Background: The aim of the study was to investigate a series of complete blood cell count-based biomarkers of systemic inflammation as predictors of clinical outcomes in patients who underwent first-line chemoimmunotherapy for advanced NSCLC. Methods: Consecutive patients with pathologically diagnosed stage III/IV NSCLC and PD-L1 < 50% who underwent first-line chemoimmunotherapy were retrospectively enrolled. The clinical outcomes used for biomarker evaluation were Objective Response Rate (ORR) and Overall Survival (OS). Results: Non-responders had significantly higher values of neutrophil to lymphocyte ratio (NLR, median: 5.36; IQR: 2.78–10.82 vs. 3.31; IQR: 2.15–4.12, p = 0.019), neutrophil to monocyte ratio (NMR, median: 14.00; IQR: 8.82–21.20 vs. 9.20; IQR: 7.45–11.20, p = 0.013), and systemic inflammation index (SII, median: 1395; IQR: 929–3334 vs. 945; IQR: 552–1373, p = 0.025), but only NLR and NMR remained independently associated with clinical response in multivariate logistic regression. In the univariate analysis, white blood cells (OR:1.2202; 95% CI: 1.0339–1.4400, p = 0.019), neutrophils (OR:1.2916; 95% CI: 1.0692–1.5604, p = 0.008), NLR (OR:1.3601: 95% CI: 1.0949–1.6896, p = 0.005) and NMR (OR:1.2159; 95% CI: 1.00396–1.4221, p = 0.015) were significantly associated with survival; Cox regression models confirmed that neutrophils, NLR, and MLR were independently associated with survival; NLR, at a cut-off value of 4.0, showed the better AUC (0.749) in predicting OS. Conclusions: Baseline complete blood cell count biomarkers, especially the NLR, can predict clinical outcomes in patients with advanced NSCLC treated with first-line chemoimmunotherapy. Neuroinflammation, a pivotal factor in the pathogenesis of various brain disorders, including neurodegenerative diseases, has become a focal point for therapeutic exploration. This review highlights neuroinflammatory mechanisms that hallmark neurodegenerative diseases and the potential benefits of essential oils in counteracting neuroinflammation and oxidative stress, thereby offering a novel strategy for managing and mitigating the impact of various brain disorders. Essential oils, derived from aromatic plants, have emerged as versatile compounds with a myriad of health benefits. Essential oils exhibit robust antioxidant activity, serving as scavengers of free radicals and contributing to cellular defense against oxidative stress. Furthermore, essential oils showcase anti-inflammatory properties, modulating immune responses and mitigating inflammatory processes implicated in various chronic diseases. The intricate mechanisms by which essential oils and phytomolecules exert their anti-inflammatory and antioxidant effects were explored, shedding light on their multifaceted properties. Notably, we discussed their ability to modulate diverse pathways crucial in maintaining oxidative homeostasis and suppressing inflammatory responses, and their capacity to rescue cognitive deficits observed in preclinical models of neurotoxicity and neurodegenerative diseases. The first step in reintroducing plastic waste into the recycling cycle is to use material recovery facilities (MRFs). However, while the composition and types of plastic waste are changing over time, the layout of MRFs does not always adapt to this change. In this paper, an existing MRF in Southern Italy was chosen as a reference to evaluate its current performance and to estimate possible improvements in sorting through a specific upgrade. First, an analysis of the amount, composition, and sources (in terms of type of waste and distance from the MRF) of the input waste was conducted. The composition of the input waste was then compared with the amount of selected output waste streams in order to calculate the current sorting efficiency of each stream and compare it with the values obtained from the upgrade. Lastly, the current performance of the plant was compared with a previous assessment of the same MRF in order to highlight possible variation. Results showed how the incoming waste was mainly composed of packaging plastic waste, and that some plastic waste not yet selected by the plant ended up in specific output streams. Therefore, the current performance of the MRF resulted high for PET and PE bottles (80.2% and 92.8%, respectively), in contrast to mixed or flexible packaging, where the efficiency achieved lower values (55–50%). These values were caused by a weakness in the 2D flow sorting line, which the upgrade mostly addressed. The upgraded configuration increased the production of recyclable waste from 34.32% to 50.39%, especially due to the recovery of small flexible packaging films in PE and biopolymers. In 2014, UNAIDS launched renewed global targets for HIV control to achieve by 2025, known as "the three 95": 95% of people living with HIV (PWH) diagnosed, of which 95% are receiving treatment, of which 95% are on sustained virological suppression. In Italy, new HIV diagnoses have been steadily decreasing since 2012. However, in 2020, 41% of new diagnoses presented with less than 200 CD4+ cells/µL and 60% with less than 350 CD4+ cells/µL. Implementing testing and early treatment is a key strategy to prevent AIDS, late presentation, and HIV transmission. We selected non-Infectious Diseases Units based on the European project HIDES and engaged colleagues in a condition-guided HIV screening