Pulsating ultraluminous X-ray sources are thought to be X-ray bright, accreting, magnetized neutron stars. Their measured soft (<20 keV) X-ray luminosity can exceed the Eddington luminosity for a neutron star by a few orders of magnitude. Although several scenarios have been proposed to explain the different components observed in the X-ray spectrum and the characteristics of the X-ray pulse profile of these systems, detailed quantitative calculations are still missing. In particular, the observed soft X-ray pulse profiles are almost sinusoidal and show an increase in the pulsed fraction (from 8 up to even 30 per cent) with increasing energy. In this work, we discuss how emission originating from an optically thick envelope, expected to be formed during super-Eddington accretion, can match the observed spectra and pulse profiles. Detailed studies of the Sun have shown that sunspots and solar flares are closely correlated. Photometric data from Kepler / K2 has allowed similar studies to be carried out on other stars. Here, we utilize Transiting Exoplanet Survey Satellite (TESS) photometric 2-min cadence of 167 low-mass stars from Sectors 1 to 3 to investigate the relationship between star-spots and stellar flares. From our sample, 90 per cent show clear rotational modulation likely due to the presence of a large, dominant star-spot and we use this to determine a rotational period for each star. Additionally, each low-mass star shows one or more flares in its light curve and using Gaia Data Release 2 parallaxes and SkyMapper magnitudes we can estimate the energy of the flares in the TESS band-pass. Overall, we have 1834 flares from the 167 low-mass stars with energies from 6.0 × 1029 to 2.4 × 1035 erg. We find none of the stars in our sample show any preference for rotational phase, suggesting the lack of a correlation between the large, dominant star-spot, and flare number. Classification of stellar spectra from voluminous spectra is a very important and challenging task. In order to better classify stellar spectra, inspired by the principle of deep convolutional neural network (CNN), we propose a supervised algorithm for stellar spectra classification based on 1D stellar spectra convolutional neural network (1D SSCNN). In 1D SSCNN, we modify the traditional 2D convolutional neural network into 1D network to adapt to the spectral classification. On the basis of using convolution algorithm, the spectral features are extracted and used for classification. We first use the stellar spectra data to train a 1D SSCNN to obtain a well-trained model, and then we apply the well-trained model to classify the unknown spectra. To evaluate the performance of the proposed algorithms, we apply 1D SSCNN to classify three spectral types: F-type spectra, G-type spectra, and K-type spectra and 10 subclasses of K-type spectra: A0-type, A5-type, F0-type, F5-type, G0-type, G5-type, K0-type, K5-type, M0-type, and M5-type spectra from Sloan Digital Sky Survey (SDSS). Our 1D SSCNN algorithm obtain higher classification accuracy compared with support vector machine (SVM), random forest (RF), and artificial neural network (ANN). [ABSTRACT FROM AUTHOR]

We present an analysis of K2 short cadence data of 34 M dwarfs which have spectral types in the range M0–L1. Of these stars, 31 showed flares with a duration between ∼10 and 90min. Using distances obtained from Gaia DR2 parallaxes, we determined the energy of the flares to be in the range ∼1.2 × 1029–6 × 1034 erg. In agreement with previous studies we find rapidly rotating stars tend to show more flares, with evidence for a decline in activity in stars with rotation periods longer than ∼10 d. The rotational modulation seen in M dwarf stars is widely considered to result from a starspot which rotates in and out of view. Flux minimum is therefore the rotation phase where we view the main starspot close to the stellar disc centre. Surprisingly, having determined the rotational phase of each flare in our study we find none show any preference for rotational phase. We outline three scenarios which could account for this unexpected finding. Summary: Calciphylaxis is associated with significant morbidity and mortality. Palliative care (PC) is a subspecialty that treats the pain and stress of serious illness. To assess whether the role of quality of life (QoL) indices, patient‐reported outcome measures and PC have been studied in patients with calciphylaxis, we performed a systematic literature review. Several databases were searched from inception to October 2016 according to modified Preferred Reporting Items for Systematic Reviews and Meta‐Analyses criteria. We searched for papers on calciphylaxis that mentioned the symptoms and supportive needs of patients, QoL or outcome measures to report symptom severity, and the involvement of PC. Twelve papers met the inclusion criteria. Reported patient symptoms included pain, skin lesion resolution and pruritus, with the first being the most frequently reported. Four papers measured pain using a previously verified patient‐reported outcome measure, including the Visual Analogue Scale. One paper used a verified QoL measure, the Dermatology Quality of Life Index. No tool was used consistently. Eight papers reported the use of hospice care or PC in the treatment of calciphylaxis. No outcome measure was used to prompt PC involvement. Overall, QoL indices, patient‐reported outcome measures and PC are underreported in the treatment of calciphylaxis. Background Postoperative hypocalcaemia due to dysfunction of the parathyroid glands is the most common complication after total thyroidectomy plus central neck dissection ( CND). There is a lack of surgical techniques described to help preserve the inferior parathyroid gland in situ during CND. The objective of this study was to introduce the ' TBP layer' (layer of thymus-blood vessel-inferior parathyroid gland) concept for preserving the inferior parathyroid gland in situ during CND, and to evaluate its effectiveness. Methods The study group included patients with primary papillary thyroid cancer who underwent total thyroidectomy