The world's population is aging with the fastest growth in the older population projected to take place in Africa. In this article, we present the challenges of the growing older population in Ethiopia, outline some key changes that are needed to address those challenges, and consider the opportunities that can come about when older adults' basic needs are met and they are supported in contributing to their communities. Older adults in Ethiopia are faced with multidimensional challenges that call for collaborative efforts from different stakeholders at local, regional, and national levels. However, some measures should be given the utmost priority: combating negative attitudes toward older people, strengthening geriatric/gerontology and social work education and research, and developing aging-specific policies and services. Although the challenges faced by older adults in Ethiopia seem like a problem of aging compounded by poverty, the root source of the problem is a poverty of policy fueled by ageism. If income support and appropriate health care was provided to older adults throughout Ethiopia, health and well-being in late life would improve, food and housing insecurity among older adults would lessen, and all Ethiopians could anticipate a dignified late life. Background and Objectives The United Nations has projected a 218% increase in older people in Sub-Saharan Africa (SSA) between 2019 and 2050, underscoring the need to explore changes that would occur over this time. Longitudinal studies are ideal for studying and proffering solutions to these changes. This review aims to understand the breadth and use of longitudinal studies on aging in the SSA regions, proffering recommendations in preparation for the projected aging population. Research Design and Methods This paper is the third of a four-part series paper of a previous systematic mapping review of aging studies in SSA. We updated the search (between 2021 and 2023) and screened the titles/abstracts and full-text articles by a pair of independent reviewers. Data were extracted using a standardized data-charting form, identifying longitudinal studies in SSA. Results We identified 193 studies leveraging 24 longitudinal study data sets conducted at 28 unique sites. The World Health Organization's Study on Global AGEing and Adult Health (WHO-SAGE) (n  = 59, 30.5%) and Health and Aging in Africa: A Longitudinal Study of an INDEPTH Community in South Africa (HAALSI) (n  = 51, 26.4%) were the most used longitudinal data sets. Four studies used more than one longitudinal study data set. Eighteen of the longitudinal study data sets were used only in 1–4 studies. Most (n  = 150, 77.7%) of the studies used a cross-sectional analytical approach. Discussion and Implications Longitudinal studies on aging are sparingly being utilized in SSA. Most analyses conducted across the longitudinal data set were cross-sectional, which hindered the understanding of aging changes that occurred over time that could better inform aging policy and interventions. We call for funding bodies, such as WHO-SAGE, to develop funding competitions that focus on conducting longitudinal analyses, such as structural equation modeling, highlighting changes occurring among the aging population in SSA. Background and Objectives In Nigeria and many Sub-Saharan African countries where the majority are poor and cannot afford formal postpartum care, nursing mothers rely primarily on their mothers or older female relatives for postpartum care. Despite their invaluable contributions, such grandmothers often operate in a reality of inadequate social and institutional support. Yet, little is known about how women perceive this form of traditional care in Sub-Saharan Africa. This study explored the views of older grandmothers in Southeast Nigeria about postpartum caregiving, called "Ọmụgwọ." Research Design and Methods Using a qualitative descriptive inquiry, the author employed criterion-purposive sampling and snowballing to recruit 17 older grandmothers who participated in the interviews, and data were analyzed in themes. Results Three themes and eight subthemes emerged. The themes cover perceptions supporting the continuity of Ọmụgwọ practice irrespective, the influence of culture on Ọmụgwọ practice, and the indirect rewards of the practice. Discussion and Implications The findings of this study could potentially influence postpartum caregiving policies for female older adults in Sub-Saharan Africa and further advance the quality of informal care during postpartum periods in Africa. Background and Objectives Alcohol causes more than 3 million deaths a year globally and contributes to over 5% of global disease and injury. Heavy drinking and alcohol use disorders among older adults have increased in the last 10–15 years. For individuals living in low-income countries, where wages are low and unemployment is high, old age pensions may provide a significant increase in household income. In turn, the receipt of supplementary income may increase spending on alcohol. Earlier life factors and socioeconomic status may affect alcohol consumption, making it difficult to directly assess the impact of income on alcohol consumption. This study reduces the potential for endogeneity with other life factors by exploiting an exogenous increase in income from old age pensions to isolate the impact of extra income on alcohol consumption for older adults. Research Design and Methods We used a regression discontinuity design to assess changes in drinking patterns among rural, low-income adults who were 3 years below and 3 years above South Africa's Old Age Pension Grant eligibility threshold (age 60). We assessed this relationship separately by gender and for employed and unemployed individuals. Results We observed a significantly increased alcohol use associated with the Old Age Pension Grant eligibility for employed men (β = 4.57, 95% confidence interval: 1.72–12.14). We did not observe this same trend for unemployed men or for women. Discussion and Implications The analysis in this study indicates that increased income from reaching the pension eligibility age may contribute to an increase in alcohol consumption for employed men. Interventions, such as informational campaigns on the risks of alcohol consumption for older adults or age-appropriate health interventions to help individuals reduce alcohol consumption, targeted around the time of pension eligibility age for employed men may help to reduce alcohol-related harms in low-income, rural sub-Saharan African settings. In Kenya, the number of adults aged ≥60 is expected to nearly quadruple by 2050, making it one of the most rapidly aging countries in Sub-Saharan Africa (SSA). Accordingly, we designed the Longitudinal Study of Health and Ageing in Kenya (LOSHAK) to generate novel data to address the health and economic consequences of this demographic transition. Specifically, LOSHAK will investigate the social, economic, environmental, biological, and policy processes that shape late-life health and economic well-being in Kenya. Modeled on the U.S. Health and Retirement Study (HRS), LOSHAK joins a network of harmonized studies on aging in >45 countries worldwide; however, LOSHAK will be only the 2nd such study in SSA. The current feasibility and pilot phase of LOSHAK will validate measures and data collection procedures in a purposive sample of Kenyan adults aged ≥45 years. We have linguistically and culturally translated instruments while aiming to maintain harmonization with both existing HRS network studies and the ongoing Kenya Life Panel Survey. The current phase of LOSHAK is nested within the Kaloleni/Rabai Community Health and Demographic Surveillance System on the coast of Kenya. LOSHAK will advance population aging research in low- and middle-income countries through the study of (a) biomarkers and physiological measures ; (b) the impacts of air pollution and climate vulnerability ; (c) Alzheimer's disease and related dementias, mental health, disability, caregiving, and psychosocial wellbeing ; and (d) economic security, including the impact of social welfare. LOSHAK will inform future public health and economic policy to address challenges related to rapid aging in Kenya and throughout SSA. Background and Objectives Global organizations are advocating that older persons' voices should guide communities in age-friendly design. An important aspect of age friendliness to enable daily function and health is ensuring that physical activity can occur, regardless of age, within local neighborhoods. Research Design and Methods This study used a specific citizen science approach, Our Voice , to engage a sample (N = 13) of older adults (60 or older) in Festac Town, Nigeria. The citizen scientists' roles were to assess and identify how different aspects of the neighborhood environment act as supports or barriers to their physical activity participation. They were individually enabled using a tablet-based mobile application called the Stanford Healthy Neighborhood Discovery Tool to record a total of 156 geocoded photos and 151 commentaries of neighborhood environmental features that facilitate or hinder physical activity in and around their neighborhoods. In a guided process, the following occurred: collaborative discussions of findings with other citizen scientists to determine common targets, setting of priority targets for change, and brainstorming strategies and solutions. Results Facilitators of physical activity included: pedestrian and traffic facilities (e.g. traffic lights, walkways); green areas and parks; multigenerational community features (e.g. programs/facilities); opportunities for social connection (e.g. neighborhood associations, churches); safety of destinations and services; and public toilets. Barriers to physical activity included: hazardous walkways/traffic; noise pollution; refuse, selling of public parks; crime (e.g. kidnapping, criminal hideouts); no safe drinking water; and ageism. The priorities for changes were social connectivity; improved pedestrian and traffic facilities; and green and beautiful environments. Discussion and Implications In this study, both physical and social aspects of the environment were deemed important for older Nigerians to enable physical activity in their local community. Background and Objectives It has been evidenced that retirement transitions are accompanied by preretirement anxiety about transitioning from a work-oriented lifestyle to retirement. Most employees do not proactively address these concerns during this transitional period. Thus, identifying the factors inherent in preretirement anxiety is imperative for a positive retirement transition. This study explored the role of financial self-efficacy and preretirement goal clarity on preretirement anxiety and the mediating role of job embeddedness in such relationships among prospective retiree nurses in Sub-Saharan Africa. Research Design and Methods This cross-sectional study used self-report measures of the Pre-retirement Anxiety Scale, Financial Self-Efficacy Scale, Retirement Goal Clarity Scale, and Job Embeddedness Scale for data collection. A total of 236 nurses participated in the study. Descriptive analyses were done to determine the bivariate correlations among the study variables, while regression-based path analysis was carried out to test the hypotheses. Results Results revealed that goal clarity and financial self-efficacy showed a strong negative association with preretirement anxiety. Also, higher job embeddedness was negatively associated with preretirement anxiety. In addition, there was a significant indirect relationship between financial self-efficacy and preretirement anxiety, as well as preretirement goal clarity and preretirement anxiety through job embeddedness. Hence, the influence of financial self-efficacy and preretirement goal clarity on preretirement anxiety was mediated by job embeddedness. Discussion and Implications The results emphasized that financial self-efficacy and preretirement planning are imperative for a positive perception toward retirement transition. In addition, job embeddedness should be encouraged among employees because it facilitates connectedness and interrelatedness in social fusion, ideas, and projections toward retirement transition. This connotes that the development of attachment to place and the formation of strong social ties are sacrosanct for retirement transition. Background and Objectives The population of older individuals in Africa is increasing, and at a rapid rate. Although the numbers of these individuals increase, many African countries, including Nigeria, are devoid of state-sponsored welfare systems that address their well-being. This situation has placed the bulk of support on the family members including spouses. Studies have shown that spouses are considered to be veritable sources of support in later life. However, spousal death tends to reduce the perceived support including emotional support, which usually occasions lower well-being, life dissatisfaction, and higher mortality rates. This study ascertains the impact of spirituality/religious participation on the emotional well-being of widowed older adults. Research Design and Methods The study was conducted in Enugu State, southeast Nigeria. The study implemented a qualitative approach to collect data from 71 widowed older adults, aged 60 and older. The thematic analytical method was used to analyze the generated data. Results Participating in spiritual/religious activities like prayers, church programs/activities reading, studying, and meditating on the word of God have enabled the widowed older adults to buffer some of the emotional challenges. Discussion and Implications Older adults should be encouraged to participate in spiritual and religious activities. The placebo effect is now generally defined widely as an individual's response to the psychosocial context of a clinical treatment, as distinct from the treatment's characteristic physiological effects. Some researchers, however, argue that such a wide definition leads to confusion and misleading implications. In response, they propose a narrow definition restricted to the therapeutic effects of deliberate placebo treatments. Within the framework of modern medicine, such a scope currently leaves one viable placebo treatment paradigm: the non-deceptive and non-concealed administration of "placebo pills" or open-label placebo (OLP) treatment. In this paper, I consider how the placebo effect occurs in OLP. I argue that a traditional, belief-based account of OLP is paradoxical. It is widely agreed that living kidney donation is permitted but living kidney sales are not. Call this the Received View. One way to support the Received View is to appeal to a particular understanding of the conditions under which living kidney transplantation is permissible. It is often claimed that donors must act altruistically, without the expectation of payment and for the sake of another. Call this the Altruism Requirement. On the conventional interpretation, the Altruism Requirement is a moral fact. It states a legitimate constraint on permissible transplantation and is accepted on the basis of cogent argument. The present paper offers an alternative interpretation. I suggest the Altruism Requirement is a moral fiction—a kind of motivated falsehood. It is false that transplantation requires altruism. But the Requirement serves a purpose. Accepting it allows kidney donation but not kidney sale. It, in short, rationalizes the Received View. [ABSTRACT FROM AUTHOR]