strategy. We enrolled 300 patients, of which 202 were males (67.3%) and 98 were females (32.7%). Most of the screening was performed in Infectious Diseases (ID) and Hematologic wards. In total, we diagnosed eleven new HIV infections with a hospital prevalence in the study population of 3.7%. Five (45.4%) had a CD4 count <100/mm3, one (9.1%) <200/mm3, and one (9.1%) <300/mm3. Regarding risk factors, 81.8% declared having had unprotected sexual intercourse and 54.5% were heterosexual. All patients promptly started a combination antiretroviral regimen and 10 (90.9%) obtained an undetectable HIV-RNA status. Eight of the eleven (72.7%) patients are currently on follow-up in our outpatient clinic. A proactive indicator disease-guided screening can help avoid missed opportunities to diagnose HIV infection in a hospital setting. This paper proposes an energy management system based on an Artificial Neural Network (ANN) to be integrated with the standard ISO 50001 and aims to describe the definition and the enhancement of the energy baselines by means of artificial intelligence techniques applied and tested on the real electrical absorption data of the auxiliary units of different thermal power plants in Italy. Power plant optimized operations are important both for cost and energy performance reasons with related effects on the environment in the next future energy transition scenario. The improvement of the energy baselines consists in determining more accurate consumption monitoring models that are able to track inefficiencies and absorption drifts through data analytics and Artificial Intelligence. Starting from the analysis of the energy vectors at the production site level, we performed a multi-scale analysis to define the consumption at macro areas level and finally find the most relevant consumption units within the plants. A comparison of different ANNs applied to several real power plant data was performed to model complex plant architecture and optimize energy savings with respect to pre-set thresholds according to the ISO 50001 standard procedure. The energy baselines are determined through the analysis of the data available in the power plants' Distributed Control System (DCS), and we can identify the consumption derived from the unit's proper operation. Based on the reported numerical simulations, improved baselines have been reached up to a 5% threshold for different plant sub-units, thus representing a relevant overall saving in terms of alert threshold definition and related control efficiency: a potential saving of about 140 MWh throughout the considered three-year dataset was obtained taking into account a cooling tower sub-unit, representing a considerable economic benefit. The results obtained highlight the neural technique efficiency in defining more accurate energy baselines and represents a valuable tool for large energy plant asset management to face relevant energy drifts observed in the last years of plant operation. A new analytical approach to optimize the thicknesses of a two-layer absorbing structure constituted by a graphene-based composite and a polymer dielectric spacer backed by a metallic layer acting as perfect electric conductor (PEC) is proposed. The lossy sheet is made by an epoxy-based vinyl ester resin filled with graphene nanoplatelets (GNPs) characterized by known frequency spectra of the complex permittivity. The optimal thicknesses are computed at the target frequencies of 26, 28, and 39 GHz in order to obtain a –10 dB bandwidth able to cover the 5G frequency bands between 23.8 and 40 GHz. The resulting absorbing structures, having a total thickness lower than 1 mm, are excited by transverse magnetic (TM) or electric (TE) polarized plane waves and the absorption performances are computed in the 5G high frequency range. Background: Since the beginning of the SARS-CoV-2 pandemic, the ability to predict the trajectory of the disease has represented a major challenge for clinicians. There is recent evidence that complete blood cell count (CBC)-derived inflammation indexes have predictive value in COVID-19. We aimed to describe any changes in the clinical features, CBC-derived ratios, and outcomes of patients admitted to our hospital across two temporally distinct waves. Methods: We retrospectively assessed and compared the clinical characteristics and blood cell count values of patients hospitalized during the second and fourth waves of COVID-19, and explored any outcome differences in terms of the level of respiratory support required and transfer to intensive care. Results: We observed that fourth-wave patients were older, less male-predominant, and carried more comorbidities compared to the second-wave patients but, nevertheless, experienced more favorable outcomes. A strong internal correlation was documented for both waves between outcomes and CBC-derived ratios, with the fourth-wave cases displaying lower admission values of the neutrophil-to-lymphocyte ratio (NLR), derived NLR (dNLR), platelet-to-lymphocyte ratio (PLR), and systemic inflammation index (SII). No significant differences were found for lymphocyte-to-monocyte ratio (LMR), systemic inflammation response index (SIRI), and aggregate index of systemic inflammation (AISI). Conclusions: We observed that both admission values of CBC-derived indexes and adverse respiratory outcomes decreased from the second