1. 197. A Predictive Model for the Prompt Identification of Suspected Sepsis Patients with High Death Risk

Abstract

Background How to improve the high mortality rate of sepsis? The prompt identification of at-risk patients, and the interdisciplinary sepsis treatment protocol implementation are interventions that can reverse such unacceptable outcome. The objective of our study is to summarize main results of the protocol for the management of sepsis and septic shock, implemented at Biocor Instituto, a general hospital in Belo Horizonte, a 3,000,000 inhabitants city from Brazil. Methods Prospective cohort study of patients with sepsis, evaluated between May/2018-Apr/2020. Univariate and multivariate analysis by logistic regression to identify risk factors for hospital death. Results Over 28 months, 220 patients were included in sepsis protocol: 121 hospital deaths, a crude mortality = 121/220 = 55% (95%C.I. = [48%;62%]). 136 patients (62%) came from the emergency room. In 97 cases (44%) it was possible to isolate 111 microorganisms, with a predominance of Klebsiella, E.coli, and S.aureus. 75% of the cases (165) had definition of APACHE, with the absolute majority of these (88%) having APACHE between 25 and 40. Most patients (52%) received antibiotic (ATB) in 15 minutes and only 4% received ATB after 60 minutes of waiting time. In 198 patients (90%) it was possible to identify the focus of sepsis, with a predominance of pulmonary (47%), urinary (21%) and abdominal (15%). Hospital mortality varied from 30 to 62%, when the focus was pulmonary (p-value = 0.045). In univariate analysis (Figure 1), pulmonary sepsis, creatinine, lactate, and APACHE were significantly associated with hospital death. The time for ATB administration was typically close to 20 minutes, and time to receive the therapeutic antibiotic were not associated with the patient’s death. By using the logistic model (Figure 2) to assign cases of predicted hospital death for probabilities >= 0.5 and controls for probabilities < 0.5, the prediction model had a sensitivity of 0.68 (0.59–0.76), a specificity of 0.58 (0.48–0.67), an area under the curve of the receiver operating characteristic curve of 0.75 (0.68–0.82). 2. Child sleep: assessment of the knowledge of parents of children aged zero to two years

Abstract

Changes in sleep behavior are considered a public health problem, since they interfere with the health of the individual, especially in early childhood, and can impair growth and development. The objective of this study was to evaluate the knowledge about children's sleep in parents of children aged 0 to 24 months. Quasi-experimental study, of the type before and after, with a single group, developed in a pediatric outpatient clinic in northeastern Brazil. Convenience sample of 60 parents of children discharged from the neonatal unit, who represent preliminary data from a larger study. Data collected between November / 2019 and January / 2020, using the CAP survey (Knowledge, Attitude and Practice). A child sleep instrument was used in the pre-test (first time with the parents) and post-test (15 days after the first application). After the pre-test, the parents read an educational booklet containing the information to which they were asked. The knowledge about sleep was assessed including, time, place and sleeping clothes, position in the crib, prevention of sudden infant death syndrome and total sleep time. Scores ≤ 3 represented inadequate knowledge; 4-7 adequate knowledge. Only 5 parents had inadequate knowledge in the pre-test. In the post-test, everyone demonstrated adequate knowledge. The average of points in the pre-test was 4.3, median of 4. In the post-test, the average of points (6.15), median (6). In the pre-test, less correct questions were total sleep time, adequate sleeping position, with 12 and 20 errors, respectively. After the application of the booklet, the question regarding the total sleep time remained with an equal number of errors, however only 7 parents were wrong about the position of the child at the time of sleep. It is concluded that there is a need for guidance to parents on sleep, and it is pertinent to evaluate their previous knowledge, contributing to the strengthening of public policies aimed at children's sleep. Key messages Parents should receive guidance on infant sleep hygiene. Strengthening the promotion of children's sleep by health professionals.

Abstract

Currently, authorities and specialists in child development and public health have been concerned with the growth and nutritional status of children, especially due to the uncontrollable increase in overweight / obesity that has occurred in recent years. Obesity causes organic-functional complications, such as chronic-degenerative diseases (diabetes, high blood pressure, coronary heart disease), and psychosocial complications, such as withdrawal from social relationships, shame, exclusion and disturbances in body awareness. The objective was to identify in the literature the parents' knowledge about complementary feeding for infants. Integrative literature review based on the guidelines recommended by PRISMA. The search was carried out between the months of May and June 2017 by two researchers independently in the Scopus, PubMed, Scielo, LILACS and CINAHL databases. The following descriptors were used: “infant”, “complementary feeding”, “parents” and “knowledge” and “infant”, “complementary feeding”, “relatives” and “knowledge” in the DECs and MESH respectively. 1,012 studies were identified and a sample of 06 articles was obtained after applying the eligibility criteria. The knowledge evidenced was: use of appropriate utensils for feeding the child, not using cow's milk, caution with processed foods, issues related to breastfeeding and age of introduction of food and other liquids. Even so, there were still mistakes by parents regarding the recommendations of the main bodies dealing with food, such as the use of processed foods from the fourth month of life of the child and the relationship of the “fat” baby being the health model. It was concluded that the knowledge of parents / caregivers about complementary feeding is still permeated by mistakes. Thus, the nursing consultation should focus on health education and search for strategies that facilitate the process of introducing complementary feeding and help parents in this period. Key messages Targeting health strategies on complementary feeding in a timely manner. Guidance for the prevention of obesity in early childhood and the development of non-infectious diseases.

