During the early part of the past century, hundreds of clinical studies involving more than 37,000 patients were conducted that showed radiotherapy (RT) to be a successful and safe alternative to drug therapy for the treatment of many diverse inflammatory conditions and diseases (e.g. tendonitis, bursitis, arthritis, and serious inflammatory lung conditions). Data from these studies were collected and analyzed with the intent of estimating an optimal dosing range for RT that would induce an efficacious treatment response. RT was reported to be frequently effective after only a single treatment, with a rapid (within 24 h) and often long-lasting (from months to years) relief from symptoms. Over a two-decade span from the 1920s to the 1940s, the therapeutic responses to a single RT treatment consistently improved as the dosing for multiple ailments decreased over time to between 30 roentgen (r) and 100 r. These findings are significant and in agreement with a number of contemporary reports from Germany where RT has been commonly and successfully employed in treating ailments with an inflammatory origin. A proposed mechanism by which RT mitigates inflammation and facilitates healing is via the polarization of macrophages to an anti-inflammatory or M2 phenotype.

The topic of hormesis research funding has been a focus of deliberation within the scientific community for several decades. A common assumption/belief is that most hormesis research is funded by the private sector. With this assumption may emerge questions revolving around potential bias of such research. To provide some clarification to this issue, all hormesis research articles were obtained through online databases for 5-year increments starting with 1995 and ending with 2015 and were subsequently categorized by their funding source. A total of 710 articles were found for those years and 383 of those reported information on funding sources. Reporting funding is not required by law and until more recently was not encouraged or required by funders, research institutions, and/or scientific publishers. The analysis revealed that the assumption that the majority of hormesis research has been privately funded was not supported, with the public sector (i.e. federal and state governmental agencies) exclusively contributing to 78% of the reported research funding. Going forward, funding transparency for scientific research as a whole is essential within the scientific community as it may affect how research may be perceived, accepted, and applied.

In 2006, Henschler disputed the claim of Calabrese and Baldwin that Hugo Schulz should be considered the originator of the hormesis concept. Henschler cited an 1854 paper by Rudolf Virchow on the effects of two agents on the beating of cilia, which showed a hormetic-biphasic dose response. The interpretation of Henschler became broadly accepted over the past decade based on citations in the literature. However, a recent translation of the Virchow paper from German into English reveals that the claims of Henschler are not supported by the article.

The US Air Force Office of Scientific Research convened a meeting of researchers in the fields of neuroscience, psychology, engineering, and medicine to discuss most pressing issues facing ongoing research in the field of transcranial direct current stimulation (tDCS) and related techniques. In this study, we present opinions prepared by participants of the meeting, focusing on the most promising areas of research, immediate and future goals for the field, and the potential for hormesis theory to inform tDCS research. Scientific, medical, and ethical considerations support the ongoing testing of tDCS in healthy and clinical populations, provided best protocols are used to maximize safety. Notwithstanding the need for ongoing research, promising applications include enhancing vigilance/attention in healthy volunteers, which can accelerate training and support learning. Commonly, tDCS is used as an adjunct to training/rehabilitation tasks with the goal of leftward shift in the learning/treatment effect curves. Although trials are encouraging, elucidating the basic mechanisms of tDCS will accelerate validation and adoption. To this end, biomarkers (eg, clinical neuroimaging and findings from animal models) can support hypotheses linking neurobiological mechanisms and behavioral effects. Dosage can be optimized using computational models of current flow and understanding dose-response. Both biomarkers and dosimetry should guide individualized interventions with the goal of reducing variability. Background: Alzheimer's disease (AD) is a highly prevalent neurodegenerative disorder. Rate of decline and functional restoration in AD greatly depend on the capacity for neural plasticity within residual neural tissues; this is at least partially influenced by polymorphisms in genes that determine neural plasticity, including Apolipoprotein E4 (ApoE4) and synaptosomal-associated protein of 25 kDa (SNAP-25)., Objective: We investigated whether correlations could be detected between polymorphisms of ApoE4 and SNAP-25 and the outcome of a multidimensional rehabilitative approach, based on cognitive stimulation, behavioral, and functional therapy (multidimensional stimulation therapy [MST])., Methods: Fifty-eight individuals with mild-to-moderate AD underwent MST for 10 weeks. Neuro-psychological functional and behavioral evaluations were performed blindly by a neuropsychologist at baseline and after 10 weeks of therapy using Mini-Mental State Examination (MMSE), Functional Living Skill Assessment (FLSA), and Neuropsychiatric Inventory (NPI) scales. Molecular genotyping of ApoE4 and SNAP-25 rs363050, rs363039, rs363043 was performed. Results were correlated with ΔMMSE, ΔNPI and ΔFLSA scores by multinomial logistic regression analysis., Results: Polymorphisms in both genes correlated with the outcome of MST for MMSE and NPI scores. Thus, higher overall MMSE scores after rehabilitation were detected in ApoE4 negative compared to ApoE4 positive patients, whereas the SNAP-25 rs363050(G) and rs363039(A) alleles correlated with significant improvements in behavioural parameters., Conclusions: Polymorphisms in genes known to modulate neural plasticity might predict the outcome of a multistructured rehabilitation protocol in patients with AD. These data, although needing confirmation on larger case studies, could help optimizing the clinical management of individuals with AD, for example defining a more intensive treatment in those subjects with a lower likelihood of success., (© The Author(s) 2016.)

