Objectives: We aim to describe the epidemiological and prognostic characteristics of non valvular atrial fibrillation related stroke. Patients and methods: We retrospectively analyzed data of hospitalized patients from January 1 st 2010 to June 30th 2012 in the two contiguous departments of cardiology and neurology, at the teaching hospital of Yalgado Ouedraogo, Burkina Faso. Results: We recorded 391 cases of ischaemic stroke. Cardioembolic stroke was observed in 159 patients out of who, non valvular atrial fibrillation accounted for 43.5%. Atrial fibrillation was persistent/permanent in 60 cases (87%). The mean age ± SD was 63.3 ± 14.2 years (extremes: 26-91 years). Hypertension was observed in 85.5% of the cases. The mean CHA 2 DS 2 -VASc score was 4.72 ± 1.16. The mean HAS-BLED score was 2.35 ± 0.92 with a low to intermediate bleeding risk score (≤ 2) in 60.9 % of patients. The rate of vitamin K antagonists use was 52%. They were introduced at least a week after stroke onset in 34 cases. A mean follow up of two weeks in hospital noticed haemorrhagic transformation and ischaemic recurrence in respectively 8.7% and 2.9% of the cases. Introduction: Hypertension in pregnancy remains one of the leading causes of maternal and fetal morbidity and mortality worldwide. We aim to assess the prognostic pattern of hypertension (HPT)during pregnancy in the cardiology and obstetrics/gynaecology departments of Yalgado Ouedraogo University Hospital. Patients and methods: An observational cohort study was conducted from July 1st 2012 to March 31st 2013 in pregnant women with HPTwho consent to participate. Follow-up visits were monthly performed. Results: Overall 3247 pregnant women attended the Obstetrics/Gynaecology department and 312 were hypertensive. The prevalence of HPT during pregnancy was 9.6%. Study population comprised 126 patients who consented to participate in the study. Chronic HTP and preeclampsia accounted for 19% and 49.2 % of all cases respectively. The mean age was 29 ± 6.7 years. Antihypertensive drugs were used in 108 cases (85.7%). At least a bitherapy was found in 46 (36.5%). Cesarean section was the mode of delivery in 79 cases (62.7%). Blood pressure control was significantly higher in patients with new cases (gravidic HPT) compared to those with chronic hypertension over time. Eclampsia was observed in 19.8% and no maternal death occurred. Intrauterine fetal death and prematurity were found in 14.5% and 28.3% of fetuses. Proteinuria and hyperuricemia were associated with fetal complications. Conclusion: Hypertension in pregnancy remains a public health concern in developing countries. Efforts should be made to strengthen and promote maternal and perinatal heath care.

Cardio embolic stroke: Data from 145 cases at the Teaching Hospital of YalgadoOuedraogo, Ouagadougou, Burkina Faso Introduction: Cardioembolic strokes represent a major public health concern worldwide due to associated high morbidity and mortality. Cardiac sources of embolism are leading cause of stroke after atherosclerosis. We aim to describe the epidemiological profile and outcome of cardioembolic stroke. Patients and methods: We retrospectively analyzed medical records of hospitalized patients who were admitted from January 1rst 2010 to May the 31th 2012 in the departments of Cardiology and Neurology at the Teaching University Hospital of YalgadoOuedraogo, Burkina Faso, West Africa. All patients diagnosed with ischaemic stroke on the basis of CT-scan and known to have a heart disease were included in the study. Results: Overall 582 cases of stroke were reported. Ischaemic stroke was observed in 370 patients (63.6%). Cardioembolic disease was reported in 145 patients (39.2%) among whom 73 female. Mean age was 61.7 ± 15 years (extremes: 21 - 90 years). Hypertension and active smoking were respectively observed in 65.5% and 25.5% of cases. Etiologic factors were atrial fibrillation (42.8%) and intra-cardiac blood clot (13.8%). Vitamin K antagonists were prescribed in 41.4% of cases. A two-week in-hospital follow-up reported hemorrhagic transformation in 8.3% of cases. In-hospital mortality rate was 15.2% and was significantly associated with hemorrhagic transformation (n = 10, RR = 9.24, CI95% = [5.1-16.8], p < 0.001) and congestive heart failure (n = n= 10, RR = 4, CI95%= [1.9-8.2], p < 0.001) and altered consciousness on admission (n= 8, RR = 2.7, CI95% = [1.3-5.8], p =0.009). Conclusion: Cardioembolic strokes are of frequent occurrence and associated with high in-hospital mortality. Therefore there is a need for early management of their etiologic factors.