with CND using the new surgical concept between January and December 2014. The control group included sex- and age-matched patients who underwent conventional total thyroidectomy with CND between January 2012 and December 2013. The proportion of inferior parathyroid glands preserved in situ and postoperative hypoparathyroidism rates in the two groups were compared. Results There were 181 patients in the study group and 306 in the control group. There were no significant differences between the groups in tumour size, multifocality, extrathyroidal extension, and number of harvested and metastatic central lymph nodes. The rate of inferior parathyroid gland preservation in situ was significantly improved from 37·9 to 76·3 per cent on the left side ( P < 0·001), and from 52·0 to 77·9 per cent on the right side ( P < 0·001), in the study group compared with the control group. The incidence of transient hypoparathyroidism decreased significantly from 35·0 to 7·2 per cent ( P < 0·001). Conclusion Applying the proposed surgical concept improved the rate of inferior parathyroid gland preservation in situ and decreased the incidence of transient postoperative hypoparathyroidism. We present the results of a series of short radio observations of six ultracool dwarfs made using the upgraded Very Large Array in S (2-4GHz) and C (4-7GHz) bands. LSR J1835+3259 exhibits a 100 per cent right-hand circularly polarized burst that shows intense narrow-band features with a fast negative frequency drift of about -30 MHz s-1. They are superimposed on a fainter broad-band emission feature with a total duration of about 20 min, bandwidth of about 1 GHz, centred at about 3.5 GHz, and a slow positive frequency drift of about 1 MHz s-1. This makes it the first such event detected below 4 GHz and the first one exhibiting both positive and negative frequency drifts. Polarized radio emission is also seen in 2MASS J00361617+1821104 and NLTT 33370, while LP 349-25 and TVLM 513-46546 have unpolarized emission and BRI B0021-0214 was not detected. We can reproduce the main characteristics of the burst from LSR J1835+3259 using a model describing the magnetic field of the dwarf as a tilted dipole. We also analyse the origins of the quiescent radio emission and estimate the required parameters of the magnetic field and energetic electrons. The 'Magnificent Seven' (M7) are a group of radio-quiet isolated neutron stars discovered in the soft X-rays through their purely thermal surface emission. Owing to the large inferred magnetic fields (B ≈ 1013 G), radiation from these sources is expected to be substantially polarized, independently of the mechanism actually responsible for the thermal emission. A large observed polarization degree (PD) is, however, expected only if quantum-electrodynamic (QED) polarization effects are present in the magnetized vacuum around the star. The detection of a strong linearly polarized signal would therefore provide the first observational evidence of QED effects in the strong-field regime. While polarization measurements in the soft X-rays are not feasible yet, optical polarization measurements are within reach also for quite faint targets, like the M7 which have optical counterparts with magnitudes ≈26-28. Here, we report on the measurement of optical linear polarization for the prototype, and brightest member, of the class, RX J1856.5-3754 (V ~ 25.5), the first ever for one of the M7, obtained with the Very Large Telescope. We measured a PD = 16.43 ± 5.26 per cent and a polarization position angle PA = ... ± ..., computed east of the North Celestial Meridian. The observed polarization properties of thermal radiation from isolated, cooling neutron stars depend on both the emission processes at the surface and the effects of the magnetized vacuum which surrounds the star. Here, we investigate the polarized thermal emission from X-ray dim isolated neutron stars, taking RX J1856.5-3754 as a representative case. The physical conditions of the star outermost layers in these sources is still debated, and so we consider emission from a magnetized atmosphere and a condensed surface, accounting for the effects of vacuum polarization as the radiation propagates in the star magnetosphere. We have found that, for a significant range of viewing geometries, measurement of the phase-averaged polarization fraction and phase-averaged polarization angle at both optical and X-ray wavelengths allow us to determine whether this neutron star has an atmosphere or a condensed surface. The pectin lyase gene pnl-zj5a from Aspergillus niger ZJ5 was identified and expressed in Pichia pastoris. PNL-ZJ5A was purified by ultrafiltration, anion exchange and gel chromatography. The Km and Vmax values determined using citrus pectin were 0·66 mg ml−1 and 32·6 μmol min−1 mg−1, respectively. PNL-ZJ5A exhibited optimal activity at 43°C and retained activity over 25-50°C. PNL-ZJ5A was optimally active at pH 5 and effective in apple juice clarification. Compared with controls, PNL-ZJ5A increased the fruit juice yield significantly. Furthermore, PNL-ZJ5A reduced the viscosity of apple juice by 38·8% and increased its transmittance by 86·3%. PNL-ZJ5A combined with a commercial pectin esterase resulted in higher juice volume. Significance and Impact of the Study In this study, pectin lyase gene pnl-zj5a from Aspergillus niger ZJ5 was expressed successfully in Pichia pastoris. The purified enzyme PNL-ZJ5A exhibited optimal activity at 43°C and pH 5. In addition, the acidic pectin lyase PNL-ZJ5A can effectively decreased the viscosity and improved the yield of apple juice and light transmittance. These favourable enzymatic properties of PNL-ZJ5A show potential application in food industry, especially in juice-processing applications and the extraction of plant tissue. [ABSTRACT FROM AUTHOR]

