Maritime shipping companies have identified continuous tracking of intermodal containers as a key tool for increasing shipment reliability and generating important economies of scale. Equipping all dry containers with an Internet-connected tracking device is a need in the global shipping market that is still waiting to be met. This paper presents the methods and tools to build and test a prototype of a Container Tracking Device (CTD) that integrates NB-IoT, BLE Mesh telecommunication and low-power consumption technologies for the massive deployment of the IoT. The work was carried out as part of a project to build the so-called “5G Global Tracking System”, enabling several different logistic applications relying on massive IoT, M2M standard platforms, as well as satellite networks to collect data from dry containers when the vessel is in open sea. Starting from a preliminary phase, in which state-of-the-art technologies, research approaches, industrial initiatives and developing standards were investigated, a prototype version of the CTD has been designed, verified and developed as the first fundamental step for subsequent industrial engineering. The results of specific tests are shown: after verifying that the firmware is capable of handling the various functions of the device, a special focus is devoted to the power consumption measurements of the CTD to size the battery pack.

The literature shows evidence of the mechanical investigation of numerous polymeric dental biomaterials using a static approach. A more representative mechanical analysis of such materials must take into account the dynamic masticatory load of the oral cavity. The aim of this work is to study the dynamic mechanical proprieties and provide an in vitro characterization of 11 3D-printed new dental biomaterials to understand their clinical applications under physiological conditions. The analysis included Dynamical Mechanical Analysis (DMA) and an MTT cytotoxicity assay. The mechanical results at low frequencies (1–11 Hz) show high uncertainty, less fragility, and less strength. The biological results show a significant reduction in cell viability (p < 0.01) at both the 3 and 24 h timepoints, with a degree of recovery observed at 24 h. To assess the clinical potential of dental biomaterials, it is necessary to determine whether there are good dynamic mechanical properties and reduced adverse biological effects on oral cells. This may allow for the facile fabrication via 3D printing of prosthetic devices that can support masticatory loads over long periods of time. Further investigations of the presented polymeric materials are needed, exploring biological assessments for longer than 24 h.

The purpose of this work was to investigate the production of reactive oxygen species (ROS) in Allium fistulosum seeds stored under different conditions. Optimized seed storage conditions are essential to maintain seed viability, otherwise accumulation of ROS-induced oxidative damage can lead to seed aging. The A. fistulosum seed lots used in this study have been selected based on their breeding background and reproduction site. Seed samples were stored up to 22 months under six different conditions of temperature (25, 10, and 7.5 °C) and relative humidity (RH) (25% and 45% RH). A germination test and ROS quantification assay were performed on the samples collected after 12 and 22 months of storage, respectively. Within a time-window of 10 months, the tested seed lots evidenced a decrease in the germination rate associated with increased ROS levels. Correlation analysis also showed that ROS production was influenced by genotype. The reported data showed that ROS accumulation was dependent on the storage condition and genotype. Some of the tested seed lots appeared to be prone to ROS accumulation, independent of storage conditions. On the other hand, specific condition storages (25 °C, 25% RH; 25 °C, 45% RH; 10 °C, 25% RH; 7.5 °C, 25% RH) resulted in a lower impact on seed aging.

Air pollution is a paramount issue, influenced by a combination of natural and anthropogenic sources, various diffusion modes, and profound repercussions for the environment and human health. Herein, the power of time series data becomes evident, as it proves indispensable for capturing pollutant concentrations over time. These data unveil critical insights, including trends, seasonal and cyclical patterns, and the crucial property of stationarity. Brescia, a town located in Northern Italy, faces the pressing challenge of air pollution. To enhance its status as a smart city and address this concern effectively, statistical methods employed in time series analysis play a pivotal role. This article is dedicated to examining how ARIMA and LSTM models can empower Brescia as a smart city by fitting and forecasting specific pollution forms. These models have established themselves as effective tools for predicting future pollution levels. Notably, the intricate nature of the phenomena becomes apparent through the high variability of particulate matter. Even during extraordinary events like the COVID-19 lockdown, where substantial reductions in emissions were observed, the analysis revealed that this reduction did not proportionally decrease PM2.5 and PM10 concentrations. This underscores the complex nature of the issue and the need for advanced data-driven solutions to make Brescia a truly smart city.

Introduction: Apraxia is a neurological disorder that is common after a stroke and impairs the planning and execution of movements. In the rehabilitation field, virtual reality (VR) presents new opportunities and offers advantages to both rehabilitation teams and individuals with neurological conditions. Indeed, VR can stimulate and improve cognitive reserve and abilities, including executive function, and enhance the patient’s emotional status. Aim: The objective of this research is to determine the effectiveness of VR in improving praxis skills and behavioural functioning in individuals with severe stroke. Methods: A total of 20 stroke patients were enrolled from February 2022 to March 2023 and divided by the order of their recruitment into two groups: the experimental group (EG: n = 10) received training to improve their praxis skills using VR whereas the control one (CG: n = 10) received the same amount of standard training. All patients underwent an evaluation using a psychometric battery that consisted of the Hamilton Rating Scale for Depression (HRS-D), Mini-Mental State Examination (MMSE), Frontal Assessment Battery (FAB), Spinnler and Tognoni test, and De Renzi and Faglioni test. Valuations were performed before rehabilitation (T0) and after its completion (T1). Results: Both groups demonstrated significant improvements post-intervention. The EG showed a greater enhancement in their MMSE scores (p = 0.002), and reductions in both ideomotor and constructive apraxia (p = 0.002 for both), compared to the CG. The VR-based training also resulted in significant improvements in their depression symptoms (HRSD scores improved, p = 0.012 in EG vs. p = 0.021 in CG). Conclusions: This pilot study suggests that VR could help reduce cognitive, constructive apraxia and ideomotor apraxia symptoms caused by stroke injury.

In recent years, there has been marked interest in looking at the psychological consequences of medical conditions, such as traumatic or acquired brain injuries. Coping strategies are essential for clinical recovery and for dealing with the stressful events that a clinical condition brings with it. The purpose of this review is to analyze studies that explore how coping strategies influence psychological changes in patients with acquired brain injury. Studies were identified from research in the PubMed, Scopus, and Embase databases. According to our findings, patients with ABI utilize different coping strategies based on the circumstances and factors such as the diagnosis severity, their age, time lived with the pathology, and personal characteristics, which have an influence on quality of life and rehabilitation. This review demonstrated that coping strategies have an impact on different aspects of the clinical and personal lives of patients with ABI. The rehabilitation process must consider the influence of these mechanisms on dealing with situations, as they can change cognitive and emotional perceptions of patients’ experience with the disease, as well as laying the foundations for functional or dysfunction in terms of the propensity of a person for the path of psychological and physical recovery.