The following article presents preliminary reflections on a processual theory of health and disease. It does this by steering the discussion more toward an ontology of organisms rather than conceptual analysis of the semantic content of the terms "health" and "disease." In the first section, four meta-theoretical assumptions of the traditional debate are identified and alternative approaches to the problems are presented. Afterwards, the view that health and disease are constituted by a dynamic relation between demands imposed on an organism and individual presuppositions for adequate response is developed. Metaphysical realism about mental disorder is the thesis that mental disorder exists mind-independently. There are two ways to challenge metaphysical realism about mental disorder. The first is by denying that mental disorder exists. The second is by denying that mental disorder exists mind-independently. Or, differently put, by arguing that mental disorder is mind-dependent. The aim of this paper is three-fold: (a) to examine three ways in which mental disorder can be said to be mind-dependent (namely, by being causally dependent on the human mind, by being weakly dependent on human attitudes, and by being strongly dependent on human attitudes), (b) to clarify their differences, and (c) to discuss their implications regarding metaphysical realism about mental disorder. I argue that mental disorder being mind-dependent in the first two senses is compatible with metaphysical realism about mental disorder, whereas mental disorder being mind-dependent in the third sense is not. The controversy over the equivalence of continuous sedation until death (CSD) and physician-assisted suicide/euthanasia (PAS/E) provides an opportunity to focus on a significant extended use of CSD. This extension, suggested by the equivalence of PAS/E and CSD, is designed to promote additional patient autonomy at the end-of-life. Samuel LiPuma, in his article, "Continuous Sedation Until Death as Physician-Assisted Suicide/Euthanasia: A Conceptual Analysis" claims equivalence between CSD and death; his paper is seminal in the equivalency debate. Critics contend that sedation follows proportionality protocols for which LiPuma's thesis does not adequately account. Furthermore, sedation may not eliminate consciousness, and as such LiPuma's contention that CSD is equivalent to neocortical death is suspect. We not only defend the equivalence thesis, but also expand it to include additional moral considerations. First, we explain the equivalence thesis. This is followed by a defense of the thesis against five criticisms. The third section critiques the current use of CSD. Finally, we offer two proposals that, if adopted, would broaden the use of PAS/E and CSD and thereby expand options at the end-of-life. [ABSTRACT FROM AUTHOR]

Classic models of urban inequality acknowledge the importance of politics for resource distribution and service provision. Yet, contemporary studies of spatial inequality rarely measure politics directly. In this paper, we introduce political dynasties as a way of integrating political economy approaches with ecological theory to better understand the political construction of urban spatial inequality. To do so, we examine the case of political dynasties within the Chicago city council. We show that, from 2011 to 2018, blocks in dynastic wards saw fewer homicides, assaults, robberies, and thefts relative to those in non-dynastic wards. We then leverage the 2015 ward redistricting to provide evidence that dynastic effects play some role in producing these outcomes: blocks annexed into dynastic wards experienced a decline in assaults and robberies and an increase in pothole coverings. While dynastic politicians improve outcomes for blocks they annex, they also withdraw power from those they displace; and displaced blocks had relatively higher levels of crime than annexed blocks in 2015. This paper explores how social capital and property regulations shape cultural producers' ability to own copyrights for the products they create. Because individual producers lack the resources required to develop and distribute their creations, they partner with large firms who demand the copyrights for products they invest in. I argue two types of social capital—status and partner substitutability—enable producers to own their creations by increasing their bargaining power over the firms they partner with. Moreover, I propose that different types of property regulations condition whether status or partner substitutability have a stronger role in producers' ownership outcomes. Analyzing all shows made for American television from 1956 to 1996, I show that increases in showrunners' status and number of broadcasters they collaborated with increased the probability they owned shows they produced. However, the effects of these social capitals were contingent on the property regulations showrunners operated under. These findings advance our understanding of the allocation of copyrights, the power dynamics between creators and firms, and the effects of social capital in cultural industries. Whether immigrants and their descendants are catching up socioeconomically with the rest of society is a fundamental question in the study of immigrant assimilation. In this paper, we examine the progress that Hispanic immigrant men make catching up with the earnings of later-generation Whites across generations. We rely on data from multiple years of the Current Population Survey linked with individuals' tax earnings. This unique dataset allows us to overcome some important limitations of previous studies that employ a synthetic generation approach in which individuals born approximately one generation earlier are used as proxies for actual parents. Our matching strategy also enables us to identify the exact third generation and evaluate the contribution of ethnic attrition to estimates of intergenerational mobility. Second-generation Hispanic men are found to experience lower mobility than later-generation Whites for most values of parental earnings. However, their lower mobility can be explained by their immigrant parents' lower education levels. In contrast, third-generation Hispanic men experience lower mobility even after accounting for parental education and ethnic attrition. This finding is consistent with a stalling or reversal in the socioeconomic progress of Hispanics beyond the second generation. [ABSTRACT FROM AUTHOR]

Although there has been much writing and speculation on the deliberate use of smallpox as a tool of genocide, this article documents the use of the bluff threat of spreading smallpox as a tool of power and manipulation in the early days of European trade and settlement in the Pacific Northwest. By documenting ten cases when a bluff threat was used, the article argues that it was a common strategy of Europeans when they felt threatened or thwarted. Because it was compatible with existing Indigenous beliefs about the spread of disease, it was highly credible and was occasionally used by Indigenous people to manipulate others. While Europeans in this era did not actually have the power to control smallpox, the fact that outbreaks of the disease often occurred following a threat to spread it gave credence to the threat and to today's widespread belief that some or all of the epidemics were deliberate genocide. Recognizing bluff threat bioterrorism as a tool in the newcomer's arsenal is essential to understanding how the heavily outnumbered and out-gunned newcomers were so often able to manipulate Indigenous people and then establish the settlements that eventually evolved into full scale colonial occupations of Indigenous territory. In March 1866, the Sicangu (Brulé) Lakota chief Spotted Tail brought the body of his seventeen- or eighteen-year-old daughter, Mni-Akuwin, to Fort Laramie. She had died somewhere in the Powder River country. Before she