allocating social resources (bureaucrat bashing), as well as political leaders’ impulses toward control (overhead democracy). The ombudsman model of governing managed care is advanced as an alternative to managed care bashing and overhead democracy.

Parents’perceptions and expectations of health care providers within the context of the pediatric lung transplant experience were investigated using a phenomenological approach. Fifteen parents of 12 children were interviewed. Two theme clusters, concerned care and collaborative care, were formulated. Themes that reflected parents’perceptions of concerned care included: being treated as an individual, seeing familiar faces, feeling that their children really mattered, and conversely, experiencing a feeling of abandonment. Parents’ perceptions of collaborative care included: being part of the team, and conversely, feeling caught in the middle. The theme cluster, concerned care, reflected the value parents place on continuity of care. Humanistic nursing theory provided the link between study results and nursing practice.

There is little informationfrom the perspective of parents living with a child who has undergone lung transplantation. A phenomenological approach integrating Schutz's postulatesfor the construction of social reality with Colaizzi's method of analysis was used to investigate the meaning of the experience. Fifteen parents of 12 children were interviewed. Theme clusters reflecting the meaning of the stages of the transplantation experience were formulated. Parents perceived lung transplantation as creating hope for the future, creating a context of uncertainty and an opportunity for normalcy, and promoting the development of a new perspective on life. Humanistic nursing practice theory provided the link between study results and nursing practice. Implicationsfor nursing practice were identified.

Excitatory amino acids can modify the tone of cerebral vessels and permeability of the blood-brain barrier (BBB) by acting directly on endothelial cells of cerebral vessels or indirectly by activating receptors expressed on other brain cells. In this study we examined whether rat or human cerebromicrovascular endothelial cells (CEC) express ionotropic and metabotropic glutamate receptors. Glutamate and the glutamate receptor agonists N-methyl-d-aspartate (NMDA), α-amino-3-hydroxy-5-methyl-isoxazole-4-propionic acid (AMPA), and kainate failed to increase [Ca2+]iin either rat or human microvascular and capillary CEC but elicited robust responses in primary rat cortical neurons, as measured by fura-2 fluorescence. The absence of NMDA and AMPA receptors in rat and human CEC was further confirmed by the lack of immunocytochemical staining of cells by antibodies specific for the AMPA receptor subunits GluR1, GluR2/3, and GluR4 and the NMDA receptor subunits NR1, NR2A, and NR2B. We failed to detect mRNA expression of the AMPA receptor subunits GluR1 to GluR4 or the NMDA receptor subunits NR11XX, NR10XX, and NR2A to NR2C in both freshly isolated rat and human microvessels and cultured CEC using reverse transcriptase polymerase chain reaction (RT-PCR). Cultured rat CEC expressed mRNA for KA1 or KA2 and GluR5 subunits. Primary rat cortical neurons were found to express GluR1 to GluR3 and NR1, NR2A, and NR2B by both immunocytochemistry and RT-PCR and KA1, KA2, GluR5, GluR6, and GluR7 by RT-PCR. Moreover, the metabotropic glutamate receptor agonist 1-amino-cyclopentyl-1 S, 3 R-dicorboxylate (1 S,3 R-trans-ACPD), while eliciting both inositol trisphosphate and [Ca2+]iincreases and inhibiting forskolin-stimulated cyclic AMP in cortical neurons, was unable to induce either of these responses in rat or human CEC. These results strongly suggest that both rat and human CEC do not express functional glutamate receptors. Therefore, excitatory amino acid-induced changes in the cerebral microvascular tone and BBB permeability must be affected indirectly, most likely by mediators released from the adjacent glutamate-responsive cells.

It is proposed that satisfying, stable relationships reflect intimates' ability to see imperfect relationships in somewhat idealized ways-to make a leap of faith. Both members of dating and married couples completed a measure of relationship illusions, tapping idealized perceptions of the partners' attributes, exaggerated perceptions of control, and unrealistic optimism. Results of concurrent analyses revealed that relationship illusions predicted greater satisfaction, love, and trust, and less conflict and ambivalence in both dating and marital relationships. A longitudinal follow-up of the dating sample revealed that relationships were more likely to persist the stronger individuals' initial illusions. Relationship illusions also predicted increases in later satisfaction but not vice versa. These results suggest that positive illusions capture a prospective sense of conviction or security that is not simply isomorphic with satisfaction.