Quantum computers have the potential to break the public-key cryptosystems widely used in key exchange and digital signature applications. To address this issue, quantum key distribution (QKD) offers a robust countermeasure against quantum computer attacks. Among various QKD schemes, BB84 is the most widely used and studied. However, BB84 implementations are inherently imperfect, resulting in quantum bit error rates (QBERs) even in the absence of eavesdroppers. Distinguishing between QBERs caused by eavesdropping and QBERs due to channel imperfections is fundamentally infeasible. In this context, this paper proposes and examines a practical method for detecting eavesdropping via partial intercept-and-resend attacks in the BB84 protocol. A key feature of the proposed method is its consideration of quantum system noise. The efficacy of this method is assessed by employing the Quantum Solver library in conjunction with backend simulators inspired by real quantum machines that model quantum system noise. The simulation outcomes demonstrate the method’s capacity to accurately estimate the eavesdropper’s interception density in the presence of system noise. Moreover, the results indicate that the estimation accuracy of the eavesdropper’s interception density in the presence of system noise is dependent on both the actual interception density value and the key length.

The tribological properties of steels used to realise railway wheels play a fundamental role in the performances of both vehicle and infrastructure. In particular, the wear process, caused by the wheel–rail interaction, modifies the shape of wheel and rail profiles, changing the performances of the vehicle. For this reason, research institutes and vehicle manufacturers have worked hard to develop predictive tools able to estimate the evolution of the wheel and rail profiles. The efficiency of these tools is strongly influenced by the tribological properties of the materials, i.e., the wear coefficients, which are used as input data. The characterisation of these properties requires specific tools and long-lasting experimental campaigns, which are usually performed under controlled operating conditions, using twin-disc test benches. These devices usually do not consider the real contact conditions in terms of normal load, contact geometry, and slip velocity, since they are equipped with small-size rollers. The paper proposes an innovative 1:5 scaled twin-disc, which allows the reproduction of the real wheel–rail contact conditions, thanks to Pascal’s scaling technique. The testing device allows the reproduction of a wide range of typical operating conditions of railway vehicles, thanks to high-power independent brushless motors, used to actuate the rollers, and an innovative loading system.

Nowadays, control is pervasive in vehicles, and a full and accurate knowledge of vehicle states is crucial to guarantee safety levels and support the development of Advanced Driver-Assistance Systems (ADASs). In this scenario, real-time monitoring of the vehicle sideslip angle becomes fundamental, and various virtual sensing techniques based on both vehicle dynamics models and data-driven methods are widely presented in the literature. Given the need for on-board embedded device solutions in autonomous vehicles, it is mandatory to find the correct balance between estimation accuracy and the computational burden required. This work mainly presents different physical KF-based methodologies and proposes both mathematical and graphical analysis to explore the effectiveness of these solutions, all employing equal tire and vehicle simplified models. For this purpose, results are compared with accurate sensor acquisition provided by the on-track campaign on passenger vehicles; moreover, to truthfully represent the possibility of using such virtual sensing techniques in real-world scenarios, the vehicle is also equipped with low-end sensors that provide information to all the employed observers.

A single-server queueing system with n classes of customers, stationary superposed input processes, and general class-dependent service times is considered. An exponential splitting is proposed to construct classical regeneration in this (originally non-regenerative) system, provided that the component processes have heavy-tailed interarrival times. In particular, we focus on input processes with Pareto interarrival times. Moreover, an approximating GI/G/1-type system is considered, in which the independent identically distributed interarrival times follow the stationary Palm distribution corresponding to the stationary superposed input process. Finally, Monte Carlo and regenerative simulation techniques are applied to estimate and compare the stationary waiting time of a customer in the original and in the approximating systems, as well as to derive additional information on the regeneration cycles’ structure.

Recently, a large number of nosocomial infections have been caused by an emerging pathogen that is rising as a worldwide issue in human health: Candida auris. This yeast is considered resistant to antifungals of the first-line therapies, and consequently it is related to morbidity and mortality. Therefore, the aim of this research was to determine the in vitro anti-C. auris activity against twenty-three resistant clinical strains of different essential oils (EOs), pure or in combination with traditional antifungal agents, mainly caspofungin, fluconazole, micafungin and 5-flucytosine. Broth dilution assay was performed to evaluate the fungistatic and fungicidal effectiveness of fifteen EOs towards all the C. auris isolates. The data demonstrated that EOs were able to prevent C. auris growth, with MIC values ranging from 0.03 to 1% for the efficacious EOs (thyme, cinnamon, geranium, clove bud, lemongrass and mentha of Pancalieri), whereas the MICs were >1% for the ineffective ones. Thereafter, the six most effective EOs were used to perform the checkerboard experiments by assaying simultaneously the activity of EOs and traditional antifungals towards two selected strains. The most promising synergic combinations towards C. auris, depending on the isolate, were those with micafungin and geranium, thyme, cinnamon, lemongrass or clove bud EOs, with fluconazole and mentha of Pancalieri EO, and with 5-flucytosine and mentha of Pancalieri EO. These EOs and their combinations with antifungal drugs may provide a useful therapeutic alternative that could reduce the dose of the individual components, limiting the overall side effects. These associations might be a prospective option for the future treatment of infections, thus helping to overcome the challenging issue of resistance in C. auris.

Breaking is a performative art that has recently undergone a process of sportification, developing into an aesthetic sport included in the 2024 Paris Olympic Games. Despite its growing worldwide popularity, there is a lack of research on Breaking. Accordingly, this pilot study’s aim was twofold: (a) to provide an initial understanding of the anthropometric measures, body composition data, somatotype profiles, and strength performance of male (B-boys) and female (B-girls) Italian Breakers divided into elite (international) and sub-elite (national) levels and (b) to guide further research on the area, providing the methodological approach for future investigations. A total of 24 B-boys (elite n = 5; sub-elite n = 19) and 9 B-girls (elite n = 3; sub-elite n = 6) were included in this study. Descriptive analyses revealed that B-boys and B-girls displayed low height and weight (1.70 m (63.8 kg) and 1.58 m (54.2 kg), respectively), low levels of body fat percentages (10.3% and 17.6%, respectively), and a balanced mesomorph somatotype (2.28–4.64–2.69 and 2.34–5.16–2.38, respectively), revealing a marked development of muscular mass. Due to the small sample size, Welch’s test and correlation analyses did not report any elite vs. sub-elite difference. It was hypothesized that Breakers’ morphological profiles result from the selection procedures and training regimens related to Breaking aesthetic, athletic, and physiological demands.

