Background: Vaginal calculi are rare and can grow quite large if they remain undetected. Vaginal stones are caused by the pooling of urine in the vagina and can be classified as either primary or secondary, depending on the absence or presence, respectively, of a nidus. Primary stones without any urethrovaginal or vesicovaginal fistula are even more uncommon but appear to be more commonly reported in incontinent women with significant physical disabilities. Case Presentation: We present a case of an ∼11 cm primary vaginal stone in a 61-year-old woman with cerebral palsy. This was removed using a nephroscope and an endoscopic ultrasonic lithotrite through the vaginal introitus with subsequent analysis demonstrating a struvite stone composition. Conclusion: This case is unique not only for the large size of the calculi but also for our less invasive approach, using a nephroscope and endoscopic ultrasonic lithotrite to fragment and remove the stone. We hope that this report will assist other providers in the timely and accurate diagnosis and treatment of future vaginal stone patients.

Holmium:yttrium-aluminum-garnet (Ho:YAG) laser lithotripsy is the standard lithotrite in ureteroscopy. We investigated the influence of pulse frequency, energy and length on the fragmentation efficiency of Ho:YAG laser lithotripsy in non-floating artificial stones in vitro.Stone fragmentation efficiency of three different Ho:YAG laser devices were evaluated in vitro at different pulse energy (1.0 and 2.0 J) and frequency settings (5 and 10 Hz), resulting in a standardized output power of 10W, respectively. Where possible, pulse length was modified (350 vs 700 microsec). Each setting was performed with a 273 microm and a 365 microm fiber. Lithotripsy was conducted using non-repulsive stones consisting of soft stone (plaster of Paris) and hard stone composition (Fujirock type 4).Our results showed an increased stone disintegration efficiency at higher pulse energy (2.0 J/5 Hz vs 1.0 J/10 Hz) independently of two fiber diameters and stone types applied in this study (P0.05 in 18 of 20 groups). Similarly, reduction of the pulse length from 700 to 350 microsec resulted in a higher stone disintegration (P0.05 in 13 of 16 groups). This effect was most prominent when applied to soft stones. Higher fiber diameter was not constantly associated with an increase in stone disintegration.We demonstrate that an increase of pulse energy and a reduction of pulse length at a standardized output power of 10W can improve Ho:YAG laser fragmentation efficiency in vitro in nonfloating stones. These results may potentially affect clinical practice of Ho:YAG laser lithotripsy in impacted or large stones, when retropulsion is excluded.

To compare the efficiency and cost effectiveness of a combined pneumatic and ultrasonic lithotrite (Lithoclast Ultra) and a standard ultrasonic lithotrite, (LUS-1) during percutaneous nephrolithotomy.In a prospective randomized trial, 30 patients undergoing percutaneous nephrolithotomy (PCNL) were randomized to PCNL with either the combined pneumatic and ultrasonic lithotrite (PUL) or a standard ultrasonic lithotrite (SUL). Patient demographics, stone composition, location, pre- and post-operative stone burden, fragmentation rates, and device failures were compared.There were 13 patients in the PUL group and 17 patients in the SUL group. Stone burden and location were equal. Overall, 64% of the PUL group had hard stones (defined as stones that were either pure or a mixture of cystine [3], calcium oxalate monohydrate [CaOxMono; 2], and calcium phosphate [CaPO4; 2]), and four had soft stones (3 struvite and 1 uric acid [UA]). In the SUL group, there were eight hard stones (5 CaOxMono and 3 CaPO4), and six soft stones (4 calcium oxalate dihydrate [CaOxDi] and 2 UA) (P = 0.51). Stone composition data were unavailable for five patients. Fragmentation time for the PAL was 37 minutes versus 31.5 minutes for the SUL (P = 0.22). Stone retrieval and mean operative times were similar for both groups. There were a total of three (23.1%) device-related problems in the PUL group, and eight (47%) in the SUL group. There was one (7.7%) device malfunction in the PUL group due to probe fracture. There were two (11.7%) device failures in the SUL group; one failure required the device to be reset every 30 minutes, and the second was an electrical failure. Suction tubing obstruction occurred twice (15.3%) in the PUL group and 35.3% in the SU group (P = 0.35). The stone-free rates for the PUL and SUL were 46% and 66.7%, respectively (P = 0.26).Although the PUL was more costly, stone ablation and clearance rates were similar for both the combined pneumatic and ultrasonic device and the standard ultrasonic device. When stratified with respect to stone composition, the PUL was more efficient for harder stones, and the SUL was more efficient for softer stones.

