Background Pharmacists can contribute to improved patient outcomes, improve medicine knowledge, reduce drug costs and minimise errors. However, their role within hospice-based services is not well described. Objective The objective of this paper was to explore the role of pharmacists within UK hospices. Methods: Methods include an online survey and follow-up telephone contact of pharmacists working in UK hospices assessing pharmacist provision, duties, communication, medicine sourcing and training. Results Eighty-nine responses were received from 82 hospices (response rate 50%). Pharmacists had a role in 75% of hospices providing between 6.6 min and 5.5 hrs of pharmacist support per bed per week. The most frequent duty reported was provision of medicines information to the clinical team. Access to patient records varied considerably: 13% had full read and write access to GP records while 29% had no access. Job-specific training had not been received by 36% of the respondents and 47% reported training needs including basic training in palliative care. Conclusions: Three-quarters of UK hospices have pharmacy provision, although this falls below the recommended levels in the majority. Hospice pharmacists lack access to training and records. Medicines sourcing for hospices is variable and could provide opportunities for efficiencies with further research. [ABSTRACT FROM AUTHOR]

Background: Recruitment of patients with advanced cancer into studies is challenging. Objective: To evaluate recruitment methods in a study of pharmacist‐led cancer pain medicine consultations and produce recommendations for future studies. Method: Two methods of recruitment were employed: (1) community‐based (general practitioner computer search, identification by general practitioner, community pharmacist or district nurse and hospital outpatient list search) and (2) hospice‐based (in and outpatient list search). Patients identified in method 1 were invited by post and in method 2 were invited face‐to‐face. Information was designed in collaboration with patients and carers. Results: A total of 128 patients were identified (85 from the community and 43 from the hospice), and 47 met the inclusion criteria. Twenty‐three agreed to take part and 19 completed the study, 17 of whom were already under specialist palliative care. Recruitment rates were 7% for community‐based methods and 40% for hospice. The recruitment methods differed in intensity of resource use. Recruitment via letter and a lack of engagement by healthcare professionals were found to be barriers. Facilitators included the researcher having personal involvement in recruitment. Conclusion: The overall recruitment rate was in line with other studies for this patient cohort. Attempts to identify and engage patients through community‐based postal contact were less effective than where personal contact with patients was both possible and occurred. Objectives: Educational interventions by pharmacists for patients with cancer pain aim to improve pain management, but little is known about the different components of interventions and their effectiveness. Our aim was to assess the benefit of pharmacist delivered educational interventions for patients with cancer pain. A systematic review and meta‐analysis of experimental trials testing pharmacist delivered educational interventions for cancer pain was carried out to identify the components of interventions and effectiveness at improving pain‐related outcomes for patients with cancer. A literature review was conducted in EMBASE, MEDLINE, CINAHL, PsycINFO, ASSIA, Web of Science and CENTRAL from inception until January 2018 searching for educational interventions involving a pharmacist for patients with cancer pain. Four studies were included involving 944 patients. Meta‐analysis was carried out where possible. Key findings: Meta‐analysis of three of the four studies found that mean pain intensity in the intervention group was reduced by 0.76 on a 0–10 scale (95% confidence interval), although only two of the studies used validated measures of pain. Improvements in knowledge, side effects and patient satisfaction were seen although with less reliable measures. Summary: Pharmacist educational interventions for patients with cancer pain have been found to show promise in reducing pain intensity. Studies were few and of varying quality. Further, good quality studies should be carried out in this area and these should be comprehensively reported. Trials measuring patient self‐efficacy and patient satisfaction are needed before the impact of the pharmacist delivered interventions on these outcomes can be established. [ABSTRACT FROM AUTHOR]

Background making informed decisions about cancer care provision for older cancer patients can be challenging and complex. Evidence suggests cancer care varies by age, however the relationship between age and care experiences from diagnosis to death for cancer patients within the UK has not previously been examined in detail. Patients and methods retrospective cohort linking cancer registry and secondary care data for 13,499 adult cancer patients who died between January 2005 and December 2011. Cancer therapies (chemotherapy, radiotherapy, surgery), hospital palliative care referrals, hospital admissions and place of death were compared between age groups using multivariable regression models. Trends in cancer care over time, overall and within age groups were also assessed. Results compared with adult patients under 60 years, patients aged 80 years and over were less likely to receive chemotherapy, radiotherapy, a hospital palliative care referral; or be admitted to hospital but were more likely to die in a care home. Overall, the percentage of patients receiving chemotherapy, surgery, hospital palliative care referrals and hospital admissions have increased while deaths in hospital have decreased. Deaths at home have increased for patients aged 80 years and over. Conclusion older patients are less likely to