Electrification of aircraft is a very challenging task as the demand for energy and power is high. While the storage and generation of electrical energy are widely studied due to the limited specific energy and specific power of batteries and fuel cells, electric machines (power electronics and motors) which have years of experience in many industrial fields must be improved when applied to aviation: they generally have a high efficiency but the increase in power levels determines significant thermal loads which, unlike internal combustion engines (ICE), cannot be rejected with the exhaust. There is therefore a need for thermal management systems (TMSs) with the main objective of maintaining operating temperatures below the maximum level required by electric machines. Turboprop aircraft, such as the ATR 72 or the Dash 8-Q400, are commonly used for regional transport and are equipped with two gas turbine engines whose combined power is in the order of 4 MW. Electric and hybrid propulsion systems for these aircraft are being studied by several leading commercial aviation industries and start-ups, and the 1MW motor size seems to be the main option as it could be used in different aircraft configurations, particularly those that exploit distributed electric propulsion. With reference to the topics mentioned above, the present work presents the design of a TMS for a high-power motor/generator whose electrical architecture is known. Once integrated with the electrical part, the TMS must allow a weight/power ratio of 14 kW/kg (or 20 kW/kg at peak power) while maintaining the temperature below the limit temperature with reasonable safety margins. Submerged jet oil is the cooling technique here applied with a focus on diathermic oil. Parameters affecting cooling, like rotor speed and filling factor, are analysed with advanced CFD.

Staphylococcus aureus bacteraemia (SAB) is a life-threatening bloodstream infection. Improved adherence to quality-of-care indicators (QCIs) can significantly enhance patient outcomes. This quasi-experimental study evaluated the impact of a bundle of interventions on QCI adherence in adult patients with SAB. Additionally, a molecular rapid diagnostic test (mRDT) for S. aureus and methicillin resistance was introduced during weekdays. We compared pre-intervention (January–December 2022) and post-intervention (May 2023–April 2024) data on QCI adherence and time to appropriate treatment. A total of 56 and 40 SAB episodes were included in the pre- and post-intervention periods, respectively. Full QCI adherence significantly increased from 28.6% to 67.5% in the post-intervention period (p < 0.001). The mRDT diagnosed SAB in eight cases (26.6%), but the time to achieve appropriate target therapy did not improve in the post-intervention period (54 h (IQR 30–74) vs. 72 h (IQR 51–83), p = 0.131). The thirty-day mortality rate was comparable between the two periods (17.9% vs. 12.5%, p = 0.476). This study demonstrates that a bundle of interventions can substantially improve adherence to SAB management QCIs.

Artificial intelligence algorithms have become extensively utilized in survival analysis for high-dimensional, multi-source data. However, due to their complexity, these methods often yield poorly interpretable outcomes, posing challenges in the analysis of several conditions. One of these conditions is obstructive sleep apnea, a sleep disorder characterized by the simultaneous occurrence of comorbidities. Survival analysis provides a potential solution for assessing and categorizing the severity of obstructive sleep apnea, aiding personalized treatment strategies. Given the critical role of time in such scenarios and considering limitations in model interpretability, time-dependent explainable artificial intelligence algorithms have been developed in recent years for direct application to basic Machine Learning models, such as Cox regression and survival random forest. Our work aims to enhance model selection in OSA survival analysis using time-dependent XAI for Machine Learning and Deep Learning models. We developed an end-to-end pipeline, training several survival models and selecting the best performers. Our top models—Cox regression, Cox time, and logistic hazard—achieved good performance, with C-index scores of 0.81, 0.78, and 0.77, and Brier scores of 0.10, 0.12, and 0.11 on the test set. We applied SurvSHAP methods to Cox regression and logistic hazard to investigate their behavior. Although the models showed similar performance, our analysis established that the results of the log hazard model were more reliable and useful in clinical practice compared to those of Cox regression in OSA scenarios.

Cardiovascular and neurological diseases are a major cause of mortality and morbidity worldwide. Such diseases require careful monitoring to effectively manage their progression. Artificial intelligence (AI) offers valuable tools for this purpose through its ability to analyse data and identify predictive patterns. This review evaluated the application of AI in cardiac and neurological diseases for their clinical impact on the general population. We reviewed studies on the application of AI in the neurological and cardiological fields. Our search was performed on the PubMed, Web of Science, Embase and Cochrane library databases. Of the initial 5862 studies, 23 studies met the inclusion criteria. The studies showed that the most commonly used algorithms in these clinical fields are Random Forest and Artificial Neural Network, followed by logistic regression and Support-Vector Machines. In addition, an ECG-AI algorithm based on convolutional neural networks has been developed and has been widely used in several studies for the detection of atrial fibrillation with good accuracy. AI has great potential to support physicians in interpretation, diagnosis, risk assessment and disease management.

Macroalgae seaweeds such as Ulva lactuca and Gracilaria verrucosa cause problems on the northern coast of the Italian Adriatic Sea because their overabundance hinders the growth of cultivated clams, Rudatapes philippinarum. This study focused on the green synthesis of CuO nanoparticles from U. lactuca and G. verrucosa. The biosynthesized CuO NPs were successfully characterized using FTIR, XRD, HRTEM/EDX, and zeta potential. Nanoparticles from the two different algae species are essentially identical, with the same physical characteristics and almost the same antimicrobial activities. We have not investigated the cause of this identity, but it seems likely to arise from the reaction of Cu with the same algae metabolites in both species. The study demonstrates that it is possible to obtain useful products from these macroalgae through a green synthesis approach and that they should be considered as not just a cause of environmental and economic damage but also as a potential source of income.

