Although many international relations (IR) theory and international political economy textbooks consistently reference dependency theory, it is commonly considered a passé , outdated, or defunct theoretical approach. This paper challenges conventional wisdom, stressing the continued relevance of dependency as an analytical approach. Overall, it argues that Dependency theory represents a successful effort at decentering IR. To do so, it first discusses decentering as an effort to challenge and engage core concepts in IR to transform the "universal" understandings of global politics. In this sense, Dependency theory decentered IR by introducing an understanding of the world in which Western and non-Western spaces are mutually constitutive, highlighting the role of non-core contexts in creating and maintaining the status quo of the universal. Second, the paper analyzes the influence of Dependency theory in modern discourses of political economy like the resource curse, globalization, Post-Colonialism, and Post-Developmentalism. Sin or public health taxes are excise taxes imposed on the consumption of potentially harmful goods for health [sugar-sweetened beverages (SSBs), tobacco, alcohol, among others], aiming to reduce consumption, raise additional revenue and/or improve population health. This paper assesses the extent to which sin taxes (a) can reduce consumption of potentially harmful goods, (b) raise revenue for national health systems and (c) contribute to population health in Latin America. A systematic literature review was conducted on peer-reviewed and grey literature; endpoints included: impact of raising sin taxes on consumption, ability to raise revenue for health and the possibility of population health improvements. Risk of bias for each study was assessed. The synthesis of the literature on sin tax implementation showed improvements in all three endpoints across the study countries. Following the introduction of sin taxes or by simulating their potential impact, nearly all studies explicitly reported that consumption of potentially harmful goods (mainly SSBs and tobacco) declined; revenue was found to have increased in almost all countries, suggesting that there may be additional scope for further tax increase. Simulated improvements in population health have also been shown, by demonstrating a relationship between sin tax increases and reduction in prevalence of diabetes, stroke, heart attacks and associated deaths. However, sin tax effects on health would be better quantified over the long-term. Data quality and availability challenges did place some limitations on sin tax impact assessment. Sin taxes can be effective in reducing consumption of potentially harmful goods, improve population health and generate additional revenue. It is easy to presume that age's legal value rests in the autonomy and rights that accrue to the liberal (male, propertied) citizen who has reached the age of majority. But this is not universally so. In Spain's American colonies, legal age talk involved multiplying privileges rather than exclusionary subtraction. Few indigenous peoples, enslaved people of African descent, or members of the free casta poor tallied the years they had lived in a manner that meets modern standards of precision. Instead, the ages that Spanish American officials set down on paper in criminal trials, censuses, and freedom suits derived from complicated cultural equations, including reconciling local ethno-numeracies with European counting systems. For all its seeming imprecision, age was of critical importance to colonial Spanish Americans, since rather than guaranteeing access to rights, age was a language that colonial subjects used to turn legal incapacities into beneficial protections. The policies for restructuring health systems in Latin America during the 1990s have included an emphasis on changing in the model of health care delivery to one that incorporates prevention and promotion activities. At the same time, health systems have been decentralized in their management, allowing room for greater variation in local interpretation and implementation of policy directives. Despite rhetoric and policy debate, there is no documentation or evaluation of actual experiences of prevention and promotion within decentralized health systems in Latin America. This paper explores the ways in which the national structure of a health system influences the implementation of activities for prevention and promotion through a comparison of the experiences in four local health systems in each of Brazil and Chile. These experiences in Brazil and Chile are presented by key themes of national health system structure, local health system structure, partnership and intersectorality, human resources and introducing a family health approach. Five clear factors emerge as operating at the national level that influence prevention and promotion activities in local health systems: vertical (Chile) versus horizontal (Brazil) structure of health system; greater awareness of prevention and promotion issues in Chile; greater urban bias in Chile compared with Brazil; strategies to attract human resources to primary care and rural areas; importance of local capacity building especially in rural