Background In recent years, increasing attention has been focused on the skin hypopigmentation that develops after the initiation of imatinib mesylate therapy in patients with chronic myeloid leukaemia ( CML). Aim To understand the underlying mechanism of this hypopigmentation effect, and to explore the possibility of using imatinib in the treatment of pigmentation disorders. Methods We examined the effects of imatinib on the proliferation, apoptosis, melanin content and melanogenic activity of human primary epidermal melanocytes. The responsible molecular events were also investigated in a mechanism study. Results We found that imatinib led to a dramatic decrease in total melanin content in cultured melanocytes, by affecting melanocyte number and/or melanogenesis in a dose-dependent manner. This inhibition of melanogenesis was due to suppressed expression of tyrosinase and microphthalmia-associated transcription factor ( Mi TF). Furthermore, stem cell factor ( SCF)-stimulated c- Kit activation and melanocyte proliferation were completely abrogated by imatinib. Conclusions Inactivation of c- Kit signalling by imatinib has inhibitory effects on melanocyte survival, proliferation and melanogenesis, which explains the clinical hypopigmentation seen in patients with CML. Cognitive deficits occur in up to 30% of patients with early Parkinson's disease, some of which are thought to result from dysfunction within the fronto-striatal dopaminergic network. Recently, it has been shown that a common functional polymorphism (Val(158)Met) in the catechol-O-methyltransferase (COMT) gene is associated with changes in executive performance in tasks that have a fronto-striatal basis. This is thought to relate to changes in cortical dopamine levels as catechol-O-methyltransferase is the main mode of inactivation for dopamine in frontal areas. However to date, no study has investigated dopamine turnover as a function of this genetic polymorphism in Parkinson's disease. We, therefore, set out to investigate in vivo changes in presynaptic dopamine storage in patients with idiopathic Parkinson's disease as a function of the catechol-O-methyltransferase Val(158)Met polymorphism using (18)F-DOPA positron emission tomography. Twenty patients with Parkinson's disease (10 homozygous for Val/Val and 10 for Met/Met catechol-O-methyltransferase polymorphisms) underwent (18)F-DOPA positron emission tomography using a prolonged imaging protocol. The first dynamic scan was acquired from 0 to 90 min (early), and the second scan (late) from 150 to 210 min post-intravenous radioligand administration. Patients were matched for age, sex, verbal IQ, disease duration and severity of motor features. (18)F-DOPA influx constants (Ki) were calculated and compared for frontal and striatal regions. Late scan mean frontal and striatal Ki values were significantly reduced in both Parkinson's disease groups relative to early scan Ki values. Met/Met patients had significantly higher late scan Ki values compared with their Val/Val counterparts in anterior cingulate, superior frontal and mid-frontal regions but early frontal Ki values were not different between the two groups. As late Ki values reflect rates of dopamine metabolism to 3,4-dihydroxyphenylacetic acid and homovanillic acid, our results indicate that Met homozygotes have higher presynaptic dopamine levels in frontal regions than Val homozygotes, which may help to explain how this genotypic variation may influence the fronto-striatal cognitive deficits of Parkinson's disease. It states that carbamazepine is one of the most prescribed medications to several disorders, but it is also one of the most common causes of drug-induced SJS and TEN in ethnic groups. Moreover, HLA typing can help predict and prevent carbamazepine-induced hypersensitivity reactions in certain individuals.

Summary Background Some probiotics can ameliorate childhood atopic dermatitis (AD). Prebiotics have also shown some efficacy, although when combined with probiotics as synbiotics, their efficacy may improve. Objective We compared the effects of Lactobacillus salivarius and fructo-oligosaccharide (synbiotic) with fructo-oligosaccharide alone (prebiotic) on children with moderate to severe AD. Methods We randomly assigned 60 children aged 2-14 years with moderate to severe AD [SCORing AD (SCORAD) > 25] to a treatment (synbiotic) or a control (prebiotic) group (30 per group). They received one capsule twice daily for 8 weeks containing either L. salivarius plus fructo-oligosaccharide (treatment) or fructo-oligosaccharide only (control). SCORAD indices were monitored at weeks 0, 4, 8 and 10 (post-treatment). Laboratory results and AD medication use were also monitored. Results Baseline demographic and clinical characteristics and SCORAD scores were similar between the two groups. At 8 weeks, the treatment group SCORAD scores (27·4 ± 12·7) were significantly lower than for the controls (36·3 ± 14·9) ( P = 0·022); this difference remained at 10 weeks. At 8 weeks, treatment group AD intensity was significantly lower ( P = 0·013); more children had mild AD in the treatment group (52%; 14/27) than the control group (30%; 8/27) ( P = 0·024). Medication use frequency and eosinophil cationic protein levels were significantly reduced in the treatment group at 8 weeks compared with 4 weeks. Conclusion A synbiotic combination of L. salivarius plus fructo-oligosaccharide is superior to the prebiotic alone for treating moderate to severe childhood AD. Background: Dialysis patients with uremic pruritus have worse outcomes. However, the pathophysiology of the high mortality in these patients remains inconclusive except for links with calcium/phosphate imbalance and sleep disturbance. Whether inflammation, an outcome predictor in dialysis patients, plays a role is unknown.Methods: This prospective study included 321 chronic hemodialysis (HD) patients (>3 months) for survival analysis. A visual analog scale (VAS) was used to measure the severity of itching, and the patients were divided into four groups: no pruritus (VAS = 0, N = 118), mild (VAS 1–3, N = 76), moderate (VAS 4–7, N = 89) and severe pruritus (VAS 8–10, N = 38). The Pittsburgh Sleep Quality Index (PSQI) was used to define sleep disturbance, while high-sensitive C-reactive protein (hs-CRP) and tumor necrosis factor α (TNF-α) were used to evaluate inflammation. The patients were followed-up for 