Background: Correction of high posterior tibial slope is an important treatment option for revision of anterior cruciate ligament (ACL) failure as seen in clinical and biomechanical studies. In cases with moderate to severe medial compartment arthritis, an additional varus correction osteotomy may be added to improve alignment., Purpose: To investigate the influence of coronal and sagittal correction high tibial osteotomy in ACL-deficient and ACL-reconstructed knees on knee kinematics and ACL graft load., Study Design: Controlled laboratory study., Methods: Ten cadaveric knees were selected according to previous computed tomography measurements with increased native slope and slight varus tibial alignment (mean ± SD): slope, 9.9°± 1.4°; medial proximal tibia angle, 86.5°± 2.1°; age, 47.7 ± 5.8 years. A 10° anterior closing-wedge osteotomy, as well as an additional 5° of simulated varus correction osteotomy, were created and fixed using an external fixator. Four alignment conditions-native, varus correction, slope correction, and combined varus and slope correction-were randomly tested in 2 states: ACL-deficient and ACL-reconstructed. Compressive axial loads were applied to the tibia while mounted on a free-moving X-Y table and free-rotating tibia in a knee testing fixture. Three-dimensional motion tracking captured anterior tibial translation (ATT) and internal tibial rotation. Change of tensile forces on the reconstructed ACL graft were recorded., Results: In the ACL-deficient knee, an isolated varus correction led to a significant increase of ATT by 4.3 ± 4.0 mm ( P = .04). Isolated slope reduction resulted in the greatest decrease of ATT by 6.2 ± 4.3 mm ( P < .001). In the ACL-reconstructed knee, ATT showed comparable changes, while combined varus and slope correction led to lower ATT by 3.7 ± 2.6 mm ( P = .01) than ATT in the native alignment. Internal tibial rotation was not significantly altered by varus correction but significantly increased after isolated slope correction by 4.0°± 4.1° ( P < .01). Each isolated or combined osteotomy showed decreased forces on the graft as compared with the native state. The combined varus and slope osteotomy led to a mean decrease of ACL graft force by 33% at 200 N and by 58% at 400 N as compared with the native condition ( P < .001)., Conclusion: A combined varus and slope correction led to a relevant decrease of ATT in the ACL-deficient and ACL-reconstructed cadaveric knee. ACL graft forces were significantly decreased after combined varus and slope correction. Thus, our biomechanical findings support the treatment goal of a perpendicular-aligned tibial plateau for ACL insufficiencies, especially in cases of revision surgery., Clinical Relevance: This study shows the beneficial knee kinematics and reduced forces on the ACL graft after combined varus and slope correction.

Background: Recent biomechanical and clinical studies have demonstrated the effectiveness of suture button and pulley-type fixations for surgical treatment of acromioclavicular instability. Concerns remain that such procedures can "overconstrain" (overreduce the lateral clavicle in relation to the acromion to a nonphysiological position) the joint. Purpose/Hypothesis: The purpose of this study was to investigate the intra-articular pressure of native and reconstructed acromioclavicular (AC) joints in relation to the configuration of the joint. Anatomic (0 mm), overconstrained (-3 mm), and underconstrained (+3 mm) AC joint reconstructions were simulated. The hypothesis was that reconstructions using suture pulley systems do not increase the intra-articular pressure of the AC joint., Study Design: Controlled laboratory study., Methods: Eleven fresh-frozen cadaveric shoulders were used in this study (mean age ± SD, 60.8 ± 6.7 years). Each specimen underwent radiographic analysis by using a Zanca view to determine the basic configuration of the AC joint. A pressure Tekscan sensor was inserted in the AC joint. A servohydraulic materials testing system was used for testing. The specimens were kept in the testing machine, and the native AC position was marked at 0 mm. This allowed moving the clavicle during the surgical procedure with reference to the native anatomic position. Intra-articular pressure in the native AC joint during cyclic loading (1000 cycles; 1 Hz) was measured. After native testing, the AC ligaments and coracoclavicular ligaments were cut and reconstructed using a cortical button technique. Anatomic, -3 mm, and +3 mm