areas. This account of case study experiences in Brazil and Chile provides a series of examples of arrangements and strategies that can facilitate implementation and usefully highlights a number of issues that policy-makers and health system managers need explicitly to consider. As such, the paper hopes to provoke debate about the structures and strategies for supporting the implementation of prevention and promotion programmes in Latin America and further health systems research in this field. Doubts have been raised about the benefits to an economy with high rates of GDP growth without at the same time accounting for distributional effects. This difference arises from the emphasis by some on the mean income, while others are concerned with the distribution around the mean. This paper assesses the per capita income that would be needed, if income were equally distributed to reach the same level of welfare, which is available from current distribution of the per capita GDP. This approach allows for the combination of both growth and distribution in ranking the success of some Latin American countries in their attempt to raise the living standard of the population. Despite the abundant research on liberation theology and religious change in Latin America, there is little agreement regarding how to assess the political orientation of national churches. This paper reviews previous strategies and proposes that any new approach should do three things. First, the strength of liberation theology should be assessed holistically, that is, by focusing on the official statements and organizational practices of bishops, priests, nuns, and laity. Second, liberation theology should be approached as a “family resemblance concept” and not a “classic concept.” Third, while most scholars identify national churches in nominal terms, I argue liberation theology varies along a continuum, from strong to weak and that its strength varies over time. Background Antimicrobial resistance has worsened in Latin America. There is an urgent need to understand the development of antimicrobial stewardship programs (ASPs) and the barriers to implementing effective ASPs in light of limited national action plans or policies to promote ASPs in the region. Methods We performed a descriptive mixed-methods study of ASPs in 5 Latin American countries in March-July 2022. An electronic questionnaire with an associated scoring system (hospital ASP self-assessment) was used, and ASP development was classified based on the scores (inadequate, 0–25; basic, 26–50; intermediate, 51–75; or advanced, 76–100). Interviews among healthcare workers (HCWs) involved in antimicrobial stewardship (AS) inquired about behavioral and organizational factors that influence AS activities. Interview data were coded into themes. Results from the ASP self-assessment and interviews were integrated to create an explanatory framework. Results Twenty hospitals completed the self-assessment, and 46 AS stakeholders from these hospitals were interviewed. ASP development was inadequate/basic in 35% of hospitals, intermediate in 50%, and advanced in 15%. For-profit hospitals had higher scores than not-for-profit hospitals. Interview data validated the self-assessment findings and provided further insight into ASP implementation challenges, which included limited formal hospital leadership support, inadequate staffing and tools to perform AS work more efficiently, limited awareness of AS principles by HCWs, and limited training opportunities. Conclusions We identified several barriers to ASP development in Latin America, suggesting the need to create accurate business cases for ASPs to obtain the necessary funding for their effective implementation and sustainability. Today it is rare to find a political scientist who rejects that "ideas matter"—at least in the abstract. But if ideationally inclined theorists have gained seats at the disciplinary table, their approaches still occupy disadvantaged positions. Ideational theories typically must confront nonideational alternatives to achieve salient publication, but nonideational theorists routinely design and publish research without considering ideational alternatives. This is even true on topics where all scholars seem to agree a priori on the importance of ideas. Erratic attention to ideas appears to be justified by widespread views that even if ideas plausibly "matter," they are too intractable to address in concrete research: too difficult to measure empirically, to relate in explanatory ways to action, or to connect to goals of theoretical generalization. This article first highlights major problems with these views in the abstract, and then illustrates them in the example of early Latin American constitutional design. On this terrain, there are good reasons to think that an ideational account connects in more concrete ways to available evidence than leading nonideational hypotheses about constitutional choice. In this paper, we explore the state of the art of public affairs education in Latin America and compare it with the evolution and current character of MPA/MPP education in the United States. In doing so, we are able to identify ways in which there is a Latin American model (or at least identifiable patterns) of