30 months.Results: Patients with moderate/severe pruritus had higher hs-CRP, but similar TNF-α levels; they also had a worse survival rate (P = 0.0197, log rank test). By stratifying hs-CRP levels, those with higher hs-CRP had worse survival regardless of the severity of uremic pruritus. In a Cox proportional hazard model, hs-CRP levels and moderate/severe uremic pruritus were independent predictors of mortality after adjusting for age, poor sleeper (PSQI > 5), diabetes, albumin, phosphate, hemoglobin and parathyroid hormone levels and (hs-CRP) × (moderate/severe uremic pruritus) (all P < 0.05).Conclusion: In moderate/severe pruritic HD patients, those with higher hs-CRP suffer from worse overall mortality. The role of mast cells (MCs) in the generation of adaptive immune responses especially in the transplant immune responses is far from being resolved. It is reported that mast cells are essential intermediaries in regulatory T cell (Treg) transplant tolerance, but the mechanism has not been clarified. To investigate whether bone marrow-derived mast cells (BMMCs) can induce Tregs by expressing transforming growth factor beta 1 (TGF-β1) in vitro, bone marrow cells obtained from C57BL/6 (H-2b) mice were cultured with interleukin (IL)-3 (10 ng/ml) and stem cell factor (SCF) (10 ng/ml) for 4 weeks. The purity of BMMCs was measured by flow cytometry. The BMMCs were then co-cultured with C57BL/6 T cells at ratios of 1:2, 1:1 and 2:1. Anti-CD3, anti-CD28 and IL-2 were administered into the co-culture system with (experiment groups) or without (control groups) TGF-β1 neutralizing antibody. The percentages of CD4+CD25+forkhead box P3 (FoxP3)+ Tregs in the co-cultured system were analysed by flow cytometry on day 5. The Treg percentages were significantly higher in all the experiment groups compared to the control groups. These changes were deduced by applying TGF-β1 neutralizing antibody into the co-culture system. Our results indicated that the CD4+ T cells can be induced into CD4+CD25+FoxP3+ T cells by BMMCs via TGF-β1. [ABSTRACT FROM AUTHOR]

We revisit the discovery outburst of the X-ray transient XTE J1550−564 during which relativistic jets were observed in 1998 September, and review the radio images obtained with the Australian Long Baseline Array, and light curves obtained with the Molonglo Observatory Synthesis Telescope and the Australia Telescope Compact Array. Based on Hi spectra, we constrain the source distance to between 3.3 and 4.9 kpc. The radio images, taken some 2 d apart, show the evolution of an ejection event. The apparent separation velocity of the two outermost ejecta is at least and may be as large as ; when relativistic effects are taken into account, the inferred true velocity is . The flux densities appear to peak simultaneously during the outburst, with a rather flat (although still optically thin) spectral index of −0.2. [ABSTRACT FROM AUTHOR]

Only a few studies have been performed comparing subjective symptom improvements with objective improvement in esophageal emptying after pneumatic dilation (PD), and discrepancy existed. We evaluated whether esophagogram measurements adds to the traditional subjective symptoms scores in assessing achalasia patients after PD. We enrolled 32 new patients with achalasia who received endoscope-guided PD treatment between January 1998 and June 2004. Postdilation investigations were performed by using esophageal emptying on esophagogram prospectively in a blinded manner, along with symptom scores before and after PD at the initial investigation, 6 weeks later, and every 1 year thereafter. Our results showed that seven patients who noted complete relief showed less than 50% improvement in barium column height and esophageal diameter. There was no linear correlation between the degree of patient symptom improvement and esophageal emptying measured by esophagogram ( r = 0.181, P = 0.322). A trend of association existed in the relationship between clinical remissions and initial post-PD esophageal emptying improvement ( P = 0.067). In summary, the association between the post-PD symptom score improvement and degrees of esophageal emptying may be hampered by the small sample size in the current study. The malignant transformation of esophageal mucosa is a progressive process, which includes basal cell hyperplasia, dysplasia, carcinoma in situ, and invasive esophageal squamous cell carcinoma (ESCC). The objectives of this study were to prove the relationship of squamous cell carcinoma antigen 2 (SCCA2) mRNA expression in peripheral blood with non-malignant lesion, premalignant lesion, and carcinoma of the esophagus at the same assay, as well as to evaluate whether or not SCCA2 mRNA expression in peripheral blood may be a biomarker for monitoring the premalignant lesion of the disease. The subjects consisted of 50 patients with basal cell hyperplasia, 50 patients with dysplasia, 50 patients with ESCC (12 carcinoma in situ, 38 carcinoma in invasive stage), and 50 controls who were pathologically diagnosed to be normal and whose esophageal mucosa were stained brown by iodine. All the subjects are residents of Feicheng, China, which is considered an area with a high incidence of esophageal cancer. All subjects were diagnosed by two separate histopathologists, and the expression of SCCA2 mRNA in peripheral blood was detected by semiquantitative reverse transcription-polymerase chain reaction (RT-PCR). Furthermore, SCCA2 concentration in the serum was measured using an enzyme-linked immunosorbent assay (ELISA). In the cancer group, SCCA2 mRNA expression was also detected in 20 tissues of esophageal cancer. By using the band intensity ratios of SCCA2 to β-actin, with a positive cut-off value of ≥ 0.4, the positive rates of the SCCA2 mRNA expression in peripheral blood were found to be 82% (41/50), 60% (30/50), 48% (24/50), and 36% (18/50) in the cancer, dysplasia, basal cell hyperplasia, and control groups, respectively. The positive rate of the cancer group was significantly different from the three other groups ( P < 0.05), and there was also a significant difference in the SCCA2 mRNA