positions, relative to the acromion, were cyclically loaded, and intra-articular pressure was documented., Results: According to the AC joint classification of inclination, we identified five type 1 (46%), four type 2 (36%), one type 3 (9%), and one incongruous (9%) configurations. Changes in superior displacement across the 4 conditions were not statistically significant (0.5 ± 0.8 [native], 0.01 ± 0.00 [0 mm repair], 0.02 ± 0.02 [-3 mm repair], and 0.01 ± 0.01 [+3 mm repair]; P = .162). Before testing (time point 1), pressure in the -3 mm repair (62.9 ± 70.1) differed between the native state (11.3 ± 21.8; P = .042) and the +3 mm repair (7.1 ± 18.4; P = .023). All other changes at time points 2 (after cyclic loading unloaded) and 3 (after cyclic loading loaded) in pressure were not significant. ( P = .086 and .226, respectively)., Conclusion: AC joint reconstruction (within -3 to +3 mm of reduction) with a coracoclavicular suture button device does not significantly increase the intra-articular pressure of the AC joint after cyclic loading in our experimental cadaveric setup., Clinical Relevance: Recent biomechanical and clinical studies have demonstrated the effectiveness of suture button and pulley-type fixations for coracoclavicular reconstruction of the joint. Concerns remain that such procedures would "overconstrain" the joint because of the high rigidity of these pulley systems and the preservation of the lateral clavicle. This overconstraining may potentially result in pain at the lateral end of the clavicle, osteolysis, or a later increased risk of early osteoarthritis. Therefore, our results indicate that within a range of ±3 mm to the anatomic position, overconstraining may not result in a higher intra-articular pressure.

Background: Reconstructions of the coracoclavicular (CC) ligament in an open or arthroscopically assisted procedure are often combined with a resection of the distal clavicle to prevent or treat osteoarthritic degenerations of the acromioclavicular (AC) joint. However, increased horizontal translation leading to symptomatic instability may be associated with resection of the distal clavicle., Hypothesis: Horizontal translation increases in direct correlation to subsequent resection of the distal clavicle., Study Design: Controlled laboratory study., Methods: Twelve fresh-frozen specimens (mean age, 82.0 ± 5 years) were examined using a servohydraulic testing system. A 70-N force was applied in the anterior, posterior, and superior directions, and the translations were quantified for all directions. After native testing, sequential resection of the distal clavicle at 5 and 10 mm was performed after incision of the inferior capsule, and anterior-posterior loads were retested. Finally, the AC ligaments were incised, and repeated testing was performed., Results: Native mean (± standard deviation) anterior translation was 7.90 ± 2.62 mm. After section of the inferior AC capsule, anterior translation was 8.05 ± 2.62 mm (P = 1.0). After clavicle resection of 5 mm, anterior translation was 10.75 ± 2.38 mm (P = .122), and after resection of 10 mm, anterior translation was significantly increased to 11.6 ± 2.37 mm (P = .012). Complete AC capsule sectioning led to significantly greater amounts of anterior translation (12.12 ± 3.0 mm; P = .003). Posterior translation for the native specimen was 8.88 ± 2.63 mm. Sectioning of the inferior AC capsule resulted in 9.21 ± 2.71-mm translation (P = 1.0). After clavicle resection of 5 mm, posterior translation was 10.42 ± 2.42 mm (P = 1.0), and after 10-mm resection, it was 11.31 ± 2.86 mm (P = .39). Sectioning of the complete AC capsule led to significantly greater amounts of posterior translation (12.31 ± 3.12 mm; P = .043)., Conclusion: Ten millimeters of resection increased anterior translation of the distal clavicle with both the superior and posterior AC capsules as well as the CC ligaments intact. Both anterior and posterior translations increased after 10-mm resection and complete AC capsule sectioning., Clinical Relevance: These results suggest that resection of the distal clavicle leads to increased horizontal translation despite an intact superior and posterior AC capsule. Only sparing resection of the distal clavicle should be performed and only if strictly indicated. Violation of the AC capsule further increases horizontal translation and should therefore be avoided, or if indicated in AC joint dislocations, a reconstruction of the AC capsule should be considered.