died, she had asked her father to bury her among the Whites at the fort. Her subsequent scaffold burial in the post cemetery was an unusual mix of military display, Christian rites, and Lakota traditions; and the events surrounding it became a dramatic episode in the story of Native-White relations and western settlement. Almost 140 years later she was memorialized at the fort, a National Park Service site, as a symbol of racial reconciliation. The story of Mni-Akuwin has been told and retold many times through the years, often as a romantic tragedy comparable to the story of early Jamestown and the much-celebrated Pocahontas. The two Native women had a surprising amount in common, including what we know and don't know about them, what has been ascribed to their motives, and how they have been championed as defenders of European colonists and non-Native settlers. Indian princess mythology, with its insistence on innocence, purity, romantic purpose, and tragedy, has been vital to the persistence of this view—a perspective that has often put reconciliation in the service of White supremacy, dispossession, and rejection of Native culture. We introduce a comprehensive new dataset of conflicts, in chronological order, between the Indigenous Peoples of North America and Europeans seeking to colonize what is now the Continental United States. These data, covering 1,375 conflicts, were originally compiled as a book in 2007 by Michael L. Nunnally, a self-taught historian with a talent for careful data collection and analysis. A research team within the Rights Lab (University of Nottingham, U.K.), drew upon Nunnally's American Indian Wars to code each recorded conflict (battle, skirmish, raid, massacre, etc.) along with other relevant information. The conflicts were then further researched by the Rights Lab team to add any missing information, resolve ambiguities or questions, and to find common conflict patterns through these centuries. The database holds thirty possible variables for each conflict. An example of one such variable is "genocidal massacre," following the work of historian Benjamin Madley. Other variables include specifying and/or clarifying the identity of all participants; introducing, as often as possible, the exact or approximate GPS coordinates for each conflict; and noting who were the initiators of each conflict. The full Codebook is an Appendix to this article. To our knowledge, the database we are presenting is unique. We offer this dataset to scholars in the hope that it will allow for a more nuanced and deeper consideration of the history of the European invasion of North America over a four-hundred-year period. Globally, there are over 1 billion people infected with soil-transmitted helminths (STHs), mostly living in marginalized settings with inadequate sanitation in sub-Saharan Africa and Southeast Asia. The World Health Organization recommends an integrated approach to STH morbidity control through improved access to sanitation and hygiene education and the delivery of preventive chemotherapy (PC) to school-age children delivered through schools. Progress of STH control programs is currently estimated using a baseline (pre-PC) school-based prevalence survey and then monitored using periodical school-based prevalence surveys, known as Impact Assessment Surveys (IAS). We investigated whether integrating geostatistical methods with a Markov model or a mechanistic transmission model for projecting prevalence forward in time from baseline can improve IAS design strategies. To do this, we applied these 2 methods to prevalence data collected in Kenya, before evaluating and comparing their performance in accurately informing optimal survey design for a range of IAS sampling designs. We found that, although both approaches performed well, the mechanistic method more accurately projected prevalence over time and provided more accurate information for guiding survey design. Both methods performed less well in areas with persistent STH hotspots where prevalence did not decrease despite multiple rounds of PC. Our findings show that these methods can be useful tools for more efficient and accurate targeting of PC. The severe acute respiratory syndrome coronavirus 2 virus has, up to the time of this article, resulted in >770 million cases of COVID-19 illness worldwide, and approximately 7 million deaths, including >1.1 million in the United States. Although defined as a respiratory virus, early in the pandemic, it became apparent that considerable numbers of people recovering from COVID-19 illness experienced persistence or new onset of multi-system health problems, including neurologic and cognitive and behavioral health concerns. Persistent multi-system health problems are defined as Post-COVID-19 Condition (PCC), Post-Acute Sequelae of COVID-19, or Long COVID. A significant number of those with PCC report cognitive problems. This paper reviews the current state of scientific knowledge on persisting cognitive symptoms in adults following COVID-19 illness. A brief history is provided of the emergence of concerns about persisting cognitive problems following COVID-19 illness and the definition of PCC. Methodologic factors that complicate clear understanding of PCC are reviewed. The review then examines research on patterns of cognitive impairment that have been found, factors that may contribute to increased risk, behavioral health variables, and interventions being used to ameliorate persisting symptoms. Compared with other health disciplines, there is a stagnation in technological innovation in the field of clinical neuropsychology. Traditional paper-and-pencil tests have a number of shortcomings, such as low-frequency data collection and limitations in ecological validity. While computerized cognitive assessment may help overcome some of these issues, current computerized paradigms do not address the majority of these limitations. In this paper, we review recent literature on the applications of novel digital health approaches, including ecological momentary assessment, smartphone-based assessment and sensors, wearable devices, passive driving sensors, smart homes, voice biomarkers, and electronic health record mining, in neurological populations. We describe how each digital tool may be applied to neurologic care and overcome limitations of traditional neuropsychological assessment. This paper identifies an absence in currently constituted criminological discourse on the Global North and Global South. This absence is the Global East. The Global East is not a defined region but a relation of betweenness, geographically and geo-politically, within and between the South and North, representing peoples from countries and societies which fit imperfectly into a North/South binary. This paper draws on interview data and published court judgment reports to reveal first-hand accounts of illegal transactions involving protected wildlife and how criminals collaborate with one another. This research finds that wildlife supply is controlled by a small number of key suppliers. Further, trust is embedded in the criminals' relationship based on kinship and friendship ties, while individuals' ethnicity and reputation also facilitate collaboration. These arrangements shape the criminal networks that exploit protected wildlife. This paper suggests further research into online trading of protected wildlife products, the presence of women in the illegal wildlife trade and the impact of COVID-19 on the criminal networks supplying wildlife. This paper provides insights into the predominant styles of political reasoning in England and Wales that inform penal policy reform. It does so in relation to a particular development that constitutes a dramatic, perhaps even unique, wholesale reversal of a previously introduced market-based criminal justice delivery