public affairs education in sharp contrast to what might be expected in terms of emulation of international programs, particularly from the United States and Europe. As the basis for our analysis, we rely on three main sources of information. We use extensive data gathered from a survey of nearly 50 programs of public affairs in Latin America, which we administered in 2015 in order to identify patterns and trends in public affairs education in the region. Then, we compare our data with two data bases: (1) the Atlas of Public Policy and Management (Carleton University and the University of Toronto), which provides an international profile of programs, and (2) NASPAA's Data Center, which summarizes the characteristics of NASPAA-member programs based on Annual Survey data. This comparison allows us to establish to what degree there is a phenomenon of isomorphism in public affairs education between countries in Latin America and similar programs in North America. Background. Numerous epidemiological factors affect the probability of developing breast or ovarian cancer, but no predictor is as determinant as inheriting a mutation in BRCA1 or BRCA2. The concept of the founder effect explains the reduced genetic variability in some populations, according to the theory that new populations can be formed from a reduced number of individuals, so the new population would carry only a small fraction of the genetic variability of the original population. The main purpose of this review is to provide an update on the state of the art in founder mutations and some recurrent mutations that have recently been described in Latin America. Methods. A literature search was performed in the electronic databases of PUBMED,EMBASE, LILACS, and BIREME using the terms BRCA1, BRCA2, founder mutation, Latin American population, and Hispanic. Sixty-two papers were identified, of which 38 were considered relevant for this review. Each result is shown per country. Results. In Latin America, clear founder effects have been reported in Mexico (BRCA1 del exons 9-12), Brazil (BRCA1 5382insC and BRCA2 c.156_157insAlu), and Colombia (BRCA1 3450del4, A1708E, and BRCA2 3034del4) and in Latinas residing in Southern California (BRCA1185delAG, IVS5 + 1G>A, S955x, and R1443x). Of these, mutation BRCA1 3450del4 has also been reported in Brazil and Chile, whereas mutation BRCA2 3034del4 has been reported in Argentina and Peru. These data support the idea that although most Hispanic populations are the result of a mixture between Europeans, Africans, and Amerindians, the relative proportion of each genetic component varies throughout the Hispanic populations, making it necessary to identify the mutations characteristic of each population to generate mutation profiles adjusted to each one of them. Conclusion. In Latin American countries, and even among regions of the same country, there is great heterogeneity of ancestors. Therefore, Latinas should not be analyzed like other population groups without taking into account their genetic ancestry. The presence of founder mutations in specific population groups represents a cost-effective analysis. The importance of determining the founder mutations lies mainly in the decrease in costs. Latin American countries (LAC), with their culturally and ethnically diverse populations, form a region that is difficult to define and to understand. The region's health systems are deeply fragmented, which poses great challenges to overall equity levels in health. This is also one of the fastest ageing regions in the world, with increasing demands as well for acute and long-term care (LTC). Demographic and epidemiological transitions across the region are heterogeneous. In this context, health systems are in general, largely unprepared to face the challenge of promoting healthy ageing. This unpreparedness has been magnified by the Coronavirus disease-2019 pandemic. Here, we analyse the burden of disease in the older population and identify priorities to improve the care and quality of life for people living in LAC. Besides an adequately prepared workforce, we must remediate disparities and inequities; develop and implement integrated care; achieve patient-centred care and further develop palliative and end-of-life care; simultaneously, we must develop the structure and financing of LTC services and strengthen the role of public health making healthy ageing an essential component. A Domar–Pasinetti model of an emerging-market open economy with output growth endogenous to aggregate demand is presented, with foreign debt, interest and exchange rates playing a relevant role in the dynamics of the debt-to-GDP ratio (d). The model is used to assess the macroeconomic effects of the orthodox policy (fiscal austerity) implemented by policy makers to cope with the foreign debt crisis of Latin America in the 1980s. Such policy involved high costs in terms of welfare, job and output losses to no avail, since the region’s financial fragility remained largely unresolved, in the end resulting in the so-called ‘lost decade’. The main policy insight of Domar’s and Pasinetti’s analysis, we argue, posits a pro-growth expansionary policy as the most efficient strategy to abate and bring d to a sustainable path. Finally, we contend that Latin America’s experience provides insight for dealing with the ongoing crisis of the periphery of the eurozone. [ABSTRACT FROM AUTHOR]

