Background: Dyslipidaemia is an important cardiovascular risk factor for people with severe mental illness, contributing to premature mortality. The link between antipsychotics and dyslipidaemia is well established, while evidence on antidepressants is mixed. Aims: To investigate if antidepressant/antipsychotic use was associated with lipid parameters in UK Biobank participants and if CYP2C19 and CYP2D6 genetic variation plays a role. Methods: Review of self-reported prescription medications identified participants taking antidepressants/antipsychotics. Total, low-, and high-density lipoprotein cholesterol (L/HDL-C) and triglycerides derived from blood samples. CYP2C19 and CYP2D6 metabolic phenotypes were assigned from genetic data. Linear regression investigated aims, adjusted for key covariates. Results: Of 469,739 participants, 36,043 took antidepressants (53% female, median age 58, 17% taking cholesterol-lowering medications) and 3255 took antipsychotics (58% female, median age 57, 27% taking cholesterol-lowering medications). Significant associations were found between use of each amitriptyline, fluoxetine, citalopram/escitalopram, sertraline, paroxetine and venlafaxine with higher total cholesterol, LDL-C, and triglycerides and lower HDL-C, compared to participants not taking each medication. Venlafaxine was associated with the worst lipid profile (total cholesterol, adjusted mean difference: 0.21 mmol/L, 95% confidence interval (CI): 0.17 to 0.26, p Conclusions: Antidepressants were significantly associated with adverse lipid profiles, potentially warranting baseline and regular monitoring. Further research should investigate the mechanistic pathways underlying the protective effects of the CYP2C19 intermediate metaboliser phenotype on HDL-C and triglycerides in people taking sertraline.

Glucose 6 Phosphate Dehydrogenase (G6PD) enzyme activity estimation in a freshly collected blood sample is the most widely used diagnostic method for the diagnosis of G6PD deficiency. The objective is to evaluate the need for newborn screening for G6PD deficiency over post-malarial diagnosis and the feasibility and reliability of using dried blood spots (DBS) as samples for screening. A total of 562 samples were analyzed for G6PD and parallel measurement of G6PD activity by the colorimetric method in whole blood and DBS was carried out in the neonatal subset. Among 466 adults, 27 (5.7%) showed G6PD deficiency, out of whom 22 (81.48%) were diagnosed after the malarial encounter. In the pediatric group, 8 neonates showed G6PD deficiency. G6PD activity estimated from DBS samples agreed with whole blood, with a statistically significant strong positive correlation. Screening of G6PD deficiency at birth to prevent future unwarranted complications, using DBS is feasible.

Purpose of study: The need for forgiveness education for nursing self-care and forgiveness facilitation has risen. Therefore, the present pilot study tested the efficacy of an 8-week forgiveness bibliotherapy with a small number of undergraduate nursing students. Design of study: Matched pairs of nursing students were randomly assigned to either the experimental group or no-contact control group. The experimental group, using 8 keys to forgiveness by R. Enright (2015) as the treatment manual, read one chapter a week for 8 weeks and provided weekly reflections. Forgiveness and forgiveness-related outcome measures were administered at pretest, posttest, and one-month follow-up. Findings: At the posttest, the experimental group had significantly greater improvement in forgiveness compared to the control group with a large effect size, which was maintained at one month follow-up. There was no other significant difference between the two groups. Within-group comparisons of the experimental group showed improvement in forgiveness, anxiety, depression, and fatigue from pre to post testing periods and forgiveness, anger, anxiety, depression, and fatigue from pre to follow-up testing periods. Conclusion: Use of bibliotherapy may be a cost-effective way to promote the virtue of forgiveness for students in nursing programs.

