Coalescence of infection of the epiglottis, or epiglottic abscess, is a rare manifestation of epiglottitis. We report the case of a 49-year-old Hispanic man with HIV (CD4 count 243 [16.2%]), HIV viral load 175,689 copies per milliliter, antiretroviral-naïve) contracted from his wife who presented to the emergency department with a 3-week history of sore throat, odynophagia, left temporal headache, left neck pain, and occasional blood-streaked sputum. This case represents the first reported case of epiglottic abscess in an HIV-positive individual. Epiglottic abscess formation is associated with potentially rapid airway compromise and carries a high mortality rate. The diagnosis of epiglottic abscess is often difficult. In HIV-infected individuals, a variety of infectious and oncologic sources of respiratory compromise should be considered in addition to epiglottic abscsess. Prompt diagnosis and treatment of this condition is crucial for ensuring optimal outcomes in this rare but often lethal infection. [ABSTRACT FROM AUTHOR]

ABSTRACTBackground: Subphrenic abscess is rare after cesarean section.Methods: Case report and review of the pertinent world literatureCase Report: A 22 year-old primigravida underwent a lower-segment cesarean section in the setting of chorioamnionitis, and had a good postoperative recovery initially. Eleven days after surgery, dyspnea and fever prompted a computed tomography scan, which revealed a large subphrenic abscess. The abscess resolved with percutaneous drainage and intravenous antibiotics. A placental swab, baby's skin swab, and abscess aspirate all grew group B Streptococcus.Conclusion: Although subphrenic abscess is rare in obstetric practice, it should be suspected in patients who remain unwell after chorioamnionitis. We report the case of an 18-year-old boy who developed an intrathoracic abscess with rupture of the diaphragm following a laparoscopic appendectomy (LA)for a perforated appendicitis. LA is an established procedure in the treatment of appendicitis. It is a safe and efficacious technique, but several complications can occur with this procedure, and surgeons should be aware of the potential dangers. Complications of gynecologic laparoscopic surgery have been reported, including damage to the colon, bladder, or ureter. However, subphrenic abscess is rare except in patients undergoing laparoscopic cholecystectomy. The authors describe a 44-year-old woman who underwent laparoscopic surgery for hemosalpinx, which was complicated by subphrenic abscess formation postoperatively. The abscess formation in this patient may have been caused by the spread of infection whose source was the pelvic cavity, which may have been facilitated by placement of the patient in the 30 degrees Trendelenburg position during laparoscopic surgery. A case study is presented of a 25 year old female with ruptured retrocecal appendicitis at her third trimester of pregnancy. Topics discussed include the patient went through abdominal ultrasound and magnetic resonance imaging after initial diagnosis for right renal colic, delivery performed through caesarean after preparation with corticosteroids, and sub-diaphragmatic collection detected as subphrenic abscess after computed tomography.
