Background: The research aimed to investigate the incidence of SARS-CoV-2 breakthrough infections and their determinants in a large European cohort of more than 60,000 health workers. Methods: A multicentric retrospective cohort study, involving 12 European centers, was carried out within the ORCHESTRA project, collecting data up to 18 November 2021 on fully vaccinated health workers. The cumulative incidence of SARS-CoV-2 breakthrough infections was investigated with its association with occupational and social–demographic characteristics (age, sex, job title, previous SARS-CoV-2 infection, antibody titer levels, and time from the vaccination course completion). Results: Among 64,172 health workers from 12 European health centers, 797 breakthrough infections were observed (cumulative incidence of 1.2%). The primary analysis using individual data on 8 out of 12 centers showed that age and previous infection significantly modified breakthrough infection rates. In the meta-analysis of aggregated data from all centers, previous SARS-CoV-2 infection and the standardized antibody titer were inversely related to the risk of breakthrough infection (p = 0.008 and p = 0.007, respectively). Conclusion: The inverse correlation of antibody titer with the risk of breakthrough infection supports the evidence that vaccination plays a primary role in infection prevention, especially in health workers. Cellular immunity, previous clinical conditions, and vaccination timing should be further investigated.

In the last decade, several exoskeletons for shoulder rehabilitation have been presented in the literature. Most of these devices focus on the shoulder complex and limit the normal mobility of the rest of the body, forcing the patient into a fixed standing or sitting position. Nevertheless, this severely limits the range of activities that can potentially be simulated during the rehabilitation, preventing the execution of occupational therapy which involves the execution of tasks based on activities of daily living (ADLs). These tasks involve different muscular groups and whole-body movements, such as, e.g., picking up objects from the ground. To enable whole-body functional rehabilitation, the challenge is to shift the paradigm of robotic rehabilitation towards machines that can enable wide workspaces and high mobility. In this perspective, here we present Float: an upper-limb exoskeleton designed to promote and accelerate the motor and functional recovery of the shoulder joint complex following post-traumatic or post-surgical injuries. Indeed, Float allows the patient to move freely in a very large workspace. The key component that enables this is a passive polyarticulated arm which supports the total exoskeleton weight and allows the patient to move freely in space, empowering rehabilitation through a deeper interaction with the surrounding environment. A characterization of the reachable workspace of both the exoskeleton and the polyarticulated passive arm is presented. These results support the conclusion that a patient wearing Float can perform a wide variety of ADLs without bearing its weight.

Our aim is evaluating the changes in weight and dietary habits in a sample of outpatients with obesity after 1 month of enforced lockdown during the COVID-19 pandemic in Northern Italy. In this observational retrospective study, the patients of our Obesity Unit were invited to answer to a 12-question multiple-choice questionnaire relative to weight changes, working activity, exercise, dietary habits, and conditions potentially impacting on nutritional choices. A multivariate regression analysis was performed to evaluate the associations among weight/BMI changes and the analyzed variables. A total of 150 subjects (91.5%) completed the questionnaire. Mean self-reported weight gain was ≈1.5 kg (p < 0.001). Lower exercise, self-reported boredom/solitude, anxiety/depression, enhanced eating, consumption of snacks, unhealthy foods, cereals, and sweets were correlated with a significantly higher weight gain. Multiple regression analyses showed that increased education (inversely, β = −1.15; 95%CI −2.13, −0.17, p = 0.022), self-reported anxiety/depression (β = 1.61; 0.53, 2.69, p = 0.004), and not consuming healthy foods (β = 1.48; 0.19, 2.77, p = 0.026) were significantly associated with increased weight gain. The estimated direct effect of self-reported anxiety/depression on weight was 2.07 kg (1.07, 3.07, p < 0.001). Individuals with obesity significantly gained weight 1 month after the beginning of the quarantine. The adverse mental burden linked to the COVID-19 pandemic was greatly associated with increased weight gain.

Accurate tracking and forecasting of ash dispersal in the atmosphere and quantification of its uncertainty are of fundamental importance for volcanic risk mitigation. Numerical models and satellite sensors offer two complementary ways to monitor ash clouds in real time, but limits and uncertainties affect both techniques. Numerical forecasts of volcanic clouds can be improved by assimilating satellite observations of atmospheric ash mass load. In this paper, we present a data assimilation procedure aimed at improving the monitoring and forecasting of volcanic ash clouds produced by explosive eruptions. In particular, we applied the Local Ensemble Transform Kalman Filter (LETKF) to the results of the Volcanic Ash Transport and Dispersion model HYSPLIT. To properly simulate the release and atmospheric transport of volcanic ash particles, HYSPLIT has been initialized with the results of the eruptive column model PLUME-MoM. The assimilation procedure has been tested against SEVIRI measurements of the volcanic cloud produced during the explosive eruption occurred at Mt. Etna on 24 December 2018. The results show how the assimilation procedure significantly improves the representation of the current ash dispersal and its forecast. In addition, the numerical tests show that the use of the sequential Ensemble Kalman Filter does not require a precise initialization of the numerical model, being able to improve the forecasts as the assimilation cycles are performed.

