In India Doctorate of Medicine (MD) pharmacology is primarily knowledge oriented based on teaching, seminars, lectures and research related activities including animals and paper-based experiments and day to day management of undergraduate classes. MD pharmacology student should be competent of both clinical and experimental pharmacology. So, the postgraduate pharmacology curriculum should be competent to meet all the job requirements. Therefore, medical council of India (MCI) has introduced new post graduate curriculum which is based on knowledge, practical, clinical skills, thesis skills, and attitudes including communication and training in research. In India demand for skilled clinical research professionals is increasing day by day for growing pharma industries and good academician. So, there is an urgent need for the experienced and skilled pharmacologist to fulfil the requirements. MD pharmacology students should get posting in different clinical departments and observatory posting in industry, clinical research organization (CRO), regulatory body and research organisations. The course of MD Pharmacology should be like that fulfil all the skills that a pharmacologist must have.

Background: Analgesics including nonsteroidal anti-inflammatory drugs (NSAIDs) are commonly prescribed group of drugs in clinical practice for the management of pain and inflammation. This study was carried out to evaluate the prescribing pattern of analgesics in orthopedic in-patient department of tertiary care teaching hospital at Bareilly (Uttar Pradesh). Methods: This hospital based cross-sectional study was carried out for 3 months from March to May 2015. Collected data included age, sex, diagnosis and line of management during the study. The generic name and the average cost of treatment per patient were evaluated using Indian Drug Review, 2014. The prescribed drugs were assessed with respective National Model List of Essential Medicines (NLEM), 2011 and the rationality of prescriptions was determined using the World Health Organization indicators of drug utilization. The patients' details were recorded in a data collection form and results were analysed by descriptive statistics. Results: Out of 400 patients, 246 were male and 154 were female. The average number of analgesic per prescription was 1.48. In this study, 52.5% of patients had received single analgesic. Diclofenac was the most commonly prescribed analgesic (45.39%). During hospitalization, majority of the patients have received parenteral preparation. Gastro protective agents and antimicrobials were frequently prescribed along with analgesics. Out of 584 analgesics prescribed, 356 (60.96%) were from the NLEM, India. Furthermore, 276 (47.26%) analgesics were prescribed by generic name. The average cost of treatment per patient was 2050.70 INR. Utilization of analgesic in terms of defined daily dose/100 bed-days was 102.06. Conclusions: The percentages of analgesics prescribing from NLEM and the use of analgesic by generic name were found satisfactory.

Background: The objective of the study was to study the pattern and trends of adverse effects of drugs used in department of neurology in a tertiary care hospital.Methods: A prospective, observational study was carried out for a duration of 12 months from November 2018 to October 2019 at Department of Neurology and Department of Pharmacology and Therapeutics, G.S.V.M. Medical College, Kanpur after getting an approval from institutional ethical committee. Data was collected by analyzing OPD prescription slip, treatment charts and investigation reports. All relevant information regarding adverse drug reactions (ADRs) were collected as per norms of Indian Pharmacopoeia commission (IPC).Results: During the study period, a total of 130 ADRs reported. Most of the ADRs were reported due to antiepileptic drugs followed by antiparkinsonian drugs. Dizziness was the most frequent ADR reported. Most of the ADRs were reported due to phenytoin. Other ADRs observed were drowsiness, nausea/vomiting, weakness, joint pain, dyskinesia.Conclusions: Most of the ADRs were due to anti-epileptic drugs. Most of the reactions were of mild severity.

Background: Diabetes mellitus (DM) refers to a group of common metabolic disorders that associated with abnormalities in carbohydrate, fat, and protein metabolism which results in chronic complications. Attainment of optimal blood sugar level is generally based on appropriate usage and proper adherence to prescribed medications. The study was, therefore, aimed to assess adherence to oral antidiabetic drugs among diabetic patients attending outpatient clinic of L. L. R. Hospital, G.S.V.M. Medical College, Kanpur, U.P.Methods: Hospital based cross-sectional study design was conducted from April 2017 to June 2018. The data was collected by interviewing T2DM patients receiving antidiabetic medications using Morisky’s four item adherence assessment questionnaire. The collected data was processed and analyzed with SPSS version 20.Results: From the 126 patients of diabetes, when asked about adherence to their medications as per the Morisky's four item method, 114 (90.47%) of them did not forget to take the drugs, 108 (85.71%) of patients reported that they had been being careful in taking their medications, 90 (71.42%) patients did not stop medications when they felt better and the other 108 (85.71%) patients reported that they did not stop medications when they felt worse while taking medications. This study shows that 54 (42.86%) respondents were adherent to their medications.Conclusions: This study revealed a moderate level of adherence among the participants and statistically significantly depended upon their socioeconomic status. Efforts are needed to increase the medication adherence of these patients’, so they can realize the full advantage of prescribed therapies.

Background: Around 34.6% of Indian population consumes tobacco. The tobacco consumption is higher in some vulnerable population such as drivers, daily wage laborers, and policemen. Tobacco consumption is known to cause oral cancers, and screening for oral cancer in these individuals is known to reduce mortality from cancer. The study was designed to assess the determinants of tobacco use and the prevalence of oral precancerous lesions in auto-rickshaw drivers.Methods: This is a cross-sectional study among auto-rickshaw drivers at Bareilly (UP). A total of 450 auto-rickshaw drivers were enrolled in the study, of which 225 auto-rickshaw drivers were interviewed during morning hours and remaining half at night time using a semi-structured questionnaire. All were screened for oral cancer/precancerous lesions. Results: Nearly 64.44% of auto-rickshaw drivers were consuming tobacco in any form. Long working hours, working at night, and family members consuming tobacco were significant risk factors for tobacco use among auto-rickshaw drivers. 56 (19.31%) auto-rickshaw drivers were detected to have oral precancerous lesions. Conclusions: It was very evident that long hours of driving and infrequent shifts played a greater role in acquiring the habit. Behavioral counselling and new laws need to be formed to limit the working hours in drivers to have an effective tobacco control.