expression between the dysplasia group and the control group (χ2=5.769, P= 0.016). In the multinomial logistic regression analysis, the odds ratios (ORs) were 1.71 [95% confidence interval (95% CI), 0.73–3.99] in the basal cell hyperplasia group, 2.77 (95% CI, 1.14–6.71) in the dysplasia group, and 7.87 (95% CI, 2.88–21.55) in the cancer group after being adjusted for age, gender, smoking index, drinking index, and family history of esophageal cancer. The SCCA2 mRNA expression in peripheral blood was then divided into different grades according to the band intensity ratios of SCCA2 to β-actin. By using a positive cut-off value of ≥ 0.4, the testing sensitivities in the basal cell hyperplasia, dysplasia, and cancer groups were found to be 48%, 60%, and 82%, respectively, with the same testing specificity at 64%. On the other hand, SCCA2 mRNA expression in peripheral blood had a 97.5% agreement with that in tissue, and there was a significant correlation between the ELISA SCCA2 levels in the serum and the SCCA2 mRNA expression levels in the peripheral blood ( r= 0.80, P= 0.01). The results indicate that SCCA2 mRNA expression in peripheral blood is linked with the different stages of esophageal pathological changes, despite the fact that SCCA2 mRNA was not a biomarker for screening early esophageal cancer. Compton scattering within the accretion column of magnetic cataclysmic variables (mCVs) can induce a net polarization in the X-ray emission. We investigate this process using Monte Carlo simulations and find that significant polarization can arise as a result of the stratified flow structure in the shock-ionized column. We find that the degree of linear polarization can reach levels up to ∼8 per cent for systems with high accretion rates and low white dwarf masses, when viewed at large inclination angles with respect to the accretion column axis. These levels are substantially higher than previously predicted estimates using an accretion column model with uniform density and temperature. We also find that for systems with a relatively low-mass white dwarf accreting at a high accretion rate, the polarization properties may be insensitive to the magnetic field, since most of the scattering occurs at the base of the accretion column where the density structure is determined mainly by bremsstrahlung cooling instead of cyclotron cooling. Compton scattering of X-rays in the bulk flow of the accretion column in magnetic cataclysmic variables (mCVs) can significantly shift photon energies. We present Monte Carlo simulations based on a non-linear algorithm demonstrating the effects of Compton scattering on the H-, He-like and neutral Fe Kα lines produced in the post-shock region of the accretion column. The peak line emissivities of the photons in the post-shock flow are taken into consideration and frequency shifts due to Doppler effects are also included. We find that line profiles are most distorted by Compton scattering effects in strongly magnetized mCVs with a low white dwarf mass and high mass accretion rate and which are viewed at an oblique angle with respect to the accretion column. The resulting line profiles are most sensitive to the inclination angle. We have also explored the effects of modifying the accretion column width and using a realistic emissivity profile. We find that these do not have a significant overall effect on the resulting line profiles. A comparison of our simulated line spectra with high-resolution Chandra/HETGS observations of the mCV GK Per indicates that a wing feature redward of the 6.4-keV line may result from Compton recoil near the base of the accretion column. Background: Low vitamin D status has long been implicated in colorectal carcinogenesis. We investigated this relationship in a nested case-control study within the Health Professionals Follow-up Study (HPFS), a large ongoing study of male health professionals living in the United States.Methods: Between 1993 and 2002, 179 colorectal cancer patients were diagnosed and matched to 356 control subjects by age and by month and year of blood collection. Results were also pooled with previously published results from the Nurses' Health Study (NHS) cohort, a large female cohort. Conditional logistic regression was used to analyze the association between plasma 25-hydroxyvitamin D [25(OH)D] and colorectal cancer, and pooled estimates were calculated using the method of DerSimonian and Laird. All statistical tests were two-sided.Results: In the HPFS, we observed a non-statistically significant inverse association between higher plasma 25(OH)D concentration and risk of colorectal cancer and a statistically significant inverse association for colon cancer (highest versus lowest quintile: odds ratio [OR] = 0.46, 95% confidence interval [CI] = 0.24 to 0.89; P(trend) = .005). After pooling the results from the HPFS and NHS, higher plasma 25(OH)D concentrations were statistically significantly associated with decreased risks of both colorectal cancer (highest versus lowest quintile, OR = 0.66, 95% CI = 0.42 to 1.05; P(trend) = .01) and colon cancer (highest versus lowest quintile, OR = 0.54, 95% CI = 0.34 to 0.86; P(trend) = .002). Inverse associations with plasma 25(OH)D concentration did not differ by location of colon cancer (proximal versus distal), but the number of patients was small and none of the associations was statistically significant. Opposite relationships between plasma 25(OH)D levels and risk of rectal cancers were found among men (positive) and women (inverse).Conclusion: Our data provide additional support for the inverse association between vitamin D and colorectal and, in particular, colon cancer risk. We present anXMM–Newtonobservation of the Seyfert–LINER (low-ionization nuclear emission-line region) galaxy NGC 7213. The RGS soft X-ray spectrum is well fitted with a power law plus soft X-ray collisionally ionized thermal plasma. We confirm the presence of Fe i, Fe xxvand Fe xxviKα emission in the EPIC spectrum and set tighter constraints on their equivalent widths ofand 24+10−13 eV, respectively. We compare the observed properties