model. This is the 'unification' of probation services in England and Wales, which unwound the consequential privatization reforms introduced less than a decade earlier. This paper draws on in-depth interviews with senior policy makers to present a narrative reconstruction of the unification of probation services in England and Wales. This paper examines new meanings that police–citizen interactions take on when officers make sense of them through the lens of body-worn cameras (BWCs). Drawing on 30 interviews with frontline police officers in a large Canadian city, we analyse the embodied character of BWCs to show how officers reframe their role and the subtleties of their approach in dealing with the public as more robotic. First, the participants believe BWCs curb their ability to build rapport with citizens, and therefore dehumanize interactions. Second, they report a need to operate more mechanically to follow protocol for case-building and use-of-force. The rise of exclusionary populism is widely regarded as one of the most significant phenomena in today's political world. Despite this, the relationship between populism and security remains under-explored in the literature, including the affective power of populist security narratives. Against this background, this paper conducts a comparative analysis of radical right populist discourse in response to two recent shocking crimes in France and Ireland. The different expression given to security concerns in the two countries, such as a much less antagonistic 'flaunting of the low' in France, is suggestive of a more contingent and institutionally mediated relationship between security and populism than the existing literature would suggest. EB is a rare genetic condition that affects around 500,000 people worldwide and causes fragile skin and potential internal organ involvement. This paper proposes a model of the distribution channel to study the decisions of manufacturers, retailers and consumers. The model is used to study how retail distribution affects manufacturing and consumers. As the cost of distribution falls, the retail sector increases assortment size and allows a larger mass of manufacturers to enter, allowing consumers to buy more variety. Entry primitives in manufacturing and retailing determine vertical pricing power. The equilibrium of the model predicts that the national retail sector is a fraction of the total economy that depends on the demand for variety. This paper relates curiosity to economic development through its impact on human capital formation and technological advancement in pre-modern times. More specifically, we propose that exposure to inexplicable phenomena prompts curiosity and thinking in an attempt to comprehend these mysteries, thus raising human capital and technology, and, ultimately, fostering growth. We focus on solar eclipses as one particular trigger of curiosity and empirically establish a robust relationship between their number and several proxies of economic prosperity. We also offer evidence compatible with the human capital and technological increases we postulate, finding a more intricate thinking process and more developed technology among societies more exposed to solar eclipses. Among other factors, we study the development of written language, the playing of strategy games and the accuracy of folkloric explanations for eclipses, as well as the number of tasks undertaken in a society, their relative complexity and broad technological indicators. Lastly, we document rising curiosity both at the social and individual levels: societies incorporate more terms related to curiosity and eclipses in their folklore, and people who observed a total solar eclipse during their childhood were more likely to have entered a scientific occupation. This paper studies important determinants of adult self-control using population-representative data and exploiting Germany's division as quasi-experimental variation. We find that former East Germans have substantially more self-control than West Germans and provide evidence for government surveillance as a possible underlying mechanism. We thereby demonstrate that institutional factors can shape people's self-control. Moreover, we find that self-control increases linearly with age. This paper assesses the effects of adverse economic shocks on children's mental health. We rely on the Brazilian High-Risk Cohort Study for mental conditions, which provides an unprecedented array of data on psychopathology, life events, family medical history as well as parental behaviour and polygenic scores for mental disorders over a ten-year period. Our empirical strategy exploits parental job loss events over time in a difference-in-differences framework. We document that parental job loss significantly worsens children's mental health, resulting in increased clinical diagnoses of mental disorders. These results are robust to several specifications and pre-trends. Heterogeneous results and mechanism analysis indicate that psychological distress in the household brought about by job loss events may be a key mechanism affecting children's mental health. The M w 6.8 Murghob earthquake is the third earthquake in an M w 6.4+ sequence occurring in the Pamir initiated by the 2015 Sarez M w 7.2 earthquake. It is of great significance to investigate their interactions and to assess future seismic hazards in the region. In this paper, we use Sentinel-1 radar interferometric data to retrieve coseismic deformation, invert for the slip distributions of the four events, and then investigate their interactions. The cumulative Coulomb failure stress changes (ΔCFS) suggest that the 2023 Murghob earthquake was promoted by the three prior earthquakes in the sequence. Pre-stress from historical earthquakes is a key factor in explaining the triggering mechanism of the two 2016 M w 6.4+ earthquakes. Stress loading and unloading effects on major faults in the region indicate that future attention should be paid in (1) the segment of the Sarez-Karakul fault north of the Kokuibel Valley, (2) the segment of the Sarez-Murghab thrust fault west of the Sarez-Karakul fault and (3) the east segments of the Pamir thrust fault system, all with a large positive ΔCFS. More accurate inversion of source fault geometry and slip parameters under the constraint of the Bayesian algorithm has become a research hotspot in the field of geodetic inversion in recent years. In nonlinear inversion, the determination of the weight ratio of the joint inversion of multisource data is more complicated. In this context, this paper proposes a simple and easily generalized weighting method for inversion of source fault parameters by joint geodetic multisource data under the Bayesian framework. This method determines the relative weight ratio of multisource data by root mean square error (RMSE) value and can be extended to other nonlinear search algorithms. To verify the validity of the method in this paper, this paper first sets up four sets of simulated seismic experiment schemes. The inversion results show that the joint inversion weighting method proposed in this paper has a significant decrease in the large residual value compared with the equal weight joint inversion and the single data source joint inversion method. The east–west deformation RMSE is 0.1458 mm, the north–south deformation RMSE is 0.2119 mm and the vertical deformation RMSE is 0.2756 mm. The RMSEs of the three directions are lower than those of other schemes, indicating that the proposed method is suitable for the joint inversion of source parameters under Bayesian algorithm. To further verify the applicability of the proposed method in complex earthquakes, the source parameters of the Maduo earthquake were inverted using the method of this paper. The focal depth of the inversion results in this paper