Although positron emission tomography (PET) assesses myocardial viability (V) accu rately, a rapid, inexpensive substitute is needed. Therefore, the authors developed a low- dose (1 mCi) Iodine-123-Iodophenylpentadecanoic Acid (IPPA) myocardial viability scan requiring analysis of only the first three minutes of data acquired at rest with a standard multicrystal gamma camera. Twenty-one patients > 2 weeks after myocardial infarction (MI) (24 MIs, 10 anterior, 14 inferoposterior, 21 akinetic or dyskinetic) had cardiac catheterization and resting IPPA imaging. V was determined by either transmural myocar dial biopsy during coronary bypass surgery (12 patients, 14 MIs) or reinjection tomo graphic thallium scan (9 patients, 10 MIs), and 50% of MIs were viable. The IPPA variables analyzed were: time to initial left ventricular (LV) uptake in the region of interest (ROI), the ratio of three-minute uptake in the ROI to three-minute LV uptake, three-minute clearing (counts/pixel) in the ROI (decrease in IPPA after initial uptake), and three-minute accumulation (increase in IPPA after initial uptake) in the ROI. Rules for detecting V were generated and applied to 10 healthy volunteers to determine normalcy. While three-minute uptake in nonviable MIs was only 67% of volunteers (P < 0.0001) and 75% of viable MIs, uptake alone identified only 50% of viable MIs and 75% of nonviable MIs. IPPA clearing, however, was ≥ 13.5 counts/pixel in 10/12 (83%) of viable MIs, and IPPA accumulation ≥ 6.75 counts/pixel identified one more viable MI, for a sensitivity for V of 11/12 (92%), with a specificity of 11/12 (92%), and a 100% normalcy rate. The authors conclude low-dose IPPA (five-minute acquisition with analysis of the first three minutes of data) has potential for providing rapid, inexpensive V data after MI. Since newer multicrystal cameras are mobile, IPPA scans can be done in emergency rooms or coronary care units, generating information that might be useful in decisions regarding thrombolysis, angioplasty, or bypass surgery.

This article explores the narratives that individuals construct about their romantic partners. The authors believe that individuals weave stories that encapsulate or defuse negativity, such as evidence of an intimate's faults. Such storytelling essentially masks feelings of uncertainty engendered by the specter of negativity and thus bolsters felt confidence. The paradigm used for examining narrative construction is described and the postulates or principles guiding the storytelling process are outlined. A discussion follows of how individuals integrate and defuse negativity within positive stories about their partners. The article concludes by exploring the content and structure of confidence-instilling narratives.

Having observed polychlorinated biphenyls (PCBs) in brain tissue obtained post mortem from two men we have carried out a study of organochlorine compounds in frontal cortex from patients with Parkinson's disease (PD) and from controls. No PCBs were found in any of those samples. There was no difference in the concentra tion of the DDT metabolite pp'-DDE in the PD brain samples. Dieldrin (HEOD) was significantly decreased in PD brain when analysed by lipid weight. While these findings would not support the hypothesis that PCBs may contribute to the development of Parkinson's disease in humans it remains possible that they may cause damage to the basal ganglia before being displaced from brain tissue.

Only 13% of postmenopausal advanced breast cancer patients responded to endocrine treatment with aminoglutethimide plus danazol whilst 33% responded to aminoglutethimide alone, despite a similar suppression of serum oestradiol levels in the two groups. The patients on aminoglutethimide and danazol showed a marked suppression of sex hormone binding-globulin binding capacity and a consequent rise in percent-free oestradiol. This led to the concentration of free oestradiol being less well suppressed than that of total oestradiol and in some patients free oestradiol concentrations were higher on treatment than before. Patients on aminoglutethimide alone showed no change in the binding of oestradiol and the percentage suppression of the free oestradiol concentration was similar to that of total oestradiol. The opposing effects of danazol and aminoglutethimide on the concentration of the free, biologically active fraction of oestradiol in the circulation may explain the poor therapeutic efficacy of this combination in breast cancer patients.
