STUDY DESIGN This was a systematic review study. PURPOSE This study aims to review current literature to determine the rates of complications in relation to spine surgery in ambulatory surgery centers (ASC). BACKGROUND Recent improvements in anesthesia, surgical techniques, and technological advances have facilitated a rise in the use of ASC. Despite the benefits and lower costs associated with ASCs, there is inconsistent reporting of complication rates. METHODS This systematic review was completed utilizing the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Pertinent studies were identified through Embase and PubMed databases using the search string ((("ambulatory surgery center") AND "spine surgery") AND "complications"). Articles were excluded if they did not report outpatient surgery in an ASC, did not define complications, were in a language other than English, were non-human studies, or if the articles were classified as reviews, book chapters, single case reports, or small case series (≤10 patients). The primary outcome was the frequency of complications with respect to various categories. RESULTS Our query identified 150 articles. After filtering relevance by title, abstract, and full text, 22 articles were included. After accounting for 2 studies that were conducted on the same study sample, a total of 11,245 patients were analyzed in this study. The most recent study reported results from May 2019. While 5 studies did not list their surgical technique, studies reported techniques including open (6), minimally invasive surgery (2), endoscopic (4), microsurgery (1), and combined techniques (4). The following rates of complications were determined: cardiac 0.29% (3/1027), vascular 0.25% (18/7116), pulmonary 0.60% (11/1839), gastrointestinal 1.12% (2/179), musculoskeletal/spine/operative 0.59% (24/4053), urologic 0.80% (2/250), transient neurological 0.67% (31/4616), persistent neurological 0.61% (9/1479), pain related 0.57% (20/3479), and wound site 0.68% (28/4092). CONCLUSIONS After literature review, this is the first study to comprehensively analyze the current state of literature reporting on the complication profile of all ASC spine surgery procedures. The most common complications were gastrointestinal (1.12%) and the most infrequent were vascular (0.25%). Case reports varied significantly with regard to the type and rate of complications reported. This study provides complication profiles to assist surgeons in counseling patients on the most realistic expectations.

Background Reconstructive choices for the defects of the sacral and ischial regions include various steps of the reconstructive ladder from primary closure to free flaps. This study aimed to present repair of sacral and ischial region defects with lateral sacral artery perforator (LSAP) flaps. Methods We enrolled a total of 18 patients with sacral and ischial region defects reconstructed with LSAP flaps in the study between September 2014 and October 2015. The patients were evaluated in terms of age, sex, neurological situation, etiology, defect size, defect region, flap size, perforator number, and postoperative complications. Results No patient had hematoma, seroma, and complications of the donor area. In the postoperative period, 2 patients were observed to have short-term complications (11.1%) including a partial flap loss (5.5%) due to distal venous failure (flap survival rate, 95%) and a wound site infection (5.5%). Duration of follow-up of the patients ranged between 26 and 38 months. Recurrence was observed from long-term complications of 4 patients' pressure sores (22%). Conclusions The LSAP flap has not been frequently described in the literature. We believe that LSAP flap is a flap of choice that should be considered preferably for sacral and ischial defects, which can be reliably elevated over pedicles and has short surgery duration and low surgical morbidity.

Background Cesarean delivery (CD) is a commonly performed obstetric surgical procedure and causes moderate to severe postoperative pain. Wound site infiltration (WSI) is becoming a technique to provide postoperative analgesia in a limited-resource setting in regardless of controversy on its effectiveness. The current study is to assess its effectiveness as a part of postoperative analgesia for parturients undergoing elective Cesarean section. Methods A Hospital-based prospective cohort study was employed on 58 parturients that underwent elective Cesarean section. Study participants were allocated into the Wound site infiltration and Control group based on planned postoperative pain management. A student t-test was used for normally distributed data while non-normally distributed data were analyzed by Mann Whitney U test. Pearson Chi-squared or Fisher's exact test were used to analyzing categorical data as appropriate. A p-value < 0.05 considered as statistically significant. Results The median time to request the first analgesia was significantly prolonged within Wound site infiltration 314.31 ± 47.71 in minutes compared to control group 216.9 ± 43.18 with a P-value of, Highlights • WSI might prolong duration of analgesia for cesarean section. • WSI might decrease postoperative pain severity for cesarean section. • WSI might reduce total analgesic consumption within 24 h in postoperatively.

