This clinical report details a novel surgical modification at the time of maxillectomy to improve obturator function. A vestibuloplasty and split thickness skin graft of the zygomaticoalveolar crest created a denture-bearing surface that, when engaged, provided support and stability for the obturator prosthesis. This report describes the surgical and prosthetic reconstruction of a partial mandibular resection due to an osteomyelitic variant with a free fibula microvascular graft, dental implant placement, and reconstruction with a computer-aided design/computer-aided manufacturing (CAD/CAM) partial fixed dental prosthesis. Prosthodontic rehabilitation of a surgically resected/reconstructed jaw with a conventional tissue-borne dental prosthesis is often challenging, if not impossible, because of the suboptimal conditions of the soft and hard tissue topography/architecture of the reconstructed site. Placing dental implants in grafted bone to provide appropriate support, stability, and retention for prosthodontic rehabilitation offers the potential for improved oral function. There are, however, some clinical conditions for which an implant-supported removable prosthesis may be preferred to a fixed implant prosthesis. During soft tissue matrix expansion (tent pole) graft (STMEG) surgery, the traditional intraoral surgical guide for dental implant placement cannot be used because the surgery is initiated and maintained outside the oral cavity. The essential records established from an intraoral surgical guide must be transferred to an extraoral surgical guide. This clinical report describes the fabrication and use of an extraoral surgical guide employed during STMEG surgery.

A system in which hydroxylapatite is used for augmentation of the mandibular ridge in conjunction with vestibuloplasty is presented. This technique was found to have the benefit of combining a vestibular procedure with an augmentation procedure and thus avoiding an additional surgical operation. The results were uniformly good in the twelve cases presented.

A combined surgical-prosthetic approach for the treatment of the unstable lower denture due to displacement by the circumoral musculature has been presented. This procedure increases the stabilizing forces of the tongue and obicularis oris muscles, while eliminating the displacing forces of muscles which attach high on the crest of the residual ridge.

A simplified two-stage procedure for extension of the atrophic mandibular ridge with the use of a modified application of previously known techniques can provide excellent improvement in the stability of a denture base. This technique can be performed on an outpatient basis, without the use of free grafts or stents, and shows minimal to no relapse. Complications, surgical time, discomfort, and cost to the patient are all decreased. Satisfactory results have been obtained with this procedure over a 10 year period, with greatly improved patient acceptance.

Results are reported from a series of twenty-four patients who received intraoral dermis grafts for a variety of conditions and have been followed for between 6 months and 4 years. Advantages and disadvantages of dermal grafting, including contracture, the final histologic and clinical appearance of the graft, and problems with the donor area, are discussed.

A one-step procedure has been designed to help the patient with abnormal maxillary anterior alveolar atrophy. The technique combines an anterior maxillary osteotomy with a maxillary vestibuloplasty. Bone grafting and skin grafting are not required, and prosthetic rehabilitation can begin 6 to 8 weeks postoperatively. This is a preliminary report; however, 18-month follow-up examinations show continued stability and minimal regression.