receive cancer therapies or hospital palliative care before death. Introduction: in the United Kingdom dementia is generally diagnosed by mental health services. General hospitals are managed by separate healthcare trusts and the handover of clinical information between organisations is potentially unreliable. Around 40% of older people admitted to hospital have dementia. This group have a high prevalence of psychological symptoms and delirium. If the dementia diagnosis or symptoms are not recognised, patients may suffer unnecessarily with resulting negative outcomes. Methods: to understand areas of unmet need we have described the prevalence of dementia in over 75 year olds admitted to a general hospital, the accuracy of diagnostic recording, and the prevalence of recorded psychological symptoms and delirium. To achieve this we conducted a retrospective review of 116 patients admitted to hospital with known dementia. Psychiatric and medical notes were reviewed, identifying the accuracy dementia diagnosis recording by the hospital and all episodes of documented psychological symptoms and delirium. Results: the prevalence of documented dementia in the population was estimated at 15%; 74% of dementia diagnoses were recorded in the medical notes; 10% had documented psychological symptoms (depression 4%, anxiety 3%, hallucinations 3%, delusions 4%); and 11% had documented delirium. There were no associations between the specialty providing care and the recognition of dementia or the reporting of symptoms. Discussion: this work suggests an under reporting of dementia and symptoms associated with it in the general hospital. Improving this requires closer collaboration between metal health and hospital healthcare services and training for staff on how to access diagnostic information and recognise common psychological symptoms. Objective To evaluate smartphone apps intended for self-management of pain using quality assessment criteria and usability testing with prospective users. Design 1) Survey and content analysis of available apps; and 2) individual usability study of two apps. Setting University of Leeds, United Kingdom. Participants Forty-one participants (aged 19-59 years) with experience of chronic or recurrent pain episodes. Methods We undertook a survey, content analysis, and quality appraisal of all currently available mobile phone apps for self-management of pain. Two apps were then selected and assessed with usability testing. Results Twelve apps met the inclusion criteria. The quality assessment revealed wide variation in their clinical content, interface design, and usability to support self-management of pain. Very little user or clinician involvement was identified in the development of the apps. From the usability testing, participants stated a preference for an interface design employing a lighter color scheme and particular text font. Although very few participants were aware of pain-reporting apps prior to participation, many would consider use in the future. Conclusions Variation in app quality and a lack of user and clinician engagement in development were found across the pain apps in this research. Usability testing identified a range of user preferences. Although useful information was obtained, it would be beneficial to involve users earlier in the process of development, as well as establishing ways to merge end user requirements with evidence-based content, to provide high-quality and usable apps for self-management of pain. Objective To systematically identify and appraise the current literature of pregabalin in the treatment of neuropathic pain resulting from cancer or cancer treatment. Design A systematic review of the literature was conducted based on Preferred Reporting Items for Systematic Reviews and Meta- Analyses. Interventions and Subjects Studies reporting pregabalin data for adult (>18 years) patients with cancer experiencing neuropathic pain due to cancer or cancer treatment/surgery were considered eligible for inclusion. Methods A literature search was conducted in Pub Med on February 22, 2012 using the following search terms: 'neuropath* AND pain AND cancer OR oncology OR tumor OR tumour AND pregabalin.' Open access journals were also searched. Abstracts were screened and reviewed for eligibility based on predetermined criteria for inclusion. Data reporting pain intensity, pain interference, quality of life, symptom quality and intensity, global impression of change, treatment satisfaction, and adverse effects were the predefined factors for analysis. Data were summarized descriptively due to variations in study outcome measures. Results Five articles were eligible for inclusion; one double-blind National Cancer Institute common toxicity criteria controlled trial, one single-arm open-label study, two observational analyses, and one case report. Conclusions There were limited published data reporting efficacy and safety outcomes for pregabalin in the treatment of neuropathic pain in adult patients with cancer. Due to limitations within the studies included in this review, it is not possible to draw any conclusions on the descriptive summary of pregabalin for the treatment of cancer-related neuropathic pain, and further studies are required. Objectives The aim was to explore and describe community pharmacists' current and potential place in the cancer pain pathway. Objectives were to describe pharmacists' role in advising patients and their carers on optimum use of opioid drugs for pain relief, identify elements of medicines management that could be modified and identify opportunities for improved communication with patients and other professionals. Methods Semi-structured interviews were conducted with 25 community pharmacists in three areas of England. Data were analysed using the Framework method. Key findings Pharmacists had no reliable method to identify patients with cancer and no access to disease stage and treatment plan information. There was little evidence of any routine