Abstract

Background/Aims: Physicians’ perception of antecedent physical/cognitive status may account for the suboptimal implementation of acute coronary syndrome (ACS) guidelines. Methods: In an ACS survey of all cardiac wards, physicians’ perception of antecedent physical/cognitive status was prospectively recorded and categorized as either normal, mildly impaired or significantly impaired. We examined the impact of antecedent status on the use of evidence-based medications and procedures and on mortality. Results: Of the 2,021 patients, 1,025 (51%) had ST elevation. Impaired antecedent physical/cognitive status was diagnosed in 417 patients (20.6%), more commonly among non-ST-elevation patients (26.2 vs. 15.2%). Patients with impaired physical/cognitive status, with or without ST elevation, had significantly worse baseline demographic and clinical characteristics. They less often received aspirin, clopidogrel, platelet glycoprotein IIb/IIIa receptor antagonists, statins and β-adrenergic blockers, and significantly less often underwent in-hospital catheterization and revascularization. Reperfusion treatment was given significantly less frequently to ST elevation patients with impaired status (63.0% for normal vs. 50.8% and 33.3% for mildly and significantly impaired status, respectively; p = 0.001). After adjustment for differences in baseline characteristics, impaired antecedent status remained independently associated with lower use of these therapies and higher mortality rates. Conclusions: ACS guideline implementation is significantly influenced by physicians’ perception of antecedent physical/cognitive status, and thus is a crucial parameter for understanding ACS management and outcomes.

Abstract

Background: Although clopidogrel and aspirin (dual therapy, DT) are used for acute coronary syndrome (ACS), sometimes treatment with warfarin (triple therapy, TT) is required. Aim: To determine the incidence, complications, and outcomes of TT. Methods: We analyzed Israeli surveys of ACS from 2000 to 2004. Results: In these surveys, 5,706 (96%) were discharged alive from hospital. Post-ACS TT and DT were 76 patients (1.3%) and 2,661 patients (46.7%), respectively. The TT group was older with more prior cardiac disease. During hospitalization, the TT patients received more intravenous anticoagulant and antithrombotic agents, and had more heart failure, arrhythmias, ischemia, and major bleeding (2.6 vs. 0.6%, p = 0.03). There were no differences in adjusted 30-day and 6-month mortality between the 2 groups. Conclusion: TT is feasible among ACS patients who require concomitant warfarin treatment.

Abstract

Background: It remains unknown whether patients with severe decompensated class IV heart failure (HF) receiving intravenous inotropic treatment benefit from cardiac resynchronization therapy (CRT). Methods: We identified patients who underwent urgent CRT implantation due to decompensated class IV HF necessitating intravenous inotropic therapy. Results: Of 10 patients with chronic ischemic cardiomyopathy (median QRS duration of 170 ms), CRT implantation was associated with symptomatic improvement in 8 patients. The mortality rate was 50% during a median follow-up of 9.5 months, with a median CRT-to-death duration of 6 months. Conclusions: CRT was feasible among class IV patients receiving inotropic treatment and was associated with clinical improvement.

Abstract

Background/Aims:A large proportion of patients with a ST-elevation acute coronary syndrome do not receive reperfusion therapy. In order to contribute to a better understanding of the clinical decision making process, we analyzed which factors are associated with the application of reperfusion therapy. Methods: From the Euro Heart Survey of Acute Coronary Syndromes I, 4,260 patients with ST-elevation acute coronary syndrome were selected for the current analysis, of which 1,539 (36%) patients received fibrinolysis and 904 (21%) primary percutaneous coronary intervention (PCI). The analysis contained 32 variables on demographics, medical history, admission parameters and reperfusion therapy. Results: A short pre-hospital delay, arrival in a hospital with PCI facilities, severe ST-elevation, and participation in a clinical trial were the strongest predictors for receiving reperfusion therapy. Primary PCI was more likely to be performed than fibrinolysis in patients with a long pre-hospital delay, arriving in a hospital with PCI facilities, not participating in a clinical trial, and with at least one previous PCI. Conclusion:Hospital facilities and culture, pre-hospital delay and infarction size play a major role in management decisions regarding reperfusion therapy in ST-elevation acute coronary syndrome. This analysis indicates which factors require special attention when implementing and reviewing the reperfusion guidelines.

Abstract

We measured body temperature in 40 consecutive patients treated for a first ST elevation acute myocardial infarction (AMI) with primary percutaneous coronary interventions. Left ventricular function was assessed by echocardiography, and blood samples were drawn for highly sensitive C-reactive protein (hs-CRP), white blood cell (WBC) count, fibrinogen, creatine kinase (CK), and cardiac troponin I levels (cTnI). The median (25th, 75th quartiles) peak 24-hour temperature was 37.4 degrees C (36.9 degrees C, 37.6 degrees C). Variables significantly associated with peak 24-hour temperature were CK (p = 0.01, r = 0.42), wall motion index (p = 0.01, r = 0.41), hs-CRP (p = 0.01, r = 0.41), and cTnI (p = 0.03, r = 0.35). There was no significant correlation between peak 24-hour temperature and WBC count (p = 0.39, r = 0.14) and fibrinogen (p = 0.12, r = 0.21). Thus, peak 24-hour body temperature after ST elevation AMI probably reflects infarct size rather than a nonspecific inflammatory response.

