Background: In spite of the tremendous progress achieved in medical sciences in the last century, the management of diabetes mellitus, a disease as old as mankind, is poor. Diabetes is currently the world's largest endocrine disorder, and estimates are that it affects almost 5% of the population. Ayurveda, the Indian traditional system of medicine, is one of the world's oldest systems to have documented the diagnosis and treatment of diabetes., Methods: Experimental studies performed in accordance with the modern medicine principles have shown that some of the medicinal plants and polyherbal preparations made using the plants used in Ayurveda are effective in preventing both hyperglycemia and its complications. Syzygium jambolanum (Syn Syzygium cumini, Eugenia cumini, Eugenia jambolana), commonly known as black plum and originally indigenous to India, is one of the important antidiabetic plants., Results: Jamun has been used in various complementary and alternative systems of medicine and, before the discovery of insulin, was a frontline antidiabetic medication even in Europe. The brew prepared by boiling the Jamun seeds in boiling water has been used in the various traditional systems of medicine in India., Conclusions: This review includes the validated antidiabetic effects of Jamun and some of its compounds. Emphasis is also placed on addressing the various mechanisms of action contributing to the pharmacological effects and the aspects that need future investigations for Jamun to be of clinical use.

Despite significant advances and development of novel anti-emetics, nausea and vomiting (emesis) is a major side-effect of cancer chemotherapy. At times, severe nausea and vomiting may also lead to reduction in adherence to the treatment regimen, and this will concomitantly affect the patient's survival. The rhizome of Zingiber officinale, commonly known as ginger, is globally an important spice. It has been used for centuries in the Indian, Chinese, Arabic, Tibetan, Unani, and Siddha systems of traditional medicine to treat nausea and vomiting induced by different stimuli. Preclinical studies with experimental animals (dogs and rats) have shown that the various extracts of ginger and the ginger juice possess anti-emetic effects against chemotherapy-induced nausea and vomiting. Gingerol, the active principle, is also shown to possess anti-emetic effects in minks. However, with regard to humans, while most studies have been supportive of the preclinical observations, a few have been contradictory. The exact mechanism responsible for the anti-emetic effects of ginger is unknown; however, the ginger phytochemicals, especially 6-gingerol, 8-gingerol, 10-gingerol, and 6-shogaol, may function as a 5-hydroxytryptamine (5-HT3) antagonist, NK1 antagonist, antihistaminic, and possess prokinetic effects. The present review for the first time attempts to address the anti-emetic observations and the variability in response of the anti-emetic effects of ginger in cancer chemotherapy. An attempt is also made to address the lacunae in the published studies and emphasize aspects that need further investigations for ginger to be of use in clinics as an anti-emetic agent in the future.

Background: Triphala (Sanskrit tri = three and phala = fruits), composed of the three medicinal fruits Phyllanthus emblica L. or Emblica officinalis Gaertn., Terminalia chebula Retz., and Terminalia belerica Retz. is an important herbal preparation in the traditional Indian system of medicine, Ayurveda. Triphala is an antioxidant-rich herbal formulation and possesses diverse beneficial properties. It is a widely prescribed Ayurvedic drug and is used as a colon cleanser, digestive, diuretic, and laxative. Cancer is a major cause of death, and globally studies are being conducted to prevent cancer or to develop effective nontoxic therapeutic agents. Experimental studies in the past decade have shown that Triphala is useful in the prevention of cancer and that it also possesses antineoplastic, radioprotective and chemoprotective effects., Conclusions: This review for the first time summarizes these results, with emphasis on published observations. Furthermore, the possible mechanisms responsible for the beneficial effects and lacunas in the existing knowledge that need to be bridged are also discussed.

The effective use of radiotherapy in cancer cure and palliation is compromised by the side-effects resulting from radiosensitivity of bordering normal tissues, which are invariably exposed to the cytotoxic effects of ionizing radiation during treatment. In this situation, use of radioprotective compounds that can protect normal tissues against radiation injury are of immense use. In addition to protecting normal tissue these compounds will also permit use of higher radiation doses to obtain better cancer control and possible cure. However, to date, no ideal radioprotectors are available as most synthetic compounds are toxic at their optimal concentrations and have produced little success in clinics. Radiation ill-effects are principally the result of generation of free radicals, and the antioxidant compounds that counter them are supposed to be of immense use in preventing them. In Ayurveda, the traditional Indian system of medicine, several plants have been observed to avert/ameliorate free radical-mediated ailments--an effect that has been documented--and such plants have recently been the focus of attention. Aegle marmelos (L.) Correa (Bael), commonly known as bael, has been used since antiquity for treating various ailments, some of which are now known to be the result of oxidative stress. In studies spanning nearly a decade, it has been observed that bael prevented radiation-induced ill-effects, and the results of these studies indicate that it has the potential to be an effective, nontoxic radioprotective agent. In this current review, for the first time, an attempt is made to summarize these observations and to discuss the plausible reasons responsible for bael's radioprotective effects.