By 2030, it is expected that a trillion things will be connected. In such a scenario, the power required for the trillion nodes would necessitate using trillions of batteries, resulting in maintenance challenges and significant management costs. The objective of this research is to contribute to sustainable wireless sensor nodes through the introduction of an energy-autonomous wireless sensor node (EAWSN) designed to be an energy-autonomous, self-sufficient, and maintenance-free device, to be suitable for long-term mass-scale internet of things (IoT) applications in remote and inaccessible environments. The EAWSN utilizes Low-Power Wide Area Networks (LPWANs) via LoRaWAN connectivity, and it is powered by a commercial photovoltaic cell, which can also harvest ambient light in an indoor environment. Storage components include a capacitor of 2 mF, which allows EAWSN to successfully transmit 30-byte data packets up to 560 m, thanks to opportunistic LoRaWAN data rate selection that enables a significant trade-off between energy consumption and network coverage. The reliability of the designed platform is demonstrated through validation in an urban environment, showing exceptional performance over remarkable distances.

Glioblastoma (GBM), the most frequent and lethal brain cancer in adults, is characterized by short survival times and high mortality rates. Due to the resistance of GBM cells to conventional therapeutic treatments, scientific interest is focusing on the search for alternative and efficient adjuvant treatments. S-Adenosylmethionine (AdoMet), the well-studied physiological methyl donor, has emerged as a promising anticancer compound and a modulator of multiple cancer-related signaling pathways. We report here for the first time that AdoMet selectively inhibited the viability and proliferation of U87MG, U343MG, and U251MG GBM cells. In these cell lines, AdoMet induced S and G2/M cell cycle arrest and apoptosis and downregulated the expression and activation of proteins involved in homologous recombination DNA repair, including RAD51, BRCA1, and Chk1. Furthermore, AdoMet was able to maintain DNA in a damaged state, as indicated by the increased γH2AX/H2AX ratio. AdoMet promoted mitotic catastrophe through inhibiting Aurora B kinase expression, phosphorylation, and localization causing GBM cells to undergo mitotic catastrophe-induced death. Finally, AdoMet inhibited DNA repair and induced cell cycle arrest, apoptosis, and mitotic catastrophe in patient-derived GBM cells. In light of these results, AdoMet could be considered a potential adjuvant in GBM therapy.

The adaptability of seed metabolism to different environmental conditions represents a crucial aspect to understand the effects of climate change on plant populations in wild environments. Among the indicators of stress and repair in seeds, tocopherols and malondialdehyde have been related to membrane stability in seed deterioration. Alpine plants constitute an interesting system to understand stress response dynamics because of the relevant climate variations challenging seed viability in alpine environments. This study considered five accessions of Viscaria alpina seeds collected over five years, highlighting significant correlations between environmental parameters such as precipitations and temperature, and several indicators of the oxidative stress response. These provide new insights on how changes in indicators of the seed stress response can reflect annual variations in temperature and precipitations affecting their parental plants, with possible implications on the current understanding of seed persistence in alpine environments threatened by climate change and on the effects of seed storage.

Alzheimer’s Disease (AD) and Frontotemporal Dementia (FTD) are the two major neurodegenerative diseases with distinct clinical and neuropathological profiles. The aim of this report is to conduct a population-based investigation in well-characterized APP, PSEN1, PSEN2, MAPT, GRN, and C9orf72 mutation carriers/pedigrees from the north, the center, and the south of Italy. We retrospectively analyzed the data of 467 Italian individuals. We identified 21 different GRN mutations, 20 PSEN1, 11 MAPT, 9 PSEN2, and 4 APP. Moreover, we observed geographical variability in mutation frequencies by looking at each cohort of participants, and we observed a significant difference in age at onset among the genetic groups. Our study provides evidence that age at onset is influenced by the genetic group. Further work in identifying both genetic and environmental factors that modify the phenotypes in all groups is needed. Our study reveals Italian regional differences among the most relevant AD/FTD causative genes and emphasizes how the collaborative studies in rare diseases can provide new insights to expand knowledge on genetic/epigenetic modulators of age at onset.

Heart failure (HF) is a growing epidemic, affecting millions of people worldwide, and is a major cause of mortality, morbidity, and impaired quality of life. Traditional cardiac rehabilitation is a valuable approach to the physical and quality-of-life recovery of patients with cardiovascular disease. The innovative approach of remote monitoring through telemedicine offers a solution based on modern technologies, enabling continuous collection of health data outside the hospital environment. Remote monitoring devices present challenges that could adversely affect patient adherence, resulting in the risk of dropout. By applying a cognitive-behavioral model, we aim to identify the antecedents of dropout behavior among patients adhering to traditional cardiac rehabilitation programs and remote monitoring in order to improve the latter. Our study was conducted from October 2023 to January 2024. In the first stage, we used data from literature consultation. Subsequently, data were collected from the direct experience of 49 health workers related to both remote monitoring and traditional treatment, recruited from the authors’ workplace. Results indicate that patients with cardiovascular disease tend to abandon remote monitoring programs more frequently than traditional cardiac rehabilitation therapies. It is critical to design approaches that take these barriers into account to improve adherence and patient satisfaction. This analysis identified specific antecedents to address, helping to improve current monitoring models. This is crucial to promote care continuity and to achieve self-management by patients in the future.

The growing obesity epidemic necessitates increased research on adipocyte and adipose tissue function and disease mechanisms that progress obesity. Historically, adipocytes were viewed simply as storage for excess energy. However, recent studies have demonstrated that adipocytes play a critical role in whole-body homeostasis, are involved in cell communication, experience forces in vivo, and respond to mechanical stimuli. Changes to the adipocyte mechanical microenvironment can affect function and, in some cases, contribute to disease. The aim of this review is to summarize the current literature on the mechanobiology of adipocytes. We reviewed over 100 papers on how mechanical stress is sensed by the adipocyte, the effects on cell behavior, and the use of cell culture scaffolds, particularly those with tunable stiffness, to study adipocyte behavior, adipose cell and tissue mechanical properties, and computational models. From our review, we conclude that adipocytes are responsive to mechanical stimuli, cell function and adipogenesis can be dictated by the mechanical environment, the measurement of mechanical properties is highly dependent on testing methods, and current modeling practices use many different approaches to recapitulate the complex behavior of adipocytes and adipose tissue. This review is intended to aid future studies by summarizing the current literature on adipocyte mechanobiology.

