Dynamic reflexivity is central to enabling flexible and emergent qualitatively driven inductive mixed-method and multiple methods research designs. Yet too often, such reflexivity, and how it is used at various points of a study, is absent when we write our research reports. Instead, reports of mixed-method and multiple methods research focus on what was done rather than how it came to be done. This article seeks to redress this absence of emphasis on the reflexive thinking underpinning the way that mixed- and multiple methods, qualitatively driven research approaches are thought about and subsequently used throughout a project. In the worlds inhabited by qualitative inquirers working in health-related areas, health care, evidence, qualitative research, and qualitative researchers are four areas of potential tension and, at times, collision. These areas, or at least aspects of them, are constantly reinvented and realigned as the effects of such encounters are navigated. This article is about some of these close encounters, what we might learn from them, and how we might use this to "survive" as qualitative inquirers in an evidence-based world of health care and health care delivery.

Often discussions about collaborative research, and collaboration generally, begin at the point of how to collaborate, who to collaborate with, and what to collaborate about. Rarely do they include equally important questions of why we are having discussions about collaboration, where such an impetus and emphasis is coming from, and how it connects to the contemporary political research context. In a recent editorial in Qualitative Health Research, Janice Morse highlighted the need for reflection about collaboration. This article responds to that call, providing reflections on collaboration, the imperative to collaborate, and what this all might mean for both qualitative research and qualitative researchers. I hope to stimulate new points of departure for thinking and action shaping collaborative research endeavors without-and just as crucially, within-qualitative research.

Health care is a space occupied, shaped, and colonized by a variety of players at a variety of times. Such players include health practitioners of different kinds; individuals referred to variously and even simultaneously as consumers/patients/clients/customers, depending on who is doing the speaking and in what contexts; families and significant others; managers; accountants; lawyers; educators; and researchers, as well as governments both local and national. Players move in and out of this space (or are allowed in or are moved out) all the time. They might find parts of themselves/their lives/their situations in this space, whereas other parts are not found there. Health care is indeed a contested and troubled space, one that is increasingly uncertain and ambiguous. In this article, I explore aspects of this uncertainty and ambiguity in current health care practice and what this might mean for individuals receiving the services and those providing and/or researching those services.

Genital herpes is a prevalent sexually transmitted viral infection. While genital herpes is not life-threatening, it can cause physical discomfort and psychosocial difficulties, and may increase the risk of contracting HIV. Given that genital herpes cannot be cured, both the condition itself, and the possibility of passing it on to others, becomes a part of the everyday reality of those individuals diagnosed with genital herpes. In this article we explore the ways in which people with genital herpes attending the Sexually Transmitted Infections (STI) clinic govern their 'contagious bodies'. The discussion draws on the Foucauldian concept of governmentality, and uses Foucault's idea of ethics as a framework to identify the technologies of the self by which individuals with genital herpes govern their own thoughts and behaviours in relation to the contagiousness of the condition. Implications for practice and other ways of thinking about what happens in the STI clinic context are suggested.

This introduction to the Tenth Anniversary Issue surveys articles that have been published in health: since its launch. Reviewing the original aims of the journal, the editors discuss some of the main issues that authors have raised, both about health and illness. Focusing upon articles that have been published in this particular journal, we discuss the way that new medical technologies--particularly global ones--have shaped ideas about disease and its treatment, and in consequence about what 'good health' should be. Related to the growth in medical and other technology--not least the rise of the Internet during the life of this journal--is the number of articles that discuss the rights of patients and the establishment of what might be termed an 'illness culture'. We conclude that there continues to be more to health than it being the background to illness or disease, and ask the question: should the 'taken for grantedness' of health be taken for granted any longer?

One of the challenges facing qualitative researchers in a climate in which audit culture has permeated many facets of the institutions in which they research is how to establish the impact and quality of their research. When examining track records, granting institutions place significant emphasis on publication performance. Although the quality and impact of publications have traditionally been assessed by peer review, there is currently a global trend toward the development, refinement, and increased use of quantitative metrics, particularly citation analysis and journal impact factor. In this article, the authors share their experience of using the metrics citation analysis and journal impact factor in the preparation of an application for funding. Their aim is twofold: to raise awareness about potential issues in the practical application of these metrics; and to offer critique about and, they hope, "quality" to the writing and rhetoric concerning how to measure publication impact and quality.

Discourse analysis is a qualitative research approach that offers the potential to challenge our thinking about aspects of the reality of health and health care practice. In this article, the author explores one approach to discourse analysis and examines how it offers possibilities for different ways of viewing health and health care practices. She concludes by raising questions as to whether discourse analysis is at the margins of qualitative research, whether that matters, and where discourse analysis might take those margins.

This exploratory, descriptive study examined the search and selection process for an aged care facility following discharge of a family member from an acute setting. Few studies have examined this process and its effects on families. Individuals from 25 families where a family member had been recently admitted to an aged care facility following discharge from an acute setting were interviewed. This article reports participants' perceptions of the search and selection process and its effect on the family. Five major themes emerged from the data: good fortune, wear and tear on the sponsor, dealing with the system, urgency, and adjusting. The results can be used to inform and assist families and health professionals working with families in this situation.

Objective: Although the state of Oklahoma has traditionally reported very high incidence rates of Rocky Mountain spotted fever (RMSF) cases, the incidence of RMSF among the American Indian population of the state has not been studied. The authors used data from several sources to estimate the incidence of RMSF among American Indians in Oklahoma., Methods: The authors retrospectively reviewed an Indian Health Service (IHS) hospital discharge database for 1980-1996 and available medical charts from four IHS hospitals. The authors also reviewed RMSF case report forms submitted to the Centers for Disease Control and Prevention (CDC) for 1981-1996., Results: The study data show that American Indians in the IHS Oklahoma City Area were hospitalized with RMSF at an annual rate of 48.2 per million population, compared with an estimated hospitalization rate of 16.9 per million Oklahoma residents. The majority of cases in the IHS database (69%) were diagnosed based on clinical suspicion rather than laboratory confirmation. The incidence of RMSF for Oklahoma American Indians as reported to the CDC was 37.4 cases per million, compared with 21.6 per million for all Oklahoma residents (RR 1.7, 95% confidence interval [CI] 1.5, 2.1)., Conclusions: Rates derived from the IHS database may not be comparable to state and national rates because of differences in case inclusion criteria. However, an analysis of case report forms indicates that American Indians n Oklahoma have a significantly higher incidence of RMSF than that of the overall Oklahoma population. Oklahoma American Indians may benefit from educationa campaigns emphasizing prevention of tick bites and exposure to tick habitats.

Flanagan's critical incident technique was used to explore the beliefs of a cross-section of careers (both nursing and others) and consumers about the value of nursing and the nature of nursing's contribution in aged and extended care. The exploratory study found that nurses' being there, with and for residents, their families, and other health professionals, was influenced by two major themes that emerged from the data: personal and structural considerations. In this article, the potential of critical incident technique as a research method in nursing and for generating information about critical aspects of nursing work in aged and extended care is demonstrated.