A central component of percutaneous nephrostolithotomy is an endoscopic lithotrite that safely and efficiently clears stone of all compositions from the intrarenal collecting system. The current multicenter trial studied 3 lithotrite combinations employing both mechanical ballistic and ultrasonic energy to fragment and evacuate with suction stone debris. What is clear from this work is that, with regard to the timely clearance of the stone burden and peri-operative complications, there is minimal difference between the devices studied, nothing special and nothing new. The real question here is: are these devices sufficient, since the basic technologies employed are well over 10 years mature? Percutaneous nephrostolithotomy in the era of retrograde flexible ureteroscopic lithotripsy is relegated to those with the largest stone burdens, matrix stones requiring complete evacuation of this material at the time of the surgical intervention, and those who have failed ureteroscopic lithotripsy. The initial clinical experience with holmium laser energy applied for endoscopic lithotripsy was positive. The current study is presented as a contrast to the preliminary findings and as a means of defining the clinical usefulness of this specific laser lithotrite.Calculi were treated endoscopically with the holmium laser lithotriptor and data was gathered prospectively. The youngest patient in the series was a thirteen-month--old who underwent percutaneous therapy, while the youngest patient on whom a retrograde endoscopic procedure was performed was a six-year old male patient with a proximal ureteral calculus. Lower water density, quartz fibers delivery systems were developed and employed. Fiber diameters ranged from 200-1000 micrograms. The smaller fibers were employed most commonly through the actively deflectable, flexible endoscope to facilitate treatment with maximum deflection. Larger fibers, with their much larger vaporization bubbles, were used through rigid endoscopes to debulk large stone burdens.A total of 210 patients with 249 calculi were treated. All major stone compositions were treated with minimal variation in laser efficiency. All but three of 109 ureteral calculi were treated in a retrograde fashion to completion (i.e., "stone free") in one sitting (97%). One-hundred thirteen renal stone burdens were treated with the holmium laser; 99 of these were treated solely in a retrograde fashion. Of the latter, 79 (80%) required only a single session. The combination of the actively deflectable, flexible ureteroscope and the 200-micrograms fiber facilitated treatment to completion of 38 to 45 lower-pole caliceal calculi (85%). The success of ureteropyeloscopic lithotripsy with the holmium laser for all intrarenal calculi, including staged or second sitting for large complex stone burdens, was 90%. Sixteen percutaneous procedures (13 renal and 3 ureteral calculi) employed the holmium laser as an endoscopic lithotrite. All 28 patients with large bladder calculi with a mean diameter of 41.8 mm were treated to completion in one sitting. Complications from holmium laser energy, including postoperative ureteral stricture disease, were not encountered in this series.Holmium laser energy is uniquely suited to treat urinary calculi safely regardless of stone size, location, or metabolic composition, and has particular efficacy in complex clinical presentations.

The Browne Pneumatic Impactor (BPI; Browne Medical Systems, Minneapolis, MN) is a new mechanical lithotripter that relies on a pneumatically driven metal probe. It can be employed through both rigid and actively deflectable flexible endoscopes. Prior in vivo studies have shown excellent fragmentation of hard human calculi while employing the BPI with active endoscope deflection of as much as 45 degrees. In this study, we set out to define the efficiency of stone fragmentation with active endoscope deflection and to quantify the extent of retrograde stone migration that is often noted when mechanical lithotripters are employed. Pressure transducer testing of the BPI revealed an inverse relation between increased deflection of the endoscope and transduced voltage. Calculus fragmentation tests showed that as the endoscope was actively deflected to 90 degrees, the BPI still was able to fragment human calcium oxalate monohydrate calculi into extractable fragments with as few as six pulses. The maximum active endoscope deflection was 95 degrees with the 0.020-inch Nitinol probe passed through the working channel. At this deflection, the BPI was able to fragment an 8 mm pure calcium oxalate monohydrate calculus into two fragments after 14 pulses. Retrograde migration was evaluated by employing the BPI in a simulated ureter in a waterbath. The travel distance of a 5-mm 5-g calculus with one pulse averaged 12.2 mm (range 1-44 mm). Smaller, less dense calculi travelled farther. A rebound effect was occasionally noted, and this was associated with an off-center delivery. In conclusion, the BPI is an efficient mechanical lithotrite able to fragment hard human calculi when employed with active endoscope deflection of as much as 95 degrees. Delivered energy decreased with active endoscope deflection, but this change did not prevent fragmentation. Retrograde migration was an important variable more notable with smaller calculi.