The Italian Association for Laboratory Animal Sciences (AISAL) held its annual meeting in September 2022. The main topic of the meeting was the section ‘Quality Control of the Components in the Study’ of the PREPARE guidelines. Autoimmune pancreatitis (AIP) is a unique form of chronic pancreatitis with a multifactorial pathogenesis. Historically, it has been classified as type 1 and type 2, according to its clinical and histological features. The diagnosis of AIP is challenging and relies on a combination of clinical, histopathologic, serologic, and imaging characteristics. In the available guidelines, the imaging hallmarks of AIP are based on cross-sectional imaging and cholangiopancreatography retrograde endoscopic findings. Endoscopic ultrasound (EUS) is generally used for pancreatic tissue acquisition to rule out pancreatic cancer and diagnose AIP with limited accuracy. Several papers reported the reliability of EUS for providing informative morphologic features of AIP. Nowadays, the improvement in the resolution of EUS conventional images and the development of new ancillary technologies have further increased the diagnostic yield of EUS: contrast-enhanced EUS and EUS elastography are non-invasive and real-time techniques that strongly support the diagnosis and management of pancreatic diseases. In this review article, we will present the role of conventional EUS and ancillary diagnostic techniques in the diagnosis of AIP to support clinicians and endosonographers in managing this condition.

The SH2 domains of SHP2 play a crucial role in determining the function of the SHP2 protein. While the folding and binding properties of the isolated NSH2 and CSH2 domains have been extensively studied, there is limited information about the tandem SH2 domains. This study aims to elucidate the folding and binding kinetics of the NSH2-CSH2 tandem domains of SHP2 through rapid kinetic experiments, complementing existing data on the isolated domains. The results indicate that while the domains generally fold and unfold independently, acidic pH conditions induce complex scenarios involving the formation of a misfolded intermediate. Furthermore, a comparison of the binding kinetics of isolated NSH2 and CSH2 domains with the NSH2-CSH2 tandem domains, using peptides that mimic specific portions of Gab2, suggests a dynamic interplay between NSH2 and CSH2 in binding Gab2 that modulate the microscopic association rate constant of the binding reaction. These findings, discussed in the context of previous research on the NSH2 and CSH2 domains, enhance our understanding of the function of the SH2 domain tandem of SHP2.

Fibromyalgia (FM), a chronic disease with a high incidence in women, poses a significant challenge for diagnosis and treatment, especially due to the absence of specific biomarkers and the multifaceted nature of its symptoms, which range from neuromuscular pain to mood disorders and intestinal dysbiosis. While diagnosis currently relies on rheumatological clinical evaluations and treatment options mainly focus on symptom management, FM seems to have possible links with systemic metabolic dysfunctions with a common inflammatory root. In this context, a new therapeutic avenue emerges: could a therapeutic nutritional approach be the missing piece of the puzzle? Indeed, diet therapies employed particularly for metabolic syndromes proved recently to be efficacious for correcting systemic dysmetabolism and a high number of chronic inflammation conditions. In particular, the very-low-calorie ketogenic diet (VLCKD) demonstrated therapeutic benefits in many disorders. In the present study, we aimed to investigate the specific effects of two dietary interventions, namely the oloproteic VLCKD and the low-glycemic insulinemic (LOGI) diet, on two groups of female FM patients (FM1 and FM2) over a 45-day period. Utilizing clinical and laboratory tests, as well as non-invasive NMR metabolomic analysis of serum, urine, and saliva samples, we sought to uncover how these dietary regimens impact the metabolic dysfunctions associated with FM.

Pavement adhesion plays a crucial role in driving safety, while traditional test methods exhibit some limitations. To improve the efficiency and accuracy of asphalt pavement texture characterization and adhesion assessments, this paper uses three-dimensional (3D) laser technology to detect the continuous point cloud data of road surface and reconstruct the 3D topography of pavement texture. On this basis, a volume parameter Volume of peak materials (Vmp) is innovatively proposed to comprehensively characterize the 3D spatial characteristics of road surface texture. The correlation analysis between the proposed Vmp and the traditional adhesion evaluation index Transversal Adhesion Coefficient (CAT) is conducted, and then refined graded adhesion prediction models based on the proposed Vmp are proposed. Results show that the proposed volume parameter Vmp can reliably and accurately characterize the asphalt pavement texture by considering more structural properties of the road surface texture. According to the research findings of this paper, it is feasible to achieve rapid and correct assessment of asphalt pavement adhesion using 3D laser detection technology by comprehensively considering the 3D characteristics of the road surface texture.

A seven-year-old healthy female Chow Chow was referred for pregnancy monitoring. Ultrasonography was used to evaluate all pregnancy and fetus parameters, and they were found to be normal. During the examination of the 42 day pregnant bitch, an unusual mass was seen in a fetus’s heart. This fetus had a cardiac frequency of 273–300 beats, while the others had heart rates of 220–240 beats. Natural vaginal birth occurred at 63 days pregnant: the first two puppies were stillborn but perfectly formed, and the other three were alive and had optimal APGAR. In one of two deceased puppies, an unusual, reddish, smooth mass occupying the space in the heart was found through necroscopy. The organ was submitted for histological examination. Histopathology, immunohistochemical, and histochemical analyses all indicated a cardiac tumor with increased Purkinje cells. This type of tumor has been described in infants, swine, bearded seals, and deer but never in fetuses and neonates of dogs. To our knowledge, this is the first such case reported in veterinary medicine.

Seed priming is routinely applied to improve germination rates and seedling establishment, but the decrease in longevity observed in primed seeds constitutes a major drawback that compromises long-term storability. The optimization of priming protocols able to preserve primed seeds from aging processes represents a promising route to expand the scope of seed priming. The present work explores this possibility in the model legume Medicago truncatula by testing the effectiveness of quercetin- and rutin-supplemented seed priming at improving the response to subsequent artificial aging. In comparison with a non-supplemented hydropriming protocol, supplementation with quercetin or rutin was able to mitigate the effects of post-priming aging by increasing germination percentage and speed, improving seed viability and seedling phenotype, with consistent correlations with a decrease in the levels of reactive oxygen species and an increase in antioxidant potential. The results suggest that quercetin and rutin can reduce the effects of post-priming aging by improving the seed antioxidant profiles. The present work provides novel information to explore the physiological changes associated with seed priming and aging, with possible outcomes for the development of tailored vigorization protocols able to overcome the storability constrains associated with post-priming aging processes.