Background and Objectives: Breast cancer (BC) is the most common cancer in women. It imposes a huge disease burden and a significant impact on health‐related quality of life (HRQoL). Our study focused on HRQoL of patients with BC in Latin America and the Caribbean (LAC). We conducted a systematic review to identify relevant articles published between 2008 and August 2018. We conducted several meta‐analyses and subgroup analyses by country, disease stage, and instrument used (Prospective Register Of Systematic Reviews registration number: CRD42018106835). Results: From 2,265 initial references, we finally included 75 articles (8,806 participants) that assessed HRQoL. The European Organization for the Research and Treatment of Cancer Quality of Life Questionnaire C30 and B23 modules (34 studies; 8 countries; 4,866 participants) were the most used instruments, followed by the Short Form 36‐item, the abbreviated version of the World Health Organization Quality of Life instrument, and the Functional Assessment of Cancer Therapy – Breast instrument. Only four studies reported specific HRQoL data of patients with metastatic disease. Half the studies were rated as having moderate quality (38/75), and 38% (29/75) as high quality. We identified substantial heterogeneity. As expected, the meta‐analyses revealed that patients with metastatic disease reported lower HRQoL values and high symptom burden compared with patients at earlier stages. Similar results can be observed when we compared patients with early breast cancer in active treatment phases versus those in follow‐up. Conclusion: This study provides a synthesis of breast cancer HRQoL reported in LAC and exposes existing evidence gaps. Patients with BC in active treatment or with metastatic disease had worse HRQoL compared with survivors during the follow‐up period. Implications for Practice: This systematic review provides an exhaustive synthesis of breast cancer health‐related quality of life in women in the Latin American and Caribbean region. Patients with breast cancer in active treatment or with metastatic disease had worse health‐related quality of life compared with survivors during the different follow‐up periods. Owing to the emerging impact of bioinformatics and computational biology, in this article, we present an overview of the history and current state of the research on this field in Latin America (LA). It will be difficult to cover without inequality all the efforts, initiatives and works that have happened for the past two decades in this vast region (that includes >19 million km2 and >600 million people). Despite the difficulty, we have done an analytical search looking for publications in the field made by researchers from 19 LA countries in the past 25 years. In this way, we find that research in bioinformatics in this region should develop twice to approach the average world scientific production in the field. We also found some of the pioneering scientists who initiated and led bioinformatics in the region and were promoters of this new scientific field. Our analysis also reveals that spin-off began around some specific areas within the biomolecular sciences: studies on genomes (anchored in the new generation of deep sequencing technologies, followed by developments in proteomics) and studies on protein structures (supported by three-dimensional structural determination technologies and their computational advancement). Finally, we show that the contribution to this endeavour of the Iberoamerican Society for Bioinformatics, founded in Mexico in 2009, has been significant, as it is a leading forum to join efforts of many scientists from LA interested in promoting research, training and education in bioinformatics. What happens when researchers foreground Latin American and Caribbean agency in design? Guided by this challenge, we sought to create a new approach to the discourse on Latin American and Caribbean design history. Through the formation of a network of scholars working across the globe, the organization of dedicated themed panels at international conferences, and the culmination of these efforts in this edited volume, we propose to decolonize and globalize the study of design history in and from that region. To this end, this Introduction situates the fluctuation of interest in Latin American and Caribbean Studies in Europe and the United States, and discusses how the development of Latin American and Caribbean design history need not follow some of the colonialist premises instigated by area studies' scholars. Rather, through a critical review of historiographical trends found in both area studies and in design studies and history journal articles, this Introduction offers an interpretation of how and why design historical understanding has been produced in and from that region, and points to fruitful developments in this scholarship. Recent debates about challenges to the liberal international order (LIO) have led International Relations (IR) scholars, both those critical and supportive of the concept, to examine its origins and effects. While this work has