The process of wound healing is often accompanied by bacterial infection or critical colonization, resulting in protracted inflammation, delayed reepithelization, and production of pungent odors. The malodor produced by these wounds may lower health-related quality of life and produce psychological discomfort and social isolation. Current management focuses on reducing bacterial activity within the wound site and absorbing malodorous gases. For example, charcoal-based materials have been incorporated into dressing for direct adsorption of the responsible gases. In addition, multiple topical agents, including silver, iodine, honey, sugar, and essential oils, have been suggested for incorporation into dressings in an attempt to control the underlying bacterial infection. This review describes options for controlling malodor in chronic wounds, the benefits and drawbacks of each topical agent, and their mode of action. We also discuss the use of subjective odor evaluation techniques to assess the efficacy of odor-controlling therapies. The perspectives of employing novel biomaterials and technologies for wound odor management are also presented.

This paper presents the analysis of urine bioassay data, spanning four decades, from five workers who had wounds contaminated with plutonium at the Department of Energy Rocky Flats Plant during the period 1961-1967. The cases were selected from participants in the Department of Energy-sponsored Former Radiation Worker Medical Surveillance Program at Rocky Flats, which provided medical monitoring, modern bioassay measurements, and internal dose re-evaluations for former Rocky Flats workers. The cases include a variety of wound types, excision treatment regimes, and monitoring information. These wound cases illustrate the use of two multi-compartment wound models and three plutonium urine excretion models for retrospective calculation of internal plutonium depositions resulting from wounds for which no chelation therapy was administered. Wound model compartment fractions and half times are determined for each case and urine excretion model as are composite parameter values. The urine analysis and wound count measurements obtained under the program provide data with state-of-the art measurement sensitivity, as well as the opportunity to include long-term excretion and wound site data that exceed 10,000 d post-exposure for retrospective intake and dose evaluations. These data are provided to the radiation dosimetry community for use in developing and testing improved models for plutonium deposition in wounds.

Background. Research into the pathophysiology of photodamaged skin has revealed correlations with certain aspects of acute and chronic wound healing. In wound healing, growth factors accumulate at the wound site and interact synergistically to initiate and coordinate wound healing. Objectives. To review the effects of topical growth factors in wound healing and their potential use in treating photodamaged skin. Methods. A mixture of multiple growth factors derived from human fibroblasts was applied topically to 14 patients twice daily for 60 days to stimulate the remodeling phase of wound healing. Results. A total of 78.6% of patients with photodamaged skin showed clinical improvement at 60 days. New collagen formation increased by 37%, and epidermal thickening increased by 27%. Conclusions. These data, as well as other studies of growth factors in wound healing, demonstrate positive cosmetic and clinical outcomes of topical application of growth factors for the treatment of photodamaged skin. BACKGROUND The management of postostomy takedown surgical wound sites can be challenging. Complications from these contaminated wounds can lead to serious complications such as hernia formation and increased healthcare costs. Negative pressure wound therapy (NPWT) has been shown to be potentially helpful in managing these heavily colonized wound sites. We report the case of a mechanically powered ambulatory NPWT device (SNaP Wound Care System; Spiracur Inc, Sunnyvale, California) for treating these postcolostomy takedown wounds. CASE A young 9-year-old boy in Port-Au-Prince, Haiti, had under gone colostomy as a protective measure after pelvic fracture 5 months prior. Having healed the pelvic fracture and being fully ambulatory, he underwent takedown of his colostomy with reanastomosis of the bowel. At the completion of surgery, the ostomy wound site was managed by a mechanically powered NPWT device. This allowed the patient to remain ambulatory without the need for attachment to a heavier electrically powered NPWT device during healing. Dressing changes were limited to twice weekly instead of 3 times daily. CONCLUSION This case demonstrates the feasibility of an underdescribed application for a new mechanically powered ambulatory negative pressure device. Findings from this case study suggest that this device may be clinically applicable for patients undergoing ostomy takedown in the United States and in developing nations such as Haiti.