The development of robotic devices for rehabilitation is a fast-growing field. Nowadays, thanks to novel technologies that have improved robots’ capabilities and offered more cost-effective solutions, robotic devices are increasingly being employed during clinical practice, with the goal of boosting patients’ recovery. Robotic rehabilitation is also widely used in the context of neurological disorders, where it is often provided in a variety of different fashions, depending on the specific function to be restored. Indeed, the effect of robot-aided neurorehabilitation can be maximized when used in combination with a proper training regimen (based on motor control paradigms) or with non-invasive brain machine interfaces. Therapy-induced changes in neural activity and behavioral performance, which may suggest underlying changes in neural plasticity, can be quantified by multimodal assessments of both sensorimotor performance and brain/muscular activity pre/post or during intervention. Here, we provide an overview of the most common robotic devices for upper and lower limb rehabilitation and we describe the aforementioned neurorehabilitation scenarios. We also review assessment techniques for the evaluation of robotic therapy. Additional exploitation of these research areas will highlight the crucial contribution of rehabilitation robotics for promoting recovery and answering questions about reorganization of brain functions in response to disease.

Conventional myoelectric prosthetic hands only offer a basic tri-digital pinch. Transradial amputees need to compensate for this lack of function with altered kinematics at the shoulder and trunk that might expose them to an increased risk of musculoskeletal injuries. A poly-articulated prosthetic hand may reduce the physical compensatory movements and close the gap between the sound and the prosthetic side. Six male transradial amputees completed four standardized reach-and-grasp activities with their tri-digital, poly-articulated and sound side hands. Trunk, shoulder girdle, scapula and humerus kinematics were measured with an optoelectronic system. Differences between hands were analyzed in terms of the amplitude of motion, the duration of the altered kinematics over the motion cycle, peak-to-peak amplitude and time to complete the activity. An overall score was defined, which assigned three points when the kinematics of a joint angle was altered for over 41% of the motion cycle, two points between 11 ÷ 40% and one point between 1 ÷ 10%; thus, a lower score indicates less variation from normal kinematics. Despite no changes in times, tri-digital vs. sound hand scored 93 points, tri-digital vs. poly-articulated hands scored 49 and poly-articulated vs. sound hand scored 28, supporting the hypotheses of the poly-articulated hand positively affects shoulder and trunk kinematics.

Powered Lower Limb Exoskeletons (PLLE) have attracted much interest due to their potential applications. They provide assistance for persons with disabilities to accomplish activities of daily living (ADL), and more importantly, assist them in achieving their rehabilitation goals. However, there is still uncertainty regarding the quality and benefits that PLLEs can offer to patients. This is due to limited usability and performance of current PLLEs, insufficient clinical use of PLLEs for different patients with high diversity in their disability type and impairment, and also the large gap between the technological state of the art and clinical expectations. In this study, we review and analyse various factors that can improve the effectiveness of PLLEs at yielding better assistance and rehabilitation training for patients with motor impairments. First, we define a set of criteria that characterize the majority of expectations for the rehabilitation and assistance domains and we use them for evaluating PLLEs depending on the context. Then, we include the effects of control strategies and combined approaches which include auxiliary devices such as functional electrical stimulation and smart crutches applied to PLLEs with regard to the criteria we defined.

Acute myocardial edema (AME) is increased water content in the myocardium and represents the first and transient pathophysiological response to an acute myocardial injury. In-vivo and non-invasive evaluation is feasible with cardiac magnetic resonance (CMR), which is a powerful imaging technique capable of tissue characterization. In the clinical setting, early demonstration of AME has a recognized diagnostic value for acute coronary syndromes and acute myocarditis, although its prognostic value is not well established. This article provides a comprehensive narrative review on the clinical meaning of AME in heart diseases. In particular, the available evidence of a possible favourable prognostic value in several clinical scenarios is addressed.

Pulmonary arterial hypertension (PAH) is a life-threatening complication of connective tissue diseases (CTDs) characterised by increased pulmonary arterial pressure and pulmonary vascular resistance. CTD-PAH is the result of a complex interplay among endothelial dysfunction and vascular remodelling, autoimmunity and inflammatory changes, ultimately leading to right heart dysfunction and failure. Due to the non-specific nature of the early symptoms and the lack of consensus on screening strategies—except for systemic sclerosis, with a yearly transthoracic echocardiography as recommended—CTD-PAH is often diagnosed at an advanced stage, when the pulmonary vessels are irreversibly damaged. According to the current guidelines, right heart catheterisation is the gold standard for the diagnosis of PAH; however, this technique is invasive, and may not be available in non-referral centres. Hence, there is a need for non-invasive tools to improve the early diagnosis and disease monitoring of CTD-PAH. Novel serum biomarkers may be an effective solution to this issue, as their detection is non-invasive, has a low cost and is reproducible. Our review aims to describe some of the most promising circulating biomarkers of CTD-PAH, classified according to their role in the pathophysiology of the disease.

Numerical modelling of tephra fallout is a fast-developing research area in volcanology. Several models are currently available both to forecast the dispersion of volcanic particles in the atmosphere and to calculate the particles deposited at different locations on the ground. Data from these simulations can then be used both to manage volcanic crises (e.g., protect air traffic) or perform long-term hazard assessment studies (e.g., through hazard maps). Given the importance of these tasks, it is important that each model is thoroughly tested in order to assess advantages and limitations, and to provide useful information for quantifying the model uncertainty. In this study we tested the coupled PLUME-MoM/HYSPLIT models by applying them to the Puyehue–Cordon Caulle 2011 sub-Plinian eruption. More specifically, we tested new features recently introduced in these well-established models (ash aggregation, external water addition, and settling velocity models), we implemented a new inversion procedure, and we performed a parametric analysis. Our main results reaffirm the pivotal role played by mass eruption rate on the final deposit and show that some choices for the input parameters of the model can lead to the large overestimation in total deposited mass (which can be reduced with our inversion procedure). The parametric analysis suggests a most likely value of the mass eruption rate in the range 2.0–6.3 × 106 kg/s. More studies with a similar approach would be advisable in order to provide final users with useful indications about the parameters that should be carefully evaluated before being used as input for this kind of model.