to the fourth wave of COVID-19. These data represent a contribution to the existing knowledge on the role of CBC-derived indexes as a potential tool to help clinicians to quickly differentiate in-hospital patients at increased risk of serious illness and death. This article aims to provide a review of major challenges and research needs for the diffusion of conservation agriculture (CA) and the improvement of crop–soil–water conditions in Southern Europe and Northern Africa. A multidisciplinary study and a participatory approach are at the basis of an international project of research and innovation action, "Research-based participatory approaches for adopting conservation agriculture in the Mediterranean Area-CAMA". It aims to understand the reasons and the research needs that limit a large CA diffusion in the Mediterranean countries. CAMA aims to provide significant advances to CA through multidisciplinary research at the field and farm scales (with main emphasis on smallholder), encompassing a socio-economic analysis of the reasons that obstacle the CA diffusion, legume crop improvement as a component of improved CA cropping systems, and a network of long-term experiments on CA and soil characteristic modification. Objectives: Our study aimed to evaluate the usefulness of Vitamin D3 (VitD3) among patients hospitalized for COVID-19. The primary endpoint was to evaluate the difference in survival rates between patients receiving and not VitD3. The secondary endpoints were to evaluate clinical outcomes, such as needing non-invasive ventilation (NIV), ICU transfer, and laboratory findings (inflammatory parameters). Methods: We conducted a retrospective, monocentric matched-cohort study, including patients attending our ward for COVID-19. Patients were divided into two groups depending on VitD3 administration (Group A) or not (Group B) among patients with low VitD levels (defined as blood levels < 30 ng/mL), which depended on physicians' judgment. Our internal protocol provides VitD3 100,000 UI/daily for two days. Findings: 58 patients were included in Group A, and 58 in Group B. Patients were matched for age, sex, comorbidities, COVID-19-related symptoms, PaO2/FiO2 ratio, blood exams, and medical treatments. Regarding the principal endpoint, there was a statistically significant difference between the two groups in survival rates [Group A vs. Group B = 3 vs. 11 (p = 0.042)]. When considering secondary endpoints, Group A patients were less likely to undergo NIV [Group A vs. Group B = 12 vs. 23 (p = 0.026)] and showed an improvement in almost all inflammatory parameters. Conclusions: The link between VitD3 deficiency and the clinical course of COVID-19 during hospitalization suggests that VitD3 level is a useful prognostic marker. The role of inflammatory responses in predicting outcomes in oncological thoracic surgery is still unclear. The aim of this study was to evaluate a series of blood count inflammation indexes as predicting factors for postoperative complications. We retrospectively studied 249 patients undergoing elective thoracic surgery in our institution between 2008 and 2020. A total of 184 patients underwent open surgery, and 65 underwent VATS. The neutrophil-to-lymphocyte (NLR), monocyte-to-lymphocyte (MLR), and platelet-to-lymphocyte (PLR) ratios, Systemic Inflammation Response Index (SIRI) were calculated preoperatively and on the first and fourth postoperative days, as well as a new derivative index, the Aggregate Inflammation Systemic Index (AISI). Univariate correlations evidenced a statistically significant association between the NLR at the fourth postoperative day and the occurrence of surgical complications in the global cohort (rho = 0.15, p = 0.03). A similar significant association with MLR on the fourth postoperative day is found in the open group (rho = −0.15, p = 0.048). NLR and LMR on the fourth postoperative day are associated with postoperative complications in the whole and open groups, respectively. Simple, easy-to-perform and inexpensive, blood cell count indexes may be useful in predicting complications in oncological thoracic surgery. In this study, a novel computational method capable of reproducing hysteresis loops of hard magnetic materials is proposed. It is conceptually based on the classical Preisach model but has a completely different approach in the modeling of the hysteron effect. Indeed, the change in magnetization caused by a single hysteron is compared here to the change in velocity of two disk-shaped solids elastically colliding with each other rather than the effect of ideal geometrical entities giving rise to so-called Barkhausen jumps. This allowed us to obtain a simple differential formulation for the global magnetization equation with a significant improvement in terms of computational performance. A sensitivity analysis on the parameters of the proposed method has indeed shown the capability to model a large class of hysteresis loops. Moreover, the proposed method permits modeling of the temperature effect on magnetization of neodymium magnets, which is a key point for the design of electrical machines. Therefore, application of the proposed method to the hysteresis loop of a real NdFeB magnet has been proven to be very accurate and efficient for a large temperature range. [ABSTRACT FROM AUTHOR]