together with the inferred mass accretion rate of NGC 7213 with those of other Seyfert and LINER galaxies. We find that NGC 7213 has intermediate X-ray spectral properties lying between those of the weak active galactic nucleus found in the LINER M81 and higher-luminosity Seyfert galaxies. There appears to be a continuous sequence of X-ray properties from the Galactic Centre through LINER galaxies to Seyferts, probably determined by the amount of material available for accretion in the central regions. Background: Recently we discovered that the human oviduct synthesizes abundant prostacyclin (PGI(2)). Gene knock-out studies suggest that PGI(2) is essential to endometrial decidualization, but the effects of PGI(2) on sperm and embryos have not been reported.Methods: The effects of PGI(2) on human sperm were analysed by a computer-assisted semen analysis system. The effects of PGI(2) on mouse embryos were examined based on the rates of complete hatching. The expression of PGI(2) receptor (IP) was evaluated by Western blot analysis and immunohistochemistry. The binding of PGI(2) to embryos was confirmed by radioligand binding assay. Finally, cAMP levels were assessed in PGI(2)-challenged embryos.Results: Iloprost (a stable PGI(2) analogue) did not affect the motility or the overnight survivability of human sperm. Western blot analysis did not detect IP in the sperm plasma membrane. In contrast, the hatching of mouse embryos was enhanced by iloprost (ED(50) 6.7 nmol/l). Exposure to iloprost during 8-cell to morulae or morulae to early blastocyst stages was critical to enhanced hatching. This coincided with the developmental stage-specific expression of IP. We present soft X-ray spectroscopy of the black hole binary LMC X-3 from the XMM–Newton Reflection Grating Spectrometer. The observations span the full range of spectral states seen in LMC X-3. The spectra are completely dominated by continuum emission, and the neutral absorbing column measured from the O i edge is consistent with the Galactic interstellar column density towards LMC X-3. We find no evidence for variability of the neutral absorbing column. We also constrain the ionized column density using the upper limits to the equivalent width of the O ii–O viii K-shell resonance lines: we find that the equivalent hydrogen column density of gas in which O is partially ionized is . From this upper limit we can rule out a line-driven stellar wind as the power source for the X-ray emission of LMC X-3 except when it is faint. At wavelengths longward of the peak emission the spectral shape is well described by a multitemperature disc–blackbody spectrum; the power-law component, which dominates at shorter wavelengths, does not continue longward of the disc–blackbody peak. This implies that the multitemperature disc–blackbody component supplies the seed photons that are Compton upscattered in the hot corona, consistent with the standard paradigm for black hole X-ray binary spectral states. Background: Laboratory data suggest that calcium and vitamin D, found at high levels in dairy products, might reduce breast carcinogenesis. However, epidemiologic studies regarding dairy products and breast cancer have yielded inconsistent results. We examined data from a large, long-term cohort study to evaluate whether high intake of dairy products, calcium, or vitamin D is associated with reduced risk of breast cancer.Methods: We followed 88 691 women in the Nurses' Health Study cohort from the date of return of their food-frequency questionnaire in 1980 until May 31, 1996. Dietary information was collected in 1980 and updated in 1984, 1986, 1990, and 1994. We identified 3482 women (premenopausal = 827, postmenopausal = 2345, and uncertain menopausal status = 310) with incident invasive breast cancer. We used pooled logistic regression to estimate multivariable relative risks (RRs) using 2-year time increments. The RRs and 95% confidence intervals (CIs) were calculated for each category of intake compared with the lowest intake group. All statistical tests were two-sided.Results: Intakes of dairy products, calcium, or vitamin D were not statistically significantly associated with breast cancer risk in postmenopausal women. In premenopausal women, however, consumption of dairy products, especially of low-fat dairy foods and skim/low-fat milk, was inversely associated with risk of breast cancer. The multivariable RRs comparing highest (>1 serving/day) and lowest (800 mg/day versus 500 IU/day versus Conclusions: We found no association between intake of dairy products and breast cancer in postmenopausal women. Abstract The X-ray-bright Seyfert 1 galaxy III Zw 2 was observed with XMM–Newton in 2000 July. Its X-ray spectrum can be described by a power law of photon index Γ= 1.7 and an extremely broad (FWHM∼ 140 000 km s[sup -1] ) Fe Kα line at 6.44 keV. The iron line has an equivalent width of ∼800 eV. To study the long-term X-ray behaviour of the source we have analysed 25 yr of data, from 1975 to 2000. There is no evidence of significant intrinsic absorption within the source or of a soft X-ray excess in the XMM or archival data. We do not detect rapid X-ray variability (a few × 10[sup 3] s ) during any of the individual observations; however, on longer time-scales (a few years) the X-ray light curve shows 10-fold flux variations. We infer a black hole mass of ∼10[sup 9] M[sub ⊙] (from Hβ FWHM) for III Zw 2 which is much higher than some previous estimates. A comparison of X-ray variability with light curves at other wavelengths over a 25-yr period reveals correlated flux variations from radio to X-ray wavelengths. We interpret the variable radio to optical emission as synchrotron radiation, self-absorbed in the radio/millimetre region, and the X-rays mainly as a result of Compton up-scattering of low-energy photons by the population of high-energy electrons that give rise to the synchrotron radiation. Background: Calcium has been hypothesized to reduce the risk of colon cancer, and in a recent randomized trial, calcium supplementation was associated with reduction in the risk of recurrent colorectal