The goal of this study was to apply image registration-based automated segmentation methods to measure diffusion tensor imaging (DTI) metrics within the canine brain. Specifically, we hypothesized that this method could measure DTI metrics within the canine brain with greater reproducibility than with hand-drawn region of interest (ROI) methods. We performed high-resolution post-mortem DTI imaging on two canine brains on a 7 T MR scanner. We designated the two brains as brain 1 and brain 2. We measured DTI metrics within the corpus callosum of brain 1 using a hand-drawn ROI method and an automated segmentation method in which ROIs from brain 2 were transformed into the space of brain 1. We repeated both methods in order to measure their reliability. Mean differences between the two sets of hand-drawn ROIs ranged from 4% to 10%. Mean differences between the hand-drawn ROIs and the automated ROIs were less than 3%. The mean differences between the first and second automated ROIs were all less than 0.25%. Our findings indicate that the image registration-based automated segmentation method was clearly the more reproducible method. These results provide the groundwork for using image registration-based automated segmentation methods to measure DTI metrics within the canine brain. Objective: This study examined the effect of mucopolysaccharidosis (MPS) type 1 on diffusion tensor imaging (DTI) metrics in the canine brain. We hypothesized 1) white matter regions in the MPS brain would show decreased fractional anisotropy (FA) and increased radial diffusivity (RD) compared to the same regions in normal brain, 2) compared to FA, RD would more closely correlate with myelin density and fiber coherence, and 3) DTI and histological data from the normal brain could be used to accurately predict degree of myelination in the MPS brain using DTI metrics., Methods: We performed DTI imaging on one normal canine brain and two MPS brains on a 7T MR scanner and generated FA and RD maps. Brains were sectioned and stained with a gold chloride stain for myelin to obtain myelin optical density and fiber coherence values. The three brains were compared using the DTI and histology metrics., Results: Most measured regions in one MPS brain and all measured regions in the other MPS brain exhibited decreased FA, increased RD, and decreased myelin density in white matter. FA and RD significantly correlated with myelin density in the normal brain but failed to reach significance in either MPS brain. A predictive model using FA but not RD was able to accurately predict degree of myelination in one MPS brain., Conclusion: Dysmyelination in the MPS brain results in decreased FA and increased RD. The goal of this study was to determine the degree to which ex vivo diffusion tensor imaging (DTI) parameters correlate to one another in white matter regions on very high resolution MR scans. Specifically, we hypothesized that radial diffusivity (RD) and apparent diffusion coefficient (ADC) would correlate more closely than either would correlate with fractional anisotropy (FA). We performed post mortem DTI imaging on three canine brains on a 7 T MR scanner (TR = 100 ms, NEX = 1, gradient amplitude = 600 mT/m, b = 1492-1,565 s/mm²) and generated maps of FA, RD, and ADC. We measured RD, FA and ADC within 14 regions of interest representative of various portions of white matter. We compared the three combinations of values, i.e., FA vs ADC, FA vs RD and ADC vs RD, using linear regression models. Linear regression demonstrated that RD was significantly correlated with FA (p << 0.01; R² = 0.3053) and also with ADC (p << 0.01; R² = 0.6755), but to a much greater degree. However, ADC was not significantly correlated with FA (p = 0.526; R² = 0.101). Our findings suggest that both RD and ADC reflect similar cytoarchitectural features, most likely that of myelination, whereas FA values likely reflect both myelination and additional microstructural features that constrain the diffusion of water in white matter., (© The Author(s) 2015 Reprints and permissions:sagepub.co.uk/journalsPermissions.nav.)