is closer to the focal depth released by the GCMT agency. In terms of strike angle and dip angle, the joint inversion in this paper is also more inclined to the GCMT results. The effects of pore fluids on the elastic properties of sedimentary rocks can be broadly categorized into two mechanisms: variations in pore compressibility due to the physical properties of the fluids and alterations in the stiffness of the rock frame resulting from rock–fluid interactions. Particularly, as rock–fluid interactions alter the stiffness of contacts between mineral grains, changes in the fluid properties around grain contacts can induce volumetric deformation of the rock as well as in variations of the associated elastic coefficients. Even though many previous studies have explored the influence of swelling of clay minerals, the relationships between changes in elastic moduli and deformation in rocks, which hardly contain swellable minerals, remain to date enigmatic. In this paper, to evaluate quantitatively these effects, drying rates, strains and ultrasonic velocities of small cylindrical Berea sandstone samples were measured as their water saturation was decreased by evaporative drying. The measurements clearly showed drying shrinkage and drastic increases in shear and compressional moduli for all the samples under an almost fully dried condition. Previous studies considered that the alteration in surface energy of grain minerals between wet and dry states affected the contact stiffness between them. Some of them used micromechanical model, uniting the Digby grain contact model with the effective medium theory, to interpret the changes in elastic moduli observed in the non-swellable sandstones during water adsorption on the grain surface. The conventional micromechanical model assumes that a mineral grain is a pure sphere and that the number of contacts between the grains is one. However, grains in a sedimentary rock are generally not purely spherical, and the contact surface is composed of several adhesive asperities. We therefore modified the conventional model by introducing the curvature radius and the number of asperities per contact surface. The modified model well reproduced the shear moduli under wet conditions using the strain and moduli measured under dry conditions. On the other hand, the predictions of the compressional moduli using the model were partially in agreement with the experimental results. Therefore, we attempted to qualitatively interpret the relationship by combining the model with the viscoelastic effect associated with wave-induced fluid flow. Substandard and falsified (SF) medical products pose a major threat to public health and socioeconomic development, particularly in low- and middle-income countries. In response, public education campaigns have been developed to alert consumers about the risks of SF medicines and provide guidance on 'safer' practices, along with other demand- and supply-side measures. However, little is currently known about the potential effectiveness of such campaigns while structural constraints to accessing quality-assured medicines persist. This paper analyses survey data on medicine purchasing practices, information and constraints from four African countries (Ghana, Nigeria, Sierra Leone and Uganda; n  > 1000 per country). Using multivariate regression and structural equation modelling, we present what we believe to be the first attempt to tease apart, statistically, the effects of an information gap vs structural constraints in driving potential public exposure to SF medicines. The analysis confirms that less privileged groups (including, variously, those in rural settlements, with low levels of formal education, not in paid employment, often women and households with a disability or long-term sickness) are disproportionately potentially exposed to SF medicines; these same demographic groups also tend to have lower levels of awareness and experience greater levels of constraint. Despite the constraints, our models suggest that public health education may have an important role to play in modifying some (but not all) risky practices. Appropriately targeted public messaging can thus be a useful part of the toolbox in the fight against SF medicines, but it can only work effectively in combination with wider-reaching reforms to address higher-level vulnerabilities in pharmaceutical supply chains in Africa and expand access to quality-assured public-sector health services. Effective citizen engagement is crucial for the success of social health insurance, yet little is known about the mechanisms used to involve citizens in low- and middle-income countries. This paper explores citizen engagement efforts by the National Health Insurance Fund (NHIF) and their impact on health insurance coverage within rural informal worker households in western Kenya. Our study employed a mixed methods design, including a cross-sectional household survey (n  = 1773), in-depth household interviews (n  = 36), six focus group discussions with community stakeholders and key informant interviews (n  = 11) with policymakers. The findings reveal that NHIF is widely recognized, but knowledge of its services, feedback mechanisms and accountability systems is limited. NHIF enrolment among respondents is low (11%). The majority (63%) are aware of NHIF, but only 32% know about the benefit package. There was higher awareness of the benefit package (60%) among those with NHIF compared to those without (28%). Satisfaction with the NHIF benefit package was expressed by only 48% of the insured. Nearly all respondents (93%) are unaware of mechanisms to provide feedback or raise complaints with NHIF. Of those who are aware, the majority (57%) mention visiting NHIF offices for assistance. Most respondents (97%) lack awareness of NHIF's performance reporting mechanisms and express a desire to learn. Negative media reports about NHIF's performance erode trust, contributing to low enrolment and member attrition. Our study underscores the urgency of prioritizing citizen engagement to address low enrolment and attrition rates. We recommend evaluating current citizen engagement procedures to enhance citizen accountability and incorporate their voices. Equally important is the need to build the capacity of health facility staff handling NHIF clients in providing information and addressing complaints. Transparency and information accessibility, including the sharing of performance reports, will foster trust in the insurer. In many countries, healthcare systems suffer from fragmentation between hospitals and primary care. In response, many governments institutionalized healthcare networks (HN) to facilitate integration and efficient healthcare delivery. Despite potential benefits, the implementation of HN is often challenged by inefficient collaborative dynamics that result in delayed decision-making, lack of strategic alignment and lack of reciprocal trust between network members. Yet, limited attention has been paid to the collective dynamics, challenges and enablers for effective inter-organizational collaborations. To consider these issues, we carried out a scoping review to identify the underlying processes for effective inter-organizational collaboration and the contextual conditions within which these processes are triggered. Following appropriate methodological guidance for scoping reviews, we searched four databases [PubMed (n  = 114), Web of Science (n  = 171), Google Scholar (n  = 153) and Scopus (n  = 52)] and used snowballing (n  = 22). A total of 37 papers addressing HN including hospitals were included. We used a framework synthesis informed by the collaborative governance framework to guide data extraction and analysis, while being sensitive to emergent themes. Our review showed the prominence of balancing between top-down and