Background: Earlier studies have reported socioeconomic differences in coronary heart disease incidence and mortality and in coronary treatment, but less is known about outcomes of care. We examined trends in income group differences in outcomes of coronary revascularizations among Finnish residents in 1998-2010., Methods: First revascularizations for 45-84-year-old Finns were extracted from the Hospital Discharge Register in 1998-2009 and followed until 31 December 2010. Income was individually linked to them and adjusted for family size. We examined the risk of major adverse cardiac events (MACEs), coronary mortality and re-revascularization. We calculated age-standardized rates with direct method and Cox regression models., Results: Altogether 69 076 men and 27 498 women underwent revascularization during the study period. Among men [women] in the 1998 cohort, 41% [35%] suffered MACE during 29 days after the operation and 30% [28%] in the 2009 cohort. Myocardial infarction mortality within 1 year was 2% among both genders in both cohorts. Among men [women] 9% [14%] underwent revascularization within 1 year after the operation in 1998 and 12% [12%] in 2009. Controlling for age, co-morbidities, year, previous infarction and disease severity, an inverse income gradient was found in MACE incidence within 29 days and in coronary mortality. The excess MACE risk was 1.39 and excess mortality risk over 1.70 among both genders in the lowest income quintile. All income group differences remained stable from 1998 to 2010., Conclusions: In health care, more attention should be paid to prevention of adverse outcomes among persons with low socioeconomic position undergoing revascularization., (© The Author 2015. Background: The present study examines the role of six aspects of socio-economic status (income, occupational position, education, unemployment, living alone and type of residential area) in mortality among types 1 and 2 diabetic people in Finland. All-cause mortality and causes of deaths (for example, alcohol diseases) are assessed., Methods: People with diabetes aged 30-79 years in 2000-03 were identified from national registers. The data comprised 528 734 person-years and 18 841 deaths. Relative mortality risks were obtained from Poisson regression models., Results: Among type 1 diabetic men, mortality differences were largest for long-term unemployed (aged 30-64 years); RR 3.85 (3.00-4.94) compared with the employed, and for low (vs. high) income group; RR 1.96 (1.78-2.17). The findings were similar for type 2 men; RR 2.58 (2.16-3.09) for unemployment and 1.61 (1.53-1.69) for income. In type 1 diabetic women, largest differences were found according to unemployment; RR 3.32 (1.88-5.88) and education (lowest vs. highest education); RR 2.35 (1.84-3.00), but in type 2 diabetes, the strongest determinants were disposable income; RR 1.55 (1.44-1.66) and education; RR 1.50 (1.33-1.70). In most socio-economic determinants, relative differences were largest in deaths due to diabetes and alcohol diseases., Conclusions: Five aspects of socio-economic position were related to mortality among diabetic people in Finland. No systematic mortality differences were, however, found for type of municipality of residence. These findings together with the role of deaths from alcohol diseases and diabetes in mortality trends, indicate that different aspects of social disadvantage are important predictors of mortality among diabetic people.

Background: Earlier studies on gender and socioeconomic differences in the treatment of coronary heart disease have focused mainly on structural features of the health-care system on the one hand and on coronary patients' psychosocial characteristics on the other. The aim of this exploratory qualitative study was to trace key points in the pathways of treatment where patients' experience varies and which can contribute to gender and socioeconomic differences in treatment., Methods: The data consist of 30 interviews among persons diagnosed with or suspected to have coronary heart disease in the Health 2000, a representative cross-sectional interview and health examination survey. Purposive sampling was used to ensure variation in gender, socioeconomic status and disease severity. The data were analysed using qualitative content analysis., Results: Gender and socioeconomic differences were found in two key points in the pathways of treatment: doctor-patient interaction and the organization of primary care. The three features commonly distinguished in doctor-patient interaction, i.e. treatment decision-making, information exchange and interpersonal relationship, were all found to be relevant. A second key point was organization of primary care in terms of both access to examinations and care, and continuity of care., Conclusions: These results should sensitize us to gender and socioeconomic differences in coronary patients' problems in access to and continuity of care, as well as to potential problem areas in doctor-patient interaction.

Background: The study examines whether self-rated health forms a continuum from poor through average to good health in terms of two groups of health-related variables., Methods: The data come from the 1994 Finnish Survey on Living Conditions, a representative sample of Finnish men and women aged 25 years or older (n = 7290). Logistic regression analysis was used with two dependent variables: (1) average versus good/excellent self-rated health and (2) poor versus good/excellent self-rated health. Two groups of independent variables were used: (1) risk factors and (2) ill-health indicators. Separate analyses were made for men and women controlling for sociodemographic background variables., Results: Of the risk factors, BMI and physical exercise were associated both with average and poor self-rated health whereas frequency of drinking was only associated with poor health. All used indicators of ill health were strongly associated with average as well as poor self-rated health. With the exception of BMI, the associations of both risk factors and ill health were stronger with poor than with average health., Conclusions: The study suggests that self-rated health forms a continuum from poor to good health when risk factors and indicators of ill health are considered, and that there are only minor differences in the continuity of self-rated health between men and women.