Purpose: This article provides an historical assessment of the role of radiotherapy in the treatment of inner ear infections., Materials and Methods: The research utilized a literature-based evaluation of the use of x-rays during the first half of the 20th century on the treatment of otitis media (OM), mastoiditis, and cervical adenitis and their impact on the occurrence of deafness., Results: X-Rays were consistently found to be effective as a treatment modality at relatively low doses, in the range of 10-20% of the skin erythema dose, rapidly reducing inflammation, and accelerating the healing process. The mechanistic basis of the clinical successes, while addressed by contemporary researchers, is evaluated in the present article in light of current molecular biology advances, which indicate that clinically effective low doses of ionizing radiation act via the creation of an anti-inflammatory phenotype in highly inflamed tissue., Conclusions: X-Ray treatment of OM, mastoiditis, and cervical adenitis was widely accepted in the first half of the 20th century by clinicians as an effective treatment when administered within an appropriate dosage range.

This study examined the relationship between histological variables and diffusion tensor imaging (DTI) values in a normal canine brain. We hypothesized that radial diffusivity (RD) would correlate with myelin density and fractional anisotropy (FA) would correlate with white matter fiber coherence. We acquired DTI maps of a normal canine brain post mortem on a 7T MR scanner (TR = 100 ms, TE = 18.1 ms, NEX = 1, width [d] = 4 ms, separation [D] = 8.9 ms, gradient amplitude = 600 mT/m, b=1,565 s/mm(2)) and generated maps of FA, RD, and axial diffusivity. The brain was subsequently sectioned and stained for myelin with gold chloride, which also allowed for measurement of fiber coherence. DTI metrics were then directly compared with the optical density of the myelin stain and the coherence of stained fibers. Multivariate linear regression demonstrated that RD, but not FA, significantly correlated with both myelin stain intensity (p = 0.031) and fiber coherence (p = 0.035). The Pearson correlation coefficient between these two histological variables and FA was 0.122; and was 0.607 for the histological variables and RD. We found that RD significantly correlated with both optical density of myelinated fibers and fiber coherence, but FA correlated with neither histological finding. Factors other than degree of myelination and fiber coherence should be considered to fully account for regional variation in FA.

This article offers a broad assessment of the hormetic dose response and its relevance to biomedical researchers, physicians, the pharmaceutical industry, and public health scientists. This article contains a series of 61 questions followed by relatively brief but referenced responses that provides support for the conclusion that hormesis is a reproducible phenomenon, commonly observed, with a frequency far greater than other dose-response models such as the threshold and linear nonthreshold dose-response models. The article provides a detailed background information on the historical foundations of hormesis, its quantitative features, mechanistic foundations, as well as how hormesis is currently being used within medicine and identifying how this concept could be further applied in the development of new therapeutic advances and in improved public health practices.

