Abstract

PURPOSE OF REVIEW Dermatoses are one of the three most common healthcare problems in returning travelers. Knowledge of imported dermatoses among Western physicians is however limited. In order to provide more effective pretravel counseling and better posttravel diagnosis and treatment, improvement is called for. This review highlights the most recent developments regarding dermatological syndromes in returning travelers. RECENT FINDINGS The spectrum of travel-related dermatoses is broad, including tropical and cosmopolitan infections as well as environmental skin diseases. New infections such as Panton-Valentine leukocidin-positive Staphylococcus aureus infections have begun to emerge in returning travelers. Localized cutaneous leishmaniasis can be managed with multiple drug therapies, but precise identification of the species allows tailored therapeutic management. Clearer definitions of cutaneous larva migrans, a syndrome, and creeping eruption, a cutaneous sign, have been established lately whereas the term hookworm-related cutaneous larva migrans is more widely used to define the corresponding disease. Lastly, chikungunya has emerged as a cause of febrile exanthema among travelers returning from endemic areas and has to be distinguished from dengue and measles. SUMMARY Bacterial infections continue to be the main cause of consultation in returning travelers. Most of these infections are cosmopolitan and are secondary to arthropod exposure. Conversely, tropical skin infections are less commonly observed.

Abstract

Objective To evaluate the efficacy and safety of the foscarnet-ganciclovir combination in induction therapy (IT) and maintenance therapy (MT) for cytomegalovirus (CMV) central neurological disorders in HIV-infected patients. Design An open pilot non-comparative multicentre study. Methods Thirty-one patients with acute CMV encephalitis (CMVe) (n = 17) or CMV myelitis (CMVm) (n = 14) during the era before highly active antiretroviral therapy (HAART) received intravenous IT with foscarnet 90 mg/kg plus ganciclovir 5 mg/kg twice a day followed by MT. The primary endpoint was clinical efficacy, assessed at the end of the induction phase. Results The foscarnet-ganciclovir combination in IT resulted in a 74% (23 out of 31 patients) clinical improvement or stabilization. Eight patients did not respond clinically. Side-effects leading to drug discontinuation occurred in 10 patients during IT. Among the 23 patients who qualified for the maintenance phase, CMV disease progressed in 10, with a median time to the first relapse of 126 days (range 64-264 days). Overall, the median survival time was 3 months [95% confidence interval (CI), 2-4 months]. Conclusion The combination of foscarnet and ganciclovir can safely be used for CMV central nervous system (CNS) infection, with an improvement or stabilization in 74% of patients. Life-long MT with this combination is recommended as long as the immune system is profoundly impaired.

Abstract

Objectives: We evaluated the parameters influencing CD4 cell reconstitution after the introduction of highly active antiretroviral therapies in real life, as well as the frequency and the determinants of the discrepancies occurring between virus and CD4 cell count evolution. Design and methods: A total of 317 pre-treated patients starting a protease inhibitor (PI)-containing regimen were prospectively followed for 2 years on an intent-to-treat basis for CD4 cell counts and viral loads. Results: The CD4 cell counts rapidly increased from baseline (50/mm 3 ) by a median of 50/mm 3 at month 2 (+0.72 CD4 cells/mm 3 /day) and up to 137/mm 3 at the last follow-up (second slope: +0.16 CD4 cells/mm 3 /day). Two independent major factors among five parameters tested significantly affected the first phase, which was negatively correlated to the slope of CD4 cell decline before PI initiation, and was positively correlated to baseline CD4 cell counts (P = 0.0001); the second phase was mostly affected by the mean viral load reduction over time (P = 0.0001). Paradoxical CD4 cell reconstitution (15% of cases) was defined by a rapid or slow CD4 cell increase contrasting with a minor or strong viral reduction, respectively. The role of previous CD4 cell decline and the low effect of viral load reduction during the first 2 months explain the early paradoxical CD4 cell responses. The major influence of viral load reduction on the long-term reconstitution, however, reduces such paradoxical responses at 2 years. Conclusions : Early paradoxical CD4 cell reconstitution after the introduction of a PI are explained by the major influence of previous disease progression on the early CD4 cell increase, whereas the magnitude of viral load reduction over time reduces such paradoxical evolutions in the long term.

Abstract

OBJECTIVES To evaluate efficacy and tolerance of bleomycin in AIDS-associated mucocutaneous Kaposi's sarcoma. A previous study showed that bleomycin was effective and well tolerated in this setting. DESIGN A non-comparative, open, prospective study. METHODS Seventy patients were treated with 5 mg per day intramuscular bleomycin on 3 consecutive days every 2 weeks. RESULTS Two patients achieved a complete response and 50 a partial response (overall response rate, 74%). Median time to treatment response was 4 weeks (range, 2-12 weeks) and median time to relapse 10 weeks (range, 2-36 weeks). Dose-limiting toxicity consisted of cutaneous adverse reactions. CONCLUSION Bleomycin is active against AIDS-associated mucocutaneous Kaposi's sarcoma; relapse occurs after discontinuation of therapy.