shed new light on the evolution of international order, there has been a surprising absence: Latin America. I explore the theoretical consequences of this empirical gap for IR's understanding of the liberal international order. After assessing the literature's treatment of Latin American and the LIO, I offer a macro-historical sketch of the region's role in the order's critical junctures. The LIO has shaped Latin America, and Latin America has shaped the LIO—but not always in the ways supporters or critics might expect. The region's sovereignty and statehood evolved alongside the LIO—with international experiences very different from those of areas colonized during the LIO's expansion—and, in turn, Latin American engagement shaped the practices of Great Powers through international law and organization, cooperation and resistance. Despite its participation in the LIO's founding moments, Latin America was often accorded second-class treatment and benefits for the region have often been narrow. The experience of Latin American states over two centuries—independent but often internationally unequal—highlights the consequences of partial inclusion or marginalization from the LIO. Deeper study of Latin America's history with the LIO casts light on the ways in which non-Great Powers outside the order's core shaped, and were shaped by, the elements of the evolving order. In November 2010, the then British Foreign Secretary William Hague delivered Canning House's annual lecture on the topic of Britain's relations with Latin America. In launching the so-called 'Canning Agenda', Hague pledged to 'halt the decline in Britain's diplomatic presence in Latin America'. 'It is now time', Hague went on, 'for an advance to begin'. This article represents the first scholarly analysis of this policy. In particular, it assesses British efforts to enhance its economic ties with the countries of Latin America; the status granted to Latin America in Britain's strategic world-view in the post-Brexit era; the impact on Britain of other external powers in the region; and the political relationships between Britain and the countries of Latin America. The article draws on interviews with the key policy-makers involved in the Canning Agenda, including William Hague, Sir Hugo Swire, Jeremy Browne, and several other officials and diplomats from Britain and throughout Latin America. It concludes that while an identifiable effort to enhance Britain's economic and political standing in Latin America is discernible since 2010, major impediments to achieving this goal persist across the range of areas explored. Latin American truth commissions have recently expanded their purview to include cases of violence against gender and sexual minorities as human rights violations worthy of investigation. This article proposes that grappling with this emerging LGBTI (lesbian, gay, bisexual, transgender and intersex) rights challenge requires a queer, intersectional and decolonial analytical lens that underscores the relevance of global LGBTI politics, and critiques transitional justice foundational assumptions regarding temporality and binary logics. In practical terms, this analytical lens enacts a double move by unearthing the deeply tangled and life-extinguishing roots of impunity surrounding violence against gender and sexual minorities while advocating for the realization of LGBTI people's full citizenship. Background: Gastrointestinal disorders are among the most common conditions encountered on short-term, primary care medical service trips (MSTs) in Latin America and the Caribbean (LAC), but their optimal management remains unclear. There have been no previous attempts to describe the protocols that Western volunteer clinicians use in managing these patients. Methods: A systematic web search was used to identify organizations operating MSTs in LAC. Clinical protocols were downloaded from organizational websites, and organizations were contacted online to obtain those that were not publicly available. Protocols were analysed qualitatively, and content compared with existing international guidelines. Results: Two hundred and twenty-five organizations were identified and contacted to obtain their clinical protocols, and the content of each protocol was qualitatively analysed. Twenty protocols were obtained, of which 75% (15/20) discussed dyspepsia, 65% (13/20) discussed parasites and 60% (12/20) discussed diarrhoea. The protocols infrequently included literature citations. Conclusion: Gastrointestinal disorder protocols used by MSTs in LAC sometimes neglected important aspects of clinical management that are emphasized in international guidelines. Our argument revalidates and advances the idea of learning that is central to literatures on economic development, innovation and industrial policy. Global shifts where developing countries are advancing and innovating at the technological frontier challenge the conventional view of learning that focused on adapting existing technologies from advanced economies to build domestic firms' capabilities. Our research contributes to capture new dynamics of innovation, emerging technologies and government industrial policy, crucial in the learning perspective. Our field study of Brazilian