Palladium (Pd) is a heavy metal belonging to the platinum group elements. It has been shown that Pd and its compounds can cause sensitization, asthma and dermatologic disorders. In this study, Wistar rats were exposed for 3 months to 0, 1, 10, 100, and 250 ng/ml of Pd (as potassium hexachloro-palladate) in drinking water. At the end of exposure, possible changes in two type-1 cytokines [interleukin (IL)-2, interferon (INF)-gamma] and one type-2 cytokine (IL-4) in the serum were measured. After the sub-chronic exposure, analysis of variance of IL-2 and INF-gamma response shows that the difference between mean values in the groups of animals exposed reaches statistical significance for IL-2 (P = 0.001), showing a J-shaped dose-response relationship. At the higher dose of 250 ng/ml Pd, it was observed a significant increase in IL-2 production when compared with the controls. Furthermore at 1 and 10 ng/ml of Pd we observed an increase of INF-gamma production that was significant at 100 ng/ml of Pd, while at the higher dose of 250 ng/ml of Pd the response was indistinguishable from the control. At the doses investigated, Pd has been shown not to determine a modification of IL-4 response (P = 0.10). These preliminary findings indicate the need to carry out further investigations regarding the effect of other Pd compounds and the measurement of other types of cytokines both in the animal model used in our study and other models.

Neurofibromatosis regroups at least two different autosomal dominant genetic disorders: neurofibromatosis type 1 (NF1) and neurofibromatosis type 2 (NF2). Vascular disease is an underestimated complication of NF1. Few studies are available on this, all based on case reports. Neurofibromin, NF1 protein product, has also been detected in aortic smooth muscle. The purpose of this study was to evaluate the physical properties of the vessels, by measuring the carotid-femoral pulse wave velocity (PWV). This parameter was assessed by the Complior, a new noninvasive, validated device, used to screen a large population. The authors studied 64 neurofibromatosis patients (34 boys and 30 girls) with a mean age of 12 years (range 5-25 years). To investigate the presence of vascular lesions, aortic stiffness was evaluated by carotid-femoral PWV by using an automatic processor (Complior). They compared data from the PWV with a control group (30 healthy children, 17 boys and 13 girls, mean age 11 years, range 5-23 years). The calculated mean PWV in the control group was 6.5 +/- 1.15 m/s. The mean PWV of the 64 young patients with NF1 was 6.3 +/- 1.02 m/s. There was no difference between the two groups (p=0.39). Nevertheless, analysis of the linear regression has shown a linear relationship between systolic blood pressure (SBP) and PWV in the control group, while in NF1 patients this relationship is not present. The authors suggest that the coexistence of different factors, such as intimal proliferation, thinning media, fragmentation of the elastic tissue, irregularity, stenosis and tortuosity of the vessels, dysplasia of the small vessels, that counterbalance PWV, normalize the mean value. They emphasize the importance of a careful vascular evaluation, using noninvasive method, such as Complior. This device is well accepted by NF1 patients.

Despite the long history of hormesis-related experimental research, no systematic effort to describe its early history has been undertaken. The present paper attempts to reconstruct and assess the early history of such research and to evaluate how advances in related scientific fields affected the course of hormesis-related research. The purpose of this paper is not only to satisfy this gap in current knowledge but also to provide a foundation for the assessment of how the concept of hormetic dose-response relationships may have affected the nature of the bioassay, especially with respect to hazard assessment practices within a modern risk assessment framework.