Background: Chronic suppurative otitis media (CSOM) is one of the most common ear diseases in the South East Asia having a prevalence of approximately 5.2% in the general population. The objective of the study was to find the prevalence of CSOM in school going children in urban field practice area of tertiary care hospital in Bareilly (UP). Methods: Cross-sectional study involving 495 schools going children in Bareilly district of Uttar Pradesh. Results: The prevalence of CSOM was found to be 6.46%, with tubotympanic type constituting majority 81.25% trailed by atticoantral type 18.75%. Most patients presented with moderate hearing impairment (43.75%) and mild hearing impairment (31.25%). With respect to socioeconomic groups, upper lower, groups had 40.63%, and lower group had 34.37% prevalence. Conclusions: Improvement of health care facilities and awareness among health-care providers would definitely be helpful in reducing the prevalence of ear diseases in the developing countries like India.

Background: Lipid profile parameters may be used as biomarker for depression. Sertraline belongs to selective serotonin reuptake inhibitors (SSRIs), the most commonly used group to treat the depression in multidrug resistant tuberculosis patients.Methods: A prospective clinical trial was carried out in department of Psychiatry and department of Tuberculosis and Respiratory disease G.S.V.M. Medical College, Kanpur. Diagnosed MDR TB patients were screened for depression applying Hamilton Depression Rating Scale (HDRS) and these patients were referred to Psychiatrist for diagnosis of depression. Total 25 diagnosed patients of MDR TB with mild to moderate depression were selected. HDRS Score and morning blood sample of 5ml were collected to analyze biomarker for depression before intervention. Same test was repeated in 18 patients who completed the study at day 30 and 120 after administering Sertraline (50mg). Data were compiled and analyzed using SPSS 20.0 and paired t - test.Results: The mean decrease in HDRS score from base line at day 30 and 120 of administering Sertraline were 6.22 (±1.26) and 2.72 (±0.67) which were significant (p˂0.001). The mean increase in serum cholesterol at day 30 was 153.94 (±19.31) and at day 120 was 157.83 (±19.36) which were significant (p˂0.001). Rest of Tg, HDLc, LDLc and VLDL cholesterole levels were not increased significantly.Conclusions: As the depression symptoms improved by sertraline. The biomarkers of depression were also increased (within the normal range) from baseline but significant increase was observed in serum cholesterol only.

Background: Soil-transmitted helminths are mostly prevalent in developing countries due to poor sanitation and lack of adequate clean water. The present study examines adverse events (AEs) experienced following administration of albendazole to children (2-19 Years) at Uttarakhand on national de-worming day.Methods: Children were given single doses of albendazole on national de-worming day. Some of children experienced adverse events and were admitted in hospital of Govt Medical college Haldwani (Uttarakhand). Data were collected and analyzed.Results: Total twenty five children were admitted due to albendazole adverse events. Out of these 92% were female. Mean age of admitted children was 14.14 years with standard deviation 3.45. Mean onset of adverse events was 5.6 hours with standard deviation of 1.5 hours. All children were treated symptomatically and were discharged once they recovered. No fatality due to adverse events was observed. Average duration of stay in hospital was 3.4 days. Out of twenty five children 12% children reported four or more adverse events, 40% children reported three adverse events and 48% reported two adverse events. Out AEs, 33% AEs were mild, 19% AEs were moderate, 31% AEs were severe and 17% AEs were serious. Abdominal pain was reported by 76%, headache by 44%, loss of consciousness by 32%, vomiting by 28%, nausea by 16%, convulsions by 12%, rashes by 8%, fever by 8%, and breathlessness by 14% and vertigo by 4%.Conclusions: The adverse events were mild to serious but transient, but all of them recovered after hospitalization. Therefore, it is imperative that mass drug administration programmes put in place surveillance measures in order to ensure timely detection, management and reporting of potential life threatening AEs.

Background: Sick Newborn Care Unit (SNCU) is meant to reduce the case fatality among sick newborns. Although it has been shown that patterns of drug utilization in SNCU are changing dynamically, current data on drug utilization patterns in SNCU is limited. This study was done to find out drug utilization pattern in newborn admitted at SNCU.Methods: This prospective observational drug utilization study was carried out in SNCU of Government Medical College and Hospital, Haldwani (Uttrakhand). The pattern of drug use in 206 sick newborn admitted at SNCU was assessed.Results: 75.25% sick newborns were in early neonatal period and 63.11% sick newborns were male. 33.98% sick newborn were preterm and mean±SD of weight of sick newborn was 2.16±0.6kg. Neonatal sepsis was commonest reason for admission followed by birth asphyxia . Three or more drugs (average 2.35drug/ Range 3-9 drug) were given to 52.91% new born and two drugs were given to 40.78% new born. Commonest route of drug administration was intravenous (97.08%), followed by intramuscular (72.82%), followed by orally (13.59%) and 11.65% new born received drug by inhalation. Most frequently used drugs in SNCU were antibiotics (Ampicillin, Gentamicin, Amikacin, Cefotaxime). Only two adverse drug reactions (mild rash by ampicillin and fever) were reported during study period. Out of 206 sick newborn, 18.93% died. Most common causes for death were respiratory distress syndrome (41.03%) followed by sepsis (23.08%) and hypoxic ischemic encephalopathy (17.95%).Conclusions: Antibiotics were of major concern in SNCU. The uncertainty regarding the choice of antibiotic can be minimized by periodic survey of etiological agent and their antibiotic susceptibility pattern.