government programmes in the bio-based industry illustrates how industrial policy is reinvented to catalyse firm innovation at the technological frontier. One of the purposes of cooperation in the refugee regime is to enhance protection by helping refugees find a durable solution outside States of first asylum. In this cooperation option, the challenge is to encourage States of resettlement to voluntarily accept refugees. This article aims to question the "Solidarity Resettlement Programme" developed by Latin American States as a strategy to foster cooperation. To do so, it adopts two approaches. First, it gives a general account of how cooperation works in the refugee regime and eliminates the theoretical and empirical isolationism presented in the literature that praises the programme. Secondly, it explores the Latin American cooperation framework by applying E.H. Carr's analysis of utopia and reality. The findings indicate that regional cooperation in Latin America to enhance the protection of refugees continues to have more utopian elements than realist ones. [ABSTRACT FROM AUTHOR]

Objective Determine the frequency and preventability of adverse events (AEs) from available information sources in selected ambulatory care (AC) sites in Latin America (LA). Design Multinational observational cohort was conducted to determine the period prevalence (retrospective focus) and the cumulative incidence (prospective focus) of AEs. Setting Outpatient clinics in Mexico, Peru, Brazil and Colombia. Participants A random selection of 2080 patients. Interventions The existence of AE was decided based on trigger information provided by the patient and crossing the data with each patient's medical history. Main Outcome Measures AE occurrences 6 months prior (prevalence) and 42 days after (incidence) the patient receiving AC were identified. AE type and preventability were also described. Results Two thousand eighty patients participated in the study. AEs prevalence was 5.2% (108/2080) [95% confidence interval (CI) 4.2–6.1%], and cumulative incidence was 2.4% (42/1757) (95% CI 1.7–3.1%). AEs considered preventable were 44% (55/108) of prevalence period, and 52.4% (22/42) of incidence period. Preventability was associated with patient socioeconomic status (OR 3.5, 95% CI 1.4–8.8), medication error (OR 0.1, 95% CI 0.0–0.4), diagnostic error (OR 0.1, 95% CI 0.0–0.8) and a minor impact on the patient (OR 0.2 95% CI 0.1–0.9). Conclusion The frequency of AE in ambulatory settings in LA is in the high-frequency range of research focused on the prevalence of AEs. Fifty percent was preventable. What explains the recent rise in non-contributory social insurance programmes in Latin America? Since the 1990s, Latin American countries have enacted significant social policy reforms that have supplemented contributory insurance policies with new or expanded non-contributory programmes, financed by general tax revenues. We present a political explanation for this phenomenon, arguing that the process of deindustrialization, and especially the increased labour insecurity that it entails, has changed the mix of social insurance policies favoured by different individuals in the income and employment distributions. A long-term increase in economic insecurity has increased the relative demand for tax-financed, rather than contribution-financed, social insurance. Elected officials, especially those in the recent ‘rise of the Left’ in Latin America, have proved eager to meet these demands. We find strong evidence for this argument using both cross-national data on non-contributory pension policy adoption and individual-level data on policy preferences. [ABSTRACT FROM PUBLISHER]

Snakebite envenoming constitutes an important public health problem in Latin America and some countries of the Caribbean. The advances and pending tasks in the study and control of this neglected tropical disease in this region are reviewed in the light of a roadmap proposed in 2006. Significant progress has been achieved in the study of snake venoms, particularly regarding venom proteomics, i.e.‘venomics’, and the analysis of the mechanism of action of toxins. Likewise, a deeper understanding has been gained in the preclinical efficacy of antivenoms produced in the region. In contrast, despite advances made in the study of clinical manifestations of envenomings and safety and efficacy of antivenoms at the clinical level, much remains to be done in this subject. Improvements have occurred in antivenom manufacturing technologies and availability, although there are still countries where there is insufficient supply of antivenoms, or where manufacture has to be improved. In spite of considerable efforts in some countries in prevention, accessibility to treatment, and training of health staff in the management of envenomings, important challenges remain for the region as a whole, with the long term goal of reducing the impact of this disease in terms of personal and social suffering. Background Acute kidney injury (AKI) has increasingly been recognized as an important public health issue due to its rising frequency, its associations with early and late adverse outcomes and its economic burden. Methods Given the importance of determining the available