adenomas. We examined the association between calcium intake and colon cancer risk in two prospective cohorts, the Nurses' Health Study (NHS) and the Health Professionals Follow-up Study (HPFS).Methods: Our study population included 87 998 women in NHS and 47 344 men in HPFS who, at baseline (1980 for NHS and 1986 for HPFS), completed a food frequency questionnaire and provided information on medical history and lifestyle factors. Dietary information was updated at least every 4 years. During the follow-up period (1980 to May 31, 1996 for the NHS cohort; 1986 to January 31, 1996 for the HPFS cohort), 626 and 399 colon cancer cases were identified in women and men, respectively. Pooled logistic regression was used to estimate relative risks (RRs), and all statistical tests were two-sided.Results: In women and men considered together, we found an inverse association between higher total calcium intake (>1250 mg/day versus < or =500 mg/day) and distal colon cancer (women: multivariate RR = 0.73, 95% confidence interval [CI] = 0.41 to 1.27; men: RR = 0.58, 95% CI = 0.32 to 1.05; pooled RR = 0.65, 95% CI = 0.43 to 0.98). No such association was found for proximal colon cancer (women: RR = 1.28, 95% CI = 0.75 to 2.16; men: RR = 0.92, 95% CI = 0.45 to 1.87; pooled RR = 1.14, 95% CI = 0.72 to 1.81). The incremental benefit of additional calcium intake beyond approximately 700 mg/day appeared to be minimal.Conclusions: Higher calcium intake is associated with a reduced risk of distal colon cancer. The observed risk pattern was consistent with a threshold effect, suggesting that calcium intake beyond moderate levels may not be associated with a further risk reduction. Using a parametrized function for the mass loss at the base of the post-shock region, we have constructed a formulation for magnetically confined accretion flows which avoids singularities, such as the infinity in density, at the base associated with all previous formulations. With the further inclusion of a term allowing for the heat input into the base from the accreting white dwarf, we are also able to obtain the hydrodynamic variables to match the conditions in the stellar atmosphere. (We do not, however, carry out a mutually consistent analysis for the match.) Changes to the emitted X-ray spectra are negligible unless the thickness of mass leakage region at the base approaches or exceeds one per cent of the height of the post-shock region. In this case the predicted spectra from higher-mass white dwarfs will be harder, and fits to X-ray data will predict lower white dwarf masses than previous formulations. [ABSTRACT FROM AUTHOR]

Sharp dips observed in the pulse profiles of three X-ray pulsars (GX 1+4, RX J0812.4-3114 and A 0535+26) have previously been suggested to arise from partial eclipses of the emission region by the accretion column occurring once each rotation period. We present pulse-phase spectroscopy from Rossi X-ray Timing Explorer satellite observations of GX 1+4 and RX J0812.4-3114, which for the first time confirms this interpretation. The dip phase corresponds to the closest approach of the column axis to the line of sight, and the additional optical depth of photons escaping from the column in this direction gives rise to both the decrease in flux and increase in the fitted optical depth measured at this phase. Analysis of the arrival time of individual dips in GX 1+4 provides the first measurement of azimuthal wandering of a neutron star accretion column. The column longitude varies stochastically with a standard deviation ranging between 2° and 6° depending on the source luminosity. Measurements of the phase width of the dip both from mean pulse profiles and from the individual eclipses demonstrate that the dip width is proportional to the flux. The variation is consistent with that expected if the azimuthal extent of the accretion column depends only upon the Keplerian velocity at the inner disc radius, which varies as a consequence of the accretion rate M·. We present a general formulation for stability analyses of radiative shocks with multiple cooling processes, longitudinal and transverse perturbations, and unequal electron and ion temperatures. Using the accretion shocks of magnetic cataclysmic variables as an illustrative application, we investigate the shock instabilities by examining the eigenfunctions of the perturbed hydrodynamic variables. We also investigate the effects of varying the condition at the lower boundary of the post-shock flow from a zero-velocity fixed wall to several alternative types of boundaries involving the perturbed hydrodynamic variables, and the variations of the emission from the post-shock flow under different modes of oscillations. We found that the stability properties for flow with a stationary-wall lower boundary are not significantly affected by perturbing the lower boundary condition, and they are determined mainly by the energy-transport processes. Moreover, there is no obvious correlation between the amplitude or phase of the luminosity response and the stability properties of the system. Stability of the system can, however, be modified in the presence of transverse perturbation. The luminosity responses are also altered by transverse perturbation. [ABSTRACT FROM AUTHOR]