Despite the substantial development and publication of highly reproducible toxicological data, the concept of hormetic dose-response relationships was never integrated into the mainstream of toxicological thought. Review of the historical foundations of the interpretation of the bioassay and assessment of competitive theories of dose-response relationships lead to the conclusion that multiple factors contributed to the marginalization of hormesis during the middle and subsequent decades of the 20th Century. These factors include the following: (a) the close association of hormesis with homeopathy, which led to the hostility of modern medicine toward homeopathy, thereby creating a guilt-by-association framework, and the carryover influence of that hostility toward hormesis in the judgements of medically based pharmacologists/toxicologists; (b) the emphasis of high-dose effects linked with a lack of appreciation of the significance of the implications of low-dose stimulatory effects; (c) the lack of an evolution-based mechanism(s) to account for hormetic effects; and (d) lack of appropriate scientific advocates to counter aggressive and intellectually powerful critics of the hormetic perspective.

The development of a comprehensive database of chemical hormetic responses (i.e., U- or inverted U-shaped dose-response relationships) using objective a priori study design, statistical and study replication criteria has recently been reported. An assessment of this database reveals the existence of a wide range of hormetic dose-response relationships including those demonstrating a direct stimulation or an overcompensation response to a disruption of homeostasis. These two broad types of hormetic responses are affected by temporal factors and display unique patterns of dose-range stimulation, magnitude of stimulatory response and relationship of the maximum stimulatory response to the NOAEL. A general classification of U-shaped dose-response relationships is proposed to provide a more organized framework to evaluate the highly distinctive and diverse hormetic responses within the context of establishing underlying biological mechanisms and exploring risk assessment implications.

In vitro inhibition studies with beta-glucuronidase from purified E. coli and mouse intestinal contents indicated that the polymer, poly(vinylbenzyl D-glucaro(1,4)lactonate, is an effective beta-glucuronidase inhibitor. Purified E. coli beta-glucuronidase was inhibited by 99.6% with 177 mM D-glucaro(1,4)lactone using the polymer-inhibitor. Similarly, 95% inhibition of beta-glucuronidase activity of mouse intestinal contents was obtained with 177 mM and 50% inhibition was obtained with 31.5 mM D-glucaro(1,4)lactone based on the modified polymer. The structural requirements of an effective beta-glucuronidase inhibitor based on the structure of the polymer-inhibitor are also discussed.

The interactive potential of three known peroxisome proliferators, omega-3 fatty acids, clofibrate and di(2-ethylhexylphthalate (DEHP), was evaluated in male weanling Wistar rats for the effect on peroxisomal beta-oxidation. Omega-3 fatty acids were supplied by menhaden oil which was fed in six regimens: low fat (5% w/w), low fat and clofibrate (0.3% w/w) or DEHP (0.25% w/w), high fat (20% w/w), high fat and clofibrate or DEHP in the aforementioned concentrations. Induction of peroxisomal beta-oxidation was measured by changes in liver-to-body weight ratio, fatty acyl-CoA oxidase (FAO) activity, and peroxisomal bifunctional enzyme (PBE) quantity. Analysis of transformed data indicated a less than additive response in FAO activity with no deviation from additivity seen with liver-to-body ratios and PBE.

D-Glucaro(1,4)lactone, a potent beta-glucuronidase inhibitor was attached via the carboxylic acid moiety to polyvinylbenzyl chloride (PVBC) using a bimolecular nucleophilic displacement reaction. First, the caesium salt of D-glucaro(1,4)lactone was prepared by titrating the carboxylic acid to neutrality with aqueous caesium bicarbonate. The polyvinylbenzyl D-glucaro(1,4)lactonate was obtained in maximum yields of between 50 and 60% when caesium D-glucaro(1,4)lactonate was incubated with PVBC in DMF at 50 degrees C for 7 d in the presence of a catalyst, caesium iodide.

Simultaneous exposure to chemicals which can oxidize the haemoglobin of the red blood cell to methaemoglobin is common. Although the effects of some of these agents have been documented individually, little research considers the interactive effects. In-vitro experiments on the treated blood of female Dorset sheep assessed the interactive capacity of chlorite, copper and nitrite to affect methaemoglobin formation. All combinations of doses which produced 2.5, 5, 10% methaemoglobin were tested in all possible combinations (a total of 80), as were the controls. This included data on each chemical alone, each two-way combination and the three-way combination. The response is largely additive (the sum of the individual effects) except for one of the two-way interactions, chlorite/nitrite (P less than .01), which showed antagonism. Chlorite may oxidize nitrite which could explain the less-than-additive response. Overall, the result of combining these agents on methaemoglobin was additive.