resources to address this serious issue, the AKI Committee of SLANH conducted a survey to obtain information about infrastructure, human resources and equipment devoted to the treatment of AKI in Latin America Results A total of 246 units from 14 countries participated in the survey, the majority of them pertaining to nephrology divisions in teaching hospitals. Intermittent hemodialysis was universally performed by all of the units, and less frequently, slow extended dialysis (40%) and continuous renal replacement therapy (23%) were performed. Seventy-nine units (30%) perform peritoneal dialysis, but only 51 (19%) of them reported having treated at least 1 patient with this technique in the last 3 months pre-survey. The vast majority of the units reported adequate water treatment and use of modern filter membranes. Most of the patients received renal replacement therapy (RRT) in the intensive care unit. Specific causes of AKI were reported in different frequencies, with a heterogeneous pattern among the countries. Septic abortion, hemolytic-uremic syndrome, community-acquired diarrhea and leptospirosis were the etiologies most frequently associated with AKI. Through the 1980s the Australian Government ran an in-country programme out of Chile and El Salvador, rescuing and resettling dissidents, trade unionists, and former political prisoners. This programme operated discreetly, and relied on careful negotiation with local authorities. It complicated Australia’s diplomatic relationship with the Reagan administration and caused tensions between the departments of Immigration and Foreign Affairs. As a result, the in-country programme left little documentary evidence and has remained unexamined by immigration historians and refugee scholars. This article provides an original study of the programme, drawing on archival records from the Fraser Government and the first Hawke Government, from 1979 to 1984, as well as oral history interviews with former politicians and Australian and Canadian Immigration officials. The in-country programme was a unique initiative, driven by humanitarian concerns and a desire to diversify the refugee intake. Mental health services reforms in Latin America and the Caribbean in the last 20 years have led to a significant improvement of mental health services. They also contributed to the development of new evidence that may help the implementation of future reforms. These advances, however, were clearly insufficient to respond to the huge challenges countries of Latin American and the Caribbean face to improve mental health services. Insufficient funding, one of the most important barriers to mental health services development found in most countries, was related to the absence of a strong consensus among all stakeholders and the weakness of user and family associations. Other barriers were the lack of technical capacity of the coordination unit responsible for development of services in the ministries of health, resistance from professionals towards changing to new models of care and lack of human resources.Transition to democracy in some countries and natural disasters proved to be windows of opportunity for mental health services reform. Facilitating factors included alliance with the human rights defence movement, development of research capacity in Latin American and the Caribbean countries, and international cooperation. This review presents an overview of breast cancer care, burden, and outcomes in Latin America, as well as the challenges and opportunities for improvement. Information was gleaned through a review of the literature, public databases, and conference presentations, in addition to a survey of clinical experts and patient organizations from the region. Breast cancer annual incidence (114,900 cases) and mortality (37,000 deaths) are the highest of all women's cancers in Latin America, and they are increasing. Twice as many breast cancer deaths are expected by 2030. In Peru, Mexico, Colombia, and Brazil, diagnosis and death at younger ages deprives society of numerous productive years, as does high disease occurrence in Argentina and Uruguay. Approximately 30%-40% of diagnoses are metastatic disease. High mortality-to-incidence ratios (MIRs) in Latin America indicate poor survival, partly because of the late stage at diagnosis and poorer access to treatment. Between 2002 and 2008, MIRs decreased in all countries, albeit unevenly. Costa Rica's change in MIR outpaced incidence growth, indicating impressive progress in breast cancer survival. The situation is similar, although to a lesser extent, in Colombia and Ecuador. The marginal drops of MIRs in Brazil and Mexico mainly reflect incidence growth rather than progress in outcomes. Panama's MIR is still high. Epidemiological data are scattered and of varying quality in Latin America. However, one could ascertain that the burden of breast cancer in the region is considerable and growing due to demographic changes, particularly the aging population, and socioeconomic development. The influence of cultural patterns and economic conditions on health perceptions in Latin America is studied using the results of the 2007 Gallup World Poll. The differences in health satisfaction between countries around the world