This paper reports findings of a qualitative study examining the perceptions of 21 Australian women professionals who conduct home visiting with families experiencing intimate partner violence. There is scant evidence documenting how home visiting professionals adapted practice to address the safety concerns of women and their children within the context of the pandemic. Practitioners noted an increase in the risk level and complexity of intimate partner violence (IPV), including the ways that perpetrators weaponized the pandemic to exert power and control over women and children. Practitioners reported on their rapid adaptation of practices, to ensure the continuation of services which included moving to online delivery methods, wearing PPE, and negotiating practice from a distance. While responses to these changes were mixed, most reported their desire to continue to use online platforms post-pandemic, reporting increased safety, flexibility, and accessibility for the majority of clients. This research addresses a gap in respect of professionals’ perceptions of the issues facing survivors of IPV and of their professional practice during the COVID-19 pandemic. As policies, practices, and protocols continue to adapt to the challenging environment posed by the pandemic the experiences of professionals and service users are critical to inform these changes.

Outcome expectations of direct pulp capping in carious teeth are obscured by a clinically unknown infiltration and breakdown of the dental pulp tissue. Histologic studies showed that this soft tissue breakdown is related to the innate immune system. We hypothesized 1) that a neutrophil biomarker could predict the outcome of direct pulp capping and 2) that using sodium hypochlorite (NaOCl) as a lavage solution to remove necrotized infected pulp tissue could improve it. In this randomized trial in mature posterior teeth causing no or mild discomfort with carious pulpal exposures, pulpal fluid was collected to assess neutrophil gelatinase (matrix metalloproteinase 9 [MMP-9]) per total protein (TP) levels as a predictive local biomarker. Subsequently, the dentin-pulp wound was randomly washed with a 2.5% NaOCl or a physiologic saline solution (1:1 allocation), capped with mineral trioxide aggregate, and the tooth was immediately restored with a resin-based composite restoration. Ninety-six patients were included, and 84 individuals could be followed up to treatment failure or clinically confirmed pulp survival after a minimum of 1 y. The entire data were fitted to a Cox proportional hazards model to assess the influence of the observational variables MMP-9/TP and discomfort with the randomized lavage treatment on pulp survival. Thoughts about life after school can be frightening for college students. The uncertainty about employment expectancies can engender crippling anxiety, especially in a time of a major pandemic—COVID-19, and urgent attention is needed. This study, drawing on the self-determination theory, demonstrates preliminary protective effect of positive psychological capital (PsyCap) on employment anxiety among a relatively understudied group—graduating college students (Chinese sample = 546). It further illustrates the mediating mechanism of coping styles in this relation. Thus, the motivational impetus of PsyCap facilitates positive coping style (PCS) while diminishing negative coping style (NCS) which, in turn, hinders students’ employment anxiety. Furthermore, the results revealed that students’ internship experience strengthens the influence of graduating students’ PsyCap on their PCS, but that with NCS and anxiety was nonsignificant. This research proffers valuable insights on college students’ from-school-to-work transition for higher education institutions and career counselors, particularly in this turbulent labor market.

One of the popular missional consensuses in the context of migration is seeing migrants as “moving targets” for evangelism. There is an urge to respond differently realising that migrants are not merely workers for economic welfare but persons created in the image of God. To reconstruct a model of mission that is embedded in the complex reality of migration, this paper will look into the details of three narratives of Indonesian and Filipino migrants who live and attend a Charismatic church in Sabah, Malaysia. It will give insights into the process of othering and discover the quality of compassion in their lives. Based on their experiences, I argue mission as a compassionate presence provides a sound theological basis for churches to respond to the continuous othering of migrants. It invites us to rethink evangelism, not solely as winning souls for Christ, but to embody Christ by being present.

This qualitative descriptive study analyzed the role that natural mentors had in cultivating a sense of purpose and meaning among 38 diverse male and female high school students. The types of mentoring support provided and the students’ perceived experiences of receiving the support were considered in the analysis. Directed content analysis revealed four initial overarching types of supportive actions that were provided by mentors: emotional, instrumental, informational, and appraisal support. The types of support offered were further categorized across three dimensions of perceived support by the mentees: (a) affirmation (e.g., the experience of emotional and appraisal support), (b) cultivation (e.g., the experience of being challenged and obtaining skills), and (c) guidance (e.g., the experience of receiving information and advice). In order for mentees to perceive and accept cultivation and guidance support, they first needed to experience an affirming relationship with their mentors. Findings may inform best practices for mentoring youth toward lives of meaning and purpose.