The application of sound wave technology to different plant species has revealed that variations in the Hz, sound pressure intensity, treatment duration, and type of setup of the sound source significantly impact the plant performance. A study conducted on cotton plants treated with Plant Acoustic Frequency Technology (PAFT) highlighted improvements across various growth metrics. In particular, the treated samples showed increases in the height, size of the fourth expanded leaf from the final one, count of branches carrying bolls, quantity of bolls, and weight of individual bolls. Another study showed how the impact of a 4 kHz sound stimulus positively promoted plant drought tolerance. In other cases, such as in transgenic rice plants, GUS expression was upregulated at 250 Hz but downregulated at 50 Hz. In the same way, sound frequencies have been found to enhance the osmotic potential, with the highest observed in samples treated with frequencies of 0.5 and 0.8 kHz compared to the control. Furthermore, a sound treatment with a frequency of 0.4 kHz and a sound pressure level (SPL) of 106 dB significantly increased the paddy rice germination index, as evidenced by an increase in the stem height and relative fresh weight. This paper presents a complete, rationalized and updated review of the literature on the effects of sound waves on the physiology and growth parameters of sound-treated plants.

The purpose of this review is to focus on the diagnostic endoscopic ultrasound of the gastrointestinal tract. In the last decades, EUS has gained a central role in the staging of epithelial and sub-epithelial lesions of the gastrointestinal tract. With the evolution of imaging, the position of EUS in the diagnostic work-up and the staging flow-chart has continuously changed with two extreme positions: some gastroenterologists think that EUS is absolutely indispensable, and some think it is utterly useless. The truth is, as always, somewhere in between the two extremes. Analyzing the most up-to-date and strong evidence, we will try to give EUS the correct position in our daily practice.

The real-world, retrospective, NEROnE registry investigated the impact of next-generation sequencing (NGS) in advanced non-small-cell lung cancer (NSCLC) patients (pts) at three oncology units in the north of Italy between January 2020 and December 2022. We focused on the clinical characterization and outcomes of NSCLC with rare molecular alterations: EGFR exon 20 insertion, non-activating EGFR mutations, BRAF V600E and non-V600, ROS1 and RET rearrangements, MET, ErbB2, and FGFR mutations. Overall, these represented 6.4% (62/970) of the pts analysed with NGS in the daily practice. The most heavily represented rare alterations were ROS1 rearrangement (15 pts—24%) and MET exon 14 skipping mutation (11 pts—18%). No associations were found with the demographic and clinical features. Forty-nine pts received targeted therapies, of which 38.8% were first- and 9.8% were second-line. The remaining pts received chemotherapy and/or immunotherapy. In terms of the clinical outcomes, although not statistically significant, a tendency toward shorter OS was seen when therapies other than specific targeted therapies were used (HR: 1.84, 95% CI: 0.79–4.33, p = 0.158). The pts with co-mutations (19.4%) seemed to receive an advantage from the front-line chemotherapy-based regimen. Finally, an NLR score (a well-known inflammatory index) ≥ 4 seemed to be related to shorter OS among the pts treated with immunotherapy alone or in combination with chemotherapy (HR: 2.83, 95% CI: 1.08–7.40, p = 0.033). Prospective evaluations need to be performed to clarify whether these indexes may help to identify patients with oncogene-addicted NSCLC who could benefit from immunotherapy.

Background/Objectives: Osteoarthritis (OA) represents the most frequent chronic joint disease worldwide. Facing an aging population, resulting from the demographic change, the number of primary total hip arthroplasties (THA) will further increase. Although the geriatric patient strongly differs from the younger one, the current literature on elective orthopedic surgery in the geriatric patient is scarce. This work analyses, whether geriatric patients receiving primary THA significantly improve in terms of their (1) mobility and functional outcome and (2) health-related quality of life at four to six weeks as well as three months postoperatively. Methods: In a prospective study design, we analyzed 101 geriatric patients with osteoarthritis of the hip receiving primary THA. The study is part of the ongoing “Special Orthopaedic Geriatrics” (SOG) trial, which is funded by the German Federal Joint Committee (GBA). In addition to a preoperative comprehensive geriatric assessment (CGA), the Western Ontario and McMaster Universities Arthritis Index (WOMAC) and the EQ5D-5L were imposed preoperatively (t0), at four to six weeks (t1), and at three months (t2) postoperatively. Results: The 101 enrolled patients had a mean age of 78.1 ± 4.9 years. The total WOMAC score and almost all subcategories significantly improved at four to six weeks as well as three months postoperatively in comparison to the preoperative results (p < 0.001). The same was observed for the EQ-5D-5L, showing significant improvement in overall health at both time points (p < 0.001) and all subcategories (p < 0.05). Conclusions: This study implies that a geriatric patient benefits as much from elective primary THA as a younger patient. However, the preoperative comprehensive geriatric assessment with screening for risk factors is of utmost importance. Regarding the aging population, a lot of effort is needed to obtain more knowledge about geriatric patients receiving elective orthopedic surgery.

SATB1 (MIM #602075) is a relatively new gene reported only in recent years in association with neurodevelopmental disorders characterized by variable facial dysmorphisms, global developmental delay, poor or absent speech, altered electroencephalogram (EEG), and brain abnormalities on imaging. To date about thirty variants in forty-four patients/children have been described, with a heterogeneous spectrum of clinical manifestations. In the present study, we describe a new patient affected by mild intellectual disability, speech disorder, and non-specific abnormalities on EEG and neuroimaging. Family studies identified a new de novo frameshift variant c.1818delG (p.(Gln606Hisfs*101)) in SATB1. To better define genotype–phenotype associations in the different types of reported SATB1 variants, we reviewed clinical data from our patient and from the literature and compared manifestations (epileptic activity, EEG abnormalities and abnormal brain imaging) due to missense variants versus those attributable to loss-of-function/premature termination variants. Our analyses showed that the latter variants are associated with less severe, non-specific clinical features when compared with the more severe phenotypes due to missense variants. These findings provide new insights into SATB1-related disorders.

Due to limited infrastructure and remote locations, rural areas often need help providing reliable and high-quality network connectivity. We propose an innovative approach that leverages Software-Defined Wide Area Network (SD-WAN) architecture to enhance reliability in such challenging rural scenarios. Our study focuses on cases in which network resources are limited to network solutions such as Long-Term Evolution (LTE) and a Low-Earth-Orbit satellite connection. The SD-WAN implementation compares three tunnel selection algorithms that leverage real-time network performance monitoring: Deterministic, Random, and Deep Q-learning. The results offer valuable insights into the practical implementation of SD-WAN for rural connectivity scenarios, showing its potential to bridge the digital divide in underserved areas.