The effects of 10 mg/kg of triphala extract (TE) was studied on radiation-induced sickness and mortality in mice exposed to 7-12 Gray (Gy) of gamma-irradiation. Treatment of mice with triphala once daily for 5 consecutive days before irradiation delayed the onset of mortality and reduced the symptoms of radiation sickness when compared with the non-drug double distilled water treated irradiated controls (DDW). Triphala provided protection against both gastrointestinal and hemopoetic death. However, animals of both the TE + irradiation and DDW + irradiation groups did not survive up to 30 days post-irradiation beyond 11 Gy irradiation. The LD50/30 was found to be 8.6 Gy for the DDW + irradiation group and 9.9 Gy for TE + irradiation group. The administration of triphala resulted in an increase in the radiation tolerance by 1.4 Gy, and the dose reduction factor was found to be 1.15. To understand the mechanism of action of triphala, the free radical scavenging activity of the drug was evaluated. Triphala was found to scavenge (.)OH, O(2) (.) 2,2'-azinobis(3-ethylbenzthiazoline-6-sulfonate) diammonium salt (ABTS)(.+) and NO(.) radicals in a dose dependent manner.

The plant extracts of 17 commonly used Indian medicinal plants were examined for their possible regulatory effect on nitric oxide (NO) levels using sodium nitroprusside as an NO donor in vitro. Most of the plant extracts tested demonstrated direct scavenging of NO and exhibited significant activity. The potency of scavenging activity was in the following order: Alstonia scholaris > Cynodon dactylon > Morinda citrifolia > Tylophora indica > Tectona grandis > Aegle marmelos (leaf) > Momordica charantia > Phyllanthus niruri > Ocimum sanctum > Tinospora cordifolia (hexane extract) = Coleus ambonicus > Vitex negundo (alcoholic) > T. cordifolia (dichloromethane extract) > T. cordifolia (methanol extract) > Ipomoea digitata > V. negundo (aqueous) > Boerhaavia diffusa > Eugenia jambolana (seed) > T. cordifolia (aqueous extract) > V. negundo (dichloromethane/methanol extract) > Gingko biloba > Picrorrhiza kurroa > A. marmelos (fruit) > Santalum album > E. jambolana (leaf). All the extracts evaluated exhibited a dose-dependent NO scavenging activity. The A. scholaris bark showed its greatest NO scavenging effect of 81.86% at 250 microg/mL, as compared with G. biloba, where 54.9% scavenging was observed at a similar concentration. The present results suggest that these medicinal plants might be potent and novel therapeutic agents for scavenging of NO and the regulation of pathological conditions caused by excessive generation of NO and its oxidation product, peroxynitrite.

The chemomodulatory activity of Alstonia scholaris extract (ASE) was studied in combination with berberine hydrochloride (BCL), a topoisomerase inhibitor, in Ehrlich ascites carcinoma-bearing mice. The tumor-bearing animals were injected with various doses of ASE, and 8 mg/kg of BCL (one-fifth of the 50% lethal dose) was combined with different doses of ASE (60-240 mg/kg). The combination of 180 mg/kg of ASE with 8 mg/kg of BCL showed the greatest antitumor effect; the number of tumor-free survivors was more, and the median survival time and the average survival time increased up to 47 and 40.5 days, respectively, when compared with either treatment alone. Similarly, when 180 mg/kg of ASE was combined with different doses of BCL (2-12 mg/kg), a dose-dependent increase in the anticancer activity was observed up to 8 mg/kg of BCL. However, a further increase in the BCL dose to 10 and 12 mg/kg resulted in toxic side effects. The best effect was observed when 180 mg/kg of ASE was combined with 6 or 8 mg/kg of BCL, where an increase in the antineoplastic activity was reported. The efficacy of the combination of 180 mg/kg of ASE was also tested with 6 mg/kg body weight of BCL in various stages of tumorigenesis, and it was effective when given in the early stages, although the efficiency decreased with an increase in the tumor developmental stages.

The radiosensitizing effect of 5 micrograms/mL of alkaloid fraction of Alstonia scholaris (ASERS) was evaluated in various neoplastic cell lines, namely: HeLa, HePG2, HL60, MCF-7, and KB exposed to 0, 0.5, 1, 2, 3, and 4 Gy of gamma-radiation. The irradiation of various cells caused a dose-dependent elevation in the cytotoxicity, and a maximum cytotoxic effect was observed at 4 Gy (the highest dose) in all the cell lines studied. The ASERS pretreatment increased the effect of radiation as evidenced by enhanced cell killing when compared with the concurrent phosphate-buffered saline (PBS) treated irradiation group. The greatest elevation in cell killing was observed for HeLa and KB cells, followed by HL60, MCF7, and HePG2 cells. The in vitro observations were confirmed by in vivo studies, where the Ehrlich ascites carcinoma (EAC) bearing mice were treated with 120 mg/kg body weight of ASERS before exposure to 0, 1, 2, 4, 6, and 8 Gy of hemibody (below the rib cage) gamma-radiation. Irradiation of EAC mice caused a dose-dependent tumor regression, as evidenced by increased life span of the animals. The pretreatment of tumor-bearing animals with 120 mg/kg ASERS resulted in a further remission in the tumor when compared with the concurrent nondrug-treated irradiated controls; as a result there was a radiation dose-dependent increase in the life span of tumor-bearing animals receiving 120 mg/kg ASERS, except for 8 Gy, where it was less than the concurrent control. The above findings corroborate with a time-dependent decrease in the glutathione (GSH) contents, accompanied by an increase in lipid peroxidation. Our study demonstrates that ASERS treatment enhances the effect of radiation and results in disease-free survival of the mice.