The adverse effects of climate change on agricultural productivity are on the increase. This study employed both descriptive statistics and the multivariate probit model to estimate factors constraining the adoption of climate change adaptation strategies among smallholder farmers in the study area. The empirical results of the multivariate probit model showed that a lack of knowledge of climate change constraints was influenced by smallholder farmers’ age, gender, off-farm activity, susceptibility and membership in farmer-based organizations. Thus, to improve the adaptive capacity of farmers, government and development partners should work together to improve the conditions under which farmers can gain access to climate change information and suitable agricultural credit as well as policy incentives aimed at lowering the stringent conditions of borrowing in the agricultural sector.

The International Classification of Diseases– 11 th Revision (ICD-11) includes the diagnosis of complex posttraumatic stress disorder (CPTSD). Clinical practice guidelines support the use of phased care for individuals with CPTSD. This case study illustrates the use of synchronous telehealth to deliver phased treatment to a rural woman veteran with CPTSD. Mrs. A experienced sexual, physical, and emotional abuse throughout her life, perpetrated by family members, intimate partners, and military authority figures. She sought treatment for posttraumatic nightmares and body image issues; she also had pain related to fibromyalgia and chronic migraine headaches. Mrs. A participated in 19 sessions of Skills Training in Affective and Interpersonal Regulation (STAIR) Narrative therapy via synchronous telehealth. Trauma and eating disorder symptoms were assessed before and after treatment and the patient demonstrated clinically significant improvement on measures of these disorders. Patient-provider working alliance and quality of life were assessed post-treatment. Synchronous telehealth use drastically increased with the onset of COVID-19; however, little information on treating CPTSD via synchronous video teleconferencing is available. This case study illustrates an evidence-based, phased therapy for CPTSD while highlighting the feasibility and value of in-home delivery of psychotherapy for CPTSD via synchronous telehealth.

This article is underpinned by the hypothesis that if Australia is to reassess and improve its relationship to and use of rivers and river systems, then more holistic ways of understanding rivers, and strategies for representing and communicating this understanding, must be developed and brought together. Held over two days in August 2019 at the Lismore campus of Southern Cross University, ‘Speaking With the River’ was an interdisciplinary symposium exploring the capacities of creative research practice to develop new understandings of rivers and river systems as simultaneously environmental, cultural, historical and economic phenomena. In this article, we bring together the voices and disciplinary insights from the symposium and the rivers of Northern New South Wales, and we reflect on the way that riverine language ran throughout our discussions and ideas, providing a connective model of confluences and conjunctions for the interdisciplinary enterprise we were engaged in. This article presents perspectives on rivers and river systems from law, history, art and science, exploring common ground and common purposes. Developing a legal framework for recognising the rights and ‘voices’ of rivers, that is informed by Indigenous knowledges, historical contexts, and scientific understanding, and that employs artistic innovation in representation and translation, is to us the ultimate goal of such an enquiry. While this paper does not undertake the formal steps of developing this framework, it provides the necessary background and instantiates its elements and working methods within the context of the Richmond River in Northern New South Wales.

Realist evaluation submits that theories and models of how, why, for whom and under what circumstances programs work could be formulated by conceptualizing the relational links between the context within which programs are implemented, the generative mechanisms the programs trigger, and the outcomes of interest. Qualitative and quantitative data collection and analysis allow for the description of the relevant context, the generative mechanisms, and the emergent outcomes of programs and provide explanatory power to link these elements. The ‘realist interviewing technique’, whereby interviewees comment on a suggested ‘program theory’ to provide refinement, is proposed as a distinctive approach for conducting interviews in a realist-informed inquiry. However, the application of this interviewing strategy within the realist evaluation studies is underutilized. In this study, we demonstrate how the realist interview technique reinforces and maintains theoretical awareness and contributes to trustworthiness through three theory-building phases: theory gleaning, theory refining, and theory consolidation.