have a robust association with variables that may reflect cultural differences rather than with aggregate economic variables or traditional health indicators. Simple health self-rating indicators reveal huge cultural differences in health perceptions in Latin America. However, within each country, differences correlate strongly with individuals’ economic and health conditions. What are the underpinnings of workplace transformation in the developing world? I address the question by examining the diffusion of modular production practices across 30 export processing zones in the Dominican Republic (DR) in the turbulent 1990s. I find that public officials responded to a series of external shocks by facilitating the transfer of modular methods to domestically owned factories in particular and conclude that organizational innovations are more likely to be co-produced by developing country firms and their governments in turbulent times than produced in isolation (or good times) by either. Finally, I corroborate my findings by exploring the negative case of Mexico, where the absence of government support has stymied the diffusion of modular manufacturing methods. Background Breastfeeding has large benefits for mothers and infants. The short-term benefits for child survival and reduced morbidity differ by population subgroup because of differences in underlying risk factors. Although breastfeeding is more common among poor than well-off women, how breastfeeding patterns change between these subgroups is important from a policy perspective as the poor will benefit more from increased duration of breastfeeding.Methods We use nationally representative data from eight countries in Latin America and the Caribbean to document changes in breastfeeding duration between 1986 and 2005, and separate the overall change into the portion attributable to changing population characteristics and the portion resulting from changing breastfeeding behaviour within population subgroups.Results Breastfeeding duration increased in six out of the eight countries and the changes observed are largely explained by changing behaviour within population subgroups rather than changing population characteristics. Changes in breastfeeding duration did not tend to be equitably distributed, but in four countries (Bolivia, Brazil, Colombia and Peru) the population subgroups whose children are most at risk for mortality and increased morbidity from not being breastfed were least likely to show improvements in breastfeeding duration. Between 1986 and 2004 in Peru, breastfeeding duration declined by 0.6 months among rural women while increasing by 9.7 months among urban women; it increased by 6.3 months among women with prenatal care but only by 3.7 months among women with no prenatal care. Changes in breastfeeding in Guatemala and Haiti tended to favour the well-off compared with the poor, though not consistently. In Nicaragua changes in breastfeeding duration tended to favour the less well-off.Discussion While promoting breastfeeding is a must for all women, to maximize its benefits for child survival and health, additional efforts are needed to reach poorly educated and rural women with little access to health care. In a comparative institutional or ‘variety of capitalism’ perspective, the distinctive traits of labour markets in Latin America differ in most respects from labour markets in developed countries. Moreover, there are strong economic complementarities among five core features of labour markets in Latin America: low skill levels, high labour regulation, short job tenure, a large informal sector, and small, politicized unions that lack plant level representation. While numerous and strong, economic complementarities among these five components do not tell the whole story, and we analyse additional political complementarities. This integrated perspective on the economic and political interactions helps explain continuities in labour markets in Latin America and their disappointing response in recent decades to market reform and globalization. The current world economic crisis has hit Latin America very hard. Although financial conditions have deteriorated, particularly since September 2008, the financial shock has been less severe than during the two previous crises. Thanks to improvements in external balance sheets, there has been room for counter-cyclical credit and monetary policies. The decision to absorb large capital inflows during the boom as foreign exchange reserves is one of the major sources of the increased room to manoeuvre. However, these strengths have been insufficient in the face of a strong trade shock. In 2009, the WHO Special Programme for Research and Training in Tropical Diseases (TDR) and the International Development Research Centre (IDRC) launched a call for innovative community-based ecosystem management research projects for dengue and Chagas disease prevention in low and middle income countries in Latin America and the Caribbean. Eight research institutions were selected. The outputs of these projects led to a better understanding of the interaction between ecological, biological, social and economic (eco-bio-social) determinants of dengue and Chagas disease in Latin America and the Caribbean.
