A new report from the Institute for Cancer Research and Treatment (IRCCS) in Ancona, Italy discusses the challenges of anticoagulation therapy for elderly patients with atrial high-rate episodes (AHREs). The report highlights that atrial fibrillation (AF) is associated with higher morbidity and mortality rates in older patients. The meta-analysis of clinical trials suggests that oral anticoagulation therapy with Edoxaban or Apixaban can reduce the risk of stroke by approximately 32%, but also increase the risk of major bleeding by approximately 62%. The report proposes incorporating additional parameters such as chronic kidney disease, obesity, enlarged left atrial volume, blood pressure management efficacy, and frailty into the traditional CHA2DS2-VASc score to better stratify the risk of stroke and systemic embolism in patients with AHREs.
