Background Antiplatelet therapy has been shown to reduce adverse cardiovascular (CV) events in patients with PAD, but despite the well-established medication benefit, large-scale epidemiological data evaluating antiplatelet agent of choice with low bleeding risk and high antithrombotic effect is sparse. Purpose We explored Danish healthcare registers to examine guideline-recommended antiplatelet therapy and associated risk of adverse CV outcomes in patients with PAD. Methods All patients with a first-time diagnosis of PAD between January 1, 1997 and December 31, 2016 were identified. Patients alive six months after diagnosis were divided into four treatment groups: aspirin, clopidogrel, dual therapy (DAPT), and no antiplatelet therapy. Logistic regression models were used to estimate 1-year risk of myocardial infarction (MI), stroke, CV death, all-cause mortality, and gastrointestinal (GI) bleeding. Results We identified a total of 85,771 PAD patients [median age 70.6 (IQR 63–77 years), 52% male]. The results suggest that compared to no antiplatelet therapy, use of clopidogrel alone significantly decreased 1-year risk of incident MI, stroke, CV death and all-cause mortality with odds ratios 0.56 (CI 0.46–0.68), 0.69 (CI 0.59–0.81), 0.72 (CI 0.56–0.94), and 0.79 (CI 0.65–0.97) respectively. Likewise, aspirin monotherapy was associated with only a marginal reduction in 1-year risk of stroke by 11% and all-cause mortality by 7%. Odds ratios for GI bleeding were estimated to be 1.18 (CI 1.04–1.34), 1.34 (CI 0.94–1.91), and 1.54 (CI 1.05–2.26) for aspirin, clopidogrel, and DAPT groups, respectively. Conclusion In this nationwide study, clopidogrel is associated with significantly reduced risk of MI, stroke, CV death and all-cause mortality with low risk of bleeding and appears to support clopidogrel as antiplatelet agent of choice for CV risk reduction in patients with PAD. Background Acute identification and treatment of myocardial infarction (MI) is crucial for survival. MI symptom presentation is typically characterized by chest pain. Regular use of opioids to relief pain of chronic diseases is common. However, it has not been investigated whether opioid administered prior to onset of MI is associated more with atypical presentation of ischemia compared to non-opioid users. Purpose To investigate if opioid therapy is associated with atypical symptoms of MI. Methods The study is registry-based. MI is defined as patients with the ICD-10 discharge diagnoses I20.0 and I21 as primary diagnosis Results In total, 866 MI patients with opioid treatment and 10,061 MI patients without opioid treatment were included. The median age was 72.0 vs 67.8 years, 50.5% vs 34.5% were women, 61.0% vs 54.7% called the Emergency Number (1–1-2), ischemic heart disease was present in 48.5% vs 34.3% and type 2 diabetes in 23.1% vs. 16.2%. Atypical symptoms of MI were associated with opioid treatment and most prevalent in calls to OOHS, in women and by higher age. Acute symptoms of MI are visualized in Figure 1 and 2. Conclusion Opioid treatment prior to onset of MI was associated with increased atypical symptoms of MI, especially dyspnea, and increased in calls to the OOHS, in women and by higher age. This might challenge the clinician in early diagnose of MI in patients with opioid treatment and thereby delay acute lifesaving treatment. Funding Acknowledgement Type of funding sources: Foundation. We study a hundred of galaxies from the spectroscopic Sloan Digital Sky Survey with individual detections in the Far-Infrared Herschel PACS bands (100 or 160 $\mu$m) and in the GALEX Far-UltraViolet band up to z$\sim$0.4 in the COSMOS and Lockman Hole fields. The galaxies are divided into 4 spectral and 4 morphological types. For the star forming and unclassifiable galaxies we calculate dust extinctions from the UV slope, the H$\alpha$/H$\beta$ ratio and the $L_{\rm IR}/L_{\rm UV}$ ratio. There is a tight correlation between the dust extinction and both $L_{\rm IR}$ and metallicity. We calculate SFR$_{total}$ and compare it with other SFR estimates (H$\alpha$, UV, SDSS) finding a very good agreement between them with smaller dispersions than typical SFR uncertainties. We study the effect of mass and metallicity, finding that it is only significant at high masses for SFR$_{H\alpha}$. For the AGN and composite galaxies we find a tight correlation between SFR and L$_{IR}$ ($\sigma\sim$0.29), while the dispersion in the SFR - L$_{UV}$ relation is larger ($\sigma\sim$0.57). We study the dust properties of galaxies in the redshift range 0.1 0.5 with L_IR>5x10^{10} L_\odot, appears to be 2-5 K colder than that of AKARI-selected local galaxies with similar luminosities; and the dispersion in T_dust for high-z galaxies increases with L_IR due to the existence of cold galaxies that are not seen among local galaxies. We show that this large dispersion of the L_IR-T_dust relation can bridge the gap between local star-forming galaxies and high-z submillimeter galaxies (SMGs). Background. Diabetes mellitus and its complications are a public health problem of epidemic proportions. Both diabetes and chronic kidney disease (CKD) increase the risk of acute kidney injury (AKI). Months after a single episode of acute ischaemia to the diabetic kidney, we have found an accelerated progression of nephropathy, with impaired function, severe renal inflammation, microvascular dysfunction, fibrosis and apoptotic cell death. We termed this entity the post-ischaemic inflammatory syndrome. We now test the hypothesis that blocking inflammation ameliorates the post-ischaemic inflammatory syndrome. Methods. Obese–diabetic ZS rats (F1 hybrids of spontaneously hypertensive heart failure and Zucker fatty diabetic rats) were treated with mycophenolate mofetil (MMF), subjected to renal ischaemia or sham surgery, and monitored via the powerful technique of intravital microscopy. Results. Amelioration of post-ischaemia inflammation with MMF therapy improved long-term renal function, microvascular dysfunction, fibrosis and apoptosis. Conclusion. These data support the hypothesis that the post-ischaemic inflammatory syndrome accelerates diabetic CKD, is a critical determinant of injury, and can be successfully treated.