The interaction potential of peroxisome proliferators of similar and dissimilar structure was examined in B6C3F1 mice. Mice were fed diets containing varying concentrations of ciprofibrate (Cipro), clofibrate (Clof) or di(2-ethylhexyl)phthalate (DEHP), or combinations of Cipro and Clof or Cipro and DEHP for 4 d. Induction of peroxisomal beta-oxidation, measured by increased acyl-CoA oxidase activity, was used as the endpoint for analysis. An additive response occurred following joint exposure to the structurally related compounds Cipro and Clof, whereas a possible synergistic response occurred at low dose combinations of the structurally dissimilar Cipro and DEHP. These findings represent the first report assessing the in-vivo interaction potential of structurally similar and dissimilar peroxisome proliferators and provides insight into the dose-response nature of joint exposures to certain non-genotoxic carcinogens.

A previously published report on soil ingestion in children, revealed evidence that one of the subjects, a 3.5-year-old female, displayed soil-pica behaviour. Soil ingestion estimates for this child ranged from approximately 5 to 7 g d-1 for seven of eight soil tracers employed. The present paper revealed that the soil-pica behaviour occurred only in the second of the two weeks of observation, during which daily soil ingestion rates ranged from 10 to 13 g d-1. Tracer recovery studies displayed acceptable precision (i.e. 100% less than +/- 30%) for six of the eight tracers during the soil-pica episode (week 2). Evaluation of the weekly soil ingestion values of the remaining 63 subjects indicated no convincing evidence of soil-pica behaviour in this group of children aged 1-4.

The effects of oral supplementation of ascorbic acid (1500 mg d-1) on urinary beta-glucuronidase (beta-G) activity was assessed in a double-blind crossover study design over a 3-week period. The subjects on ascorbic acid treatment displayed a statistically significant (P less than 0.05) decrease in beta-G with an approximate decrease of 25%.

Continuing epidemiologic studies at the University of Massachusetts have examined the hypothesis that elevated levels of sodium (Na) in drinking water contribute to elevations of blood pressure (BP). Comparing tenth graders from a town with 107 mg Na/L in the drinking water to those from a town with 8 mg Na/L revealed statistically significant and medically important higher BP distributions among the high Na town students relative to their low Na town peers for both systolic and diastolic BP in both boys and girls. The differences were upheld when potentially confounding factors, including dietary Na intake and other water factors occurring differentially in the two water supplies, were controlled in the analysis. A replication study among third graders in the same communities showed similar results.

The health implications of a mean increase in blood pressure on the order of 5 mm Hg among young persons are explored primarily from a public health point of view. After an overview of the epidemiologic behavior of human blood pressure is provided, the phenomenon of "tracking" of blood pressure is examined as is the possible relationship between the rate of blood pressure change over time and the initial value. In the last part of the paper, the community benefits of lowering the average blood pressure 5 mm Hg (in terms of number of heart attacks prevented) are estimated under several simplifying assumptions.

An experimental bottled water study assessed the effect on blood pressure of lowering Na concentration in the water of some of the high sodium community fourth graders. For three months, trios of children matched by sex, school, and baseline BP each used different water for all cooking and drinking purposes, with BP monitored bi-weekly. Pupils were randomly allocated to the three water conditions: high sodium water bottled from their own community distribution system, low sodium water bottled from the distribution system of the comparison community with sodium added to the level of the high sodium community water and low sodium water bottled from the distribution system of the low sodium community but with no sodium added. The results indicate that BP levels among the girls but not boys on the low sodium water exhibited marked decreases in BP over the test period when compared to the other two groups.