This paper presents the design, development, and validation of a novel e-textile leg sleeve for non-invasive Surface Electromyography (sEMG) monitoring. This wearable device incorporates e-textile sensors for sEMG signal acquisition from the lower limb muscles, specifically the anterior tibialis and lateral gastrocnemius. Validation was conducted by performing a comparative study with eleven healthy volunteers to evaluate the performance of the e-textile sleeve in acquiring sEMG signals compared to traditional Ag/AgCl electrodes. The results demonstrated strong agreement between the e-textile and conventional methods in measuring descriptive metrics of the signals, including area, power, mean, and root mean square. The paired data t-test did not reveal any statistically significant differences, and the Bland–Altman analysis indicated negligible bias between the measures recorded using the two methods. In addition, this study evaluated the wearability and comfort of the e-textile sleeve using the Comfort Rating Scale (CRS). Overall, the scores confirmed that the proposed device is highly wearable and comfortable, highlighting its suitability for everyday use in patient care.

Accidents caused by Bothrops jararaca (Bj) snakes result in several local and systemic manifestations, with pain being a fundamental characteristic. The inflammatory process responsible for hyperalgesia induced by Bj venom (Bjv) has been studied; however, the specific roles played by the peripheral and central nervous systems in this phenomenon remain unclear. To clarify this, we induced hyperalgesia in rats using Bjv and collected tissues from dorsal root ganglia (DRGs) and spinal cord (SC) at 2 and 4 h post-induction. Samples were labeled for Iba-1 (macrophage and microglia), GFAP (satellite cells and astrocytes), EGR1 (neurons), and NK1 receptors. Additionally, we investigated the impact of minocycline, an inhibitor of microglia, and GR82334 antagonist on Bjv-induced hyperalgesia. Our findings reveal an increase in Iba1 in DRG at 2 h and EGR1 at 4 h. In the SC, markers for microglia, astrocytes, neurons, and NK1 receptors exhibited increased expression after 2 h, with EGR1 continuing to rise at 4 h. Minocycline and GR82334 inhibited venom-induced hyperalgesia, highlighting the crucial roles of microglia and NK1 receptors in this phenomenon. Our results suggest that the hyperalgesic effects of Bjv involve the participation of microglial and astrocytic cells, in addition to the activation of NK1 receptors.

Concern has emerged about the prevalence of second cancers among patients with hairy cell leukemia (HCL) treated with purine analogs. We investigated 513 patients with HCL treated with cladribine over the last 30 years at 18 Italian centers and calculated their standardized incidence ratios (SIRs). We identified 24 patients with a second cancer diagnosed at a median time from treatment with cladribine of 59.9 months (range: 9.2–169.7 months). All patients with solid neoplasms presented with a limited-stage disease, except four cases of locally advanced cancer; multiple myeloma patients had a smoldering disease, while lymphoma patients had stage Ie and stage IV diseases. Response to therapy was complete in 19 cases; 1 patient is still receiving treatment for a relapsing bladder disease, while 2 patients progressed during treatment and died. These two patients died from unrelated causes: one from infection and one due to surgery complications. The median OS from HCL was 98.5 months (range: 38.4–409.2 months), while the median OS from second cancer was 27.6 months (range: 1–117.8 months). The SIR was 0.86 (95% CI: 0.54–1.30) for males and 1.13 (95% CI: 0.36–2.73) for females: no statistically significant differences were highlighted. We were not able to demonstrate an excess of second cancer or a significant association with the specific studied neoplasm.

The “red code” (RC) represents the highest level of emergency in the emergency department (ED). This study retrospectively analyzed RCs in the Regina Margherita Children’s Hospital ED, a regional referral center in north Italy, between 1 July 2020 and 30 June 2023. The aim was to describe RC characteristics and to identify significant correlations between presenting complaints and clinical management. The study includes 934 RCs (0.9% of overall ED admissions); 64% were assigned based on the Pediatric Assessment Triangle alteration. Most patients, 86.5%, followed the medical pathway, while 13.5% were surgical cases. Admission complaints were respiratory (46.9%), neuropsychiatric (26.7%), traumatic (11.8%), cardiologic (9.3%), metabolic (3.8%), and surgical (1.5%). Seventy-six percent of patients received vascular access, and intraosseous access was obtained in 2.2% of them. In one-third of RCs, an urgent critical care evaluation was necessary, and 19% of cases required admission to the intensive care unit. The overall mortality rate was 3.4% (0.4% in ED setting). The study identified six distinct diagnostic pathways, each associated with specific characteristics in clinical presentation, management, therapeutic interventions, and outcomes. Our findings underscore the need for a systematic approach in pediatric emergency settings, supported by international and national guidelines but also by clearly defined diagnostic pathways, aiming to enhance the quality of care and patient outcomes.

Substantial amounts of pesticides, used in agricultural production to control pests, diseases, and weeds, and thereby attain high product quantities and quality, can severely affect the ecosystem and human health. The amounts of pesticides used depend on the specifics of the current production system but also exhibit large effects of past practices. Pesticides do not act only on the target organisms but also on organisms for which the chemicals were not specifically formulated, constituting hazardous molecules for humans and the environment. Pesticides, therefore, also influence soil microbial communities including organisms that engage in mutualistic plant symbioses that play a crucial role in its mineral nutrition, such as arbuscular mycorrhizal fungi. In this review, we summarize the current knowledge on the effects of synthetic and natural (‘green’) pesticides (fungicides, herbicides, and insecticides) on arbuscular mycorrhizal symbiosis. We deal with both the direct effects (spore germination and extraradical and intraradical growth of the mycelium) and indirect effects on the agroecosystem level. Such indirect effects include effects through the spread of herbicide-resistant crops and weeds to neighboring ecosystems, thereby modifying the mycorrhizal inoculum potential and altering the plant–plant interactions. We also briefly discuss the possibility that mycorrhizal plants can be used to enhance the phytoremediation of organic pesticides.