Split-thickness skin grafts remain central to the strategy of burn wound treatment. The dressing used to cover the donor wound site has a significant effect on healing parameters. The purpose of this study was to compare split-thickness skin graft donor site reepithelialization under Xeroform and Jelonet dressings. A dermatome was used to cut two consecutive strips of skin from 25 paired donor sites on the thigh, calf, or back of 19 participants. Standardization of the harvest method was achieved by using the same surgeon to harvest the compared skin graft strips, with attention to consistency of dermatome skin-thickness setting, downward pressure, and angle of dermatome approach. A strip of Xeroform or Jelonet was applied to one of each pair of wounds. Epidermal and dermal thickness was measured from biopsy specimens cut at the midpoint of each split-thickness graft strip. The day of final dressing separation was declared the day of complete donor reepithelialization (healing). The mean healing time for Xeroform and Jelonet was 10.4 +/- 2.6 days (n = 25) and 10.6 +/- 2.8 days (n = 25) (p = 0.76) at sites cut to a mean depth of 0.23 +/- 0.08 mm and 0.23 +/- 0.09 mm (p = 0.89), respectively. There was no correlation between graft thickness and healing time for sites dressed with Xeroform (r = 0.17) or Jelonet (r = 0.02). Donors sites reharvested 10 to 21 days after a prior harvest healed an average of 3.1 days earlier than virgin sites (8.4 +/- 1.6 versus 11.5 +/- 2.6 days, p < 0.001), although reharvested grafts were on average 0.05 mm thicker (p = 0.10). The mean thickness of reepithelialized donor-site epidermis (0.13 +/- 0.04 mm, n = 30) was found to be twice the thickness of virgin epidermis from the same sites (0.06 +/- 0.02 mm, n = 38, p < 0.001). Thirty-six grafts harvested with dermatomes set to cut 8/1000 inch (0.20 mm) deep ranged from 0.12 to 0.42 mm thick, with only eight of these grafts measuring within +/-10 percent of the desired thickness setting. Before donor dressing separation, Xeroform and Jelonet dressings were judged to be more comfortable by nine patients and one patient, respectively, whereas no difference was detected by six patients. The authors now use Xeroform as the preferred donor dressing.

This paper describes the use of localized transient gene therapy for the augmentation of fracture healing. It introduces a method involving the delivery of plasmid deoxyribonucleic acid via a three dimensional matrix into a wound, with in vivo transfection of wound repair cells resulting, their subsequent expression of factors that condition the wound site and the promotion of healing. Based on experience with critical and noncritical defect models in small and large animals, the potential advantages of this approach are discussed and experimental evidence of promoting bone formation is provided. The studies show the potential of this technology not only to promote bone healing but also to repair or to regenerate other connective tissues.

UNLABELLED Intraoperative autotransfusion of scavenged blood is an established method to reduce the need for perioperative homologous blood transfusion. However, if fat particles contaminate blood suctioned from the wound site, no reliable method is available to remove them during the washing and concentration of the recycled blood. A new generation of autotransfusion devices (e.g., continuous autotransfusion system [CATS]), based on separation chambers used in cell separators or plasmapheresis devices, allows continuous procession of the collected blood, in contrast with the discontinuous process used in conventional autotransfusion devices such as the Cell Saver 5. Theoretically, the continuous system should be more efficient than the discontinuous system in eliminating fat. Outdated, 36-day-old packed red blood cells, 600 mL, were mixed with 500 mL of lactated Ringer's solution and 200 mL of soya oil. Soya oil was used because it has a fatty acid composition similar to that of fat found in bone marrow. The blood mixture was then washed and concentrated by using either the CATS or the Cell Saver 5. Six samples were processed by each device. The CATS eliminated the soya oil (200 mL) completely, whereas the Cell Saver 5 delivered 30.3 +/- 7.8 mL soya oil into the retransfusion bag. The new generation of autotransfusion devices allows complete removal of fat particles. IMPLICATIONS Autotransfusion devices serve to wash and retransfuse blood scavenged from the wound site. However, they cannot completely remove fat particles. This in vitro investigation showed that a new device completely removes fat particles and thus prevents retransfusion of fat.