communication with other professionals about patient care. Contact with patients was limited. Access to palliative care medicines could be problematic for patients and medicines use reviews (MURs) were rarely done. Interview data suggested variable levels of knowledge about optimal opioid use in cancer pain or awareness of patients' priorities. For some pharmacists, proactive involvement appeared to be inhibited by fear of discussing emotional and wider social aspects and accounts showed that a wide range of issues and concerns were raised by family members, indicating considerable unmet need. Pharmacists tended to assume information had already been provided by others and felt isolated from other care team members. Many felt that their potential contribution to cancer pain management was constrained but aspired to do more. Conclusions There is significant scope for improving access to and interaction with, community pharmacists by people with cancer pain and their families. Finding ways to embed pharmacists within palliative care teams could provide a starting point for a greater contribution to cancer pain management. [ABSTRACT FROM AUTHOR]

Background. Neuropathic pain (NP) is a very frequent and unrecognized condition in clinical practice. Therefore, it is important to have a reliable instrument to assess pain subtypes in various cultures. The Leeds Assessment of Neuropathic Symptoms and Signs (LANSS) has been widely used and validated in many countries. Up to now, there has been no reliability study of this instrument in Brazil. Methods. The scores of the Brazilian Portuguese version of the LANSS were studied in a sample of 90 chronic pain outpatients from southern Brazil. LANSS was translated into Portuguese and then back translated to English. Intraclass correlation coefficient (ICC) and internal consistency (IC) were estimated. The intensity of pain complaints, other demographic data, and LANSS scores distribution according to pain subtypes (nociceptive, neuropathic, and mixed) were also evaluated. Results. The Brazilian Portuguese version of the LANSS showed good ICC ( r = 0.97) and IC (Cronbach's α = 0.67 for total LANSS score). Patients with NP provided significant higher LANSS scores (19.1 ± 3.3) in comparison with those with nociceptive (7.3 ± 4.5) and mixed (13.9 ± 3.7) types of pain. Conclusions. This LANSS version was found to be a reliable instrument for the evaluation of pain complaints due to a variety of causes. The profile of pain scores was similar to that observed in other countries. To examine whether the Leeds Assessment of Neuropathic Symptoms and Signs (LANSS) screening tool can satisfy Rasch model expectations and therefore be transformed into an interval level measurement scale, suitable for use as an outcome measure in clinical studies. Rasch analysis (using the software RUMM2020) of LANSS data from both a random selection of all chronic pain patients and also specific chronic pain diagnostic categories. Original LANSS data from a previous study of 2,480 patients with chronic pain were used. Specific diagnostic groups examined included diabetic neuropathy, postsurgical pain, osteoarthritis, posttraumatic injury, and low back pain. The following assessments were made: fit to the Rasch model, scale reliability, scale multi-dimensionality, and differential item functioning. The overall fit to the Rasch model was only acceptable in two groups; diabetic neuropathy and chronic postsurgical pain. For these groups, the scale is unidimensional (measures a single construct) and there is no evidence of differential item functioning across gender and age groups. The scale only has reliability consistent with use at the group level. Neuropathic screening tools such as the LANSS have been used as outcome measures in clinical studies. Rasch analysis demonstrates that the LANSS can be used as such in specific populations of patients with neuropathic pain, however it's reliability in this context does not support use at the individual level and it cannot be used as a generalised measurement tool across pain diagnostic groups. This discussion document about the management of cancer pain is written from the pain specialists' perspective in order to provoke thought and interest in a multimodal approach to the management of cancer pain, not just towards the end of life, but pain at diagnosis, as a consequence of cancer therapies, and in cancer survivors. It relates the science of pain to the clinical setting and explains the role of psychological, physical, interventional and complementary therapies in cancer pain. Methods. This document has been produced by a consensus group of relevant healthcare professionals in the United Kingdom and patients' representatives making reference to the current body of evidence relating to cancer pain. In the second of two parts, physical, invasive and complementary cancer pain therapies; treatment in the community; acute, treatment-related and complex cancer pain are considered. Conclusions. It is recognized that the World Health Organization (WHO) analgesic ladder, whilst providing relief of cancer pain towards the end of life for many sufferers world-wide, may have limitations in the context of longer survival and increasing disease complexity. To complement this, it is suggested that a more comprehensive model of managing cancer pain is needed that is mechanism-based and multimodal, using combination therapies including interventions where appropriate, tailored to the needs of an individual, with the aim to optimize pain relief with minimization of adverse effects. The article discusses a study which aimed to determine the feasibility of collecting pain data using a system accessed through various modes of technology to explore participant preferences for the different modes and gain participant input into potential uses of the data collected. The results indicate the feasibility of collecting pain data on a single system accessed using four modes of technology.