This analysis of functional family therapy (FFT) studies examines whether their variable outcomes are attributable to implementation issues. Studies were identified firstly, by way of a recent overview, supplemented by an update of a highly sensitive search including 15 databases, 10 websites, all existing relevant reviews, gray literature as well as contacting experts in the field. Updated searches were conducted in August 2018 and were analyzed according to the Oxford Implementation Index and an assessment of supervision quality. In total, the search yielded 150 records; 48 full texts were retrieved of which 32 were excluded leaving 16 studies containing 5,320 unique participants included for analysis. There was no evidence of reported harm. Improved training and supervision were associated with better core outcomes. Although there was no apparent dose relationship, it appears that implementation issues are important and also that class and ethnicity were identified for areas of further study.

Data from this article are derived from responses to questionnaires provided by 232 frontline workers and team leaders from a range of organizations across four Australian states who participated in The Invisible Practices Project: Engaging With Fathers Who Use Violence. This project was designed to explore the practices of workers who see perpetrators of domestic violence outside the relative safety of group-work programs. Thematic analysis was employed to examine the research question: What is the organizational context that contributes to, or constrains, the practices of frontline workers intervening with fathers who use violence? This article reports the ways that agencies increase the visibility of, and practices with, fathers who use violence. Increasing the visibility of fathers who use violence is posited as a crucial driver of policy and practice reform aligned with feminist goals of eliminating sexist institutional practices. It is argued that such practices render fathers who use violence invisible, reinforce patriarchal views of family roles and responsibilities, intensify the scrutiny on the mothering of women survivors, and compound the impact of domestic violence. This article documents emerging practices that seek to redress practices that support patriarchal views of family roles and responsibilities and has implications for understanding the capacity of social workers and other professionals to intervene with fathers who use violence.

New models of career education are needed to prepare young people for changes and challenges in the world of work. We propose that the psychology of working framework/theory (PWF/PWT) has the potential to shape career education in transformative ways that are attentive to shifting dimensions of the local context and the marginalization of large segments of the world population in seeking access to decent work. In this article, we present theory and research supporting the PWF/PWT and its application for career development intervention, with specific attention to the constructs of youth purpose and critical consciousness as important resources for coping with massive changes in the world of work and growing inequality. We provide examples of how this might be done, drawing from our current research–practice partnerships that are designing, delivering, and evaluating PWF-/PWT-informed career intervention.

Persistent hiring discrimination as demonstrated by correspondence experiments incites immigrant job candidates and their descendants to modify their résumé to hide their immigrant status, that is, résumé whitening. To date, we have little to no empirical evidence on how common this is in practice. We test the extent of résumé whitening with a representative survey of immigrants in Switzerland (N = 7659). Around 9% of the immigrants used some résumé whitening. Immigrants appear to use résumé whitening strategically when experiencing or anticipating discrimination. Future correspondence experiments should take this into account to maximize external validity.

This qualitative descriptive study examined the process of purpose development of nine (seven females, two males; Mage = 20.2, age range 18–21 years) college students who completed interviews in fall 2017 and spring 2019 at a private university. Across the two time points, participants engaged in an iterative process that led them to refine (i.e., narrow or specify) or redefine (i.e., change or adapt) their purpose or ultimate aims. Participants used the time between interviews to restructure their purpose in a way that integrated their experiences until they believed their purpose was best aligned with their skills, values, and interests. More specifically, consensual qualitative research analysis revealed seven themes that suggested ways in which participants acted to refine and/or redefine their purposes: (1) clarifying definition of purpose; (2) engaging known strengths or skills; (3) exploring new activities or experiences; (4) identifying a beyond-the-self intention that aligned with their purpose; (5) discerning the fit between their passions and interests with their circumstance or plans; (6) determining whether their strengths and skills aligned well with their purpose; and (7) integrating their passions into their beyond-the-self intention. A supplemental theme was added that outlines researchers’ observations about the features of participants’ purpose.