bottom-up decision-making (e.g. strategic vs steering committees), formal procedural arrangements and strategic governing bodies in stimulating participative decision-making, collaboration and sense of ownership. In a highly institutionalized context, the inter-organizational partnership is facilitated by pre-existing legal frameworks. HN are suitable for tackling wicked healthcare issues by mutualizing resources, staff pooling and improved coordination. Overall performance depends on the capacity of partners for joint action, principled engagement and a closeness culture, trust relationships, shared commitment, distributed leadership, power sharing and interoperability of information systems To promote the effectiveness of HN, more bottom-up participative decision-making, formalization of governance arrangement and building trust relationships are needed. Most policy analysis methods and approaches are applied retrospectively. As a result, there have been calls for more documentation of the political-economy factors central to health care reforms in real-time. We sought to highlight the methods and previous applications of prospective policy analysis (PPA) in the literature to document purposeful use of PPA and reflect on opportunities and drawbacks. We used a critical interpretive synthesis (CIS) approach as our initial scoping revealed that PPA is inconsistently defined in the literature. While we found several examples of PPA, all were researcher-led, most were published recently and few described mechanisms for engagement in the policy process. In addition, methods used were often summarily described and reported on relatively short prospective time horizons. Most of the studies stemmed from high-income countries and, across our sample, did not always clearly outline the rationale for a PPA and how this analysis was conceptualized. That only about one-fifth of the articles explicitly defined PPA underscores the fact that researchers and practitioners conducting PPA should better document their intent and reflect on key elements essential for PPA. Despite a wide recognition that policy processes are dynamic and ideally require multifaceted and longitudinal examination, the PPA approach is not currently frequently documented in the literature. However, the few articles reported in this paper might overestimate gaps in PPA applications. More likely, researchers are embedded in policy processes prospectively but do not necessarily write their articles from that perspective, and analyses led by non-academics might not make their way into the published literature. Future research should feature examples of testing and refining the proposed framework, as well as designing and reporting on PPA. Diabetes stigma is a pervasive social phenomenon with significant impacts on individuals living with type 2 diabetes mellitus (T2DM). Despite the negative health impact of diabetes stigma, little is known about the experience of this phenomenon in Africa. This review aimed to synthesize existing quantitative and qualitative studies that examine the experiences and outcomes of T2DM stigma in Africa. A mixed studies review methodology was utilized to conduct this research. Relevant articles were identified by searching the Cumulative Index to Nursing and Allied Health Literature, PubMed, MEDLINE and PsycINFO databases. The mixed method appraisal tool was used to assess the quality of included studies. Of 2626 records identified, 10 articles met the inclusion criteria. The prevalence of diabetes stigma was as high as 70%. The results of the review indicate that individuals with T2DM in Africa are labelled as 'having HIV', 'nearing their death' and 'wasting resources'. These experiences were associated with low quality of life, disease concealment and avoidance of self-management behaviours. The findings highlight the urgent need for further stigma-centric studies to fully understand how T2DM stigma is experienced in Africa. The evidence from such studies would inform the development and evaluation of effective interventions to address this social consequence of T2DM. [ABSTRACT FROM AUTHOR]

Trichomonas vaginalis is a protozoan parasite and probably one of the main causes of adverse birth outcomes. Still, there is a paucity of information regarding the burden of T. vaginalis infection in Iranian women. To address this gap, we conducted a systematic review and meta-analysis of studies on trichomoniasis among females in Iran from January 2000 to January 2023. We searched multiple electronic databases (PubMed, Science Direct, Scopus, ProQuest, Web of Science, Google Scholar, Scientific Information Database, Iran Medex, Magiran and Iran Doc) and identified 108 articles with a total of 200 728 women. Using a random effects model, we found a pooled prevalence of 4.30% (95% CI 3.23 to 5.52%). Subgroup analysis, also, demonstrated a statistically significant association between the T. vaginalis prevalence in Iranian women, and the age range, city, sample type, being in prison, sex working and symptomatic infections. Our findings provide important information for healthcare authorities and can inform prevention strategies against trichomoniasis in Iran. [ABSTRACT FROM AUTHOR]

Background The aim of the current study was to examine the prevalence and predictors of unmet need for contraception among women in sexual unions in Benin. Methods Data for the study was extracted from the recent 2017–2018 Benin Demographic and Health Survey. A weighted sample of 9513 women of reproductive age was included in the study. We used multivariable multilevel binary logistic regression analysis to examine the factors associated with unmet need for contraception. Results The prevalence of unmet need for contraception was 38.0% (36.7, 39.2). The odds of unmet need for contraception was higher among women with ≥4 births compared with those with no births, and among those who reported that someone else or others usually made decisions regarding their healthcare compared with those who make their own healthcare decisions. Wealth index was associated with a higher likelihood of unmet need for contraception. Also, the region of residence was associated with unmet need for contraception, with the highest odds being among women from the Mono region (adjusted odds ratio [aOR]=2.18, 95% CI 1.33 to 3.58). Conclusions Our study shows that the unmet need for contraception among women in Benin is relatively high. Our findings call on relevant stakeholders, including government and non-governmental organisations, to enhance women's empowerment as part of interventions that seek to prioritise contraceptive services for women. Background Human landing catches (HLCs) are required to collect blackflies for entomological evaluation to verify onchocerciasis elimination. However, there are ethical concerns regarding exposure of vector collectors to infectious blackflies and safer alternative methods are needed. This study evaluated a modified HLC technique where collectors wore coloured trousers (blue, black or blue-black), protecting them from bites during fly collection, and their performance was compared with the standard. Methods The study was conducted in Makouopsap, Cameroon, in the Massangam health district for 4 months. Four collector pairs—one 'standard' (bare-legged) and three modified—were placed 50 m apart along known breeding sites on the Mbam and Nja Rivers. Collections were performed from 07:00 to 17:00 h, 4 d/month. Hourly rates of flies caught were analysed using a negative binomial generalised linear model to explore associations between flies caught and collection techniques and seasons. Results Overall, 17 246 blackflies were caught. There was no significant statistical difference in the number of blackflies and parous flies caught between black trousers and the standard. Thus there