The paper presents the first archaeometric results on a selection of ancient Roman coins (antoniniani) from a treasure found in the Roman necropolis area of ancient Cumae (Southern Italy) during archaeological campaigns by the Centre Jean Bérard. A multi-analytical approach consisting of non-destructive techniques (stereomicroscopy, FESEM-EDS, pXRF, Raman, μ-CT) was implemented on the better-preserved coins of the treasure in order to investigate the chemical composition of the alloy. Chemical analysis showed that the Roman mint prepared the metal with an argentiferous lead–bronze alloy. A group of coins presents a low content of the precious metal, which is in agreement with the financial crisis of the Roman Empire of the third century. Another group of two coins shows a higher silver content, which is in agreement with their issue or with the Mediolanum mint standards. In addition, the external layers of corroded coins were analysed to explore the state of conservation of the patina and to identify the corrosion products. Some typical corrosion patinas due to post-depositional conditions were identified. The mineralogical characterisation of the corrosion products enables us to implement adequate conservation strategies, and the presence of more aggressive patinas suggests immediate interventions.

Silybin is a natural compound extensively studied for its hepatoprotective, neuroprotective and anticancer properties. Envisioning the enhancement of silybin potential by suitable modifications in its chemical structure, here, a series of new 7-O-alkyl silybins derivatives were synthesized by the Mitsunobu reaction starting from the silybins and tyrosol-based phenols, such as tyrosol (TYR, 3), 3-methoxytyrosol (MTYR, 4), and 3-hydroxytyrosol (HTYR, 5). This research sought to explore the antioxidant and anticancer properties of eighteen new derivatives and their mechanisms. In particular, the antioxidant properties of new derivatives outlined by the DPPH assay showed a very pronounced activity depending on the tyrosyl moiety (HTYR > MTYR >> TYR). A significant contribution of the HTYR moiety was observed for silybins and 2,3-dehydro-silybin-based derivatives. According to the very potent antioxidant activity, 2,3-dehydro-silybin derivatives 15ab, 15a, and 15b exerted the most potent anticancer activity in human prostate cancer PC-3 cells. Furthermore, flow cytometric analysis for cell cycle and apoptosis revealed that 15ab, 15a, and 15b induce strong G1 phase arrest and increase late apoptotic population in PC-3 cells. Additionally, Western blotting for apoptotic marker cleaved caspase-3 confirmed apoptosis induction by these silybin derivatives in PC-3 cells. These findings hold significant importance in the investigation of anticancer properties of silybin derivatives and strongly encourage swift investigation in pre-clinical models and clinical trials.

An excess of ozone (O3) is currently stressing plant ecosystems and may negatively affect the nutrient use of plants. Plants may modify leaf turnover rates and nutrient allocation at the organ level to counteract O3 damage. We investigated leaf turnover rate and allocation of primary (C, N, P, K) and secondary macronutrients (Ca, S, Mg) under various O3 treatments (ambient concentration, AA, with a daily hourly average of 35 ppb; 1.5 × AA; 2.0 × AA) and fertilization levels (N: 0 and 80 kg N ha−1 y−1; P: 0 and 80 kg N ha−1 y−1) in an O3-sensitive poplar clone (Oxford: Populus maximowiczii Henry × P. berolinensis Dippel) in a Free-Air Controlled Exposure (FACE) experiment. The results indicated that both fertilization and O3 had a significant impact on the nutrient content. Specifically, fertilization and O3 increased foliar C and N contents (+5.8% and +34.2%, respectively) and root Ca and Mg contents (+46.3% and +70.2%, respectively). Plants are known to increase the content of certain elements to mitigate the damage caused by high levels of O3. The leaf turnover rate was accelerated as a result of increased O3 exposure, indicating that O3 plays a main role in influencing this physiological parameter. A PCA result showed that O3 fumigation affected the overall allocation of primary and secondary elements depending on the organ (leaves, stems, roots). As a conclusion, such different patterns of element allocation in plant leaves in response to elevated O3 levels can have significant ecological implications.

This review focuses on ablative techniques for small renal masses (SRMs), including radiofrequency ablation (RFA), cryoablation (CA), microwave ablation (MWA), and irreversible electroporation (IRE), and discusses recurrence management. Through an extensive literature review, we outline the procedures, outcomes, and follow-up strategies associated with each ablative method. The review provides a detailed examination of these techniques—RFA, CA, MWA, and IRE—elucidating their respective outcomes. Recurrence rates vary among them, with RFA and CA showing comparable rates, MWA demonstrating favorable short-term results, and IRE exhibiting promise in experimental stages. For managing recurrences, various strategies are considered, including active surveillance, re-ablation, or salvage surgery. Surveillance is preferred post-RFA and post-CA, due to slow SRM growth, while re-ablation, particularly with RFA and CA, is deemed feasible without additional complications. Salvage surgery emerges as a viable option for larger or resistant tumors. While ablative techniques offer short-term results comparable to surgery, further research is essential to understand their long-term effects fully. Decisions concerning recurrence management should consider individual and tumor-specific factors. Imaging, notably contrast-enhanced ultrasounds, plays a pivotal role in assessing treatment success, emphasizing the necessity of a multidisciplinary approach for optimal outcomes. The lack of randomized trials highlights the need for further research.

Renal cell carcinoma (RCC) remains a formidable diagnostic challenge, especially in the context of small renal masses. The quest for non-invasive screening tools and biomarkers has steered research towards liquid biopsy, focusing on microRNAs (miRNAs), exosomes, and circulating tumor cells (CTCs). MiRNAs, small non-coding RNAs, exhibit notable dysregulation in RCC, offering promising avenues for diagnosis and prognosis. Studies underscore their potential across various biofluids, including plasma, serum, and urine, for RCC detection and subtype characterization. Encouraging miRNA signatures show correlations with overall survival, indicative of their future relevance in RCC management. Exosomes, with their diverse molecular cargo, including miRNAs, emerge as enticing biomarkers, while CTCs, emanating from primary tumors into the bloodstream, provide valuable insights into cancer progression. Despite these advancements, clinical translation necessitates further validation and standardization, encompassing larger-scale studies and robust evidence generation. Currently lacking approved diagnostic assays for renal cancer, the potential future applications of liquid biopsy in follow-up care, treatment selection, and outcome prediction in RCC patients are profound. This review aims to discuss and highlight recent advancements in liquid biopsy for RCC, exploring their strengths and weaknesses in the comprehensive management of this disease.
