Trichophyton indotineae is a newly described dermatophyte species. This fungal pathogen has recently emerged in India and is responsible for chronic or recurrent widespread superficial infections. Resistance to terbinafine is frequently associated to this pathogen and is related to point mutations in the gene encoding the squalene epoxidase. T. indotineae infections have been reported outside India, highlighting the risk of worldwide diffusion of this microorganism. Species identification and antifungal susceptibility determination are key points for infection control but still remain challenging. Systemic treatment is usually required and itraconazole is frequently prescribed in case of terbinafine resistance. Background: Digital necrosis is rarer than lower limb necrosis and constitutes a medical or surgical emergency. Etiological evaluation is required. Cold agglutinin disease is a cause of digital necrosis but diagnosis is difficult., Patients and Methods: Herein we report the case of a 57-year-old man presenting recent paroxysmal acrosyndrome of the left hand subsequently complicated by digital necrosis following occupational exposure to cold in his work as a forklift driver. After etiological evaluation, a diagnosis of primary cold agglutinin disease was made. Intravenous rituximab and topical treatment resulted in complete healing., Discussion: Cold agglutinin disease is a rare type of auto-immune hemolytic anemia. Following exposure to cold, paroxysmal cutaneous signs are frequent. The disease may be either primary or secondary with B-cell lymphoproliferative disorder, auto-immune disease or infection. A thorough workup is required. Introduction: Cutaneous tuberculosis is rare in the developed countries. We report a case of tuberculous gumma in an immunocompetent patient diagnosed late., Case-Report: A 33-year-old Moroccan woman was diagnosed with tuberculous gumma after presenting cutaneous nodules and subcutaneous abscesses that had developed during pregnancy and were present for 11 months. Cutaneous and pulmonary tuberculosis was discovered. The patient recovered under antimycobacterial treatment., Comments: Cutaneous tuberculosis presents with a wide spectrum of clinical lesions. It should be considered in the presence of poorly known lesions such as gumma, which require investigation for underlying tuberculosis.

Introduction: Cutaneous squamous cell carcinoma is an invasive and destructive tumor, and may cause death by local extension or because of metastasis. We report the case of a patient with a giant squamous cell carcinoma of the nose and extension to the brain and discuss the main risk factors from this extension., Observation: Resection of a giant squamous cell carcinoma was performed of the nose in a 45 year-old man after debulking radiotherapy. Histological examination disclosed a well-differentiated tumor and perineural involvement, with at least a 6 mm margin. A first relapse occurred on the orbital edge of the initial resection, the lesion was removed revealing an involvement of the infra-orbital nerve. Whilst the patient was receiving chemotherapy, a second relapse occurred responsible for ophthalmoplegia and loss of vision, with involvement of the left orbital apex, cavernous sinus and temporal lobe. The patient died from grand mal seizures., Discussion: Brain extension of cutaneous squamous cell carcinoma of the head is rare. It develops along the anatomic pathways, especially perineural spread. Main risk factors for such a poor course are discussed, including the size of tumor, the anatomic site, the depth and perineural invasion.

Background: The diagnostic and prognostic value of specific cutaneous lesions in acute leukemia is well-known. Paradoxically, these lesions may initially develop without peripheral blood or bone marrow involvement. We report the case of a patient with cutaneous lesions of acute monoblastic leukemia whereas peripheral blood was normal and massive infiltration of dermis was demonstrated., Observation: A 49 year-old man had papules and nodules of the back and upper arms evolving for several months. Histological examination with appropriate immunostaining led to the diagnosis of specific cutaneous lesions of acute monoblastic leukemia. Several hemograms with peripheral blood smears were normal, bone marrow smear demonstrated an important blastic infiltration on one site and a discrete infiltration on another. Cutaneous lesions disappeared with chemotherapy., Discussion: Specific cutaneous lesions may be isolated during acute leukemia, and called aleukemic leukemia cutis. These are a rare form, the underlying mechanism of which relies on the accumulation of small quantities of myeloblasts in bone narrow and with high tropism for the dermis.

Clinically silent adrenal involvement in lymphoma is a relatively common occurrence, involvement of bilateral adrenal glands by lymphoma as the sole manifestation of lymphoma is very rare, with only 30 cases reported in the literature. We report a patient with this unusual conditions, with CT scan and MRI aspects.

Introduction: Mephenesin is a muscle relaxant, mostly applied locally in trauma. It is a component of Traumalgyl cream in association with phenylbutazone. Contact dermatitis due to mephenesin is exceptional., Case Report: We report here a case of a woman who developed after application of Traumalgyl cream purpuric dermatitis, erythema multiforme-like and urticarial lesions. Patch-tests were strongly positive (+ + +) for both Traumalgyl cream and mephenesin and (+) for phenylbutazone. Improvement of the lesions was observed within 3 weeks., Discussion: Physicians must be aware of the risk of severe contact dermatitis after application of products including mephenesin because of their widely prescription.

Introduction: The development of pyogenic granuloma of the tongue is rare. We report such a case in a patient with chronic graft-versus-host disease., Case Report: A 5-year-old girl was treated with allogenic bone marrow transplantation for aplastic anemia. Complications consisted in chronic graft-versus-host disease with cutaneous, mucosal and hepatic involvement. Seven months later, a voluminous pyogenic granuloma of the tongue developed on previous erosive lichenoid lesions of chronic graft-versus-host disease while the patient was under cyclosporin. Surgical resection was done. Relapse occurred and treatment was identical., Discussion: Six cases of pyogenic granuloma of the oral cavity in chronic graft-versus-host disease have been reported in the literature. Pyogenic granuloma must be considered in tumoral lesion of the oral cavity of patients with chronic graft-versus-host disease. All patients had erosive lichenoid lesions and were under cyclosporin; its role in the genesis of these lesions is debated. Treatment relies on surgical resection confirming the diagnosis.

Deep cutaneous lesions are seldom encountered in lupus erythematosus. The disease described in the literature as lupus erythematosus profundus or lupus erythematosus panniculitis usually occurs in middle-aged women. The authors report a case of deep lupus erythematosus which was exceptional in that it occurred in an 11-year old girl. The lesions were situated on the face and consisted of solid subcutaneous nodules and clear-cut ulcerations leaving atrophic pigmented scars. The histological image of a nodule was one of lobular lymphocytic panniculitis with homogeneous hyalinization of adipose nodules, to which must be added periadnexal and perivascular dermo-epidermal lymphocytic infiltrates. Granular IgM deposits arranged along the dermo-epidermal junction were observed at direct cutaneous immunofluorescence. Laboratory examinations showed leucopenia (3,300/mm3) with neutropenia (1,100/mm3) and the presence of antinuclear antibodies at 1/100 speckled fluorescence, as well as antibodies directed against native DNA. Studies of renal function and for complement gave normal results. The other causes of lobular panniculitis were excluded. The lesions regressed within 3 weeks under hydroxychloroquine; this drug was also successful in arresting a relapse consecutive to withdrawal of treatment. The authors have analysed the 17 paediatric cases of deep lupus erythematosus and were able to determine their main characteristics: 1. The lesions occur mostly in girls (70 p. 100). They are located electively on the face and the lateral aspect of the shoulders. They consist of well-limited, solid or hard subcutaneous nodule which may congregate to form plaques. The epidermis may be normal or pathological, poikilodermic, looking like a discoid or ulcerated lupus erythematosus.