Background Atopic dermatitis (AD) is a chronic inflammatory skin disease expressed early in life. Disease development is primarily determined by as yet unknown genetic factors, leading to the accumulation of activated T lymphocytes in the skin.Objectives To investigate the nature of these T cells.Methods T-cell lines could be established from AD skin biopsies, but not from normal skin or AD peripheral blood, when placed in RPMI 1640 medium with 10% human AB serum, antibiotics, and the T-lymphocyte growth factors interleukins 2 and 4. The cell lines were subjected to phenotypic analysis using a fluorescence-activated cell sorter and compared with lymphocytes from AD and normal control peripheral blood.Results T-cell lines from 22 of 24 consecutive skin biopsies taken from 24 adult patients with AD were established. All cells were T lymphocytes expressing several activation markers. A significant proportion of the lymphocytes had stable expression of a CD4+ CD8+ phenotype (26% ± 6%; mean ± SEM). Such double-positive T lymphocytes are normally only seen in the thymus and not in the peripheral immune system. CD4+ CD8+ cells in peripheral blood of the patients (12·5% ± 3·3%) were also detected.Conclusions We suggest that a basic pathophysiological change in AD may be a faulty maturation of the T-lymphocyte system, leading to skin inflammation with CD4+ CD8+ T lymphocytes resembling immature T cells. We have detected the optical counterpart of the proposed double degenerate polar RX J1914+24. The I-band light curve is modulated on the 9.5-min period seen in X-rays. There is no evidence for any other periods. No significant modulation is seen in J. The infrared colours of RX J1914+24 are not consistent with a main-sequence dwarf secondary star. Our ASCA spectrum of RX J1914+24 is typical of a heavily absorbed polar and our ASCA light curve also shows only the 9.5-min period. We find that the folded I band and X-ray light curves are out of phase. We attribute the I-band flux to the irradiated face of the donor star. The long-term X-ray light curve shows a variation in the observed flux of up to an order of magnitude. These observations strengthen the view that RX J1914+24 is indeed the first double degenerate polar to be detected. The structure of the hot downstream region below a radiative accretion shock, such as that of an accreting compact object, may oscillate because of a global thermal instability. The oscillatory behaviour depends on the functional forms of the cooling processes, the energy exchanges of electrons and ions in the shock-heated matter, and the boundary conditions. We analyse the stability of a shock with unequal electron and ion temperatures, where the cooling consists of thermal bremsstrahlung radiation which promotes instability, plus a competing process which tends to stabilize the shock. The effect of transverse perturbations is considered also. As an illustration, we study the special case in which the stabilizing cooling process is of order 3/20 in density and 5/2 in temperature, which is an approximation for the effects of cyclotron cooling in magnetic cataclysmic variables. We vary the efficiency of the second cooling process, the strength of the electron–ion exchange and the ratio of electron and ion pressures at the shock, to examine particular effects on the stability properties and frequencies of oscillation modes. We carried out spectroscopic observations of the candidate black hole binary GX 339-4 during its low–hard and high–soft X-ray states. We have found that the spectrum is dominated by emission lines of neutral elements with asymmetric, round-topped profiles in the low–hard state. In the high–soft state, however, the emission lines from both neutral and ionized elements have unambiguously resolved double-peaked profiles. The detection of double-peaked emission lines in the high–soft state, with a larger peak separation for higher ionization lines, indicates the presence of an irradiatively heated accretion disc. The round-topped lines in the low–hard state are probably caused by a dense matter outflow from an inflated non-Keplerian accretion disc. Our data do not show velocity modulations of the line centres caused by the orbital motion of the compact object, neither do the line basewidths show substantial variations in each observational epoch. There are no detectable absorption lines from the companion star. All these features are consistent with those of a system with a low-mass companion star and low orbital inclination. We carried out I, R, V and B photometric observations of the neutron star X-ray binary RXTE J2123-058 shortly after the end of the X-ray outburst in mid-1998. We adopt the low-mass binary model to interpret our observations. After folding our data on the 0.24 821-d orbital period, and correcting for the steady brightness decline following the outburst, we observed sinusoidal oscillations with hints of ellipsoidal modulations which became progressively more evident. Our data also show that the decline in brightness was faster in the V band than in the R and I bands. This suggests both the cooling of an irradiation-heated secondary star and the fading of an accretion disc over the nights of our observations. [ABSTRACT FROM AUTHOR]

ABSTRACT We calculate the temperature and density structure of the hot post-shock plasma in magnetically confined accretion flows, including the gravitational potential. This avoids the inconsistency of previous calculations which assume that the height of the shock is negligible. We assume a stratified accretion column with 1D flow along the symmetry axis. We find that the calculations predict a lower shock temperature than previous calculations, with a flatter temperature profile with height. We revise previous determinations of the masses of the white dwarf primary stars, and find that for higher mass white dwarfs there is a general reduction in derived masses when the gravitational potential is included. We present new optical and infrared (IR) observations of Cir X-1 taken near apastron. Both sets of spectra show asymmetric emission lines. Archival optical observations show that an asymmetric Hα emission line has been in evidence for the past 20 years, although the shape of the line has changed significantly. We present an eccentric (e∼0.7–0.9) low-mass binary model, where the system consists of a neutron star orbiting around a (sub)giant companion star of 3–5 M[sub ⊙]. We suggest that the broad components of the emission lines arise in a high-velocity, optically thick flow near the neutron star, while the narrow components of the optical and the IR lines arise near the companion star and a heated ejecta shell surrounding the binary respectively. In this model, the velocity of the narrow component reflects the space velocity of the binary; the implied radial velocity (+430 km s[sup -1] after correcting for Galactic rotation) is the highest velocity known for an X-ray binary.