4. The Functioning of Outreach Programs Of/On the Street and the Work Processes of the Nurse

Abstract

Problem Homeless people (PSR) are a heterogeneous and fragile group. They are among the most excluded ones from health service access because they cannot present a regular home. They are also a population who have great difficulty of accessing the entrance and mostly search only for emergency and urgency services when their lives are at stake. This intervention is important because it aims at decreasing the inequalities to health access and, moreover, it decreases the difficulties to health access. Description of the problem A field visit to an outreach program on the street (CR) in Belo Horizonte was made on April, 22nd and on May, 6th in 2019. This program is linked to a Health Center. The objectives were knowing the functioning of outreach programs on the street and understanding the assignments and participation of the team nurse on working with PSR. Results By acting with and approaching PSR, the nurse can create contact and humanized listening. This is an attempt to establish user / professional bonds. During this contact, the professional struggles to encourage the user to search the health attention net. This professional also approaches the rupture with home, with the family, with the work, and with others. He/She tries to support users to face the challenges and to stimulate social reinsertion as well. Tests on the street to detect HIV/AIDS and syphilis are made, guaranteeing the individual privacy right. By approaching PSR, it is crucial that the nurse, who is part of the multi-professional team, be free of any prejudice and stigmas which can make the attendance success difficult. The application of health expanded concept was also present because it considers the complexity of health, mainly to those who inhabit the streets. Lessons The nurse can succeed in facing inequity. This decreases not only the barriers established by social inequalities but also the organizational barriers PSR are exposed all the time. Key messages Attendance prioritizes local care. It not only tries to attend social and health problems but also shared actions with Health Centers. At last, it should be an example to other countries. It is important for health professionals to reinvent their practices, searching partnerships and the acquisition of new knowledge to succeed in decreasing individuals’ demands.

Abstract

Salinity may limit plant growth especially in arid and semiarid regions. Arbuscular mycorrhizal fungi (AMF) and the supply of inorganic phosphorus (Pi) could alleviate the negative effects of such stress by improvement in stomatal conductance, photosynthesis and biomass. The aim of this study is to evaluate the ecophysiological performance of Cenostigma pyramidale (Tul.) E. Gagnon & G. P. Lewis (Fabaceae) in a greenhouse under salinity conditions in combination with the supply of AMF and leaf Pi. The experiment was conducted in a factorial design considering two levels of salinity (+NaCl and -NaCl), two levels of AMF (+AMF and -AMF) and two levels of leaf Pi supply (+Pi and -Pi). The variables gas exchange, leaf primary metabolism, dry biomass and nutrients were measured. Plants with AMF under non-saline conditions presented a high photosynthesis and biomass. In saline conditions, AMF promoted lower decrease in photosynthesis, high shoot dry matter and low content of leaf and root Na+ and Cl-. Plants treated with leaf Pi increased biomass and photosynthetic pigments under both conditions and accumulated more Cl- in shoots under salinity conditions. When combined, AMF * Pi increased photosynthesis only in non-saline conditions. Plants under salinity conditions without AMF and Pi had higher decreases in gas exchange and high content of Cl- in roots. Therefore, C. pyramidale plants improved their metabolism under both growth conditions in the presence of AMF, Pi or a combination of both. However, the greatest increases in growth and tolerance to salinity occurred in the isolated presence of AMF.

Abstract

Phenotypes relevant to oxidative phosphorylation (OXPHOS) in eukaryotes are jointly determined by nuclear and mitochondrial DNA (mtDNA). Thus, in humans, the variable clinical presentations of mitochondrial disease patients bearing the same primary mutation, whether in nuclear or mitochondrial DNA, have been attributed to putative genetic determinants carried in the “other” genome, though their identity and the molecular mechanism(s) by which they might act remain elusive. Here we demonstrate cytoplasmic suppression of the mitochondrial disease-like phenotype of the Drosophila melanogaster nuclear mutant tko25t, which includes developmental delay, seizure sensitivity, and defective male courtship. The tko25t strain carries a mutation in a mitoribosomal protein gene, causing OXPHOS deficiency due to defective intramitochondrial protein synthesis. Phenotypic suppression was associated with increased mtDNA copy number and increased mitochondrial biogenesis, as measured by the expression levels of porin voltage dependent anion channel and Spargel (PGC1α). Ubiquitous overexpression of Spargel in tko25t flies phenocopied the suppressor, identifying it as a key mechanistic target thereof. Suppressor-strain mtDNAs differed from related nonsuppressor strain mtDNAs by several coding-region polymorphisms and by length and sequence variation in the noncoding region (NCR), in which the origin of mtDNA replication is located. Cytoplasm from four of five originally Wolbachia-infected strains showed the same suppressor effect, whereas that from neither of two uninfected strains did so, suggesting that the stress of chronic Wolbachia infection may provide evolutionary selection for improved mitochondrial fitness under metabolic stress. Our findings provide a paradigm for understanding the role of mtDNA genotype in human disease.

Abstract

One of the most important seismic areas in Brazil is the northeastern region with earthquake activity occurring mainly in the exposed Precambrian basement around the Potiguar Mesozoic marginal basin. Since 1986, temporary seismographic networks have been deployed at many sites near the border of the Potiguar basin and also further inland. This instrumental effort has allowed a better understanding of the seismicity patterns and crustal stresses in the region.