Introduction: Although specimen extraction site selection for laparoscopic donor nephrectomy (LDN) is relatively flexible and is mostly selected by surgeons from the patient's standpoint, the patient's request may differ from the medical worker's recommendation. The cosmetic aspect may also differ with age, gender, and the extent of medical knowledge. We performed an unsigned questionnaire assessment of individual preferences for LDN wound sites. Materials and Methods: Between August 2007 and October 2008, we surveyed LDN wound site preferences among 148 physicians, 263 nurses, and 266 outpatients of urology at Akita University Hospital. They were questioned for their age, gender, occupation (medical worker or not), and for the most preferred surgical wound site among the following: A, lower vertical midline: B, upper vertical midline: C, anterior subcostal: D, Pfannenstiel: E, Gibson: and F, subcostal flank. Cross-linked, allogeneic, telopeptide-depleted dermal grafts were lyophilized and laminated with silicone rubber elastomer. Resultant bilayers were studied for incorporation into the wound site and capacity to inhibit cutaneous wound contraction in experimental animals. Bilateral full-thickness skin wounds were made in 20 male New Zealand white rabbits. One side was grafted with the processed graft, while the contralateral side remained ungrafted as a control wound. Over 63 days, wound sites were analyzed at intervals on the basis of the extent and rate of wound contraction and by histologic examination. Cutaneous wounds successfully incorporated graft matrix and were significantly inhibited in their rate and extent of wound contraction. Notably, by day 63, grafted wounds retained 71 percent of their original area, whereas ungrafted control wounds retained only 16 percent of their original area. There were no graft rejections, and the bilayer graft's dermal analogue appeared to function as a biodegradable template that physically conformed neodermis to a preestablished pattern while counteracting contractile forces. This investigation suggests that, in experimental animals, the success of bilayer dermal grafts is less dependent on highly specialized and complex preparative techniques than typically has been presumed and that relatively simple, previously published, nonproprietary techniques, when adapted to a bilayer format, yield acceptable results as defined in terms of biocompatibility, capacity for graft incorporation, and inhibition of wound contraction.

RATIONALE This patient was so debilitated that her heel ulcer was not expected to close. Her foot was cold, with poor capillary refill and an absent pedal pulse. Dressings were needed to prevent any infection from becoming systemic and to contain the wound exudate. Bordered polymeric membrane dressings conform well to heels and have a semipermeable membrane to protect wounds from contamination. They contain glycerol, a surfactant, and a super-absorbent starch. Polymeric membrane dressings’ ingredients draw and concentrate natural healing substances from the body into the wound bed to promote rapid healing while facilitating autolytic debridement directly by loosening the bonds between the slough and the wound bed. The liquefied slough is absorbed by the dressing, often eliminating the need for potentially damaging and painful manual wound bed cleansing during dressing changes. Together the dressing’s components reduce wound pain by inhibiting nociceptor activity at the wound site. In the absence of sufficient nutrients from the bloodstream, the glycerol in polymeric membrane dressings can be utilized by the body as an energy source and a component of new cells. The decision was made to use only silver or standard polymeric membrane dressings, depending upon the condition of the wound bed.

1. Arthroplasties of the first metatarsophalangeal joints in rabbits have been examined histologically after varying periods of immobilization and subsequent motion. 2. In this laboratory model, motion started one week after surgery consistently produced the best joint as evidenced by histological examination. 3. The proposition is presented that wound repair should be looked upon as an inflammatory response and that by proper control of local factors at the wound site it should be possible to direct the tissue response into channels leading to the formation of the desired mature tissue.

A 19-year-old man was shot in the right chest about 2 cm below the nipple; bloody fluid was drained through a chest tube. Two days later sero-sanguineous fluid tinged with bile was seen to drain from the wound site. 131I-rose bengal, injected intravenously, collected in the dome of the liver and drained into a collection bag placed over the wound site, thus identifying a hepato-pleural-cutaneous fistula, which was confirmed at surgery.

We have implanted cylinders (wound) of Edward's Teflon arterial graft in the panniculus of adult mongrel dogs of both sexes. We have noted the wound fluid and plasma zinc concentrations at varying intervals after cylinder implantation and recorded a rise in plasma and wound fluid zinc concentration post implant. There appears to be an inverse relationship of wound fluid pH and zinc concentration which is indicative of a metabolic mechanism for the concentration of zinc at, and the transport of zinc to a surgically inflicted wound site. We have not discounted the possibility of an electrochemical mechanism for zinc concentration and transport. We rather consider the metabolic/electrochemical activity at the wound site as being a prime factor in zinc transport. At present we are studying zinc concentrations in wounds with aerobic and anerobic metabolism.
