We report a case of a 32-year-old female with a 6-month history of excruciating pain in hypothenar region of the left hand. Pain was aggravated by pressure, touch, and cold temperature. Musculoskeletal ultrasonography revealed a well-defined, hypoechoic lesion in the deep dermis and subcutaneous fat in the left hypothenar eminence with mixed arterial and venous signals within on Doppler study, suggestive of subcutaneous vascular lesion, and a diagnosis of glomus tumor was suggested. The patient underwent excision of the lesion. Pathological examination of the specimen showed a glomus tumor and excluded malignant transformation to glomangiosarcoma. Extra digital glomus tumor can be diagnosed on ultrasound with high confidence in appropriate clinical setting.

The wrist is a common site of injury and the most frequently injured body part among professional golfers. A 37-year-old, right-handed male golfer presented with pain at the ulnar aspect of his left palm, which grew worse after an initial traumatic impact from the golf club handle. There was tenderness over the hypothenar eminence of the left palm. Computed tomography of the left wrist showed an undisplaced fracture through the base of the hamate hook. The golf-induced hamate hook fracture was managed conservatively, with cessation of physical activity involving the left hand and wrist for eight weeks. The patient made a full recovery. Repetitive trauma, exacerbated by improper wrist motion, leads to typical wrist injuries affecting golfers, such as ulnar impaction syndrome, de Quervain’s disease, and tendinopathy affecting the flexor carpi ulnaris and extensor carpi ulnaris, all of which can be diagnosed on imaging.

Flexor digiti minimi is a muscle of hypothenar eminence. It flexes the little finger at metacarpo­ phalangeal joint. During routine Anatomical dissection, an extra muscle was noticed in the flexor compartment of right forearm of a male cadaver. The identified muscle took origin from medial aspect of the Palmaris longus tendon just proximal to the wrist joint. When traced distally this muscle joined the fibers of the Flexor digiti minimi, and finally got inserted to palmar aspect of base of proximal phalanx of 5th digit. In its course, the muscle crossed median nerve and Ulnar artery. Because of this reason, this variation has the potential to cause compression of the structures mentioned above and is of interest to clinicians.

The palmaris longus muscle (PL) is described as probably the most variable muscle in the human body, being variable both in number and form. 1 It can be absent in about 11% of bodies and this absence is more often in females and on the left side. 1,2 Bilateral absence is more common than unilateral absence. 1 Concerning the kind of muscle, it may be fleshy throughout its entire length or may be digastric. The muscle may have a proximal tendon or a distal one. It may be fleshy distally and tendinous proximally (Palmaris longus inversus), being known as the reversed palmaris longus muscle (RPL). The insertion is also highly variable and the muscle may be attached to the fascia of the forearm, the tendon of flexor carpi ulnaris, the flexor retinaculum, the pisiform or the scaphoid bones, the abductor pollicis brevis muscle, the fascia or muscles of the hypothenar eminence, one of the flexor tendons, or near the metacarpophalangeal joints. 1 The distal end of the PL is of clinical interest because of its possible relationships with the median and ulnar nerves. Usually the anatomical variations of the PL are not symptomatic. Nevertheless, a RPL can cause median nerve compression, 2,3 and less frequently ulnar nerve compression. 4