Abstract

Aim: To determine the frequency and outcomes of coronary artery bypass graft (CABG) surgery in patients with a wide spectrum of acute coronary syndromes (ACS). Methods and Results: We prospectively enrolled 10,484 ACS patients from 103 hospitals in 25 countries across Europe and the Mediterranean basin. Of the 10,204 patients with complete data, 460 (4.5%) underwent CABG while in hospital; 3.4% had ST elevation ACS, 5.4% had non-ST elevation ACS, and 4.4% had undetermined ECG ACS (p = 0.001 for non-ST elevation ACS vs. others). In general, patients who underwent CABG were more likely to be males, to have diabetes mellitus, hyperlipidemia, a positive family history of premature coronary disease, and prior angina pectoris, but had less often prior heart failure. While in hospital, all CABG patients underwent coronary angiography and 15.2% also underwent percutaneous revascularization, as compared with 51.3 and 33.1% in the remaining patients, respectively. The in-hospital mortality was 3.7% for ACS patients who underwent CABG and 4.8% for non-CABG ACS patients (p = nonsignificant) with an adjusted odds ratio of in-hospital death for CABG patients of 1.00 (95% CI 0.59–1.61). Conclusions: Approximately 4.5% of ACS patients underwent CABG during their initial hospitalization, with a greater likelihood among non-ST elevation ACS patients. Of the CABG patients, 15.2% also underwent percutaneous revascularization. The outcome of CABG patients was as good as non-CABG patients, indicating that CABG remains an effective and safe means to achieve revascularization among ACS patients in current clinical practice.

Abstract

Objective: To examine whether the outcomes of diabetic patients after successful percutaneous coronary revascularization are influenced by the modality of treatment for hyperglycemia at the time of percutaneous coronary revascularization. Design: Retrospective analysis of the Mayo Clinic PTCA Registry. Material and Methods: We examined whether the modality of treatment of diabetes mellitus (diet alone vs. oral hypoglycemics vs. insulin) impacted on the occurrence of death and death/myocardial infarction among diabetic patients after successful percutaneous coronary revascularization from January 1, 1995 through December 1998. Results: Of the 821 diabetic patients with known treatment status, 129 received diet alone, 370 oral agents, and 322 insulin. The clinical success rates of percutaneous coronary revascularization were similarly high among the groups (>90%). The adjusted risk of suffering either death or death/myocardial infarction after successful percutaneous coronary revascularization was greater for insulin-treated patients (vs. non-insulin): relative risk (95% confidence intervals) of 1.65 (0.99, 2.72) and 1.65 (1.20, 2.27), respectively. Patients treated with oral agents had lower risk of suffering death or death/myocardial infarction than insulin-treated patients (0.59 [0.36, 0.99] and 0.57 [0.38, 0.84], respectively), with a similar trend for patients on diet alone (0.96 [0.48, 1.99] and 0.65 [0.36, 1.16], respectively). Conclusions: Although the clinical success rates of PCR were similar for insulin- and non-insulin-treated diabetic patients, insulin-treated patients had worse long-term outcomes after successful percutaneous coronary revascularization.

9. Aspirin and Percutaneous Coronary Angioplasty Are Associated with a Decline in Mortality from Cardiogenic Shock

Abstract

Background and Methods: Despite the significant progress in the care and outcome of patients with acute myocardial infarction (AMI), the impact of evolving therapies on the incidence and outcome of patients with cardiogenic shock complicating AMI has been questioned. We analyzed trends in the incidence, care and outcome of cardiogenic shock from four national surveys conducted during 1992–1998. Results: Of the 5,351 AMI patients admitted to all coronary care units in Israel, 254 (4.7%) developed cardiogenic shock. The incidence of cardiogenic shock decreased over time (5.8, 5.1, 4.3 and 4.4% for the years 1992, 1994, 1996 and 1998, respectively, p = 0.08). Concomitantly, there was an increase in utilization of coronary angiography, urgent angioplasty and intra-aortic balloon counterpulsation. In addition, there was an increase in hospital use of aspirin, nitrates, ACE inhibitors and β-blockers. Patients with shock were more likely to die within 7 days compared with AMI patients not having shock (65 vs. 4%; p < 0.001). During the study period, the mortality of patients with shock decreased: at 7 days (72% in 1992 to 60% in 1998; p = 0.09), at 30 days (87 to 70%, respectively; p = 0.01) and at 6 months (89 to 77%, respectively; p = 0.02). Both aspirin and angioplasty were independently associated with improved outcome after adjustment for baseline characteristics and study period. Conclusions: Although the mortality rate of cardiogenic shock complicating AMI remains high, the increased utilization of aspirin and angioplasty is associated with improved outcome.

Abstract

Objective: To examine whether the outcome of patients after successful percutaneous coronary revascularization is influenced by diabetic status. Design: Retrospective analysis of the Mayo Clinic PTCA Registry. Materials and Methods: We analyzed the occurrence of all-cause death and death/myocardial infarction among diabetic (n = 2,155) and nondiabetic patients (n = 9,354) after successful percutaneous coronary revascularization at Mayo Clinic from October 1979 through December 1998. Results: Of the 11,509 patients who underwent percutaneous coronary revascularization during the study period, 2,155 were diabetic at the time of their index percutaneous coronary revascularization. The clinical success rate was similar for the two groups (≈87%). Among the patients with clinically successful interventions, the median (25th, 75th interquartiles) time of follow-up was 3.1 (1.1, 6.1) years for diabetic patients and 4.3 (2.0, 8.2) years for nondiabetic patients (p < 0.001). Diabetes mellitus was associated with increased adjusted risk (95% confidence interval) of death (1.73 [1.54, 1.94]) and death/infarction (1.62 [1.46, 1.79]). Conclusions: Although diabetic and nondiabetic patients had similar clinical success rates for percutaneous coronary revascularization, diabetic patients had significantly worse long-term outcomes.