Recently, graphene-polymer composites gained a central role in advanced stress and strain sensing. A fundamental step in the production of epoxy-composites filled with graphene nanoplatelets (GNPs) consists in the exfoliation and dispersion of expanded graphite in a proper solvent, in the mixing of the resulting GNP suspension with the polymer matrix, and in the final removal of the solvent from the composite before curing through evaporation. The effects of traces of residual solvent on polymer curing process are usually overlooked, even if it has been found that even a small amount of residual solvent can affect the mechanical properties of the final composite. In this paper, we show that residual traces of N,N'-Dimethylformamide (DMF) in vinylester epoxy composites can induce relevant variations of the electrical, mechanical and electromechanical properties of the cured GNP-composite. To this purpose, a complete analysis of the morphological and structural characteristics of the composite samples produced using different solvent mixtures (combining acetone and DMF) is performed. Moreover, electrical, mechanical and electromechanical properties of the produced composites are assessed. In particular, the effect on the piezoresistive response of the use of DMF in the solvent mixture is analyzed using an experimental strain dependent percolation law to fit the measured electromechanical data. It is shown that the composites realized using a higher amount of DMF are characterized by a higher electrical conductivity and by a strong reduction of Young's Modulus. [ABSTRACT FROM AUTHOR]

Development of epoxy or epoxy-based vinyl ester composites with improved mechanical and electromagnetic properties, filled with carbon-based nanomaterials, is of crucial interest for use in aerospace applications as radar absorbing materials at radio frequency. Numerous studies have highlighted the fact that the effective functional properties of this class of polymer composites are strongly dependent on the production process, which affects the dispersion of the nanofiller in the polymer matrix and the formation of micro-sized aggregations, degrading the final properties of the composite. The assessment of the presence of nanofiller aggregation in a composite through microscopy investigations is quite inefficient in the case of large scale applications, and in general provides local information about the aggregation state of the nanofiller rather than an effective representation of the degradation of the functional properties of the composite due to the presence of the aggregates. In this paper, we investigate the mechanical, electrical, and electromagnetic properties of thermosetting polymer composites filled with graphene nanoplatelets (GNPs). Moreover, we propose a novel approach based on measurements of the dielectric permittivity of the composite in the 8-12 GHz range in order to assess the presence of nanofiller aggregates and to estimate their average size and dimensions. Mediastinal lymphadenopathy is a condition in which one or more mediastinal lymph nodes are enlarged for malignant or benign causes, generally more than 10 mm. For a long time, the only way to approach the mediastinum was surgery, while in last decades endoscopic techniques gained their role in neoplastic diseases. At the present time, EBUS is the technique of choice for studying the mediastinum in the suspicion of cancer, while there are not strong indications in guidelines for the study of benign mediastinal lymphadenopathy. We reviewed the literature, looking for evidence of the role of EBUS in the diagnostics of non-neoplastic mediastinal lymphadenopathy, with special regard for granulomatous disease, both infectious and non-infectious. EBUS is a reliable alternative to surgery in non-neoplastic mediastinal lymphadenopathy, even if more evidence is needed for granulomatous diseases other than tuberculosis and sarcoidosis. Obstructive Sleep Apnoea (OSA) is characterized by a pro-oxidant state that results from the recurrent hypoxia-reoxygenation cycles. Superoxide dismutase (SOD), a key antioxidant enzyme involved in the detoxification of superoxide radicals, could represent a reliable marker to monitor the antioxidant defences in OSA. In order to capture and critically appraise the available evidence, we performed a systematic review and meta-analysis of studies reporting SOD concentrations in OSA patients and non-OSA controls in the electronic databases Pubmed, Web of Science, Scopus and Google Scholar. In total, 13 studies in 847 OSA patients and 438 non-OSA controls were included in the meta-analysis. Blood SOD concentrations were significantly lower in OSA patients (SMD = 0.87, p < 0.001). By contrast, serum/plasma SOD concentrations were not significantly different between the two groups. Although extreme between-study heterogeneity was observed, the SMD was not substantially modified when individual studies were sequentially removed. In conclusion, we observed that whole blood, but not serum/plasma, SOD concentrations were significantly lower in OSA patients compared with controls. Our meta-analysis suggests an impaired antioxidant defence in OSA that is more robustly assessed in the corpuscular biological matrix and provides useful background information for further studies investigating the association between SOD changes and clinical status in OSA. Sarcoidosis is a rare, systemic inflammatory