is a strong indication that wearing black trousers is a viable non-inferior alternative to the standard HLC. Objective Results of ethanol ablation (EA) for controlling neck nodal metastases (NNM) in adult patients with papillary thyroid carcinoma (APTC) beyond 6 months have rarely been reported. We now describe outcome results in controlling 71 NNM in 40 node-positive stage I APTC patients followed for 66 to 269 months. Methods All 40 patients were managed with bilateral thyroidectomy and radioiodine therapy and followed with neck ultrasound (US) for >48 months after EA. Cumulative radioiodine doses ranged from 30 to 550 mCi; pre-EA 27 patients (67%) had 36 additional neck surgeries. Cytologic diagnosis of PTC in 71 NNM selected for EA was confirmed by US-guided biopsy. EA technique and follow-up protocol were as previously described. Results The 40 patients had 1 to 4 NNM; 67/71 NNM (94%) received 2 to 4 ethanol injections (total median volume 0.8 cc). All ablated 71 NNM shrank (mean volume reduction of 93%); nodal hypervascularity was eliminated. Thirty-eight NNM (54%) with initial volumes of 12-1404 mm3 (median 164) disappeared on neck sonography. Thirty-three hypovascular foci from ablated NNM (pre-EA volume range 31-636 mm3; median 147) were still identifiable with volume reductions of 45% to 97% observed (median 81%). There were no complications and no postprocedure hoarseness. Final results were considered to be ideal or near ideal in 55% and satisfactory in 45%. There was no evidence of tumor regrowth after EA. Conclusion Our results demonstrate that for patients with American Joint Committee on Cancer stage I APTC, who do not wish further surgery or radioiodine, and are uncomfortable with active surveillance, EA can achieve durable control of recurrent NNM. Context Despite a high prevalence of obesity in the veteran population, antiobesity medications (AOMs) have been underused in the Veterans Health Administration. Real-world reports on outcomes when AOMs have been used in veterans is limited. Objective To analyze weight loss outcomes from a local Veterans Health Administration pharmacotherapy-based weight management clinic (WMC). Methods This was a retrospective cohort study of veterans enrolled in a local WMC for 15 months from August 2016 through September 2018 and followed through November 2019. Patients were offered 1 of 5 available AOMs based on their comorbidities. Factors associated with weight loss (5% or more weight loss) were assessed. Key results A total of 159 patients were seen in a WMC, 149 (93.7%) veterans were prescribed an AOM, and 129 returned for follow-up. Overall, 61/129 (47%) patients achieved 5% or greater weight loss and 28/129 (22%) achieved 10% or greater weight loss within 15 months. Clinically significant weight loss (%) over the first 15 months was achieved with phentermine/topiramate ER (−6.3%) and liraglutide (−7.5%), but not with orlistat (−3.9%) and lorcaserin (−3.6%). Comorbid obstructive sleep apnea was negatively associated with achieving ≥5% weight loss. Conclusion Phentermine/topiramate ER and liraglutide were found to be effective AOMs among veterans. Context Sexual and ancestral differences in driver gene prevalence have been described in many cancers but have not yet been investigated in pheochromocytoma and paraganglioma (PPGL). Objective This study aims to assess whether sex and ancestry influence prevalence of PPGL driver genes and clinical presentation. Methods We conducted a retrospective analysis of patients with PPGL considering studies from 2010 onwards that included minimal data of type of disease, sex, mutated gene, and country of origin. Additional features were recorded when available (age, tumor location, bilateral or multifocal, somatic or germline, and metastatic disease). Results We included 2162 patients: 877 in Europe and 757 in Asia. Males presented more often with germline pathogenic variants (PVs) in genes activating hypoxia pathways (P =.0006) and had more often sympathetic paragangliomas (P =.0005) and metastasis (P =.0039). On the other hand, females with PPGLs due to MAX PVs were diagnosed later than males (P =.0378) and more often developed metastasis (P =.0497). European but not Asian females presented more often with PPGLs due to PVs in genes related to kinase signaling (P =.0052), particularly RET and TMEM127. Contrary to experiences from Europe, Asian patients with PPGL due to PVs in kinase signaling genes NF1 , HRAS , and FGFR1 showed a high proportion of sympathetic tumors, while European patients almost exclusively had adrenal tumors (P <.005). Conclusion Personalized management of patients with PPGL might benefit from considering sexual and ancestral differences. Objective This work aimed to study whether fibroblast growth factor 23 (FGF23) is predictive for incident posttransplant diabetes mellitus (PTDM) in kidney transplant recipients (KTRs). Methods We repeatedly analyzed plasma C-terminal FGF23 concentrations in 170 KTRs enrolled in the TransplantLines Biobank and Cohort Study. Associations of time-updated plasma FGF23 with incident PTDM were studied by Cox regression. Results A total of 170 KTRs (46% female, aged 54.4 ± 12.4 years) with 540 FGF23 measurements were included. Plasma FGF23 concentrations at transplantation were 31.1 (0.76-2576) pmol/L. During a follow-up of 24 (12-24) months, 38 patients developed PTDM. The highest FGF23 tertile (compared to the lowest) was associated with an increased risk for PTDM (fully adjusted hazard ratio 20.9; 95% CI, 3.4-130.0; P <.001). Conclusion In KTRs without diabetes at baseline, the highest tertile of FGF23, compared to the lowest, is predictive for development of PTDM. [ABSTRACT FROM AUTHOR]

Context Metabolic surgery remains the most effective and durable treatment for severe obesity and related metabolic diseases. Objective We examined cardiometabolic improvements after metabolic surgery and associated presurgery demographic and clinical factors in a large multiracial cohort. Methods Included were 7804 patients (20-79 years) undergoing first-time metabolic surgery at Vanderbilt University Medical Center from 1999 to 2022. Pre- and 1-year postsurgery cardiometabolic profiles were extracted from medical records, including body mass index (BMI), blood pressure, blood lipids, glucose, and hemoglobin A1c. The 10-year atherosclerotic cardiovascular disease (ASCVD) risk was estimated per American College of Cardiology/American Heart Association equations. Pre- to postsurgery cardiometabolic profiles were compared by paired t-test, and associated factors were identified by multivariable linear and logistic regression. Results Among 7804 patients, most were women and White, while 1618 were men and 1271 were Black; median age and BMI were 45 years [interquartile range (IQR): 37-53] and 46.4 kg/m2 (IQR: 42.1-52.4). At 1-year postsurgery, patients showed significant decreases in systolic blood pressure (10.5 [95% confidence interval: 10.1, 10.9] mmHg), total cholesterol (13.5 [10.3, 16.7] mg/dL), glucose (13.6 [12.9, 14.4] mg/dL), hemoglobin A1c (1.13% [1.06, 1.20]), and 10-year ASCVD risk (absolute reduction: 1.58% [1.22, 1.94]; relative reduction: 34.4% [29.4, 39.3]); all P <.0001. Older, male, or Black patients showed less reduction in 10-year ASCVD risk and lower odds of diabetes/hypertension/dyslipidemia remission than younger, female, or White patients. Patients with a history of diabetes, hypertension, dyslipidemia, or cardiovascular disease showed less cardiometabolic improvements than those without. Results were similar with or without further adjusting for weight loss and largely sustained at 2-year postsurgery. Conclusion Metabolic surgery results in significant cardiometabolic improvements, particularly among younger, female, or White patients and those without comorbidities. [ABSTRACT FROM AUTHOR]