disease whose diagnosis and management can pose a challenge for clinicians and specialists. Scientific knowledge on the molecular pathways that drive its development is still lacking, with no standardized therapies available and insufficient strategies to predict patient outcome. In recent years, oxidative stress has been highlighted as an important factor in the pathogenesis of sarcoidosis, involving several enzymes and molecules in the mechanism of the disease. This review presents current data on the role of oxidative stress in sarcoidosis and its interaction with inflammation, as well as the application of antioxidative therapy in the disease. Variable patterns of disease progression are typically observed in patients with idiopathic pulmonary fibrosis (IPF). We sought to determine the prognostic capacity of blood cell count indexes, derived from routine complete blood cell (CBC) count, in a cohort of IPF patients. The neutrophil-to-lymphocyte ratio (NLR), derived neutrophil-to-lymphocyte ratio (dNLR), monocyte-to-lymphocyte ratio (MLR), platelet-to-lymphocyte ratio (PLR), systemic inflammation index (SII), systemic inflammation response index (SIRI), and aggregate index of systemic inflammation (AISI) were calculated at baseline in a consecutive series of 82 IPF patients followed for four years. After adjusting for age, gender, body mass index, smoking status, and disease stage, only the AISI was significantly associated with mortality (HR 1.0013, 95% CI 1.0003–1.0023, p = 0.015). Patients with AISI <434 and ≥434 had a median survival from the diagnosis of 35.3 ± 15.2 and 26.6 ± 16.3 months (p = 0.015), and a four-year survival rate of 54% and 34%, respectively. The AISI, easily derivable from routine laboratory tests, is independently associated with mortality in patients with IPF. In this work, new highly sensitive graphene-based flexible strain sensors are produced. In particular, polyvinylidene fluoride (PVDF) nanocomposite films filled with different amounts of graphene nanoplatelets (GNPs) are produced and their application as wearable sensors for strain and movement detection is assessed. The produced nanocomposite films are morphologically characterized and their waterproofness, electrical and mechanical properties are measured. Furthermore, their electromechanical features are investigated, under both stationary and dynamic conditions. In particular, the strain sensors show a consistent and reproducible response to the applied deformation and a Gauge factor around 30 is measured for the 1% wt loaded PVDF/GNP nanocomposite film when a deformation of 1.5% is applied. The produced specimens are then integrated in commercial gloves, in order to realize sensorized gloves able to detect even small proximal interphalangeal joint movements of the index finger. Hot-melt extrusion (HME) is the most preferred and effective method for manufacturing amorphous solid dispersions at production scale, but it consumes large amounts of samples when used for formulation development. Herein, we show a novel approach to screen the polymers by overcoming the disadvantage of conventional HME screening by using a minimum quantity of active pharmaceutical ingredient (API). Vacuum Compression Molding (VCM) is a fusion-based method to form solid specimens starting from powders. This study aimed to investigate the processability of VCM for the creation of amorphous formulations and to compare its results with HME-processed formulations. Mixtures of indomethacin (IND) with drug carriers (Parteck® MXP, Soluplus®, Kollidon® VA 64, Eudragit® EPO) were processed using VCM and extrusion technology. Thermal characterization was performed using differential scanning calorimetry, and the solid-state was analyzed via X-ray powder diffraction. Dissolution studies in the simulated gastric fluid were performed to evaluate the drug release. A new, transparent, metal-free absorber, based on the use of multilayer graphene/dielectric laminates (GLs), is proposed for applications in the low-terahertz frequency range. The designed absorber has a total thickness of around 70 µm and consists of a front matching dielectric layer followed by a GL, a dielectric spacer and a back GL. The laminates are periodic structures constituted of graphene sheets separated by 50-nm-thick polyethylene terephthalate (PET) interlayers, while the matching layer and the spacer are one-quarter-wavelength thick and made of PET. The GLs are modeled as homogeneous-equivalent single layers (ESLs) characterized by their sheet resistances Rs. An innovative analytical method is proposed in order to select Rs values optimizing the electromagnetic wave absorption either in low-gigahertz or low-terahertz frequency range. The frequency spectra of the absorption, reflection and transmission coefficients are computed in the range up to 4 THz by using different values of Rs. Then, realistic Rs values of chemically doped graphene monolayers over PET substrates are considered. The designed absorbers are characterized by an absorption coefficient with a peak value of about 0.8 at the first resonant frequency of 1.1 THz, and a 1.4 THz bandwidth centered at 1.5 THz with reflection coefficient below - 10 dB. Moreover, the optical transmittance of the proposed absorbers are computed by means of the optical matrix theory and it is found to be greater than 86% in all the visible ranges.
